In the formula, the phellodendron bark is used for clearing heat and drying dampness, purging fire and removing steam, detoxifying and treating sore, the honeysuckle stem is used for clearing heat and removing toxin, and is used as monarch drug for playing the effects of removing dampness and clearing heat, and expelling pus and removing toxin to eliminate residual pathogen, the astragalus root is used for tonifying qi and strengthening superficies, expelling toxin and expelling pus, and promoting granulation, the bighead atractylodes rhizome is used for tonifying spleen and replenishing qi and drying dampness, and is used as ministerial drug by being matched with the astragalus root and used together with the phellodendron bark and the honeysuckle stem to strengthen body resistance and eliminate pathogenic factors, and the cold property of the phellodendron bark and the honeysuckle stem can be alleviated to prevent the defect of damaging body resistance. The Chinese medicinal composition has the effects of clearing away heat and toxic materials, tonifying qi and nourishing blood, and astringing and promoting granulation; mainly treats the chronic nonunion of the wound surface caused by lingering pathogen due to deficiency of vital qi; has obvious effect on treating chronic intractable wound.

Detailed Description
EXAMPLE 1 preparation of the Chinese medicinal composition of the present invention
1. The raw material medicaments: 50g of phellodendron, 50g of honeysuckle stem, 30g of astragalus, 20g of bighead atractylodes rhizome and 10g of angelica.
2. The preparation method comprises the following steps: weighing the raw materials according to the weight ratio; 3 doses of the raw materials are taken and put into a disposable non-woven gauze, and the raw materials are fully soaked for 30 minutes by using 700mL of drinking water meeting the national sanitary standard; decocting with strong fire for 30min at normal pressure, and filtering and sterilizing to obtain filtrate; adding 600mL of drinking water again, decocting for 20 minutes twice, and taking the filtrate; adding 400mL of drinking water for the third time, decocting for 15 minutes, mixing all filtrates to obtain 900mL of filtrate, and packaging the filtrate into 100mL of vacuum packaging bag by using Chinese medicinal decoction packaging machine (type: YB50-250, manufactured by Beijing Tohua original medical equipment, Ltd.).
EXAMPLE 2 preparation of the Chinese medicinal composition of the present invention
1. The raw material medicaments: 40g of phellodendron, 60g of honeysuckle stem, 24g of astragalus, 16g of bighead atractylodes rhizome and 12g of angelica.
2. The preparation method comprises the following steps: weighing the raw materials according to the weight ratio; 3 doses of the raw materials are put into a disposable non-woven gauze, and the raw materials are fully soaked for 20 minutes by using 700mL of drinking water meeting the national sanitary standard; decocting with strong fire for 40min at normal pressure, and filtering and sterilizing to obtain filtrate; adding 600mL of drinking water again, decocting for 30 minutes twice, and taking the filtrate; adding 400mL of drinking water for the third time, decocting for 20 minutes, mixing all filtrates to obtain 900mL of filtrate, and packaging the filtrate into 100mL of vacuum packaging bag by using Chinese medicinal decoction packaging machine (type: YB50-250, manufactured by Beijing Tohua original medical equipment, Ltd.).
EXAMPLE 3 preparation of the Chinese medicinal composition of the present invention
1. The raw material medicaments: 60g of phellodendron, 40g of honeysuckle stem, 36g of astragalus root, 24g of bighead atractylodes rhizome and 8g of angelica.
2. The preparation method comprises the following steps: weighing the raw materials according to the weight ratio; 3 doses of the raw materials are put into a disposable non-woven gauze, and the raw materials are fully soaked for 40 minutes by using 700mL of drinking water meeting the national sanitary standard; decocting with strong fire for 30min at normal pressure, and filtering and sterilizing to obtain filtrate; adding 600mL of drinking water again, decocting for 15 minutes twice, and taking the filtrate; adding 400mL of drinking water for the third time, decocting for 15 minutes, mixing all filtrates to obtain 900mL of filtrate, and packaging the filtrate into 100mL of vacuum packaging bag by using Chinese medicinal decoction packaging machine (type: YB50-250, manufactured by Beijing Tohua original medical equipment, Ltd.).
The beneficial effects of the present invention are demonstrated by specific clinical trials below.
Test example 1 clinical efficacy of the Chinese medicinal composition of the present invention
1. Clinical cases
The observed cases were 55 cases in total, with an average age of 46.74 ± 15.31 years.
1.1 inclusion criteria
Chronic refractory wound surface caused by chronic osteomyelitis, infectious wound surface, diabetic foot infection, tophus infection, rheumatism immune system disease complicated wound surface, and wound surface area of not less than 1cm2At the same time less than 50cm2Thereby, the number of the parts can be reduced.

2. Method of treatment
Using iodophor to disinfect skin around a wound surface, wiping the wound surface with physiological saline, then using forceps for changing the drug to gently scrape necrotic tissues on the surface of the wound surface, flushing the necrotic tissues with the physiological saline, then using the traditional Chinese medicine composition prepared in the embodiment 1 of the invention to slowly flush the wound surface (making the liquid medicine stay on the surface for a little time as much as possible), and then covering with sterile gauze; the traditional Chinese medicine composition is washed for 1 time every day, the dosage of each time depends on the size of the wound surface, the washing liquid is used for washing the whole wound surface, the diameter of the wound surface is usually less than 5cm, the dosage of the wound surface is 10-20 mL, the dosage of the wound surface is more than 5cm, the dosage of the wound surface is 20-100 mL, the dosage of the wound surface with more secretion can be increased appropriately, the skin condition around the wound is poor, such as ischemic necrosis, skin paleness, eczema, pigmentation and the like, and the traditional Chinese medicine composition prepared by the invention can be soaked on sterile gauze for wet dressing.
3. Criteria for judging therapeutic effects
According to the curative effect standard in the 'standard for diagnosing the curative effect of the clinical symptoms of the traditional Chinese medicine' issued by the State administration of traditional Chinese medicine, the evaluation is carried out by combining the clinic:
and (3) healing: the epithelium of the wound surface is completely covered, the scar is solid, and the false healing phenomena such as subcutaneous pus accumulation and the like do not exist;
the effect is shown: the wound surface area is reduced by more than 75 percent, the granulation tissue is fresh and alive, and purulent secretion does not exist;
improvement: the wound surface area reduction range is more than or equal to 25 percent, no or a small amount of purulent secretion exists, and red granulation tissues exist;
and (4) invalidation: the wound area is reduced by less than 25%, and purulent secretion is still present, accompanied by dark granulation tissue.
4. Therapeutic results
(1) Negative conversion rate of wound surface bacteria culture
The wound surface of the patient was subjected to bacterial culture of wound surface secretion 1 time before the test and after the treatment of 7d, 10d, 14d and 21d, respectively, and the bacterial culture to negative conversion rate was calculated as the bacterial negative conversion rate (number of cases of wound surface bacterial culture to negative after the drug administration/number of cases of bacterial culture to positive before the drug administration) × 100%. The bacterial infection exists in 47 cases of wounds before treatment, after 7 days of treatment, 26 cases of bacteria turn negative, the negative turning rate is 55.32%, after 10 days of treatment, 38 cases of bacteria turn negative, the negative turning rate is 80.85%, after 14 days of treatment, 41 cases of bacteria turn negative, the negative turning rate is 87.23%, and after 21 days of treatment, 43 cases of bacteria turn negative, and the negative turning rate is 91.49%.
The test result shows that the traditional Chinese medicine composition has good inhibition effect on wound bacteria.
(2) Therapeutic effect
The curative effect of the traditional Chinese medicine composition is shown in table 1 after the traditional Chinese medicine composition is used for changing 55 chronic intractable wound surfaces for 21 days. The effective rate is the sum of cure rate, significant efficiency and improvement rate.

After 21 days, dressing changes were continued on 46 unhealed wounds until the wounds healed, and finally, all wounds healed, with the average healing time (29.98 ± 8.07) days.
The test result shows that the traditional Chinese medicine composition has a remarkable effect on treating chronic refractory wound surfaces.
5. Typical cases
Typical case 1 calcaneal osteomyelitis with chronic ulcer wound surface
Sex of the patients: a woman; age: age 79; basic diseases: hypertension of grade 3, extremely high risk, hypoproteinemia; the main complaints are: after the internal fixation debridement is taken out, the dressing change effect is poor through modern dressing; after the bacterial culture, the wound is proved to have pseudomonas aeruginosa.
The patient is treated by rinsing with the traditional Chinese medicine composition prepared in example 1, and is rinsed and wet-compressed 1 time per day, and the dosage is 20 mL. The patients begin to wash from 10 and 12 days in 2018 to 11 and 27 days in 2018, as shown in fig. 1, the wound surface epithelium of the patients can be seen to be completely covered, the scars are firm, and the false healing phenomena such as subcutaneous purulence and the like do not exist.
Typical case 2 tophus accompanied with infection wound
Sex of the patients: male; age 75 years; basic diseases: hypertension of grade 3, extremely high risk, gout, abnormal renal function, old tuberculosis; the main complaints are: gout is found for 20+ years, and tophus of the first metatarsophalangeal joint of the right foot is broken for 1+ month; after the bacteria culture, the staphylococcus aureus is proved to be present at the wound.
The traditional Chinese medicine composition prepared in the embodiment 1 is used for washing treatment for a patient, and the washing is performed for 1 time every day, wherein the dosage is 20-40 mL each time. The patients begin to wash from 11 and 7 days in 2018 to 12 and 18 days in 18 months, as shown in fig. 2, the wound surface epithelium of the patients can be seen to be completely covered, the scars are firm, and the false healing phenomena such as subcutaneous purulence and the like do not exist.
Typical case 3 chronic osteomyelitis postoperative chronic infectious wound surface
Sex of the patients: male; age: age 49 years old; basic diseases: chronic osteomyelitis of the left tibia; the main complaints are: after one year of left shank infection focus chronic ulcer removal operation, the exudation is carried out for 1+ month; after the bacterial culture, the presence of pseudomonas aeruginosa at the wound is proved.
The traditional Chinese medicine composition prepared in example 1 is used for washing treatment for a patient, and washing is carried out 1 time per day, and the dosage is 20 mL. The patients begin to wash from 27 th 9 th 2018 to 30 th 10 th, as shown in fig. 3, the wound surface epithelium of the patients can be completely covered, the scars are firm, and the false healing phenomena such as subcutaneous purulence and the like do not exist.
In conclusion, the Chinese medicinal composition has the effects of clearing away heat and toxic materials, tonifying qi and nourishing blood, and astringing and promoting granulation; mainly treats the chronic nonunion of the wound surface caused by lingering pathogen due to deficiency of vital qi; has obvious effect on treating chronic intractable wound.
Claims (8)
1. A Chinese medicinal composition for promoting the healing of chronic intractable wounds by clearing heat and drying dampness, tonifying qi and removing toxicity, and activating blood and enriching blood is characterized in that: the external preparation is prepared from the following raw material medicines in parts by weight:
50 parts of phellodendron, 50 parts of honeysuckle stem, 30 parts of astragalus, 20 parts of bighead atractylodes rhizome and 10 parts of angelica.
2. The traditional Chinese medicine composition according to claim 1, characterized in that: the external preparation is prepared by taking the medicinal powder of the raw material medicines, the water or organic solvent extract of the raw material medicines as active ingredients and adding pharmaceutically acceptable auxiliary materials according to the weight ratio.

The method of claim 5, wherein: the raw materials decocted by adding water for 3 times are: adding water 4-5 times v/w of the raw materials for the first time, soaking for 20-40 min, decocting for 20-40 min, and filtering; adding 3-4 times v/w of water into the raw materials at the 2 nd time, decocting for 10-30 min, and filtering; and adding water 2-3 times v/w of the raw materials for 3 times, decocting for 5-20 min, and filtering.
