Abstract
本发明公开了一种急性肝损伤保护药物,它是以紫花前胡素为活性成分,加上药学上可接受的辅料制备而成的脂质体;所述紫花前胡素与辅料的质量比为1:14~20。本发明通过特定的辅料组成开发的负载紫花前胡素的脂质体,增强了活性成分紫花前胡素在肝脏中的保留,提高了其在体内的生物利用度。动物实验显示:相较未被脂质体包封的紫花前胡素,本发明紫花前胡素脂质体显著抑制了APAP诱导的ALT和AST活性升高,改善了肝功能,具备实际推广应用的价值。
Description
技术领域
本发明属于药物制剂领域,具体涉及一种急性肝损伤保护药物及其制备方法。
背景技术
对乙酰氨基酚(Acetaminophen,APAP)是治疗发热和疼痛的常见临床药物,在治疗剂量下被认为是安全的。但高剂量的APAP可导致急性肝损伤(Acute liver injury,ALI),严重的ALI可发展为急性肝衰竭(Acute liver failure,ALF),甚至导致死亡。APAP诱导的急性肝衰竭在北美占ALF病例的45.7%,在英国占65.4%。因此,APAP诱导的肝损伤是一个重要的临床问题。N-乙酰半胱氨酸(N-acetyl cyste i ne,NAC)是FDA批准的唯一针对APAP过量的解毒。然而,NAC的疗效有限,可能引起副作用。因此,迫切需要寻找新的治疗方法来补充。
大量的研究发现,多种天然药物可通过干预不同发病机制在药物诱导的肝损伤治疗中发挥着重要作用,为治疗ALI提供了许多候选药物。紫花前胡素(Decursin,Dec)是当归中的主要药理活性成分,已被鉴定出多种药理学特性,包括抗癌、抗氧化剂、神经保护和抗炎。李宝红等,decursin对CCl4致小鼠肝损伤的保护作用[J],山东化工,2019,48(12),95~98,通过动物实验证明紫花前胡素具有改善化学性急性肝损伤的作用,然而,decursin是一种水溶性差的药物,具有低生物利用度和在肝脏中的保留不足,这阻碍了其在体内的作用效果。因此,为了克服这些限制,需要适当的递送系统来提高decurin的生物利用度并实现肝脏靶向。
脂质体是由两亲性磷脂组成的具有与细胞膜相似的结构的脂质双分子层,作为一种药物递送的载体,具有多种优点。首先,脂质体可通过将疏水性药物包封于脂质双分子层,从而改善疏水药物的水溶性。其次,可以通过PEG修饰,使得小分子药物的半衰期延长。例如,盐酸阿霉素脂质体Doxil在小鼠体内使游离药半衰期从0.29h提高到3.2h。第三,脂质体具有良好的生物相容性,安全性好,成为首个获批上市且数量最多的纳米药物。最后,脂质体作为一种纳米颗粒,由于全身给药后大多数纳米颗粒被动地积聚在肝脏中,这使得脂质体成为将药物靶向到肝脏用于肝病干预的固有优势。但如果不能改善脂质体的物理稳定性,提高脂质体的包封率,无疑会给脂质体的临床应用带来很大阻碍。
目前还没有应用递送系统来提高Decursin的生物利用度,提高decursin的肝靶向效率及其肝保护作用,更没有利用脂质体作为Decursin递送系统提高改善肝脏损伤的报道。
发明内容
为解决上述问题,本发明提供了一种急性肝损伤保护药物,它是以紫花前胡素为活性成分,加上药学上可接受的辅料制备而成的脂质体。
进一步地,所述辅料由蛋黄卵磷脂、胆固醇和DSPE-mPEG2000组成。
更进一步地:所述蛋黄卵磷脂、胆固醇和DSPE-mPEG2000的质量比为7~9:2~4:1。
更进一步地,所述蛋黄卵磷脂、胆固醇和DSPE-mPEG200 0的质量比为7.5:2:0.5。
进一步地,所述紫花前胡素与辅料的质量比为1:14~20,优选1:20。
本发明还提供了一种制备前述药物的方法,它包括如下步骤:
1)按比例称取紫花前胡素、蛋黄卵磷脂、胆固醇和DSPE-mPEG2000,溶解于溶剂,再蒸发除去溶剂,得薄膜;
2)取步骤1)所得薄膜分散于PBS中超声,离心,即得。
进一步地,步骤1)所述溶剂为甲醇-氯仿;所述甲醇-氯仿的体积比为2:3~7,优选2:5。
进一步地,步骤2)所述超声是在冰浴中超声,时间10~20分钟,期间每超声5秒就停止5秒;所述离心的速度18000×g,时间5~30分钟,温度2~8℃;所述PBS的pH值为6.5~7.5,优选7.3;所述薄膜分散于PBS中使脂质浓度为10~50mmol/L,优选20mmol/L。
本发明最后提供了一种前述药物在制备保护受损肝脏的药物中的用途。
进一步地,所述药物是对急性化学肝损伤具有保护作用的药物。
更进一步地,所述药物是对对乙酰氨基酚导致的肝损伤具有保护作用的药物。
本发明通过特定的辅料组成开发的负载紫花前胡素的脂质体,稳定性好,包封率高,将其应用于急性肝损伤,增强了活性成分紫花前胡素在肝脏中的保留,提高了其在体内的生物利用度。动物实验显示:相较未被脂质体包封的紫花前胡素,本发明紫花前胡素脂质体显著抑制了APAP诱导的ALT和AST活性升高,改善了肝功能,具备实际推广应用的价值。
显然,根据本发明的上述内容,按照本领域的普通技术知识和惯用手段,在不脱离本发明上述基本技术思想前提下,还可以做出其它多种形式的修改、替换或变更。
以下通过实施例形式的具体实施方式,对本发明的上述内容再作进一步的详细说明。但不应将此理解为本发明上述主题的范围仅限于以下的实例。凡基于本发明上述内容所实现的技术均属于本发明的范围。

具体实施方式
实施例1本发明药物的制备
1)取EPC、CHO和DSPE-mPEG2000,按质量比7.5:2:0.5的比例混合;
2)取步骤1)所得混合物与Dec按质量比20:1的比例混合,溶解在甲醇-氯仿混合物(2/5,v/v)中,37℃真空下通过旋转蒸发除去溶剂后,形成薄膜,然后将薄膜分散在pH7.3的PBS中,使脂质终浓度为20mM,再在冰浴下,使用250W的探头超声仪对混合物超声处理12分钟(开:5秒;关:5秒),最后离心10分钟(18000×g,4℃)以去除未包封的Dec,即得。
以下通过试验例来说明本发明的有益效果。
试验例1紫花前胡素脂质体的制备及其在急性肝损伤中的应用研究一、方法
1材料和方法
1.1材料
紫花前胡素(Decursin,Dec)购自成都普瑞法股份有限公司(中国成都);蛋黄卵磷脂(EPC)、胆固醇(CHO)、氢化大豆磷脂(HSPC)和DSPE-mPEG2000购自AVT(中国上海);1,1′-二十八烷基-3,3,3′,3-四甲基吲哚三碳菁碘化物(DiR)购自宇恒(中国苏州)。使用的所有其他化学试剂均为AR级。
1.2脂质体的制备
使用薄膜水化法制备脂质体。将Dec与磷脂、CHO和DSPE-mPEG2000混合,并溶解在甲醇-氯仿混合物(2/5,v/v)中。在37℃真空下通过旋转蒸发除去溶剂后,形成薄膜,然后用适量pH7.3的PBS水化(即将薄膜分散在PBS中),使脂质终浓度为20mM。再在冰浴下,使用250W的探头超声仪对混合物超声处理12分钟(开:5秒;关:5秒),然后离心10分钟(18000×g,4℃)以去除未包封的药物。获得的Lip-Dec的脂质体悬液(图1A)通过过滤灭菌,并保存在4℃。
脂质体的生物分布实验所需的负载荧光染料DiR的脂质体(Lip-DiR)也参照上述方法制备,其中使用荧光染料DiR代替紫花前胡素Dec。
1.3、制备的脂质体的表征
1.3.1粒度、多分散指数和zeta电位
脂质体制剂的粒径、多分散指数(PDI)和zeta电位通过使用zeta粒度分析仪(Nanobrook Omni,Brookhaven,US)的动态光散射测量。
1.3.2透射电子显微镜
脂质体的形态通过透射电子显微镜(HT-7800,日立,日本)进行表征,使用2%磷钨酸负染进行阴性染色。
1.3.3包封率
包封率(EE)使用以下公式计算:EE(%)=(封装药物的重量/添加的药物的初始重量)×100。Lip-Dec中的药物量通过以下方法确定。脂质体用甲醇裂解并将包封的药物释放到分析溶液中,通过18000×g离心5分钟(4℃)获得上清液,使用高效液相色谱(HPLC,Agilent,US)在330nm波长下使用FLM C18柱(150×4.6mm,5μm)分析药物含量。流动相由H2O和甲醇的混合物(30/70,v/v)组成,以1mL/min的流速用于HPLC。
1.3.4稳定性
脂质体在4℃下保存14天的稳定性。在1、3、5、7和14天后取出样品,并监测尺寸、PDI和zeta电位。
1.3.5体外释放
使用透析法研究脂质体的体外释放特性。将1mL Lip-Dec加入透析管(MWCO:10kDa,Millipore,US)中,并浸入45mL释放介质(0.01M PBS+0.5%Tween-80,pH=7.4)。将透析管转移到37℃的摇床上,转速为120rpm。在设定时间点(0.25、0.5、1、2、4、6、8、12、24小时),取出1mL释放介质,并加入等体积的预热新鲜释放介质。同时,从甲醇溶液中释放的游离Dec用作对照。如上所述,用HPLC系统进行Dec的浓度测定。
1.3.6Lip-Dec的血液相容性检测
(1)取小鼠全血于抗凝管离心10min(1000rpm,4℃),弃上清,加2mL生理盐水,混匀再离心,去掉上清液,重复这个步骤,直至离心后上清液透亮,制备压积红细胞。
(2)然后取80μL压积红细胞加入4mL生理盐水混匀,制备得到2%红细胞悬液。
(3)反应液:将500μL 2%红细胞悬液加入500μL不同浓度的Lip-Dec中,37℃水浴中孵育3小时,1500rpm,6分钟以沉淀红细胞,取200μL上清液540nm测定OD值;此外,分别以加入500μL超纯水和PBS作为阳性对照和阴性对照。
计算方法:溶血分数(%)=[(A样品–A阴性对照)/(A阳性对照-A阴性对照)]×100%
2、动物实验
8-10周龄的雄性C57BL/6小鼠用于实验。所有动物实验均符合四川大学华西医院实验动物护理和使用指导原则。本研究的实验方案由四川大学伦理委员会评估并批准。
2.1脂质体的生物分布
健康小鼠和APAP诱导的ALI小鼠静脉内给予Lip-DiR或游离DiR,剂量为1.25mgDiR/kg(b.w.)。使用体内成像系统(Lumina 3,PerkinElmer,US)在设定的注射后时间点采集荧光图像。在APAP给药后24小时处死小鼠,并收集心脏,肝脏,脾脏,肺和肾脏。使用体内成像系统光谱系统分析荧光。
2.2APAP诱导小鼠脂质体的干预
将动物随机分为五组,其中四组在禁食过夜后腹腔注射300mg/kg APAP,以诱导急性肝损伤。两个药物干预组在APAP给药前静脉注射3mg/kg Dec/Lip-Dec。两组注射PBS和空脂质体(Lip)。未经任何处理的健康小鼠被设为正常组。APAP给药24小时后,采集肝组织和血液样本用于后续分析。
2.3组织学分析
通过苏木精-伊红(H&E)染色分析肝损伤。简而言之,将组织置于4%多聚甲醛中,然后脱水、石蜡包埋和切片。此后,用H&E进行染色。然后在光学显微镜(尼康Eclipse 80i,尼康,日本)下观察。
2.4血清ALT和AST活性测定
通过检测血清中ALT和AST的水平来评估肝功能。通过以3500rpm离心10分钟从血液中获得血清。根据试剂盒((中国南京建成)说明书,测定血清ALT和AST水平。
2.5RT-PCR分析
使用MolPure TRIeasy Plus Total RNA试剂盒(Yeasen,中国上海)从肝组织中提取总RNA,然后使用iScript cDNA合成试剂盒(TaKaRa,日本京都)将其逆转录为cDNA。通过SYBR Green在QuantStudio实时系统(Applied Biosystems QuantStudio 6,US)进行qPCR。
qRT-PCR中使用的引物序列如下:
2.6统计分析
数据表示为平均值±SD。使用Prism 6(美国GraphPad软件)进行统计分析。通过单因素方差分析(ANOVA)和学生t检验分析各组间的差异。
二、结果
1、Lip-Dec制备条件的优化
脂质体通常是将药物包埋在脂质双层中形成的微泡,其中形成脂质双层的材料很多,如蛋黄卵磷脂、大豆卵磷脂、氢化大豆卵磷脂、胆固醇、二棕榈酰磷脂酰胆碱、鞘髓磷脂、单双甘油脂肪酸酯、双十二烷基二甲基溴化铵、PEG2000、DSPE-PEG2000等等。由于药物活性成分理化性质不同,选择的可包埋的脂质双层的材料不尽相同。
通过前期的大量筛选,最终确定磷脂、胆固醇(稳定磷脂膜)和DSPE-mPEG2000(作为长循环材料)组合作为Dec脂质体的辅料能形成封闭的双分子层的囊泡。在此基础上对磷脂的种类进一步筛选发现,磷脂种类显著影响到脂质体包封药物的效果,经过尝试最终从多种磷脂中选择蛋黄卵磷脂EPC和氢化大豆卵磷脂HSPC这两种在获批脂质体药物中的磷脂组分制备脂质体,测定包封率,结果见表1。从表1可见,使用EPC时,包封率显著高于使用HSPC。

确定Dec脂质体的辅料成分后,通过改变磷脂与胆固醇的比率以及药物与脂质的比率,制备具有不同配方的脂质体。从表2可见,随着磷脂与胆固醇比例增加,包封率(EE)增加,Lipo6-9的EE均高于90%,但配方为Lipo7-9时,在24h内观察到沉淀产生,提示这些配方脂质体的稳定性不佳。因此,我们选择Lipo 6作为最优配方。

2、优化脂质体的表征
动态光散射(d ynam i c light scattering,DLS)结果(图1B)表明,Lip-Dec粒径为优化脂质体的尺寸为97.17±1.47nm,PDI为0.204±0.004,表明颗粒均匀分散。Lip-Dec的zeta电位为负(-25.41±5.13mV),这是由于添加了DSPE-mPEG2000所致。TEM数据(图1C)证实,Lip-Dec的形状接近球形,Lip-Dec的直径与DLS结果一致。在4℃下保持14天后,优化脂质体的粒径、PDI和zeta电位没有显著变化(图2)。
图3显示了游离Dec和Lip-Dec的体外释放曲线。8小时后游离Dec的累积释放率约为80%,12小时后完全释放。与游离Dec相比,Lip-Dec缓慢地从透析袋中释放药物,24小时后仅有约60%的药物被释放。
3.Lip-Dec的血液相容性
为探讨静脉注射Lip-Dec的安全性,进行了溶血实验。如图4所示,纯水导致红细胞破裂,血红蛋白从细胞中泄漏(图4A),形成透明的红色溶液。但PBS溶液保持无色,说明红细胞结构完整,未发生溶血。Lip-Dec孵育的样品大多呈现无色或浅粉色。同时测定各组上清液的吸光度,计算溶血率。160-1600μg/mL Lip-Dec的溶血率均低于阈值(5%)(图4B),提示静脉注射Lip-Dec时具有一定生物安全性。
4、脂质体的生物分布
使用含有DiR标记的脂质体对脂质体药物在动物体内的生物分布进行研究。图5A显示了一系列时间间隔的体内生物分布。与游离DiR相比,在正常和APAP诱导的急性肝损伤小鼠中,Lip-DiR组在2h至24h的时间段内在小鼠体内显示出更高的荧光强度(图5A)。此外,与健康小鼠相比,APAP诱导的急性肝损伤小鼠中Lip-DiR组的荧光强度更强(图5B)。24小时时,收集器官并使用IVIS系统检测其荧光强度。如图5C所示,Lip-DiR组肝脏中积累的荧光强度比游离DiR组中的荧光强度更强;此外,Lip-DiR组肝脏中荧光强度比其它器官中的更高,而游离DiR在肺部的平均荧光强度更高。与健康组相比,Lip-DiR在受损肝脏中的荧光强度更高。这些结果提示,本研究中的所采用的脂质体可以促进其装载的药物向受损肝脏的靶向递送。
5、Lip-Dec对APAP诱导小鼠肝脏组织病理学改变的影响
采用APAP诱导的急性肝损伤小鼠模型探讨Lip-Dec的干预效果。我们使用苏木精和伊红(H&E)染色的肝脏切片评估肝脏损伤。如图6a~b所示,正常肝脏没有炎症或坏死,显示出清晰的小叶结构和中央静脉。APAP给药导致血管周围的肝脏结构受损。Lip-Dec的干预改善了肝脏结构,减少了肝脏坏死面积,而Lip和游离的Dec没有显著影响。注射APAP可导致肝细胞坏死,从而释放出ALT和AST等肝细胞酶。如图6c~d所示,与正常组相比,注射APAP后,血清中的ALT和AST水平显著升高。Lip、Dec和PBS组之间没有显著差异。Lip-Dec干预显著抑制了APAP诱导的ALT和AST活性升高,改善了肝功能。
6、Lip-Dec对炎症因子的影响
为了确定Lip-Dec是否能减轻APAP诱导的ALI中的炎症,我们分析了肝脏中炎症细胞因子的表达水平。如图7A所示,APAP诱导后,肝组织中促炎因子TNF-α和IL-6的mRNA水平显著升高,Lip-Dec和游离Dec降低了TNF-α和IL-6的表达水平并且Lip-Dec比游离Dec在更大程度上降低了促炎因子水平。此外,抗炎因子IL-10的mRNA表达水平在APAP诱导后显著降低,与模型组相比,Lip-Dec增加了抗炎因子IL-10的水平(图7B),游离的Dec干预组的IL-10的水平没有显著升高。这些结果表明,Lip-Dec可以通过更有效地抑制促炎因子表达和促进抗炎因子表达来减轻肝脏炎症反应,对APAP诱导的肝损伤发挥比Dec更强的保护作用。
综上,本发明以特定比例的蛋黄卵磷脂、胆固醇和DSPE-mPEG2000为辅料开发的负载紫花前胡素的脂质体,稳定性好,包封率高,将其应用于急性肝损伤,增强了活性成分紫花前胡素在肝脏中的保留,提高了其在体内的生物利用度,在急性肝损伤保护方面效果显著。
Claims (8)
1.一种急性肝损伤保护药物,其特征在于:它是以紫花前胡素为活性成分,加上药学上可接受的辅料制备而成的脂质体;
所述辅料由蛋黄卵磷脂、胆固醇和DSPE-mPEG2000组成;
所述蛋黄卵磷脂、胆固醇和DSPE-mPEG2000的质量比为7.5:2:0.5;
所述紫花前胡素与辅料的质量比为1:20。
2.一种制备权利要求1所述药物的方法,其特征在于:它包括如下步骤:
1)按比例称取紫花前胡素、蛋黄卵磷脂、胆固醇和DSPE-mPEG2000,溶解于溶剂,再蒸发除去溶剂,得薄膜;
2)取步骤1)所得薄膜分散于 PBS中超声,离心,即得。
3.根据权利要求2所述的方法,其特征在于:步骤1)所述溶剂为甲醇-氯仿;所述甲醇-氯仿的体积比为2:3~7。
4.根据权利要求3所述的方法,其特征在于:所述甲醇-氯仿的体积比为2:5。
5.根据权利要求2所述的方法,其特征在于:步骤2)所述超声是在冰浴中超声,时间10~20分钟,期间每超声5秒就停止5秒;所述离心的速度18000×g,时间5~30分钟,温度2~8℃;所述PBS的pH值为6.5~7.5;所述薄膜分散于 PBS中使脂质浓度为10~50mmol/L。
