Abstract
El uso de taurolidina y interleucina-2 en la fabricación de una preparación combinada parasimultáneamente, separadamente o secuencialmente tratar el melanoma maligno.
Description
Tratamiento de las metastasis tumorales y el cancer
La presente invencion se refiere al campo del tratamiento de las metastasis de tumores y el cancer.
La interleucina-2 (IL-2) es un agente que ha sido sugerido para inhibir el crecimiento de celulas tumorales. Sin
5 embargo, la administracion de IL-2 a los pacientes presenta graves problemas de toxicidad, ya que la IL-2 provoca una reaccion de sindrome de respuesta inflamatoria sistemica muy fuerte (SIRS) en los pacientes. La toxicidad de la IL-2 es tan severa que aproximadamente el 70% de los pacientes no pueden tolerar el tratamiento.
Adicionalmente, un problema comun en pacientes en tratamiento de cancer es la recurrencia del tumor o la metastasis.
10 Por lo tanto, a pesar de los avances en el tratamiento del cancer, sigue existiendo una necesidad significativa en la tecnica de nuevas y mejores terapias de tratamiento del cancer.
De acuerdo con la presente invencion, se inhibe la metastasis tumoral en un paciente de cancer mediante la administracion a dicho paciente de una terapia de combinacion que comprende cantidades eficaces de IL-2 y un agente de transferencia de metilol
15 Sorprendentemente, se ha encontrado que el agente de transferencia de metilol taurolidina reduce o elimina sustancialmente la toxicidad grave y los efectos secundarios de la IL-2 en una terapia de combinacion para inhibir las metastasis tumorales y tratar el cancer en los pacientes, mientras que inesperadamente se ha encontrado que con la terapia de combinacion de la presente invencion la eficacia de la IL-2 en realidad es reforzada por el agente de transferencia de metilol.
20 La IL-2 cuando se utiliza de acuerdo con la presente invencion incluye la interleucina-2 natural o recombinante, o derivados biologicamente activos o equivalentes sustanciales de los mismos.
Los agentes de transferencia de metilol incluyen la taurolidina. Los compuestos taurolidina y taurultam se dan a conocer en la patente de los Estados Unidos N° 5.210.083. Otros compuestos que contienen metilol se pueden encontrar entre los identificados en la publicacion PCT de la patente internacional N° WO 01/39763. Los agentes de
25 transferencia de metilol para la utilizacion segun la presente invencion es la taurolidina.
Las formas de realizacion preferidas de la invencion implican el tratamiento de canceres seleccionados del grupo que consiste en el melanoma maligno y la inhibicion de las metastasis tumorales del mismo.
Cantidades de dosificacion diaria eficaces de IL-2 pueden comprender unidades de dosificacion farmaceutica dentro del intervalo de 1.000.000-100.000.000 de unidades (U) de IL-2 por m2 de area de superficie corporal. Las
30 cantidades de dosificacion de IL-2 tambien se pueden encontrar dentro del intervalo de 100.000-1.000.000 U por kilogramo de peso corporal. Las cantidades de dosificacion de IL-2, ademas se pueden encontrar dentro del intervalo de 0,1-100 microgramos de IL-2 por kilogramo de peso corporal.
Cantidades de dosificacion eficaces de taurolidina segun la presente invencion pueden comprender unidades de dosificacion farmaceutica dentro del intervalo de alrededor de 0,1-1.000 mg/kg. Las dosis preferidas pueden estar en
35 el intervalo de aproximadamente 10-20 gramos de taurolidina.
Las unidades de dosificacion farmaceuticas de la terapia combinada de la presente invencion pueden ser administradas por cualquier via adecuada, lo que incluye la administracion por via oral, topica o peritoneal, por ejemplo, subcutanea, intraperitoneal, intramuscular, o intravenosa por ejemplo, por infusion o inyeccion.
En realizaciones preferidas, se administran 250 ml de taurolidina en solucion al 2% por infusion intravenosa
40 aproximadamente 1-6 veces al dia, mas preferiblemente alrededor de 2-4 veces al dia, durante un periodo de tratamiento, concurrentemente con la administracion de alrededor de 10.000.000-40.000.000 m2 unidades de IL-2 mediante infusion intravenosa, por dia, durante el periodo de tratamiento.
La presente invencion tambien se refiere a una combinacion de IL-2 y un agente de transferencia de metilol, en cantidades eficaces para el uso simultaneo, separado o secuencial a fin de inhibir la metastasis tumoral en un
45 paciente de cancer. La invencion tambien esta dirigida a combinaciones farmaceuticas que incluyen unidades de dosificacion farmaceuticas que comprenden cantidades eficaces de interleucina-2 y un agente de transferencia de metilol para inhibir la metastasis tumoral en un paciente de cancer, asi como las composiciones farmaceuticas que comprenden tales combinaciones.
La invencion se ilustra adicionalmente mediante los siguientes ejemplos.
Se trato a un paciente de 63 anos diagnosticado con melanoma maligno metastasico como sigue. Presentacion Masa supra-clavicular derecha. Originalmente habia melanoma nodular extirpado desde el codo derecho, y tuvo altas dosis de interferon despues de la cirugia. Requirio eliminacion axilar para una masa en axila derecha ocho meses despues. Ademas el escenario estuvo claro en ese momento. Se
presento un ano mas tarde con una masa inoperable en el area supra clavicular derecha. Tratamiento IL-2 y taurolidina Regimen Interleucina-2
Dia 1: 18 millones unidades/m2 IL-2 en infusion durante 6 horas Dia 2: 18 millones unidades/m2 IL-2 en infusion durante 12 horas Dia 3: 18 millones de unidades/m2 IL-2 en infusion durante 24 horas Dias 4-7: 18 millones de unidades/m2 IL-2 en infusion durante 78 horas Taurolidina Taurolidina 2% 250 ml en infusion durante doce horas diariamente durante la administracion de IL-2 Completo cinco sesiones del tratamiento anteriormente
Despues de un ano, el paciente esta vivo y bien, sin evidencia de la enfermedad en las imagenes.
5 Un paciente de 50 anos diagnosticado de carcinoma de celulas renales metastasico se trato como se indica a continuacion. Presentacion Hemoptisis -secundaria a metastasis pulmonares. Se observo metastasis hepaticas, ademas de una masa grande en el rinon izquierdo Tratamiento IL-2 y taurolidina Regimen Interleucina-2 Dia 1: infusion de 18 millones unidades/m2 de IL-2 durante 6 horas Dia 2: infusion de 18 millones unidades/m2 de IL-2 durante 12 horas Dia 3: infusion de 18 millones unidades/m2 de IL-2 durante 24 horas Dias 4-7: infusion de 18 millones unidades/m2 de IL-2 durante 78 horas
Taurolidina Infusion de taurolidina al 2% 250 ml de infusion durante dos horas, dos veces al dia durante la administracion de IL-2
Completado cinco sesiones segun lo descrito anteriormente
Completado cinco sesiones segun lo descrito anteriormente.
Tratamiento posterior: nefrectomia radical izquierda.
Despues de cinco anos, el paciente esta vivo y bien, sin evidencia de la enfermedad en las imagenes.
Un paciente de sexo masculino que tenia melanoma nodular recurrente despues de tratamiento con interferon fue posteriormente tratado con interleucina-2 y taurolidina de la siguiente manera: Presentacion Recurrencia de la lesion de melanoma nodular en el h ombro derech o.
Tratamiento IL-2 y taurolidina
Regimen Interleucina-2 Dia 1: infusion de 36 millones unidades/m2 de IL-2 durante 6 horas Dia 2: infusion de 36 millones unidades/m2 de IL-2 durante 12 horas
5 Dia 3: infusion de 36 millones unidades/m2 de IL-2 durante 24 horas. Dias 4-7: infusion de 36 millones unidades/m2 de IL-2 durante 78 horas Taurolidina Infusion de Taurolidina al 2%, 250 ml durante doce horas, de forma
secuencial con la administracion de IL-2, durante los dias 1-610 Se llevo a cabo cinco sesiones -- en la segunda sesion, el tratamiento fue interrumpido y se detuvo a las 78 h oras, y durante la quinta sesion, el tratamiento fue interrumpido durante el dia 4. El seguimiento por tomografias computarizadas indico una reduccion en el tamano de la lesion, y, posteriormente, no indico ninguna evidencia de enfermedad. 15

- 1.
- El uso de taurolidina y interleucina-2 en la fabricacion de una preparacion combinada para simultaneamente, separadamente o secuencialmente tratar el melanoma maligno.

- El uso segun se reivindica en la reivindicacion 1, en donde dicha preparacion combinada se fabrica de manera que dicha interleucina-2 y dicha taurolidina estan cada una presente en la misma unidad de dosificacion farmaceutica.
