Den foreliggende opfindelse angår anordninger til transdermal administration af fentanyl med henblik på lindring af smerte efter 5 operation og ved kronisk cancer. Opfindelsen angår nærmere bestemt en lamineret komposit til transdermal administration af fentanyl i ca. én dag, på hvilket tidspunkt en væsentlig mængde af fentanylet i anordningen er blevet udtømt derfra.
10 Baggrund for opfindelsen
Fentanyl, der et syntetisk opiat med en styrke på 50-100 gange morphins styrke, anvendes klinisk til lindring af smerte hos patienter efter operation og patienter med cancer i slutstadiet. Farmako-15 dynamiske undersøgelser efter intramuskulær administration af fentanyl har indiceret, at den største analgetiske virkning almindeligvis forekommer én time efter intravenøs administration og opretholdes i kortere tid end morphin. De gennemsnitligt effektive analgetiske koncentrationer af fentanyl i plasma er ca. 1 henholds-20 vis 3 ng/ml ved post-operativ og intraoperativ administration, selv om der er betydelig variation fra individ til individ. En koncentration på op til 10 ng fentanyl/ml plasma giver tilsvarende analgetiske virkninger ved smerter hos cancerpatienter i slutstadiet.
Udviklingen af tolerance og fysisk afhængighed ved gentagen anven-25 del se af fentanyl svarer til den for andre opi oide medikamenter. En lavere initiel dosis kan imidlertid mindske fentanyls toleranceniveau.
Faldet i fentanylkoncentrationen i plasma som funktion af tiden 30 efter intravenøs administration synes at være trieksponentiel i den initielle fordelingsfase fulgt af to eliminationsfaser. Fordelingsvolumenet ligger på fra 4,4 til 59,7 liter, halveringstiden for den terminale eliminering ligger på fra 141 til 853 minutter, medens i værdier for fuldstændig eliminering fra legemet ligger på fra 160 35 til 1530 ml/minut. Plasmaproteinbindingen hos mennesker er beskrevet at være 85%. Fentanyl elimineres primært ad metabolisk vej hos sunde frivillige forsøgspersoner og operative patienter, medens renal udskillelse af fentanyl kun står for ca. 6% af dosen hos forsøgspersoner.

Beskrivelse af opfindelsen 5 Opfindelsen angår en lamineret komposit med fast tilstandsform til transdermal administration af fentanyl, hvilken komposit omfatter: (a) et baglag, der i alt væsentligt er uigennemtrængeligt for fentanyl og afgrænser kompositens overflade, 10 (b) et klæbende lægemiddel reservoirlag, som afgrænser kompositens basisoverflade under anvendelse, og som omfatter: (i) 1-5 vægtprocent fentanyl; 15 (ii) 1-10 vægtprocent propylenglycolmonolaurat (PGML); (iii) 85-98 vægtprocent af en aminmodstandsdygtig, trykfølsom, klæbende polymer med en diffusivitet -8 (gennemtrængeligehed) for fentanyl på fra 10 til -11 2 10 cm /sekund og en fentanyl opløselighed på fra 20 1,5 til 5 mg/ml, hvilken komposit udviser en stationær fentanylflux gennem hud på fra 2 ca. 2 til ca. 10 mcg/cm /time og afgiver mindst ca. 75% af fentany-let i kompositen i løbet af ca. den første anvendelsesdag.

Laget tjener også som reservoir for permeationsforstærknings-/solubiliseringsmidlet, PGML. Kommercielt PGML indeholder væsentlige mængder, dvs. op til 40 vægtprocent, af dilauratet (PGDL) og kan også indeholde mindre mængder (f.eks. op 10 til 10-15 vægtprocent) af andre bestanddele, såsom methyl 1 aurat eller propylenglycol. Som det anvendes heri, betegner udtrykket "PGML" således kommercielt PGML såvel som renere former af materi- 2 alet. PGML-Mængden i laget vil almindeligvis ligge på 0,1-10 mg/cm , 2 fortrinsvis 0,15-0,5 mg/cm , og vil normalt udgøre 1-10 vægtprocent, 15 fortrinsvis fra 1,5 til 5 vægtprocent, af laget. Matricen vil normalt udgøre 85-98 vægtprocent, fortrinsvis 90-96 vægtprocent, af reservoirlaget. Kompositens basisoverfladeareal (afgrænset af lag 2 12) vil almindeligvis have en størrelse på fra 5 til 50 cm , for- 2 trinsvis fra 10 til 40 cm .

Laget 13 tjener som en frigørligt dæklag, som afrives fra kompositen umiddelbart før anvendelse til blotlæggelse af laget 12. Det er fremstillet af en polymer, der i alt væsentligt er uigennemtrængelig for fentanyl og PGML og i sig selv er afrive!ig eller kan gøres 30 afrivelig ved metoder, såsom silicone- eller carbonfluoridbehand-1 i ng.
Formålet med laget 16 i kompositen på figur 2 er at tilvejebringe et område for aflejring af fentanyl ved én fremgangsmåde til fremstil-35 ling af kompositen og/eller, når bagbeklædningslaget er tyndt og fleksibelt, at bibringe kompositen fasthed for at hindre bagbeklædningslaget i at krølle. Et porøst lag bestående af en 25 øm tyk Novonette-folie (et nylon-spindebundet uvævet stof fra Monsanto Corp.) blev lamineret til den anden side af folien af den lægemiddel holdige, trykfølsomme klæbemiddel sammensæt-10 ning til dannelse af en anden undersamling (L2).
Novonette-Folieoverfladen af L2 blev lamineret til den klæbende side af LI til tilvejebringelse af den endelige laminerede femlagskompo-sit med carbonfluoridbelagt polyesterfolie, der tjener som en 15 afrivelig frigørbar strimmel til det endelige laminat. Det laminerede system fik lov at ækvilibrere i 1 uge før vurdering af hudtransmissionen.
Forsøg med hudtransmission blev udført under anvendelse af den 20 generelle procedure, der er beskrevet i J. Pharm. Sci. (1983) 72:968.
Laminatet blev udskåret til kvadratmeter, der passede til diffu- sionsceller, og den stationære fentanylstrøm gennem kadaverhud blev 2 25 ved 32eC bestemt til at være 6,5 mcg/cm /time. >Perfekt hudoptagelsesforhold blev opretholdt ved anvendelse af phosphatpuffer (pH = 6,0) som modtagervæske. Den kumulative mængde fentanylbase frigjort ved in vitro-opløsning var ved 25eC afhængig af kvadratroden af 2 tiden (korrelationskoefficient = 0,99, hældning 139,5 mcg/cm /-30 time^), hvilket indicerer, at fentanylbasediffusi on var hudkontrolleret. Ved en hudpermeationsundersøgelse over 24 timer blev der afgivet 85% af den totale mængde fentanylbase, og der var kun 15% medikament (rest) tilbage i kompositen.
