Den foreliggende opfindelse angår anvendelsen af en peroxydi-phosphatforbindelse til fremstilling af et lægemiddel til in-hibering af hypotensivt chok og lokal knogleresorption forårsaget af bakterieendotoxiner.
5 Endotoxiner er komplekse makromolekyler indeholdende lipid, carbohydrat og protein. De findes hovedsageligt i overfladen af gramnegative organismer og omtales sædvanligvis som lipopo-lysakkarider. Disse makromolekyler er toksiske for værten og kan være skæbnesvangre. De kan f.eks. forårsage alvorlige hy-10 potensive chok og kan også udløse forskellige toksiske reaktioner i legemer, herunder knogleresorption. I munden har endotoxiner været indblandet som en hovedfaktor i betændelsen af gummevæv og i lokaliseret knogletab, såsom alveoleknogletab.
Teoretisk kunne forbindelser, som frigiver oxygen, inaktivere 15 endotoxiner. På grund af den hurtighed, med hvilken mange oxygenudviklende forbindelser frigiver oxygen, har de imidlertid ringe effekt med hensyn til regulering af endotoxinvækst. De forbindelser, som frigiver oxygen langsommere, kunne regulere endotoxinvirkning. Deres effektivitet er imidlertid sædvanlig-20 vis begrænset, idet betingelserne for oxygenfrigivelse ikke svarer til de betingelser, der er fremherskende i legemet. Som beskrevet i DK patentansøgning nr. 2480/85 har varmblodede pattedyr, såsom fra gnavere op til og inklusive mennesker, alkalisk phosphatase eller sur phosphatase i legemet. Peroxy-25 diphosphatforbindelser har evnen til langsom oxygenfrigivelse.
Den mængde oxygen, som de frigiver, er en tiendedel af den mængde, der frigives af hydrogenperoxid. Kun ca. 50% af deres aktive oxygen frigives i løbet af 20 timer ved 25°C i nærværelse af alkalisk phosphatase eller sur phosphatase.

10 Det er blevet observeret, at bakterielt endotoxin også reagerer med intakt PDP. Denne reaktion foregår uafhængigt af tilstedeværelsen af phosphataser, dvs. den foregår også uden for legemet af et varmblodet dyr. Reaktionen foregår imidlertid ganske overraskende, selv i nærværelse af phosphatase, også 15 når varmblodede pattedyr behandles med PDP i overensstemmelse med den foreliggende opfindelse. Det er ønskeligt at tilvejebringe en kur, hvori behandlingen fortsætter, indtil endotoxi-ner er inaktiveret.
Formålet med denne opfindelse er at inaktivere endotoxiner og 20 derved hæmme deres toksiske virkninger, såsom betændelse, knogleresorption og hypotensive chok.
Andre formål med denne opfindelse vil fremgå af den følgende beskrivelse.
Denne opfindelse angår derfor anvendelse af en ikke-toksisk, 25 vandopløselig, farmaceutisk acceptabel peroxydiphosphatforbindelse i form af et alkalimetal-, jordalkalimetal-, zink-, tineller kvaternært ammoniumsalt eller en C1_12-alkyl-, adeny-lyl-, guanylyl-, cytosylyl- eller thymylylester til fremstilling af et lægemiddel til inhibering af hypotensivt chok og 30 lokal knogleresorption forårsaget af bakterieendotoxiner, hvilken forbindelse ved kontakt med endotoxin forårsager inaktivering af de bakterielle endotoxiner.

En procedure til at godtgøre inaktivering af endotoxin er at overkomme induktion af dannelse af en faktor, som er kemotak-sisk over for polymorfkernede leukocytter, herefter kaldet "PMN". En sådan faktor kan blive vurderet i overensstemmelse 5 med Boydens kemotaksimetode, hvori hvide blodlegemer fra en kanin tiltrækkes (kemotaksi) af endotoxininduceret faktor, der er dannet i området. Ved Boyden-metoden er det der sker, når et bakterielt endotoxinlipopolysakkarid inkuberes med serum fra et pattedyr, at:

Når endotoxin in-15 ducerer kemotaksi som i den foreliggende opfindelse, kan % hæmning bestemmes kvantitativt under anvendelse af Boydens ke-motaksitest.
Endotoxinmateriale kan indføres i legemet af et varmblodet dyr gennem dets tilstedeværelse i overfladen af gramnegative mi-20 kroorganismer, såsom Actinobacillus actinomycetemcomitens (A.a.), Escherichia coli (E. coli), Bacteroides melanenogeni-cus (B. mel.) og Salmonella typhi (S. typhi).
Oralt endotoxin isoleret fra A.a. er toksisk for alveoleknogle. Ikke-oralt endotoxin, som er oprenset fra E. coli, kan 25 vise sig skæbnesvangert for værten.
Andre kendte fremgangsmåder til hæmning af endotoxindannelse foretages under anvendelse af resorption i et knoglekulturmedium. En kyllingeembryolethalitets-test kan også anvendes.

Den toksiske reaktion hæmmes effektivt ved behandling af endotoxin in s i tu i en varmblodet vært med en hæmningseffektiv mængde af en ikke-toksisk, vandopløselig, farmaceutisk acceptabel peroxydiphosphatforbindelse. Peroxydiphosphatet reagerer 5 med endotoxinet i legemet som et intakt molekyle, mens det in-aktiverer peroxydiphosphatforbindelsen. Eftersom endotoxinet inaktiveres, er det tydeligt, at endotoxin reagerer med peroxydiphosphatet .
Sædvanligvis er ca. 0,1-7% peroxydiphosphatf orbindelse i en 10 farmaceutisk bærer, såsom i opløsning, effektiv i en ordineret dosis på ca. 0,2-14 mg pr. kg. legemsvægt. Hæmningseffektivitet kan vurderes ved reduceret endotoxinvirkning og bestemmes kvantitativt på basis af inhiberet kemotaksi over for PMN.
Typiske ikke-toksiske, vandopløselige, farmaceutisk acceptable 15 peroxydiphosphatforbindelser er alkalimetalsaltene (f .eks. lithium, natrium og kalium), jordalkalimetalsaltene (f.eks. magnesium, calcium og strontium) og zink- og tinsaltene samt kva-ternære ammoniumsalte samt C-L_12-alkyl-, adenylyl-, guanylyl-, cytosylyl- og thymylylestrene. Alkalimetalsalt, især kalium-20 salt, foretrækkes blandt de uorganiske kationer. Tetrakalium-peroxydiphosphatet er et stabilt, lugtfrit, findelt, frit-strømmende, hvidt, ikke-hygroskopisk, krystallinsk, fast stof med en molekylvægt på 346,35 og et indhold af aktivt oxygen på 4,6%.
25 Tetrakaliumperoxydiphosphat er 47-51% vandopløseligt ved 0-61 °C, men uopløseligt i almindelige opløsningsmidler, såsom acetonitril, alkoholer, ethere, ketoner, dimethylformamid, di-methylsulfoxid og lignende. En 2% vandig opløsning har en pH-værdi på ca. 9,6, og en mættet opløsning deraf en pH-værdi på 30 ca. 10,9. En 10% opløsning i vand ved 25°C viste intet tab af aktivt oxygen efter 4 måneders forløb, og ved 50°C viste en 10% opløsning et tab af aktivt oxygen på 3% i løbet af 6 måneder.

Blandt de organiske forbindelser foretrækkes de forbindelser, der tilvejebringer hydrofobe egenskaber, såsom C1_12-alkylra-dikalet og dem, der letter den hurtige optagelse af peroxydi-phosphatdelen af cellerne, såsom adenylyl-, guanylyl-, cytosy-5 lyl- og thymylylestere.
Peroxydiphosphatforbindelsen kan indgives oralt eller systemisk for at hæmme endotoxiner i mundhulen eller andre dele af legemet.
Farmaceutiske bærere, som er egnede til oral fordøjelse, er be-10 lagte tabletter bestående af materiale, som modstår nedbrydning af mavesyrer ved mave-pH-værdien (ca. 1-3) , eftersom peroxydiphosphat ville blive inaktiveret af sådanne mavesyrer. Bærerne med tabletterede korn af det faste peroxydiphosphorsyre-saltmateriale deri opløses nærmere af tarmvæsker, som har en 15 højere pH-værdi (ca. 5,5-10) og ikke inaktiverer peroxydiphos-phatet og lader det blive underkastet enzymatisk virkning af phosphatase, som er til stede i mennesker eller andre varmblodede dyr. En ønskelig tabletbelægningsopløsning består af en fedtsyreester, såsom N-butylstearat (typisk ca. 40-50, for-20 trinsvis 45 vægtdele), voksarter såsom carnaubavoks (typisk ca. 15-25, fortrinsvis ca. 20 vægtdele), fedtsyre såsom stearinsyre (typisk ca. 20-30, fortrinsvis 25 vægtdele) og celluloseester, såsom celluloseacetatphthalat (typisk ca. 5-15, fortrinsvis ca. 10 vægtdele) og organisk opløsningsmiddel (typisk 25 ca. 400-900 dele). Andre ønskelige belægningsmaterialer omfatter shellak og copolymerer af maleinsyreanhydrid og ethyleni-ske forbindelser, såsom polyvinylmethylether. Sådanne belægninger adskiller sig fra tabletter, der nedbrydes i mundhulen, og hvori tabletmaterialet typisk indeholder ca. 80-90 vægtdele 30 mannitol og ca. 30-40 vægtdele magnesiumstearat.
Tabletterede korn af peroxydiphosphatsaltet dannes ved at blande ca. 30-50 vægtdele af peroxydiphosphatsaltet med ca. 45-65 vægtdele af et fast polyhydroxysukker, såsom mannitol og befugte med ca. 20-35 vægtdele af en polyhydroxysukkerforbin- 6 DK 168513 B1 delsesopløsning, såsom sorbitol, sigte for at sortere efter størrelse, blande med ca. 20-35 vægtdele af et bindemiddel, såsom magnesiumstearat, og sammenpresse kornene til tabletter ved en tabletkomprimeringsmaskine. De tabletterede korn belæg-5 ges ved at sprøjte et skum af en opløsning af belægningsmaterialet derpå og tørre for at fjerne opløsningsmiddel.
Sådanne tabletter adskiller sig fra dentaltabletter, som typisk er sammenpressede korn uden en speciel beskyttende belægning.
10 En effektiv indgiftsdosis af peroxydiphosphat er ved en fore-skreven kur, når indgiften er ved oral indtagelse, ca. 0,1-6 g pr. kg. legemsvægt dagligt, og når indgiften er systemisk, såsom ved intramuskulær, intraperitoneal eller intravenøs injektion, er dosen ca. 0,1-2 g pr. kg. legemsvægt dagligt.
15 Fysiologisk acceptable pyrogenfrie opløsningsmidler er hensigtsmæssige bærere til anvendelse inden for den inden for fagområdet anerkendte måde til systemisk indgift. Saltopløsning, puf ret med phosphat til en fysiologisk pH-værdi på ca. 7 til 7,4, er den foretrukne bærer til systemisk indgift. Sådan- 20 ne opløsningsmidler adskiller sig fra vand-befugtningsmiddel-bærere, som typisk anvendes i tandplejemidler. En sådan opløsning fremstilles typisk ved sterilisation af deioniseret, destilleret vand, undersøgelse for at sikre ikke-pyrogenicitet under anvendelse af Limulus amebocytlysattesten (LAL), der er 25 beskrevet af Tsuji et al i "Pharmaceutical Manufacturing", oktober, 1984, side 35-41, og derpå tilsætning dertil af en phos-phatpuffer (pH-værdi f.eks. 8,5-10), der er fremstillet i py-rogenfrit, sterilt vand og ca. 1-100 mg peroxydiphosphatfor-bindelsesderivat og natriumchlorid til en koncentration på ca.
30 0,5-1,5 vægt%. Opløsningen kan pakkes i hætteflasker til brug efter gensterilisation ved passage igennem et mikroporefilter. Som alternativer kan der anvendes andre opløsninger, såsom Ringer's opløsning indeholdende 0,86% natriumchlorid, 0,03 vægt% kaliumchlorid og 0,033 vægt% calciumchlorid.

De følgende eksempler illustrerer peroxydiphosphatforbindel-sens (PDP) evne til at hæmme kemotaksi, som er induceret af endotoxinfremkaldt faktor i serum og til at hæmme endotoxin-toksicitet over for knogler.
5 Eksempel 1.
PMN opnås fra de peritoneale kaviteter fra voksne hvide New Zealand kaniner 12 timer efter intraperitoneale injektioner af 200 ml opløsning indeholdende 0,2% glycogen i steril, isoto-nisk saltopløsning (0,85% NaCl). Cellerne (PMN) oprenses fra 10 eksudatet opnået fra den peritoneale kavitet fra kanin og oprenses som beskrevet af Taichman et al (Arch. Oral Biol. 21 side 257, 1976). Bakterielt endotoxin oprenset fra E. coli, der fås fra Associates of Cape Cod Inc. Woods Hole, Maine, forbehandles med forskellige koncentrationer af PDP (tetrakalium-15 salt) ved 37°C i 1 time. Kemotaksianalysen udføres derpå med behandlede og ubehandlede endotoxiner under anvendelse af Boy-den-kammeret som beskrevet ovenfor. Dataene er summeret i tabel 1 og 2.

PDP (0,5%) har ingen virkning på PMN (nr. 3), og endotoxiner forbehandlet med PDP har signifikant reduceret kemotaksisk virkning af toxinet (behandlinger 15 nr. 4, 5 og 6). Disse data antyder, at en behandling af endotoxin med PDP deaktiverer toxinets biologiske virkning.
Eksempel 2.
Tabel 2 viser de data, der blev opnået med yderligere Boyden-kammertest som i eksempel 1. PDP anvendes som tetrakaliumsal-20 tet.

Dataene i den ovenstående tabel viser, at en effektiv koncentration af PDP på så lidt som 0,1% deaktiverer endotoxinets biologiske aktivitet.
10 Eksempel 3.
PDP's virkninger på endotoxinaktivitet i knoalekultursvstem
Den test, i hvilken et endotoxin isoleret fra Acintobacillus actinomycetemcomitans Y4 (AAY4) inducerer knogleresorption i et knoglekultursystem (Kiley og Holt, Infect. Immun. 30:262-15 373, 1980) anvendes til at vurdere, hvorvidt PDP deaktiverer den knogleresorptive aktivitet af endotoxin fra Y4. Føtal rot-teknoglekultur, som beskrevet af Raisz, J. Clin. Invest. 44:-103-115, 1965, fremstilles ved at injicere rotter med ^CaCl2 på den 18. dag af svangerskabet. Rotterne aflives derefter på 20 den 19. dag, og spoleben og albueben fra embryoerne, med deres bruskender, fjernes og placeres til dyrkning i BGJ-medium (Gibco, Buffalo, NY) ved 37°C sammen med 5% C02· Mediet suppleres med 5% opvarmet (57°C i 3 timer) føtalt kalveserum. Knogler placeres med 4 i en brønd i 24 brøndskåle (Nunc, Gibco) 25 indeholdende 0,5 ml medium pr. brønd. Frigivelsen af ^Ca til dyrkningsmedierne fra knogle inkuberet i nærværelse af et test-middel sammenlignes med frigivelsen fra knogler inkuberet i kontrolmedier, og resultaterne af knogleresorption udtrykkes som et forhold.
30 Endotoxin fra AAY4 fås fra University of Pennsylvania, School of Dentistry. AAY4-endotoxin behandles med forskellige koncen- DK 168513 B1 10 trationer af PDP-tetrakaliumsalt ved 37°C. Overskydende PDP fjernes ved hjælp af dialysemembran (maksimal molekylvægt 3500). Dette tillader ikke-reaktivt PDP at diffundere ud, medens endotoxinet, som har en molekylvægt, der er højere end 5 3500, tilbageholdes inde i sækken. Tabel 3 opsummerer dataene.

1. Anvendelse af en ikke-toksisk, vandopløselig, farmaceutisk acceptabel peroxydiphosphatforbindelse i form af et alkalimetal-, jordalkalimetal-, zink-, tin- eller kvaternært ammonium- 10 salt eller en C1_12-alkyl-, adenylyl-, guanylyl-, cytosylyl-eller thymylylester til fremstilling af et lægemiddel til in-hibering af hypotensivt chok og lokal knogleresorption forårsaget af bakterieendotoxiner.
2. Anvendelse ifølge krav 1, kendetegnet ved, at 15 peroxydiphosphatforbindelsen er til stede i en mængde på ca. 0,1-7% i en farmaceutisk bærer.
3. Anvendelse ifølge krav 1 eller 2, kendetegnet ved, at peroxydiphosphatforbindelsen er til stede i tablette-rede korn med en belægning derpå, der ikke nedbrydes under 20 passage igennem maven hos varmblodede dyr, og hvilken belægning opløses af tarmvæsker med en pH-værdi på 5-10.
