背景技术
双氯芬酸,即2-[(2,6-二氯苯基)-氨基]苯乙酸,是一种广泛用于抗炎、抗风湿、解热和镇痛治疗的活性成分。最常见的适应症是类风湿性关节炎、强直性脊柱炎、关节病、非关节风湿病和其他肌肉骨骼疾病(如腰椎痛(lumbosciatalgia))中的急性疼痛和炎症;其还可用于急性痛风、术后疼痛、日光性角化病、过敏性结膜炎、痛经和外伤继发炎症等。
当口服施用双氯芬酸时,由于存在有利于其吸收的pH和酶的最佳条件,其几乎可以完全被吸收;在施用后约2小时内达到血液高峰。

发明内容
在申请人进行的研究中,现在已经令人惊讶地发现,并且本发明的目的是,在其中双氯芬酸-依泊胺以等于至少2重量%的浓度存在的用于局部使用的制剂中,皮肤对双氯芬酸的吸收实体(率)出乎意料地高:特别地,其相对于载体中双氯芬酸浓度的增加以超比例的方式增加:因此获得的局部吸收显著高于基于双氯芬酸的较大量计算出的比例。考虑到在本发明的测试制剂中,卵磷脂的量相对于含有1.3%双氯芬酸-依泊胺的参考制剂没有增加,因此所发现的效果更加令人惊讶:在本发明组合物中,在潜在的更不利的吸收条件下操作,有利于吸收的胶束成分的发生因此降低。尽管如此,由于本发明提供了更高的吸收率,因此有可能以最佳方式进行需要施用高且精确剂量的双氯芬酸的治疗。
具体实施方式

除非有不同说明,本文所标注的所有百分比值并非是严格意义上的,而是在±0.2%范围内的近似变量,例如,5%的值包括从4.8%到5.2%的所有值。对于所有给定的范围,范围的上限和下限均包含在该范围内;在更有限的子实施方案中,排除了上限和下限。
本文报道的双氯芬酸-依泊胺的所有浓度都旨在以作为盐的产品本身(而不是以双氯芬酸本身)来计算。

本发明还涉及一种制备上述组合物的方法,其特征在于,在用于局部应用的合适载体和可能的共配剂和赋形剂的存在下,配制基于所述组合物的重量浓度等于或高于2重量%的双氯芬酸-依泊胺。
现通过以下非限制性实施例对本发明进行描述。
实验
1.组合物的制备
已经制备了两种局部使用的水凝胶制剂;第一种制剂中的双氯芬酸-依泊胺的浓度为1.3重量%(参考值);根据本发明的第二种制剂中的双氯芬酸-依泊胺的浓度为2.6重量%。在保持所有其他成分不变的情况下,组成如表1所示:

2.透皮吸收试验(IVPT法)
使用安装在FranzTM垂直扩散池(静态型)中的皮节人类皮肤,对实施例1组合物中双氯芬酸-依泊胺的体外透皮吸收进行定量评估。在成品剂量和封闭条件下,使用每种半固体外用产品(试验和参考)进行分析。体外渗透试验是在从12位人类供体收集的皮肤样本上进行的。每个供体使用3种产品(参考、试验)进行重复处理。具有相同表型(高加索人)的12名人类女性供体(年龄为27岁至59岁)的皮肤样本是通过腹壁整形术获得的。采集的样本在采集后立即冷冻,并保存在-20℃下,直到用于体外分析。皮肤样本解冻后于+4℃放置1小时后使用,然后在不进行任何其他处理的情况下按照随机列表安装到扩散池中。在应用凝胶产品之前,通过测量跨表皮失水(TEWL)来评估部分厚度皮肤样本的完整性。考虑到可接受的值不超过15g/h m2,测量直接在皮膜上进行。每个皮肤样本都被安装在形成两个隔室的两个池部件之间的水平位置,皮肤样本的每侧各一个:
·供体隔室具有1.767cm2的扩散表面,其大小和形状都经过仔细限定,应用在皮肤的上侧。
·接受隔室由固定容积为7ml的隔室组成,具有用于分析的样本门,并含有接受液,皮肤样本安装在其上。两个隔室对齐,并用马蹄夹固定。
根据标准方法,在皮肤表面(1.767cm2)上直接进行研究的每种制剂的应用剂量为约8.8mg。
将接受室浸没于水浴中,并保持在32±1℃。通过磁力搅拌器(800rpm,持续搅拌)确保接受室温度的均匀分布,其中扩散池安装在搅拌装置上。每次取样时,取2.5mL液体并用等体积的新鲜液体代替。接受相的样本分别在3、6、9、12、18、24、32、40和48小时后采集。
在这段时间结束时,去除残留在皮肤表面上的制剂的量,并重新评估皮肤的完整性(TEWL),以确保膜未受损。
使用用紫外检测进行验证的高效液相色谱法(HPLC)对从接受室收集到的每个液体样本中的双氯芬酸的量进行定量,以确定相对Qn(Qn_Rel),以%/cm2表示,对应于通过剂量校正的渗透总量。
获得的结果如表2所示。

据观察,当凝胶中双氯芬酸-依泊胺的浓度从1.3%变化至2.6%时,吸收剂量分数即施用剂量和吸收剂量之间的百分比的平均增加等于约+11%。值得注意的是,由此计算出的百分比值与所吸收的药物的绝对量无关(所吸收的药物的绝对量在浓缩凝胶中必然更高):在所采用的计算方法中排除了这一干扰因素,因此两种组合物的比较数据是完全可比的;它们表明根据本发明的凝胶更有能力使施用剂量得到更完全的吸收。为了便于比较,除双氯芬酸-依泊胺外,制剂的所有成分都保持不变;这导致本发明凝胶中卵磷脂/双氯芬酸-依泊胺的比例的降低;也就是说,相对于参考凝胶,卵磷脂增加双氯芬酸-依泊胺溶解度的有效性降低:尽管这种情况显然不利,但本发明凝胶记录的吸收量高于参考凝胶。
Claims (12)
1.一种用于局部使用的药物组合物,包含与依泊胺以盐形式存在的双氯芬酸(双氯芬酸-依泊胺),基于所述组合物的重量,其浓度为至少2.0重量%。
2.根据权利要求1所述的组合物,其中双氯芬酸-依泊胺的浓度范围为2.0%至7.0%。
3.根据权利要求1-2所述的组合物,其中双氯芬酸-依泊胺的浓度为2.6%。
4.根据权利要求1-3所述的组合物,其形式为水凝胶。
5.根据权利要求1-4所述的组合物,还包含一种或多种乳化剂。
6.根据权利要求5所述的组合物,包含卵磷脂。
7.根据权利要求6所述的组合物,其中所述卵磷脂以相对于双氯芬酸-依泊胺低于1:1的重量比使用。
8.根据权利要求7所述的组合物,其中所述卵磷脂以相对于双氯芬酸-依泊胺范围为0.5:1至0.9:1的重量比使用。
9.根据权利要求1-8所述的组合物,其用于治疗。
10.根据权利要求1-9所述的组合物,其用于抗炎、消炎、解热和/或抗风湿的局部治疗。
11.一种制备权利要求1-8所述的组合物的方法,其特征在于,在用于局部应用的合适载体和可能的共配剂和赋形剂的存在下,配制基于所述组合物的重量浓度等于或高于2.0重量%的双氯芬酸-依泊胺。
12.根据权利要求11所述的方法,其中所述载体是水凝胶和/或所述组合物包含卵磷脂。
