Abstract
Un método de ajuste de una separación (Y) de grapado en un aparato (100) de grapado quirúrgico, que comprende: proporcionar un aparato de grapado quirúrgico que tenga una estructura (120) de mandíbula con una porción (122) para alojamiento del yunque y una estructura (110) de mandíbula con una porción (112) para alojamiento del cartucho, siendo las estructuras de mandíbula funcionalmente acoplables la una a la otra para proporcionar una separación de grapado entre una porción (parte) delantera respectiva de las porciones para alojamiento del yunque y del cartucho, teniendo una de las estructuras de mandíbula una pareja de elementos (112, 114) de montaje situados sobre ella para montar la otra de las estructuras de mandíbula sobre ellos; colocar un elemento (200) de ajuste de la separación sobre los elementos (112, 114) de montaje entre las estructuras de mandíbula para proporcionar una separación deseada entre las porciones (partes) delanteras de las porciones para alojamiento del yunque y del cartucho; y enclavar el elemento (200) de ajuste de la separación a los elementos de montaje para proporcionar la separación de grapado deseada, donde el elemento (200) de ajuste de la separación se enclava en su sitio mediante al menos uno de deformación, punteado, golpeteo, engarzado, punzonado, soldadura y pegado del elemento (200) de ajuste de la separación contra al menos uno de los elementos (112, 114) de montaje.

Método de ajuste de aparato de grapado quirúrgico
Antecedentes
1. Campo Técnico
La presente descripción se refiere a un método de colocación de elementos de ajuste de la separación de grapado en aparatos de grapado quirúrgico.
2. Antecedentes de la Técnica Relacionada
Los dispositivos quirúrgicos en los cuales el tejido primero se agarra o se pinza entre estructuras de mandíbula enfrentadas y se une a continuación por medio de elementos de sujeción quirúrgicos son muy conocidos en la técnica. En algunos instrumentos de este tipo se proporciona una cuchilla para cortar el tejido que se ha unido mediante los elementos de sujeción. Típicamente, los elementos de sujeción tienen la forma de grapas quirúrgicas, sin embargo, también se pueden utilizar otros elementos de sujeción quirúrgicos, tales como, por ejemplo, clips o elementos de sujeción quirúrgicos poliméricos de dos piezas.
Los instrumentos para la aplicación de elementos de sujeción quirúrgicos incluyen típicamente dos elementos de viga alargados, los cuales se utilizan respectivamente para capturar o pinzar tejido entre ellos. Típicamente, uno de los elementos de viga soporta a un cartucho desechable que contiene una pluralidad de grapas organizadas en al menos dos filas laterales mientras el otro elemento de viga comprende un yunque que define una superficie para conformar las patas de la grapa a medida que éstas van siendo impulsadas desde el cartucho. En los casos en que se usan elementos de sujeción de dos piezas, este elemento de viga soporta a la parte complementaria, por ejemplo, el receptor, para los elementos de sujeción impulsados desde el cartucho. Generalmente, el proceso de conformado de las grapas se efectúa mediante la interacción entre una superficie de leva que se mueve longitudinalmente y una serie de elementos empujadores de grapas individuales. A medida que la superficie de leva se desplaza longitudinalmente a través del elemento que soporta al cartucho, los elementos empujadores individuales son empujados lateralmente, actuando de esta forma sobre las grapas para expulsarlas secuencialmente desde el cartucho. Una cuchilla se puede desplazar con el empujador entre las filas de grapas para cortar longitudinalmente el tejido entre las filas de grapas conformadas. Ejemplos de instrumentos de este tipo se describen en las Patentes de EE.UU. Nº 3.079.606 y 3.490.675.
Una grapadora posterior descrita en la Patente de EE.UU. Nº 3.499.591 aplica una doble fila de grapas a cada lado de la incisión. Esto se consigue proporcionando un conjunto de cartucho en el cual un elemento de leva se mueve a través de una trayectoria de guiado alargada entre dos conjuntos de surcos de transporte de grapas colocados al tresbolillo. En el interior de los surcos están situados elementos para impulsión de las grapas y dichos elementos están colocados de tal manera que la leva que se mueve longitudinalmente hace contacto con ellos para provocar la expulsión de las grapas.
Se desea y es un objetivo principal de la presente descripción proporcionar aparatos para la aplicación de elementos de sujeción quirúrgicos mejorados, y proporcionar mecanismos y métodos mejorados para producir aparatos para la aplicación de elementos de sujeción quirúrgicos mejorados, teniendo dichos aparatos una separación de grapado uniforme fija que esté siempre dentro de un rango estrecho, claramente dentro del rango de tolerancia para la separación de grapado aceptable, y que produzca siempre grapas conformadas que estén dentro de un rango estrecho claramente dentro del rango de tolerancia para la separación de grapado aceptable.
Por consiguiente, existe la necesidad de un aparato para la aplicación de elementos de sujeción quirúrgicos que tenga una separación de grapado uniforme fija que esté siempre dentro de un rango estrecho y que permanezca uniforme durante una carrera de disparo. Además, existe la necesidad de un aparato para la aplicación de elementos de sujeción quirúrgicos que no disparará si en el aparato no está cargado de manera correcta un cartucho de grapas desechable o si no hay ningún cartucho cargado en el aparato, o que no disparará si el aparato para la aplicación de elementos de sujeción quirúrgicos se encuentra en un estado no pinzado. Existe la necesidad continuada de un aparato para la aplicación de elementos de sujeción quirúrgicos que no acepte en su interior un cartucho de grapas desechable completamente disparado o parcialmente disparado.
Con respecto a la técnica anterior, un punto de comienzo realista para la valoración de paso innovador bajo la técnica problema-solución es la Patente US 4608981, la cual describe un método de ajuste de una separación de grapado en un aparato de grapado quirúrgico que comprende: proporcionar un aparato de grapado quirúrgico que tenga una estructura de mandíbula con una porción para alojamiento del yunque y una estructura de mandíbula con una porción para alojamiento del cartucho, siendo las estructuras de mandíbula acoplables funcionalmente la una a la otra para proporcionar una separación de grapado entre las respectivas partes delanteras de la porción para alojamiento del yunque y de la porción para alojamiento del cartucho, teniendo una de las estructuras de mandíbula una pareja de elementos de montaje situados sobre ella para montar la otra de las estructuras de mandíbula sobre ellos; colocar un elemento de ajuste de la separación sobre los elementos de montaje entre las estructuras de mandíbula para proporcionar una separación deseada entre las partes delanteras de la porción para alojamiento del yunque y de la porción para alojamiento del cartucho, ajustándose la posición del elemento de ajuste de la separación con respecto a los elementos de montaje mediante un mando de ajuste.

Resumen
Se desea y es un objetivo principal de esta invención proporcionar métodos para producir grapadoras quirúrgicas mejoradas, teniendo dichas grapadoras una separación de grapado uniforme fija que esté siempre dentro de un rango estrecho, claramente dentro del rango de tolerancia para la separación de grapado aceptable, y que produzca siempre grapas conformadas que estén dentro de un rango estrecho claramente dentro del rango de tolerancia para la separación de grapado aceptable.
También es un objeto de la invención producir grapadoras quirúrgicas que, cuando se vendan para su uso, o antes de ser usadas, no tengan virtualmente ninguna variación en la separación de grapado de una grapadora a otra de un modelo dado y que proporcionen grapas mejoradas.
La presente descripción se dirige a elementos de ajuste de la separación de grapado lineal para ser usados en aparatos de grapado quirúrgico. La presente descripción también se dirige a métodos de ajuste y fijación de la separación de grapado de un aparato de grapado quirúrgico.
La presente invención es un método como define la reivindicación 1.

Descripción detallada de realizaciones preferidas
El mecanismo de ajuste de la separación – de grapado lineal descrito en este documento se describirá ahora en detalle haciendo referencia a los dibujos adjuntos, en los cuales números de referencia similares designan a elementos idénticos o correspondientes en cada una de las diferentes vistas.
Haciendo referencia inicialmente a las Figuras 1-3, un aparato de grapado quirúrgico se muestra de manera general con el número de referencia 100. El aparato 100 de grapado quirúrgico incluye una primera estructura 110 de mandíbula que tiene una estructura 112 para alojamiento del cartucho adaptada para alojar a un conjunto 130 de cartucho y una segunda estructura 120 de mandíbula que tiene una estructura 122 para alojamiento del yunque. El yunque (no mostrado) está unido a, fijado a, o es integral con, la estructura 122 para alojamiento del yunque. La estructura 112 para alojamiento del cartucho incluye un par de elementos de montaje, preferiblemente placas de montaje, mostradas aquí como placas 114 de bisagra verticales para conectar de forma abisagrada la primera estructura 110 de mandíbula a la segunda estructura 120 de mandíbula. Cada pareja de placas 114 de bisagra incluye una pareja de elevaciones 116 que definen un rebaje 118 para alojamiento conformado en ellas. Preferiblemente, el rebaje 118 para alojamiento está configurado y adaptado para alojar a un elemento de ajuste de la separación, en este caso una leva 200 excéntrica, por ejemplo mediante una conexión del tipo de encaje a presión.
La estructura 112 para alojamiento del cartucho y la estructura 122 para alojamiento del yunque están acopladas entre sí por medio de una pareja de pasadores de pivote, pasando un primer pasador 140 de pivote a través de las paredes laterales de la estructura 122 para alojamiento del yunque y pasando un segundo pasador 142 de pivote a través de las paredes laterales de la estructura 112 para alojamiento del cartucho. Los pasadores 140, 142 de pivote están interconectados por una pieza de conexión interna (no mostrada) que tiene una longitud fija, impidiendo dicha pieza de conexión el movimiento vertical de la estructura 112 para alojamiento del cartucho con respecto a la estructura 122 para alojamiento del yunque. La pareja de pasadores 140, 142 de pivote están situados entre la porción para conformado de las grapas del aparato de grapado y el rebaje 118 para alojamiento conformado en la pareja de placas 114 de bisagra. De esta manera, la estructura 112 para alojamiento del cartucho y la estructura 122 para alojamiento del yunque pivotarán alrededor de los pasadores 140, 142 de pivote.
Como se ve en las Figuras 2, 4 y 5, la leva 200 excéntrica incluye una porción 210 delantera cilíndrica, una porción 220 de cuerpo cilíndrica y una porción 230 trasera cilíndrica. La porción 210 delantera cilíndrica y la porción 230 trasera cilíndrica comparten un eje “A” central común. Aunque se representa la leva 200 excéntrica, en la Figura 2, teniendo la porción 210 delantera un diámetro diferente al de la porción 230 trasera, se prevé que tanto la porción 210 delantera como la porción 230 trasera puedan tener el mismo diámetro siempre y cuando compartan un eje central común. La porción 220 de cuerpo tiene un eje “B” central separado una distancia “X” del eje “A” central de las porciones 210 delantera y 230 trasera. Cuando la porción 220 de cuerpo es un cilindro, a medida que las porciones 210 delantera y 230 trasera giran alrededor del eje “A” común, la porción 220 de cuerpo actúa como una leva que mueve a través de una distancia máxima “2X” uno o más objetos colocados sobre la porción 220 de cuerpo o que engranan con la misma. Aunque el giro de la leva 200 excéntrica produce una máxima distancia de desplazamiento de “2X” se prevé que la leva 200 excéntrica se pueda dimensionar para provocar un desplazamiento de cualquier distancia apropiada con un giro de la misma.
Aunque la porción 220 de cuerpo se ha mostrado y descrito como un cilindro, se prevé que la porción 220 de cuerpo pueda adoptar cualquier otra forma (por ejemplo, un óvalo) que defina una superficie de leva y donde dicha otra forma determinaría la distancia “X” a través de la cual se moverían los uno o más objetos colocados sobre la porción 220 de cuerpo o que engranan con la misma. Por ejemplo, si la superficie de leva no es un cilindro, por ejemplo, si tiene forma de lágrima, entonces la parte más ancha o de copa de la lágrima no sería excéntrica con respecto al eje “A”. Aquí se entiende que una leva es una estructura que tiene una periferia con al menos dos puntos diferentes a lo largo de la periferia, teniendo cada punto un radio diferente medido desde el eje de giro. Las levas que se pueden utilizar de acuerdo con la presente descripción pueden tener cualquier forma apropiada (por ejemplo, triangular, oblonga, de lágrima y similares).
Volviendo a las Figuras 2 y 3, la porción 210 delantera y la porción 230 trasera de la leva 200 excéntrica están configuradas y adaptadas para alojarse dentro de la pareja de rebajes 118 para alojamiento de las placas 114 de bisagra mientras que la porción 220 de cuerpo tiene una longitud substancialmente igual a la separación existente entre las superficies interiores de la pareja de placas 114 de bisagra. trasera se colocan cada una en el interior de un respectivo rebaje 118 para alojamiento, las citadas porciones 210 delantera y 230 trasera se consideran estructuras portantes. Además, la estructura 122 para alojamiento del yunque está configurada y adaptada para estar situada entre la pareja de placas 114 de bisagra y para descansar y quedar apoyada sobre la porción 220 de cuerpo de la leva 200 excéntrica. De esta manera, a medida que se va haciendo girar la leva 200 excéntrica alrededor del eje “A” común de las porciones 210 delantera y 230 trasera, la porción 220 de cuerpo presiona contra la estructura 122 para alojamiento del yunque, haciendo girar a una porción posterior de la estructura 122 para alojamiento del yunque alrededor del pasador 140 de pivote, modificando de ese modo la distancia espacial entre la estructura 122 para alojamiento del yunque y la estructura 112 para alojamiento del cartucho en una distancia “2X” máxima. Dado que la porción 220 de cuerpo provoca el desplazamiento de la estructura 122 para alojamiento del yunque y de la estructura 112 para alojamiento del cartucho, una con respecto a la otra, la porción 220 de cuerpo se considera la estructura de leva. Se prevé que el aparato 100 de grapado quirúrgico se pueda modificar de tal manera que la porción 220 de cuerpo se considere la estructura portante y que las porciones 210 delantera y 230 trasera se consideren las estructuras de leva.
La leva 200 excéntrica proporciona al aparato 100 de grapado quirúrgico un elemento de ajuste sencillo mediante el cual una distancia espacial o separación “Y” (Figuras 3 y 6) entre una parte 126 delantera de la estructura 112 para alojamiento del cartucho y una parte 124 delantera de la estructura 122 para alojamiento del yunque se puede ajustar y configurar a una distancia predeterminada precisa con independencia de las tolerancias o variaciones que aparezcan como resultado del proceso de fabricación y o de montaje de los componentes individuales del aparato 100 de grapado quirúrgico. En otras palabras, la separación “Y” entre la parte 126 delantera de la estructura 112 para alojamiento del cartucho y la parte 124 delantera de la estructura 122 para alojamiento del yunque, se puede configurar a una tolerancia estrecha debido a la capacidad de ajuste proporcionada por la leva 200 excéntrica con independencia de si los componentes individuales del aparato 100 de grapado quirúrgico se fabrican o no con una tolerancia grande. Preferiblemente, como se ve en la Figura 6, la separación “Y” se mide entre una superficie o cara 112a superior de la estructura 112 para alojamiento del cartucho y una superficie o cara 122a inferior situada enfrente de la estructura 122 para alojamiento del yunque, a lo largo de la longitud de la parte delantera de la grapadora. Se considera que la parte delantera de la grapadora es la parte de la grapadora que es distal de las placas 114 de bisagra y que abarca o incluye a la extensión o porción longitudinal de trabajo del cartucho y del yunque. De forma alternativa, como se ve en la Figura 6A, la separación “Y” se mide entre una superficie 112b de contacto con el tejido de un conjunto 113 de cartucho que está situado en el interior de la estructura 126 para alojamiento del cartucho, y una superficie 122b de contacto con el tejido de un elemento 123 de yunque que está situado en el interior de la estructura 122 para alojamiento del yunque.
Como se ve en la Figura 6, después del montaje de la mayoría o de todos los componentes individuales del aparato 100 de grapado quirúrgico, se configura la separación “Y”, para la parte delantera del aparato 100 de grapado quirúrgico. De acuerdo con un método de configuración de la separación “Y”, se inserta un elemento 160 de calibración que tiene un espesor fijo predeterminado dentro de la separación “Y” en la punta distal del aparato 100 de grapado quirúrgico. La leva 200 excéntrica está fija o se hace girar entonces hasta que se configura la separación “Y” para que sea igual al espesor predeterminado de los medios 160 de calibración. La separación “Y” se configura en una condición descargada o cargada, preferiblemente descargada, es decir, sin tejido presente, o con algo de carga. Otros medios y métodos apropiados se pueden emplear al mismo tiempo o en otros momentos durante el proceso de fabricación o montaje o como parte del mismo.
Se ajusta la leva 200 excéntrica hasta que las estructuras 112, 122 para alojamiento hacen contacto con los medios 160 de calibración. Una vez que se ha fijado o configurado la posición de la leva 200 excéntrica, la citada leva 200 excéntrica se sujeta de forma fija o se enclava, mediante cualquier medio apropiado, en los rebajes 118, de tal manera que se impide cualquier giro adicional de la leva 200 excéntrica. Típicamente, la separación “Y” será menor en la punta distal y aumentará progresivamente desde la punta distal hasta las placas de bisagra, de tal manera que en la condición cargada, con tejido presente, se producirá deflexión en la punta para crear una separación más uniforme a lo largo de la parte 124, 126 delantera del aparato 100 de grapado quirúrgico. Aunque el ajuste y la configuración de la separación “Y” es un paso que se describe como si tuviera lugar durante la fabricación y montaje del aparato 100 de grapado quirúrgico, se prevé que el ajuste y configuración de la separación “Y” puedan hacerse después de la fabricación y del montaje, preferiblemente antes del empaquetado de la grapadora. Se prevé que, con algunas realizaciones, el usuario pueda realizar un pequeño ajuste.
Haciendo referencia a las Figuras 7-11, diferentes medios para hacer girar a la leva 200 excéntrica se proporcionan sobre la misma y se describirán ahora. Como se ve en la Figura 7, la porción 210 delantera de leva 200 excéntrica está provista de un rebaje 240 hexagonal conformado en una superficie final o superior de la misma. El rebaje 240 hexagonal está configurado para alojar a un extremo de tipo “Llave Allen” de una herramienta giratoria (no mostrada) para hacer girar a la leva 200 excéntrica. Aunque se muestra un rebaje 240 hexagonal, se prevé que se pueda proporcionar cualquier rebaje poligonal (es decir, triangular, cuadrado, pentagonal, etc.) junto con una herramienta giratoria configurada con una protrusión poligonal cuya forma sea correspondiente con la del rebaje poligonal conformado en la superficie final de la porción 210 delantera.
Como se ve en la Figura 8, la porción 210 delantera de la leva 200 excéntrica está provista de un rebaje 250 cónico conformado en una superficie final, superior o exterior de la misma, estando dicho rebaje 250 cónico provisto preferiblemente de una superficie 251 rugosa para acoplarse por rozamiento con una protrusión 252 cónica que se extiende desde un extremo distal de una herramienta 254 giratoria.

Como se ve en la Figura 9, la porción 210 delantera de la leva 200 excéntrica está provista de un rebaje 260 para destornillador convencional (es decir, para destornillador plano, de estrella o Phillips, torx, etc.) conformado en una superficie final, superior o exterior de la misma. El rebaje 260 para destornillador está configurado para alojar a un extremo de un destornillador estándar (no mostrado) para hacer girar a la leva 200 excéntrica.
Como se ve en la Figura 10, la porción 210 delantera de la leva 200 excéntrica está provista de un rebaje 270 con forma irregular conformado en una superficie final o superior de la misma. El rebaje 270 con forma irregular se puede conformar con una parte que esté situada a una distancia radial del eje “A” giratorio de la leva 200 excéntrica mayor que otra parte del rebaje 270. De la misma manera que los rebajes antes mencionados, para hacer girar a la leva 200 excéntrica se utiliza una herramienta giratoria (no mostrada) que tiene una protrusión configurada y adaptada para interaccionar con el rebaje 270.
Como se ve en la Figura 11, la porción 210 delantera de la leva 200 excéntrica está provista de un rebaje 280 circular conformado en una superficie final o superior de la misma. Una parte expandible de una herramienta 282 giratoria se representa con una parte 284 delantera cilíndrica que se puede expandir después de su inserción en el rebaje 280 circular de la leva 200 excéntrica. La expansión de la parte 284 delantera cilíndrica de la herramienta 282 giratoria, después de su inserción en el rebaje 280 circular, provoca que la superficie exterior de la parte 284 delantera cilíndrica presione contra la superficie interior del rebaje 280 circular permitiendo de ese modo que la herramienta 282 giratoria transmita giro a la leva 200 excéntrica. De forma alternativa, como se ve en la Figura 11A, se puede proporcionar una herramienta 286 giratoria con capacidad de contracción, incluyendo dicha herramienta 286 giratoria con capacidad de contracción una parte 288 delantera cilíndrica que se puede contraer alrededor del diámetro exterior de la porción 210 delantera de la leva 200 excéntrica. Por consiguiente, la contracción de la parte 288 delantera cilíndrica de la herramienta 286 giratoria, después de su colocación alrededor de la porción 210 delantera de la leva 200 excéntrica, provoca que la superficie interior de la parte 288 delantera cilíndrica presione contra la superficie exterior de la porción 210 delantera de la leva 200 excéntrica permitiendo de ese modo que la herramienta 286 giratoria transmita giro a la leva 200 excéntrica.
Aunque los rebajes están situados en, y los métodos y dispositivos para hacer girar a la leva 200 excéntrica se describen como realizados con respecto a, el extremo superior o la porción delantera de la leva 200 excéntrica, se entiende que los rebajes se pueden situar en, y los métodos y dispositivos se pueden realizar con respecto a, uno cualquiera de los extremos o ambos extremos de la leva 200 excéntrica.
Yendo ahora a las Figuras 12-14, se describen métodos preferidos de fijar firmemente una leva excéntrica a un aparato de grapado. De acuerdo con un método preferido, se describe una pareja de placas de bisagra para ser usadas con una leva 200 excéntrica que tiene una porción 230 trasera definida por una pared 232 anular. Como se ve en la Figura 12, la estructura 112 para alojamiento del cartucho está provista de una primera placa 160 de bisagra vertical y de una segunda placa 162 de bisagra vertical, cada una de las cuales se extiende desde la superficie lateral de la estructura 112 para alojamiento del cartucho. La primera placa 160 de bisagra está provista de un orificio 164 pasante que tiene al menos una muesca 166 conformada en un perímetro circunferencial del mismo y dimensionado para alojar en su interior o a través de él de forma ceñida a una parte de la porción 230 trasera de la leva 200 excéntrica. La segunda placa 162 de bisagra está provista de un orificio 168 pasante que está dimensionado para alojar en su interior o a través de él de forma ceñida a la porción 210 trasera de la leva 200 excéntrica. La Figura 13 pretende representar a la leva 200 excéntrica introducida de forma ceñida en el interior de la pareja de orificios 164, 168 pasantes. La Figura 14 pretende mostrar que después de que se haya hecho girar la leva 200 excéntrica hasta la posición deseada, dicha leva 200 excéntrica se sujeta de forma fija o se enclava en su sitio deformando la pared 232 anular para introducirla a presión en el interior de muescas 166 conformadas a lo largo del perímetro circunferencial del orificio 164 pasante.
Yendo ahora a las Figuras 15-17, se describen métodos alternativos de fijar firmemente la leva 200 excéntrica a la pareja de placas 160, 162 de bisagra. Como se ve en la Figura 15, la porción 230 trasera de la leva 200 excéntrica se fija firmemente en su sitio mediante una o más soldaduras 300 por puntos (es decir, soldadura con metal-gasinerte “MIG”, soldadura con tungsteno-gas-inerte “TIG”, soldadura por arco, láser, sónica, por haz de electrones, soldadura fuerte, soldadura con plata, soldadura blanda, etc.) entre la porción 230 trasera y la placa 160 de bisagra. Como se ve en la Figura 16, la leva 200 excéntrica se puede fijar firmemente en su sitio mediante una deformación 310 de una porción del metal en forma de lámina de la primera placa 160 de bisagra (por ejemplo, mediante punteado, golpeteo, engarzado, punzonado, etc.) para introducirla en el interior de al menos uno, preferiblemente de cada uno, de los rebajes 234 conformados a lo largo de la circunferencia de la porción 230 trasera. Como se ve en la Figura 17, la leva 200 excéntrica se puede fijar firmemente en su sitio con uno o más golpeteos 320 alrededor de la porción 230 trasera de la leva 200 excéntrica hasta que la porción 230 trasera se expanda lo suficiente para rellenar y cerrar el orificio 164 pasante para impedir el giro de la leva 200 excéntrica en su interior. También se prevé que el metal en forma de lámina de la placa 160 de bisagra se pueda golpetear alrededor del orificio 164 pasante hasta que el metal en forma de lámina agarre con fuerza a la porción 230 trasera de la leva 200 excéntrica. De forma alternativa, se puede aplicar cualquier adhesivo o agente de unión apropiado, por ejemplo, un adhesivo anaeróbico, tal como LoctiteTM (Marca Registrada de American Sealants Company, Corp.) o equivalentes, u otros cementos fluidos de tipo adhesivo, entre las superficies de las porciones 210 delantera y 230 trasera y los orificios 164, 168 pasantes, respectivamente.

Como se ve en la gráfica de la Figura 18, se muestran las separaciones de grapado aceptables para grapadoras quirúrgicas fabricadas de acuerdo con el estado del arte actual en comparación con las fabricadas de acuerdo con la presente descripción. El estado del arte actual proporciona como resultado la fabricación y montaje de grapadoras quirúrgicas que tienen una gran variación en el rango de la separación de grapado resultante. Durante la fabricación 5 y montaje de una línea o de un modelo específico de grapadoras quirúrgicas, debido a diferentes factores inherentes
o implicados en la fabricación y montaje de las grapadoras quirúrgicas, por ejemplo, variaciones en materiales, dimensiones y condiciones, ha habido una gran variación en el tamaño de la separación “Y” de grapado dentro del rango de tolerancia para la separación de grapado aceptable “ASG”, como se representa mediante la curva “D” con forma de campana en la Figura 18. Sin embargo, de acuerdo con la presente descripción, a pesar de los diferentes 10 factores descritos anteriormente, las grapadoras quirúrgicas fabricadas y montadas pueden tener siempre un rango más estrecho de variaciones que está completamente dentro del rango aceptable, como se representa mediante la curva “E” con forma de campana en la Figura 18. De esta forma, las grapadoras quirúrgicas fabricadas y montadas de acuerdo con la presente descripción tienen separaciones de grapado que son más consistentes y uniformes que las separaciones de grapado de las grapadoras quirúrgicas fabricadas o producidas de acuerdo con el estado del
15 arte actual.
Aunque se muestra que la leva 200 excéntrica está fijada o sujeta a una placa de bisagra, se prevé que la leva 200 excéntrica pueda estar sujeta a ambas placas de bisagra. Además, aunque se ha descrito una leva circular, se prevé que se puedan utilizar otros tipos de elementos de leva sin apartarse del alcance de la descripción, por ejemplo, se puede colocar una rampa entre la estructura para alojamiento del cartucho y la estructura para alojamiento del
20 yunque, se puede configurar una lengüeta plegable, fijada a una de las estructuras y adaptada para mover la estructura para alojamiento del cartucho con respecto a la estructura para alojamiento del yunque, y se pueden emplear uno o más husillos para provocar el movimiento hasta hacer contacto con la estructura para alojamiento del yunque. Además, la presente descripción es especialmente adecuada para grapadoras quirúrgicas lineales, sin embargo, se prevé que la presente descripción no esté limitada únicamente a grapadoras quirúrgicas lineales.

Un método de ajuste de una separación (Y) de grapado en un aparato (100) de grapado quirúrgico, que comprende:proporcionar un aparato de grapado quirúrgico que tenga una estructura (120) de mandíbula con una porción (122)5 para alojamiento del yunque y una estructura (110) de mandíbula con una porción (112) para alojamiento del cartucho, siendo las estructuras de mandíbula funcionalmente acoplables la una a la otra para proporcionar una separación de grapado entre una porción (parte) delantera respectiva de las porciones para alojamiento del yunque y del cartucho, teniendo una de las estructuras de mandíbula una pareja de elementos (112, 114) de montaje situados sobre ella para montar la otra de las estructuras de mandíbula sobre ellos;10 colocar un elemento (200) de ajuste de la separación sobre los elementos (112, 114) de montaje entre las estructuras de mandíbula para proporcionar una separación deseada entre las porciones (partes) delanteras de las porciones para alojamiento del yunque y del cartucho; yenclavar el elemento (200) de ajuste de la separación a los elementos de montaje para proporcionar la separación de grapado deseada,15 donde el elemento (200) de ajuste de la separación se enclava en su sitio mediante al menos uno de deformación, punteado, golpeteo, engarzado, punzonado, soldadura y pegado del elemento (200) de ajuste de la separación contra al menos uno de los elementos (112, 114) de montaje.
- 2. El método de acuerdo con la reivindicación 1, en el cual:el elemento (200) de ajuste de la separación es una leva (200) excéntrica que comprende una porción (210)20 delantera, una porción (230) trasera definida por una pared (232) anular, y una porción (220) de cuerpo, compartiendo la porción (210) delantera y la porción (230) trasera un eje (A) central común;una porción de la porción trasera está alojada de forma ceñida por un perímetro circunferencial de un orificio (164) pasante conformado sobre una primera (160) de las placas de montaje;la porción delantera está alojada de forma ceñida por un orificio (168) pasante conformado sobre una segunda (162) 25 de las placas de montaje; ytras el enclavamiento del elemento (200) de ajuste de la separación, la pared anular se deforma para introducirla a presión en el interior de muescas (166) conformadas a lo largo del perímetro circunferencial del orificio pasante de la primera de las placas de montaje.8
