Description
- Gebiet der Erfindung
- Die vorliegende Erfindung bezieht sich im Allgemeinen auf eine Schlaferfassung, und insbesondere auf eine Schlaferfassung durch Einstellen eines Schlafschwellenwerts, der mit einem ersten schlafbezogenen Signal unter Verwendung eines zweiten schlafbezogenen Signals verbunden ist.
- Hintergrund der Erfindung
- Schlaf ist allgemein heilsam und erholsam für einen Patienten und übt einen großen Einfluss auf die Lebensqualität aus. Ein typischer Nachtschlaf für eine normale Person beginnt mit einer Schlafphase, die als ein SWS-Schlaf ("SWS": slow wave sleep) bekannt ist, gekennzeichnet durch eine niederfrequente Elektroenzephalogramm(EEG)-Aktivität. Wenn die Person einschläft, nimmt die Gehirnaktivität ab und es liegt ein fortschreitender Anstieg der Schlaftiefe vor. In ungefähr 90 Minuten Intervallen wird der Schlaf leichter und eine Schlafphase, die als REM-Schlaf ("REM": rapid eye movement) bekannt ist, beginnt. REM-Schlaf ist gekennzeichnet durch eine hochfrequente EEG-Aktivität, Anhäufungen von schnellen Augenbewegungen, Skelettmuskelatonie, und eine erhöhte autonome Aktivität.
- Es gibt typischerweise vier bis sechs REM-Perioden pro Nacht, mit einer erhöhten Dauer und Intensität sobald es Morgen wird. Während Träume entweder während des REM- oder des SWS-Schlafs auftreten können, variiert die Art der Träume abhängig von der Art von Schlaf. REM-Schlafträume neigen dazu, lebendiger und emotional intensiver als SWS-Schlafträume zu sein. Außerdem ändert sich die Aktivität des autonomen Nervensystems dramatisch, wenn REM-Schlaf beginnt.
- Bei Patienten mit respiratorischen oder Herzproblemen, kann das Gehirn während des Schlafs Atemprobleme, Myokard-Ischämie oder Arryhthmie verursachen. Obwohl REM-Schlaf eine notwendige Komponente von normalem Schlaf ist, können ernsthafte Konsequenzen mit z. B. der Erhöhung von autonomer Aktivität als auch den intensiven emotionalen Antworten, die die Träume bei Patienten mit kardiovaskulären Krankheiten oder respiratorischen Beschwerden begleiten, verbunden sein.
- Störungen des respiratorischen Systems während des Schlafs können die Bedingungen von Schlaf-Apnoe oder Schlaf-Hypopnoe beinhalten. Schlaf-Apnoe ist eine ernsthafte Atemstörung, die durch eine Atemwegblockierung, als blockierte Schlaf-Apnoe bezeichnet, oder eine Unordnung der zentralen Nervensystemsteuerung von Respiration, als zentrale Schlaf-Apnoe bezeichnet, verursacht wird. Ungeachtet der Art von Apnoe, hören Personen mit Schlaf-Apnoe während ihres Schlafs wiederholend auf, zu atmen, manchmal hunderte Male pro Nacht, und oft für eine Minute oder länger. Während sich Schlaf-Apnoe auf eine Beendigung des Atmens bezieht, ist Hypopnoe mit Perioden von abnormalem langsamen oder flachen Atmen verbunden. Mit jedem Auftreten von Apnoe oder Hypopnoe erregt die Person im Allgemeinen eine Wiederaufnahme von normalem Atmen. Als Ergebnis können Personen mit Schlaf-Apnoe oder -Hypopnoe einen Schlaf erleben, der durch häufige Erregungen fragmentiert ist.
- Eine angemessene Qualität und Quantität von Schlaf wird benötigt, um eine physiologische Homöostase aufrecht zu halten. Anhaltender Schlafentzug oder Perioden von hoch fragmentiertem Schlaf werden letztendlich ernsthafte Gesundheitskonsequenzen mit sich ziehen. Chronischer Schlafentzug kann mit verschiedenen Herz- oder respiratorischen Beschwerden verbunden sein, die die Gesundheit und die Qualität des Lebens eines Patienten beeinflussen.

- Die vorliegende Erfindung bezieht sich auf ein Erfassen von Schlaf.
- In einem Ausführungsbeispiel der Erfindung, wie in Anspruch 1 definiert, beinhaltet eine Vorrichtung zum Erfassen von Schlaf einen ersten Sensor zum Abtasten eines ersten schlafbezogenen Signals und einen zweiten Sensor zum Abtasten eines zweiten schlafbezogenen Signals, wobei die ersten und zweiten schlafbezogenen Signale Schlaf anzeigen. Ein Schlafdetektor, der mit dem ersten und dem zweiten Sensor gekoppelt ist, ist eingerichtet, einen dem ersten schlafbezogenen Signal zugehörigen Schlafschwellenwert unter Verwendung des zweiten schlafbezogenen Signals anzupassen. Der Schlafdetektor erfasst einen Schlafzustand durch Vergleichen des ersten schlafbezogenen Signals mit dem angepassten Schwellenwert. Eine Komponente von einem oder mehreren des Schlafdetektors, des ersten Sensors und des zweiten Sensors ist implantierbar.

Im Gegenteil, die Erfindung wird beabsichtigt, alle Modifikationen, Äquivalente und Alternativen, die in den Bereich der Erfindung, wie durch die angehängten Ansprüche definiert, fallen, abzudecken.
- Detaillierte Beschreibung von verschiedenen Ausführungsbeispielen
- In der folgenden Beschreibung der dargestellten Ausführungsbeispiele, werden Bezüge auf die begleitenden Zeichnungen gemacht, die einen Teil hiervon bilden, und in denen eine Darstellung von verschiedenen Ausführungsbeispielen gezeigt wird, durch die die Erfindung ausgeführt werden kann. Es ist zu verstehen, dass andere Ausführungsbeispiele verwendet werden können und dass strukturelle und funktionelle Änderungen gemacht werden können, ohne sich von dem Gebiet der vorliegenden Erfindung, die durch die Ansprüche definiert wird, zu entfernen.
- Eine angemessene Dauer und Qualität von Schlaf wird benötigt, um eine schlafbezogene Homöostase aufrecht zu halten. Anhaltender Schlafentzug oder Perioden von schlechter Schlafqualität werden letztendlich ernsthafte Gesundheitskonsequenzen haben. Um die Gründe für Schlafstörungen zu diagnostizieren, können Personen, die an Schlafbeschwerden leiden, eine oder mehrere Nächte in einem Schlaflabor verbringen. In einem Schlaflabor wird ein Patient typischerweise für eine Datenakquisition mit Instrumenten ausgerüstet und durch ausgebildetes Personal beobachtet. Eine Schlafbeurteilung in einer Laborumgebung stellt eine Anzahl von Hindernissen in einer Akquisition von einem akkuraten Bild eines typischen Schlafverhaltens eines Patienten dar. Z. B. verursacht das Verbringen einer Nacht in einem Schlaflabor bei einem Patienten typischerweise, eine Bedingung, die als "Erste-Nacht-Syndrom" bekannt ist, zu erfahren, das einen gestörten Schlaf während der ersten Nächte in einer unbekannten Umgebung einschließt. Außerdem kann ein Schlaf, während man verkabelt ist und beobachtet wird, nicht in einer realistischen Perspektive eines normalen Schlafmusters eines Patienten resultieren.
- Schlafqualitätsbeurteilungen hängen von einer Datenakquisition hinsichtlich eines typischen Schlafmusters eines Patienten ab. Ein anfänglicher Schritt zu einer Schlafqualitätsbeurteilung ist ein akkurates und zuverlässiges Verfahren zum Erkennen, dass ein Patient eingeschlafen ist. Ein Erfassen des Einbruchs, der Beendigung, der Dauer, der Phasen, und der Schlafqualität, die von einem Patienten erfahren wird, kann in Verbindung mit der Behandlung von verschiedenen Bedingungen verwendet werden. Z. B. kann eine Erfassung von Atembeschwerden während des Schlafs hilfreich sein, um geeignete Therapien für Patienten zu liefern, die an Schlafbeschwerden leiden, die von Schnarchen bis zu Schlaf-Apnoe reichen. Außerdem kann eine Hochrechnung von Schlafdaten über eine lange Zeit mit einer Anzahl und einer Schwere von Episoden über Atembeschwerden, erregten Episoden oder Perioden von gestörtem Schlaf einen Einblick in die emotionale und physische Gesundheit eines Patienten liefern. Z. B. kann eine Kenntnis von Schlafmustern eine Anzahl von Aspekten von einer Therapie eines Patienten mit einer kardialen oder respiratorischen Therapie beeinflussen.
- In dem Kontext einer Herzrhythmusverwaltungstherapie ("CRM": cardiac rhythm management) kann es z. B. vorteilhaft sein, die niedrigere Frequenzgrenze eines Schrittmachers basierend auf einer Erkennung von Schlaf- oder Nicht-Schlafzuständen zu regulieren. Eine Einstellung der niedrigeren Frequenzgrenze, um Schlafperioden entgegenzukommen, mit einer Verlängerung einer Batteriedauer der CRM-Vorrichtung, kann die Qualität des Schlafs des Patienten verbessern. Außerdem kann eine Arrhythmie-Therapie mit einer Schlaferkennung verbessert werden. Die Perioden einer Erregung von REM-Schlaf wurden mit einer erhöhten Wahrscheinlichkeit von Arrhythmie für Patienten mit Herzkrankheiten verbunden. Deshalb kann die Fähigkeit, Schlaf zu erkennen, die Fähigkeit, Arrhythmie, verbunden mit Schlaf, vorherzusagen und zu erfassen, verbessern, und eine Anti-Arrhythmie-Therapie während des Schlafs bereitstellen.
- Respiratorische Therapie kann auch durch ein Verfahren zum akkuraten Erkennen eines Schlafzustands verbessert werden. Schlaf-Apnoe-Behandlungen können positive Luftwegdruckvorrichtungen beinhalten, die den Patienten während des Schlafs mit einem konstanten oder einstellbaren Luftzug versorgen, periodische elektrische Stimulation des Nervus hypoglossus, um die oberen Atemwege zu öffnen, und eine Herzvorhof-Schrittübersteuerung, um Schlaf-Apnoe-Ereignisse zu unterdrücken oder den Patienten zu wecken, um ein apnoeisches Ereignis zu beenden. Jedes dieser Verfahren, als auch Verfahren zum Behandeln von respiratorischen Beschwerden können durch eine zuverlässige Erfassung, dass der Patient schläft, verbessert werden.
- Verschiedene Ausführungsbeispiele der Erfindung schließen eine Schlaferfassung unter Verwendung von mit einem Schlafzustand verbundenen Signalen ein. Ein Ausführungsbeispiel der Erfindung schließt ein Einstellen eines Schlafschwellenwerts, der mit einem ersten schlafbezogenen Signal unter Verwendung eines zweiten schlafbezogenen Signals verbunden ist, ein. Das erste schlafbezogene Signal wird mit dem eingestellten Schwellenwert verglichen und Schlaf wird, basierend auf dem Vergleich, erfasst. Zumindest eines von einem Abtasten der schlafbezogenen Signale, einem Vergleichen des ersten schlafbezogenen Signals mit dem Schlafschwellenwert und einem Erfassen von Schlaf wird zumindest teilweise implantierbar durchgeführt.
- Ein weiteres Ausführungsbeispiel der Erfindung schließt ein Definieren einer Beziehung zwischen zwei oder mehreren schlafbezogenen Signalen ein. Die Beziehung ist mit einer Schlaferfassung verbunden. Schlaf wird unter Verwendung der Beziehung erfasst. Ein Abtasten der schlafbezogenen Signale und/oder ein Erfassen von Schlaf wird zumindest teilweise implantierbar durchgeführt.
- Ein Definieren einer Beziehung beinhaltet z. B. ein Einrichten eines Schlafkriteriums, das mit zumindest einem der schlafbezogenen Signale verbunden ist. Das Kriterium kann z. B. ein Schwellenwert sein, oder ein anderer auf den Schlafzustand bezogener Index. Ein Erfassen von Schlaf beinhaltet ein Vergleichen des Schlafkriteriums mit dem Zustand von einem oder mehreren der schlafbezogenen Signale.
- Gemäß einem Ausführungsbeispiel der Erfindung können schlafbezogene Signale von externen oder implementierbaren Sensoren abgeleitet werden, und durch einen externen Schlafdetektor analysiert werden. Einige oder alle der Sensoren können Fernkommunikationsfähigkeiten, wie etwa einen drahtlosen Bluetooth-Kommunikationstransmitter einer Sendeempfangseinrichtung haben, um sie mit dem Schlafdetektor zu verbinden.
- Gemäß einem weiteren Ausführungsbeispiel der Erfindung können schlafbezogene Signale von externen oder implementierbaren Sensoren abgeleitet werden und durch eine implementierbare Vorrichtung analysiert werden. Der Schlafdetektor kann eine Komponente einer Vorrichtung sein, die auch andere Funktionen durchführt, wie etwa Herzschrittmacher oder Defibrillationsfunktionen. Einige oder alle der Sensoren können drahtlos mit der implementierbaren Vorrichtung durch z. B. Telemetrie gekoppelt sein.
- Gemäß einem Ausführungsbeispiel der vorliegenden Erfindung können Verfahren einer Schlaferfassung in einem implementierbaren Herzrhythmusverwaltungssystem (CRM) implementiert werden, das als eine Zweikammerschrittmachervorrichtung konfiguriert ist, die in mehreren aus dem Stand der Technik bekannten Schrittmachermodi betrieben werden kann. Die Systeme und Verfahren der vorliegenden Erfindung können auch in verschiedenen Arten von implementierbaren oder externen diagnostischen medizinischen Vorrichtungen implementiert werden, die z. B. Polysomnographievorrichtungen, Atmungsmonitore, und Herzmonitore beinhalten. Zusätzlich können die Systeme und Verfahren der vorliegenden Erfindung in einer Anzahl von implementierbaren oder externen therapeutischen medizinischen Vorrichtungen, wie etwa kontinuierlichen positiven Atemwegdruckvorrichtungen (CPAP) oder Nervus hypoglossus-Stimulatoren implementiert werden.

1 ist ein Blockdiagramm einer Schlaferfassungsvorrichtung100 , die verwendet werden kann, um Schlaf gemäß einem Ausführungsbeispiel der Erfindung zu erfassen. Die Schlaferfassungsvorrichtung beinhaltet eine Anzahl von Sensoren101 ,102 ,103 ,104 , die schlafbezogene, mit Schlaf verbundene Signale abtasten. Ein repräsentativer Satz von abgetasteten schlafbezogenen, mit Schlaf verbundenen Signalen, beinhaltet Körperbewegung, Herzfrequenz, QT-Intervall, Augenbewegung, Respirationsfrequenz, transthorakale Impedanz, Atemvolumen, Atmungsprotokollierung, Körperhaltung, Elektroenzephalogramm (EEG), Elektrokardiogramm (ECG), Elektrookulogramm (EOG), Elektromyogramm (EMG), Muskeltonus, Körpertemperatur, Tageszeit, historische Schlafzeiten, Blutdruck und Pulsoxymetrie. - Ein erstes von einem Schlaferfassungssensor
101 abgeleitetes schlafbezogenes Signal ist ein mit Schlaf verbundenes Signal, das mit einem Schlafschwellenwert zum Erfassen des Einbruchs und der Beendigung von Schlaf verglichen wird. Ein zweites von einem Schwellenwerteinstellungssensor102 abgeleitetes schlafbezogenes Signal wird verwendet, um den Schlafschwellenwert einzustellen. Obwohl ein Schlaferfassungssensor und ein Schwellenwerteinstellungssensor in1 gezeigt werden, kann irgendeine Anzahl von Schwellenwerten oder anderen Indizes entsprechend einer Anzahl von Schlaferfassungssensoren verwendet werden. Außerdem können Signale von irgendeiner Anzahl von Einstellungssensoren verwendet werden, um die Schwellenwerte oder Indizes einer Vielzahl von Schlaferfassungssignalen einzustellen. Zusätzliche, von Bestätigungssensoren103 ,104 abgeleitete schlafbezogene Signale können optional verwendet werden, um den Einbruch oder die Beendigung des Schlafzustands zu bestätigen. - Die von den Sensoren

Die Speicherschaltung132 kann verwendet werden, um Programmdaten zu speichern, um eine Schlaferfassung zu implementieren, um mit einer Schlaferfassung verbundene Parameter zu speichern, wie etwa einen Schlafschwellenwert oder um historische Daten bezüglich eines Schlafeinbruchs und eine Beendigung über einer ausgewählten Periode zu speichern. - Der Schlafdetektor
130 ist eingerichtet, das Niveau eines ersten schlafbezogenen Signals mit einem durch ein zweites schlafbezogenes Signal eingestellten Schlafschwellenwert zu vergleichen und einen Schlafeinbruch oder eine Beendigung basierend auf dem Vergleich zu bestimmen. Der Schlafdetektor130 kann einen oder mehrere mit einem oder mehreren schlafbezogenen Signalen verbundene Schwellenwerte oder Indizes verwenden. Zusätzlich kann der Schlafdetektor130 ein oder mehrere schlafbezogene Signale verwenden, um die Schlafschwellenwerte oder Indizes einzustellen. Außerdem kann der Schlafdetektor130 den Einbruch oder die Beendigung von Schlaf unter Verwendung einer zusätzlichen Anzahl von schlafbezogenen Signalen bestätigen. - Der Schlafdetektor
130 kann eine Ausgabeschaltung133 zum Kommunizieren von verschiedenen mit Schlaf verbundenen Signalen zu anderen Vorrichtungen, zu anderen Komponenten einer Schlaferfassungsvorrichtung, eine Datenspeichervorrichtung und/oder eine Anzeigevorrichtung beinhalten. Die mit Schlaf verbundenen Signale können z. B. ein Schlaferfassungssignal, mit einer Schlaferfassung verbundene Parameter, wie etwa einen Schlafschwellenwert und/oder für Schlaf relevante historische Daten beinhalten (z. B. historische Schlafzeitdaten oder eine Mittelung derselben die verwendet werden kann, um einen Schlafschwellenwert zu errichten). Der Schlafdetektor kann mit anderen Vorrichtungen über z. B. einen verdrahteten oder einen drahtlosen Kommunikationskanal kommunizieren. - Die Sensoren
101 ,102 ,103 ,104 können implantierbare Sensoren oder externe Sensoren umfassen. In einem Ausführungsbeispiel sind die Sensoren101 ,102 ,103 ,104 mit der Sensor-Antriebs-/Erfassungsschaltung110 und somit mit dem Schlafdetektor130 über eine verdrahtete Verbindung gekoppelt. In einem anderen Ausführungsbeispiel sind die Sensoren101 ,102 ,103 ,104 und eine Sensor-Antriebs-/Erfassungsschaltung110 in Abtastvorrichtungen beinhaltet, die drahtlose Kommunikationsfähigkeiten beinhalten, z. B. einen Bluetooth-Transmitter oder eine Sendeempfangseinrichtung, und können mit dem Schlafdetektor130 über einen drahtlosen Link gekoppelt sein. Der Schlafdetektor130 und/oder die Sensor-Antriebs-/Erfassungsschaltung110 kann in einer implementierbaren oder externen Vorrichtung beinhaltet sein. -
2 ist eine Teilansicht eines Ausführungsbeispiels einer implantierbaren medizinischen Vorrichtung, die für eine Schlaferfassung in Verbindung mit den Prinzipen der Erfindung verwendet werden kann. Die in2 dargestellte implementierbare Vorrichtung ist ein Herzrhythmusverwaltungssystem (CRM), das einen implementierbaren Schrittmacher200 , der elektrisch und physisch mit einem intrakardialen Kabelsystem202 gekoppelt ist, beinhaltet. Das intrakardiale Kabelsystem202 wird in einen menschlichen Körper implantiert, wobei Anteile des intrakardialen Kabelsystems202 in ein Herz201 eingeführt werden. Das intrakardiale Kabelsystem202 wird verwendet, um durch das Herz201 erzeugte elektrische Herzsignale zu erfassen und zu analysieren und eine elektrische Energie an das Herz201 unter vorbestimmten Bedingungen bereitzustellen, um Herzarrhythmien des Herzens201 zu behandeln. - Das in
2 dargestellte CRM200 ist eine Zweikammervorrichtung, die in der Lage ist, Signale von dem rechten Vorhof und dem rechten Ventrikel abzutasten und Schrittmacherimpulse an den rechten Vorhof und den rechten Ventrikel bereitzustellen. Schrittmacherimpulse mit einer niedrigen Energie können an das Herz geliefert werden, um z. B. den Herzschlag zu regulieren oder eine niedrigere Herzschlagrate aufrechtzuerhalten. In einer Konfiguration, die Kardioversions-/Defibrillationsfähigkeiten beinhalten, können Impulse mit einer hohen Energie auch an das Herz geliefert werden, falls eine Arrhythmie erfasst wird, die eine Kardioversion oder Defibrillation benötigt. - Das intrakardiale Kabelsystem
202 beinhaltet ein rechtes ventrikuläres Kabelsystem204 und ein rechtes atriales Kabelsystem205 . Das rechte ventrikuläre Kabelsystem204 beinhaltet eine "RV-Tip"-Schritt-/Abtastelektrode212 und eine oder mehrere Elektroden213 ,214 ,216 , die zum Messen einer transthorakalen Impedanz geeignet sind. In einer Anordnung sind die Impedanzabtast- und Antriebselektroden216 ,214 ,213 als Ringelektroden eingerichtet. Die Impedanzantriebselektrode213 kann z. B. in dem rechten Ventrikel218 angeordnet sein. Die Impedanzabtastelektrode214 kann in dem rechten Atrium220 angeordnet sein. Alternativ oder zusätzlich kann eine Impedanzabtastelektrode216 in dem höheren rechten Vorhof220 oder in der Nähe des rechten Vorhofs220 innerhalb der höheren Hohlvene222 angeordnet sein. - Eine Zweielektrodenimpedanzabtastkonfiguration ist auch möglich, wobei das rechte ventrikuläre Kabelsystem eine Impedanzantriebselektrode
213 und eine "Tip"-Elektrode212 beinhaltet. In dieser Konfiguration kann die "Tip"-Elektrode212 sowohl als die Impedanzabtastelektrode als auch als eine Herzabtast-/-schrittelektrode verwendet werden. Andere Orte und Kombinationen von Impedanzabtast- und Antriebselektroden sind auch möglich. - Das atriale Kabelsystem
205 beinhaltet eine "A-Tip"-Herzschritt-/-abtastelektrode256 . - In der Konfiguration von
2 ist das intrakardiale Kabelsystem202 innerhalb des Herzens201 positioniert, wobei sich ein Abschnitt des atrialen Kabelsystems205 in den rechten Vorhof220 erstreckt und Abschnitte des rechten ventrikulären Kabelsystems204 sich durch den rechten Vorhof220 in den rechten Ventrikel218 erstrecken. Die "A-Tip"-Elektrode256 befindet sich an einer geeigneten Stelle innerhalb des rechten Vorhofs220 zum Schrittsteuern des rechten Vorhofs220 und zum Abtasten der Herzaktivität in dem rechten Vorhof220 . Die "RV-Tip"-Elektrode212 befindet sich an geeigneten Stellen innerhalb des rechten Ventrikels218 zum Schrittsteuern des rechten Ventrikels218 und zum Abtasten der Herzaktivität in dem rechten Ventrikel218 . - Zusätzliche Konfigurationen von Abtast-, Schrittsteuerungs- und Defibrillationselektroden können in dem intrakardialen Kabelsystem beinhaltet sein, um verschiedene Abtast-, Schrittsteuerungs- und Defibrillationsfähigkeiten von multiplen Herzkammern zu ermöglichen. In einer Konfiguration können die rechten ventrikulären und rechten atrialen Kabel zusätzliche Elektroden z. B. für ein bipolares Abtasten und/oder Schrittsteuern beinhalten. Weiterhin können die rechten ventrikulären und rechten atrialen Kabel auch zusätzliche Elektroden für eine Kardioversion oder Defibrillation beinhalten.
- In anderen Konfigurationen kann das intrakardiale Kabelsystem nur ein einziges Kabel haben, wobei die Elektroden in dem rechten Vorhof oder dem rechten Ventrikel angeordnet sind, um eine Schlaferfassung und ein Einzelkammer-Herzschrittsteuern zu implementieren. In anderen Ausführungsbeispielen kann das intrakardiale Kabelsystem endokardiale Kabel beinhalten, die in den Koronarsinus und koronaren Venen verlaufen, um die distale(n) Elektrode(n) zu lokalisieren, die sich neben dem linken Ventrikel oder dem rechten Vorhof befinden.
- Andere nach dem Stand der Technik bekannte intrakardiale Kabel- und Elektrodenanordnungen und Konfigurationen sind auch möglich und werden als innerhalb des Bereichs des vorliegenden Systems liegend betrachtet, das durch die Ansprüche definiert wird.
- Nun wird bezogen auf
3 ein Blockdiagramm eines Ausführungsbeispiels eines CRM-Systems300 gezeigt, das als ein Schrittmacher und geeignet für ein Implementieren einer Schlaferfassungsmethodik der vorliegenden Erfindung eingerichtet ist.3 zeigt die CRM300 , die in funktionale Blöcke eingeteilt ist. Es wird von Fachleuten verstanden werden, dass viele mögliche Konfigurationen existieren, in denen diese funktionalen Blöcke angeordnet und implementiert werden können. Das in3 dargestellte Beispiel ist eine mögliche funktionale Anordnung. Die CRM300 beinhaltet eine Schlaferfassungsschaltung320 zum Empfangen von schlafbezogenen Signalen und zum Erfassen von Schlaf gemäß einem Ausführungsbeispiel der Erfindung. - In einem Ausführungsbeispiel ist die Schlaferfassungsschaltung
320 als Teil der CRM-Schaltung310 einbezogen, die von einem Gehäuse390 umhüllt und hermetisch versiegelt zum Implantieren in einen menschlichen Körper geeignet ist. Energie wird der CRM300 durch eine elektrochemische Batterieenergieversorgung312 , die sich innerhalb der CRM300 befindet, zugeführt. Ein Verbindungsblock (nicht gezeigt) ist zusätzlich an das CRM-Gehäuse390 angebracht, um für das physische und elektrische Anbringen der intrakardialen Kabelsystemleiter an die CRM-Schaltung310 zu sorgen. - Die CRM-Schaltung
310 kann als ein programmierbares mikroprozessorbasiertes System konfiguriert werden, mit einer Schaltung zum Erfassen von Schlaf zusätzlich zu einem Bereitstellen einer Schrittsteuerungstherapie für das Herz. Herzsignale können durch die Detektorschaltung360 erfasst werden und an das Schrittmachersteuerungssystem350 geliefert werden. Schrittimpulse, die durch die Schrittmachersteuerung350 gesteuert werden und durch den Impulsgenerator340 erzeugt werden, werden an das Herz geliefert um verschiedene Arrhythmien des Herzens zu behandeln. - Die Speicherschaltung
316 kann Parameter für verschiedene Vorrichtungsoperationen speichern, die in eine Schlaferfassung und/oder kardiale Schrittsteuerung und Abtastung einbezogen sind. Die Speicherschaltung316 kann auch Daten speichern, die schlafbezogene Signale anzeigen, die durch Komponenten der CRM-Schaltung310 empfangen wurden, wie etwa die Impedanzantriebs-/-abtastschaltung330 , das Herzsignalerfassungssystem360 , und der Schwingungssensor335 . - Die Schlaferfassungsschaltung
320 empfängt Signale, die von dem Herzsignalerfassungssystem360 , der Impedanzantriebs-/-erfassungsschaltung330 und dem Schwingungssensor335 abgeleitet sind, um Operationen durchzuführen, die ein Erfassen eines Schlafeinbruchs und einer Beendigung gemäß den Prinzipien der vorliegenden Erfindung einbeziehen. Die Datenverlaufsspeicherung318 kann mit der Schlaferfassungsschaltung320 zum Speichern von historischen schlafbezogenen Daten gekoppelt sein. Solche Daten können an eine externe Programmierereinheit380 übertragen und für verschiedene diagnostische Zwecke wie benötigt oder gewünscht verwendet werden. - Die Telemetrieschaltung
314 ist mit der CRM-Schaltung310 gekoppelt, um die CRM300 zu befähigen, mit einer externen Programmierereinheit380 zu kommunizieren. In einem Ausführungsbeispiel verwenden die Telemetrieschaltung314 und die Programmierereinheit380 eine "Wire-Loop"-Antenne und einen Hochfrequenztelemetrielink, um Signale und Daten zwischen der Programmierereinheit380 und der Telemetrieschaltung314 zu empfangen und zu übertragen. Auf diese Weise werden Programmierbefehle und Daten zwischen der CRM-Schaltung310 und der Programmierereinheit380 während und nach einer Implantierung übertragen. - Die Programmierbefehle erlauben einem Arzt, verschiedene Parameter, die durch die CRM verwendet wurden, einzustellen oder zu modifizieren. Diese Parameter können ein Einstellen von Schlaferfassungsparametern für eine Verwendung während einer Schlaferfassung beinhalten, wie etwa, welche schlafbezogenen Signale für eine Schlaferfassung und ein Schwellenwerteinstellen zu verwenden sind und die anfänglichen Schlaferfassungsschwellenwerte. Zusätzlich kann das CRM-System
300 an die Programmierereinheit380 gespeicherte Daten in Bezug auf abgetastete Schlafperioden herunterladen, die z. B. die Menge der Schlafzeiten, die Tageszeit, zu der Schlafperioden auftraten, historische Daten über Schlafzeiten, und die Anzahl von Erregungen während der Schlafperioden beinhalten. - Die mit einer Patientenaktivität und -haltung verbundenen Signale können durch die Verwendung eines innerhalb des Gehäuses
390 der CRM300 positionierten Schwingungssensors335 erfasst werden. Der Schwingungssensor antwortet auf eine Patientenaktivität und das Schwingungssensorsignal kann mit einem Aktivitätsniveau, einer Arbeitslast und/oder einer Haltung korreliert werden. Signale, die von dem Schwingungssensor335 abgeleitet werden, werden mit der Schlaferfassungsschaltung320 gekoppelt und können auch z. B. durch die Schrittmacherschaltung zum Implementieren einer frequenzadaptiven Schrittsteuerungsvorgehensweise verwendet werden. - Die Impedanzabtastelektrode
214 , die Impedanzantriebselektrode213 , und die Impedanzantriebs-/-erfassungsschaltung330 werden verwendet, um ein Spannungssignal mit Bezug auf die transthorakale Impedanz zu erfassen. Die transthorakale Impedanzmessung kann verwendet werden, um verschiedene mit Respiration verbundene Parameter zu berechnen. Unter der Steuerung der Schlaferfassungsschaltung320 erzeugt die Impedanzantriebsschaltung332 einen Strom, der durch das Blut zwischen der Impedanzantriebselektrode213 und der "Can"-Elektrode309 fließt. Die Spannung an der Impedanzabtastelektrode214 relativ zu der "Can"-Elektrode309 ändert sich, wenn sich die transthorakale Impedanz ändert. Das zwischen der Impedanzabtastelektrode214 und der "Can"-Elektrode309 erzeugte Spannungssignal wird durch den Impedanzabtastverstärker334 , der sich innerhalb der Impedanzantriebs-/-erfassungsschaltung330 befindet, erfasst, und wird an die Schlaferfassungsschaltung320 für eine weitere Verarbeitung geliefert. - Das bei der Impedanzabtastelektrode
214 entwickelte Spannungssignal, das in4 dargestellt wird, ist proportional zu der transthorakalen Impedanz, wobei sich die Impedanz während einer respiratorischen Inspiration erhöht und während einer respiratorischen Expiration verringert. Der Spitzenübergang der Impedanzmessung, die in4 dargestellt wird, ist proportional zu dem Betrag von inhalierter Luft während eines Atemzugs, der als das Atemvolumen gekennzeichnet ist. Die Impedanzmessung kann weiterverarbeitet werden, um das Atemvolumen zu bestimmen, entsprechend dem Volumen von während eines Atmens bewegter Luft, oder eine Atmung entsprechend dem Betrag von bewegter Luft pro Minute zu protokollieren. - Zusätzlich zu Impedanz- und Schwingungssensoren können Herzsignale, die eine Herzfrequenz oder andere Herzfunktionen anzeigen, auch in Verbindung mit einer Schlaferfassung verwendet werden. Mit Rückbezug auf
3 werden Herzsignale durch eine Verwendung der "RV-Tip"- und "RA-Tip"-Abtastelektroden212 ,256 abgetastet. Insbesondere kann das rechte Ventrikelsignal als eine Spannung, die zwischen der "RV-Tip"-Elektrode212 und der "Can"-Elektrode309 entwickelt wird, erfasst werden. Rechtsventrikel-Herzsignale werden abgetastet und durch einen Rechtsventrikel-V-Abtastverstärker362 verstärkt, der sich in dem Erfassungssystem360 befindet. Die Ausgabe des Rechtsventrikel-V-Abtastverstärkers362 kann zum Beispiel mit einem Signalprozessor und einem A/D-Wandler innerhalb des Erfassungssystems360 gekoppelt werden. Die verarbeiteten Rechtsventrikel-Signale können an die Schrittmachersteuerung350 und die Schlaferfassungsschaltung320 geliefert werden. - Rechtsvorhof-Herzsignale werden abgetastet und durch einen Rechtsatrialen-A-Abtastverstärker
364 , der sich in dem Erfassungssystem360 befindet, verstärkt. Die Ausgabe des Rechtsvorhof-A-Abtastverstärkers364 kann durch eine Signalverarbeitungsschaltung verarbeitet und durch die Schrittmachersteuerung350 und die Schlaferfassungsschaltung320 empfangen werden. - Die Schrittmachersteuerung
350 teilt der Impulsgeneratorschaltung340 Schrittsteuerungssignale zum Übermitteln von Schrittstimulationsimpulsen an jeweils die "RV-Tip"- und "RA-Tip"-Elektroden220 und256 gemäß einer vorbestimmten Schrittvorgehensweise unter geeigneten Bedingungen mit. -
5 stellt ein Verfahren einer Schlaferfassung gemäß Prinzipien der Erfindung dar. Ein mit einem ersten schlafbezogenen Signal verbundener Schlafschwellenwert wird errichtet. Der Schlafschwellenwert kann z. B. aus klinischen Daten eines Schlafschwellenwertes in Verbindung mit einem Schlaf, der unter Verwendung einer Gruppe von Subjekten erfasst wird, bestimmt werden. Der Schlafschwellenwert kann auch unter Verwendung von historischen Daten, die von dem bestimmten Patienten genommen wurden, für den die Schlafbedingung zu erfassen ist, bestimmt werden. Ein Verlauf von Schlafzeiten eines gegebenen Patienten kann z. B. gespeichert werden, und ein Schlafschwellenwert kann unter Verwendung von Daten in Verbindung mit dem Schlafverlauf des Patienten entwickelt werden. - Das erste schlafbezogene Signal wird abgetastet
510 . Ein zweites schlafbezogenes mit Schlaf verbundenes Signal wird abgetastet520 . Die ersten und zweiten schlafbezogenen Signale können z. B. von in dem Patienten implantierten, extern an den Patienten angebrachten oder sich in der Nähe des Patienten befindenden Sensoren abgetastet werden. Die ersten und zweiten schlafbezogenen Signale können irgendwelche mit dem Zustand des Schlafs verbundenen Signale sein, wie etwa die repräsentativen schlafbezogenen mit Schlaf verbundenen Signale, wie vorstehend aufgelistet. - Der für das erste schlafbezogene Signal errichtete Schlafschwellenwert wird unter Verwendung des zweiten schlafbezogenen Signals
530 eingestellt. Falls zum Beispiel das zweite schlafbezogene Signal ein hohes Aktivitätsniveau anzeigt, das mit einem Schlafzustand inkompatibel ist, kann der Schlafschwellenwert des ersten schlafbezogenen Signals nach unten eingestellt werden, um ein Abtasten eines verringerten Niveaus des ersten schlafbezogenen Signals zu benötigen, bevor ein Schlafzustand erfasst wird. - Falls das erste schlafbezogene Signal mit Schlaf gemäß dem eingestellten Schlafschwellenwert
540 konsistent ist, wird ein Schlafzustand550 erfasst. Falls das erste schlafbezogene Signal mit Schlaf unter Verwendung des eingestellten Schlafschwellenwerts nicht konsistent ist, wird fortgesetzt, die ersten und zweiten schlafbezogenen Signale abzutasten510 ,520 und den Schwellenwert einzustellen530 , bis eine Bedingung eines Schlafes erfasst wird550 . - In einem weiteren Ausführungsbeispiel der Erfindung, das in
6 dargestellt wird, werden ein Schwingungssensor und ein Atmungsprotokolliersensor als erste und zweite mit Schlaf verbundenen Signale verwendet. Ein vorläufiger Schwingungssensor-Signalschlafschwellenwert wird bestimmt610 . Der vorläufige Schlafschwellenwert kann z. B. aus klinischen Daten, die aus einer Gruppe von Subjekten genommen wurden oder aus historischen Daten, die von dem Patienten über eine Zeitperiode genommen wurden, bestimmt werden. - Das Aktivitätsniveau des Patienten wird unter Verwendung eines Schwingungssensors
620 wie vorstehend beschrieben beobachtet, der in einen implantierbaren Herzschrittmacher einbezogen werden kann. Alternativ kann der Schwingungssensor extern an den Patienten angebracht werden. Das Atmungsprotokolliersignal (MV) des Patienten wird beobachtet625 . Das MV-Signal kann zum Beispiel unter Verwendung des vorstehend beschriebenen transthorakalen Impedanzverfahrens unter Verwendung einer implantierbaren kardialen Vorrichtung erhalten werden. Andere Verfahren eines Bestimmens des MV-Signals sind auch möglich und werden als innerhalb des Gebiets dieser Erfindung liegend betrachtet. - In diesem Beispiel repräsentiert das Schwingungssensorsignal das mit dem Schlafschwellenwert verbundene Schlaferfassungssignal. Das MV-Signal ist das Schwellenwerteinstellungssignal, das verwendet wird, um den Schlafschwellenwert einzustellen. Eine Herzfrequenz wird in diesem Beispiel beobachtet
630 , um ein Schlafbestätigungssignal bereitzustellen. - Eine Schwellenwerteinstellung kann durch Verwenden des MV-Signals des Patienten durchgeführt werden, um den Schwingungssensor-Schlafschwellenwert zu mäßigen. Falls das MV-Signal des Patienten relativ zu einem erwarteten mit Schlaf verbundenen MV-Niveau niedrig ist, wird der Schwingungssensor-Schlafschwellenwert erhöht. Falls in ähnlicher Weise das MV-Signalniveau des Patienten relativ zu einem erwarteten mit Schlaf verbundenen MV-Niveau hoch ist, wird der Schwingungssensor-Schlafschwellenwert verringert. Wenn das MV-Niveau des Patienten hoch ist, ist somit weniger Aktivität nötig, um eine Bestimmung durchzuführen, dass der Patient schläft. Wenn umgekehrt das MV-Niveau des Patienten relativ gering ist, kann ein höheres Aktivitätsniveau in einer Erfassung von Schlaf resultieren. Die Verwendung von zwei schlafbezogenen Signalen, um einen Schlafzustand zu bestimmen, verbessert die Genauigkeit einer Schlaferfassung im Vergleich zu vorherigen Verfahren, die nur ein schlafbezogenes Signal verwenden, um zu bestimmen, dass ein Patient schläft.
- Verschiedene Signalverarbeitungstechniken können betrieben werden, um die Rohsensorsignale zu verarbeiten. Ein gleitender Mittelwert einer Vielzahl von Abtastpunkten von jedem schlafbezogenen Signal kann z. B. berechnet werden und als das schlafbezogene Signal verwendet werden. Weiterhin können die schlafbezogenen Signale gefiltert und/oder digitalisiert werden. Falls das MV-Signal relativ zu einem erwarteten mit Schlaf verbundenen MV-Niveau hoch ist
635 , wird der Schwingungssensor-Schlafschwellenwert verringert640 . Falls das MV-Signal relativ zu einem erwarteten mit Schlaf verbundenen MV-Niveau niedrig ist635 , wird der Schwingungssensor-Schlafschwellenwert erhöht645 . - Falls das abgetastete Schwingungssensorsignal niedriger oder gleich dem eingestellten Schlafschwellenwert ist
650 , und falls der Patient gegenwärtig nicht in einem Schlafzustand ist665 , dann wird die Herzfrequenz des Patienten überprüft680 , um den Schlafzustand zu bestätigen. Falls die Herzfrequenz des Patienten mit Schlaf kompatibel ist680 , dann wird ein Schlafeinbruch bestimmt690 . Falls die Herzfrequenz des Patienten mit Schlaf inkompatibel ist, dann wird ein Beobachten der schlafbezogenen Signale des Patienten fortgesetzt. - Falls das Schwingungssensorsignal niedriger oder gleich dem eingestellten Schlafschwellenwert ist
650 , und falls der Patient gegenwärtig in einem Schlafzustand ist665 , dann wird ein fortsetzender Schlafzustand bestimmt und ein Beobachten der schlafbezogenen Signale des Patienten wird fortgesetzt, bis eine Schlafbeendigung auftritt. - Falls das Schwingungssensorsignal größer als der eingestellte Schlafschwellenwert ist
650 und der Patient gegenwärtig nicht in einem Schlafzustand ist660 , dann wird ein Beobachten der schlafbezogenen Signale des Patienten fortgesetzt, bis ein Schlafeinbruch erfasst wird690 . Falls das Schwingungssensorsignal größer als der eingestellte Schlafschwellenwert ist650 , und der Patient gegenwärtig in einem Schlafzustand ist660 , dann wird eine Schlafbeendigung erfasst670 . - Die Kurven von
7 bis9 stellen die Einstellung des Schwingungssensor-Schlafschwellenwerts unter Verwendung des MV-Signals dar. Die Beziehung zwischen einer Patientenaktivität und dem Schwingungssensor und der MV-Signale wird über eine Zeitperiode hochgerechnet, um relative Signalniveaus in Verbindung mit einem Schlafzustand zu bestimmen.7A stellt eine Aktivität dar, wie durch das Schwingungssensorsignal angezeigt. Die Herzfrequenz des Patienten für dieselbe Periode ist in7B dargestellt. Das Schwingungssensorsignal stellt eine Schlafperiode in Verbindung mit einem relativ niedrigen Aktivitätsniveau dar, das kurz vor 23 Uhr beginnt und sich bis 6 Uhr fortsetzt. Die Herzfrequenz nähert sich ungefähr dem Aktivitätsniveau an, das durch den Schwingungssensor angezeigt wird, das eine ähnliche Periode einer geringen Herzfrequenz, die Schlaf entspricht, darstellt. Die Schwingungssensor-Hochrechnungen werden verwendet, um einen Schwellenwert für eine Schlaferfassung zu errichten. -
8 ist eine Kurve einer Grundlinie eines Hochrechnens für ein MV-Signal. Historische Daten einer Atmungsprotokollierung eines Patienten sind über eine achtmonatige Periode dargestellt. Die MV-Signal-Hochrechnungsdaten werden verwendet, um das mit Schlaf verbundene MV-Signalniveau zu bestimmen. In diesem Beispiel zeigt ein zusammengesetztes MV-Signal unter Verwendung der historischen Daten eine grobe sinusoidale Form mit den relativ niedrigen MV-Niveaus dar, die ungefähr während der Perioden von 21 Uhr bis 8 Uhr auftreten. Die niedrigen MV-Niveaus sind mit Schlafperioden verbunden. Das mit Schlaf verbundene MV-Signalniveau wird verwendet, um eine Schlafschwellenwerteinstellung zu implementieren. -
9 stellt eine Einstellung des Schwingungssensor-Schlafschwellenwerts unter Verwendung des MV-Signals dar. Der anfängliche Schlafschwellenwert910 wird unter Verwendung der Grundlinien-Schwingungssensorsignaldaten, die wie vorstehend diskutiert erhalten werden, errichtet. Falls das MV-Signal des Patienten relativ zu einem erwarteten mit Schlaf verbundenen MV-Niveau niedrig ist, wird der Schwingungssensor-Schlafschwellenwert erhöht920 . Falls das MV-Signalniveau des Patienten relativ zu einem erwarteten mit Schlaf verbundenen MV-Niveau hoch ist, wird der Schwingungssensor-Schlafschwellenwert verringert930 . Wenn das MV-Niveau des Patienten hoch ist, wird weniger Aktivität, die durch den Schwingungssensor erfasst wird, benötigt, um die Bestimmung zu machen, dass der Patient schläft. Falls jedoch das MV-Niveau des Patienten relativ niedrig ist, kann ein höheres Aktivitätsniveau in einer Erfassung von Schlaf resultieren. Die Verwendung von zwei schlafbezogenen Signalen, um einen Schlafschwellenwert zum Bestimmen eines Schlafzustands einzustellen, verbessert die Genauigkeit einer Schlaferfassung gegenüber vorherigen Verfahren. - Zusätzliche schlafbezogene Signale können abgetastet werden und verwendet werden, um den Schlaferfassungsmechanismus wie vorstehend beschrieben zu verbessern. Ein Haltungssensor kann z. B. verwendet werden, um die Haltung des Patienten zu erfassen und verwendet werden, um Schlaf zu bestätigen. Falls der Haltungssensor eine vertikale Haltung anzeigt, kann der Haltungssensor dann verwendet werden, um eine Bestimmung von Schlaf unter Verwendung der Schlaferfassungs- und -schwellenwerteinstellungssignale aufzuheben. Andere Signale können auch in Verbindung mit einer Schlafbestimmung oder -bestätigung verwendet werden, die den repräsentativen Satz von schlafbezogenen mit Schlaf verbundenen Signalen, wie vorstehend angezeigt, beinhalten.
- Verschiedene Modifikationen und Zusätze können für die bevorzugten Ausführungsbeispiele, die vorstehend diskutiert wurden, gemacht werden, ohne von dem Gebiet der vorliegenden Erfindung abzuweichen. Dementsprechend soll das Gebiet der vorliegenden Erfindung durch die bestimmten Ausführungsbeispiele, wie vorstehend beschrieben, nicht limitiert, sondern nur durch die Ansprüche, wie nachstehend beschrieben, definiert werden.
Claims (28)
- Schlaferfassungsvorrichtung (
100 ) mit: einem ersten Sensor (101 ), der eingerichtet ist, ein erstes schlafbezogenes Signal abzutasten; einem zweiten Sensor (102 ), der eingerichtet ist, ein zweites schlafbezogenes Signal abzutasten; und einem Schlafdetektor (130 ), der mit dem ersten und dem zweiten Sensor gekoppelt ist, dadurch gekennzeichnet, dass der Schlafdetektor eingerichtet ist, einen dem ersten schlafbezogenen Signal zugehörigen Schlafschwellenwert unter Verwendung des zweiten schlafbezogenen Signals anzupassen, und einen Schlafzustand durch Vergleichen des ersten schlafbezogenen Signals mit dem angepassten Schwellenwert zu erfassen, und wobei der erste Sensor, der zweite Sensor oder der Schlafdetektor eine implantierbare Komponente aufweist. - Vorrichtung gemäß Anspruch 1, wobei der Schlafschwellenwert basierend auf klinischen Daten ermittelt wird.
- Vorrichtung gemäß Anspruch 1, wobei der Schlafdetektor weiterhin eingerichtet ist, den Schlafschwellenwert unter Verwendung des ersten schlafbezogenen Signals zu ermitteln.
- Vorrichtung gemäß Anspruch 1, wobei der Schlafdetektor eingerichtet ist, einen Schlafeinbruch zu erfassen, wenn das erste schlafbezogene Signal unter den angepassten Schwellenwert fällt, und eine Schlafende zu erfassen, wenn das erste schlafbezogene Signal über den angepassten Schwellenwert ansteigt.
- Vorrichtung gemäß Anspruch 1, wobei das erste schlafbezogene Signal mit einer Patientenaktivität in Beziehung steht.
- Vorrichtung gemäß Anspruch 1, wobei das erste schlafbezogene Signal ein kardiales Signal aufweist.
- Vorrichtung gemäß Anspruch 1, wobei das erste schlafbezogene Signal ein Respirationssignal aufweist.
- Vorrichtung gemäß Anspruch 1, wobei das erste schlafbezogene Signal ein Schwingungssensorsignal aufweist.
- Vorrichtung gemäß Anspruch 1, wobei das erste schlafbezogene Signal aus einem implantierbaren Sensor stammt.
- Vorrichtung gemäß Anspruch 1, wobei das erste schlafbezogene Signal von einem externen Sensor stammt.
- Vorrichtung gemäß Anspruch 1, wobei das zweite schlafbezogene Signal ein kardiales Signal aufweist.
- Vorrichtung gemäß Anspruch 1, wobei das zweite schlafbezogene Signal ein Respirationssignal aufweist.
- Vorrichtung gemäß Anspruch 1, wobei das zweite schlafbezogene Signal ein transthorales Impedanzsignal aufweist.
- Vorrichtung gemäß Anspruch 1, wobei das zweite schlafbezogene Signal ein Atmungsprotokolliersignal aufweist.
- Vorrichtung gemäß Anspruch 1, wobei das zweite schlafbezogene Signal aus einem implantierbaren Sensor stammt.
- Vorrichtung gemäß Anspruch 1, wobei das zweite schlafbezogene Signal von einem externen Sensor stammt.
- Vorrichtung gemäß Anspruch 1, wobei der Schlafdetektor (
130 ) eine implantierbare Vorrichtung aufweist. - Vorrichtung gemäß Anspruch 1, wobei der Schlafdetektor (
130 ) eine externe Vorrichtung aufweist. - Vorrichtung gemäß Anspruch 1, wobei der Schlafdetektor (
130 ) als eine Komponente eines Pulsgenerators eingerichtet ist. - Vorrichtung gemäß Anspruch 1, wobei der Schlafdetektor (
130 ) als eine Komponente einer therapeutischen Vorrichtung eingerichtet ist. - Vorrichtung gemäß Anspruch 1, wobei der Schlafdetektor (
130 ) als ein Herzschrittmacher (200 ) eingerichtet ist. - Vorrichtung gemäß Anspruch 1, wobei der Schlafdetektor (
130 ) als eine Komponente einer Patientenüberwachungseinrichtung eingerichtet ist. - Vorrichtung gemäß Anspruch 1, weiterhin mit: einem dritten Sensor, um ein drittes schlafbezogenes Signal abzutasten; und wobei der Schlafdetektor weiterhin eingerichtet ist, das dritte schlafbezogene Signal mit einem Schwellenwert zu vergleichen und einen Schlafzustand basierend auf dem Vergleich zu bestätigen.
- Vorrichtung gemäß Anspruch 23, wobei das dritte schlafbezogene Signal ein kardiales Signal aufweist.
- Vorrichtung gemäß Anspruch 23, wobei das dritte schlafbezogene Signal ein Respirationssignal aufweist.
- Vorrichtung gemäß Anspruch 23, wobei das dritte schlafbezogene Signal ein Körpertemperatursignal aufweist.
- Vorrichtung gemäß Anspruch 23, wobei das dritte schlafbezogene Signal ein Blutdruckssignal aufweist.
- Vorrichtung gemäß Anspruch 1, wobei der Schlafschwellenwert durch Verwenden von Daten, die mit historischen Schlafzeiten verknüpft sind, ermittelt wird.
