- UNTERSTÜTZUNG DURCH DIE REGIERUNG
- Die Erfindung wurde ganz oder teilweise durch Zuschüsse der Defense Advanced Research Projects Agency (N65236-00-1-5434) sowie durch die NIH-Zuschüsse
GM58484 GM37631 - HINTERGRUND DER ERFINDUNG
- Ester der alpha-Ketoalkansäuren wie z.B. Ethylpyruvat sind für die Verwendung bei der Behandlung von Reperfusionsschäden (WO01/24793), Entzündungen (WO02/074301) und Nierenversagen WO02/081020) beschrieben worden. Trotz der Akzeptanz der Ester der alpha-Ketoalkansäuren als wirksame Therapeutika sind die Ester der alpha-Ketoalkansäuren typischerweise in wässrigen Lösungen nicht stabil, wahrscheinlich in Folge einer Hydrolyse der Ester. So kann z.B. binnen vier Stunden ein steiler Abfall des pH-Werts von 6 auf etwa 2 auftreten, wenn solche Ester in wässrigen Lösungen gelöst werden. Solche Lösungen sind für eine Verabreichung an Patienten ungeeignet, wenn sie nicht unmittelbar nach dem Vermischen eingesetzt werden. Es ist aussichtslos, das vollständige Potential von Estern der alpha-Ketoalkansäuren als Therapeutika zu nutzen, ohne dass Formulierungen entwickelt werden, in welchen der Ester stabil ist.
- ZUSAMMENFASSUNG DER ERFINDUNG
- Es ist nun entdeckt worden, dass Ester der alpha-Ketoalkansäuren in Lactat enthaltenden wässrigen Formulierungen stabil bleiben. Beispielsweise zeigte eine ausgewogene Ringer-Lactatlösung mit 1 Gew.-%, 5 Gew.-% und 10 Gew.-% E thylpyruvat über einen Zeitraum von 48 Stunden einen relativ geringen pH-Abfall (Beispiel 1). Auf der Grundlage dieser Entdeckung werden hier nun neue pharmazeutische Formulierungen mit Estern der alpha-Ketoalkansäuren und Verfahren zum Einsatz derselben in der Therapie offenbart.
- Die offenbarte Erfindung ist einer pharmazeutische Zusammensetzung mit einem Ester oder Amid einer alpha-Ketoalkansäure und Milchsäure oder einem pharmazeutisch verträglichen Salz der Milchsäure (d.h. Lactat).
- Die pharmazeutischen Zusammensetzungen der vorliegenden Erfindung bieten im Vergleich mit vorher bekannten Formulierungen solcher Verbindungen einige Vorteile, einschließlich einer großen Stabilität der Ester der alpha Ketoalkansäuren. Sie sind zur Verwendung in Menschen für einen Zeitraum von mindestens 48 Stunden nach dem Vermischen geeignet.
- GENAUE BESCHREIBUNG DER ERFINDUNG
- Die vorliegende Erfindung ist eine pharmakologische Zusammensetzung mit einer wässrigen Lösung eines Esters von alpha Ketoalkansäuren oder eines Amids von alpha Ketoalkansäuren.
- In einem Aspekt der Erfindung ist der in der offenbarten Formulierung verwendete Ester der alpha Ketoalkansäure z.B. ein Ester einer geradkettigen oder verzweigtkettigen C3-C8-alpha Ketoalkansäure. Beispiele sind Ester der alpha-Ketobutansäure, alpha-Ketopentansäure, alpha-Keto-3-methylbutansäure, alpha-Keto-4-methylpentansäure oder der alpha-Keto-hexansäure. Brenztraubensäure ist bevorzugt. Für die Esterposition im Molekül sind verschiedene Gruppen geeignet, z.B. Alkyl (vorzugsweise ein C1-C3-Alkyl), Aralkyl, Alkoxy-alkyl oder Carbalkoxyalkyl. Spezielle Beispiele sind Ethyl, Propyl, Butyl, Carbmethoxy-methyl (-CH2COOCH3), Carbethoxymethyl (-CH2COOCH2CH3), Acetoxymethyl (-CH2OC(O)CH3), Carbmethoxyethyl (-CH2CH2COOCH3), Carbethoxyethyl -CH2CH2COOCH2CH3), Metboxymethyl (-CH2OCH3) und Ethoxymethyl (-CH2OCH2CH3). Ethylester sind bevorzugt. Thiolester (wo z.B. der Thiolanteil Cystein oder Homocystein ist) gehören ebenfalls dazu. Andere zur Esterbildung mit alpha Ketoalkansäuren geeignete Gruppen sind Dihydroxyacetonester der Formel in welcher R1 Pyruvyl ist und R2 für H, Pyruvyl oder ein C1-C3-Acyl wie z.B. Acetyl oder Propionyl steht;
und Monosaccharidester wie z.B. Ribosylester worin R jeweils unabhängig H, Pyruvyl oder ein C1-C3-Acyl wie z.B. Acetyl oder Propionyl ist und mindestens ein R Pyruvyl ist,
oder Glucosylester worin R jeweils unabhängig H, Pyruvyl oder ein C1-C3-Acyl wie z.B. Acetyl oder Propionyl ist und mindestens ein R Pyruvyl ist. - In einer bevorzugten Ausführungsform umfasst die offenbarte pharmazeutische Zusammensetzung Ethylpyruvat, Propylpyravat, Butylpyruvat, Carbmethoxymethyl pyruvat, Carbethoxymethylpyruvat, Acetoxymethylpyruvat, Carbmethoxyethylpyruvat, Carbethoxyethylpyruvat, Methoxymethylpyruvat und Ethoxymethylpyruvat. Andere bevorzugte Beispiele sind Ethyl-alpha-ketobutyrat, Ethyl-alphaketopentanoat, Ethyl-alpha-keto-3-methylbutyrat, Ethyl-alpha-keto-4-methyl pentanoat oder Ethyl-alpha-ketohexanoat. Ethylpyruvat ist mehr bevorzugt.
- Geeignete Amide der alpha-Ketoalkansäuren für die Verwendung in den offenbarten Formulierungen sind Verbindungen mit den folgenden Strukturformeln: RCOCONR1R2.
- R ist eine Alkylgruppe; R1 und R2 sind unabhängig -H, Alkyl, Aralkyl, Alkoxyalkyl, Carboxyalkyl oder -CHR3COOH; und R3 ist die Seitenkette eine natürlich vorkommenden Aminosäure. Das Amid einer alpha-Ketoalkansäure ist vorzugsweise ein Pyruvamid.
- Geeignete Alkylgruppen sind geradkettige oder verzweigte C1-C8-Alkylgruppen, vorzugsweise geradkettige C1-C6-Alkylgruppen.
- Geeignete Arylgruppen sind carbocyclische (z.B. Phenyl und Naphthyl) und heterocyclische aromatische Gruppen (z.B. Furanyl und Thiophenyl), vorzugsweise Phenyl.
- Eine Alkoxygruppe ist -OR4, in der R4 eine wie oben definierte Alkylgruppe ist. Eine Alkoxyalkylgruppe ist eine mit -OR4 substituierte Alkylgruppe.
- Eine Aralkylgruppe ist -XY, in welcher X eine Alkylgruppe und Y eine Arylgruppe sind und beide wie oben definiert sind.
- Eine Carboxyalkylgruppe ist eine mit -COOH substituierte Alkylgruppe. Eine Carbalkoxyalkylgruppe ist eine mit einem Ester substituierte Alkylgruppe. Spezielle Beispiele für Estersubstituenten sind Ethyl, Propyl, Butyl, Carbmethoxymethyl, Carbethoxymethyl, Acetoxymethyl, Carbmethoxyethyl, Carbethoxyethyl, Methoxymethyl und Ethoxymethyl.. Ethylester sind bevorzugt.
- Die Zusammensetzungen der vorliegenden Erfindung können pharmazeutisch verträgliche Salze der Milchsäure in einer Konzentration von ca. 1 mM bis ca. 100 mM aufweisen. Beispiele für pharmazeutisch verträgliche Salze der Milchsäure sind, jedoch nicht ausschließlich Lithiumlactat, Natriumlactat, Kaliumlactat, Ammoniumlactat, Calciumlactat und Magnesiumlactat. In einer bevorzugten Ausführungsform werden Natrium- oder Kaliumlactat verwendet.
- Die pharmazeutischen Zusammensetzungen der vorliegenden Erfindung können ferner eine oder mehrere pharmazeutisch verträgliche Salze von zweiwertigen Kationen wie Ca2+ oder Mg2++ und einwertigen Kationen wie Li+, Na+, K+, NH4 + und dergl. aufweisen. Natrium, Kalium und Calcium sind bevorzugt. Die Konzentration eines jeden Salzes kann von 0 bis ca. 250 mM, vorzugsweise von ca. 10mM bis ca. 250 mM variieren. Die Salze sind vorzugsweise Natriumchlorid, Kaliumchlorid und Calciumchlorid. In einer bevorzugten Ausführungsform reicht die Menge eines jeden Salzes oder einer Kombination der Salze aus, um die Zusammensetzung isotonisch zu machen (mit der physiologischen Osmolarität des Blutplasmas).
- Der Ester der alpha-Ketoalkansäure (z.B. Pyruvat) liegt in der pharmazeutischen Formulierung vorzugsweise in einer Konzentration von ca. 0,1 bis ca. 10 Gew.-% vor, vorzugsweise mit 2 bis ca. 5 Gew.-%, mehr bevorzugt mit 2,5 bis 3,5 Gew.-%.
- In einer bevorzugten Ausführungsform umfasst die pharmazeutisch Zusammensetzung der vorliegenden Erfindung:
- (a) ungefähr 0,1 bis ungefähr 10 Gew.-% eines Esters einer alpha-Ketoalkansäure (z.B. Ethylpyruvat); und
- (b) Natrium- oder Kaliumlactat in einer Konzentration von ungefähr 1 mM bis ungefähr 100 mM.
- In bevorzugten Ausführungsformen umfassen die Zusammensetzungen der vorliegenden Erfindung ferner:
- (c) ungefähr 10 mM bis ungefähr 250 mM NaCl;
- (d) ungefähr 0,1 mM bis ungefähr 25 mM KCl;
- (e) ungefähr 0,1 mM bis ungefähr 25 mM CaCl2.
- In einer bevorzugten Ausführungsform umfasst die Zusammensetzung der vorliegenden Erfindung:
- (a) ungefähr 2 bis ungefähr 5 Gew.-% Ethylpyruvat;
- (b) ungefähr 50 mM bis ungefähr 150 mM NaCl;
- (c) ungefähr 1 mM bis ungefähr 8 mM KCl;
- (d) ungefähr 1 mM bis ungefähr 5 mM CaCl2; und
- (e) ungefähr 10 mM bis ungefähr 40 mM Natriumlactat,
- Ein bevorzugtes Beispiel für eine Zusammensetzung der vorliegenden Erfindung ist eine Ringer-Lactatlösung, welche beispielsweise Kaliumchlorid (0 mM bis ungefähr 4 mM), Natriumchlorid (ungefähr 100 mM bis ungefähr 156 mM); Calciumchlorid (ungefähr 2,5 mM bis ungefähr 3,0 mM) und Natriumlactat (ungefähr 25 mM bis ungefähr 30 mM) wie hier beschrieben enthält.
- Die pharmazeutische Formulierung der vorliegenden Erfindung kann zur Behandlung und/oder Besserung von Krankheiten wie Nierenversagen und Ileus eingesetzt werden. Der hier verwendete Ausdruck "Ileus" ist ein teilweiser oder vollständiger nicht mechanischer Verschluss des Dünn- und/oder Dickdarms. Ein I leus tritt auf, wenn die Peristaltik, die rhythmische Kontraktion, welche Material durch die Eingeweide bewegt, aufhört. Ein Ileus kann beispielsweise durch Manipulation des Darms während einer Bauchoperation, durch eine Entzündung des Bauchfells oder durch Verabreichung von Narkotika oder Chemotherapeutika hervorgerufen werden.
- Die pharmazeutischen Zusammensetzungen der vorliegenden Erfindung können ferner zur Wiederbelebung und Reanimation von Säugern wie z.B. des Menschen vor, während und nach einer Mesenterialischämie, einem Mesenterialthrombus oder dem Verschluss einer Mesenterialvene, der Operation eines Aortenaneurysmas, dem Bypass einer Koronararterie, der operativen Behandlung eines Arterienverschlusses, eines hämorrhagischen Schocks und der Erhaltung oder Transplantation von Organen verwendet werden. Der hier verwendete Begriff "Ischämie" ist definiert als über den Blutfluss erfolgende Unterbrechung der Sauerstoffzufuhr an ein Organ oder Organteil.
- Die pharmazeutischen Zusammensetzungen der vorliegenden Erfindung lassen sich ferner zur Behandlung und/oder Linderung Cytokin-vermittelter Entzündungen einsetzen. Solche Störungen sind, jedoch nicht ausschließlich, lokale und systemische Entzündungen, eine entzündliche Darmerkrankung (Morbus Crohn und Colitis ulcerosa), rheumatische Arhritis, Asthma, Sepsis, septischer Schock, Hautentzündungen wie Psoriasis und Ekzeme, rheumatische Arthritis, rheumatische Spondylitis, Osteoarthritis, Gichtarthritis und andere Arthritisbeschwerden, endotoxischer Schock, Gram-negativer Schock, das toxische Schocksyndrom, eine ARDS-Schocklunge, zerebrale Malaria, chronische Lungenentzündung, Steinstaublunge, Lungensarkoidose, die Knochenresorptionskrankheit, ein Reperfusionsschaden, eine Transplantat-Wirt-Abstoßung, eine Allotransplantatabstoßung, Fieber und infektionsbedingter Muskelschmerz, eine Auszehrung nach Infektion oder in Folge einer bösartigen Geschwulst oder von AIDS, der AIDS-related-Complex, eine Keloid- und Narbenbildung.
- Die in einer pharmazeutischen Zusammensetzung der vorliegenden Erfindung einzusetzende genaue Dosis hängt von dem Weg der Verabreichung und dem Ernst der Umstände ab und sollte sich nach dem Urteil des Arztes und der Verfas sung des Patienten richten. Wirksame Dosen sollten aus Dosis-Wirkungs-Kurven extrapoliert werden, die aus in vitro- oder Tiermodellsystemen gewonnen wurden.
- Die pharmazeutischen Zusammensetzungen der Erfindung lassen sich in Abhängigkeit von dem Mittel, der Krankheit oder den zu behandelnden Bedingungen auf verschiedenen Wegen verabreichen, z.B. oral, mit der Nahrung, topisch, intravenös, intramuskulär oder mittels Inhalation (z.B. intrabronchiale, intranasale oder orale Inhalation, intranasale Tropfen), indem eingefahrene Verfahren für physiologisch verträgliche inerte Trägersubstanzen eingesetzt werden. Andere geeignete Verfahren zur Verabreichung können auch nachfüllbare oder bioabbaubare Vorrichtungen sowie polymere Vorrichtungen für eine langsame Freisetzung umfassen. Beispielsweise können die therapeutischen Zusammensetzungen in einer Trägerformulierung zur Freisetzung unter Verwendung eines bioabbaubaren biokompatiblen Polymers verabreicht werden oder mittels punktueller Ausschüttung unter Verwendung von Mizellen, Gelen, Liposomen oder einer Pufferlösung. Vorzugsweise wird die pharmazeutische Zusammensetzung als Infusionslösung mit einer Konzentration von z.B. 10 mM bis 200 mM, vorzugsweise 20 mM bis 90 mM des aktiven Mittels mit einer Geschwindigkeit von 1 mg/kg Körpergewicht/Tag bis zu 200 mg/kg Körpergewicht/Tag wie beschrieben in einer Pufferlösung verabreicht. Mehr bevorzugt wird die pharmazeutische Zusammensetzung als Infusionslösung mit einer Konzentration von etwa 28 mM des aktiven Mittels in einer Dosis von 100 mg/kg Körpergewicht/Tag bis zu 150 mg/kg Körpergewicht/Tag einer alpha-Ketoalkansäure in einer Pufferlösung verabreicht. In Bolusform kann das aktive Mittel mit einer ähnlichen Dosis verabreicht werden, z.B. mit 1 mg/kg Körpergewicht/Tag bis zu 200 mg/kg Körpergewicht/Tag des aktiven Mittels, wobei die Dosierung in Aliquots unterteilt wird und 1 bis 4 Mal täglich ausgeschüttet wird (bis zu einer Gesamtdosierung von 1 mg/kg Körpergewicht/Tag bis zu 200 mg/kg Körper-gewicht/Tag), wobei die Konzentration des aktiven Mittels entsprechend eingestellt wird.
- BEISPIELE
- Beispiel 1 Ethylpyruvat ist in Ringer-Lactatlösung achtundvierzig Stunden lang stabil
- Es wurden Formulierungen mit Ethylpyruvat in verschiedenen Konzentrationen (5, 10 und 10 Gew.-%) hergestellt, indem das Ethylpyruvyt entweder in im Handel erhältlicher Ringer-Lactatlösung (Kaliumchlorid (0 bis ca. 4 mM), Natriumchlorid (ca. 100 bis ca. 156 mM), Calciumchlorid (ca. 2,5 mM bis ca. 3,0 mM) und Natriumlactat (ca. 25 mM bis ca. 30 mM)) oder in einer Ringer-ähnlichen Lösung (Kaliumchlorid (0 bis ca. 4 mM), Natriumchlorid (ca. 100 bis ca. 156 mM) und Calciumchlorid (ca. 2,5 mM bis ca. 3,0 mM) oder in IntralipidTM (1000 ml enthalten: 100–300 g gereinigtes Sojaöl, 12 g gereinigte Phospholipide aus Eiern, 22 g wasserfreies Glycerin, mit Wasser für Injektionszwecke zu 1000 ml aufgefüllt. Der pH-Wert wurde mit Natriumhydroxid auf ein pH von etwa 8 eingestellt. Die Osmolarität betrug ca. 300 mosm/kg Wasser) gelöst wurde. Die Azidität jeder Lösung wurde bei 0, 5, 30 und 60 Minuten 2, 4, 24 und 48 Stunden nach dem Auflösen des Ethylpyruvats gemessen. Die Ergebnisse sind in Tabelle 1 wiedegegeben. Wie durch die pH-Werte angezeigt, bleibt Ethylpyruvat in Ringer-Lactatlösung stabil, nicht jedoch die Ringer-ähnliche Lösung oder Intralipid.

- Pharmazeutische Zusammensetzung, umfassend: (a) einen Ester einer alpha Ketoalkansäure oder ein Amid einer alpha Ketoalkansäure und (b) Milchsäure oder ein pharmazeutisch akzeptables Milchsäuresalz, das in wässriger Lösung aufgelöst ist.
- Pharmazeutische Zusammensetzung, umfassend: (a) einen Ester einer alpha Ketoalkansäure und (b) Milchsäure oder ein pharmazeutisch akzeptables Milchsäuresalz, das in wässriger Lösung aufgelöst ist.
- Pharmazeutische Zusammensetzung nach Anspruch 2, dadurch gekennzeichnet, dass der Ester einer alpha Ketoalkansäure ein Ester einer geradkettigen C3-C8 alpha Ketoalkansäure ist.
- Pharmazeutische Zusammensetzung nach Anspruch 3, dadurch gekennzeichnet, dass der Ester einer alpha Ketoalkansäure ein Pyrotraubensäureester ist.
- Pharmazeutische Zusammensetzung nach Anspruch 3, dadurch gekennzeichnet, dass der Ester einer alpha Ketoalkansäure ein Ethylester ist.
- Pharmazeutische Zusammensetzung nach Anspruch 3, dadurch gekennzeichnet, dass der Ester einer alpha Ketoalkansäure ein Alkyl-, Aralkyl-, Alkoxyalky- oder Carbalkoxyalkylester ist.
- Pharmazeutische Zusammensetzung nach Anspruch 4, dadurch gekennzeichnet, dass der Ester der Pyrotraubeusäure aus der Gruppe ausgewählt ist, die aus Ethylpyruvat, Propylpyruvat, Carbmethoxymethylpyruvat, Acetoxymethylpyruvat, Carbethoxyethylpyruvat und Ethoxymethylpyruvat besteht.
- Pharmazeutische Zusammensetzung nach Anspruch 5, dadurch gekennzeichnet, dass der Ester einer alpha Ketoalkansäure aus der Gruppe ausgewählt ist, die aus Ethyl-alpha-ketobutyrat, Ethyl-alpha-keto-pentanoat und Ethyl-alpha-ketohexanoat besteht.
- Pharmazeutische Zusammensetzung nach Anspruch 3, dadurch gekennzeichnet, dass der alpha Ketoalkansäureester ein Glycerylester, ein Dihydroxyacetonester, ein Monosaccharidester oder ein Thiolester ist.
- Pharmazeutische Zusammensetzung nach Anspruch 6, dadurch gekennzeichnet, dass der Ester einer alpha Ketoalkansäure ein C1-C3-Alkylester ist.
- Pharmazeutische Zusammensetzung nach Anspruch 10, dadurch gekennzeichnet, dass die pharmazeutische Zusammensetzung einen Ester einer alpha Ketoalkansäure und Natrium- oder Kaliumlactat umfasst.
- Pharmazeutische Zusammensetzung nach Anspruch 11, dadurch gekennzeichnet, dass sie weiterhin ein pharmazeutisch akzeptables Salz von Na+, K+, NH4 +, Mg2+, Ca2+ oder einer Kombination davon umfasst.
- Pharmazeutische Zusammensetzung nach Anspruch 12, dadurch gekennzeichnet, dass der Ester einer alpha Ketoalkansäure Ethylpyruvat ist.
- Pharmazeutische Zusammensetzung nach Anspruch 12, dadurch gekennzeichnet, dass die pharmazeutische akzeptablen Salze NaCl, KCl, CaCl2 oder Kombinationen davon sind.
- Wässrige pharmazeutische Zusammensetzung, umfassend: (a) 0,1 Gew.-% bis 1,0 Gew.-% Ethylpyruvat und (b) Natrium- oder Kaliumlactat mit einer Konzentration von ungefähr 1 mM bis ungefähr 100 mM.
- Pharmazeutische Zusammensetzung nach Anspruch 15, dadurch gekennzeichnet, dass sie weiterhin umfasst: (c) ungefähr 10 mM bis ungefähr 250 mM NaCl; (d) ungefähr 0,1 mM bis ungefähr 25 mM KCl; (e) ungefähr 0,1 mM bis ungefähr 25 mM Ca Cl2.
- Pharmazeutische Zusammensetzung, umfassend: (a) ungefähr 2 Gew.-% bis ungefähr 5 Gew.-% Ethylpyruvat; (b) ungefähr 50 bis ungefähr 150 mM NaCl; (f) ungefähr 1 bis ungefähr 8 mM KCl; (g) ungefähr 1 bis ungefähr 5 mM CaCl2 und (h) ungefähr 10 bis ungefähr 40 mM Natriumlactat, wobei die Bestandteile (a) bis (e) in einer wässrigen Lösung aufgelöst sind.
- Pharmazeutische Zusammensetzung, die ungefähr 0,1 Gew.-% bis ungefähr 10 Gew.-% Ethylpyruvat in Ringers Lactatlösung umfasst.
- Pharmazeutische Zusammensetzung nach Anspruch 18, dadurch gekennzeichnet, dass die Konzentration von Ethylpyruvat von ungefähr 2,5 Gew.-% bis ungefähr 3,5 Gew.-% ist.
