Abstract
Un inhibidor de la actividad IL-17 para uso en un método de tratamiento y/o profilaxis de la esclerosis múltiple de recaída-remisión (MS), en el que el inhibidor es una proteína de fusión IL-17R:Fc, o es un anticuerpo o un fragmento de anticuerpo funcionalmente activo de éste que se une a IL-17 o IL-17R.

Como alternativa, el polipéptido puede biotinilarse utilizando técnicas muy conocidas por los expertos en la técnica (por ejemplo, el kit de biotinilación de Pierce Chemicals; Rockford, IL). La capacidad del agente candidato para interaccionar con el polipéptido puede determinarse mediante métodos conocidos por los expertos en la técnica, por ejemplo, ELISA, BIAcore™, citometría de flujo o la tecnología de ensayo de microvolúmenes fluorescente (FMAT). En otro ejemplo en el que se utiliza un ensayo basado en células, una población de células que expresan IL-17 o IL17R se pone en contacto con un agente candidato, y se determina la capacidad del agente candidato para interaccionar con el polipéptido. Preferiblemente, la capacidad de un agente candidato para interaccionar con IL-17 o IL-17R se compara con un intervalo de referencia o control. La célula, por ejemplo, puede ser de origen eucariota (por ejemplo, de levadura o mamífero) y puede expresar el polipéptido de IL-17 o IL-17R de forma endógena, o puede modificarse genéticamente para que exprese el polipéptido. En algunos casos, el polipéptido de IL-17 o IL17R o el agente candidato se marca, por ejemplo, con un marcador radiactivo (tal como 32P, 35S o 125I) o un marcador fluorescente (tal como isotiocianato de fluoresceína, rodamina, ficoeritrina, ficocianina, aloficocianina, oftaldehído o fluorescamina) para permitir la detección de una interacción entre un polipéptido y un agente candidato. También pueden emplearse métodos alternativos, tales como ELISA, citometría de flujo y FMAT.
Los agentes que inhiben la actividad IL-17 pueden identificarse mediante cualquier método adecuado, por ejemplo:
- (i)
- comparando la actividad de IL-17 en presencia de un agente candidato con la actividad de dicho polipéptido en ausencia del agente candidato o en presencia de un agente control; y
- (ii)
- determinando si el agente candidato inhibe la actividad de IL-17.
Estos ensayos pueden emplearse para seleccionar agentes candidatos, en controles clínicos o en el desarrollo de fármacos.
Tal como se describió anteriormente, los agentes puede preseleccionarse cuando resulte apropiado para identificar agentes (por ejemplo, un anticuerpo) que interaccionen con IL-17 o IL-17R antes de seleccionar aquellos agentes que se unen por su capacidad para inhibir la actividad IL-17.
En un ejemplo se utiliza un sistema de ensayo basado en células para identificar agentes capaces de inhibir la actividad de IL-17. En un ejemplo concreto, el ensayo utilizado para identificar inhibidores de la actividad IL-17 es el ensayo de liberación de IL-6 desde fibroblastos convencional (Yao et al., 1995, Journal of Immunology, 155, 54835486). Se añaden inhibidores potenciales al ensayo y se determina la liberación de IL-6 mediante ELISA. Por tanto, la inhibición se mide como una reducción en la liberación de IL-6 comparada con controles.
Pueden identificarse agentes que inhiban la actividad de IL-17 o ensayarse más a fondo, por ejemplo, para determinar las cantidades terapéuticamente eficaces en uno o más modelos animales. Los ejemplos de animales adecuados incluyen, pero no se limitan a ratones, ratas, conejos, monos, cobayas, perros y gatos. Preferiblemente, el animal utilizado representa un modelo de MS.
En otro ejemplo, la inhibición de la actividad IL-17 puede determinarse controlando una mejoría en los síntomas de la enfermedad, un retraso en la aparición o una progresión lenta de la enfermedad, por ejemplo, pero sin limitación, una reducción en la parálisis. Pueden utilizarse técnicas conocidas por los médicos familiarizados con la MS para determinar si un agente candidato ha alterado uno o más síntomas asociados con la enfermedad.
En la técnica se conoce una serie de diferentes modelos de MS ('t Hart y Amor, 2003, Current Opinion in Neurology, 16:375-383). En particular, la encefalomielitis autoinmunológica experimental (EAE) en ratones ABH se considera un modelo pertinente para la MS en seres humanos (Baker et al., 1990, Journal of Neuroimmunology, 28:261-270). Se han desarrollado modelos agudos y de recaída-remisión.
Tal como se analiza en la presente, los inhibidores de la actividad IL-17 pueden utilizarse en el tratamiento y/o profilaxis de la MS. Para este uso, los agentes se administrarán, en general, en forma de una composición farmacéutica.
También se describe una composición farmacéutica que comprende un inhibidor de la actividad IL-17 y un vehículo farmacéuticamente aceptable.
La composición se suministra habitualmente como parte de una composición farmacéutica estéril que normalmente incluye un vehículo farmacéuticamente aceptable. Esta composición puede estar en cualquier forma adecuada (dependiendo del método deseado de administración a un paciente).
Los inhibidores para su uso en la invención se administran al sujeto preferiblemente mediante una diversidad de vías diferentes, tales como la vía oral, transdérmica, subcutánea, intranasal, intravenosa, intramuscular, intratecal e intracerebroventricular. La vía más adecuada para la administración en cualquier caso concreto dependerá del inhibidor particular, el sujeto, y la naturaleza y gravedad de la enfermedad y el trastorno físico del sujeto.

Las composiciones farmacéuticas adaptadas para la administración tópica se pueden formular como ungüentos, cremas, suspensiones, lociones, polvos, disoluciones, pastas, geles, vendas impregnadas, pulverizadores, aerosoles
o aceites, dispositivos transdérmicos, polvos para empolvar y similares. Estas composiciones pueden prepararse mediante métodos convencionales que contengan el agente activo. Por tanto, también pueden comprender vehículos y aditivos convencionales compatibles, tales como conservantes, disolventes para ayudar a la penetración del fármaco, emolientes en cremas o ungüentos, y etanol o alcohol oleílico para lociones. Tales vehículos pueden estar presentes de aproximadamente 1% a aproximadamente 98% de la composición. De forma más habitual, formarán hasta aproximadamente 80% de la composición. Sólo como ilustración, una crema o ungüento se prepara mezclando cantidades suficientes de un material hidrófilo y agua, conteniendo entre aproximadamente 5-10% en peso del compuesto, en cantidades suficientes para producir una crema o ungüento que tenga la consistencia deseada.
Las composiciones farmacéuticas adaptadas para la administración transdérmica se pueden presentar como parches discretos destinados a permanecer en íntimo contacto con la epidermis del receptor durante un periodo de tiempo prolongado. Por ejemplo, el agente activo puede administrarse desde el parche mediante iontoforesis.
Para aplicaciones a tejidos externos, por ejemplo, la boca y la piel, las composiciones se aplican preferiblemente como una crema o ungüento tópico. Cuando se formula en un ungüento, el agente activo puede emplearse con una base de ungüento parafínica o miscible en agua. Como alternativa, el agente activo puede formularse en una crema con una base de crema de aceite en agua o una base de agua en aceite.
Las composiciones farmacéuticas adaptadas para la administración tópica en la boca incluyen píldoras para chupar, pastillas y enjuagues bucales.
Las composiciones farmacéuticas adaptadas para la administración tópica al ojo incluyen colirios, en los que el agente activo se disuelve o suspende en un vehículo adecuado, en especial un disolvente acuoso. También incluyen cremas o ungüentos tópicos como anteriormente.
Las composiciones farmacéuticas adecuadas para la administración rectal en las que el vehículo es un sólido se presentan, lo más preferiblemente, como supositorios de dosis unitaria. Los vehículos adecuados incluyen manteca de cacao u otro glicérido o materiales usados habitualmente en la técnica, y los supositorios se pueden formar de manera conveniente mediante una mezcla de la combinación con el(los) vehículo(s) reblandecido(s) o fundido(s), seguido de un enfriamiento y conformación en moldes. También se pueden administrar como enemas.
La dosificación de un inhibidor de la actividad IL-17 que se va a administrar variará según el inhibidor concreto, el tipo de MS, el sujeto, y la naturaleza y gravedad de la enfermedad y la condición física del sujeto, y la vía de administración seleccionada; un experto en la técnica puede determinar con facilidad la dosificación apropiada. Para el tratamiento y/o la profilaxis de la MS en seres humanos y animales, pueden administrarse a los pacientes (por ejemplo, sujetos humanos) composiciones farmacéuticas que comprendan anticuerpos a unas dosificaciones terapéutica o profilácticamente aceptables (por ejemplo, unas dosificaciones que den como resultado la inhibición de la MS y/o el alivio de los síntomas de la MS) utilizando cualquier vía de administración apropiada, tal como una inyección y otras vías de administración conocidas en la técnica para los productos clínicos, tales como productos clínicos basados en anticuerpos.
Las composiciones pueden contener del 0,1% en peso, preferiblemente del 10-60% o más en peso, del inhibidor de la invención, dependiendo del método de administración.
Un experto en la técnica reconocerá que la cantidad óptima y la distribución de las dosificaciones individuales de un inhibidor de la invención serán determinadas por la naturaleza y grado del trastorno que se va a tratar, la forma, vía y sitio de la administración, y la edad y condición del sujeto particular que se está tratando y que, último término, será un médico quien determine las dosificaciones apropiadas que deben utilizarse. Esta dosificación puede repetirse tan a menudo como se considere apropiado. Si se producen efectos secundarios, la cantidad y/o frecuencia de la dosificación puede alterarse o reducirse, según la práctica clínica normal.
La invención se describirá a continuación haciendo referencia a los siguientes ejemplos, que son meramente ilustrativos y no deberían considerarse de ninguna manera que limitan el alcance de la presente invención.

Ejemplos
Ejemplo 1. Aislamiento de un anticuerpo anti-IL-17
Se inmunizaron conejos tres veces con IL-17 humana y después dos veces con IL-17 de ratón. Utilizando un ensayo en placa hemolítico con eritrocitos de oveja biotinilados revestidos con IL-17 murina mediante estreptavidina, se aislaron 9 genes de anticuerpos utilizando los métodos descritos en Babcook et al., 1996, Proc. Natl. Acad. Sci, 93, 7843-7848, y en el documento WO92/02551. Los genes de anticuerpos se expresaron en células CHO y los anticuerpos recombinantes se seleccionaron por su capacidad para neutralizar la IL-17 murina en un bioensayo utilizando células 3T3-NIH de ratón (Yao et al., 1995, Immunity, 3:811-821). Todos los anticuerpos en el panel neutralizaron la IL-17 murina en este ensayo, y se seleccionó un anticuerpo, m170013 (Ab#13) para el ensayo in vivo. Para ensayar la eficacia del anticuerpo en EAE, se produjo una IgG quimérica (Ab#13 mIgG1) utilizando la región variable de conejo del anticuerpo m170013 y regiones constantes de ratón.
Ejemplo 2. Efecto de Ab#13 mIgG1 sobre los síntomas de la EAE
Se utilizó un modelo de MS, la encefalomielitis autoinmunológica experimental (EAE), esencialmente como se describe en Baker et al., 1990, Journal of Neuroimmunology, 28:261-270. Ratones hembra ABH de 8-10 semanas de edad (Harlan) se inmunizaron con un homogeneizado de médula espinal (SCH, 3,33 mg/ml) de ratón en adyuvante de Freund completo mediante una inmunización subcutánea en cada uno de los flancos (150 µl/sitio) en los días 0 y

Dos grupos fueron dosificados con anticuerpo a 10 mg/kg, por vía subcutánea en los días -1,6, , 13 y 20. Un grupo (n = 14) se dosificó con Ab#13 mIgG1, y el otro (n = 13) con 101,4 (control de isotipo).
ii) Dosificación a lo largo de la fase de recaída
Se hizo un seguimiento de un total de 30 ratones a lo largo de la fase aguda de la enfermedad, y en el día 27 se realizaron análisis de la fase aguda de la enfermedad para seleccionar dos grupos con unos perfiles de enfermedad similares en la fase aguda (día de aparición, puntuación del pico de la enfermedad, puntuación clínica acumulada y pérdida de peso). Se seleccionaron dos grupos de 12 ratones para la dosificación con anticuerpo a 10 mg/kg, por vía subcutánea en los días 28, 35, 42 y 49. Un grupo se dosificó con Ab#13 mIgG1, y el otro con 101,4 (control de isotipo).
Los pesos y las puntuaciones clínicas fueron registrados a diario por un asesor ciego al tratamiento y se recogió el EDTA-plasma terminal.

Dos grupos fueron dosificados de modo terapéutico (tras 50% de incidencia) con Ab#13 mIgG1 y anticuerpo control 101,4 (10 mg/kg; o vía subcutánea, comenzando en PSD 16 hasta el final del experimento)
Los pesos y las puntuaciones clínicas se registraron a diario y se recogió el EDTA-plasma terminal.

Los análisis estadísticos se realizaron como se describió en el ejemplo 3.
La figura 10 muestra que la dosificación profiláctica (a) de Ab#13 mIgG1 produjo un retraso significativo en la aparición de la enfermedad, la puntuación clínica acumulada y máxima. La dosificación terapéutica (b) redujo significativamente la puntuación clínica acumulada. Otros análisis demostraron que Ab#13 mIgG1 dosificado de modo profiláctico producía unos efectos significativos en la reducción de la puntuación acumulada (p = 0,0006), un
15 retraso en la aparición (p = 0,0184) y la puntuación clínica máxima (p = 0,0032, todos ensayos de la U de Mann-Whitney) (figura 11). El Ab#13 mIgG1 dosificado de modo terapéutico también mostró una reducción significativa en la puntuación acumulada (b, p = 0,0229, ensayo de la U de Mann-Whitney; figura 12).
El anticuerpo anti-IL17 de ratón Ab#13 mIgG1 se dosificó en experimentos diferentes de modo profiláctico y
20 terapéutico. Los efectos fueron más pronunciados de modo profiláctico, con una reducción significativa en la puntuación de enfermedad máxima y acumulada, y además la incidencia de la enfermedad fue significativamente retrasada. El tratamiento terapéutico mostró una reducción en la puntuación de enfermedad acumulada.
