Den foreliggende opfindelse angår en fremgangsmåde til dannelse af små, unilamellare vesikler på mindre end 0,2 μπι (2000 Å), som indkapsler et polyen-an-tibiotikum, idet vesiklerne omfatter et phospholipid og 5 cholesterol.
Systemiske fungale infektioner optræder oftest hos individer med kompromitterede immunsystemer. Det, der forårsager sådanne infektioner, er ofte fungi, som normalt findes i det menneskelige legeme eller i omgi-10 velserne, men som gøres ikke-invasive af et kompetent immunsystem. Sådanne fungi henhører under slægterne Candida, Aspergillus, Cryptococcus, Histoplasma og Coccidioides. De patienter, der er mest modtagelige for disse typer infektioner, indbefatter individer med can-15 cer, diabetes, alkoholisme, narkomani, omfattende forbrændinger, organtransplantater, immundefektsygdomme, samt gravide. Når disse patienter er på enten kemotera-peutiske, immunosuppressive og/eller antibakterielle kure, forøges sandsynligheden for at de pådrager sig 20 fungale infektioner yderligere. Behandling af systemiske fungale infektioner er primært begrænset til to medikamentgrupper: polyen-an-25 tibiotikaene, såsom amphotericin B og nystatin,, og imidazolerne, såsom ketaconazol og miconazol. Strukturmæssigt indeholder polyen-antibiotikaene tre til syv konjugerede dobbeltbindinger. Dobbeltbindingerne indgår i en lacton med en stor ring (26 til 44 carbon-30 atomer). På den modsatte side af makrocyclen i forhold til dobbeltbindingerne er ringen substitueret med fra 6 til 12 hydroxylgrupper, hovedsagelig i 1,3-stilling i forhold til hinanden, men også i 1,2- og 1,4-stilling i forhold til hinanden. Amphotericin B og nystatin har 35 begge en tilknyttet aminosukkergruppe og en carboxyl-syregruppe. De modsat rettede virkninger af den lipo- 2 DK 168982 B1 file polyenregion og den lipofobe polyolregion gør polyenerne tungtopløselige i vand.
Den foretrukne behandling af systemiske fungale infektioner består i administrering af amphotericin B 5 (Fungizone® ), da det er det mest effektive systemiske antifungale medikament og er forbundet med det mindste antal tilbagefald. Polyenerne absorberes ikke fra mavetarmkanalen efter oral administrering og må administreres ved intravenøs infusion. Amphotericin B og nysta-10 tin udviser imidlertid begge akut og kronisk toksicitet overfor patientens celler, og de doser, der kan administreres, er således begrænsede, hvilket ofte forhindrer fuldstændig helbredelse. Nystatin har af den grund faktisk kun været til rådighed til inhalationsterapi og 15 oral og topisk anvendelse.
Antifungale polyen-antibiotika binder let til steroler, der er til stede i cellemembraner i dyre- og menneskeceller, f.eks. cholesterol, og forårsager sammenbrud af membranpermeabiliteten og cellelysering.
20 Toksiciteten af amphotericin B hos mammalia-individer har vist sig at være større for visse membransystemer, såsom nyretubulerne. Klinisk anvendelse af amphotericin B har været forbundet med akutte bivirkninger og til sidst nyresvigt ved terapeutiske dosisniveauer.

Undersøgelser hos dyremodeller, og i begrænset 20 omfang hos mennesker, har vist, at amphotericin B inkorporeret i phospholipidvesikler (liposomer) udviser formindsket værts-toksicitet, når det anvendes til behandling af systemiske fungale infektioner. Baseret på at nystatin har den samme virkningsmekanisme som ampho-35 tericin B, må det forventes, at nystatin inkorporeret i phospholipidvesikler også udviser formindsket værts- 3 DK 168982 B1 toksicitet. Frit og liposomalt amphotericin B er omtrent ækvipotent med hensyn til behandling af dissemineret Candidiasis hos mus. (Metha et al., Biochimica et Biophysica Acta, 770, side 230-234 (1984)). Der kan 5 derfor administreres medikamentdoser af liposomalt amphotericin B, der overstiger den maksimalt tolererede dosis for frit amphotericin B, hvilket resulterer i en tydelig forbedring i overlevelsen af inficerede mus.
Id. F.eks. har liposom-indkapslet amphotericin B været 70 anvendt til behandling af murine systemiske fungale infektioner, såsom Candidiasis, (Lopez-Berestein et al., J. Infect. Soc. for Human and Animal Mycol., (1985)), og til behandling af dødssyge humane cancerpatienter, som ikke har reageret på tradionel amphotericin-B-terapi (Lopez-Berestein et 20 al., Abst. for 23rd Intersci. Conf. on Antimicrob. Agents and Chemotherapy (1983); Lopez-Berestein et al., Liposomal Amphotericin B for the Treatment of Systemic Fungal Infections in Patients with Cancer: A. Preliminary Study, J. Inf. Dis., 151, 704-710 (1985)). Under 25 anvendelse af denne type liposomal medikamenttilførsel kan den mængde af antifungal polyen-antibiotikum, der kan administreres uden risiko, og det terapeutiske indeks for medikamentet forøges i væsentlig grad.
Ved de fleste af de ovenfor omtalte undersøgel-30 ser er der blevet anvendt multilamellare vesikler (MLV'er). Størrelsen af disse vesikler (0,2-5 ym) begunstiger makrofagers fagocytose af de medikamentholdi-ge vesikler. Når de indføres i blodstrømmen, fagocyte-res fungi også i begyndelsen af makrofager i organerne 35 i det reticuloendotheliale system (RES), såsom leveren, lungerne og milten. Når fungiene invaderer andre væv, DK 168982 Bl 4 vandrer makrofagerne, der er en vigtig del af immunre-sponsen mod fungale infektioner, til disse steder og bliver knyttet til inficerede steder. Som følge af den multilamellare sammensætning af MLV'erne kan indkaps-5 ling af en stor mængde amphotericin B let opnås. En indkapslingseffektivitet for MLV'er på over 90% er blevet rapporteret. Uheldigvis dannes MLV'er som populationer af vesikler af uensartet størrelse, og dette gør det meget vanskeligt at standardisere præparater fremstillet i forskellige charger. Det kan være uønsket at administrere sådanne vesikler til mammalia-15 individer, da de større partikelstørrelser (ofte adskillige mikron), og deres sandsynlige aggregation og sammensmeltning til dannelse af endog større partikler under oplagring, forøger muligheden for emboli i organer, navnlig i lungerne, efter administrering. Fornylig er amphotericin B blevet indkapslet i 25 små unilamellare vesikler (SUV'er) (mindre end 1 ym) som beskrevet af Tremblay et al., Antimicrob. Agents Chemother., 26, side 170-173 (1984) og suspenderet i saltopløsning. Højere værtsoverlevelsestal og lavere antal levedygtige kolonier af fungi fra nyrene, leve-30' ren og milten sammenlignet med de tilsvarende data for uindkapslet medikament observeredes hos mus med dissemineret Candidiasis, der var behandlet med SUV-indkaps-let amphotericin B. Den akutte 50% letale dosis (LD50), et standardmål for akut murin toksicitet, var 11,8 35 mg/kg sammenlignet med 2,3 mg/kg for uindkapslet medikament. Der opnåedes kun en 70%'s indkapslingseffekti- 5 DK 168982 B1 vitet med disse suv'er.
De anvendte formuleringer ved den af Tremblay et al. foretagede undersøgelse indbefattede heller ikke modificering af vesiklerne til fremkaldelse af for-5 trinsvis optagelse gennem RES, og som sådanne er det sandsynligt, at kun en brøkdel af SUV'erne optages gennem organer, der indeholder makrofager. Ved undersøgelser, hvor der anvendtes ægge-phosphatidylcholin-chole-sterol-vesikelformuleringer af lignende art som de af 10 Tremblay et al., supra, anvendte, blev kun 40 til 60% af det administrerede, til mærkning af vesiklerne anvendte radiomærkningsmiddel knyttet til større organer i RES. I tidligere arbejde er det blevet vist, at ve-sikelformuleringer med aminosukkerderivater på deres 15 overflader fremkalder kemotaxi og efterfølgende optagelse gennem polymorfonukleære leukocytter, når de injiceres subkutant i mus. Når SUV'er med 6-ami-20 nomannosederivatet af cholesterol knyttet til membranen injiceres intravenøst, foreligger trefjerdedel af de radiomærkede vesikler i leveren og milten indenfor tre timer. Id. Senere arbejde af Wu og medarbejdere bekræftede, at inkorporering af en forlænget amin på en mi-25 celles overflade forøger fagocytose ved hjælp af peri-toneale makrofager hos mus. Wu et al., Proc. SUV'er med en aminmodificeret overflade er også blevet anvendt til mærkning af fagocytiske celler in vitro, såsom leuko-30 cytter, til påvisning af infektionssteder. US-Patent-skrift nr. 4.497.791. Før den foreliggende opfindelse har man imidlertid ikke gennemført in-situ-målretning af SUV'er til makrofager i RES til medvirken til behandling af fungale infektioner.
35 Et formål med den foreliggende opfindelse er at tilvejebringe stabile, homogene liposomer (SUV'er) med 6 DK 168982 B1 indkapslede antifungale polyen-antibiotika i kommercielle mængder, hvilke liposomer vil forblive intakte i blodstrømmen, indtil de træder ind i de makrofagholdige 5 organer i RES, såsom leveren og milten. Et andet formål med opfindelsen er at målrette de amphotericin B indkapslende SUV'er mod makrofagerne, som bærer medikamentet direkte til fungale infektionssteder. Denne tilførselsmetode kan forøge det terapeutiske indeks for det 10 SUV-indkapslede medikament i forhold til uindkapslet medikament og nedsætte medikamentets akutte og kroniske toksicitet.
Tremblay et al. (1984) som nævnt ovenfor beskriver fremstillingen af liposompræparater, der indeholder 15 amphotericin B, som små, unilamellare vesikler med et størrelsesområde fra 0,06 til 0,1 ym, ved sonificering ud fra en lipid-sammensætning af phosphitidyl-cholin, cholesterol og tocopherol-succinat i et molforhold på 5:3:1, idet den opnåede suspension fortyndes med 5% 20 vandig glucose før infektion.Vesiklerne fremstilles ikke i en vandig glucoseopløsning med lav ionstyrke.
Marcia et al., PNAS T_7, No. 8, side 4430-4434 (1980); Marcia et al., Science, 207, 309-311 (1980); og EP-A-0 179 444 beskriver medikament-målretning med ve-25 sikler med 6-aminomannose på deres overflade.
Fra EP-A-119 020 kendes et liposompræparat indeholdende amphotericin B i en bærer af saccharid. Heller ikke her fremstilles liposomerne eller vesiklerne i en vandig glucoseopløsning med lav ionstyrke, og ifølge 30 eksemplerne ligger de i størrrelsesområdet fra 2,5 til '5,3 ym.
Ifølge opfindelsen tilvejebringes der, i overensstemmelse med det indledningsvis anførte, en fremgangsmåde til dannelse af små, unilamellare vesikler 35på mindre end 0,2 ym (2000 Å), som indkapsler et po-lyen-antibiotikum, idet vesiklerne omfatter et phospholipid og cholesterol, hvilken fremgangsmåde er ejen- 7 DK 168982 B1 dommelig ved, at man tilvejebringer en tilstrækkelig forskydningskraftpåvirkning til dannelse af vesiklerne i en vandig saccharidopløsning med lav ionstyrke.
Der kan benyttes forskellige hensigtsmæssige ud-5 førelsesformer for fremgangsmåden ifølge opfindelsen som angivet i krav 2-7.
Opfindelsen angår også et præparat, dannet ved ovennævnte fremgangsmåde ifølge opfindelsen, til anvendelse ved en fremgangsmåde til behandling af systemiske 10 fungale infektioner hos en menneskelig eller dyrisk vært.
Fremgangsmåden og præparatet ifølge opfindelsen opfylder de ovennævnte formål.
Der beskrives injektionspræparater, der består 15 af antifungale polyen-midler indkapslet i liposomer bestående af phospholipider og cholesterol, som også kan have en aminmodificeret overflade. Liposomerne er suspenderet i en vandig fase med lav ionstyrke, såsom en saccharidopløsning. Denne vandige fases pH-værdi ligger 20 fortrinsvis mellem ca. 6,0 og 8,0. Disse præparater administreres intravenøst til behandling af systemiske fungale infektioner.
Antifungale præparater opnås ved, at man anvender specifikke formuleringer i form af små unilamella-25 re vesikler til inkapsling af antifungale polyen-anti-biotika, såsom amphotericin B og nystatin,· og suspenderer disse partikler i en vandig fase med lav ionstyrke, hvori et mono- eller disaccharid er opløst. Denne vandige fase er fysiologisk isotonisk med en pH-værdi 30 på ca. 6,0 til 8,0. Disse præparater kan forbedres yderligere ved modificering af vesiklernes overflade med f.eks. en amin, hvorved der sørges for, at makrofager i mammalia-individets legeme kan genkende dem, hvilket muliggør målretning mod RES og således forøget 35 effektivitet af antifungal terapi.
Fremgangsmåder til dannelse af små vesikler er 8 DK 168982 B1 velkendte, og herunder kan der nævnes fremgangsmåder, som giver tilstrækkelig forskydningskraftpåvirkning (f.eks. sonificering, homogenisering, detergent-dialyse osv.). Det har vist sig, at ved den foreliggende op-5 findelse anvendelige små vesikler kan opnås ved sonificering eller homogenisering af følgende komponenter: antifungalt polyen-antibiotikum, mindst ét phospholipid, cholesterol og eventuelt 6-aminomannosederivater af cholesterol.
10 Det foretrukne præparat til anvendelse ved den foreliggende opfindelse består af et antifungalt polyen-antibiotikum, mindst ét phospholipid og cholesterol i en vandig fase med lav ionstyrke og med pH ca.
6,0 til 8,0 i et molforhold AMB:PL::CHOL på 0,2:2:1.
1 5 Vilkårlige velkendte phospholipider kan anvendes enkeltvis eller i kombination til dannelse af vesikler-ne. Repræsentative eksempler på sådanne phospholipider er: phosphatidylcholin (i det følgende betegnet som "PC"), både naturligt forekommende og syntetisk frem-20 stillet phosphatidsyre (i det følgende betegnet som "PA"), phosphatidylserin (i det følgende betegnet som "PS"), phosphatidylethanolamin (i det følgende betegnet som "PE"), phosphatidylglycerol (i det følgende betegnet som "PG"). De nævnte PC, PG, PA og PE kan fås 25 fra renset æggeblomme. Der kan også anvendes mættet syntetisk PC og PG, såsom dipalmitoylderivatet.
Cholesterol kan være til stede i en mængde i området fra 10 til 40 mol%. Hvis der anvendes for meget cholesterol, kan det forstyrre det antifungale polyen-30 antibiotikums binding til steroler, såsom ergosterol, i de fungale membraner. Cholesterol bibringer imidlertid vesiklen stabilitet, og en vis mængde er således ønskelig til forhindring af udsivning af det høj toksiske medikament. Det foretrækkes, at der opnås en koncentra-35 tion på mindst 1 mg antifugalt polyen-antibiotikum/ml til injektion.

Anvendelsen af en vandig fase med lav ionstyrke til suspendering af vesiklerne forbedrer vesikelstabi-liteten. Den vandige fase med lav ionstyrke består af mono- eller disacchrider opløst i Tris-puffer. Når der 5 anvendes et monosaccharid, såsom glucose, fructose eller galactose, er en 4-6%'s opløsning ønsket, og en 8-10%'s opløsning er ønsket, når der anvendes et disac-charid, såsom sucrose eller lactose. I modsætning til de kendte fremgangsmåder, ved hvilke der anvendes salt-10 opløsning som den suspenderende opløsning, hvilket forårsager udfældning af ikke-liposomalt amphotericin B (Jurgens, R. W. et al., Compatibility of Amphotericin B with certain large-volume parenterals, Am. J. Hosp. Pharm, 1981, 2å' 377-378), er anvendelsen af saccharid 15 i vandig fase med lav ionstyrke ikke forbundet med udfældning af antifungalt polyen-antibiotikum, således at liposom-indkapslingseffektiviteten og -stabiliteten forøges.
Målretning af veksiklerne mod makrofagerne i 20 RES-systemet kan fortrinsvis også opnås ved anvendelse af et forlænget aminmolekyle inkorporeret i vesiklernes overflade under anvendelse af de heri beskrevne fremgangsmåder. Ifølge en foretrukken udførelsesform er aminen et 6-aminomannosederivat af cholesterol.
25 Det første trin ved fremgangsmåden består i, at man blander mindst ét phospholipid og cholesterol opløst i et organisk opløsningsmiddel, såsom en alifatisk alkohol med 4 til 8 carbonatomer, f.eks. octanol og heptanol, med en DMSO-(dimethylsulfoxid-) eller dime-30 thylformamidopløsning af det antifungale polyen-antibiotikum.
Efter afdampning af opløsningsmidlet bliver phospholipidet tilbage sammen med lipidet cholesterol og det antifungale polyen-antibiotikum på siderne af 35 en passende reaktionsbeholder.
En vandig fase med lav ionstyrke (mindre end 20 10 DK 168982 B1 mM) og med pH 6,0-8,0, bestående af et mono- eller di-saccharid opløst i Tris-puffer, sættes derefter til beholderen. Lipidkomponenten og det antifungale polyen-antibiotikum, der tidligere er afsat på beholdervægge-5 ne, hydratiseres og suspenderes under omblanding. Den herved opnåede blanding sonificeres under anvendelse af en mikrosonde-sonifikator (Sonics & Materials, Inc., Model VC800) i ca. 12-45 minutter. Præparatet underkastes derefter centrifugering til fj ernelse af større 10 partikler, hvorved der opnås en suspension af små, unilamellare vesikler. Vesiklerne kan steriliseres ved passage gennem et 0,8 μ og derefter et 0,45 μ filter.
Dette filtrerede præparat af små, unilamellare vesikler kan derefter lyofiliseres til en tynd film el-15 ler et pulver, der er stabilt og kan oplagres til fremtidig anvendelse. Lyofilisering refererer til den proces, ved hvilken et stof fremstilles i tør form ved hurtig frysning og dehydratisering under højvakuum. Det vesikel-indkapslede antifungale polyen-antibiotikum om-20 dannes til et brugsfærdigt injektionspræparat mod fun-gale infektioner ved tilsætning af sterilt, destilleret vand til dette pulver og inkubering ved 37°C i ca. 15 minutter under lejlighedsvis omrystning.
Mængden af antifungalt polyen-antibiotikum, der 25 er indkapslet i de opnåede, ved de heri beskrevne fremgangsmåder fremstillede vesikler, kan bestemmes ved f.eks. en analyse med højtryksvæskechromatografi (HPLC) eller ved en spektrofotometrisk analyse under anvendelse af extinktionskoefficienten af antibiotikummet 30 ved bølgelængden med maksimal absorption. Størrelsen af de opnåede vesikler kan bestemmes under anvendelse af lysspredningsmetoder. De ved de heri beskrevne fremgangsmåder fremstillede vesikler har vist sig at være stabile ved oplagring.
35 Til bestemmelse af anvendeligheden af vesikel- formuleringerne ifølge opfindelsen administreres præpa- 11 DK 168982 B1 raterne med indkapslede antifungale polyen-antibiotika til dyremodeller, såsom mus, og forskellige parametre undersøges. Til påvisning af de omhandlede vesiklers toksicitet og biologiske effektivitet bestemmes både 5 akut og kronisk toksicitet. Til bestemmelse af akut toksicitet kan forskellige mængder af i SUV indkapslet antifugalt polyen-antibiotikum injiceres intravenøst i mus. Kontroldyr anvendes til bestemmelse af LD50 for uindkapslet medikament. Kronisk toksicitet hos menne-10 sker afspejler sig i lever- og nyrefunktionen. Denne toksicitet kan bestemmes i en dyremodel ved testning af sera for transaminaseaktivitet (SGOT/SGPT), blodurin-stofnitrogen (BUN) og creatininkoncentration. Hunde injiceres med subakut-toksiske (terapeutiske) koncentra-15 tioner af indkapslet eller uindkapslet medikament i flere dage. Blodprøver tages fra overlevende dyr over nærmere angivne tidsperioder og testes og sammenlignes med kontrolsera opnået fra sunde hunde.
Effektiviteten af antifungal terapi under an-20 vendelse af indkapslede og uindkapslede antifungale polyen-antibiotika kan også sammenlignes under anvendelse af en dyremodel. Mus kan podes med en letal eller subletal dosis af en virulent fungusstamme, såsom Candida albicans. Musene undersøges derefter for dødelig-25 hed (letal dosis) eller aflives på et nærmere angivet tidspunkt efter injektion (subletal dosis)· og de fun-guskolonidannende enheder fra ekstrakter af nyrerne bestemmes . Udviklingen af funguskolonier fra nyreekstrakterne angiver, om den fungale infektion er fuldstændig 30 forsvundet.
Stabiliteten af vesiklerne i blodstrømmen og optagelsen i forskellige væv i legemet, såsom i RES, bestemmes også til påvisning af anvendeligheden af præparaterne ifølge opfindelsen.
35 De nedenstående eksempler belyser fremstillin gen, karakteriseringen og in-vivo-anvendelsen i en dy- 12 DK 168982 B1 remodel af indkapslet amphotericin B under anvendelse af vesikelformuleringerne ifølge opfindelsen.
Fremstilling af vesikel-indkapslet amphotericin B 5
Eksempel A-C
En stamopløsning af amphotericin B (Squibb) fremstilledes under anvendele af 25 mg amphotericin B 10 pr. ml DMSO. Opløsningen rystedes godt, indtil alt amphotericin B var opløst. Ægge-phosphatidylcholin (Avanti) fremstilledes under anvendelse af 27 mg lecithin pr. ml octanol. Denne opløsning opbevaredes ved stuetemperatur, indtil den skulle anvendes. En 8 mg/ml 15 octanolopløsning af cholesterol (Calbiochem) fremstilledes og opbevaredes ligeledes ved stuetemperatur. Ami-nomannose opnået fra Vestar Research Inc. (Pasadena, CA) tilberedtes under anvendelse af 3 mg aminomannose pr. ml chloroform og opbevaredes i fryseren. Under an-20 vendelse af 250 ml rundbundede kolber fremstilledes phospholipid-vesikler ved blanding af stamopløsninger af amphotericin B (AMB), ægge-phosphatidylcholin (PL), cholesterol (CHOL) og aminomannose i forskellige molforhold som angivet i tabel I. De organiske opløsnings-25 midler afdampedes under anvendelse af en rotationsfordamper og et meget højt vakuum i 1 til 1½ time ved 65°C. Den opnåede film anbragtes under vakuum i en lyofilisator natten over. Denne film kan opbevares i op til en uge i lyofilisatoren, før vesiklerne fremstil-30 les.
Til den ovenfor beskrevne film sattes 8 ml phos-phat-pufret saltopløsning (PBS) [pH 7,2] til hver 250 ml rundbundet kolbe, og der omrørtes ved lave temperaturer med en lille magnetomrører. Der sattes endnu 2 ml 35 PBS til hver kolbe til skylning. Flere 2 ml delmængder af suspensionen, der indeholdt multilamellare vesikler 13 DK 168982 B1 (MLV'er), sonificeredes i 15 minutter i en vandbads-sonifikator (Sonics & Material Inc.) i et runbundet kulturrørglas med skruehætte. Den opnåede opløsning indeholdende SUV'er centrifugeredes ved 2500 o/min i 5 10 minutter til fjernelse af uopløselige materialer, og supernatanten fra denne centrifugering passeredes gennem en Sephadex ® 50-80 kolonne i PBS [pH 7,2]. Kolonnen tilberedtes i en 5 ml sprøjte, og efter at SUV'erne var fyldt på den, centrifugeredes den ved 10 2500 o/min i 10 minutter til fjernelse af uinkorporeret amphotericin B og alle øvrige uopløselige materialer.
Den opnåede SUV-suspension, dækket med folie, opbevaredes ved stuetemperatur. Ved efterfølgende anvendelse opvarmedes præparaterne til 37°C i 15 minutter og cen-15 trifugeredes derefter ved 2500 o/min i 10 minutter til fjernelse af alt uopløseligt materiale, som kunne være dannet under oplagring.
Eksempel D-0 20
En stamopløsning af amphotericin B (Squibb) fremstilledes under anvendelse af 25 mg amphotericin B pr. ml DMSO og rystedes godt, indtil alt amphotericin B var opløst. Ægge-phosphatidylcholin (PL) (Avanti) frem-25 stilledes under anvendelse af 20 mg lecithin pr. ml oc-tanol. Denne opløsning opbevaredes ved stuetemperatur, indtil den skulle anvendes. En 20 mg/ml octanolopløs-ning af cholesterol (Calbiochem) fremstilledes og opbevaredes ligeledes ved stuetemperatur. Stamopløsningen 30 af aminomannose indeholdt 3 mg/ml chloroformopløsning af aminomannose (Vestar Research, Inc., Pasadena, CA). Phospholipid-vesiklerne fremstilledes ved at blande stamopløsningerne af amphotericin B (AMB), ægge-phos-phatidylcholin (PL), cholesterol (CHOL) og aminomanno-35 se (AM) i forskellige molforhold som angivet i tabel I i 125 ml runbundede kolber.

De organiske opløsninger afdampedes under anvendelse af en rotationsfordamper og et meget højt vakuum i ca. 45 til 60 minutter ved 65°C. Den opnåede film anbragtes under vakuum i en lyofilisator natten over og 5 opbevaredes under nitrogen i fryseren i op til 2 uger. Kolben holdtes tildækket med folie.
Til den som ovenfor beskrevet opnåede film sattes 10 ml 5% dextrose i 10 mM Tris-Cl (pH 7,2) til hver kolbe, og hver kolbe opvarmedes i et 65°C varmt vandbad 10 i 40 minutter. Filmen i kolben resuspenderes ved kraftig omrystning og derefter ved anvendelse af en lille magnetomrører til fjernelse af de dele af filmen, der var vanskelige at resuspendere. Suspensionen indeholdende multilamellare vesikler (MLV'er) sonificeredes i 15 mindst 12 minutter ved 65°C med en sonde-sonifikator (Sonics & Materials, Inc., Model VC800) i en konisk glaskolbe. Det opnåede præparat indeholdende SUV'er centrifugeredes ved 2500 o/min i 10 minutter til fjernelse af uopløselige materialer og filtreredes derefter 20 gennem et 0,8 μ og derpå et 0,45 μ filter.
Alene til eksemplerne E og H overførtes 8 ml af det ovenfor opnåede filtrerede præparat til en rund-bundet kolbe og blev nedfrosset som en tynd film i et tøris/isopropanol-bad. Denne kolbe anbragtes i lyofili-25 satoren i 4 dage. Efter dette tidsrum fjernedes kolben fra lyofilisatoren, og der sattes 8 ml sterilt destilleret vand til filmen. Derefter inkuberedes kolben ved 37°C i 15 minutter under periodisk omrystning til re-suspendering af det lyofiliserede filmpræparat.

Karakterisering af vesikler med indkapslet amphotericin B
Mængden af amphotericin B i forbindelse med hver 5 af de under anvendelse af de ovenfor anførte procedurer fremstillede SUV-suspensioner er anført i tabel i som bestemt ved HPLC-analyse og ved spektrofotometrisk analyse under anvendelse af extinktionskoefficienten for amphotericin B ved 406 λ.
10 Størrelsen af de ifølge opfindelsen fremstillede SUVer bestemmes ved opvarmning af SUV-suspensionen til 65°C i 10 minutter og anbringelse af en 0,025 ml prøve i en Nicomp Model 200 Laser Particle Analyzer. Den under anvendelse af en Gauss' fordelingskurve bestemte 15 middeldiameter (mean) og standardafvigelse (S.D.) for SUVerne er også anført i tabel i.
Dyreundersøqelser 20 Mus inficeret med Candida albicans podedes in travenøst 5 timer efter infektion med indkapslet amphotericin B eller uindkapslet amphotericin B (Fungizone ®) under anvendelse af en enkelt dosis. 21 Dage efter infektion aflivedes de overlevende dyr, og deres nyrer 25 dyrkedes for Candida. 21 Dages overlevels'etallet anvendtes til bestemmelse af den dosis, der beskyttede 50% af dyrene mod døden (PD50)/ og nyrekulturtesten anvendtes til bestemmelse af den dosis, der helbredte 50% af de overlevende dyr (ED50). Resultaterne er anført 30 i tabel I.
Til bestemmelse af den kroniske toksicitet blev amphotericin-B-liposomer, fremstillet af Vestar Research, Inc., som en vandig formulering indeholdende 1,737 mg amphotericin B/ml, administreret intravenøst 35 til fire hunde, en gang dagligt i fire dage med en dag- 16 DK 168982 B1 lig dosis på 5 mg amphotericin B/kg. Injektionerne blev indgivet med en hastighed på 0,5 ml/sek. Blodprøver til bestemmelse af serumurinstofnitrogen, creatinin, GPT og alkalisk phosphatase blev taget før den første dosis 5 og om morgenen efter den sidste dosis. Resultaterne er anført i tabel II. Til sammenligning er der i tabel III anført resultaterne fra en hundeundersøgelse under anvendelse af uindkapslet amphotericin B (Fungizone ®) efter en anden doseringsplan.

Resultater
Den nødvendige dosis til frembringelse af akut intravenøs toksicitet (LDjjq) i en dyremodel forøgedes 15 til et højere niveau end 21,2 mg amphotericin B/kg, der er ca. 10 gange mere end for uindkapslet medikament og ca. 2 gange mere end for Tremblay-formuler ingen. Endvidere er over 90% af amphotericin B indkapslet ved den foreliggende opfindelse, en indkapslingseffek-20 tivitet der hidtil kun er set ved MLV'er. Præparatet er mindst lige så effektivt som uindkapslet amphotericin B som målt ved dyreoverlevelsestallet og antal levedygtige kolonier i visse organer i dyr inficeret med fungie-ne.
25 Det fremgår af undersøgelsen af kronisk toksici tet hos hunde, at den indkapslede form ifølge opfindelsen er ca. 1,5 til 2 gange mindre toksisk end uindkapslet amphotericin B (se tabellerne II og III).
Kliniske observationer indbefattede inaktivitet 30 og depression hos alle hunde gennem hele undersøgelsen efter administrering af den daglige dosis. Én hun blev yderst lethargisk og fik kropsrystelser på dag 4. Mild til moderat injektion af sclera i begge øjne indtrådte hos alle dyrene under eller kort efter dosering. Tåre-35 dannelse observeredes hos tre hunde efter den første dosis og var ledsaget af ringe salivation og en nasal 17 DK 168982 B1 udsondring hos én hund. Ajidre symptomer noteret efter dosering var opkastningsbevægelser, opkastning, opkastning med blod, bløde og løse fæces med blod og/eller mørke tjæreagtige fæces. Ved afslutningen af doserings-5 perioden var serumurinstofnitrogenet betydeligt forhøjet, og creatinin, GPT og alkalisk phosphatase i serum var moderat forhøjede. To af hundene blev aflivet i dårlig tilstand på dagen efter den sidste dosis.

Claims (8)
1. Fremgangsmåde til dannelse af små, unilamel-lare vesikler på mindre end 0,2 ym (2000 Å), som indkapsler et polyen-antibiotikum, idet vesiklerne omfatter et phospholipid og cholesterol, kendeteg- 5. e t ved, at man tilvejebringer en tilstrækkelig for-skydningskraftpåvirkning til dannelse af vesiklerne i en vandig saccharidopløsning med lav ionstyrke.
2. Fremgangsmåde ifølge krav 1, kendetegnet ved, at det antifungale polyen-antibioti- 10 kum er amphotericin B eller nystatin.
3. Fremgangsmåde ifølge krav 1 eller 2, kendetegnet ved, at den vandige opløsning har en ionstyrke på mindre end 20 mM.
4. Fremgangsmåde ifølge et vilkårligt af de fo- 15 regående krav, kendetegnet ved, at sacchari- det er valgt blandt sucrose, lactose, glucose, fructose og galactose.
5. Fremgangsmåde ifølge et vilkårligt af de foregående krav, kendetegnet ved, at saccha- 20 ridopløsningen omfatter et monosaccharid pufret med Tris.
6. Fremgangsmåde ifølge et vilkårligt af de foregående krav, kendetegnet ved, at molpro-centmængden af antifungalt polyen-antibiotikum er fra 25 5% til 15%.
7. Fremgangsmåde ifølge et vilkårligt af de foregående krav, kendetegnet ved, at det antifungale polyen-antibiotikum, phospholipid og cholesterol foreligger i et molprocentforhold, der ligger i om- 30 rådet fra 5-15 mol% antifungalt polyen-antibiotikum, 40-90 mol% phospholipid og 0,10-40 mol% cholesterol.
8. Præparat, dannet ifølge et vilkårligt af de foregående krav, til anvendelse ved en fremgangsmåde til behandling af systemiske fungale infektioner hos en 35 menneskelig eller dyrisk vært.
