技术领域
本发明涉及一种药物组合物,其包括最适合用于具有特定的基因标志的癌症等的患者的O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸或其药学上可接受的盐。
而且,本发明涉及一种癌症等的治疗方法,包括对具有特定的基因标志的癌症等的患者给予包含O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸或其药学上可接受的盐的药物组合物的步骤。
背景技术
由于异烟肼神经毒性的个体差异是由N-乙酰化能力的遗传差异引起的,因此在50多年前发现了N-乙酰化多态性。除了异烟肼之外,许多芳香胺类药物如磺胺二甲嘧啶(Sulphamethazine)受到乙酰化多态性的影响,影响许多治疗剂的治疗效果和毒性。N-乙酰化转移酶2(NAT2)是将乙酰辅酶A的乙酰基转移至作为该酶的底物的含芳香族胺、肼的化合物的酶。NAT2由870-bp基因(NAT2)编码,并且NAT2的基因多态性在人类中是常见的。
而且,已知NAT2存在乙酰化速度快速型(Rapid)、中速型(Intermediate)及缓慢型(Slow)这三种类型(SNP:single nucleotide polymorphism,单核苷酸多态性)(Pharmacogenomics,13卷:31-41页,2012年)。这些类型可以通过其基因多态性来区分。
已知NAT2的基因多态性改变了许多芳基胺和肼类药物的有效性和毒性,并增加了许多芳基胺致癌物相关癌症的风险。另外,已知柳氮磺胺吡啶(SSZ)这样的药剂被NAT2乙酰化,但在乙酰化慢的患者中经常容易发生副作用(J Rheumatol.2002年,Dec;29(12):2492-2499)(Genotyping the NAT2 gene followed by estimation of diplotypeconfiguration before administration of SSZ is likely to reduce the frequencyof adverse effects in Japanese patients with RA.http://www.jrheum.org/content/29/12/2492)。
代表性的NAT2的基因多态性有七个SNP:(i)191G>A(rs1801279),(ii)282C>T(rs1041983),(iii)341T>C(rs1801280),(iv)481C>T(rs1799929),(v)590G>A(rs1799930),(vi)803A>G(rs1208),(vii)857G>A(rs1799931)。其中,SNP(i)、(iii)、(v)、(vi)和(vii)导致氨基酸变化(即,(i)191G>A从精氨酸变为谷氨酸,(iii)341T>C从异亮氨酸变为苏氨酸,(v)590G>A从精氨酸变为谷氨酰胺,(vi)803A>G从赖氨酸变为精氨酸,(vii)857G>A从甘氨酸变为谷氨酸)。
另一方面,胆道癌近年来在日本有增加的趋势。2016年的死亡人数约为18,000人,成为癌症死亡原因的第7位。胆道癌根据发生部位的不同,治疗方针、化学疗法的效果也不同。作为化学疗法,目前将联合使用吉西他滨(Gemcitabine)和顺铂的疗法作为标准化学疗法。然而,目前尚未确立对标准化学疗法无效的胆道癌患者推荐的二线治疗方法,对于作为二线治疗方法的化学疗法的临床需求高。
O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸(以下,有时称为“JPH203”)具有对在癌细胞中特异性表达的L型-氨基酸转运蛋白1(LAT1)的选择性抑制作用,因此期待作为新的抗肿瘤剂的效果(专利文献1)。目前,JPH203正在由申请人以胆道癌患者为对象在日本进行II期试验。

发明内容
技术问题
发明人等在对针对标准化学疗法无效的胆道癌患者的新的二线化学疗法进行研究的过程中,根据在日本的第I期试验中得到的JPH203的药代动力学参数分析结果发现,在单位体表面积给予相同给药量的受试者之间,与血液中的JPH203浓度相比,血中以及尿中的N乙酰化体浓度的个体差异大,进一步进行继续研究,发现NAT2的基因多态性中的乙酰化速度的差异与JPH203的安全性和药效之间存在关联,从而完成了本发明。
技术方案
也就是说,本发明提供一种用于在对象的疾病的治疗中使用的药物组合物,其包含O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸或其药学上可接受的盐,向具有非快速(缓慢和/或中速)型的NAT2基因的所述对象给予所述药物组合物。
另外,本发明提供一种对象的疾病的治疗方法,包括在所述对象被鉴定为具有非快速(缓慢和/或中速)型的NAT2基因后,向所述对象给予包含O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸或其药学上可接受的盐的药物组合物。
此外,本发明提供O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸或其药学上可接受的盐在制备用于治疗对象的疾病的药物中的用途,其特征在于,在所述对象被鉴定为具有非快速(缓慢和/或中速)型的NAT2基因之后,向所述对象给予包含O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸或其药学上可接受的盐的药物组合物。
进而,本发明提供一种预测O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸是否对特定的癌等对象发挥效果的方法,其通过判定在对象的NAT2基因中是否具有非快速(缓慢和/或中速)型的NAT2基因来进行。
另外,本发明提供O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸或其药学上可接受的盐在制造特定疾病的治疗用的药物中的用途,其中,包括在利用该药物治疗特定疾病之前,判定对象的NAT2基因中是否具有非快速(缓慢和/或中速)型的NAT2基因,并预测O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸是否对特定疾病的对象发挥效果。
本发明提供一种基因诊断试剂盒,其用于判定在对象的NAT2基因中是否具有非快速型的NAT2基因,以便用来诊断O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸是否对特定的疾病发挥效果,并且提供与该试剂盒或NAT2基因诊断组合使用的、包含O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸或其药学上可接受的盐的药物组合物。
术语“对象”和“患者”在本发明中可互换使用,并且优选是人类。
发明效果
通过本发明的药物组合物,能够提供提高药剂的安全性、且提高效果的奏效性的癌症等的治疗。

以下,对本发明进行详细说明。
O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸(JPH203)
作为本发明的有效成分的O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸是在专利文献1中公开的化合物,目前申请人等正在进行第II期试验。
应予说明,静脉给药O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸(JPH203)后,主要被肝脏NAT2代谢(乙酰化)而成为乙酰体(Nac-JPH203),其生理活性大幅降低(Drug Metab Pharmacokinet.2012年;27(1):155-61页)。
JPH203也可以以其药理学上可接受的盐的形式使用。例如,JPH203的分子中的氨基与酸形成盐。作为这样的盐,没有特别限定,例如可列举出与盐酸、硫酸等无机酸、羧酸的盐。其中,优选盐酸盐。
药物组合物
本发明的药物组合物包含JPH203或其药理学上可接受的盐作为有效成分。另外,可以根据需要包含医药品添加物。本发明的药物组合物可以以片剂、颗粒剂、细粒剂、粉剂、胶囊剂等固体制剂或液剂、胶冻剂、糖浆剂等形态经口给药,或者,该药物组合物药物也可以以注射剂、经鼻剂、栓剂、吸入剂、经皮剂等形态进行非经口给药。
特别是,由于本发明的药物组合物能够作为抗癌剂等使用,因此优选以注射剂的形式被制剂化。作为这样的注射剂,可例示在本发明的有效成分中包含pH调节剂、及环糊精类的注射剂。
作为具体的注射剂,例如可以列举静脉、皮下、皮内、肌肉内注射剂、滴注静脉注射剂。
作为可以在本发明的注射剂中配合的pH调节剂,例如可以举出碳酸钠、碳酸钾、氢氧化钠、氢氧化钾这样的碱金属氢氧化物、氢化钠、氢化钾这样的碱金属氢化物、碱金属或碱土金属的碳酸盐等,特别优选氢氧化钠和碳酸钠,更优选氢氧化钠。
所述注射剂可以使用pH调节剂适当调节至适当的pH。本实施方式的注射剂的pH优选为3~6,更优选为3~5,进一步优选为3~4.5,特别优选为3.5~4.5。
作为可在本发明的注射剂中配合的环糊精类,例如可列举出未修饰环糊精、修饰环糊精。作为未修饰环糊精,可列举例如α-环糊精、β-环糊精、γ-环糊精等。另外,作为修饰环糊精,例如可举出二甲基-α-环糊精、二甲基-β-环糊精、二甲基-γ-环糊精、羟丙基-α-环糊精、羟丙基-γ-环糊精、磺丁基醚-α-环糊精、磺丁基醚-β-环糊精、磺丁基醚-γ-环糊精、麦芽糖基-α-环糊精等。
环糊精类可以单独使用1种,另外也可以任意组合使用2种以上。从即使在溶解于非强酸性的水溶液的情况下也能够减少所形成的不溶性微粒数的观点及改善冻干制剂在非强酸性的水溶液中的再溶解性的观点考虑,环糊精类优选为羟丙基-β-环糊精或磺丁基醚-β-环糊精,更优选为磺丁基醚-β-环糊精。
在此,磺丁基醚-环糊精为下述式1所示的结构,环状结构的内侧的疏水性高。因此,与同样疏水性高的O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸通过疏水性相互作用而构成复合体。本说明书中提及的“磺丁基醚-环糊精复合物”是指通过如上所述的疏水相互作用而成的复合物。

或者,也可以将作为有效成分的O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸以其磺丁基醚-环糊精复合体(以下,称为“JPH203-SBECD”)的形式用于注射剂中。
根据需要,本发明的注射剂中还可以进一步添加缓冲剂、悬浮剂、助溶剂、稳定剂、等渗调节剂、防腐剂等。
作为缓冲剂,可列举例如硼酸缓冲剂、磷酸缓冲剂、柠檬酸缓冲剂、乙酸缓冲剂、Tris缓冲剂等。
作为悬浮剂,可列举甲基纤维素、聚山梨醇酯80、羟乙基纤维素、阿拉伯胶、黄蓍胶粉末、羧甲基纤维素钠、聚氧乙烯脱水山梨糖醇单月桂酸酯、泊洛沙姆、羟丙基甲基纤维素(HPMC)、海藻酸钠等。
作为增溶剂,可列举聚氧乙烯氢化蓖麻油、聚山梨醇酯80、烟酰胺、聚氧乙烯失水山梨醇单月桂酸酯、聚乙二醇(macrogol)、甘油脂肪酸酯、脂氨基酸(lipoaminoacid)、聚乙二醇(polyethylene glycol)等。
作为稳定剂,可举出亚硫酸钠、偏亚硫酸钠等,作为等渗调节剂,可举出甘油、氯化钠等,并且,作为防腐剂,可举出对羟基苯甲酸甲酯、对羟基苯甲酸乙酯、山梨酸等。
作为将冻干制剂溶解于水时的pH,优选为3~6,更优选为3~5,进一步优选为3~4.5,特别优选为3.5~4.5。
上述冻干制剂可以通过以往公知的冻干制剂的制造方法来制作,可列举出例如在-25℃以下的温度下冷冻后,一边将真空度保持为约20Pa以下,一边升温至达到室温,并同时进行干燥的方法等。
本发明的注射剂可以是冻干制剂。这样的冻干制剂可以在使用时溶解在例如注射用蒸馏水、输液、电解液中的一种或两种或更多种溶剂中,以用作使用时溶解型注射剂。
本发明的药物组合物可以用于特定疾病的治疗,作为成为对象的疾病,例如可以列举纤维瘤、脂肪瘤、粘液瘤、软骨瘤、骨瘤、血管瘤、血管内皮瘤、淋巴瘤、骨髓瘤、骨髓肉瘤、网状细胞瘤、网状细胞肉瘤、黑色素瘤、肌瘤、神经瘤、神经胶质瘤、神经鞘瘤、肉瘤、骨肉瘤、肌肉瘤、纤维肉瘤、乳头瘤、腺瘤、囊肿、脑肿瘤、颈癌、舌癌、咽癌、喉癌、甲状腺癌、食道癌、肺癌、乳腺癌、胃癌、十二指肠、空肠、回肠等小肠癌、结肠、盲肠、直肠癌等大肠癌、膀胱癌、肾癌、胆囊癌、前列腺癌、子宫体癌、宫颈癌、卵巢癌、间皮瘤、头颈癌、肾上腺癌、消化道间质肿瘤、阑尾癌、胆道癌、胰腺癌、肝癌、皮肤癌、绒毛癌、头颈癌、转移性癌、以及浸润性癌等癌瘤,以及它们的混合肿瘤、转移肿瘤等。
其中,优选胆道癌、大肠癌、胰腺癌、食道癌、乳腺癌、肺癌、前列腺癌、膀胱癌、脑肿瘤、胃癌、肝癌、皮肤癌、绒毛癌、肾癌、头颈癌、舌癌、转移性癌和浸润癌等,进一步推荐胆道癌、大肠癌、胰腺癌、食道癌和乳腺癌。
给药方案
应用本发明的药物组合物的给药方案将取决于多种因素,包括患者的类型、种族、年龄、体重、性别和医学状况;待治疗病症的严重程度;给药途径;以及患者的肝功能和肾功能。医生可以容易地确定预防、阻止或停止症状进展所需的药物的有效量并开具处方。
本发明的药物组合物中,有效成分的给药量可以根据症状的程度、患者的年龄、性别、体重、敏感性差异、给药时期、给药间隔等适当选择,从有效性和安全性的观点出发,可例示单次1mg/m2~60mg/m2(体表面积),优选例示12.5mg/m2~60mg/m2、12.5mg/m2~25mg/m2、或10mg/m2~40mg/m2(体表面积),特别推荐25mg/m2。另外,根据症状,也可以减量至12.5mg/m2等。
作为本发明的药物组合物的给药方案的例子,可以列举例如:
-以连续给药恒定时间然后停药恒定时间作为1个周期来给予药物组合物,
-以连续给药5天然后停药9天共计14天作为1个周期来给予药物组合物,
-以恒定时间连续静脉给药恒定量的药物组合物,
-用90分钟连续静脉给药100mL药物组合物,
-以1~60mg/m2向患者给药,
-以12.5~60mg/m2向患者给药,
-以12.5~25mg/m2向患者给药,
-以25mg/m2向患者给药,
等,可以根据需要将它们组合应用。
本发明的药物组合物在对具有非快速(缓慢和/或中速)型的NAT2基因的(即,由NAT2引起的乙酰化缓慢的)癌症等的患者给药时,能够发挥优良的效果。在本说明书中,“非快速型的NAT2基因”是指NAT2基因的乙酰化速度的快速型(Rapid)、中速型(Intermediate)及缓慢型(Slow)这三种表型中的中速型(Intermediate)和缓慢型(Slow)的表型。
对于代表NAT2基因多态性的7个SNP(481C>T,282C>T,857G>A,803A>G,341T>C,590G>A,191G>A):用2-SNP、3-SNP和4-SNP的组合(表2-1)分析了NAT2的乙酰化表型(Phenotype),由NAT2*4纯合子组成的无SNP(标准)单倍型被认为是乙酰化的快速型(Rapid),任一个单倍型突变的杂合子被认为是乙酰化速度为中速的类型(Intermediate),并且,其中所有单倍型突变的纯合子或两个或更多个单倍型突变的杂合子的组合被认为是乙酰化的缓慢型(Slow)(Pharmacogenomics,13卷:31-41页,2012年)。此处,“NAT2*4”等位基因是指无SNP的单倍型。另外,根据Pharmacogenomics,13卷:31-41页,2012年,关于快速型,除了“NAT2*4”以外,具有不引发氨基酸突变的“NAT2*11、NAT2*12、NAT2*13”的患者也被分类为快速型。
具体来说,两种SNP推断的NAT2乙酰化因子的表型由两种SNP:rs1041983(282C>T)和rs1801280(341T>C)的分析确定。三种SNP推断的NAT2乙酰化因子表型由三种SNP:rs1799929(481C>T)、rs1799930(590G>A)和rs1799931(857G>A)的分析确定。四种SNP推断的NAT2乙酰化因子表型由四种SNP:rs1801279(191G>A)、rs1801280(341T>C)、rs1799930(590G>A)和rs1799931(857G>A)的分析确定。
如果JPH203被NAT2乙酰化,则其对癌细胞的抑制活性降低。但是在患者主要具有难以被乙酰化的NAT2基因表型的情况下,也就是说,在癌症等患者的NAT2基因具有乙酰化速度的中速型(Intermediate)和缓慢型(Slow)的表型的情况下(Non-Rapid型的NAT2基因),因为JPH203的乙酰化速度变得缓慢,所以难以被乙酰化,JPH203的活性可持续得久。
本发明中的NAT2基因多态性的分析可以通过以往公知的方法进行。例如,可以通过从受试者的血液中提取DNA,利用微阵列法处理DNA,然后读取基因型进行检查,接着进行数据分析来进行。
因此,出于预测癌症治疗药的安全性和奏效性并向合适的癌症等患者提供治疗药的目的,本发明的药物组合物对癌症患者的给药例适合以如下方式进行。
对罹患癌症或疑似罹患癌症的人类患者进行采血,从血液样品中提取DNA,并确认NAT2的基因多态性。对于NAT2的遗传多态性被判断为非快速型(即,被判断为乙酰化速度为中速型(Intermediate)或乙酰化缓慢型(Slow))的患者,用90分钟以25mg/m2的浓度连续静脉给药100mL的O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸磺丁基醚-环糊精复合物。此时,以连续给药5天然后停药9天共计14天作为1个周期来给予药物组合物。
而且,在患者的NAT2基因诊断中,通过判定是否具有非快速型的NAT2基因,能够预测O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸是否对特定疾病、例如癌等的患者发挥效果。
另外,与NAT2的代谢能力正常的动物模型相比,在抑制了NAT2的代谢能力的动物模型、动物种类中,癌症等的治疗效果有可能变得良好。这也有可能适用于人类。
进而,通过将与本发明的药物组合物相关的给药方法应用于患者,能够提供提高药剂的安全性并提高效果的奏效性的、针对特定疾病、特别是癌症等的优良的治疗。
并且,作为本发明的进一步的方式,提供与NAT2基因诊断组合使用的药物组合物,其包含O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸或其药学上可接受的盐。该药物组合物在用于特定疾病、特别是癌症等的治疗时能够提高其效果。
应予说明,如上所述,NAT2基因诊断是进行NAT2基因多态性的检查,判定是否具有非快速型的NAT2基因。NAT2基因诊断可以通过以往公知的诊断方法、例如后述的NAT2基因多态性的分析方法来进行。或者,NAT2基因诊断可以利用提供基因检查、基因诊断的企业等检查机构的服务。
或者,NAT2基因诊断也可以通过NAT2基因诊断试剂盒来进行。NAT2基因诊断试剂盒是将为了实施后述的NAT2基因多态性的分析方法所需的样品采集器具、试剂汇总而成的。作为试剂盒所具备的试剂,可举出例如PCR酶液、引物/探针、dH2O、阳性对照DNA、及对照稀释用专用缓冲液等。
作为另一个实施方式,本发明的包含O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸或其药学上可接受的盐的药物组合物可与NAT2基因诊断或基因诊断试剂盒组合使用。通过将它们组合使用,能够提高O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸对特定疾病的效果、特别能够提高治疗癌症时的效果。
作为进一步的实施方式,提供通过将包含本发明的O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸或其药学上可接受的盐的药物组合物与NAT2基因诊断或基因诊断试剂盒组合使用而有效治疗特定疾病、例如癌症等的方法。
例如,本发明的包含O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸或其药学上可接受的盐的药物组合物可以与NAT2基因多态性的测定进行组合,并按照以下的给药方案对罹患癌症的对象给药。
(1)鉴定并选择具有非快速(缓慢和/或中速)型的NAT2基因的所述对象;
(2)对被鉴定为具有非快速(缓慢和/或中速)型的NAT2基因的该对象给予该药物组合物。
此外,可以将本发明的包含O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸或其药学上可接受的盐的药物组合物与NAT2抑制剂联合使用来用于治疗疾病,例如癌症。作为NAT2抑制剂,例如可例示对乙酰氨基酚。
JPH203在肝细胞基质中通过NAT2迅速地被N-乙酰化而成为Nac-JPH203,但已知与JPH203相比,Nac-JPH203对LAT1的选择性和活性降低,通过与NAT2抑制剂的联合使用,从而抑制JPH203的乙酰化,实现JPH203的效果的维持。
作为O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸或其药学上可接受的盐和NAT2抑制剂的用量,可以使用各自的有效量,可以根据给药对象、给药途径、疾病、组合等适当选择。
O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸或其药学上可接受的盐和NAT2抑制剂的给药时间没有特别限定,可以将本发明的O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸或其药学上可接受的盐和NAT2抑制剂同时给药于给药对象,也可以隔开时间差进行给药。
通过将O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸或其药学上可接受的盐与NAT2抑制剂联合使用,能够发挥减轻各自的给药量、能够将治疗时间设定得长、并能够得到协同效果等效果。
与硼中子俘获疗法(Boron Neutron Capture Therapy,BNCT)的联合使用
进一步地,本发明的O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸或其药学上可接受的盐可与硼中子俘获疗法联合使用来用于治疗癌症。
作为用于硼中子俘获疗法的化合物,例如可列举硼化氨基酸,特别推荐硼化苯丙氨酸(BPA)(商标名:ボロファランB10)
硼中子俘获疗法是将硼化氨基酸与中子束照射联合使用。在这个方法中,将硼化氨基酸(例如硼化苯丙氨酸BPA)注入到体内,令癌细胞摄取。此时,BPA通过LAT1被癌细胞摄取。对此,如果照射中子射线,则在癌细胞内发生核分裂,发生细胞破坏。然而,因为BPA容易从癌细胞通过LAT1排出,所以需要大量给予BPA。另一方面,如果大量给予BPA,则正常细胞也会摄入BPA。其结果是,影响周围的正常细胞,引起副作用,同时也影响代谢、排泄的脏器即肾脏。
于是,通过将本发明的O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸与硼中子捕获疗法联合使用,能够将BPA有效地封入到癌细胞中,由此减少BPA给药量,从而降低对正常细胞的影响(=副作用)(Jutabha等Journal of PharmacologicalSciences,Volume130,Issue3,Supplement(第89次日本药理学会年会抄录集)3-O-84)。
也就是说,如Jutabha等人的文献中记载的那样,BPA通过癌细胞表面的LAT1被摄入到细胞内。BPA在该状态下也被LAT1排出到细胞外。通过在BPA被癌细胞吸收后且从癌细胞排出前给予JPH203,能够封闭LAT1而将BPA封入到癌细胞内(BPA和JPH203给药的时间差变得重要)。因此,即使BPA的给药量少,也能够高效地攻击癌细胞。也就是说,JPH203对BPA的封锁有效。Nac-JPH203与JPH203相比细胞增殖抑制效果低,因此认为与NAT2快速型相比,NAT2非快速型的效果高。
目前的标准BPA给药方法是静脉滴注给药,在中子照射前以200mg/kg/h的速度给药2小时,在照射中,配合照射结束时间以约100mg/kg/h的恒定速度给予100mg/kg的量。为了将癌细胞内的BPA保持在高浓度而过度设定目标,因此细胞外基质的BPA的浓度变高,对正常细胞的影响也增大。JPH203的给药时机如下:在中子照射前将BPA以100~200mg/kg/h的速度静脉滴注给药30分钟~1小时后,立即以10~25mg/kg/h的速度静脉滴注给药JPH203。该JPH203给药开始15~30分钟后照射中子束。
应予说明,如果BPA的局部浓度变高,则通过正常细胞表面的LAT2也被摄入到正常细胞内,但与LAT1相比,LAT2的摄入效率低。由于JPH203仅封闭LAT1,因此癌细胞内的BPA在中子照射开始时以足够高的浓度持续被封锁。
将本发明的O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸或包含该化合物的药物组合物与硼中子俘获疗法联合使用时,推荐以下的给药方案。
例如,将BPA以每小时200mg/kg的速度向癌症患者静脉滴注1小时。在此中止BPA的给药,并立即以10~20mg/kg/h开始静脉给药JPH203,30分钟后开始中子束的照射,照射中JPH203的静脉滴注给药持续至照射结束。
进而,对于本发明的O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸与硼中子俘获疗法的组合,也可以组合所述NAT2基因多态性的测定方法。
作为这样的方法,可列举以下给药方案:
(1)鉴定并选择具有非快速(缓慢和/或中速)型NAT2基因的所述对象;
(2)在给予该药物组合物之前,对被鉴定为具有非快速(缓慢和/或中速)型的NAT2基因的该对象给予含硼化合物;
(3)对该对象给予该药物组合物。
实施例
以下,虽然通过具体实施方式对本发明进行了说明,但是应当理解,本发明不限于这些实施方式,并且在不脱离所附权利要求中限定的本发明的范围或主旨的情况下,本领域技术人员可以对其进行各种变更和改变。
实施例1
使用药剂:将JPH203-SBECD(有效成分50mg)溶解于注射用水9.7ml中,制成50mg/10ml浓度,最终按照患者的体表面积制成总量100ml。
将对标准的治疗方法无效或处于不耐受状态的晚期实体癌患者作为对象,为了评估JPH203-SBECD(12、25、40、60和85mg/m2)的安全性和确定下一阶段的推荐剂量,并分析其药代动力学,在日本实施了非对照非盲检的第I期试验。研究药物设定为以12mg/m2为初次给药量,并按照斐波那契(Fibonacci)变换进行增量设置5个水平(12、25、40、60、85mg/m2)。将在取得同意后28天以内登记的3名受试者作为各个水平的对象,在登记后4天以内进行单次给药。第1例的受试者接受单次给药后,在根据最初7天反复给药(周期1)的第1天的给药开始前检查的结果确认安全性没有问题的时间点,开始第2例以后的给药。
将接受单次给药并能够判断为健康状态没有问题的受试者作为对象,从单次给药后7天后直至达到中止基准为止,使反复给予研究药物的周期持续。另外,在第1例的受试者于周期1的第7天的给药开始后49.5小时确认到安全性没有问题的时间点,开始第2例以后的给药。
受试者在单次给药和周期1(1天1次,7天)中,经由包含最终过滤器(孔径0.22μm)的滴注回路和定量泵,用90分钟连续静脉给药研究药物,所述研究药物是将总量制成100mL的生理盐水、林格氏液等适当的输液剂时成为目标用量的方式调制而成的。周期2以后也同样地,受试者以1天1次、7天、与周期1相同的用法、用量接受研究药物的给药,所述研究药物是以达到目标浓度的方式用生理盐水、林格氏液等适当的输液剂调制而成的。
其结果是,对于胆道癌患者5例,确认到部分奏效(PR)1例、稳定(SD)2例(总奏效率20.0%、病情控制率60.0%),提示了本剂对胆道癌患者的有效性。在安全性方面没有观察到临床上有问题的不良事件和副作用。根据这些结果,认为耐受性没有问题,可以期待对胆道癌患者有恒定的有效性。
根据日本第I期试验中得到的药代动力学参数分析结果确认到,在单位体表面积给予相同给药量的受试者中,血液和尿中的N乙酰化体(Nac-JPH203)浓度的个体差异大。在静脉给药后,JPH203通过作为肝转运蛋白的有机阴离子转运蛋白(OATP)被摄取到肝细胞中。认为被肝细胞摄入的JPH203通过肝细胞中的NAT2转变为Nac-JPH203,并通过胆汁排泄转运蛋白被排泄到胆汁中。因此,推测尿中的Nac-JPH203浓度的个体差异是由被认为是承担JPH203的生物体内代谢的大部分的NAT2的乙酰化的速度的差异引起的。
药代动力学分析
从受试者采集血液(全血)和尿。浓度测定中使用的全血在从给药开始起0.5、1、1.5、2、3、4.5、6、12、25.5、49.5小时后采集。浓度测定中使用的尿从给药开始起0-3、3-6、6-12、12-25.5小时通过蓄尿而采集。使用经验证的LC-MS/MS的测定体系测定血中、尿中的JHP203浓度和作为其乙酰体的Nac-JPH203浓度。
使用血中JPH203浓度和Nac-JPH203浓度来算出药代动力学参数。其中,使用AUC0-∞,求出各受试者的血中的Nac-JPH203与JPH203之比。另外,对于每个受试者,求出从给药开始至25.5小时为止采集的尿中所含的JPH203和Nac-JPH203的质量之和,求出各受试者的尿中的Nac-JPH203与JPH203之比。
NAT2基因多态性的测定
NAT2基因多态性的测定可以通过以下的实时荧光定量PCR(Real-Timc PCR)法或使用Luminex System的方法中的任一方法来实施。在任一方法中,关于NAT2,将下述7种SNPs作为分析对象。

实时荧光定量PCR法
按照下述方法,通过实时荧光定量PCR法进行测定。应予说明,关于方案的详细内容,可以适当变更。
(i)在测定NAT2的DNA浓度后,设计能够扩增包含7种分析对象SNPs的区域的2种上游引物和1种下游引物。
(ii)使用这些PCR引物来研究PCR条件,并确定最终使用的PCR引物组和PCR条件。然后,根据确定的条件,对基因组DNA实施基于Sanger测序法的序列分析。
(iii)PCR后,进行PCR产物的纯化和利用毛细管测序仪的测定。测序引物使用PCR引物,从PCR产物的两侧实施测序。
使用Luminex系统的方法
按照下述方法,利用Luminex系统法进行测定。应予说明,关于方案的详细内容,也可以适当变更。
(i)制作与作为NAT2的分析对象的7种SNPs对应的探针固相珠。
(ii)用生物素标记的引物扩增NAT2基因。使该PCR扩增产物与探针结合,并用SA-PE进行荧光标记。
(iii)用Luminex系统上获取数据,并在分析软件中显示NAT2基因的类型。
NAT2基因多态性的分析
NAT2基因多态性的检查如下实施。从第I期试验时的血液样品中提取DNA,根据NAT2的基因多态性,根据各SNP的信息对表型(Phenotype)进行分类。方法的概要如下:
DNA的提取在将全血200μL溶出到100μL的AE缓冲液中后,使用QIAamp DNA BloodMini kit(注册商标)(株式会社QIAGEN)进行。使用超微量分光光度计(Nano Drop 2000(注册商标),Thermo Fisher Scientific)(浓度调节至5ng/μL)测量与编码NAT2的位点处的SNP相关的阵列和单倍型。基因多态性分析使用TaqMan(注册商标)SNP Genotyping assay(反应容量5μL)。使用Primer Express(商标)(Applied Biosystems,CA,USA)设计用于检测SNP所需的PCR引物和荧光探针。用报告染料(carboxyfluorescein或VIC(注册商标),Applied Biosystems)标记荧光探针,并使其与七个NAT2SNP(rs1801279(191G>A)、rs1041983(282C>T)、rs1801280(341T>C)、rs1799929(481C>T)、rs1799930(590G>A)、rs1208(803A>G)以及rs1799931(857G>A))中的两个碱基之一特异性反应。PCR的反应条件为:在95℃下10分钟,然后在92℃下15秒,在60℃下90秒,进行50个循环。
针对代表NAT2基因多态性的7个SNP(481C>T,282C>T,857G>A,803A>G,341T>C,590G>A,191G>A),分析了参加第I期试验的16例的基因。
用2-SNP、3-SNP、4-SNP的组合(表2-1)分析了NAT2的乙酰化表型(Phenotype),结果乙酰化的快速型(Rapid)均由标准单倍型NAT2*4的纯合子组成,乙酰化的中速型(Intermediate)为任一单倍型突变的杂合子,乙酰化的缓慢型(Slow)为突变单倍型的纯合子或两个以上的突变单倍型的杂合子的组合(表2-2)。

NAT2表型与临床结果(安全性、有效性)之间的关系
如果将16例个体的Nac-JPH203/JPH203浓度比的偏差分类为NAT2的快速型、中速型和缓慢型,则代谢物Nac-JPH203多的类型为NAT2快速型,相反,代谢物少的个体为NAT2缓慢型。在这些NAT2快速组中,在Nac-JPH203比率高的4例中确认到肝功能检查值的上升,其中2例被判断为DLT(Dose Limiting Toxicity,剂量限制毒性)。这些患者为大肠癌2例、食道癌1例、胰腺癌1例,不包括胆道癌患者。
也就是说,在具有NAT2快速型的癌症患者中,由JPH203给药引起的肝功能障碍的值高。
用DCR(Disease Control Rate,疾病控制率)评价给药JPH203患者的临床有效性,结果显示,在全部16例中,改善(PR或SD)的患者数在NAT2快速组8例中为1例(12.5%)、在NAT2非快速(缓慢和中速)组8例中为4例(50%)。仅胆道癌患者(BDC:共5例)中,NAT2快速组1例中的0例(0%)、非快速组4例中的3例(75%)显示出改善(表4)。
关于无进展生存期(PFS),利用Kaplan-Meier法制作生存曲线,对无进展生存期的中位数及其95%置信区间进行计算,结果中位数为37.0天(CSR11.4.1有效性的分析)。其中,NAT2快速组的PFS中位数为32天,NAT2非快速组为58天。而且,胆道癌患者5例中的4例NAT2非快速组的PFS中位数为99.5天。关于整体生存期(OS),16例整体的中位数为3.5个月,其中NAT2快速组为2.5个月,NAT2非快速组(4例胆道癌)为6个月。两组间有显著性差异。

JPH203被肝脏摄取并代谢。被肝脏摄入的JPH203被乙酰化而转化为N-乙酰基-JPH203(Nac-JPH203),Nac-JPH203依赖于血流速度而被排出到对外。JPH203和Nac-JPH203均通过OATP(有机阴离子转运蛋白)摄入肝细胞,从肝细胞的排出通过Mrp2(多药耐药性蛋白质:multidrug resistance associated protein 2)进行。OATP1B1对JPH203(Nac-JPH203)的摄取速度小于Mrp2对JPH203(Nac-JPH203)的排出速度,因此认为成为JPH203代谢的限速的是摄取过程(大鼠),但在人类中,因与肝血流速度的关系,在肝细胞中滞留得多,认为摄取后的在细胞内的代谢(基于NAT2的乙酰化的过程)成为限速阶段。
通过7种SNP对参加第I期试验的受试者的NAT2进行分析,对于2-SNP、3-SNP和4-SNP的任一组合来看,显示出每个受试者的NAT2的表型(乙酰化速率:快速、中速和缓慢)是一致的(表3)。
本次参加第I期试验的17例的Nac-JPH203/JPH203比率在血中和尿中大致相关,因此推测JPH203在肝脏中被代谢后,与其浓度相对应地被排出到血液中,并从肾脏被动地排出到尿中(应予说明,1例在分析的阶段无法利用尿中的浓度结果,因此被除去)。16例中Nac-JPH203/JPH203比高的病例均为NAT2快速型的患者。其中,在Nac-JPH203产生比率高的前4例中,确认到肝功能检查值的上升,其中2例判断为DLT。关于Nac-JPH203代谢物的毒性,尚不明确的部分很多,但与大鼠、人类相比,在对NAT2缺失的狗反复给予4周JPH203的试验中,向肝脏、肾脏等的组织学变化与大鼠相比轻微,这是支持了该临床成绩的结果。
另一方面,关于有效性,NAT2快速组8例中的1例(大肠癌)显示SD(12.5%),与此相对,NAT2非快速组8例中的4例显示PR或SD(50%)。NAT2非快速组的总体生存期显著长于NAT2快速组,关于无进展生存期(PFS)的中位数,NAT2快速组无进展生存期(PFS)是NAT2非快速组的约2倍长。在这些显示出有效性的4个病例中,有3例是胆道癌,由此显示出NAT2的代谢缓慢进行,在JPH203的局部浓度保持在恒定以上的组织中,JPH203的有效性可能更有效地表达。在NAT2快速组中,病例102、104、403最终为PD,但PFS在NAT2快速组中较长。这些病例的血液中Nac-JPH203/JPH203比率相对较低,推测JPH203的安全性和有效性在NAT2快速组中占优势。
因此,提示了NAT2的SNP分析作为用于鉴定JPH203的治疗有效的对象(癌症等患者)、并使其抗肿瘤活性最大化的“伴随诊断(Companion diagnostics)”是有用的。
另外,关于JPH203的安全性和有效性,对具有非快速型的NAT2基因的患者的给药占优势可以进一步通过预先将治疗对象患者分类为具有非快速型基因的组和具有快速型基因的组后进行试验的随机化比较对照试验来确认。
在这样的试验中,JPH203例如以25mg/m2、连续给药5天、停药9天、共计14天为一个基础周期进行给药,使用注射泵或输注泵用90分钟连续静脉给药而将分配给每个受试者的研究药物给药至总量(100mL)。
作为数据分析方法,例如,对于基于独立盲法中心评价(Independent blindcentral review)的无进展生存期,将给予1次研究药物并存在有效性数据的全部受试者作为对象,按照NAT2的代谢类型(快速型、非快速型),使用Cox比例风险模型算出各群组(快速型、非快速型)中的风险比及其95%置信区间,进而可以通过包含给药组与NAT2代谢类型的相互作用项的Cox比例风险模型算出相对于相互作用项的p值。
另外,对于整体生存期,按照NAT2的代谢类型(快速型、非快速型),使用Cox比例风险模型算出各群组(快速型、非快速型)中的风险比及其95%置信区间,进而利用包含给药组与NAT2代谢类型的相互作用项的Cox比例风险模型算出相对于相互作用项的p值。另外,对于基于医师的评价的无进展生存期,区分NAT2的代谢类型(快速型、非快速型),对于基于临床试验责任(分担)医师的评价的无进展生存期(PFS),使用Cox比例风险模型算出各群组(快速型、非快速型)的风险比及其95%置信区间,进而通过包含给药组与NAT2代谢类型的相互作用项的Cox比例风险模型算出对于相互作用项的p值。
实施例2
JPH203和NAT2抑制剂的联合使用试验
评估给予NAT2抑制剂对乙酰氨基酚(APAP)是否增加血浆和肝脏中的JPH203浓度或降低其代谢物Nac-JPH203浓度。
使用的动物为8周龄雄性SD大鼠,给予1.20nmol/(min·kg)的JPH203。用生理盐水稀释至该浓度,给药速率为20μL/min。在给予JPH203之前30分钟给予500、1500mg/kg的对乙酰氨基酚。(0.5%甲基纤维素水溶液)。采血时间点设为给药JPH203后0(空白)、30、60、120、180、240分钟。另外,采集的样品量为采集0.2mL血浆、采集约0.4mL血液,并采集肝脏、肾脏、脑和脑脊液(CSF)。
将结果示于图1。可知与单独使用JPH203相比,通过联合使用JPH203与NAT2抑制剂,大鼠血中的Nac-JPH203/JPH203之比显著降低,JPH203的代谢速度变慢,进而JPH203的血中浓度变高,能够维持JPH203的效果。
实施例3
使用来自胰腺癌的细胞T3M4作为代表性的癌细胞,在该细胞中添加BPA,15分钟后添加10μM的JPH203,测定封入到细胞内的BPA浓度。将结果示于图2。在不添加JPH203(●)的情况下,随着时间的推移,BPA从细胞中排出,因此细胞内浓度降低。另一方面,在添加JPH203(■)的情况下,因JPH203抑制BPA的排出,所以细胞内浓度得以保持,30分钟后两者的细胞内浓度相差10倍左右。
产业上的实用性
通过本发明的药物组合物,能够提供提高药剂的安全性、且提高效果的奏效性的癌症等的治疗。
Claims (20)
1.一种药物组合物,其特征在于,用于在对象的癌症疾病的治疗中使用,其包含O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸或其药学上可接受的盐,向具有非快速型的NAT2基因的所述对象给予所述药物组合物,所述非快速型为缓慢型和/或中速型。
2.根据权利要求1所述的药物组合物,其特征在于,癌症疾病选自由胆道癌、大肠癌、胰腺癌、食道癌、乳腺癌、肺癌、前列腺癌、膀胱癌、脑肿瘤、胃癌、肝癌、皮肤癌、绒毛癌、肾癌、头颈癌、舌癌、转移性癌以及浸润性癌组成的组。
3.根据权利要求2所述的药物组合物,其特征在于,癌症疾病选自由胆道癌、大肠癌、胰腺癌、食道癌、乳腺癌组成的组。
4.根据权利要求1至3中任一项所述的药物组合物,其特征在于,以连续给药恒定时间然后停药恒定时间为一个周期来给予药物组合物。
5.根据权利要求4所述的药物组合物,其特征在于,以连续给药5天然后停药9天共计14天为一个周期来给予药物组合物。
6.根据权利要求1至5中任一项所述的药物组合物,其特征在于,以1~60mg/m2向对象给予药物组合物。
7.根据权利要求6所述的药物组合物,其特征在于,以12.5~60mg/m2向对象给予药物组合物。
8.根据权利要求7所述的药物组合物,其特征在于,以12.5~25mg/m2向对象给予药物组合物。
9.根据权利要求1至8中任一项所述的药物组合物,其特征在于,经恒定时间连续静脉给药恒定量的药物组合物。
10.根据权利要求9所述的药物组合物,其特征在于,用90分钟连续静脉给药100mL的药物组合物。
11.一种诊断或预测方法,其特征在于,
是诊断或预测O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸是否对特定的癌症疾病发挥效果的方法,包括判定在对象的NAT2基因中是否具有非快速型的NAT2基因的步骤。
12.一种基因诊断试剂盒,其特征在于,
用于判定在对象的NAT2基因中是否具有非快速型的NAT2基因,以便用来诊断O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸是否对特定的癌症疾病发挥效果。
13.一种药物组合物,其特征在于,
与权利要求12所述的基因诊断试剂盒组合使用,所述药物组合物包含O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸或其药学上可接受的盐。
14.一种药物组合物,其特征在于,
与NAT2基因诊断组合使用,所述药物组合物包含癌症治疗用的O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸或其药学上可接受的盐。
15.一种药物组合物,其特征在于,
用于在对象的癌症疾病的治疗中使用,其包含O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸或其药学上可接受的盐,所述药物组合物与NAT2抑制剂组合使用。
16.根据权利要求15所述的药物组合物,其特征在于,
NAT2抑制剂为对乙酰氨基酚。
17.根据权利要求15所述的药物组合物,其特征在于,
向具有快速型的NAT2基因的所述对象给药。
18.一种药物组合物,其特征在于,
用于在对象的癌症的治疗中使用,所述药物组合物包含O-(5-氨基-2-苯基苯并噁唑-7-基)甲基-3,5-二氯-L-酪氨酸或其药学上可接受的盐,所述药物组合物按照下述方式给药:
(1)鉴定并选择具有非快速型的NAT2基因的所述对象,所述非快速型为缓慢型和/或中速型;
(2)在给予该药物组合物之前,对被鉴定为具有非快速型的NAT2基因的该对象给予含硼化合物,所述非快速型为缓慢型和/或中速型;
(3)向该对象给予该药物组合物。
19.根据权利要求1至10和13至17中任一项所述的药物组合物,其特征在于,
在给予该药物组合物后,进一步通过中子俘获疗法BNCT来杀死癌。
20.根据权利要求1至10和13至19中任一项所述的药物组合物、权利要求11所述的方法、或权利要求12所述的试剂盒,其特征在于,
对象为人类。
