技术领域
本发明属于特医食品领域,具体地说涉及一种成人慢性肾脏病非透析患者全营养食品配方及制备方法。
背景技术
特殊医学用途配方食品(Food for special medical purpose,以下简称特医食品)是指为了满足进食受限、消化吸收障碍、代谢紊乱或特定疾病状态人群对营养素或膳食的特殊需要,专门加工配制而成的配方食品。特医食品作为一种肠内营养制剂,可以起到营养支持的作用和改善病症的作用。
慢性肾脏病(CKD)在临床十分常见,它对人民的健康和生命可造成很大危害。60年代以前,尿毒症一直被视为“不治之症”。近20余年来,现代肾脏病学迅速发展,特别是透析、移植和非透析治疗(包括营养疗法)等方面的发展,已使尿毒症病人的生活质量和生存率有了很大提高。目前全世界已有40万以上的慢性肾脏病病人作了肾移植,另有50万以上的慢性肾脏病患者依靠透析延长存活。据报告,无论是透析还是透析前慢性肾脏病病人,营养不良及代谢紊乱均相当常见和普遍,因而,重视营养治疗是改善慢性肾脏病患者生活质量及预后的不可忽视的重要方面。虽然透析和肾移植是治疗终末期肾衰的主要措施,但治疗费用昂贵。据估计,全世界每年用于慢性肾脏病患者治疗的费用高达千亿美元;在美国,仅透析治疗的每年拨款即高达130多亿美元。高额医疗费用,对多数发展中国家的政府或个人来说,都是一个沉重的经济负担,因而他们必须把非透析治疗置于重要地位。在CKD的不同阶段,其临床表现也各不相同。在CKD3期之前,病人可以无任何症状,或仅有乏力、腰酸、夜尿增多等轻度不适;少数病人可有食欲减退、代谢性酸中毒及轻度贫血。CKD3期以后,上述症状更趋明显,进入肾衰竭期以后则进一步加重,有时可出现急性心衰、严重高钾血症、消化道出血、中枢神经系统障碍等,甚至会有生命危险。
慢性肾功能衰退可引发代谢产物、毒素在体内累积,引起水电解质及酸碱平衡紊乱,另外由于缩胆囊素、神经肽等激素的代谢异常,患者常伴发食欲降低等症状,导致热量、蛋白质、维生素、和微量因子的摄取降低,形成营养不足;这些年的分析指出CKD患者的微炎症情况,容易刺激机体的新陈代谢,降低血浆白蛋白的形成等方面,最终引发营养不足,营养不良发生率仍然很高(约30%~60%)。因此,对这些病人的营养状况需要予以及时的检查和评估;并采取有效措施积极予以纠正,以提高其生活质量和长期存活率。医用食品在早期CKD或非透析依赖性CKD患者的营养管理中,主要通过较高能量、低蛋白、低盐的配方以防止营养不良、减少尿毒症毒性、延缓CKD进展、预防继发性甲状旁腺功能亢进,以及改善代谢性酸中毒;对于晚期CKD或透析依赖性CKD患者,医用食品主要通过提供较高能量、充足蛋白质、控制钠和液体的摄入量等方式以降低并发症发病率及死亡率。目前针对非透析依赖性CKD患者,医用食品蛋白质含量应低于0.65 g/100 kJ,并适当降低钾、钠、磷、镁、钙及VA的含量;对于透析治疗患者,配方中蛋白质含量应不低于0.8 g/100 kJ。现有技术中,专门针对成人慢性肾脏病非透析患者的特医食品相关的报道较少。
发明内容
针对成人慢性肾脏病非透析患者营养不良问题及弥补现有成人慢性肾脏病非透析患者全营养配方食品的不足,本发明提供了一种营养配比均衡,维持患者的营养需要,提高生活质量,延缓病情发展的成人慢性肾脏病非透析患者全营养食品配方。
本发明还提供了一种成人慢性肾脏病非透析患者全营养食品配方的制备方法。
本发明为了实现上述目的所采用的技术方案为:
本发明提供了一种成人慢性肾脏病非透析患者全营养食品配方,是由以下原料组成:
本发明所使用的冬虫夏草、草莓、燕麦、桑葚、山药和莲子超微粉为:
(1)干燥至水分含量为3-5%的冬虫夏草、草莓、燕麦、桑葚、山药和莲子原料粉碎为80-100目的粗粉;
(2)将步骤(1)的粗粉粉碎成粒度为400-1200目,制得冬虫夏草、草莓、燕麦、桑葚、山药和莲子超微粉。
3、一种如权利要求1所述的成人慢性肾脏病非透析患者全营养食品的制备方法,其特征在于,包括以下步骤:
(1)将橄榄油、双歧杆菌、乳杆菌,采用喷雾干燥技术制备出微胶囊;
(2)将各原料按质量百分比混合,搅拌均匀得到混合粉;
(3)将步骤(2)中混合粉中加入40~50 %的水,在压力为10-15 Mpa,温度为60-80 ℃下进行初级均质处理10-20 min,再在压力15-25Mpa,温度40-50 ℃下均质10-15 min,得到浆液;
(4)将步骤(3)中的浆液,喷雾造粒;经检验合格包装,得成品。
本发明所使用的橄榄油、双歧杆菌、乳杆菌微胶囊采用以下方法制备而成:
(1)将马铃薯淀粉、醋酸及聚乙二醇水溶液,加热至90-100℃,搅拌50-60min至透明,然后加入蔗糖脂肪酸酯,搅拌均匀,得溶液A;
(2)将橄榄油、双歧杆菌或乳杆菌加入到柠檬酸溶液中,然后升温至50-60℃,搅拌状态下加入溶液A,待混合均匀后,加入可溶性大豆多糖,继续搅拌均匀后喷雾干燥即可。
进一步的,所述马铃薯淀粉、醋酸及聚乙二醇水溶液的料液比为20g:0.1mL:10mL。
进一步的,所述聚乙二醇水溶液的体积分数为0.5%。
进一步的,所述蔗糖脂肪酸酯的加入量占马铃薯淀粉重量的0.08-0.1%。
进一步的,所述橄榄油、双歧杆菌或乳杆菌在柠檬酸溶液中的浓度为20wt%;所述可溶性大豆多糖的加入量占柠檬酸溶液质量的0.5%;所述柠檬酸溶液的体积分数为2%。
本发明制备微胶囊喷雾干燥的条件为:进口温度140~150℃,出口温度80~90℃,进料速度40ml/min。
进一步的,步骤(5)中,所述喷雾造粒条件为:料液温度为70~80 ℃,进口风温为100~110 ℃,出口温度 70~80 ℃,进料流量为25~35 mL/min,得到粒径大小为300-800μm的颗粒。
本发明成人慢性肾脏病非透析患者全营养食品配方,富含优质蛋白质、维生素C和钙,高热量和低盐,营养丰富。该全营养配方食品提供的能量为 565.7 Kcal/100 g,其中含有蛋白质 40 g/100 g,提供能量162.6 Kcal/100 g;含有碳水化合物30 g/100g,提供能量121.9 Kcal/100 g;含有脂肪25 g/100g,提供能量221.1 Kcal/100 g;含有膳食纤维30 g/100 g,提供能量60.1 Kcal/100 g。
本发明可以做出粉末状的固体饮料、颗粒剂,也可制成饼干等快速、方便食品便于外出携带食用。
所述的大豆短肽采用将反胶束法制备的大豆蛋白进行蛋白酶水解制备。
本发明具有以下优点及突出性效果:
(1)原料利用率高
本发明通过超微粉碎降解了冬虫夏草、草莓、燕麦、桑葚、山药和莲子中的纤维素及其他大分子物质的结构,利于机体对其消化吸收;同时采用本发明处理原料,破坏原料细胞壁,使冬虫夏草、草莓、燕麦、桑葚、山药和莲子中生理活性成分溶出,提高原料利用率。
(2)改善成人慢性肾脏病非透析患者营养不良的症状
针对成人慢性肾脏病非透析患者的相关症状,采用科学合理的制备加工方法,发挥各个原料的优势与营养,制得改善并适合成人慢性肾脏病非透析患者的全营养配方特殊食品,该食品对成人慢性肾脏病非透析患者各个症状均有显效的作用,且用料简单,成本低。
(3)全营养食品配方品质高
在加工过程中采用纯物理加工的方法,采真空冷冻脱水技术干燥草莓和桑葚,改善其品感,在低温-15℃下超微粉碎草莓和桑葚,使其更易粉碎,且使草莓和桑葚保留特有的风味及营养物质,口感细滑;采用分段均质得到细腻均匀不分层浆液,使得成品溶于水后不分层;采用喷雾干燥制备成即食颗粒。制得的产品色泽:色泽柔和,对比协调;均匀粘稠的流动状态,固形物分布均匀无块状物;口感细腻、回味性好。
(3)本发明的成人慢性肾脏病非透析患者全营养配方食品中所含有的各种成分及含量通过缩合考虑成人慢性肾脏病非透析患者的各种需求而确定,营养配比均衡、吸收快,更重要的是添加了患者必需的组氨酸和酪氨酸,改善患者体内必需氨基酸与非必需氨基酸比例,体内蛋白质合成增加,氮代谢产物生成减少,使肾脏损害减轻。
(4)便于携带,适合工业化生产
本发明制得全营养食品方便,便于使用与携带、运输方便,易贮存,不易变质,适于大规模工业化生产。
具体实施方式
下面通过具体的实施方式对本发明的技术方案作进一步的解释和说明。
实施例1
本发明的成人慢性肾脏病非透析患者全营养配方食品由按重份来计的下述原料制备而成。
具体步骤:
(1)干燥至水分含量为3%的冬虫夏草、草莓、燕麦、桑葚、山药和莲子原料粉碎为80目的粗粉;
(2)将步骤(1)的粗粉粉碎成粒度为400目,制得冬虫夏草、草莓、燕麦、桑葚、山药和莲子微粉;
(3)将橄榄油、双歧杆菌、乳杆菌,采用喷雾干燥技术制备出微胶囊;
(4)将步骤(2)与步骤(3)冬虫夏草、草莓、燕麦、桑葚、山药和莲子微粉、橄榄油、双歧杆菌、乳杆菌微胶囊、大豆异黄酮、大豆低聚糖、低聚果糖、各种氨基酸等其他营养成分按质量百分比混合,搅拌均匀得到混合粉;
(5)将步骤(4)中混合粉中加入40 %的水,在压力为10 Mpa,温度为60 ℃下进行初级均质处理10 min,再在压力15 Mpa,温度40 ℃下均质10 min,得到浆液;
(6)将步骤(5)中的浆液,喷雾造粒;经检验合格包装,得成品。
所述的步骤(5)的喷雾造粒参数为:料液温度为70 ℃,进口风温为100℃,出口温度 70 ℃,进料流量为25 mL/min,得到粒径大小为300 μm的颗粒;
本发明所使用的橄榄油、双歧杆菌、乳杆菌微胶囊采用以下方法制备而成:
(1)将马铃薯淀粉、醋酸及体积分数为0.5%聚乙二醇水溶液按照料液比20g:0.1mL:10mL混合,加热至90-100℃,搅拌50-60min至透明,然后加入占马铃薯淀粉0.1%的蔗糖脂肪酸酯,搅拌均匀,得溶液A;
(2)将橄榄油、双歧杆菌或乳杆菌加入到体积分数为2%的柠檬酸水溶液中,然后升温至50-60℃,搅拌状态下加入溶液A,待混合均匀后,加入占柠檬酸溶液质量的0.5%的可溶性大豆多糖,继续搅拌均匀后喷雾干燥即可;
所述喷雾干燥的条件为:进口温度140~150℃,出口温度80~90℃,进料速度40ml/min。
实施例2
本发明的成人慢性肾脏病非透析患者全营养配方食品由按重份来计的下述原料制备而成。
制备方法同实施例1。
实施例3
本发明的成人慢性肾脏病非透析患者全营养配方食品由按重份来计的下述原料制备而成。
具体步骤:
(1)干燥至水分含量为5%的冬虫夏草、草莓、燕麦、桑葚、山药和莲子原料粉碎为100目的粗粉;
(2)将步骤(1)的粗粉粉碎成粒度为1200目,制得冬虫夏草、草莓、燕麦、桑葚、山药和莲子微粉;
(3)将橄榄油、双歧杆菌、乳杆菌,采用喷雾干燥技术制备出微胶囊。
(4)将步骤(2)与步骤(3)冬虫夏草、草莓、燕麦、桑葚、山药和莲子微粉、橄榄油、双歧杆菌、乳杆菌微胶囊、大豆异黄酮、大豆低聚糖、低聚果糖、各种氨基酸等其他营养成分按质量百分比混合,搅拌均匀得到混合粉;
(5)将步骤(4)中混合粉中加入50 %的水,在压力为15 Mpa,温度为80 ℃下进行初级均质处理20 min,再在压力25Mpa,温度50 ℃下均质15 min,得到浆液;
(6)将步骤(5)中的浆液,喷雾造粒;经检验合格包装,得成品。
所述的步骤(5)的喷雾造粒参数为:料液温度为80 ℃,进口风温为110 ℃,出口温度 80 ℃,进料流量为35 mL/min,得到粒径大小为800 μm的颗粒。
具体步骤:
(1)干燥至水分含量为4%的冬虫夏草、草莓、燕麦、桑葚、山药和莲子原料粉碎为90目的粗粉;
(2)将步骤(1)的粗粉粉碎成粒度为700目,制得冬虫夏草、草莓、燕麦、桑葚、山药和莲子微粉;
(3)将橄榄油、双歧杆菌、乳杆菌,采用喷雾干燥技术制备出微胶囊。
(4)将步骤(2)与步骤(3)冬虫夏草、草莓、燕麦、桑葚、山药和莲子微粉、橄榄油、双歧杆菌、乳杆菌微胶囊、大豆异黄酮、大豆低聚糖、低聚果糖、各种氨基酸等其他营养成分按质量百分比混合,搅拌均匀得到混合粉;
(5)将步骤(4)中混合粉中加入45 %的水,在压力为12 Mpa,温度为70 ℃下进行初级均质处理15 min,再在压力20Mpa,温度450℃下均质12 min,得到浆液;
(6)将步骤(5)中的浆液,喷雾造粒;经检验合格包装,得成品。
所述的步骤(5)的喷雾造粒参数为:料液温度为75 ℃,进口风温为105 ℃,出口温度 75 ℃,进料流量为20 mL/min,得到粒径大小为500 μm的颗粒。
对比例1
成人慢性肾脏病非透析患者全营养配方食品,配方同实施例1。
微胶囊的具体制备方法如下:
本发明所使用的橄榄油、双歧杆菌、乳杆菌微胶囊采用以下方法制备而成:
(1)将马铃薯淀粉、醋酸及体积分数为0.5%聚乙二醇水溶液按照料液比20g:0.1mL:10mL混合,加热至90-100℃,搅拌50-60min至透明,然后加入占马铃薯淀粉0.1%的蔗糖脂肪酸酯,搅拌均匀,得溶液A;
(2)将橄榄油、双歧杆菌或乳杆菌加入到体积分数为2%的柠檬酸水溶液中,然后升温至50-60℃,搅拌状态下加入溶液A,搅拌均匀后喷雾干燥即可;
所述喷雾干燥的条件为:进口温度140~150℃,出口温度80~90℃,进料速度40ml/min。
全营养配方食品的制备方法同实施例1。
对比例2
成人慢性肾脏病非透析患者全营养配方食品,配方同实施例1。
微胶囊的具体制备方法如下:

全营养食品的制备方法同实施例1。
(一)将实施例1-3及对比例1-2制备的全营养配方食品进行保质期检测,利用食品储存期加速测试,测定制备的食品中微生物等因素,具体检测结果见表1。

(二)将实施例1及对比例1和2制备的微胶囊进行包埋率及出粉率进行统计,具体结果见表2。

(三)建立小鼠慢性肾病模型,在SPF环境中,光照时间明暗各半,室温保持25℃左右,湿度50-60%,将SD大鼠适应性喂养一周后,注射阿霉素,剂量为5mg/kg,每日定时腺嘌呤灌胃,200mg/(kg.d),检测各种指标,4周后模型建立成功;将实施例1-3及对比例1-2制备的配方食品进行喂食小鼠,设置对照组,喂食全价颗粒料,自由进食和饮水,模型建立初期检测尿素氮、肌酐及24h尿蛋白,喂食配方食品后1周再次进行检测,具体见过见表3。

(四)检测上述动物模型对大鼠血Cys-c、肾小球滤过率GFR,具体结果见表4。

Claims (10)
1.一种成人慢性肾脏病非透析患者全营养食品配方,其特征在于,是由以下原料组成:

2.根据权利要求1所述的成人慢性肾脏病非透析患者全营养食品配方,其特征在于,所述的冬虫夏草、草莓、燕麦、桑葚、山药和莲子超微粉为:
(1)干燥至水分含量为3-5%的冬虫夏草、草莓、燕麦、桑葚、山药和莲子原料粉碎为80-100目的粗粉;
(2)将步骤(1)的粗粉粉碎成粒度为400-1200目,制得冬虫夏草、草莓、燕麦、桑葚、山药和莲子超微粉。
3.一种如权利要求1所述的成人慢性肾脏病非透析患者全营养食品的制备方法,其特征在于,包括以下步骤:
(1)将橄榄油、双歧杆菌、乳杆菌,采用喷雾干燥技术制备出微胶囊;
(2)将各原料按质量百分比混合,搅拌均匀得到混合粉;
(3)将步骤(2)中混合粉中加入40~50 %的水,在压力为10-15 Mpa,温度为60-80 ℃下进行初级均质处理10-20 min,再在压力15-25Mpa,温度40-50 ℃下均质10-15 min,得到浆液;
(4)将步骤(3)中的浆液,喷雾造粒;经检验合格包装,得成品。
4.根据权利要求3所述的制备方法,其特征在于,所述橄榄油、双歧杆菌、乳杆菌微胶囊采用以下方法制备而成:
(1)将马铃薯淀粉、醋酸及聚乙二醇水溶液,加热至90-100℃,搅拌50-60min至透明,然后加入蔗糖脂肪酸酯,搅拌均匀,得溶液A;
(2)将橄榄油、双歧杆菌或乳杆菌加入到柠檬酸溶液中,然后升温至50-60℃,搅拌状态下加入溶液A,待混合均匀后,加入可溶性大豆多糖,继续搅拌均匀后喷雾干燥即可。
5.根据权利要求4所述的制备方法,其特征在于,所述马铃薯淀粉、醋酸及聚乙二醇水溶液的料液比为20g:0.1mL:10mL。
6.根据权利要求4或5所述的制备方法,其特征在于,所述聚乙二醇水溶液的体积分数为0.5%。
7.根据权利要求4所述的制备方法,其特征在于,所述蔗糖脂肪酸酯的加入量占马铃薯淀粉重量的0.08-0.1%。
8.根据权利要求4所述的制备方法,其特征在于,所述橄榄油、双歧杆菌或乳杆菌在柠檬酸溶液中的浓度为20wt%;所述可溶性大豆多糖的加入量占柠檬酸溶液质量的0.5%;所述柠檬酸溶液的体积分数为2%。
9.根据权利要求4-8任一项所述的制备方法,其特征在于,所述喷雾干燥的条件为:进口温度140~150℃,出口温度80~90℃,进料速度40ml/min。
10.根据权利要求3所述的制备方法,其特征在于,步骤(5)中,所述喷雾造粒条件为:料液温度为70~80 ℃,进口风温为100~110 ℃,出口温度 70~80 ℃,进料流量为25~35mL/min,得到粒径大小为300-800 μm的颗粒。
