Description
- Die vorliegende Erfindung betrifft chirurgische Instrumente zum Anbringen von Reihen von Befestigungselementen an Körpergewebe, und insbesondere Amboßelemente, die in derartigen Instrumenten verwendet werden. Ein derartiges Amboßelement und ein chirurgisches Instrument mit den Merkmalen des Oberbegriffs des untenstehenden Anspruchs 1 sind in der US-A-5 014 899 beschrieben.
- Instrumente zum Anbringen von chirurgischen Befestigungselementen, die zum Anbringen von parallelen Reihen von Befestigungselementen an Körpergewebe verwendet werden, sind in der Technik bekannt und werden verbreitet zum Querdurchschneiden oder Wiederverbinden von Kärpergewebe, wie z.B. Darm-, Magen- oder Lungengewebe verwendet. Die Befestigungselemente kännen als Klammern ausgebildet sein, die durch das Umschlagen der Klammerbeine geschlossen werden, oder sie können als zweiteilige Befestigungselemente ausgebildet sein, die durch das Verrasten eines Befestigungsabschnitts mit einem Halteabschnitt geschlossen werden.
- Instrumente zum Anbringen von chirurgischen Klammern sind zum Beispiel in den US-Patenten 4 633 861; 4 633 874; 4 863 088; 3 079 606; 3 490 675; 3 499 591 und 4 429 695 dargestellt und beschrieben. Im allgemeinen weisen solche Klammerapparate ein gabelförmiges distales Ende auf. Das Körpergewebe, an dem operiert werden soll, wird fest zwischen zwei Gabeln des Instruments gehalten, und die Klammern werden nacheinander an dem Gewebe angebracht.
- Die eine Gabel weist eine Einrichtung zum Halten und Schieben von Klammern auf. Gewöhnlich werden wenigstens zwei parallele Reihen von Klammern in Schlitzen gehalten, die quer zur Längsachse des Instruments ausgerichtet sind. Hinter den Klammern sind Schiebeelemente angeordnet. Wenn eine Nockenstange durch ein Betätigungselement in Längsrichtung bewegt wird, werden die Schiebeelemente durch die Schlitze getrieben, wodurch die Klammern nach außen in das Körpergewebe geschoben werden. Die andere Gabel weist einen Amboß auf, d.h. eine Einrichtung zum Schließen von Klammern. Die Beine der Klammer durchdringen das Körpergewebe und gelangen in Mulden zum Schließen von Klammern in dem Amboß, wo sie umgeschlagen werden.
- Die chirurgischen Befestigungselemente müssen zuverlässig geschlossen sein, um sicherzustellen, daß das Körpergewebe sicher befestigt ist, andernfalls kann es zu Blutungen oder anderen Komplikationen aufgrund einer schlechten Blutstillung des befestigten Gewebes kommen. Ein zuverlässiges Schließen der Befestigungselemente erfordert eine exakte Ausrichtung der zwei Gabeln des Instruments, so daß die Befestigungselemente exakt mit der Schließeinrichtung ausgerichtet sind. Jedoch kann es zu einer Biegung oder Durchbiegung des Ambosses kommen, wenn die beiden Gabeln mit dazwischen befindlichem Körpergewebe zusammengeklemmt werden, und diese Biegung kann zu einem Versatz der Klammern und der Mulden zum Schließen der Klammern führen. Eine Durchbiegung kann ebenso aufgrund von Vertikalkräften auftreten, die auf den Amboß während der Berührung durch die Befestigungselemente aufgebracht werden. Zu einer größeren Durchbiegung kommt es an dem distalen Ende des Ambosses. Je länger der Amboß ist, desto größer ist die Tendenz, sich zu biegen. Demnach ist eine Möglichkeit, das Biegen zu beheben, die Länge des Ambosses zu begrenzen. Aber bei vielen Anwendungen ist es erwünscht, einen längeren Amboß zu benützten, in dem längere Reihen von Klammern abgelegt werden können.
- Eine andere Möglichkeit, das Biegen zu verringern, ist eine Verstärkung des Ambosses. Gegenwärtig werden Ambosse durch Kaltbearbeitung von Edelstahlblech mit einer Dicke von etwa 1,27 mm (0,05 Inch) bis etwa 1,90 mm (0,075 Inch) in die gewünschte Form gefertigt. Stärkere Ambosse können durch eine Vergrößerung der Dicke des Stahls hergestellt werden. Jedoch führt dies insofern zu Nachteilen, als das Gewicht und die Kosten des Ambosses, ebenso wie die Schwierigkeit der Bearbeitung von dickerem Stahl, vergrößert werden. Alternativ kann ein härteres Grundmaterial die Festigkeit des Ambosses erhöhen, aber dies führt möglicherweise dazu, daß auf erwünschte Eigenschaften des Edelstahls verzichtet werden muß. Deshalb besteht der Bedarf, einen Amboß herzustellen, der stärker und widerstandsfähiger gegen Biegung ist, während gleichzeitig die erwähnten Nachteile vermieden werden.
- Gemäß der vorliegenden Erfindung wird ein Amboßelement gemäß Anspruch 1 und eine Vorrichtung zum Anbringen von Befestigungselementen gemäß Anspruch 10 zur Verfügung gestellt.
- Es wird ein Amboßelement für die Verwendung in einer Vorrichtung zum Anbringen von mehreren chirurgischen Befestigungselementen an Körpergewebe zur Verfügung gestellt. Die Vorrichtung weist eine Einrichtung zum Halten von Befestigungselementen für das Tragen von mehreren chirurgischen Befestigungselementen in wenigstens zwei parallelen Reihen und eine Einrichtung auf, die auf eine vom Benutzer aufgebrachte Kraft anspricht und im wesentlichen entlang der Längsachse der Vorrichtung zum Austreiben der Befestigungselemente aus der Einrichtung zum Halten der Befestigungselemente in eine Oberfläche des länglichen Amboßelementes zum Schließen der Befestigungselemente beweglich ist. Die Reihen von Befestigungselementen sind mit der Längsachse der Vorrichtung ausgerichtet, und die Befestigungselemente werden regelmäßig aus dem Befestigungselementehalter in einer Richtung quer zu der Längsachse der Vorrichtung durch eine sich in Längsrichtung bewegende Nockenstange getrieben. Der Amboß ist ein längliches Element, das wenigstens eine von einer ersten und einer zweiten sich jeweils in Längsrichtung erstreckenden Wänden aufweist, die im wesentlichen zueinander quer angeordnete Ebenen festlegen und jeweils einen distalen Abschnitt und einen proximalen Abschnitt aufweisen, wobei der distale Abschnitt der ersten Wand eine Einrichtung zum Schließen der chirurgischen Befestigungselemente aufweist, und wobei die zweite Wand wenigstens eine längliche Vertiefung aufweist, die sich an dieser in Längsrichtung erstreckt.
- Ein alternatives aber auch optionales Merkmal zur Verhinderung einer Biegung des Ambosses, wenn die Vorrichtung an Körpergewebe geschlossen wird, ist ein Element zur Verhinderung von Durchbiegung, das seitlich von dem proximalen Abschnitt des Ambosses vorspringt.
- Fig. 1 ist eine perspektivische Explosionsansicht der Vorrichtung zum Anbringen von Befestigungselementen gemäß der vorliegenden Erfindung.
- Fig. 2 ist eine Aufrißansicht eines Abschnitts der Magazinanordnung.
- Fig. 3 ist eine perspektivische Explosionsansicht des Endes des Ambosses.
- Fig. 4 ist eine perspektivische Ansicht des Amboßelements gemäß der vorliegenden Erfindung.
- Fig. 5 ist eine Schnittansicht des Amboßelements, die das Element zur Verhinderung von Durchbiegung zeigt.
- Ein chirurgischer Klammerapparat, in dem der Amboß der vorliegenden Erfindung verwendet wird, ist im wesentlichen ähnlich aufgebaut wie der chirurgische Klammerapparat, der in dem US-Patent 5 014 899 offenbart ist, dessen Offenbarung hiermit durch Bezugnahme zum Gegenstand dieser Anmeldung gemacht wird.
- Wie Fig. 1 zeigt, weist die chirurgische Klammervorrichtung 100 der vorliegenden Erfindung einen ersten Körper 110 mit einer Magazinstütze 114, die einen distalen Vorsprung 111 zum Halten einer Magazinanordnung aufweist, eine Fingerauflage 151, einen feststehenden Griff 152, eine Schließstange 153, eine Seitenplatte 112 und eine Kerbe 113 in der Seitenplatte 112 auf. Ein zylindrischer Querträger 115 erstreckt sich seitlich über den ersten Körper in der Nähe des proximalen Endes der Vorrichtung. Ein zweiter Körper 120 weist zwei Bänder 121 für eine schwenkbare Verbindung mit dem ersten Körper 110, einen drehbaren Griff 150, eine Fingerauflage 151, und einen distalen Vorsprung 122 mit einer Amboßanordnung 200 auf. Die Bänder 121 bringen eine Hebelkraft auf die Schließstange 153 auf, um die Körper 110 und 120 für den Zusammenbau zu befestigen. Die Amboßanordnung 200 ist aus einer Platte mit Vertiefungen oder Mulden 230, wie deutlicher in Fig. 4 gezeigt, zum Umschlagen der Beine von metallischen Klammern ausgebildet. Alternativ kann die Amboßanordnung eine Einrichtung zum Halten von Reihen von Halteabschnitten von zweiteiligen chirurgischen Befestigungselementen für einen leichteren gegenseitigen Eingriff der Befestigungs- und Halteabschnitte der zweiteiligen Befestigungselemente aufweisen. Eine Spitze 124 ermöglicht, daß der distale Vorsprung 122 einfacher in Körpergewebe angeordnet werden kann. Der elastisch biegsame Arm 125 ist vorzugsweise an die Spitze 124 angebracht. Der biegsame Arm 125 kann einstückig mit der Spitze 124 ausgebildet sein, oder er kann getrennt angebracht ausgebildet sein.
- Die Vorrichtung 100 und ihre sich in Längsrichtung erstreckenden Teile können in der Vorstellung entlang ihrer Länge in drei Abschnitte geteilt werden: einen distalen Abschnitt 160, der sich in distaler Richtung über die Griffelemente 150 und 152 erstreckt, einen mittleren Abschnitt 161 in der Nähe der Bänder 121 und der Schließstange 153, wo während des Zusammenbaus der Vorrichtung die Hebelkräfte aufgebracht werden, und einen proximalen Abschnitt 162, der sich im wesentlichen proximal zu den Fingerauflagen 151 befindet.
- Eine Betätigungsanordnung 130 zum Austreiben der chirurgischen Befestigungselemente ist ein austauschbarer Einsatz, der eine Schiebeanordnung mit einem Schubknopf 131, Nockenstangen 132 und einer Messerklinge 133 aufweist. Die Betätigungsanordnung oder der Einsatz 130 weist ferner einen feststehenden Träger 134 zum Halten einer Magazinanordnung 135 auf. Eine Spitze 137 an dem distalen Ende der Magazinanordnung erleichtert die Anordnung von Körpergewebe zur Befestigung. Wie ferner Fig. 2 zeigt, weist die Magazinanordnung 135 Schiebeelemente 136 zum Ausschieben von chirurgischen Befestigungselementen aus ihren jeweiligen Schlitzen und zu einer Berührung mit dem Amboß zum Verschließen auf. Die Nockenstangen 132 und das Messer 133 sind an ihren proximalen Enden an einen Nockenstangenhalter 138 angebracht, der mit dem Schubknopf 131 verbunden ist und für eine Einrichtung zum Übertragen einer manuell aufgebrachten Kraft von dem Schubknopf 131 auf die Nockenstangen 132 sorgt.
- Bei der Verwendung wird der Einsatz 130 in den ersten Körper 110 geladen, und das Instrument wird dann derart zusammengebaut, daß das zu operierende Körpergewebe sich zwischen der Magazinanordnung 135 und dem Amboß 200 befindet. Das Messer 133 wird derart angeordnet, daß es sich gleichzeitig entlang eines Schlitzes 240 (vgl. Fig. 4) in dem Amboß 200 und eines Schlitzes 141 (Fig. 1) in der Magazinanordnung bewegen kann. Das Instrument wird dann durch ein Vorwärtsdrücken (d.h. in distaler Richtung) durch den Chirurgen auf den Schubknopf 131 abgefeuert.
- Wie wiederum Fig. 2 zeigt, werden dann die Nockenstangen 132 und das Messer 133 in distaler Richtung und längs entlang des Instruments in der durch den Pfeil A angedeuteten Richtung bewegt. Das Messer 133 bildet mittels seiner distalen Schneidkante 133a einen Einschnitt in dem (nicht gezeigten) Körpergewebe aus, und die Nockenstangen 132 treiben die Befestigungselementeschiebeeinrichtungen 136 in einer durch den Pfeil B angedeuteten Richtung, die quer zu der Richtung der Längsachse des Instruments verläuft. Die Schiebeeinrichtungen 136 wiederum treiben die Befestigungsmittel 140 aus ihren Schlitzen 139 und in die Mulden 230 in der Amboßplatte für das Umschlagen, wodurch das Gewebe zu beiden Seiten des Einschnitts befestigt wird. Wenn der Vorgang vollendet ist, kann der gebrauchte austauschbare Einsatz 130 entsorgt werden, und ein neuer kann in die Vorrichtung eingebaut werden.
- Fig. 3 zeigt die Amboßspitze 124 mit Zinken 124a und Feststellvorrichtungen 124b an den Außenseiten der Zinken.
- Die Feststellvorrichtungen 124b dienen dem Eingriff mit Seitenschlitzen 250 (vgl. Fig. 4) in dem Amboß 200.
- Wie Fig. 4 zeigt, ist der Amboß 200 vorzugsweise aus Edelstahlblech (305) mit einer Dicke von 1,73 bis etwa 1,83 mm (etwa 0,068 bis etwa 0,072 Inch) gefertigt. Eine Wand 265 legt eine ebene Gewebekontaktfläche fest und weist eine Einrichtung zum Schließen eines Befestigungselements auf, die wie gezeigt als Mulden 230 zum Schließen von Klammern ausgebildet sein kann, oder eine Einrichtung zum lösbaren Halten der Halteabschnitte von zweiteiligen chirurgischen Befestigungselementen sein kann. Die Wände 264 sind Seitenwände, welche die Wand 265 derart schneiden, daß Stege 266 (vgl. Fig. 5) festgelegt werden, und die Ebenen festlegen, die quer zu der Ebene der Wand 265 verlaufen. Der Amboß 200 weist eine längliche Vertiefung 260 auf, die sich entlang wenigstens einer und vorzugsweise beider Seiten 264 erstreckt, die an die Gewebekontaktfläche 265 anliegen und die Mulden 230 zum Verschließen der Klammern aufweisen. Die Vertiefung 260 vergrößert die Festigkeit des Ambosses, um die Durchbiegung während des Klemmens des Körpergewebes zwischen der Amboßbacke und der Klammerbacke und während des Abfeuerns der Befestigungselemente zu verringern. Die Vertiefung 260 ist der Darstellung halber an jeder Seitenwand 264 mittig angeordnet.
- Beispielhaft kann der Amboß 200 eine Länge L-1 von etwa 261 bis 266 mm (etwa 10,30 bis etwa 10,50 Inch), eine Höhe H-1 von etwa 8,88 bis 9,13 mm (etwa 0,35 bis etwa 0,36 Inch) und eine Breite W-1 von etwa 89 bis 91 mm (etwa 3,5 bis etwa 3,6 Inch) aufweisen. Die längliche Vertiefung 260 kann eine Länge L-3 von vorzugsweise 85% bis 90% der Länge L-1 des Ambosses, d.h. von etwa 231 bis 233 mm (9,1 bis 9,2 Inch) aufweisen. Alternativ kann sich die Vertiefung über die gesamte Länge des Ambosses erstrecken. Die Tiefe D (Fig. 5) der Vertiefung 260 beträgt vorzugsweise von etwa 0,51 bis etwa 0,76 mm (etwa 0,020 Inch bis etwa 0,030 Inch) 0 Die Höhe H-2 beträgt vorzugsweise zwischen etwa 2,16 bis etwa 2,28 mm (etwa 0,085 bis etwa 0, 09 Inch). Die Länge L-2 der Reihen von Mulden, welche die Klammern formen, kann bis zu 76 bis 89 mm (von 3, bis 3,5 Inch) sein.
- Diese Abmessungen dienen der Verdeutlichung und stellen die Größe einer einzigen Ausführungsform der vorliegenden Erfindung dar. Ambosse anderer Abmessungen fallen in den Bereich der vorliegenden Erfindung.
- Das proximale Ende 270 des Ambosses weist aufrechte Abschnitte 271 mit einer Kerbe 272 zur Aufnahme des Querträgers 115 in dem ersten Körper 110 auf, um einen sorgfältig ausgerichteten Eingriff des ersten und zweiten Körpers im zusammengebauten Zustand zu erleichtern.
- Der Amboß 200 der vorliegenden Erfindung weist ferner ein Element 280 zur Verhinderung einer Durchbiegung auf, das an dem proximalen Abschnitt 162 des Ambosses angeordnet ist. Das Element 280 zur Verhinderung einer Durchbiegung weist eine obere Anschlagfläche 281 auf und steht seitlich über die Seite des Ambosses hinaus vor. Bei einer Ausführungsform kann sich das Element 280 über einen Abstand W-2 von etwa 1,65 bis etwa 1,90 mm (etwa 0,065 Inch bis etwa 0,075 Inch) erstrecken. Die Anschlagfläche 281 kann wahlweise bündig mit der Gewebekontaktfläche 265 ausgebildet sein. Eine Kerbe 282 befindet sich neben dem Element 280 zur Verhinderung einer Durchbiegung.
- Wie die Fig. 1, 4 und 5 zeigen, weist die Magazinstütze 114 eine nach oben gehobene Seitenplatte 112 mit einer Kerbe 113 für einen zusammenwirkenden Eingriff mit dem Element 280 zur Verhinderung einer Durchbiegung auf. Wenn die Vorrichtung für den Gebrauch zusammengebaut wird, schlägt die Anschlagfläche 281 an die Unterseite der Kerbe 113 an, wodurch eine Biegung des Ambosses 200 verhindert wird. Das Element zur Verhinderung einer Durchbiegung ist für die Darstellung etwa 3,5 Inch von dem proximalen Ende 270 des Ambosses angeordnet, was in etwa der Mitte zwischen dem Mittelabschnitt 161 des Ambosses und dem proximalen Ende 270 entspricht.
- Wenn das Instrument durch den Zusammenbau der Körper 120 und 110 und das Drehen des Griffs 150 nach unten, um die Bänder 121 an die Schließstange 153 zu befestigen, geschlossen wird, berührt das Durchbiegungselement 280 den unteren Rand der Kerbe 113 und wirkt als ein Anschlag. Dies verhindert eine Biegung des proximalen Abschnitts des Ambosses 280, wodurch ermöglicht wird, daß eine größere Klammerkraft an dem distalen Ende ausgeübt wird. Zusätzlich werden durch die Wirkung eines Anschlags in dem proximalen Abschnitt des Ambosses die Klammer-Hebelkräfte, die durch die Bänder und die Schließstange 153 aufgebaut werden, in einem größeren Ausmaß auf den distalen Abschnitt der Vorrichtung gerichtet, bei dem das Gewebe geklammert wird, weil der proximale Abschnitt des Ambosses daran gehindert wird, sich zu biegen. Somit enthält die vorliegende Erfindung eine Einrichtung sowohl zur Verhinderung einer Biegung des Ambosses als auch zum Verstärken der Klammerkräfte auf dem Gewebe.
Claims (12)
1. Amboßelement (200) für die Verwendung in einer
Vorrichtung zum Anbringen von mehreren chirurgischen
Befestigungselementen an Körpergewebe, mit:
einem länglichen Element mit wenigstens jeweils einer
von einer ersten (265) und einer zweiten (264) sich
jeweils in Längsrichtung erstreckenden Wand, die im
wesentlichen zueinander quer verlaufende Ebenen
festlegen und jeweils einen distalen Abschnitt und einen
proximalen Abschnitt aufweisen, wobei der distale
Abschnitt der ersten Wand eine Einrichtung (230) zum
Schließen der chirurgischen Befestigungselemente
aufweist,
dadurch gekennzeichnet, daß
die zweite Wand wenigstens eine in Längsrichtung daran
ausgebildete längliche Vertiefung (260) aufweist.
2. Amboßelement nach Anspruch 1, wobei die Länge der
länglichen Vertiefung (260) etwa 85% bis etwa 90% der
Länge der zweiten Wand (264) beträgt.
3. Amboßelement nach Anspruch 1 oder 2, wobei die
Einrichtung zum Schließen der chirurgischen
Befestigungselemente mehrere Mulden (230) aufweist, die
in wenigstens zwei parallelen Reihen angeordnet sind.
4. Amboßelement nach Anspruch 1, 2 oder 3, das ferner einen
derart sich in Längsrichtung erstreckenden Schlitz (240)
in der ersten Wand aufweist, daß der Durchgang einer
Messerklinge (133) durch diesen ermöglicht wird.
5. Amboßelement nach einem der vorstehenden Ansprüche, das
ferner eine Anschlageinrichtung (280) zur Verhinderung
einer Biegung des proximalen Abschnitts des
Amboßelementes aufweist.
6. Amboßelement nach Anspruch 5, wobei die
Anschlageinrichtung ein Element (280) zur Verhinderung
einer Durchbiegung aufweist, das seitlich von dem
proximalen Abschnitt der zweiten Wand vorspringt und
eine Anschlagfläche (281) aufweist.
7. Amboßelement nach Anspruch 6, wobei die Anschlagfläche
in etwa bündig mit der ersten Wand des Amboßelements
ausgebildet ist.
8. Amboßelement nach einem der vorstehenden Ansprüche,
wobei das Amboßelement aus Edelstahl hergestellt ist.
9. Amboßelement nach einem der vorstehenden Ansprüche, das
in eine Vorrichtung zum Anbringen von mehreren
chirurgischen Befestigungselementen an Körpergewebe
eingebaut ist.
10. Vorrichtung zum Anbringen von mehreren chirurgischen
Befestigungselementen an Körpergewebe, mit:
einer Halteeinrichtung (135) für Befestigungselemente
zum Tragen von mehreren chirurgischen
Befestigungselementen (140) in wenigstens zwei
parallelen Reihen;
einer Einrichtung (130), die auf eine vom Benützer
aufgebrachte Kraft anspricht und im wesentlichen entlang
der Längsachse der Vorrichtung für das Austreiben der
Befestigungselemente aus der Einrichtung zum Halten der
Befestigungselemente in eine an einem länglichen
Amboßelement ausgebildete Fläche (230) zum Schließen der
Befestigungselemente bewegbar ist;
wobei das Amboßelement (200) wenigstens jeweils eine von
einer ersten (265) und einer zweiten (264) Wand
aufweist, die im wesentlichen quer zueinander
verlaufende Ebenen festlegen und jeweils einen distalen
Abschnitt und einen proximalen Abschnitt aufweisen,
wobei der distale Abschnitt der ersten Wand die Fläche
(230) zum Schließen der Befestigungselemente aufweist,
und
dadurch gekennzeichnet, daß
die zweite Wand wenigstens eine längliche Vertiefung
aufweist, die sich an dieser in Längsrichtung erstreckt.
11. Vorrichtung nach Anspruch 10, wobei das chirurgische
Befestigungselement (140) im wesentlichen U-förmige
verformbare Klammern aufweist, und die Fläche (230) zum
Schließen der Befestigungselemente mehrere, in
wenigstens zwei parallelen Reihen angeordnete
Vertiefungen zum Umschlagen der Klammern aufweist.
12. Vorrichtung nach Anspruch 10 oder 11, wobei die Reihen
von Befestigungselementen mit der Längsachse der
Vorrichtung fluchten, und wobei die Befestigungselemente
durch eine sich in Längsrichtung bewegende Nockenstange
(132) nacheinander aus dem Befestigungselementehalter in
einer Richtung quer zu der Längsachse der Vorrichtung
ausgetrieben werden.
