背景技术
褥疮又称压迫性溃疡，系长期压迫导致局部组织缺血坏死所致。坏死组织是细菌侵入繁殖或形成感染的主要条件，感染又会进一步加重局部血运障碍和局部组织缺氧坏死。如此恶性循环，最终可导致深达皮下、肌层，乃至达骨面形成难以愈合的溃疡。褥疮是长期卧床病人，特别是老年、昏迷、截瘫、坐轮椅者的常见并发症。其好发于骶尾、肘踝、背脊等易受压部位，以皮肤破溃、伤口经久不愈为特征。III期以上褥疮往往迁延不愈、复发率高，治疗及护理十分棘手，常因感染导致败血症而危及生命。随着中国社会老龄化程度的加深，目前褥疮患者人数急剧增加，因此开发疗效好，使用方便的治疗褥疮药物势在必行。
发明内容
本发明的目的是提供一种治疗褥疮的中药组合物及其制备方法。
本发明的一种治疗褥疮的中药组合物，其配方的重量百分比为：马勃为50％-70％，黄连为25％-45％，冰片为5％-10％。
本发明的一种治疗褥疮的中药组合物，按照君、臣、佐、使进行组方，马勃为君药，荑连为臣药，冰片为佐使药。
马勃，味辛性平，入肺经，有消肿、止血、解毒等功效，用于咽喉肿痛及各种出血。现代研究表明，马勃在治疗褥疮、臁疮、糖尿病足、肿瘤等方面效果显著。故方中马勃作为君药，起到消肿、解毒、生肌、敛疮的作用。
黄连其味苦、性寒、归心、脾、胃、肝、胆、大肠经，具有清热燥湿、泻火解毒的功效，是一种很好的清热解毒消炎药，具有广谱抗生素的作用。常用于治疗湿热内蒸、烦躁呕逆、泄泻痢疾、消渴、痈疽肿毒、急性扁桃体炎、流行性脑脊髓膜炎、胆囊炎等，且具有抗癌、抗放射及促进细胞代谢的作用。方中黄连可辅助马勃治疗褥疮溃疡，促进伤口生肌愈合，防止伤口感染等作用，作为臣药。
冰片味辛、苦、微寒，归心、脾、肺经。用于闭证神昏、用于目赤肿痛，喉痹口疮、用于疮疡肿痛，溃后不敛等。冰片在方中辅佐马勃、黄连共同起到抑菌、生肌、消炎和止痛的作用，促进褥疮伤口快速愈合。
本发明的一种治疗褥疮的中药组合物的制备方法，其步骤和条件如下：(1)按照重量百分比，称量马勃，①把马勃粉碎，过200目筛，得马勃粉末；②或者，把马勃，加8倍重量的乙醇或加8倍重量的乙酸乙酯，回流提取2次.每次1.5小时，滤过，合并滤液，在50℃下，浓缩滤液至相对密度1.30-1.35的稠膏，减压干燥，粉碎，过100目筛，得马勃粉末；
(2)按照重量百分比，称量黄连，将黄连加8倍量水，回流提取2次，滤过，合并滤液，50℃下，浓缩滤液至相对密度1.30-1.35的稠膏，减压干燥，粉碎，过100目筛，黄连粉末；(3)按照重量百分比，称量冰片，粉碎，过100目筛，得冰片粉末；(4)将上述三种药材粉末混合均匀，与制剂学上可接受的赋形剂或载体一起，用现有的中药制备技术和工艺方法制成外用剂型，包括涂膜剂、凝胶剂、巴布剂、乳膏剂、浸药绷带、创可贴、喷雾剂。
本发明的一种治疗褥疮的中药组合物的外用使用方法如下：
涂膜剂、凝胶剂、乳膏剂均匀涂于患处，薄薄一层不裸露出患处即可。每日二次。创可贴、巴布剂、绷带贴于患处固定；每日二次，每次一贴。喷雾剂每次均匀喷于患处，使创面湿润。每日二次，
上述均七天一疗程，可连续使用3-4疗程。使用过程中如有皮肤过敏，可暂停用药。中医一般在临床上把褥疮分为四度：
①1度：表皮无损伤，只是皮肤发红，但解除压迫30分钟以上发红尚无改善者，此期为急性炎症反应期。
②II度：表皮发红、糜烂，有水泡，组织缺损未及真皮，创面湿润呈粉红色，伴有疼痛，无坏死组织。⑧III度：由真皮达皮下，为喷火口状的组织缺损，伴有渗出液和感染，但几乎无疼痛，有坏死纽织。④Ⅳ度：深达肌腱及骨，有渗出液和感染，有坏死组织，如有神经损伤则伴有剧烈疼痛。
中医疗效评定标准：
治愈：创面完全愈合，结痂并脱落；显效：创面缩小，无分泌物，肉芽组织生长正常；有效：渗出液减少，创面无扩大；无效：创面无变化或者扩大。以治愈、显效和有效计算总有效率。
使用本发明的一种治疗褥疮的中药组合物对褥疮的患者进行治疗，褥疮患者的患病时间均一个月以上。治疗均七天一疗程，连续使用3-4疗程。分三个批次进行统计，治疗结果如下：
第1批次：褥疮患者30例，其中男14例，女16例，年龄61～89岁，平均75岁。根据一般临床上褥疮分类标准，II期19例，III期11例。压疮部位：骶尾部21例，足跟部2例，髋部5例，骶尾部加足跟部2例。压疮面积：2cm×lcm～5cm×6cm。治疗方法，所有患者均给予常规消毒创面皮肤，并用生理盐水清洗创面。给予涂膜剂均匀涂于局部，每天2次。情况好转后每日1次。观察治疗情况。治愈11例(36.7％)，显效13例(43.3％)，有效4例(13.3％)，无效2例(6.7％)，总有效率93.3％。
第2批次：褥疮患着30例，其中男17例，女13例，年龄56～79岁，平均66岁。根据一般临床上褥疮分类标准，II期30例。压疮部位：骶尾部18例，髋部8例，骶尾部加足跟部4例。压疮面积：2cm×lcm～4cm×6cm。治疗方法，所有患者均给予常规消毒创面皮肤，并用生理盐水清洗创面。给予喷雾剂喷于局部患处，每天2次。情况好转后每日1次。
观察治疗情况。治愈12例(40％)，显效13例(43.3％)，有效4例(13.3％)，无效1例(3.3％)，总有效率96.6％。
第3批次：褥疮患者30例，其中男20例，女10例，年龄65～88岁，平均74岁。
根据一般临床上褥疮分类标准，II期11例，III期19例。压疮部位：骶尾部20例，髋部6例，骶尾部加足跟部4例。压疮面积：2cm×2cm～5cm×6cm。治疗方法，所有患者均给予常规消毒创面皮肤，并用生理盐水清洗创面。给予凝胶剂均匀涂于局部，每天2次。情况好转后每日1次。观察治疗情况。治愈11例(36.7％)，显效12例(40％)，有效3例(10％)，无效4例(13.3％)，总有效率86.7％。
综上所述，治疗褥疮患者90例，其中男51例，女39例，年龄56～89岁，平均71.6岁。治愈34例，占37.8％；显效38例，占42.2％；有效11例，占l2.2％；无效7例，占7.8％。总有效率为92.2％。
有益效果：本发明的一种治疗褥疮的中药组合物，按照君、臣、佐、使进行组方，马勃为君药，黄连为臣药，冰片为佐使药。故方中马勃作为君药，起到消肿、解毒、生肌、敛疮的作用。黄连可辅助马勃治疗褥疮溃疡，促进伤口生肌愈合，防止伤口感染等作用，作为臣药。冰片在方中辅佐马勃、黄连共同起到抑菌、生肌、消炎和止痛的作用，促进褥疮伤口快速愈合。
本发明的一种治疗褥疮的中药组合韧的制备方法，把马勃、黄连通过现代科学方法，回流提取其有效成分，再制得马勃、黄连粉末，使得马勃、黄连的疗效更为突出。用本发明的一种治疗褥疮的中药组合物，外用，治疗褥疮患者90例，其中男51例，女39例，年龄56～89岁，平均71.6岁。治愈34例，占37.8％；显效38例，占42.2％；有效11例，占l2.2％；无效7例，占7.8％。总有效率为92.8％。
实践证明，本发明制备的一种治疗褥疮的中药组合物，成分简单，配方科学，制成外用剂型，成本低，使用方便，疗效显著。
具体实施方式
制备实施例1一种治疗褥疮的中药组合物的涂膜剂，其配方的重量百分比为：马勃为70％，黄连为25％，冰片5％。
一种治疗褥疮的中药组合物的涂膜剂的制备方法，步骤和条件如下：(1)按照重量百分比，称量马勃粉末700克；马勃粉末的制备方法如下：把马勃粉碎，过200目筛，得到马勃粉末；
(2)按照重量百分比，称量黄连粉末250克；黄连粉末的制备方法如下：称量黄连，将黄连加8倍重量的水，回流提取2次，滤过，合并滤液，在50℃下，浓缩滤液至相对密度1.30-1.35的稠膏，减压干燥，粉碎，过100曰筛，得黄连粉末；
(3)按照重量百分比，称量冰片粉末50克，冰片粉末的制备方法如下：把黄连粉碎，过100目筛，得冰片粉末；
(4)将马勃粉末、黄连粉末、冰片粉末，用现有的中药制备技术和工艺方法，加入适量的PVA-124、甘油、吐温-80、乙醇等赋形剂，搅拌研磨均匀，即得一种治疗褥疮的中药组合物的涂膜剂。
制备实施例2：一种治疗褥疮的中药组合物的软膏剂，其配方的重量百分比为：马勃为50％，黄连为45％，冰片为5％。
一种治疗褥疮的中药组合物的涂膜剂的制备方法，步骤和条件如下：按照重量百分比，称量马勃500克、黄连450克、冰片50克，步骤(1)、(2)、(3)同实施例I；(4)将马勃粉末、黄连粉末、冰片粉末，用现有的中药制备技术和工艺方法，加入适量的羊毛脂、蜂蜡、凡士林等赋形剂，搅拌研磨均匀，即得一种治疗褥疮的中药组合物的乳膏剂。
制备实施例3：一种治疗褥疮的中药组合物的喷雾剂，其配方的重量百分比为：马勃为40％，黄连为50％，冰片10％。
一种治疗褥疮的中药组合物的涂膜剂的制备方法，步骤和条件如下：按照重量百分比，称量马勃400克，黄连为500克、冰片100克，步骤(1)、(2)、(3)同实施例l；(4)将马勃粉末、黄连粉末、冰片粉末，用现有的中药制备技术和工艺方法，加入适量的乙醇、乙酸乙酯、水等溶剂，混合均匀，即得一种治疗褥疮的中药组合物的喷雾剂。
Claims (5)
1.一种治疗褥疮的中药组合物，其特征在于，其配方的重量百分比为马勃为50％-70％，黄连为25％-45％，冰片为5％-10％。
2.如权利要求l所述的一种治疗褥疮的中药组合物，其特征在于，其配方的重量百分比为马勃为70％，黄连为25％，冰片5％。
3.如权利要求l所述的一种治疗褥疮的中药组合物，其特征在于，其配方的重量百分比为马勃为50％，黄连为45％，冰片5％。
4.如权利要求l所述的一种治疗褥疮的中药组合物，其特征在于，其配方的重量百分比为马勃为70％，黄连为25％，冰片5％。
5.如权利要求l所述的一种治疗褥疮的中药组合物的制法，其特征在于，其步骤和条件如下：(1)按照重量百分比，称量马勃，①把马勃粉碎，过200目筛，得马勃粉末；②或者，把马勃，加8倍重量的乙醇或加8倍重量的乙酸乙酯，回流提取2次，每次1.5小时，滤过，合并滤液，在50℃下，浓缩滤液至相对密度1.30-1.35的稠膏，减压干燥，粉碎，过100目筛，得马勃粉末；(2)按照重量百分比，称量黄连，将黄连加8倍量水，回流提取2次，滤过，合并滤液，在50℃下，浓缩滤液至相对密度1.30-1.35的稠膏，减压干燥，粉碎，过100目笳，得黄连粉末；(3)按照重量百分比，称量冰片，粉碎，过100目筛，得冰片粉末；(4)将上述三种药材微粉混合均匀，与制剂学上可接受的赋形剂或载体一起，用现有的中药制备技术和工艺方法制成外用剂型。
