Opfindelsen angår en udspiler af den i krav l indledning angivne art og en fremgangsmåde til fremstilling heraf.
I mange forskellige situationer ønsker man at tilvejebringe organer til at ekspandere en indsnævret blodåre eller til at 5 opretholde en åben passage gennem en blodåre. Der opstår f.eks. sådanne situationer efter en angioplastisk behandling af en kranspulsåre. I disse situationer er metaltrådsudspilere anvendelige til at forhindre restenosis af den udspilede blodåre eller til at forhindre faren for tilstopning på grund 10 af "hudlapper", der opstår fra indvendige rifter i forbindelse med den angioplastiske behandling. Metaltrådsudspilere kan også anvendes til at forstærke kollapsende organer i åndedræts-og galdeområderne.
Af kendte typer af metaltrådsudspilere er f.eks. beskrevet en 15 af Gianturco i US patent nr. 4.580.568, og denne udspilings-indretning er sammenpresset og indkapslet i en skede. Skeden positioneres i det vaskulære system, og udspilingsindretningen holdes på plads ved hjælp af en fladendet skubber, mens skeden trækkes tilbage. Zigzagudformningen af denne bestemte udspi-20 lingsindretning tillader, at den kan ekspandere i passagen for at holde passagen åben og forstørret.
Udspilere med forskelligt formede spiralfjedre er beskrevet af Maass og andre i US patent nr. 4.553.545 og i skriftet "Radiology Follow-up of Transluminally Inserted Vascular Endopros-25 thesis.· An Experimental Study Using Expanding Spirals, Radiology, september 1982; 152:659-663. Anvendelse af moment på enden af disse spiralfjedre forøger antallet af vindinger, mens udspilerens diameter bliver mindre ved indsætning. Efter indsætning påføres der et modsat rettet vridningsmoment på 30 spiralfjederen, hvilket får udspilerens diameter til at forø ges. Anvendelsen af denne type af udspiler kræver et sofistikeret koaksial kabel til påføring af vridningsmoment på udspileren, når den er blevet indsat i blodåren.

Et yderligere eksempel på den i det foranstående afsnit angivne kendte teknik fremgår af FR-offentliggørelsesskrift nr. 2.512.678.
Dotter og andre beskriver anvendelsen af en protese konstrue-5 ret af en termisk formhukommelseslegering, der bliver ført ind i passagen gennem et kateter. Se hvad der er beskrevet af Dotter CT og andre i Transluminal Expandable Nitinol Coil Stent Grafting: Preliminary Report, Radiology, April 1983; 147:259-260. Denne fj ederudspilingsindretning komprimeres ved afkø-10 ling, indsættes og opvarmes derefter på stedet, indtil udspi-lingsindretningen udvider sig i passagen. Udspileren positioneres i blodåren ved hjælp af en løsbar positioneringsindretning, der er i stand til at levere elektrisk energi til opvarmning af den termiske fjeder.
15 Andre referencer, som kan have relevans i henhold til den foreliggende opfindelse, er US patent nr. 3.278.176, US patent nr. 3.868.956, US patent nr. 4.425.908 samt US patent nr. 4.214.587.
Blandt ulemperne ved metaltrådsudspilere af den hidtil kendte 20 art og ekspanderbare fjederudspilere er, at disse udspilere enten er svære at producere eller er komplicerede at indsætte i en passage i legemet. Hver af disse udspilere kræver anvendelsen af en kompleks indretning til indsætning og ekspansion af udspileren i blodåren. På den anden side har de mindre kom-25 plekse metaltrådsudspilere manglende aksial føjelighed, når de skal passere gennem et kateter, som har den mindste kurve eller bøjning. Ved den foreliggende opfindelse undgås hver af disse ulemper ved tilvejebringelse af en metaltrådsudspiler, som er nem at fremstille, simpel at installere og er i stand 30 til at blive ført rundt i kurver og bøjninger i en blodåre eller en passage.
Ifølge opfindelsen tilvejebringes en udspiler af den i krav l's og 6's indledning angivne art, som er udformet som angivet DK 168654 B1 3 i krav l's og 6's og kendetegnende del. Herved opnås en forøgelse af den effektive diameter af udspileren i forhold til den langsgående akse uden anvendelse af termisk ekspansion eller påføringen af vridningskræfter på udspileren, som det 5 tidligere har været nødvendig at anvende. Ballonkateteret er foldet og optaget inden i den cylindriske fom og strækker sig langs aksen for den cylindriske fom. De rette sektioner er fomet om ballonkateteret, således at de i umiddelbar nærhed af hinanden liggende løkker fjerner sig fra hinanden i perifβίο riretningen, når ballonen blæses op.
Opfindelsen angår også en fremgangsmåde af den i krav 10's indledning angivne art, som er ejendommelig ved det i krav 10's kendetegnende del angivne.

Med det formål at tilvejebringe en forståelse af opfindelsens principper vil der nu blive henvist til den viste udførelses- DK 168654 B1 5 form på tegningerne, og der vil blive anvendt et specifikt sprog til at beskrive samme. Det skal dog ikke forstås, at der herved er tænkt en begrænsning af opfindelsens idé, idet der kan foretages mange ændringer ved den viste indretning, æn-5 dringer som er åbenlyse for en fagmand inden for området, uden at man derved afviger fra opfindelsens idé.
Den i fig. 1 viste metaltrådsudspiler 10 omfatter en langsgående akse 26. Udspileren omfatter et antal kurvede sektioner 11, som er beliggende i hovedsagen vinkelret på aksen 26. Hos-10 liggende kurvesektioner 11 er forbundet ved hjælp af bøjninger eller buespidser 13. En løkke 18 er formet ved hver frie ende af metaltrådsudspileren 10 for at beskytte metaltrådsenden. De kurvede sektioner 11 er formet til en cirkulær form, som vist i fig. 2, således at udspileren 10 har en cylindrisk åbning 12 15 igennem sig.
De kurvede sektioner 11 og buespidserne 13 danner en serie af alternerende løkker 15 og 16 forløbende i retning med uret og i retning mod uret. Forløbet i retning med uret i forhold til aksen 26 er blevet vilkårligt valgt og angivet med den kraf-20 tige pil 27 i fig. 1. I den sammentrukne tilstand af udspileren 10 overlapper disse løkker 15 og 16 hinanden i langsgående retning, som vist ved hjælp af overlapningsområdet 20 i fig. 3. Derfor giver overlapningsområdet 20 indtrykket, at udspileren er en kontinuerlig cirkulær ring, når den ses fra 25 enden, selvom, når den ses som vist i fig. 3, det er tydeligt, at den cylindriske form af udspileren 10 er diskontinuert. Vigtigheden af dette træk kan anskueliggøres ved hjælp af fig.
3 og 4.
I fig. 3 er udspileren 10 vist fastgjort rundt om et kateter 30 22, som har en opblæselig ballon 23 fastgjort derpå, og som omgiver en tilføringsåbning 22a i kateteret. Ballonen, der anvendes ved denne udførelsesform, er en foldet ballon, hvori ballonens 23 lapper 23a er foldet ind over kateteret 22, som vist i fig. 3. De foldede lapper 23a tillader, at ballonen 23 6 DK 168654 B1 kan blæses op til en bestemt diameter, uden at ballonmaterialet strækkes for meget, og man risikerer, at ballonen rives i stykker.
Udspileren er komprimeret om kateteret 22 og ballonen 23, så-5 ledes at den indtager en sammentrukket ydre diameter 24, som er tilpasset til at tillade indsætning i en bestemt passage i kroppen. De i retning med uret forløbende løkker 15 og de i retning mod uret forløbende løkker 16 overlapper hinanden i området 20 og f jederstivheden af metaltråden holder udspileren 10 i denne position under indsætning. Udspileren 10 forbliver i stram berøring med kateteret 22, også når samlingen føres rundt i kurver og bøjninger i passagen i kroppen.
Efter at kateteret og udspileren er fuldt indsatte i passagen, bliver ballonerne 23 blæst op til en diameter 25, som er til-15 passet til at tvinge udspileren 10 i berøring med passagens indre overflade og i det mindste i nogle tilfælde til at ekspandere passagen. Når ballonen blæses op, fjerner de i retning med uret og i retning mod uret forløbende løkker 15 og 16 sig i periferiretningen fra hinanden, indtil den i længderetningen 20 sete overlapning mellem løkkerne reduceres til området 21, vist i fig. 4. Derved forøges den effektive diameter af udspileren 10 i forhold til den langsgående akse 26 uden anvendelse af termisk ekspansion eller påføringen af vridningskræfter på udspileren, som det tidligere har været nødvendig at anvende.
25 Ved den mest foretrukne udførelsesform ifølge opfindelsen er den metaltråd, som udspileren 10 er tildannet af, fremstillet af et let bearbejdeligt materiale, fortrinsvis fra gruppen, der omfatter udglødet rustfrit stål, wolfram og platin. Dette udglødede materiale er tilstrækkeligt deformerbart til, at 30 løkkerne 15 og 16 kan fjerne sig fra hinanden, når de udsættes for radialt udadrettede tryk påført ved opblæsning af membranen, som omfatter standardballonkateteret. Idet udspilerens materiale er plastisk i højere grad end det er elastisk, bibeholder udspileren den udvidede diameter, efter at ballonen 23 DK 168654 B1 7 er tømt, og kateteret 22 er fjernet. Materialet har imidlertid tilstrækkelig styrke og stivhed til at undgå, at udspileren forskubbes på ballonen under indsætning og til at undgå, at løkkerne 15 og 16 tvinges til at ligge ind over hinanden. Ud-5 spileren har endvidere tilstrækkelig styrke og stivhed til at tillade, at den holdes på plads i passagen og til at modstå, at den frigøres efter at kateteret 22 er fjernet, og ballonen ikke længere stabiliserer udspileren. Et eksempel på en velegnet metaltråd er en tråd med en ydre diameter på 0,045 mm, og 10 som er fremstillet af en rustfri stållegering såsom AISI 316.
Det foretrækkes, at overlapningsområdet 20 ved den sammentruk-ne tilstand er tilstrækkeligt stort til, at udspileren har et stort berøringsområde med kateteret, hvilket tilvejebringer yderligere beskyttelse mod, at udspileren forskubber sig, mens 15 samlingen indsættes. I den ekspanderede tilstand skal overlapningsområdet 21 være tilstrækkeligt stort til at tilvejebringe en kontinuerlig understøtning langs periferien for passagen, hvori udspileren indsættes. Ved et eksempel ifølge opfindelsen strækker overlapningsområdet 20 sig næsten 360° langs perife-20 rien. For klarheds skyld er overlappet 20 ved den viste udførelsesform noget mindre.
Den foretrukne udførelsesform kan opdeles i enkelte ringe af spiralformede sektioner. Den i fig. 5 i sprængbillede viste metaltrådsudspiler 49 har en langsgående akse 50. Forløbet i 25 retning med uret ifølge en højrehåndsregel er angivet ved hjælp af den kraftige pil 48 om aksen 50. Udspileren 49 omfatter en serie af alternerende enkelte ringe, der forløber i retning med uret som spiralformede sektioner 51 og en enkelt ring, der forløber i retning mod uret, som spiralformede sek-30 tioner 52. De spiralformede sektioner 51 og 52 har forreste ender 51a og 52a og bageste ender 51b og 52b. De modstående spiralfomede sektioner er forbundet ved hjælp af buespidser 53 med den forreste ende af en spiralformet sektion forbundet med den bageste ende af den næste spiralf omede sektion. Der-35 for er enden 51a af den i retning med uret forløbende spiral- 8 DK 168654 B1 formede sektion 51 forbundet med enden 52b af den i retning mod uret forløbende spiralformede sektion 52, mens enden 52a er forbundet med enden 51b.
Ved en fremgangsmåde til anvendelse af udspileren er en udspi-5 ler og ballonkatetersamling 80 indsat i en passage 72, såsom en arterie, i en patients krop 70, som vist i fig. 6. Samlingen 80 er vist i den ikke opblæste tilstand, hvor den kan manøvreres rundt i kurven 76 i passagen 72. Stivheden af kateteret 82 tillader, at samlingen 80 kan følge kurven 76, mens 10 styrken og stivheden af udspileren 84 holder den i stramt indgreb på kateterballonen 86 under indsætning. Passagen har en indsnævring 74 beliggende ved en anden bøjning i passagen.
Den i fig. 7 viste udspilerballonkatetersamling 80 er fuldt indsat i passagen 72, således at udspileren 84 og ballonen 86 15 er beliggende direkte over for indsnævringen 74 og følger passagens kurve. Samlingen er vist i den ekspandere konfiguration 84' i fig. 8, hvor ballonen 86' er blæst op og metaltrådsudspileren 84' er ekspanderet til berøring og udvidelse af passagen 82. For tydeligheds skyld er udvidelsen vist overdrevent 20 i fig. 8. Samlingen ekspanderes tilstrækkeligt til, at indsnævringen 74 (fig. 7) kan fjernes, og passagen kan åbnes. Ballonen tømmes derefter, og kateteret fjernes, således at udspileren efterlades til at holde passagen åben.
Fremgangsmåden ved den foreliggende opfindelse omfatter frem-25 stilling af en udspiler. Som vist i fig. 9 er en metaltråd 30 bøjet til en plan serpentineragtig form. Denne form omfatter en serie af rette sektioner 33 forbundet ved hjælp af bøjninger eller buespidser 35. Efter formningen bliver de rette sektioner 33 kurvede sektioner 11 og buespidserne 35 bliver bue-30 spidser 13. Parterne af metaltråden 30 på hver sin side af centerlinien 31 bliver de i retning med uret og i retning mod uret forløbende løkker 15 og 16, som vist i fig. 9. Den frie ende 60 kan formes til en løkke, såsom en løkke 18 vist i fig.

Et formværktøj 39 omfatter en flad plade 40 med et ret trug 42 formet deri. Ved det i fig. 10 viste tværsnit kan det ses, at truget 42 har en semicirkulær overflade 42. Metaltråden 30 er anbragt fladt på pladen, således at centerlinien 31 falder 5 sammen med trugets 42 centerlinie.
Metaltråden 30 presses ned i truget 42 og mod den semicirkulæ-re overflade 43 ved anvendelse en tildannerstang 45, som vist i fig. 11 og 12. Tildannerstangen 45 holdes på plads ved hjælp af elastiske bånd 46. Enderne af løkkerne 15 og 16 rager opad 10 og udad fra truget 42 og ovenover overfladen af den flade plade 40. Ved det næste trin ifølge den foreliggende fremgangsmåde anvendes der et trækværktøj 47 til at trække løkkerne 15 og 16 over den blotlagte overflade 45a (fig. 12) af tildannerstangen 45, som vist i fig. 13 og 14. Kroge 47a ved enden af 15 trækværktøjerne 47 indgriber med buespidserne 13 af løkkerne 15 og 16 under dette trækketrin. Herefter fjernes den delvist formede udspiler fra fladen 40 ved at løfte tildannerstangen væk fra pladen. Stangen 45 fjernes og efterlader den delvis formede udspiler, som vist i fig. 15.
20 Ballonkateteret 22 indsættes gennem den langsgående cylindriske åbning 12 i udspileren 10, som vist i fig. 15, og enderne af løkkerne 15 og 16 presses i berøring med kateteret, som vist i fig. 3. Som nævnt foretrækkes det, at løkkerne 15 og 16, når de befinder sig i deres endelige overlappende positi-25 on, overlapper en lille smule. Ved en udførelsesform ifølge opfindelsen kan denne overlapning 20 (se fig. 3) være omkring 360°. Det anvendte ballonkateter kan have forskellige udformninger, såsom beskrevet af William A. Cook i US patentskrift nr. 4.637.396 eller et ballonkateter, der kan fås fra Cook 30 Incorporated, Bloomington, Indiana, under deres katalognr. OMG

Opfindelsen er blevet beskrevet under henvisning til tegningen og specielt hensigtsmæssige udførelsesformer. Der kan dog foretages mange ændringer, uden at man derfor afviger fra opfin-
Claims (10)
1. Udspiler, som omfatter en metaltråd tildannet med en ser-5 pentineragtig udformning omfattende en serie af rette sektioner (33) og et antal bøjninger (35), hvilke rette sektioner er forbundet med bøjningerne til dannelse af en serie af alternerende løkker (15) og (16), kendetegnet ved, at den serpentineragtige udformning er formet til en cylindrisk 10 form med en langsgående akse (31), og hvor de rette sektioner er bøjet til i hovedsagen cirkulær form, der omgiver og forløber i hovedsagen vinkelret på aksen, samt organer til at ekspandere denne cirkulære udformning.
2. Udspiler ifølge krav 1, kendetegnet ved, at 15 organerne til at ekspandere er et ballonkateter (22), hvilket ballonkateter er optaget inden i den cylindriske udformning og strækker sig langs aksen.
3. Udspiler ifølge krav 2, kendetegnet ved, at ballonkatetereter et foldet ballonkateter.
4. Udspiler ifølge krav 1, kendetegnet ved, at alternerende løkker omfatter, set i længderetningen overlappende parter beliggende i umiddelbar nærheden af bøjningerne, og som er indrettet til at fjerne sig fra hinanden i periferiretningen som følge af ekspansion udøvet af ekspanderingsorga-25 nerne.
5. Udspiler ifølge krav 1, kendetegnet ved, at metaltråden omfatter to modstående ender, hvor den er formet til en løkke (18) i umiddelbar nærhed af hver ende, således at enderne i hovedsagen er beskyttet.
