Description
- Die Erfindung betrifft eine Kunststofflinse, die in Form
- einer Zwischenscheibe in die Hornhaut implantierbar und zur Korrektur von Plus- und Minuswerten geeignet ist.
- Bei hochgradig fehlsichtigen Augen liegt der von den brechenden Medien erzielte Fokus entweder vor oder hinter der Netzhaut. Die brechenden Medien sind die Hornhaut des Auges, das Vorderkammerwasser, die Linse und der Glaskörper. Die Brechung der Hornhaut und der Linse sind die maßgeblichen Größen. Die Hornhaut hat eine Brechkraft von ca. 50 Dioptrien, die Linse von ca. 18 Dioptrien. Die entscheidende Größe für die Brechkraft der gesamten Hornhaut ist deren äußere Oberflächenkrümmung.
- Bei Kurzsichtigen ist die Brechkraft des Auges im Verhältnis zu dessen Länqe zu groß. Die optimale Korrektur liegt in einem mit der Hornhaut integrierten Linsensystem, das die Oberfläche der Hornhaut flacher gestaltet. Bei Weitsichtigkeit oder Linsenlosigkeit des Auges, wie sie bei der, Operation des Grauen Stars herbeigeführt wird, ist eine Ver kleinerung des äußeren Krümmungsradius der Hornhaut zum Ausgleich der Refraktion erforderlich, um die Brechkraft der Hornhaut zu erhöhen. Zu diesem Zweck werden sowohl Brillen als auch Kontaktlinsen benutzt. Es ist bekannt, daß eine Brille, da das Glas von der Hornhaut entfernt ist, schlechtere optische Gegebenheiten hat als ein mit der Hornhaut verbundenes System. Kontaktlinsen, die an sich die bessere Lösung darstellen, haben insofern Nachteile, als insbesondere ältere Menschen sich die Linse nicht selbst einsetzen oder sie herausnehmen und reinigen können. Es kommt ferner zu wiederkehrenden Entzündungen an der Hornhaut oder der Bindehaut, so daß die Linsen überhaupt nicht mehr qetragen werden können.
- Es besteht ferner die Möglichkeit, bei einer Operation des Grauen Stars die vorhandene Linse zu entfernen und in das Augeninnere eine Kunststofflinse einzubringen, um die Brechkraft der entfernten Linse zu ersetzen. Die im Auge befindliche Kunststofflinse führt zu etwa 2 % zur Erblindung; sie ist auch nicht universell anwendbar und verbietet sich in einer Reihe von Fällen, z.B. bei Diabetikern oder bei Patienten, die einen Grünen Star haben. Die Linse kann nur in etwa 30 % der operierten Fälle zur Anwendung kommen.
- Bei einer anderen Methode, der Keratomileusis, wird die Anderung der Oberflächenkrümmung der Hornhaut durch Modifikation der Hornhaut selbst herbeigeführt. Das Verfahren ist außerordentlich schwierig durchzuführen, sehr zeitaufwendig und verlangt eine gute Mitarbeit des Patienten im postoperativen Verlauf. Das Verfahren ist auch außerordentlich aufwendig, da eine speziell gestaltete Mikro-Kältedrehbank mit der dazugehörigen CO 2-Technik benötigt wird und entsprechend ausgebildete Fachleute ständig benötigt werden.
- Die einfachste Lösung, die sich anbietet, um die Brechkraft der Hornhaut zu verändern, besteht darin, ihre Oberfläche dadurch zur gewünschten Form zu bringen, daß eine Zwischenscheibe in die Hornhaut selbst eingelegt wird. Man hat vor einigen Jahren bereits versucht, eine Zwischenscheibe aus Kunststoff in die Hornhaut einzulegen. Diese Methode hatte jedoch keinen Erfolg, da der Kunststoff die Hornhaut eintrübte oder den mundrand sprengte. Von weiteren Versuchen in dieser Richtung wurde daher Abstand genommen und stattdessen eine Zwischenscheibe aus menschlichem Gewebe in die Hornhaut eingebracht. Ein Nachteil dieser Methode ist, daß man für jede Operation ein Spender auge benötigt und einen außerordentlich langen Heilungsverlauf in Kauf nehmen muß.
- Der Erfindung liegt die Aufgabe zugrunde, eine Kunststofflinse so auszubilden, daß sie in Form einer Zwischenscheibe @n dieBornghaut implantierbar und zur Korrektur von Plus-und Minuswerten geeignet ist, ohne daß die Hornhaut sich eintrübt und den mundrand sprengt.
- Überraschenderweise hat sich herausgestellt, daß dies möqlich ist, wenn der Durchmesser der einseitig konkaven Linse 5,5 bis 8 mm beträgt, der Radius der Innenkurve R1 = R2 + EC ist, wobei R2 der Innenradius der Hornhaut und EC die Stärke der verbliebenen Hornhaut im Bereich des Implantates ist, während der Radius der Außenkurve in Abhängigkeit von der Innenkurve entsprechend der gewünschten Korrektur durch Berechnung mit bekannten Methoden festgelegt wird und wenn die Oberflächen der Linse sich an ihrem Rand schneiden.
- Der Radius R1 der Innenkurve hat zwepkmäßigerweise einen Wert von 6,75 bis 7,95 mm.
- Als zweckmäßig hat es sich erwiesen, die Linse aus einem bekannten Kunststoff zu fertigen, der aus 2-Hydroxyäthyl-Methacrylat (HEMA) besteht und Diacetonacrylamid, Trimethylolpropan, Trimethacrylat und Methacrylsäure enthält.
- Ein zweiter, ebenfalls bekannter Kunststoff, der aus einer Mischung von 2-Hydroxyäthyl-Methacrylat, N-Vinyl-2-Pyrrolidon (NVP) und Methacrylsäure besteht, ist für die Herstellung der Linse ebenfalls bevorzugt geeignet.
- Auf der Zeichnung ist eine in die Hornhaut implantierte Linse schematisch dargestellt. Es bedeuten: R1 den Radius der Innenkurve der Linse 1, R2 den Ra#dius der Innenkurve der Hornhaut 2, R3 den Radius der Außenkurve der Kunststofflinse, EC die Stärke der verbliebenen Hornhaut im Bereich des Implantates und ED die Stärke der von der Hornhaut abgehobenen Scheibe, die nach Einleqen der Kunststofflinse wieder aufgelegt wird
Claims (4)
- Kunststofflinse Ansprüche 1) Kunststofflinse, die in Form einer Zwischenscheibe in die Hornhaut implantierbar und zur Korrektur von Plus- und Minuswerten geeignet ist, d a d u r c h g e k e n n -z e i c h n e t , daß die einseitig konkave Linse einen Durchmesser von 5,5 bis 8 mm hat, daß der Radius der Innenkurve R1 = R2 + EC ist, wobei R2 der Innenradius der Hornhaut und EC die Stärke der verbliebenen Hornhaut im Bereich des Implantates ist, während der Radius R3 der Außenkurve der Linse in Abhängigkeit von der Innenkurve entsprechend der gewünschten Korrektur durch Berechnung mit bekannten Methoden festgelegt wird und daß die Oberflächen der Linse sich an ihrem Rand schneiden.
- 2. Kunststofflinse nach Anspruch 1, dadurch gekennzeichnet, daß R1 einen Wert von 6,75 bis 7,95 mm hat.
- 3. Kunststofflinse nach den Ansprüchen 1 und 2, dadurch gekennzeichnet, daß die Linse aus 2-Hydroxyäthyl-Methacrylat besteht und Diacetonacrylamid, Trimethylol#ronan, Trimethacrylat und Methacrylsäure enthält.
- 4. Kunststofflinse nach den Ansprüchen 1 und 2, dadurch gekennzeichnet, daß sie aus einer Mischung von 2-Hydroxyäthyl-Methacrylat, N-Vinyl-2-Pyrrolidon und Methacrylsaure besteht.
