Description
- Diese Erfindung betrifft Stents, die in den Körper eingepflanzt werden.
- Stents werden in Hohlräumen des Körpers wie Blutgefäßen verwendet; um sie in einem offenen Zustand zu halten. Üblicherweise wird der Stent durch ein Katheter in den Hohlraum eingebracht, der den Stent während des Einbringens durch die Haut und der Beförderung durch einen Hohlraum, z.B. ein Gefäß, zum gewünschten Ort der Einpflanzung in einer raumsparenden Form hält. Beim Erreichen dieser Stelle wird der Stent gedehnt, sodass er an der Innenwand des Gefäßes liegt. Der Katheter wird dann entfernt, wobei der Stent im Körper belassen wird.
- Zur Dehnung des Stents kann gehören, dass er radial nach außen gedehnt wird, beispielsweise durch das Aufblasen eines Ballons, der vom Katheter getragen wird, oder der Stent kann einer der selbstausdehnenden Typen sein, von denen verschiedene im Fachgebiet bekannt sind. Beispielsweise kann der Stent aus einem Gedächtnismetall wie NITINOL hergestellt sein, das sich in ausgewählten Temperaturbereichen selbst ausdehnt.
- Die US-Patentschrift 5,741,327 offenbart einen Stent für chirurgische Zwecke, der gebogene Abschnitte aufweist, die durch zwei verschiedene Arten von Verknüpfungselementen verknüpft sind.
- Es ist offensichtlich, dass der Stent, je flexibler er während des Einbringens durch die Haut ist, umso leichter um Biegungen und Kurven im Hohlraum oder Gefäß geführt werden kann, um den Ort der Einpflanzung zu erreichen.
- Die Erfindung stellt eine Stentausgestaltung aus einzelnen ringförmigen Elementen bereit, die so geformt sind, dass sie mit oder ohne Überschneidung und ohne Überkreuzungspunkte locker ineinander passen oder ineinander greifen, wodurch der Stent eine ungewöhnliche Beweglichkeit aufweist.
- Vorzugsweise sind die ringförmigen Elemente mehrere benachbarte, unabhängige, gewundene Ringe, die Abschnitte aufweisen, die seitlich mit Zwischenraum angeordnet und um 180° versetzt zueinander sind, um eine gestufte Abfolge um jeden Ring bereitzustellen, wobei jeder Abschnitt einen breiten und einen engen Zwischenbereich mit einem Satz von Abschnitten auf einer Seite aufweist, der in einen benachbarten Satz von Abschnitten auf einer Seite eines benachbarten gewundenen Rings eingepasst ist, um die Ringe miteinander gegen eine Trennung in Längsrichtung zu verbinden, sodass ein beweglicher walzenförmiger oder röhrenförmiger Aufbau entsteht.
- Gemäß einem weiteren Gesichtspunkt der Erfindung können die Ecken der ringförmigen Ringe einen Winkel mit der Mittellinie der röhrenförmigen Stentanordnung bilden, anstatt nach ihr ausgerichtet zu sein, wie beim Stand der Technik. Wird der Stent mit diesen winkligen außermittigen Seitenwänden hergestellt, greifen die Ringe gegen eine Trennung in radialer Richtung ineinander, wenn sie bezüglich der Mittellinie des Stents radial nach innen oder außen bewegt werden.

- Mit Bezug auf die
1 und2 ist eine Ausführungsform eines erfindungsgemäßen Stents gezeigt und ganz allgemein mit10 bezeichnet. Der Stent10 besitzt eine walzenförmige oder röhrenförmige Anordnung, wie in den Seitenansichten der4 und5 zu sehen ist, und umfasst eine Vielzahl ringförmiger gewundener Abschnitte12 , die über die Länge des Stentkörpers ineinander passen. - Jeder ringförmige Abschnitt
12 weist eine im Allgemeinen gewundene Anordnung auf, mit breiten seitlichen Endbereichen14 und schmalen seitlichen Bereichen16 , die einen Abstand zu den Enden14 aufweisen, wobei die breiten Bereiche eines Rings wie dargestellt in den breiten Bereich eines benachbarten Rings passen, um eine Trennung der gewundenen Ringe in Längsrichtung zu verhindern. Eine Trennung in radiale Richtung während der Handhabung und des Einbringens des Stents wird dadurch verhindert, dass die ineinander passenden Ringe auf einem Schaft (nicht gezeigt) getragen werden, wie durch den Einbringungskatheter an sich bereitgestellt sein kann. - Die Stents der Erfindung können aus Draht oder Ähnlichem hergestellt sein. Insbesondere sind sie aus einer Metallwalze mit einem Laser auf eine gewünschte Anordnung geschnitten.
- Früher waren Stents aus Streben mit runden, quadratischen, trapezförmigen, länglichen oder anderen Querschnitten aufgebaut. Der Querschnitt der Strebe war über den gesamten Stent ziemlich gleich bleibend. Die Strebenform wurde durch den Ausgangswerkstoff, den Schneid- oder Formgebungsvorgang und die Oberflächenbehandlung vorgegeben. Entwickler von Stents können aus einer Vielzahl von Formen wählen, um dem Stent einzigartige mechanische Eigenschaften zu verleihen, jedoch weist der Stent überall den gleichen allgemeinen Querschnitt auf, d.h. wenn die Strebe eine längliche Form aufweist, bleibt sie länglich, wenn die Strebe breiter am Außendurchmesser als am Innendurchmesser ist, behält sie diesen Querschnitt über den gesamten Stent. Dies trifft zu, selbst wenn sich die Querschnittsfläche der Strebe in einigen Bereichen des Stents verändert.
- Eine weitere Verbesserung ist in den
3 ,4 und5 zu sehen, in denen die Abschnitte18 aus einem Rohr oder einem Blech geformt sein können, deren Seitenwände außermittig in Bezug auf die Mittellinie des Stents liegen, sodass die Abschnitte ineinander greifen, wenn sie radial nach innen oder außen bewegt werden. Zwei Möglichkeiten sind dargestellt.4 zeigt die Streben, die mit Laser außeraxial der Mitte des Stents geschnitten wurden, alle in der gleichen Richtung.5 zeigt eine zweite Möglichkeit, bei der die außeraxiale Schneidrichtung um den Stent herum wechselt. - Das Ergebnis ist ein Stent, der Elemente umfasst, die mehrere verschiedene Querschnittsformen von Streben in einer vorher festgelegten Art aufweisen. Dieses Merkmal ist wünschenswert, da jede Querschnittsform so gewählt ist, dass dem Stent einzigartige mechanische Eigenschaften. an dieser bestimmten Stelle im Stent verliehen werden. Durch Verwendung eines Mittels, um entweder Material gezielt zu entfernen oder den Werkstoff mechanisch zu verformen, können die Stentstreben an ausgewählten Stellen in eine zugeschnittene Form gebracht werden. Durch diese zugeschnittenen Formen können die Streben stärker oder schwächer sein, während des Ausdehnens flach bleiben, sich während des Ausdehnens drehen usw.
- Außerdem kann durch die Veränderung des Querschnitts der Stentstreben das Verschachteln oder Verdichten der Streben im zusammengedrückten Zustand verbessert werden. Durch die Verbesserung der Verdichtungseigenschaften des Stents kann der Stent einen geringeren Querschnitt aufweisen. Es kann sogar Überlappungsbereiche der verschiedenen Streben geben, ohne dass die Streben in den Innendurchmesser oder Außendurchmesser des Stents verformt werden. Dadurch bleibt ein geringer Querschnitt erhalten. Zusätzlich entsteht durch das Überlappen der Streben ein Sicherungsmittel. Jede Strebenreihe trägt dazu bei, eine benachbarte Reihe zu umfassen. Dies ist in
6 dargestellt. - Mit Bezug auf
7 ist ein Beispiel eines Teils eines Stents10 gezeigt, um zu veranschaulichen, dass ein erfindungsgemäßer Stent aus einem einzelnen länglichen Stück Draht oder Ähnlichem hergestellt sein kann. - In
8 sind die Verknüpfungsstellen20 zwischen den Ringen12 in einem Stent10 dargestellt, in der Draufsicht. - Die vorstehenden Beispiele und die vorstehende Offenbarung sollen der Veranschaulichung dienen und nicht umfassend sein. Aus den Beispielen und der Beschreibung eröffnen sich für einen Durchschnittsfachmann viele Abwandlungen und Wahlmöglichkeiten.
- Die Erfindung ist wie in den angehängten Ansprüchen definiert.
Claims (10)
- Flexibler dehnbarer Stent, der eine Vielzahl gewundener Ringe (
12 ) umfasst, dadurch gekennzeichnet, dass die gewundenen Ringe und die distal benachbarten gewundenen Ringe als auch die gewundenen Ringe und die proximal benachbarten gewundenen Ringe ineinander greifen, um einen maschenartigen walzenförmigen Stentkörper (10 ) zu bilden. - Stent nach Anspruch 1, wobei jeder Ring aus einer Vielzahl von Abschnitten besteht, wobei jeder abwechselnd breite und enge Bereiche aufweist, um deren Ineinanderpassen zu vereinfachen.
- Stent nach Anspruch 1, in dem die Ringe nicht miteinander verbunden sind.
- Stent nach Anspruch 1, in dem die Ringe miteinander verbunden sind.
- Stent nach Anspruch 1, in dem die Ringe aus Draht geformt sind.
- Stent nach Anspruch 5, in dem der Draht eine zusammenhängende Länge Draht ist, der durch den gesamten Stentkörper verläuft.
- Stent nach Anspruch 1, in dem die Ringe aus einem Werkstoff geformt sind, der im Wesentlichen flache Seiten aufweist.
- Stent nach Anspruch 1, in dem die Ringe aus einem Metallrohr geschnitten sind, wodurch eine Vielzahl von Streben bereitgestellt sind, die im Querschnitt im Wesentlichen flache Seiten aufweisen.
- Stent nach Anspruch 8, in dem die Seitenwände mindestens einiger Streben in Bezug auf die Mittellinie des Stents winklig sind, wodurch eine Trennung in radiale Richtung verhindert ist.
- Stent nach Anspruch 1, in dem sich der Querschnitt des Rings in einer vorher festgelegten Art ändert, um für verschiedene Bereiche des Stents ausgewählte Eigenschaften bereitzustellen.
