Description
- KÜNSTLICKES HÜFTCKLLHK Die Erfindung betrifft künstliche Hüftgelenke, die operativ als Hüftprothesen eingesetzt werden.
- Das Gelenk dient zur Wiederherstellung der verlorenen Toweglichkeit des Hüftgelenkes infolge der Bechterowschen Krankheit, der verformenden Arthrose, der rheumatischen Arthritis, der aseptischen Nekrose des Schenkelkopfes sowie mancher Schenkelhalsfrakturen bei älteren Personen.
- Es ist bereits ein künstliches Hüftgelenk bekannt, welches eine Endoprothese für die Hüftpfanne des Kranken und eine Endoprothese für das proximale Hüftende enthält, wobei die letstere aus einem Kopf, einem Hals und einem für die Einführung in den Hüftknochenkanal bestimmten Stift besteht.
- Hierbei kann der konstruktive Aufbau der Endoprothese für die Hüftpfanne unterschiedlich sein, sei es die Gestaltung der Außenfläche oder die der Innenfläche, die in Berührung mit dem Kopf der Endoprothese für das proximale Hüftende kommen soll.
- Die Außenfläche der Endoprothese für die Hüftpfarne ist gemäß einer der Ausführungsformen des künstlichen Hüftgelerkes mit einer langen Schraube verschen, die durch die Hüftpfanne in den Darmbeinflügel eingeschraubt wird. In dieser Weise erzielt man die Fixierung der Endoprothese für die Hüftpfanne im Becken.
- Die Innenfläche der Endoprothese für die Hftpfanne ist hemisphärisch, und der Kopf der Endoprothese für das proximale Hüftende taucht darin bis an seinen Druchmesser ein, ohne hierbei irgendwie befestigt zu werden. (Siehe z.B. Jourral of bone and joint Surgery 1968, Bd. 50-B, Mr. 4, S. 720-721).
- Diesem konstruktiven Aufbau haften als Mangel eine komplizierte Einführungstechnik der langen Schraube in den Dannbeinflügel, eine Lockorung dieser Schraube und eine mögliche Ausrenkung der Endoprothese für das proximale Hüftende aus der Endoprothese für die Hüftpfanne an.
- Es ist weiterhin ein künstliches Hüftgelenk bekannt, dessen Endoprothese fur die Hüftpfanne auf ihrer Außenfläche Dorne aufweist. Diese Dorne, verbunden mit Zement, tragen zu einer besseren Fixierung der Endoprothese für die Hüftpfanne bei. (Siehe z.B. Acta Orthopaedica Belgica 1965. Bd. 31.
- Nr. 5, S. 681-691).
- Die Innenfläche dieser Endoprothese ist zur Aufnahme des Kopfes der Endoprothese für das proximale Hüftende hemisphärisch.
- Dieser Konstruktion ist der Nachteil eigen, daß bei der Fixierung der Endoprothese für die Eftpanne Zement verwendet wird, wobei mit einer eventuellen Auswirkung der Ende prothese für das proximale Hüftende zu rechnen ist.
- Die Außenfläche der Endoprothese fär die Hüftpfanne kann dabei Blätter mit Offnungen aufweisen, in die das wächst.
- Knochengewebe hinein' (Siche z.B. Urheberschein der UASSR, Nr. 278022, Kl. 30d 1/01).
- Die Innenfläche der Endoprothese für die Hüftpfanne bew sitzt eine Aussparung zur Aufnahme einer aus zwei gleichen Hälften bestehenden Einlage. Die Außenfläche der Hinlage hat die Gestalt der Aussparung, während die Innenfläche sphärisch ausgebildet ist und den sphärischen Kopf der Endoprothese für das proximale Hüftende derart umfaßt, daß din Berührungsfläche größer ist als die Hemisphäre des Kopfes.
- Die besprochenen Einlagen dienen dazu, eine Verbindung der Endoprothese für die Hüftpfanne mit der für das proximale Füftende zu bewirken. Erreicht wird dies durch ein Stauchen der Enden der Einlage seitens der Außenfläche der Endoprothese für die Hüftpfanne. In dieser Weise erzielt man eine unteilbare Verbindung.
- Die Nachteile dieser künstlichen Gelenke bestehen darin, daß sie sich bei der Operation nicht ohne Schwierig keit einbringen lassen und ihre einheiten nicht austauschbar sind. Dazu kommt, daß sich für die Anfertigung der Endoprothesen nur stauchbare Werkstoffe eignen.
- Somit sind die einen der bekarnIten künstlichen Hüftgelenke teilbar, die aber eine Ausrenkung des Kopfes der Endoprothese für das proximale Hüftende aus der Endoprothese für die W'tftpfanne nicht verhindern, während die anderen bekannten künstlichen Hüftgelenke untrennbar ausgeführt und nicht nur während der Operation, sondern auch bei einem eventuellen Austauschen eines der Bestandteile des künstlichen Hüftgelenkes schwer zu handhaben sind.
- Die Erfindung bezweckt die Beseitigung der angegebenen Nachteile.
- Der Erfindung liegt die Aufgabe zugrunde, ein künstliches Hüftgelenk zu schaffen, welches teilbar ist und sich bei der Operation zusammenbauen läßt, sowie eine Ausrenkung der Endoprothese für das proximale Hüftende aus der Endoprothese für die liüftpfanre ausschließt, wobei der konstruktive Aufbau eine breitere Wahl der Werkstoffe unabhän gig von deren Stauchbarkeit zulassen muß.
- Die Aufgabe ist dadurch gelöst, daß bei dem künstlichen Hüftgelenk, bestehend aus einer Endoprothese für die Hüftpfanne des Kranken, die eine im Knochengewebe des Beckens fixierbare Außenfläche und eine Aussparung besitzt, und einer Endoprothese für das proximale Hüftende des Kran ken, die einen sphärischen Kopf hat, welcher durch den Hals mit einem für die Einführung in den Hüftknochenkanal bestimmten Stift fest verbunden ist, während zwischen der Oberfläche der Aussparung und der sphärischen Außenfläche des Kopfes eine Einlage vorgeschen ist, die für ihre gelenkige Verbindung vorgesehen ist und eine sphärischen Innenfläehe für die Einwirkung auf den sphärisch ausgestalteten Kopf hat, gemäß der Erfindung die Höhe der durch dit sphärisehe Innenfläche gebildeten Höhle der Einlage größer als der Halbmesser und kleiner als der Durchmesser des Kopfes bemesson ist, wobei die Einlage in dem distalen Teil Ausschnitte aufweist, zwischen denen befindliche Arme eine Zange bilden, welche den Kopf der Endoprothese für das proxinale Hüfsende umfaßt, und die Einlage selbst in der Aussparung der Hüftpfanne mittels einer selbstsichernden Mutter befestigt ist; Das erfindungsgemäße künstliche Hüftgelenk verfügt übe eine Reihe von Vorteilen.
- Dieses HUftgelenk ermöglicht es, während der Operation die Endoprothese für die ilüftpfanne und die Endoprothese für das proximale Hüftende getrennt einzubringen. Durch solch eine getrennte Einbringung wird ihre genaue und feste Fixierung innerhalb des Knochens erzielt. Gegebenenfalls kann nur eine von diesen Endoprothesen ausgewechselt werden.
- Eine Ausrenkung der Endoprothese für das proximale Hüftende aus der Endoprothese für die llüftpfanne ist angeschlossen, weil diese beiden Endoprothesensicher miteinander verbunden sind.
- Die Konstruktion der Einlage bei welcher Ausschnitte im distalen Teil sowie eine Zange bildende Arme vorgeschen sind, tragt zu einer breiteren Palette an Werkstoffen für die Herstellung der Endoprothese sowie zur Bekämpfung der Reibung an den Gelenkflächen bei0 Im folgenden wird die Erfindung anhand einer konkreten Ausführungsform unter Bezugnahme auf die Zeichnungen erläutert. Es zeigen: Fig. 1 eine Gesamtansicht auf das teilweise geschnit tene erfindungsgemaße künstliche Hüftgelenk; Fig. 2 eine Gesamtansicht im ausgebauten Zustand; Fig. 3 die Ansicht A der Fig. 2; - Fig. 4 die Ansicht B der Fig. 2; und Fig. 5 die Ansicht C der Fig. 2.
- Das erfindungsgemäße künstliche Hüftgelenk enthält eine Endoprothese 1 (Fig. 1, 2) für die Hüftpfanne des Krnnken, deren Außenfläche zur Fixierung im Knochengewebe des Beckens geeignet ist, und eine Endoprothese 2 für das proximale Hüftende. Beide Endoprothesen sind gelenkig miteinander verbunden.
- Die Endoprothese 1 für die Hüftpfanne weist außen eine Reihe Blätter 3 mit abgeschrägten Enden auf, welche stufenförmig übereinander angeordnet sind und Öffnungen 4 aufweisen, in die das Knochengewebe des Beckens einwächst.Auf der anderen Fläche der Endoprothese 1 (Fig. 2) für die Hüftpfan ne ist eine Aussparung 5 vorgesehen, deren komplizierte Gestalt durch eine zylindrische und kegelförmige Fläche ausgebildet ist. Diese Aussparung kann durchgehend, wie das die Zeichnung erkennen läßt, bzw, nicht durchgehend sein.
- Der clistale Teil der Aussparung ist L:Llt einem Gewinde 5 versehen.
- Die Endoprothese 2 für das proximale Hüftende besitzt einen Kopf 7 (Fig. 1, 2) mit sphärischer Oberfläche, der durch den Hals 8 mit einem spitzen, für die Einführung in den Hüftknochenkanal bestimmten Stift 9 fest verbunden ist Damit das Knochengewebe in den Stift 9 einwachsen kann, ist dieser mit Öffnungen 10 versehen. Im proximalen Bereich geht der Stift 9 in eine Auflagefläche 11 über, die einen Bolzen 12 zur Befestigung des großen Rollhügels besitzt.
- Zur gelenkigen Verbindung der Endoprothese 1 für die Hüftpfanne mit der für das proximale Hüftende ist zwischen der sphärischen Außenflche des Kopfes 7 und der Oberfläche der Aussparung 5 eine Einlage 13 vorgesehen. Die Außenfläche der Einlage 13 hat die Gestalt der Innenfläche der Aussparung 5 mit einer Abschrägung im distalen Bereich, während ihre Innenfläche sphärisch ausgestaltet ist und aus den sphärischen Kopf 7 einwirkt.
- Die Höhe der durch die sphärische Innenfläche der Einlsge 13 gebildeten Höhle (Fig. 2) ist größer als der Halbmesser und kleiner als der Durchmesser des Kopfes 7. Im distalen Bereich weist die Einlage 13 (Fig. 2, 3,) Ausschnitte 14 bis an den Durchmesser der sphärischen Oberfläche der Einlage 13 auf, wobei zwischen diesen Ausschnitten 14 befindliche Arme 15 eine Zange bilden, die den Kopf 7 umfaßt.
- Die Einlage 13 (Fig. 2) v,ird in der Aussparung 5 mit einer selbstsichernden Mutter 16 fixiert, deren Außenfläche mit einen Gewinde 17 versehen ist, welches dem Gewinde 6 der Aussparung 5 der Endoprothese 1 für die Iiüftpfarnie entspricht. Die Innenfläche der Mutter 16 ist entsprechend der Abschrägung im distalen Teil der Außenfläche der Einlage abgeschrägt. Die Mutter 16 besitzt eine Sicherungseinrichtung, die aus einem Längsschnitt 18 und einer Schraube 19 besteht.
- Das Längsmaß der Mutter 16 ist größer als der Durchmesser des Kopfes 7 der Endoprothese 2 für das proximale HAftende.
- Falls das Längsmaß der Mutter 16 kleiner ist als der Durchmesser des Kopfes 7 der Endoprthese 2 für das proximale Hüftende (z. B. wenn bei der Herstellung der Einlage 13 die Verwendung eines Werkstoffes erforderlich ist, der unter Druck zu fließen beginnt), muß sie beim Zusammenbau der Endoprothese 2 für das proximale Hüftende zuvor auf den Hals 8 oder von seiten des distalen Endes LOS Stiftes 9 aufgesetzt werden, wozu jeweils die Abmessungen der Auflagefläche 11 zu reduzieren sind.
- Um die Mutter 16 besser einschrauben bzw. lösen zu können, ist sie gemäß Fig. 2 und 4 mit vier Flügeln 20 versehen.
- In Fig. 3 ist die Einlage 13 von der Seite des distalen Endes mit den Ausschnitten 14 her wiedergegeben, die über die Kreislinie dergestalt angeordnet sind, daß der Ausschnitt 14 in der Arbeitszone, die beim Gehen am meisten beansprucht wird, fehlt.
- An ihrem proximalen Ende besitzt die Einlage 13 (Fig.
- 1, 2 und 5) einen vierkantigen Ansatz 21 zur Senkung in eine entsprechende Öffnung 22 (Fig. 2) der Endoprothese 1 für die Hüftpfanne. Durch die Gestalt des Ansatzes 21 und der Öffnung 22 wird eine Drehung der Einlage 13 innerhalb der Endoprothese 1 für die Hüftpfanne verhindert. Falls aber die Einlage aus einem Werkstoff angefertigt wird, welcher zum Fließen neigt ist, mässen die Öffnung und der Ansatz fehlen.
- Das erfindungsgemäße künstliche Hüftgelenk wird während der Operation im Körper des Kranken folgendermaßben eingebracht.
- Vor der Operation befindet sich das künstliche Hüftgelenk im ausgebauten Zustand, d.h. man trennte zuvor die Endoprot"lese 1 (Fig. 2) für die Hüftpfanne von der Endoprothese 2 für das proximale Hüftende und der Einlage 13 durch Lösen der selbstsichernden Mutter 16. Die Endoprothese 1 für die Hüftpfanne wird in eine zuvor gefornte Vertiefung im Knohengewebe des Beckens des Kranken einseschlagen. Anschließend schlägt man die D'ndoprothese 2 i:r das proximale Hüftende zusammen mit der Einlage 13 und der Mutter 16 in den joweils vorbereiteten Hüftknochenkanal des Patienten ein, wobei die Einlage 13 und die Mutter 16 zuvor auf den Kopf 7 und den Hals 8 auf--esetzt worden sind.Dann setzt man in die Endoprothese 1 für die Hüftpfanne die Ein lage 13 mit dem 1opf 7 ein, die mittels der selbstsichernden Mutter 16 fixiert wird.
- Durch das erfindungsgemäße künstliche Hüftgelenk wird die Operation erleichtert, die Austauschbarkeit der Einheiten gesichert, die Palette an für die tierstellung der Endoprothese dienenden Werkstoffen, die die Reibung an den Gelenkflächen vermindern, verbreitert und das Risiko einer Ausrenkung der Endoprothese für das proximale Hüftende aus der Endoprothese für die Hüftpfanne ausgeschlossen, Patentanspruch
Claims (1)
- PATENTANSPRUCH Künstliches Hüftgelenk, bestehend aus einer Endoprothese für die Hüftpfanne des Kranken, die eine im Knochengewebe des Beckens fixierbare Außenfläche sowie eine Aussparung aufweist, und einer Endoprothese für das proximale Hüftende des Kranken, die einen sphärischen Kopf hat, welcher durch den Hals mit einem für die Einführung in den Hüftknochenkanals bestimmten Stift fest verbunden ist, während zwischen der Oberfläche der Aussparung und der sphärischen Außenfläche des Kopfes eine Einlae vorgesehen ist, die für ihre gelenkige Verbindung dient und eine sphärische Innenfläche zur Einwirkung auf den sphärisch ausgestalteten Kopf aufweist, dadurch g e k e n n z e i c h -n e t, daß die Höhe der durch die sphärische Innenfläche gebildeten Höhle der Einlage (13) größer als der Halbmesser und kleiner als der Durchmesser des Kopfes (7) bemessen ist, wobei die Einlage (13) in dem distalen Teil Ausschnitte (14) aufweist, zwischen denen befindliche Arme (15) eine Zange bilden, welche den Kopf (7) der Endoprothese (2) für das proximale Hüftende umfaßt, , und daß die Einlage (13) selbst in der Aussparung (5) der Endoprothese (1) für die Hüftpfanne mittels einer selbstsichernden Mutter (16) befestigt ist.
