Azelastin er et phthalazinonderivat med følgende strukturformel:

Den kemiske betegnelse er: 4-(4-chlorbenzyl)-2-(perhydro-1-methyl-azepin-4-yl)-1- (2H)phthalazinon.
Azelastin benyttes især til profylakse for astma og har derudover antiallergiske og antihistaminagtige egenska-15 ber, se tysk patent nr. 21 64 058.
Man har nu fundet, at azelastin og fysiologisk acceptable salte deraf udviser særligt fordelagtige og -------------- 0y-etfaskende virIcrTinger ] når det i dertil egnede for- mulinger indgives i næsen og/eller til øjets bindehin-20 de.
Man opnår herved en helbredelse, henholdsvis en tydelig lindring, ikke blot ved allergisk betinget rhinitis, men også ved ganske almindelig snue (f.eks. forårsaget af rhinovira) samt af vasomotorisk snue og de 25 deraf fremkaldte sygdomssymptomer.
Det er herved overraskende, at man ved lokal anvendelse i næsen også opnår en gunstig virkning på k øjets slimhinder. (Helbredelse, henholdsvis lindring af røde øjne og kløe i øjet), så man herved ofte undgår en 30 yderligere anvendelse af øjendråber.
Yderligere eksempler på indikationer for indgi-velse/anvendelse ifølge opfindelsen er: uspecifik kon-junktivitis, allergisk forårsaget konjunktivitis, allergisk ødem i øjenlågene, katarrhagtige tilstand i øj-35 nene eller i næsen, coryza.
Ved anvendelsen ifølge opfindelsen iagttager man derudover overraskende ikke den træthed, der optræder ved andre indgivelsesmåder.

Azelastin har derudover en overordentlig gennemtrængende bitter smag, der indtil nu har forhindret enhver form for oral indgivelse af azelastinopløsninger, da sådanne azelastinoplesninger eller suspensioner af-5 vises af patienterne.
Den bitre smag er så intensiv, at man endnu ved en fortynding på 1:106 vil mærke den som ubehagelig.
Det har overraskende vist sig ved forsøg med frivillige, at man ved indsprøjtning af azelastinpræparater 10 ifølge opfindelsen i næsen ikke mere mærker denne bitre smag, hvorfor det er muligt på denne måde at indgive opløsninger eller suspensioner af azelastin og salte deraf uden smagspåvirkning. Man kan heller ikke, når den indsprøjtede azelastinopløsning eller suspension 15 løber bagud i svælget, i nævneværdig grad mærke denne bitre smag.
Det er altså en opgave for opfindelsen at stille et velforligeligt og forbedret middel baseret på azelastin eller salte deraf til rådighed til behandling 20 både af allergisk betingede og vasomotorisk eller af rhinovirus forårsaget snue, og ledsagesymptomer hertil.
En foretrukken udførelsesform for opfindelsen er en steril og holdbar vandig opløsning af azelastin eller salte deraf, der kan udnyttes i dråber, salver, 25 cremer, geler, pulver til indblæsning eller i en særlig foretrukken udførelsesform som et spray (fortrinsvis næsespray), idet man herved til indgivelsen kan benytte en sædvanlig sprayflaske eller en pumpeforstøver. Der- , φ udover er det også muligt at anvende trykgasaerosoler.

Som opløsningsmiddel for præparater ifølge opfindelsen kommer fortrinsvis i betragtning: vand, mættede alifatiske mono- og divalente alkoholer med 2-3 10 C-atomer (eksempelvis ethanol, isopropanol, 1,2-propy-lenglycol, glycerol) og væskeformige polyglycoler (molekylvægt 200-600).
Fortrinsvis kommer vand i betragtning som opløsningsmiddel eller blandinger af vand og andre fysio-15 logisk acceptable opløsningsmidler (f.eks. de ovenfor nævnte), idet mængden af disse i den vandige blanding ikke skal udgøre over 15 vægt%.

Som sådanne kan man f.eks. nævne: ethylendiamin-tetraeddikesyre (EDTinsyre) og alkalisalte deraf (f.eks. dialkalisalte såsom dinatriumsaltet, calciumsaltet, calciumnatriumsaltet), lavere alkylestere af p-hydroxybenzoesyre, chlorhexidin (f.eks. som acetat 25 eller gluconat), phenylkviksølvborat. Ydermere kan man f.eks. nævne natrium-(2-ethylmercurithio)benzoat, alment kendt som "thiomersal", der kan være til stede i præparaterne ifølge opfindelsen i en mængde på 0,001-0,05, fortrinsvis 0,005-0,02, eksempelvis 0,1% (vægt/ 30 volumen ved væskeformige præparater ellers vægt/vægt).
Andre egnede konserveringsmidler er: farmaceutisk acceptable kvaternære ammoniumforbindelser, f.eks. cetyl-pyridiniumchlorid, tetradecyltrimethylammoniumbromid, almindelig kendt som "cetrimid", benzyldimethyl-[2-[2-35 [p-(1,1,3,3-tetramethylbutyl)]phenoxy]ethoxy]ammonium-chlorid, almindelig kendt som "benzethoniumchlorid" og 4 DK 174928 B1
myristyl-y-pikoliniumchlorid, idet disse forbindelser kan anvendes i koncentration på 0,002-0,05, f.eks. 0,2% I
(vægt/volumen ved væskeformige præparater, ellers vægt/ vægt). De foretrukne konserveringsmidler blandt de kva-5 ternære ammoniumforbindelser er dog alkylbenzyldime-thylammoniumchlorider og blandinger af disse, f.eks. de som "benzalkoniumchlorid" alment kendte forbindel-ser. Disse består af en blanding af forbindelser med formlen 10 Γ + r* (_)-CHi-«-* 15 CH, w hvori R betegner en alkylgruppe med formlen CnH2n+]_, n er lig et heltal 8-18. Særlig foretrukket er anven-20 delse af en blanding af forbindelser, hvori n er 10-14, og især den specielle forbindelse, hvori R = Ci2H25* "Benzalkoniumchlorid" og forbindelser med den ovennævnte formel kan anvendes i koncentrationer på 0,005-0,10, fortrinsvis 0,005-0,05, f.eks. 0,01% (vægt/rumfang ved 25 væskeformige præparater ellers vægt/vægt), og de kan eventuelt anvendes kombineret med 0,2-2,0, f.eks. 0,4% (vægt/volumen) 2-phenylethanol.
Præparaterne ifølge opfindelsen (opløsninger, A
suspensioner, også olieopløsninger, henholdsvis suspen-30 sioner, salver, emulsioner, cremer, geleer, doseringsaerosoler) indeholder 0,0005-2, fortrinsvis 0,001-1, især 0,003-05% (vægt/vægt) azelastin (udregnet som den frie azelastinbase). Hvis azelastin foreligger som et salt, skal man omregne mængden af dette under hensyn 35 hertil. Man benytter samme azelastinkoncentrationer for øjendråber som for præparater til indgivelse i næsen.
5 DK 174928 B1 Når det drejer sig om pulvere, vil koncentrationen af azelastinbase være 0,0005-2 vægt% i forhold til de fase bærestoffer.
Ved opløsninger er dosis pr. næsebor f.eks. λ 5 0,01-0,2 ml, især 0,05-0,15 ml, idet en sådan dosering f.eks. skal gives fra én til flere gange, fortrinsvis fra 1 til 5 gange om dagen (eventuelt også hver time).
ved anvendelsen i øjet (øjendråber) er doseringen f.eks. en lille dråbe (ca. 0,05 ml) af opløsningen 10 eller en tilsvarende mængde af halvfaste præparater.
Som syrer til dannelse af salte med azelastin kan man f.eks. nævne: halogenbrintesyrer (HCl, HBr), svovlsyre, phosphorsyrer (H3P04, metaphosphorsyre, po-lyphosphorsyrer), salpetersyre, organiske mono-, di-15 eller tricarboxylsyrer af alifatiske, alicycliske, aromatiske eller heterocycliske organiske syrer (embonsy-re, citronsyre, vinsyre), alifatiske og aromatiske sul-' fonsyrer ~( f .exs^ kamiersulfonsyre).-------------------------
Den totale mængde af konserveringsmidler i præ-20 paraterne (opløsninger, salver, osv.) er pr. 100 ml opløsning/suspension, henholdsvis 100 g præparat mellem 0,001-0,10, fortrinsvis 0,01 g.
ved konserveringsmidlerne kan man f.eks. benytte følgende mængder af de enkelte stoffer: 25 thiomersal 0,002-0,02%: benzalkononiumchlorid 0,002-0,02% (ved kombination med thiomersal kan mængden af thiomersal f.eks. være 0,002-Λ 0,005%); chlorhexidinacetat, henholdsvis -gluconat 0,01-0,02%; 30 phenylkviksølvnitrat, -borat, -acetat 0,002-0,004%; p-hydroxybenzoesyreestere (f.eks. en 7:3 blanding af methylesteren og propylesteren): 0,05-0,15, fortrinsvis 0,1%.
Fortrinsvis anvender man som konserveringsmiddel 35 en blanding af EDTinsyre (f.eks. som dinatriumsalt) og benzalkoniumchlorid, idet EDTinsyre anvendes i en kon- 6 DK 174928 B1 centration på 0,05-0,1% og ben2alkoniumchlorid i en koncentration på 0,005-0,05%, fortrinsvis i en koncentration på 0,1%. Ved opløsninger/suspensioner er det altid vægt%/volumen, ved faste, henholdsvis halvfaste 5 præparater er det vægt%/vægt af præparatet.

Som yderligere hjælpestoffer til præparaterne ifølge opfindelsen kan man eksempelvis nævne: polyvi-nylpyrrolidon, sorbitanfedtsyreestere såsom sorbitan-trioleat, polyethoxylerede sorbitanfedtsyreestere 10 (f.eks. polyethoxyleret sorbitantrioleat), sorbimacro-gololeat, syntetiske amfotensider (tritoner), ethylen-oxidethere af octylphenolformaldehydkondensationspro-dukter, phosphatider såsom lechitin, polyethoxylerede fedtstoffer, polyethoxylerede oligotriglycerider, poly-15 ethoxylerede fedtalkoholer. I denne forbindelse betegner polyethoxyleret, at de pågældende stoffer indeholder polyoxyethylenkæder, hvis polymerisationsgrad alment er 2-40, især 10-20. Disse stoffer skal fortrinsvis gøre azelastinkomponenten mere opløselig.
20 ved præparater, der indeholder vand, kan man eventuelt yderligere tilsætte midler til isotonisering.
Hertil kommer f.eks. i betragtning: saccharose, glucose, glycerol, sorbitol, 1,2-propylenglycol og NaCl.
Disse midler til isotonisering skal give præpa-25 raterne det samme osmotiske tryk som sekret i næsen.
Man skal til dette formål hver gang anvende så meget af disse stoffer, at man f.eks. ved en opløsning opnår en frysepunktnedsætning på 0,50-0,56eC i forhold til rent a vand. ved eksempel 1 skulle man eksempelvis anvende en 30 sådan mængde af sådanne stoffer, der ville være iso-osmotisk med 68 g natriumchlorid (0,68%).
I eksempel 1 kan man i stedet for NaCl f.eks. pr. 100 ml opløsning anvende følgende:
Glucose, 1H20 3,81 g; saccharose 6,35 g; glyce-35 rol 2,2 g; 1,2-propylenglycol 1,617 g; sorbitol 3,84 g (hvis man anvender blandinger af disse stoffer, må mængderne nedsættes tilsvarende).

Opløsningerne kan derudover indeholde fortykken-de midler, der forhindrer, at de for hurtigt flyder ud af næsen og som giver opløsningen en viskositet på ca.
1,5-3, fortrinsvis 2 mPa.s. Eksempler på sådanne for-5 tykkende midler: cellulosederivater (f.eks. cellulose-ethere), hvor cellulosehydroxygrupperne delvis har undergået etherdannelse med lavere umættede alifatiske alkoholer og/eller lavere umættede alifatiske oxyalko-holer (f.eks. methylcellulose, carboxymethylcellulose, 10 hydroxypropylmethylcellulose), gelatine, polyvinylpyr-rolidon, tragant, ethoxose (et vandopløseligt binde- og fortykkende middel baseret på ethylcellulose), algin-syre, polyvinylalkohol, polyacrylsyre, pektin og dertil svarende midler. Hvis disse stoffer indeholder sure 15 grupper, kan man også anvende tilsvarende fysiologisk acceptable salte deraf.
Hvis man anvender hydroxypropylcellulose, benyt-----t-e-r-man—eksempelvis—ø^-i—væg-t%—t-i-1— det-be—brug-;-----------
Præparaterne kan derudover indeholder puffere 20 såsom citronsyre/natriumhydrogenphosphat boratpuffer, phosphater (natriumdihydrogenorthophosphat, dinatrium hydrogenphosphat), tromethamol, henholdsvis tilsvarende sædvandlige puffere til indstilling af en pH-værdi for præparatet på f.eks. 6-7,5, fortrinsvis 6,5-7,1.
25 Den anvendte mængde citronsyre er f.eks. 0,01- 0,14, fortrinsvis 0,4-0,05 g, mængden af dinatriumhy-drogenphosphat er 0,1-0,5, fortrinsvis 0,2-0,3 g pr.
Λ 100 ml opløsning. De angivne vægtmængder er hver gang baseret på det vandfrie stof.
30 ved opløsninger og suspensioner skal den højeste værdi for totalkoncentrationen af lægemiddel og puffer være mindre end 5%, især mindre end 2% (vægt/volumen).
Fortrinsvis anvender man i næsen en opløsning eller en suspension, der indgives som aerosol, dvs.
35 på form af fint fordelte partikler i luft eller i en anden sædvanlig bæregas, f.eks. ved hjælp af en sædvanlig pumpeforstøver.

Man kan også indgive præparatet som doseringsaerosol. Herved skal man forstå trykpakninger, der indeholder azelastin, henholdsvis salte deraf, på form af I
en opløsning eller suspension i et såkaldt drivmiddel.
5 Som drivmiddel benytter man chlorerede fluorerede Λ carbonhydrider eller blandinger af forskellige chlorerede fluorerede carbonhydrider samt propan, butan, iso-butan eller blandinger af disse med hinanden eller med chlorerede fluorerede carbonhydrider, idet disse er væ-10 skeformige under tryk, men ved normal tryk og stuetemperatur er gasformige. Trykpakningen har en doseringsventil, der ved betjening frigiver en veldefineret mængde af lægemiddelopløsningen, henholdsvis lægemiddelsuspensionen. Ved den derved opnåede pludselige for-15 dampning af drivmiddel vil opløsningen, henholdsvis suspensionen, af azelastin blive spredt i meget fine dråber eller små partikler, der bliver sprøjtet ind i næsen eller kan indåndes i næsen. Man benytter til betjening af ventilen og til indførelse af den udsprøjte-20 de suspension i næsen bestemte applikatorer af kunststof. Et yderligere drivmiddel kan være C02, dinitrogen-oxid eller trykluft.
Ved aerosoler kan man endvidere anvende en sædvanlig adaptor.
25 Hvis man anvender suspensioner, skal den højeste partikelstørrelse for de faste stoffer (azelastin + hjælpestoffer) ikke være mere end 30 pm.
Ved anvendelse som et indblæsningspulver skal /, den maksimale partikelstørrelse af stofferne ikke være 30 mere end 20 pm.
Herved benytter man en forstøvning af fast azelastin eller salte deraf. I dette tilfælde blander man eksempelvis azelastin eller et salt deraf med inerte bærestoffer eller overtrækker det på inerte bærestof-35 fer. Eksempler på bærestoffer hertil er: sukkerarter såsom glucose, saccharose, lactose, fructose. Derudover DK 174928 B1 9 stivelse eller stivelsesderivater, oligosaccharider såsom dextriner, cyclodextriner og derivater deraf, poly-vinylpyrrolidon, alginsyre, tylose, kiselsyre, cellulose, cellulosederivater (f.eks. celluloseethere), 5 sukkeralkoholer såsom mannitol eller sorbitol, calcium-carbonat og calciumphosphat. Koncentrationen af azela-stin er 1 vægtdel azelastin på 50-200 000 vægtdele bærer (0,0005-2% azelastin).
10 Eksempel 1 Næsespray, næsedråber eller øjendråber med 0,1% azelastin, hydrochlorid som aktivt middel.
15 I 9,00 kg vand opløser i rækkefølge følgende: 10 g azelastin,hydrochlorid, 5 g EDTinsyre, dinatriumsalt, 2 H20, 68 g natriumchlorid, 1,25 g al- -------------------kyi-ben-z-yid-imethy-l-ammeniumehl-oråd—(-ben-za-l-k-on-i-umeh-ler-id-),------------ 4,38 g citronsyre, 64,8 g natriummoriohydrogenphosphat, 20 12 H20 samt 10 g hydroxypropylmethylcellulose1 . Man fylder efter med vand til 10,05 kg = 10 liter og filtrerer efter omhyggelig sammenblanding gennem et membranfilter med porestørrelse 0,2 μπι, idet man bortkaster et forløb på 500 ml. Filtratet har en pH-værdi på 25 6,8 ± 0,3. Blandingen fyldes i kunststofflasker, der er forsynet med et sædvanligt sprøjtelåg eller i kunststof-, henholdsvis glasflasker, der er forsynet med en ' sædvanlig sprøjtepumpe. I sidste tilfælde kan man f.eks. benytte pumper med en næsesprøjteanordning, der 30 ved hver betjening frigiver ca. 0,14 ml opløsning. På denne måde vil for hver betjening 0,14 mg azelastin, hydrochlorid blive sprøjtet på opløst form ind i næsen.
Hvis man fylder det ovennævnte filtrat i små-flasker til næsedråber eller øjendråber, der er forsy-35 net med en dråbepipette, så kan man ved hjælp af denne dråbepipette inddryppe opløsningen i næsen eller i 10 DK 174928 B1 øjet.

5 Eksempel 2 Næsesalve med 0,1% azelastin,hydrochlorid.
I en beholder, der er egnet til ophedning, sam-10 mensmelter man 5 kg polyoxyethylenstearat2, 8 kg cetyl-stearylalkohol (Lanette 0), 20 kg hvid vaseline, 15 kg flydende paraffin og 0,5 kg siliconolie. I denne smelte (temperatur 80eC) opløser man 126 g p-hydroxybenzoe-syremethylester og 53 g p-hydroxybenzoesyrepropylester.
15 Derefter indemulgerer man en opløsning, der opvarmes til 70°C, af 0,1 kg azelastin,hydrochlorid, 140 g p-hydroxybenzoesyremethylester og 60 g p-hydroxybenzoesyrepropylester i 51,021 kg renset vand, ved hjælp af en hurtigkørende omrører, og man fortsætter omrøringen 20 under afkøling af emulsionen, idet man med mellemrum foretager en kraftigere homogenisering.
Disse salte fyldes i tuber, hvor der foran gevindet findes en røragtig forlængelse, ved hvilken man kan indføre salven i næsen.

Doseringsaerosol med frigivelse af 0,5 mg azelastin, hydrochlorid pr. pust.
5 I en egnet kølebeholder afkøler man ca. 8,0 kg af en blanding af 70 vægtdele difluordichlormethan og 30 vægtdele 1,2-dichlortetrafluorethan til ca. -55eC.
I denne blanding opløser man ved -55eC under omrøring 10 en blanding af 0,086 kg forudafkølet sorbitantrioleat og 0,8600 kg forudafkølet trichlorfluormethan. Nu opløser man heri under kraftig omrøring 0,0688 kg mikro-niseret azelastin,hydrochlorid og 0,0688 kg mikronise-ret lactose i portioner, ved yderligere tilsætning af 15 en til ca. -55eC afkølet blanding af 70 vægtdele difluordichlormethan og 30 vægtdele 1,2-dichlortetrafluorethan bringer man totalvægten af denne suspension ______.... ______opstil. 9,-547kg.--------------------—
Efter lukning af kølebeholderen afkøler man igen 20 suspensionen under kraftig omrøring til ca. -55eC, hvorefter den er færdig til påfyldning.
Under stadig omrøring fylder man suspensionen i sædvanlige egnede aluminium-monoblocdåser. Disse mono-blocdåser lukkes umiddelbart efter påfyldning af sus-25 pensionen med hertil sædvanlige doseringsventiler, der for hver betjening af ventilen frigiver 0,05 ml suspension. Ved betjeningen af ventilen frigives der altså •n 0,5 mg azelastin,hydrochlorid . Ved frigivelsen benyttes en sædvanlig anordning, der tillader at den aktive ' 30 substans føres ind i patientens næse.

5 Man indrører 140 g polyvinylalkohol (handels-

navn f.eks.: Mowiol 26-88/Hoechst AG, Frankfurt 80) i 4 liter koldt injektionsvand, man opvarmer suspensionen til 90°C og opretholder denne temperatur i 45 minutter.
Efter afkøling blander man den opnåede opløsning med 10 følgende opløsninger: 5 g azelastin,hydrochlorid i 1 liter injektionsvand, 0,2 g phenylkviksølvnitrat i 2 liter injektionsvand og 70 g natriumchlorid i 1 liter injektionsvand.
Blandingen indstilles på en pH-værdi på 6,8 ved 15 tilsætning af 0,1 N natronlud, blandes med en opløsning af 15 g natriumdihydrogenphosphat,2 H20 og 21 g dina-triumhydrogenphosphat,2 H20 i 1 liter injektionsvand og fyldes op til 10 liter med injektionsvand.
Efter omhyggelig sammenblanding filtreres opløs-20 ningen først gennem et glasfiberfilter og derefter gennem et membranfilter med porestørrelse 0,2 pm, idet man herved kasserer et forløb på 500 ml, og fyldes under aseptiske betingelser i sterile øjendråbeflasker.

1. Lægemiddel med indhold af 0,0005-2% (vægt/ , vægt) azelastin, idet azelastin også kan foreligge som et fysiologisk acceptabelt salt deraf, kendetegnet ved, at det er formuleret til anvendelse i næsen eller i øjet. 5
2. Lægemiddel ifølge krav 1, kendeteg net ved, at det er til anvendelse ved behandling af allergisk betinget, vasomotorisk eller rhinovirafremkaldt snue eller sygdomssymptomer.
3. Lægemiddel ifølge det ene af eller begge de 10 forrige krav, kendetegnet ved, at det indeholder et farmaceutisk anvendeligt konserveringsmiddel i en mængde på 0,001-0,1%, beregnet ved opløsninger som vægt/opløsningsvolumen og ved faste præparater som __________væg_t/præpa.r.a±_vægt__________________________________ 15
4. Lægemiddel ifølge ét eller flere af de forri ge krav, kendetegnet ved, at det er på form af en vandig opløsning.
