可获得来自第一项研究(ARN-AR18-CT-101)的52名患者的奥那司酮的PK结果(表1)。奥那司酮PK的变化性是适度的,并且对IR比对ER制剂要高。ER形式的奥那司酮Cmax和AUC值与施用剂量成比例(图1和图2)。基于观察到的平均AUC值,ER的口服生物利用率对比于IR为约50%(图4)。ER形式的后续Tmax值导致ER形式与IR形式相比略低的剂量校正Cmax值。稳态在第8天之前达到,平均t1/2为7.5小时。

在研究ARN-AR18-CT-101中,在52名具有PR阳性实体瘤的女性患者中,以10-50mgBID剂量接受奥那司酮ER制剂的9/46的患者(20%)表现出临床益处,对比于0/6(0%)的接受每天一次的100mg奥那司酮IR制剂的患者。定义为RECIST 1.1部分响应或至少24周的稳定疾病的临床益处响应,仅在接受ER的患者中观察到。感兴趣的是,具有临床益处的7/9的患者(78%)接受低于建立的100mg IR剂量的剂量,并且具有部分响应的患者以最低的ER剂量水平(10mg BID)被治疗。
至于ARN-AR18-CT-102,21名具有前列腺癌的患者中的2名在第12周后具有SD。中位的治疗持续时间为8周。

患者和方法
合格
入选标准包含:
(1)已经被治疗过复发性或转移性孕酮受体表达癌(例如,子宫内膜、卵巢、乳腺癌或子宫肉瘤)的,按照实体瘤响应评价标准(Response Evaluation Criteria In SolidTumors)1.1版本(RECIST 1.1)具有可评价疾病的年龄≥18岁的绝经后女性患者;
(2)具有可用组织块或活检标本以确定孕酮受体(PR)和激活的孕酮受体(APR)状态的患者;和

对每个参与中心的病理部门的存档的组织块进行为了入选目的的PR确定。计划了中央PR/APR评价,但相对于入选和治疗可追溯。

实施例4
研究设计和治疗
该研究是开放的、多中心的、随机的、平行组的、两部分的1-2期研究,试验的I期部分在本文讨论。为了确定推荐的2期剂量(recommended phase 2dose)(RP2D),参加这个1期研究的患者以平行方式随机化为六(6)个组:五(5)个ER ONA片剂组(10mg BID、20mg BID、30mg BID、40mg BID、50mg BID)和一(1)个使用IR片剂制剂的组(100mg QD)。试验在法国的五(5)个中心中进行(在ClinicalTrials.gov注册为NCT02052128)。

实施例5
药代动力学方法

Claims (20)

7.如权利要求1所述的延长释放药物组合物,其中在每天两次向患者施用10mg所述延长释放制剂后,奥那司酮的Cmax在约8-12小时为至少约240ng/ml。
8.如权利要求1所述的延长释放药物组合物,其中在每天两次向患者施用所述延长释放药物组合物后约8天时,获得稳态血浆浓度。
9.一种向具有癌症的患者施用奥那司酮的方法,所述方法包括每天两次向所述患者施用延长释放奥那司酮药物组合物,其中所述延长释放奥那司酮药物组合物包括至少约2mg至约50mg的奥那司酮。
