Abstract
Utilización de interleuquina-2 y taurolidina en la fabricación de una preparación combinada destinada al tratamiento simultáneo, separado o secuencial del carcinoma metastásico de células renales.
Description
Tratamiento del carcinoma renal metastático.
La presente invención se refiere al campo del
tratamiento de las metástasis tumorales y del cáncer.
La interleuquina 2 (IL-2) es un
agente que se ha propuesto para inhibir el crecimiento de las
células tumorales. Sin embargo, la administración en los pacientes
de IL-2 presenta problemas de toxicidad severa,
debido a que la IL-2 induce un síndrome de
respuesta inflamatoria sistémica extremadamente fuerte (SIRS) en el
paciente. La toxicidad de la IL-2 es tan severa que
aproximadamente 70% de los pacientes no consigue tolerar el
tratamiento.
Además, un problema habitual en los pacientes
sometidos a tratamiento de cáncer es la recurrencia del tumor o la
metástasis.
De esta manera, a pesar de los avances en el
tratamiento del cáncer, sigue existiendo una necesidad significativa
en la técnica de terapias nuevas y mejoradas de tratamiento del
cáncer.
Según la presente invención, se inhibe la
metástasis tumoral en un paciente de cáncer mediante la
administración en dicho paciente de una terapia de combinación que
comprende cantidades eficaces de IL-2 y el agente de
transferencia de metilol llamado taurolidina.
Inesperadamente se ha encontrado que el agente
de transferencia de metilol llamado taurolidina puede reducir o
eliminar sustancialmente la toxicidad severa y los efectos
secundarios de la IL-2 en una terapia de combinación
destinada a la inhibición de las metástasis tumorales y el
tratamiento de cáncer en pacientes, e inesperadamente se ha
encontrado que la eficacia de IL-2 de hecho resulta
incrementada por el agente de transferencia de metilol en la
terapia de combinación de la presente invención.
La utilización de IL-2 según la
presente invención incluye interleuquina 2 natural o recombinante, o
derivados biológicamente activos o equivalentes sustanciales de los
mismos.
Se han dado a conocer agentes de transferencia
de metilol tales como taurolidina y taurultam como agentes
anticáncer en la patente WO 92/00743 y en el documento
EP-A-1066830. Pueden encontrarse
otros compuestos anticáncer que contienen metilol entre los
identificados en la patente WO 01/39763.
La terapia de combinación de la presente
invención se ha encontrado que resulta particularmente eficaz en la
inhibición del carcinoma metastásico de células renales.
Las dosis diarias eficaces de
IL-2 pueden comprender unidades de dosificación
farmacéutica de entre 1.000.000 y 100.000.000 unidades (U) de
IL-2 por m^{2} de superficie corporal. También
pueden encontrarse cantidades de dosificación de
IL-2 de entre 100.000 y 1.000.000 U por kilogramo de
peso corporal. Además, pueden encontrarse cantidades de
dosificación de IL-2 comprendidas en el intervalo de
entre 0,1 y 100 microgramos de IL-2 por kilogramo
de peso corporal.
Las dosis eficaces del agente de transferencia
de metilol llamado taurolidina según la presente invención pueden
comprender unidades de dosificación farmacéutica en el intervalo de
entre aproximadamente 0,1 y 1.000 mg/kg. Las dosis preferentes
pueden encontrarse comprendidas en el intervalo de entre
aproximadamente 10 y 20 gramos cada administración.
Las unidades de dosificación farmacéutica de la
terapia combinada de la presente invención pueden administrarse a
través de cualquier vía adecuada, incluyendo la administración oral,
tópica o peritoneal, por ejemplo subcutánea, intraperitoneal,
intramuscular o intravenosa, por ejemplo mediante infusión o
inyección.
En realizaciones preferentes, se administran 250
ml de solución de taurolidina al 2% mediante infusión intravenosa
aproximadamente 1 a 6 veces al día, más preferentemente
aproximadamente 2 a 4 veces al día, durante un periodo de
tratamiento, concurrentemente con la administración de entre
aproximadamente 10.000.000 y 40.000.000 unidades/m^{2} de
IL-2 mediante infusión intravenosa al día durante el
periodo de tratamiento.
De esta manera, la presente invención se refiere
a la utilización de una combinación de IL-2 y agente
de transferencia de metilol llamado taurolidina, en la preparación
de un medicamento destinado al tratamiento simultáneo, separado o
secuencial del carcinoma metastásico de células renales.
La invención se ilustra adicionalmente a partir
del ejemplo no limitativo siguiente.

Un paciente de 50 años diagnosticado de
carcinoma metastásico de células renales se sometió a tratamiento
de la manera siguiente:
- Presentación
- Hemoptisis - secundaria a metástasis pulmonares. Se observó la presencia de metástasis hepática, además de una masa grande en el riñón izquierdo.
- Tratamiento
- IL-2 y taurolidina
- Régimen
- \underbar{Interleuquina-2}

- Día 1: 18 millones de unidades/m^{2} de infusión de IL-2 durante 6 horas
- \quad
- Día 2: 18 millones de unidades/m^{2} de infusión de IL-2 durante 12 horas
- \quad
- Día 3: 18 millones de unidades/m^{2} de infusión de IL-2 durante 24 horas
- \quad
- Días 4 a 7: 18 millones de unidades/m^{2} de infusión de IL-2 durante 78 horas
- \quad
- \underbar{Taurolidina}
- \quad
- Infusión de 250 ml de taurolidina al 2% durante dos horas, dos veces al día durante la administración de la IL-2

- Se completaron cinco cursos de lo anteriormente indicado
- Tratamiento adicional
- Nefrectomía radical izquierda
Tras cinco años, el paciente se encuentra vivo y
saludable, sin indicios de enfermedad según el estudio de
imágenes.
Claims (2)
1. Utilización de
interleuquina-2 y taurolidina en la fabricación de
una preparación combinada destinada al tratamiento simultáneo,
separado o secuencial del carcinoma metastásico de células
renales.
2. Utilización según la reivindicación 1, en la
que dicha preparación combinada se fabrica de manera que dicha
interleuquina-2 y dicha taurolidina se encuentren
presentes, cada una, en forma de unidades de dosificación
farmacéutica separadas.
