Abstract

Description
发明背景
1.发明领域
本发明涉及用于治疗疤痕的治疗性医疗服装,具体地讲,涉及用于预防和处理肥大疤痕及瘢痕瘤的治疗性医疗服装。
2.现有技术描述
多年来人们一直用治疗性医疗紧身服装在外科手术例如乳房整形术、腹部整形术及皱纹去除术(face-lifts)之后对于手术区域施加压力。在烧伤后产生的肥大疤痕和瘢痕瘤的处理中,类似的施加压力的服装已成为标准的治疗方法。
如本发明人的申请日为1994年2月23日、申请号为08/200,152的共同未决申请所公开的,硅氧烷弹性体材料也可用于医疗领域,用于处理由烧伤、创伤或外科手术所造成的皮肤疤痕。
这些硅氧烷物质使疤痕组织软化并且在几周或几个月的时间内使这些疤痕在美容及功能方面得以改善。对这种效果的生物学机制还不甚了解。但是,人们已知该治疗性能的获得与施加于疤痕表面的压力无关。
在疤痕处理领域的通常作法是在高度重要的区域例如手或脸或其它具审美重要性的区域,将压力治疗与硅氧烷片层相结合。在这种情况下,可在最短的时间内得到最佳的效果。
在将硅氧烷片层材料衬在盖于人体复杂轮廓上的纺织品服装的里面时,出现了困难。这些困难主要涉及在硅氧烷弹性体内不能避免皱褶和折叠,病人须每天进行乏味的施用,在活动过程中须对片层进行定位及维持,以及使与皮肤的接触最佳。
本发明人的申请号为08/200,152的共同未决专利申请公开了硅氧烷和聚四氟乙烯(PTFE)相互渗透的聚合物网络(IPN),所述网络虽然厚度降低但却具有提高的物理性能。虽然在施压设备下将IPN材料无折叠无缝隙地施加于复杂轮廓上是可能的,这仍然是一项繁冗的工作,并且保持位置仍然困难。
为了本发明的目的,术语“硅氧烷弹性体”、“硅氧烷凝胶”、或者“硅氧烷IPN”是相当的,可互换使用,因为我们相信本发明可用这些物质中的任一种来完成。
发明概述
本发明涉及疤痕治疗,更具体地讲,涉及用于疤痕治疗的复合结构,所述复合结构将着衣的方便性与简单性结合在一起。并且,本发明涉及的复合结构在疤痕处理中将治疗性医疗紧身服装的压力治疗特性与硅氧烷弹性体的使用相结合。
通过在成衣前将硅氧烷弹性体材料施加于适当的纺织品的一面,本发明人意外地发现可制出能使表层硅氧烷与皮肤均匀接触的成型服装,即使在服装成型为复杂几何形状时亦可做到。成品很容易穿着,并且相对于应用位点可维持硅氧烷片层材料的位置。另外,所述材料耐用且能进行机洗和干燥。
本发明的治疗性医疗服装由与纺织品层接合的柔软的硅氧烷弹性体层制成,并且将硅氧烷弹性体层置于与病人的皮肤接触的位置上。
由于传统的服装制作方法生产用于最终成型衣物的片料,该生产方法可进行大规模生产。这就使得对成衣或依任一给定病人的体形而制的衣服可进行快速且成本节约的生产。
本发明比现有技术进步之处在于(a)将硅氧烷层与弹性纺织品表面进行接合以得到治疗性医疗服装,(b)该医疗服装易于穿着,且不会折叠或在硅氧烷层和皮肤之间产生缝隙,(c)可同时进行压力和硅氧烷治疗,而无需进行将两种物质分别与皮肤相配的单独和/或重复的过程,及(d)由于进行治疗大为便利,病人易于接受。

几个实施方案的详细描述
本发明的优选实施方案示于下面说明性的实施例中,它并无限制作用。
有许多硅氧烷组合物,无论通过组合或混合,都适用于本发明。另外,目前的临床研究表明其它物质,例如水凝胶或水胶体,用于减轻疤痕也同样有效,因此也可以用于本发明。但是,一般而言,水凝胶和水胶体以及许多含硅氧烷化合物,例如含硅氧烷共聚物的矿物油,缺乏足够的耐久性,因而不能经济合理地用于必须用好几个月的服装中。
有许多种纺织品材料可用于本发明的目的。而且,可以使用不同厚度的纺织品层和治疗剂层,并且可以使用不同的弹性组份。主要应考虑的是本发明的医疗服装具有适宜的接触性能,该性能使病人感觉穿着舒适,所述服装可施加适当的压力,并且具有有效的治疗剂,在需要治疗剂的皮肤处施用所述治疗剂。同样重要的是成品可用普通方法进行洗涤,并且产品具有足够的耐久性从而可进行重复使用,并且费用低廉。实施例1
本发明人的申请号为08/200,152的美国专利申请,公开了一种制造硅氧烷和聚四氟乙烯(PTFE)同时相互渗透的聚合物网络(IPN)的方法。该方法包括使用可脱(release)内衬作为载体基底,弹性片层可在该载体基底上形成。使该基底经过一涂布台,在其上涂布淀积一层未凝固的硅氧烷/PTFE混合物。然后使载体经过一燧道式烘箱以使硅氧烷硬化。
硅氧烷层与纺织品紧密地接合,使用机械方法无法将硅氧烷层从其表面去除。按照Military Specification-44187B试验方法4.5.5.用通常的家用洗衣机和干燥机将织物样品洗涤和干燥5次,无脱层或磨损的迹象。
从某零售处购得Playtex Apparel,Inc.生产的胸罩。将该衣服上的杯状护胸除去,并代之以本发明的硅氧烷纺织品复合材料,使复合纺织品材料的硅氧烷表面向里,以使其与病人的皮肤接触。所得到的产品是一件适于疤痕修复治疗,例如乳房整形术切口的修复治疗的衣服。
在该实施例中,将市售的胸罩进行改型,使其采用本发明的复合层。很显然可以从一开始就制造其织物护胸是由本发明的复合层组成的衣服。可采用通常的剪裁方式,将硅氧烷纺织品复合层用作适当的护胸,而通常的织物用作其余部分。另外,不同的胸罩式样可适用于类似的用途。
通过上面公开的方法可制造各种其它型式的治疗性医疗服装。其它类似的制品包括腹部整形术服装、用于皱纹切除术的脑部绷带,等等。另外,同样可以制造用于烧伤疤痕处理的各种形状的治疗性医疗用品,例如手套、面具、背心,等等。也可使用压缩力较小的服装以提高舒适性,从而在不再需要紧身服装的性能的期间使用。而且,人们设想可提供成品的片材以利于临床医师为患者剪裁。
