Abstract
Un catéter (10) de crioplastia, que comprende: un eje (12) que tiene extremos proximal y distal, en donde el eje define un lumen (30) de inflado, un lumen (40) de entrada de refrigerante y un lumen de salida (44) a su través, teniendo cada lumen un extremo proximal y distal próximos a los extremos proximal y distal del eje respectivamente; un balón (14) de dilatación dispuesto en el extremo distal del eje, y en comunicación fluida con el lumen de inflado; y una cámara (42) dispuesta dentro del balón y en comunicación fluida con los lúmenes de entrada y salida, en donde un orificio (48) en el extremo distal del lumen de entrada del refrigerante tiene un diámetro menor que el de un orificio en el extremo (50) del lumen de salida.
Description
La presente invención pertenece generalmente al campo de la angioplastia, y en particular a una forma de angioplastia que incluye el enfriamiento de la lesión.
La angioplastia convencional ha evolucionado durante varias décadas, prolongando las vidas de un número creciente de pacientes. Los procedimientos de la angioplastia incluyen la dilatación de un balón colocado a través de una lesión en una arteria coronaria. La dilatación del balón a su vez dilata la lesión, abriendo la arteria para conseguir un flujo sanguíneo incrementado. En algunos casos, no obstante, el objetivo del procedimiento de la angioplastia es frustrado en la totalidad o en una parte por la reoclusión completa o parcial de la arteria en la lesión. Se cree que existen 10 dos mecanismos responsables principalmente de la reoclusión de la arteria, que corresponden a la reestenosis y el retroceso elástico. Se cree que la reestenosis está provocada por el crecimiento o recrecimiento continuado de las células lisas del músculo asociadas con la lesión. El retroceso elástico es en parte un proceso mecánico que incluye el rebote elástico de la lesión dilatada.
Se han expuesto varios medios para solucionar el problema de la reestenosis. Estos medios incluyen, entre 15 otros, los tratamientos de radiación para ralentizar o prevenir la proliferación de células de músculo liso asociada con el proceso de reestenosis. Se han propuesto ciertas terapias de fármacos para prevenir o ralentizar la reestenosis.
Se han desarrollado también varios medios para solucionar el tema del retroceso elástico. Uno de los desarrollos más significativos en este campo ha sido los stents, los cuales pueden desplegarse permanentemente para el sostenimiento mecánico de las lesiones abiertas. Aunque los stents han demostrado ser altamente efectivos, pueden 20 irritar la pared de una arteria en donde se hayan implantado. Algunos facultativos creen que esto puede provocar una reestenosis limitada. El calentamiento de la lesión durante la dilatación se ha expuesto para prevenir o ralentizar el retroceso. El calentamiento de la lesión se cree que ablanda las lesiones de forma tal que puedan “remodelarse”, es decir, que se adelgacen a baja presión. Se cree no obstante que el calentamiento de la lesión provoca una respuesta de la lesión que puede generar alguna reestenosis. 25
El estado de la técnica de acuerdo con el documento US-A-5501681 se refiere a un catéter con un balón distensible que tiene un conducto de refrigerante.
La presente invención está dirigida a un aparato para la ejecución de una angioplastia y prevenir o bien ralentizar la reoclusión en el post-procedimiento de una lesión dilatada. La presente invención enfría la lesión para 30 prevenir o ralentizar la reoclusión mediante los mecanismos de la reestenosis o del retroceso elástico. Se proporciona un catéter de crioplastia para enfriar la lesión y ayudar al remodelado de la lesión, para prevenir o para impedir o ralentizar el retroceso elástico. La presente invención puede utilizarse también para enfriar la lesión para congelar una porción del tejido de la lesión. Esto se cree que elimina las células dentro de la lesión que podrían activar la reestenosis. Una realización preferida del catéter de crioplastia de acuerdo con la presente invención incluye un eje que tiene unos 35 extremos proximal y distal. El eje define un lumen de inflado, un lumen de entrada del refrigerante y un lumen de salida a través del mismo. Cada lumen tiene un extremo proximal y un extremo distal próximos a los extremos proximal y distal del eje, respectivamente. Se dispone de un balón de dilatación en el extremo distal del eje y está en comunicación fluida con el lumen de inflado. Se dispone de una cámara dentro del balón y está en comunicación fluida con los lúmenes de entrada y de salida. Se encuentra conectado un refrigerante al extremo proximal del eje en comunicación fluida con el 40 lumen de entrada del refrigerante.
Puede disponerse un sensor termo-resistivo sobre el balón de dilatación para controlar la temperatura de la lesión. Puede disponerse un segundo sensor termo-resistivo sobre el eje para proporcionar la lectura de la temperatura de control.
En otra realización preferida del catéter de crioplastia de acuerdo con la presente invención, en donde el 45 catéter de crioplastia incluye un eje que tiene unos extremos proximal y distal. El eje define un lumen de inflado y un lumen de drenaje a su través. Cada lumen tiene un extremo proximal y distal próximos a los extremos proximal y distal del eje, respectivamente. Se dispone de un balón de dilatación en el extremo distal del eje y está en comunicación fluida con los lúmenes de inflado y de drenaje. El catéter de crioplastia incluye también una fuente de refrigerante conectada al extremo proximal del eje en comunicación fluida con el lumen de inflado. 50
Esta realización del catéter de crioplastia puede incluir también un sensor termo-resistivo dispuesto sobre el balón de dilatación. Al igual que el sensor termo-resistivo dispuesto sobre el balón, puede estar dispuesto un sensor de control sobre el eje del catéter.
Un método para la ejecución de la operación de crioplastia incluye las etapas de hacer avanzar un catéter de crioplastia a través de una lesión, inflando el balón de dilatación para dilatar la lesión, y suministrar refrigerante al balón 55
para enfriar la lesión. Para ayudar al remodelado, la lesión adyacente del balón puede enfriarse entre 10oC y -10oC. Una porción de la lesión adyacente al balón puede congelarse también para eliminar las células dentro de la lesión, las cuales podrían activar de lo contrario la reestenosis. Para un rendimiento mejor, la congelación puede realizarse por congelación rápida del tejido durante 20 a 60 segundos. Las células se congelan preferiblemente a una temperatura de entre -20oC y -40oC. 5

Con referencia ahora a los dibujos en donde los numerales de referencia iguales se refieren a elementos iguales a través de las distintas vistas, la figura 1 es una vista lateral de un catéter 10 de crioplastia de acuerdo con la 15 presente invención. El eje 12 tiene un extremo proximal y un extremo distal. Se dispone de un balón de dilatación en el extremo distal del eje 12. En el extremo proximal del eje 12 se encuentra un colector 16. Conectada al colector 16 se encuentra una bomba 18, la cual puede bombearse a mano mediante el balón de inflado 14. La fuente de refrigerante 20 está conectada también al colector 16, que según se explica más adelante proporciona un suministro de refrigerante al balón 14. El catéter 10 puede estar provisto con un sensor 22 de temperatura termo-resistivo para monitorizar la 20 temperatura de una lesión y un sensor 24 de control termo-resistivo conectado a un monitor 26.
La figura 2 es una vista en sección transversal longitudinal del extremo distal del catéter 10, incluyendo el extremo distal del eje 12 y el balón 14. El eje 12 incluye un tubo exterior 28, el cual define un lumen 30 de inflado en comunicación fluida con el interior del balón 14. El tubo 32 del alambre de guía que define un lumen 34 del alambre de guía puede extenderse a través al menos de una porción del eje 12 hacia el extremo distal del balón 14. Puede 25 disponerse una banda de marcado 36 sobre el tubo 32 del alambre de guía dentro del balón 14.
El eje 12 incluye también un tubo 38 de entrada del refrigerante que define un lumen 40 del refrigerante en comunicación fluida con una cámara de enfriamiento 42 dispuesta dentro del balón 14. El eje 12 incluye también un tubo de salida o de drenaje 44, el cual define un lumen 46 de salida o drenaje, en comunicación fluida con la cámara 42. En el extremo distal del tubo 38 se encuentra un orificio 48, el cual tiene un diámetro menor que el del orificio 50 en el 30 extremo distal del tubo 44. El diámetro del orificio 48 podría ser, por ejemplo, de aproximadamente 0,102 mm o mayor o menor dependiendo del diámetro del orificio 50.
Los técnicos especializados en la técnica reconocerán los distintos materiales que pueden ser utilizados ventajosamente para fabricar el catéter de la presente invención. Aquellos elementos que no se hayan encontrado en el catéter de angioplastia convencional, tales como el tubo 38 de entrada de refrigerante, cámara 42 y el tubo de salida 44 35 pueden fabricarse a partir de materiales conocidos por los técnicos especializados en la técnica. Por ejemplo, el tubo de entrada 38 puede ser un hipotubo o un tubo de poliamida que tenga un diámetro interior de, por ejemplo, entre 0,0254 mm a 0,508 mm, pero preferiblemente entre 0,0508 mm y 0,254 mm. El tubo de salida 44 puede fabricarse a partir de poliamida y tener un diámetro interno que sea preferiblemente mayor que el diámetro interno del tubo de entrada 38. La cámara 42 puede fabricarse a partir de poliamida. Estos materiales y las dimensiones deberán considerarse solo a 40 modo de ejemplo, de la forma que los técnicos especializados en la técnica podrán apreciar en la aplicabilidad de las dimensiones y materiales alternativos para estos elementos.
La figura 3 es una vista esquemática de otra realización de un catéter de crioplastia de acuerdo con la presente invención referida al numeral 100. El catéter de crioplastia 100 incluye un eje 112 que tiene un extremo distal y un extremo proximal. El balón 114 de dilatación está dispuesto en el extremo distal del eje 112. Cerca del extremo 45 proximal del eje 112 se encuentra una bomba 118 conectada a una fuente de refrigerante 120, que puede incluir la refrigeración para controlar la temperatura del refrigerante.
El tubo 132 del alambre de guía define un lumen 134 del mismo, que se extiende a través al menos de una porción del eje 112 hacia el extremo distal del catéter 100. El tubo 138 de entrada/inflado del refrigerante tiene un extremo proximal y un extremo distal cerca de los extremos proximal y distal del eje 112 que define un lumen 140 del 50 refrigerante/inflado. El lumen 140 está en comunicación fluida con la bomba 118 cerca de su extremo proximal y el balón 114 cerca de su extremo distal. El eje 112 incluye también un tubo 144 de salida/drenaje que define un lumen 146, y que tiene un extremo proximal y un extremo distal cerca de los extremos proximal y distal del eje 112, respectivamente. El extremo distal del lumen 146 está en comunicación fluida con el balón 114. El extremo proximal del lumen 146 puede estar en comunicación fluida con la fuente de refrigerante 120 para el reciclado del refrigerante, o bien puede 55
descargarse para su desecho. El lumen 140 tiene un orificio distal 148, el cual es preferiblemente menor que el orificio distal 150 del lumen 146.
Los técnicos especializados en la técnica reconocerán que existen numerosos materiales y métodos de fabricación que pueden ser adecuados para la producción del catéter 100. Los elementos del catéter 100 que no son típicos de los catéteres de angioplastia tales como el tubo 138 de inflado/entrada y el tubo 144 de salida/drenaje pueden 5 fabricarse tal como se ha descrito anteriormente con respecto a los elementos correspondientes del catéter 10. Se observará no obstante que el tubo 138 deberá estar dimensionado debidamente para servir su doble propósito tal como un tubo de inflado y un tubo de entrada de refrigerante. Adicionalmente, las dimensiones de los tubos 138 y 144, así como también los elementos correspondientes del catéter 10 deberán dimensionarse para tener en cuenta las propiedades físicas de un medio de refrigerante en particular por ejemplo, y la tasa relativa de la transferencia de calor 10 al medio refrigerante de una lesión.
Otra realización alternativa 212 de un catéter de crioplastia de acuerdo con la presente invención es el mostrado en la figura 4. El catéter de crioplastia 212 es esencialmente similar al catéter 12 mostrado en la figura 2, excepto en que una vaina 260 rodea el tubo 28 para definir un lumen anular 262 entre la vaina 260 y el tubo 28. La vaina 260 se extiende preferiblemente desde el balón 14 proximalmente hacia el colector 16. El colector 16 para esta 15 realización puede incluir una abertura adicional en comunicación fluida con el lumen 262. Excepto para la salida adicional, el lumen 262 deberá estar completamente sellado, de forma tal que pueda mantenerse un vacío dentro del lumen 262 al aplicar una fuente de vacío a la abertura. Puede crearse un vacío en el lumen 262 durante el procedimiento para proporcionar el aislamiento entre el refrigerante y el paciente. La vaina 262 puede fabricarse a partir de materiales biocompatibles conocidos por los técnicos especializados en la técnica de la construcción de catéteres 20 que sean suficientemente rígidos para prevenir que el lumen 262 pueda plegarse al crear un vacío en el mismo.
Durante la utilización, el balón de dilatación 14 del catéter 10 se hace avanzar a través de una lesión de una forma convencional. El balón 14 se dilata forzando el fluido dentro del balón 14 a través del lumen de inflado 30 con la bomba 18. El refrigerante es entonces liberado dentro de la cámara 42 desde un contenedor presurizado o bomba (no mostrada) para enfriar una lesión adyacente a una velocidad apropiada para los objetivos de tratamiento descritos con 25 más detalles más adelante. El refrigerante es descargado desde la cámara 42 a través del lumen 46 de salida o drenaje. La flecha en la figura 1 cerca del colector 16 muestra el refrigerante que está siendo descargado en la atmósfera desde el lumen 46. El refrigerante puede recogerse para el reciclado o desecho según se desee.
En una realización preferida de la invención, el fluido de inflado es un liquido de bajo punto de congelación, tal como una mezcla de etanol. El refrigerante es del tipo que proporcione las características de transferencia térmicas 30 compatibles con los objetivos de tratamiento. El líquido N2 puede ser utilizado como un refrigerante de propósito general con el catéter 10 y es particularmente útil en la congelación de las células dentro de la lesión si así se desea. Cuando se usa el liquido N2 en el catéter 10, puede ser transportado a la cámara 42 en la fase liquida en donde se vaporiza en el orificio 48 y sale a través del lumen 46 como un gas. El gas freón, el gas N2O y CO2 pueden también ser utilizados como refrigerantes. Podrían utilizarse también otros refrigerantes tales como la solución salina fría que podría 35 entrar y salir de la cámara 42 como un líquido, Fluisol o una mezcla de solución salina y etanol. Se anticipa que los refrigerantes tales como la solución salina podrían utilizarse con el catéter 10 cuando la congelación rápida de las células dentro de una lesión no sea el objeto del tratamiento. El técnico especializado en la técnica podría apreciar que podrían utilizarse otros refrigerantes de una forma similar, para conseguir uno o más objetivos del tratamiento.
El catéter 100 puede utilizarse de una forma similar al catéter 110, excepto en que el refrigerante tiene que 40 servir también como el fluido de inflado. Se contempla que en la mayoría de aplicaciones que incluyan el catéter 100, se utilizará un refrigerante líquido como la solución salina.
La temperatura puede monitorizarse por los sensores 22 y 24 termo-resistivos, bien en forma absoluta con sensores pre-calibrados y/o en forma relativa entre los sensores 22 y 24. Dependiendo de los objetivos del tratamiento y del nivel de la temperatura monitorizada, el régimen de flujo del refrigerante en el catéter puede ajustarse para elevar o 45 bajar la temperatura de la lesión.
El objetivo del tratamiento de crioplastia es prevenir o retardar la reoclusión de una lesión dilatada mediante la prevención o el retardo de la reestenosis y/o retroceso elástico de la lesión. El enfriamiento de la lesión a un nivel cerca de 00C se cree que cambia las características de las lesiones de una forma tal que se mejore el remodelado por la dilatación a baja presión, para prevenir o retardar el retroceso elástico de la lesión. El enfriamiento adicional para 50 congelar la lesión se cree que puede crear apoptosis del tejido de la lesión, es decir, la muerte de las células dentro de la lesión.
En una realización preferida del método que puede realizarse con el catéter de crioplastia de la presente invención, el catéter de crioplastia tal como el descrito anteriormente se utiliza para dilatar la lesión y el enfriamiento lento (por ejemplo, en un periodo de 1.5 minutos) para reducir daños en el tejido sano mientras que se altera la placa 55 para que sea más susceptible al remodelado permanente mecánico. La temperatura de la lesión durante la dilatación se prefiere que sea de aproximadamente 0oC a 20oC, de forma tal que se alteren significativamente las características de la placa, aunque se haya preservado el daño en el tejido sano.
Alternativa o adicionalmente al enfriamiento lento hasta aproximadamente 0oC a 20oC para el remodelado mejorado, la lesión puede congelarse en forma instantánea de 0oC a -40oC durante entre 20 a 30 segundos, mientras que se esté todavía inflando el balón. Las inyecciones salinas o de contraste pueden utilizarse previamente o durante la congelación, para prevenir la congelación adyacente de la sangre, para evitar la creación de trombos oclusivos.
Los mecanismos de la estenosis y el retroceso elástico no están totalmente comprendidos. Se cree que en el 5 caso de la reestenosis, la congelación del tejido del trombo daña los capilares que suministran el tejido de la lesión y promocionan la proliferación de las células musculares. Con respecto al retroceso elástico, se cree que el enfriamiento de la placa la hace que sea relativamente rígida y cristalizada, haciendo que sea más susceptible de un remodelado permanente.

Claims (8)
- REIVINDICACIONES1. Un catéter (10) de crioplastia, que comprende:un eje (12) que tiene extremos proximal y distal, en donde el eje define un lumen (30) de inflado, un lumen (40) de entrada de refrigerante y un lumen de salida (44) a su través, teniendo cada lumen un extremo proximal y distal próximos a los extremos proximal y distal del eje respectivamente; 5un balón (14) de dilatación dispuesto en el extremo distal del eje, y en comunicación fluida con el lumen de inflado; yuna cámara (42) dispuesta dentro del balón y en comunicación fluida con los lúmenes de entrada y salida, en donde un orificio (48) en el extremo distal del lumen de entrada del refrigerante tiene un diámetro menor que el de un orificio en el extremo (50) del lumen de salida. 10
- 2. El catéter de crioplastia de acuerdo con la reivindicación 1, que además comprende una fuente de refrigerante (20) conectada al extremo proximal del eje en comunicación fluida con el lumen de entrada, en donde el refrigerante es cualquier liquido a partir del N2, Freón, NO2 y CO2.
- 3. El catéter de crioplastia de acuerdo con las reivindicación 2, en donde la fuente de refrigerante contiene un líquido N2. 15
- 4. El catéter de crioplastia de acuerdo con la reivindicación 2, en donde la fuente de refrigerante contiene un líquido super-enfriado.
- 5. El catéter de crioplastia de acuerdo con la reivindicación 1, que comprende además un sensor termo-resistivo dispuesto en el balón de dilatación.
- 6. El catéter de crioplastia de acuerdo con la reivindicación 5, que comprende además un segundo sensor 20 termo-resistivo dispuesto sobre el eje.
- 7. El catéter de crioplastia de acuerdo con la reivindicación 1, en donde el eje define además un lumen del alambre de guía.
- 8. El catéter de crioplastia de acuerdo con la reivindicación 1, en donde el eje está al menos rodeado en parte por un vaina de aislamiento en donde dicha parte define un lumen de vacío. 25
