Abstract
Uso de un antagonista de LHRH para la fabricación de un medicamento para el tratamiento de la infecundidad por inseminación intrauterina, consistiendo la mejora en: a) la supresión, dependiente de la dosis, de gonadotropinas endógenas, especialmente LH, con un antagonista de LHRH que permite el mantenimiento de niveles fisiológicos de estrógenos b) estimulación exógena del crecimiento ovárico folicular c) inducción de ovulación con HCG, LHRH nativa, agonistas de LHRH o LH recombinante d) para inseminación intrauterina por inyección de esperma.
Description
Método para el tratamiento de trastornos de
fertilidad.
Uno de los problemas éticos de los tiempos más
modernos es la esterilidad creciente y la indeseada carencia de
hijos de muchas parejas. Con respecto a la terapia de estos
desórdenes de la fertilidad se han establecido, entre otros, los
siguientes métodos de tratamiento de fertilización artificial:
- 1.
- Terapia de sustitución - aplicada en pacientes con amenorrea hipogonadotrópica
- 2.
- Terapia de estimulación - dada a pacientes anovulatorios con eje hipotalámico pituitario ovárico activo, aunque desarreglado.
- 3.
- Terapia de regulación - usada en mujeres con POCD
- 4.
- Terapia de hiperestimulación - usada en IVF (fertilización in-vitro), transferencia intrafalopiana de gametos (GIFT), transferencia embrionario tubal (TET), inyección intracitoplasmática de esperma (ICSI) e inseminación intrauterina (IUI).
La presente invención se refiere especialmente a
la mejora de transferencia artificial celular de esperma en el
útero, es decir, a la fertilización por inseminación intrauterina
(IUI) mencionada en el punto 4.
Para los métodos de los puntos 2 y 4 es necesario
estimular el crecimiento folicular, que se logra por la
administración de gonadotropinas, por ejemplo HMG, FSH y LH, con o
sin terapia preliminar con clomifeno.
Además se ha demostrado que el riesgo de
luteinización por una oleada prematura de LH, que conduce a unas
condiciones de implantación desfavorables y proporciones de
fecundidad relativamente bajas, puede disminuir por supresión
completa de las gonadotropinas endógenas usando agonistas de GnRH
(García et al., 1984; Navot et al., 1991; Hoffmann et al.,
1993).
Para el control de la estimulación ovárica con la
subsiguiente inducción de ovulación, con el fin de obtener células
reproductoras fértiles, se usan tanto FSH y HMG recombinantes como
FSH y HMG obtenidas de la orina.
Con relación a la IUI, se desea también controlar
el crecimiento folicular y desencadenar específicamente la
ovulación.
Los informes de la literatura especializada sobre
el acompañamiento terapéutico de IUI, en particular usando análogos
GnRH, son principalmente negativos, tales como por ejemplo los
siguientes:
- 1.
- La IUI tras la estimulación ovárica con clomifeno puede ser importante como la primera elección de terapia, a condición de que el cónyuge masculino tenga un espermograma normal (Hum. Reprod. 1997; Julio; 12(7):1458-1463).
- 2.
- Sin embargo, la estimulación por agonistas de GnRH/HMG puede ser ineficaz en IUI de rutina. El tratamiento con agonistas de GnRH con supresión máxima de las gonadotropinas endógenas requiere un periodo de tratamiento relativamente largo (alrededor de 3 semanas) y conduce a un consumo mayor de HMG y está asociado con efectos secundarios.
- 3.
- También existen informes que confirman que un aumento en la proporción de fecundación no se consigue por el uso de agonistas de GnRH/HMG frente a HMG sola para el tratamiento de IUI en el caso de infertilidad no clarificada.
- 4.
- Las diferencias de coste de la estimulación por GnRH-a/HMG comparada con clomifeno/HMG se indica por autores finlandeses en Eur. J. Obstet. Gynecol. Reprod. Biol. 1997 Julio 74: la estimulación por GnRH-a/HMG no es rentable en terapia de IUI de rutina.
En un estudio por Diedrich y colaboradores, de
1994, Hum. Reprod. Mayo 1994; 9(5), la supresión de la
no deseada oleada prematura de LH por cetrorelix durante la
estimulación ovárica con HMG y la inducción puntual de la ovulación
se describió en el contexto de un estudio COS-ART.
Fue posible disminuir la longitud del periodo de tratamiento usando
este antagonista de la LHRH, y la supresión parcial, dependiente de
la dosis, de las gonadotropinas endógenas demostró ser además
conveniente, porque fue posible reducir el consumo en comparación
con el uso de agonistas de HMG.
Por tanto, el objeto de la invención es mejorar,
es decir, hacer económico y más eficaz, la inseminación intrauterina
conocida per se y así al final satisfacer el deseo de niños por
muchas parejas.
Se ha encontrado ahora que la IUI se puede
mejorar realizando una supresión parcial de las gonadotropinas
endógenas, lo que se puede realizar solamente por medio de
antagonistas de la LHRH, preferentemente cetrorelix o antarelix. Al
mismo tiempo, el crecimiento folicular se estimula por medio de FSH,
HMG urinarias o recombinantes, o clomifeno, o una combinación de
ellos. Ulteriormente, la ovulación se puede desencadenar en un
tiempo deseado por medio de HCG, LHRH nativa, agonistas de LHRH o LH
recombinante. Sorprendentemente, esto tiene lugar cuando el folículo
dominante ha alcanzado un diámetro de alrededor de
16-18 mm. La inyección intrauterina de esperma tiene
lugar después con la finalidad de la fecundación intracorpórea. De
esta manera es posible realizar un tratamiento de estimulación que
es menos productor de tensión nerviosa en el paciente y garantiza un
alto grado de seguridad con respecto al tiempo de ovulación y
conduce a un ahorro en el coste.
Claims (6)
1. Uso de un antagonista de LHRH para la
fabricación de un medicamento para el tratamiento de la infecundidad
por inseminación intrauterina, consistiendo la mejora en
- a)
- la supresión, dependiente de la dosis, de gonadotropinas endógenas, especialmente LH, con un antagonista de LHRH que permite el mantenimiento de niveles fisiológicos de estrógenos
- b)
- estimulación exógena del crecimiento ovárico folicular
- c)
- inducción de ovulación con HCG, LHRH nativa, agonistas de LHRH o LH recombinante
- d)
- para inseminación intrauterina por inyección de esperma.
2. Uso de un antagonista de LHRH para la
fabricación de un medicamento para el tratamiento de la infecundidad
por inseminación intrauterina de acuerdo con la reivindicación 1 en
el que el antagonista de LHRH es cetrorelix.
3. Uso de un antagonista de LHRH para la
fabricación de un medicamento para el tratamiento de la infecundidad
por inseminación intrauterina de acuerdo con la reivindicación 1 en
el que el antagonista de LHRH es antarelix.
4. Uso de un antagonista de LHRH para la
fabricación de un medicamento para el tratamiento de la infecundidad
por inseminación intrauterina de acuerdo con la reivindicación 1 en
el que la estimulación se realiza por administración de FSH o HMG
urinarias o recombinantes, con o sin LH recombinante.
5. Uso de un antagonista de LHRH para la
fabricación de un medicamento para el tratamiento de la infecundidad
por inseminación intrauterina de acuerdo con la reivindicación 1 en
el que la estimulación ovárica se consigue con antiestrógenos como
por ejemplo clomifeno.
6. Uso de un antagonista de LHRH para la
fabricación de un medicamento para el tratamiento de la infecundidad
por inseminación intrauterina de acuerdo con la reivindicación 1 en
el que la estimulación ovárica se consigue con la combinación de
antiestrógenos como por ejemplo clomifeno con gonadotropinas.
