背景技术

如图2所示,钳口组件30的砧座组件40包含砧座板42,具有在其中形成的中央纵向槽41;和盖板44,其固定在砧板42上,使得盖板44限定砧座组件40的顶部或面向外的表面46。如图2A所示,砧座板42可包含限定在其面向内或面向组织的表面48中的多个缝合钉形成袋/腔45。

如图3和4所示,外科缝合器1的外科支撑件组件100包含细长构件或释放带102、折叠部分104和支撑件部分106。释放带102联接到折叠部分104,折叠部分104联接到支撑件部分106并且可通过穿孔108与其分开。折叠部分104和支撑件部分106一起形成管状主体110,所述管状主体110的尺寸和形状设计成便于通过其容纳砧座组件40和/或缝合钉钉仓组件50。
外科支撑件组件100的释放带102固定到外科支撑件组件100的折叠部分104并从其向近侧延伸。释放带102的长度足以接近患者身体外部,并且可延伸外科缝合器1的细长管状主体部分20的长度。释放带102可以是带子、绳、绳索、带、缝合线,以及与外科支撑件组件100的折叠部分104系在一起或一体形成的其它细长结构。在一些实施例中,释放带102可包含两个或更多个细长区段,例如,附接到材料条带的缝合线,所述缝合线从外科支撑件组件100的折叠部分104向近侧延伸。

外科支撑件组件100的穿孔108可以是任何尺寸和形状,例如小针孔或更大的开口,例如图3中所示的细长开口,或者穿孔108可以是单个特征件,例如支撑件材料或形成折叠和支撑件部分104、106的材料中的弱线。穿孔108可以使用激光切割、刀切、冲切、刻痕和蚀刻以及本领域技术人员知识范围内的其它方法形成。在一些实施例中,外科支撑件组件100由非织造聚乙交酯材料形成,所述材料被激光切割以形成穿孔108。
如图5所示,外科缝合器1的整个外科支撑件组件100可以由单片材料120形成。由单片材料120切割成外科支撑件组件100,并在其中形成穿孔108。对单片材料120进行切割,使得外科支撑件组件100包含释放带102,包含第一和第二区段112、114的折叠部分104,以及支撑件部分106。在外科支撑件组件100设置在外科缝合器1的砧座组件40上或周围之前(例如,在将外科支撑件组件100装载入外科支撑件施加器200(图6)中之前),或者当外科支撑件组件100应用于砧座组件40时,折叠部分104的第一和第二区段112、114可以折叠并彼此附接。如上所述,折叠部分104的第一和第二区段112、114可以通过例如焊接,使用粘合剂,系缝合线等彼此附接。

在将外科支撑件组件100装载到外科缝合器1上的方法中,如图9所示,将外科支撑件装载组件201应用于外科缝合器1的砧座组件40。使用者抓住外科支撑件施加器200的手柄220,并通过将基部210的腔213(图7)与砧座组件40的远端对齐并将外科支撑件施加器200在砧座组件40上向近侧滑动直到砧座组件40邻接基部210的远侧壁区段214c(图7),将外科支撑件装载组件201操纵到砧座组件40上。如上所讨论的,基部210的腔213的形状(例如,横截面形状/轮廓)对应于砧座组件40(或缝合钉钉仓组件50)的形状(例如,横截面形状/轮廓)以便于在其上施加外科支撑件组件100。

在使用方法中,通过套管针或其它进入装置将装载的外科缝合器1引入外科手术部位。外科缝合器1以本领域技术人员已知的方法操作。一旦将砧座和缝合钉钉仓组件40、50夹紧到组织上,外科缝合器1就被启动。在启动外科缝合器1时,驱动杆64(图2)通过钳口组件30向远侧推进,将缝合钉推动器60向上推动,以将缝合钉58推出缝合钉袋55并通过外科支撑件组件100的支撑件部分106和捕获的组织,从而将支撑件部分106缝合在组织上。在启动期间,驱动杆64的刀片62向远侧行进,而基本上同时切割和分割组织以及设置在现已形成的缝合钉58的行之间的外科支撑件组件100的支撑件部分106。
然后,通过向近侧拉动手柄220以经由穿孔108从支撑件部分106撕开折叠部分104,可以将外科支撑件组件100的折叠部分104和释放带102从现已缝合的支撑件部分106上拆下。通过将手柄220从外科缝合器1的细长管状主体部分20上拆下,可以将手柄220与外科缝合器1分开拉动,或者当外科缝合器1向近侧拉离外科手术部位时可以拉动手柄220。或者,使用者可抓住释放带102的暴露部分并直接拉动释放带102。
应当理解,附加地或替代地,外科支撑件装载组件可以应用于外科缝合器的缝合钉钉仓组件和/或砧座组件或缝合钉钉仓组件可以包含预先装载在其上的支撑件材料。例如,在具有可移除和可更换的缝合钉钉仓组件的外科缝合装置中,缝合钉钉仓组件可具有由制造商预装载到其上的支撑件。在这些情况下,使用者可以使用准备好安装在砧座组件上的外科支撑件装载组件,如上所讨论的。在实施例中,外科缝合装置可以通过用缝合钉钉仓组件重新装载在同一患者身上重新使用,所述缝合钉钉仓组件具有准备启动的新的一组缝合钉和新的支撑件材料。如上所讨论的,可以将新的外科支撑件装载组件应用于砧座组件,或者可以将新的外科支撑件安装到外科支撑件施加器中,同样如上所讨论的。
此外,外科支撑件装载组件的尺寸和形状可以设计成与特定钳口相对应,所述外科支撑件装载组件要组装到所述特定钳口上。例如,第一外科支撑件装载组件可具有与砧座组件的形状对应的形状,并且第二外科支撑件装载组件可具有与缝合钉钉仓组件对应的形状。如图1所示,砧座组件40具有较低的轮廓并且形状比缝合钉钉仓组件50更弯曲,缝合钉钉仓组件50较深并且可以是圆形或更矩形的形状。因此,本公开的外科支撑件和外科支撑件施加器可以是任何尺寸和/或形状,以适应各种外科缝合器尺寸和/或配置。

为了将外科支撑件组件300与砧座组件40分开,如图25和26所示,将释放带320向近侧拉动,如箭头“P”所示,到达第一近侧位置(图25),使得释放带320的棘爪328和侧面指或指状物330a与支撑件310的棘爪槽311分开。棘爪槽311可以配置成撕开、拉伸、折叠等,以使棘爪328的头部328ad能够与棘爪槽311分开。另外,和/或替代地,棘爪328或其部分(例如,棘爪328的头部328d和/或下颚328e)可以配置成撕开、拉伸、折叠等,以便于将释放带320的棘爪328与支撑件的棘爪槽311分开。在释放带320的第一近侧位置,释放带320的中间指或指状物330b部分地保持在支撑件310的一些保持槽313(例如,向近侧定位的保持槽313p)内,使得支撑件310的远侧突片314a、316b通过释放带320的指状物组件330的中间指或指状物330b的远侧部分保持附接到支撑件310的趾带318。将释放带320拉到第一近侧位置时,中间指或指状物330b部分地通过保持槽313缩回,如箭头“R”所示。

本公开的外科支撑件可以由生物相容性材料制成,所述材料是生物可吸收的或不可吸收的天然或合成材料。应当理解,天然的、合成的、生物可吸收的和/或非生物可吸收的材料的任何组合可用于形成外科支撑件。外科支撑件可以是生物可降解的(例如,由生物可吸收和生物可恢复的材料形成),使得外科支撑件在体内的生理条件下分解或破坏(物理或化学),并且降解产物可被身体排泄或吸收。外科支撑件的组件或部分可由相同材料或不同材料形成。
在实施例中,至少一部分外科支撑件由选自以下群组的可生物降解材料制成:天然胶原材料、肠线和合成树脂,包含衍生自碳酸亚烃酯、三亚甲基碳酸酯、四亚甲基碳酸酯、己内酯、戊内酯、二氧环己酮、聚酸酐、聚酯、聚丙烯酸酯、聚甲基丙烯酸甲酯、聚氨酯、乙醇酸、乳酸、乙交酯、丙交酯、聚羟基丁酸酯、聚原酸酯、聚羟基脂肪酸酯、其均聚物及其共聚物的那些。在实施例中,至少一部分外科支撑件可由选自以下群组的不可生物降解的材料制成:聚烯烃、聚乙烯、聚二甲基硅氧烷、聚丙烯、聚乙烯和聚丙烯的共聚物、聚乙烯和聚丙烯的共混物、超高分子量聚乙烯、聚酰胺、聚酯、聚对苯二甲酸乙二醇酯、聚四氟乙烯、聚醚酯、聚丁酯、聚四亚甲基醚二醇、1,4-丁二醇和聚氨酯。
外科支撑件可以是多孔的、无孔的或其组合。合适的多孔结构包含例如纤维结构(例如,针织结构、织造结构和非织造结构)和/或泡沫(例如,开孔或闭孔泡沫)。合适的无孔结构包含例如薄膜。外科支撑件或其部分可以是通过熔喷或熔纺方法形成的非织造结构,网状材料,编织材料和/或模制或挤出片材。外科支撑件或其部分可以是单个多孔或无孔层,或者包含多个层,包含多孔和/或无孔层的任何组合。
外科支撑件可以作为外科支撑件装载组件的一部分提供和/或出售,所述外科支撑件装载组件包含一个或多个外科支撑件和外科支撑件施加器。或者,外科支撑件和外科支撑件施加器可以单独提供和/或出售并由使用者组装。在实施例中,在套件中提供一个或多个外科支撑件和一个或多个外科支撑件施加器。在一些实施例中,套件还包含一个或多个末端执行器(和/或外科装载单元),并且在某些实施例中,套件还包含外科缝合器。
在本文公开的任何实施例中,外科支撑件可包含近距离放射治疗,化疗,其它医疗材料或药物,或与之一起使用。外科支撑件的支撑部分可以具有袋、孔或其它特征,用于保持近距离放射治疗种子与支撑部分,或者近距离放射治疗种子或材料可以结合到缝合线中,所述缝合线穿入或穿过支撑件部分或以其它方式附接于其上。可以通过喷雾或浸渍将具有近距离放射治疗材料的涂层施加到外科支撑件的支撑件部分。化学治疗药物或药剂可以结合到外科支撑件的支撑件部分中,涂覆在其上,或作为缝合线的一部分、缝合线或其它特征件施加。

释放带,其可释放地联接到所述支撑件的所述趾带,所述释放带可相对于所述支撑件移动,以将所述释放带与所述支撑件分开。
