Abstract

An internal prosthesis for a joint, particularly an elbow joint, is intended to be implanted in the bone cavities. The prosthesis consists of a first part, with a convex head (607) which is implanted in the humerus and a second part with a concave head (615) which is implanted in the ulna. Both parts are implanted in their bones by perforated tubular anchoring members (617). These two anchoring members are of similar form and are preferably identical. USE/ADVANTAGE - The perforated tubes provide firm joints for the implants in the humerus and ulna.

   BESCHREIBUNG



   Die Erfindung bezieht sich auf eine Endogelenkprothese bestehend aus einer ersten, einen konvexen Kopfteil tragenden Komponente und einer zweiten, einen konkaven Kopfteil tragenden Komponente.



   Endogelenkprothesen, insbesondere Hüftgelenkprothesen sind bereits in grosser Anzahl beim Menschen zur Beseitigung schmerzhafter Gelenkveränderungen sowie zur Wiederherstellung wesentlicher Gelenksfunktionen implantiert worden. Die meisten Endogelenksprothesen wurden unter Zuhilfenahme von Knochenzement implantiert, in neuerer Zeit aber auch ohne Verwendung von Knochenzement jedoch mit entsprechender Strukturierung der in den Knochen zu implantierenden Verankerungsteile.



   Die Strukturierung der beiden Komponenten einer Endogelenkprothese erfolgte bisher meist unterschiedlich, d. h.



  je nach Form und Aufgabe der Komponente. Beispielsweise ist eine Strukturierung des Femurteils einer Hüftgelenkprothese aus der US-PS 4 199 824 bekannt, bei der die Oberfläche des zu implantierenden Schaftes mit fischgratartig angeordneten Vertiefungen sowie mit Perforationen versehen ist.



  Eine andere Strukturierung, für eine künstliche Hüftgelenkspfanne, ist beispielsweise aus der EU-AI 0 056 753 bekannt, bei der die   Aussenfläche    der Pfannenschale mit Verankerungswulsten, welche elastische Querrippen tragen, versehen ist.



   All diesen bekannten Strukturierungen ist gemeinsam, dass sie durch die Aussenform des zu implantierenden Verankerungsteils weitgehend vorbestimmt sind und auf rein theoretischen Überlegungen basieren.



   Ein weiterer Nachteil besteht darin, dass die auf verschiedenen Verankerungsprinzipien basierenden Endogelenkprothesenkomponenten nicht miteinander kompatibel sind und in der Herstellung kompliziert sind, da sie keine Vereinheitlichung der Einzelteile zulassen.



   Hier will die Erfindung Abhilfe schaffen. Die Erfindung, wie sie in den Ansprüchen gekennzeichnet ist, löst die Aufgabe eine Zweikomponenten-Endogelenkprothese zu schaffen, welche auf einem einheitlichen Verankerungsprinzip, vorzugsweise ohne zusätzliche Verwendung von Knochenzement, beruht. Die durch die Erfindung erreichten Vorteile sind im wesentlichen darin zu sehen,

   dass durch Verwendung des bisher nur in der   Zahnimplantationstechiiik    als Einzelimplantat ohne gelenkige Verbindung zur Übertragung von Kräften bekannten   Hohlzylinderprinzips,    überraschenderweise auch hervorragende Erfolge in der Fixation von Endogelenkprothesenkomponenten unterschiedlicher Form und Funktion erzielt werden konnten und dass durch Verwendung eines einheitlichen Verankerungsteiles für beide Komponenten der Endogelenkprothese eine rationelle Fertigung und reduzierte Lagerhaltung von Einzelteilen für Prothesenkomponenten verschiedener Grösse ermöglicht wurde.



   Die Erfindung soll nun anhand der Zeichnung dargestellter Ausführungsbeispiele erläutert werden. In der Zeichnung zeigen: die Figur 1 eine schaubildliche Darstellung einer Femur Komponente einer Kniegelenkprothese, die Figur 2 eine schaubildliche Darstellung einer Tibia Komponente einer Kniegelenkprothese passend zu der in Figur 1 dargestellten Femur-Komponente, die Figur 3 einen antero-posterioren Schnitt durch eine der Kondylenflächen der in Figur 2 dargestellten Tibia Komponente, die Figur 4 eine schaubildliche Darstellung einer Patella Komponente einer Kniegelenkprothese die Figur 5 eine schaubildliche Darstellung einer Schultergelenkprothese bestehend aus einer Skapula- und einer Humerus-Komponente,

   die Figur 6 eine schaubildliche Darstellung einer Ellbogengelenkprothese bestehend aus einer Humerus- und einer Ulna-Komponente und die Figur 7 eine schaubildliche Darstellung einer Fingergelenkprothese mit einer Metakarpal- und einer Phalanx Komponente.



   In den Figuren 1 bis 3 ist eine Kniegelenkprothese dargestellt mit einer Femurkomponente 407 und einer Tibiakomponente 405 welche beide mittels je zwei Verankerungsteilen 417 im Femur, bzw. in der Tibia verankert sind.



   Die Tibiakomponente 405 weist einen plattenförmigen, metallischen Aussenteil 415 auf der proximal den Innenteil 413 und distal die beiden bereits erwähnten Verankerungsteile 417 trägt.



   Der Innenteil 413 besteht aus der Kunststoff-Schale 411 mit einer konkaven Innenfläche 413a als Gleitpartner für die beiden konvexen Kondylenpartien der Femurkomponente 407. Der Aussenteil 415 der Tibiakomponente 405 weist an seiner distalen Seite zwei Ringnuten auf zur Fixierung von je einem Verankerungsteil 417 mittels Stiften 424, welche durch miteinander fluchtend angeordnete Bohrungen von Aussenund Verankerungsteil gesteckt werden können.



   Die Befestigung der Verankerungsteile 417 an der Femurkomponente 407 kann analog oder mittels eines anderen lösbaren oder unlösbaren Befestigungsmechanismus erfolgen. Beispielsweise können die Verankerungsteile 417 auch in die Gelenkkomponenten geschraubt werden.  



   Die Verwendung zweier Verankerungsteile je auf der distalen und medialen Seite ergibt eine ausgezeichnete Rotationsstabilität der implantierten Gelenk-Komponenten.



   Die Verankerungsteile 417 sind im allgemeinen hohlzylindrisch ausgestaltet und weisen im Zylindermantel vorzugsweise Perforationen auf. Die in den Figuren dargestellten Verankerungsteile 417 sind an ihren freien Enden jeweils offen, doch sind auch geschlossene Hohlzylinder verwendbar. Für die in Figur 2 gezeigte Tibia-Komponente haben sich Verankerungsteile bewährt mit einem Durchmesser von 27-30 mm und einer Höhe von   5-20    mm.



   Die Wandstärke des Mantels der Verankerungsteile 417 ist höchstens 10% und vorzugsweise höchstens 5% des Innendurchmessers des Verankerungsteils.



   Die Verankerungsteile 417 bestehen aus Metall, beispielsweise aus Titan oder seinen Legierungen, deren am Knochen anliegende Flächen eventuell mit einer porösen, durch ein Plasmasprühverfahren aufgebrachten Titanschicht versehen sein können. Die Femur- und Tibiakomponenten 407 und 405 können entweder vor der Operation zusammengesetzt werden - was den Vorteil einer limitierten Lagerhaltung von Einzelteilen für die verschiedenen Grössen   besitzt¯    oder vorzugsweise dem Chirurgen bereits in zusammengesetztem Zustand geliefert werden, wodurch Sterilisationsprobleme vermieden werden.



   In Figur 4 ist eine   erfmdungsgemässe    Patella-Komponente 455 einer Kniegelenkprothese dargestellt, welche im Knochen 451, nämlich eine Patella, implantiert ist. Die Patellakomponente 455 weist einen metallischen Aussenteil 465 auf, welcher den Innenteil 433 und den Verankerungsteil 467 trägt. Der Innenteil 433 besteht aus Kunststoff und hat eine konkave Innenfläche 463a. Der Verankerungsteil 467 kann gemäss den vorausgehend beschriebenen Varianten von Verankerungsteilen für die Femur und Tibia-Komponenten ausgebildet sein.



   In Figur 5 ist eine Schultergelenkprothese dargestellt mit einer Skapula- und einer Humerus-Komponente. Die Ska   puln-Komponente    505 ist mit einem metallischen Aussenteil 515 und einem Innenteil 513 mit einer konkaven Innenfläche 513a als Gleitpartner für die konvexe Humerus-Komponente 507 der Schultergelenkprothese. Die Verankerungsteile 517 mit Perforationen 517c können, ähnlich wie für die in Figuren   1-4    dargestellte Kniegelenkprothese am Aussenteil 515 befestigt werden. Die Humeruskomponente 507 ist analog zur Skapulakomponente ausgebildet.



   Figur 6 zeigt eine Ellbogengelenkprothese mit einer Humerus- und einer Ulna-Komponente. Die in einer Ulna 601 implantierte Ulna-Komponente 605 weist einen gewinkelten Aussenteil 615 auf, der einen Innenteil 613 trägt mit einer eine konkave Innenfläche 613a aufweisende Erhöhung 618.



   Die Erhöhung 618 korrespondiert gelenkig mit der Vertiefung 619 der konvex ausgebildeten Humerus-Komponente 607. Beide Komponenten besitzen an der von der Gelenkfläche abgewandten Seite je einen Verankerungsteil 617 mit Perforationen 617c.



   Figur 7 zeigt schliesslich ein künstliches Metacarpophalangealgelenk mit einer im Os metacarpale 701 implantierten Metakarpal- und einer in der Phalanx implantierten Phalanx-Komponente.



   Sowohl die Phalanxkomponente 705, als auch die damit gelenkig verbundene Metakarpalkomponente 707 weisen je einen mit Perforationen 717c versehenen Verankerungsteil 717 auf. Die Verankerungsteile 417, 467, 517, 617 und 717 ergeben eine beträchtliche Vergrösserung der für die Kraft übertragung zwischen Knochen und Gelenkkomponente wirksamen Flächen. Dabei tragen die Verankerungsteile insbesondere zur Übertragung derjenigen, häufig auftretenden Kräfte bei, die entstehen, wenn der konvexe Kopfteil der einen Gelenkkomponente eine Schwenkungbewegung um eine Achse ausführt, die ungefähr horizontal und ungefähr rechtwinklig zur Symmetrieachse des konkaven Kopfteils der anderen Gelenkkomponente verläuft.

  Ein weiterer wesentlicher Vorteil der als Verankerungsmittel der Gelenkpfanne dienenden Verankerungsteile 417, 467, 517, 617 und 717 besteht darin, dass wegen ihrer zentralen Hohlräume zu ihrer Aufnahme zur verhältnismässig wenig Knochenmaterial entfernt werden muss, so dass insbesondere auch eine gute Blutversorgung aller anliegenden Knochenteile ermöglicht wird.



   Statt einer Perforation oder zusätzlich zu dieser können die Verankerungsteile 417, 467, 517, 617 und 717 mit mindestens einer Rippe versehen sein, die ein Gewinde bildet und eventuell sägezahnartig profiliert werden kann. Die Verankerungsteile können aber auch mit axialen Rippen oder mit reihenweise angeordneten Zähnen versehen sein.



   Die Implantation der mit Verankerungsteilen 417, 467, 517, 617 und 717 versehenen Gelenkkomponenten erfolgt durch die übliche Vorbereitung des Knochens durch Ausfräsen der zur Aufnahme der Gelenkkomponente notwendigen Kavität. Zusätzlich dazu wird nun mit Hilfe einer geeigneten Lehre die zur Aufnahme des hohlzylinderförmigen Verankerungsteiles notwendige Ringnut ausgefräst.



   In den so vorbereiteten Knochen kann nun die Gelenkkomponente und deren Verankerungsteil(e) satt eingesetzt werden. Eine genaue Beschreibung der Operationstechnik ist in der schweiz. Patentschrift Nr. 662 267 enthalten. 



  
 



   DESCRIPTION



   The invention relates to an endogenous joint prosthesis consisting of a first component carrying a convex head part and a second component carrying a concave head part.



   Endo-joint prostheses, in particular hip joint prostheses, have already been implanted in large numbers in humans in order to eliminate painful joint changes and to restore essential joint functions. Most endo-joint prostheses have been implanted with the help of bone cement, more recently, however, even without the use of bone cement, but with the appropriate structuring of the anchoring parts to be implanted in the bone.



   The structuring of the two components of an endogenous joint prosthesis has so far mostly been done differently, i. H.



  depending on the form and task of the component. For example, a structuring of the femoral part of a hip joint prosthesis is known from US Pat. No. 4,199,824, in which the surface of the shaft to be implanted is provided with recesses arranged in a herringbone pattern and with perforations.



  Another structuring, for an artificial acetabular cup, is known for example from EU-AI 0 056 753, in which the outer surface of the cup shell is provided with anchoring beads which carry elastic transverse ribs.



   All these known structures have in common that they are largely predetermined by the external shape of the anchoring part to be implanted and are based on purely theoretical considerations.



   Another disadvantage is that the endogenous joint prosthesis components, which are based on different anchoring principles, are not compatible with one another and are complicated to manufacture, since they do not allow the individual parts to be standardized.



   The invention seeks to remedy this. The invention, as characterized in the claims, solves the problem of creating a two-component endogenous joint prosthesis which is based on a uniform anchoring principle, preferably without the additional use of bone cement. The advantages achieved by the invention are essentially to be seen in

   that surprisingly, by using the hollow cylinder principle previously only known in tooth implantation technology as a single implant without an articulated connection for the transmission of forces, excellent results in the fixation of endogenous joint prosthesis components of different shape and function could be achieved, and that by using a uniform anchoring part for both components of the endogenous joint prosthesis rational production and reduced storage of individual parts for prosthetic components of different sizes was made possible.



   The invention will now be explained with reference to the drawing, illustrated embodiments. 1 shows a diagrammatic representation of a femoral component of a knee joint prosthesis, FIG. 2 shows a diagrammatic representation of a tibia component of a knee joint prosthesis suitable for the femoral component shown in FIG. 1, and FIG. 3 shows an antero-posterior section through one of the Condylar surfaces of the tibia component shown in FIG. 2, FIG. 4 is a diagrammatic representation of a patella component of a knee joint prosthesis, FIG. 5 is a diagrammatic representation of a shoulder joint prosthesis consisting of a scapular and a humerus component,

   FIG. 6 shows a diagram of an elbow joint prosthesis consisting of a humerus and an ulna component; and FIG. 7 shows a diagram of a finger joint prosthesis with a metacarpal and a phalanx component.



   1 to 3 show a knee joint prosthesis with a femur component 407 and a tibia component 405, both of which are anchored in the femur or in the tibia by means of two anchoring parts 417 each.



   The tibia component 405 has a plate-shaped, metallic outer part 415 on which proximally the inner part 413 and distally carries the two anchoring parts 417 already mentioned.



   The inner part 413 consists of the plastic shell 411 with a concave inner surface 413a as a sliding partner for the two convex condylar parts of the femoral component 407. The outer part 415 of the tibia component 405 has two annular grooves on its distal side for fixing each anchoring part 417 by means of pins 424 , which can be inserted through aligned holes from the outside and anchoring part.



   The anchoring parts 417 can be attached to the femoral component 407 analogously or by means of another detachable or non-detachable fastening mechanism. For example, the anchoring parts 417 can also be screwed into the joint components.



   The use of two anchoring parts on the distal and medial side results in excellent rotational stability of the implanted joint components.



   The anchoring parts 417 are generally hollow-cylindrical and preferably have perforations in the cylinder jacket. The anchoring parts 417 shown in the figures are each open at their free ends, but closed hollow cylinders can also be used. Anchoring parts with a diameter of 27-30 mm and a height of 5-20 mm have proven useful for the tibia component shown in FIG.



   The wall thickness of the jacket of the anchoring parts 417 is at most 10% and preferably at most 5% of the inside diameter of the anchoring part.



   The anchoring parts 417 consist of metal, for example titanium or its alloys, the surfaces of which lie against the bone can possibly be provided with a porous titanium layer applied by a plasma spraying process. The femoral and tibial components 407 and 405 can either be assembled prior to the operation - which has the advantage of limited storage of individual parts for the different sizes or are preferably delivered to the surgeon already assembled, thereby avoiding sterilization problems.



   FIG. 4 shows a patella component 455 of a knee joint prosthesis according to the invention, which is implanted in the bone 451, namely a patella. The patella component 455 has a metallic outer part 465 which carries the inner part 433 and the anchoring part 467. The inner part 433 is made of plastic and has a concave inner surface 463a. The anchoring part 467 can be designed in accordance with the previously described variants of anchoring parts for the femur and tibia components.



   FIG. 5 shows a shoulder joint prosthesis with a scapular and a humerus component. The Ska puln component 505 has a metallic outer part 515 and an inner part 513 with a concave inner surface 513a as a sliding partner for the convex humerus component 507 of the shoulder joint prosthesis. The anchoring parts 517 with perforations 517c can be attached to the outer part 515, similarly to the knee joint prosthesis shown in FIGS. 1-4. The humerus component 507 is designed analogously to the scapular component.



   FIG. 6 shows an elbow joint prosthesis with a humerus and an ulna component. The ulna component 605 implanted in an ulna 601 has an angled outer part 615 which carries an inner part 613 with an elevation 618 having a concave inner surface 613a.



   The elevation 618 corresponds in an articulated manner to the depression 619 of the convex humerus component 607. Both components have an anchoring part 617 with perforations 617c on the side facing away from the articular surface.



   FIG. 7 finally shows an artificial metacarpophalangeal joint with a metacarpal component implanted in the metacarpal 701 and a phalanx component implanted in the phalanx.



   Both the phalanx component 705 and the metacarpal component 707 articulated to it each have an anchoring part 717 provided with perforations 717c. The anchoring parts 417, 467, 517, 617 and 717 result in a considerable enlargement of the areas effective for the force transmission between the bone and the joint component. The anchoring parts contribute in particular to the transmission of those frequently occurring forces which arise when the convex head part of the one joint component swivels around an axis which is approximately horizontal and approximately at right angles to the axis of symmetry of the concave head part of the other joint component.

  Another important advantage of the anchoring parts 417, 467, 517, 617 and 717 serving as anchoring means of the joint socket is that, because of their central cavities, relatively little bone material has to be removed to accommodate them, so that in particular a good blood supply to all adjacent bone parts is also possible becomes.



   Instead of a perforation or in addition to this, the anchoring parts 417, 467, 517, 617 and 717 can be provided with at least one rib, which forms a thread and can possibly be profiled like a sawtooth. However, the anchoring parts can also be provided with axial ribs or with teeth arranged in rows.



   The implantation of the joint components provided with anchoring parts 417, 467, 517, 617 and 717 is carried out by the customary preparation of the bone by milling out the cavity necessary for receiving the joint component. In addition to this, the ring groove necessary for receiving the hollow cylindrical anchoring part is now milled out using a suitable gauge.



   The joint component and its anchoring part (s) can now be inserted fully into the bones prepared in this way. A precise description of the surgical technique is in Switzerland. Patent No. 662,267 included.


    

Claims (7)

PATENTANSPRÜCHE 1. Endogelenkprothese, insbesondere zur Implantation in Röhrenknochen, bestehend aus einer ersten, einen konvexen Kopfteil tragenden Komponente und einer zweiten, einen konkaven Kopfteil tragenden Komponente, dadurch gekennzeichnet, dass die beiden Komponenten mindestens je einen zur Implantation im Knochen bestimmten, Vertiefungen aufweisenden, zylinderförmigen, von der Gelenkfläche weg verlaufenden Verankerungsteil (417) aufweisen.  PATENT CLAIMS 1. Endogenous joint prosthesis, in particular for implantation in long bones, consisting of a first component bearing a convex head part and a second component bearing a concave head part, characterized in that the two components each have at least one cylindrical recess intended for implantation in the bone have anchoring part (417) running away from the articular surface. 2.
