Abstract
一种用于减肥的中药,属于中药技术领域。本发明的目的是采用中医药学理论,针对肥胖症的病因而设计的用于减肥的中药。本发明制成有效成分的重量份组成为:茯苓、白术、泽泻、车前子、荷叶、决明子、莱菔子、山楂、何首乌、火麻仁、郁李仁、陈皮。本发明为纯中药制成,是在临床基础上总结出来的经验方,有较好的疗效,在临床上应用为汤剂:在现有的剂型中,只有合剂和颗粒剂能满足制剂的要求,由于合剂是液体制剂,在贮存、运输、携带等方面存在问题,相比之下颗粒剂是固体剂型,既保持汤剂吸收快、显效迅速的特点,又具有性质稳定,便于贮存、运输,携带、服用方便,味甜适口的特点,故选择颗粒剂为本制剂的剂型。

背景技术
由于生活条件的不断改善,再加上日常工作压力等因素,肥胖一直困扰着人们,不但影响着人们的日常生活,有可能还危机生命。因此市面上相应的推出许多减肥类药物,西医药物一般以破坏脂肪细胞,然后抑制自己的食欲等方式的药物居多,而中医药物更是五花八门,然而效果不是很理想。
发明内容
本发明的目的是采用中医药学理论,针对肥胖症的病因而设计的用于减肥的中药。

制法
以上十二味,分别加入10、8、8倍量水,煎煮三次,每次1小时,合并煎液,滤过,滤液减压浓缩至相对密度1.20~1.25(80℃),加微粉硅胶、可溶性淀粉适量,混匀,制成颗粒,干燥,制成1000g,即得。

工艺合理性研究
(一) 剂型的选择
本处方是在临床基础上总结出来的经验方,有较好的疗效,在临床上应用为汤剂:在现有的剂型中,只有合剂和颗粒剂能满足制剂的要求,由于合剂是液体制剂,在贮存、运输、携带等方面存在问题,相比之下颗粒剂是固体剂型,既保持汤剂吸收快、显效迅速的特点,又具有性质稳定,便于贮存、运输,携带、服用方便,味甜适口的特点,故选择颗粒剂为本制剂的剂型。
(二) 原料药的鉴定与前处理
1. 茯苓 本品为多孔菌科真菌茯苓Poria cocos (Schw.)Wolf的干燥菌核。主产于湖北、河南、安徽、浙江等地,符合《中华人民共和国药典》(2015年版)一部第240页该药材项下有关规定。
2. 白术本品为菊科植物白术Atractylodes macrocephala Koidz.的干燥根茎。主产于四川、云南、贵州、浙江。符合《中华人民共和国药典》(2015年版)一部第104页该药材项下有关规定。
3. 泽泻本品为泽泻科植物泽泻Alisma orientale (Sam.) Juzep.的干燥块茎。主产于黑龙江、吉林。符合《中华人民共和国药典》(2015年版)一部第229页该药材项下有关规定。
4. 车前子本品为车前科植物车前Plantago asiatica L.或平车前Plantago depressssa Willd.的干燥成熟种子。主产于黑龙江、辽宁、河北。符合《中华人民共和国药典》(2015年版)一部第68页该药材项下有关规定。
5. 荷叶本品为睡莲科植物莲Nelumbo nucifera Gaertn.的干燥叶。主产于湖南、福建、江苏、浙江、南方各地。符合《中华人民共和国药典》(2015年版)一部第275页该药材项下有关规定。
6 .决明子本品为豆科植物决明Cassia obtusifolia L. 或小决明Cassia tora L.的干燥成熟种子。主产于安徽、广西、四川、浙江、广东等地。符合《中华人民共和国药典》(2015年版)一部第145页该药材项下有关规定。
7. 莱菔子本品为十字花科植物萝卜Raphanus sativus .L.的干燥成熟种子。
全国各地均有栽培。符合《中华人民共和国药典》(2015年版)一部第272页该药材项下有关规定。
8. 山楂本品为蔷薇科植物山里红Crataegus pinnatifida Bge.var.majorN.E.Br.或山楂Crataegus pinnatifida Bge.的干燥成熟果实。主产于山东、陕西、山西、河南、江苏、浙江、辽宁、吉林、黑龙江、内蒙古、河北。符合《中华人民共和国药典》(2015年版)一部第31页该药材项下有关规定。
9. 何首乌本品为蓼科植物何首乌Polygonum multiflorum Thunb.的干燥块根。主产于陕西、甘肃、华东、华中、华南、四川、云南及贵州。符合《中华人民共和国药典》(2015年版)一部第175页该药材项下有关规定。
10. 火麻仁本品为桑科植物大麻Cannabis sativa L.的干燥成熟果实。我国各地均有栽培,也有半野生者。分布于东北、华北、华东、中南等地。符合《中华人民共和国药典》(2015年版)一部第80页该药材项下有关规定。
11. 郁李仁本品为蔷薇科植物欧李Prunus humilis Bge.、郁李Prunus japonicaThunb.或长柄扁桃Prunus pedunculata Maxim.的干燥成熟种子。主产于黑龙江、吉林、辽宁、内蒙古、河北、山东。符合《中华人民共和国药典》(2015年版)一部第207页该药材项下有关规定。
12. 陈皮本品为芸香科植物橘Citrus reticulata Blanco及其栽培变种的干燥成熟果皮。主产于福建、浙江、广东、广西、江西、湖南、贵州、云南、四川等地。。符合《中华人民共和国药典》(2015年版)一部第191页该药材项下有关规定。
前处理:将各味药挑杂,淋洗,烘干,投药即可。
药材提取工艺研究
1.处方中中药的化学成分概述
(1).茯苓含β-茯苓聚糖约占干重93%和三萜类化合物乙酰茯苓酸、茯苓酸、3β-羟基羊毛甾三烯酸。此外,尚含树胶、甲壳质、蛋白质、脂肪、甾醇、卵磷脂、葡萄糖、腺嘌呤、组氨酸、胆碱、β-茯苓聚糖分解酶、脂肪酶、蛋白酶等。
(2).白术含挥发油,内有α-及β-葎草烯,β-榄香醇,α-姜黄烯,苍术酮3β-乙酰氧基苍术酮,芹子二烯酮,桉叶醇,棕榈酸,茅术醇,β-芹子烯等。还含倍半萜内酯化合物:苍术内酯-Ⅰ、-Ⅱ、-Ⅲ及8β-乙氧基苍术内酯-Ⅱ。另含东莨菪素,具免疫活性的甘露聚糖AM-3。
(3).泽泻含泻醇A、B、C、泽泻醇A单乙酸酯,23-乙酰泽泻醇B泽泻醇,泽泻醇,泽泻薁醇,泽泻薁醇氧化物,16β-甲氧基泽泻醇、16β-羟基泽泻醇B单乙酸酯,谷甾醇-3-O-硬脂酰基-β-D-吡喃葡萄糖甙。还含胆碱。
(4).车前子含月桃叶珊瑚甙,车前粘多糖,消旋-车前子甙,都桷子甙酸,车前子酸,琥珀酸,腺嘌呤,胆碱及10.43% 的脂肪油,β-谷甾醇、β-谷甾醇-3-O-β-D-吡喃葡萄糖甙。
(5).荷叶含荷叶碱、柠檬酸、苹果酸、葡萄糖酸、草酸、琥珀酸等多种成分。
(6).决明子含橙黄决明素、大黄酚等多种蒽醌类化合物。
(7).莱菔子含芥子碱和脂肪油30%,油中含大量的芥酸及亚油酸、亚麻酸,还含菜子甾醇和22-去氢菜油甾醇。另含菜菔素。
(8).山楂含黄酮类及有机酸类化合物。黄酮类化合物主要有牡荆素、槲皮素、槲皮苷、金丝桃苷和芦丁;有机酸主要有山楂酸、柠檬酸、熊果酸等。另外尚含有磷脂、维生素C、维生素B2等。
(9).何首乌块根含蒽醌类化合物,主为大黄素,大黄酚以及大黄素甲醚,大黄酸,大黄酚蒽酮。又含芪类化合物:白藜芦醇,云杉新甙,2,3,5,4’-四羟基二苯乙烯-2-O-β-D-葡萄糖苷,2,3,5,4’-四羟基二苯乙烯-2-O-β-D-葡萄糖苷-2″-O-没食子酸酯、2,3,5,4′-四羟基芪-2-O-葡萄糖甙-3″-O-没食子酸酯。还含没食子酸,右旋儿茶精,右旋表儿茶精,3-O-没食子酰(-)-儿茶精,3-O-没食子酰(-)-表儿茶精,β-谷甾醇,卵磷脂。
(10). 火麻仁含脂肪油约30%。属于干性油。油的脂肪酸,饱和的为4.5~9.5%;不饱和的脂肪酸中,油酸约为12%,亚油酸53%,亚麻酸25%。油中含一些大麻酚。种子含胡卢巴碱及dl-异白氨酸甜菜碱,此外含麻仁球朊酶及蛋白质等。
(11). 郁李仁含苦杏仁甙、脂肪油、皂甙等。郁李种子含苦杏仁甙、脂肪油58.3~74.2%、挥发性有机酸、粗蛋白质、纤维素、淀粉、油酸。又含皂甙0.96%及植物甾醇、维生素B1。从郁李的果实中分得郁李仁甙A,还有香草酸,原儿茶酸、熊果酸,以及黄酮类化合物阿福豆甙、山柰甙、多花甙A和B、多花糖甙。
陈皮含挥发油1.9%~3.5%.其中主要为柠檬烯;尚有α-侧柏烯、α-蒎烯、β-蒎烯、β-月桂烯、桧烯、辛醛、α-水芹烯、α-松油烯、对-聚伞花烯、芳樟醇、麝香草酚、香茅醛等多种成分。此外,尚含橙皮甙、新橙皮甙、柑橘素、枸橼醛、β-谷甾醇等。
2.提取工艺路线的设计
本处方是在临床基础上总结出来的经验方,临床上应用为汤剂,有较好的疗效,为了保证疗效,仍采用水煎煮的方法进行提取。
3.提取工艺条件的研究
(1). 药材吸水率的测定
按处方比例取茯苓20g、白术20g、泽泻20g、车前子15g、荷叶10g、决明子10g、莱菔子10g、山楂10g、何首乌10g、火麻仁10g、郁李仁10g、陈皮10g,(共计药材155g)加水1000ml浸泡,每隔30分钟观察一次浸透的情况,直至药材全部浸透,时间2小时,滤出全部未被吸收的水液,求得混合药材吸水率为192%。确定第一次提取时需多加2倍量的水。

(2).水煎煮工艺条件的考察
对水煎煮来说,影响提取效果的因素主要有3个,即:加水量、煎煮次数、煎煮时间,使用正交表 L9(34)安排试验,每个因素选择了三个水平,见表2 :

考察指标:对提取工艺的评价,采用出干浸膏量为考察指标。
1).正交试验方法
分别称取茯苓20g、白术20g、泽泻20g、车前子15g、荷叶10g、决明子10g、莱菔子10g、山楂10g、何首乌10g、火麻仁10g、郁李仁10g、陈皮10g等药材9份,每份155g,按照因素水平表的条件进行试验,滤过,合并提取液,减压浓缩,减压干燥,称重,计算干浸膏重量和出膏率,以干浸膏重量计算正交试验结果,见表3、表4、表5:
2).正交试验结果
以出膏量、药材中醇浸出物含量和综合评分为指标进行分析,结果见表4、表5。

以干浸膏量为评价指标,直观分析,A3>A2>A1,B 3>B 2>B 1,C 3>C 1>C 2,最优条件:A3B3C3,方差结果:A因素水平变化对试验结果有极显著的影响,选择A3水平,B因素水平变化对试验结果有显著的影响,选择B 3水平, C因素没有显著的影响,因素水平可任选其一,要考虑节省能源和工时,选择C 1水平。故选择A3B3C1为本制剂的煎煮条件,即:加8倍量水,煎煮3次,每次1小时。
5).验证试验:
根据正交试验结果,对优选出来的煎煮条件A3B3C1进行了3组验证试验,分别加入10、8、8倍量水,煎煮三次,每次1小时,结果如下:

验证性试验结果表明,三批样品的出膏率的RSD分别为2.39%,重现性良好,故确定A3B3C1为最佳提取工艺。
6).小试样品的制备
按处方量称取药材:茯苓200g、白术200g、泽泻200g、车前子150g、荷叶100g、决明子100g、莱菔子100g、山楂100g、何首乌100g、火麻仁100g、郁李仁100g、陈皮100g,(共计药材1550g),以上十二味,分别加入10、8、8倍量水,煎煮三次,每次1小时,合并煎液,滤过,滤液减压浓缩至相对密度1.20~1.25(80℃),减压干燥(80℃以下),干浸膏粉碎成细粉,称重:386.15g,出膏率24.9%,与验证性试验结果相近。
(四)分离、浓缩的研究
1.分离
在水煎煮提取过程中,药渣与提取液的分离属于粗分离,采用100目尼龙筛网为滤材进行滤过的方式。
2.浓缩与干燥
提取、过滤后的药液采用减压浓缩,浓缩温度不超过80℃,提取、过滤后的溶液需要减压浓缩至一定的相对密度的浸膏,以便于制颗粒,一般浓缩至相对密度1.15~1.25(80℃),本制剂水煎提取液浓缩至相对密度1.20~1.25(80℃)。
(五)制剂成型性研究
由于提取浸膏,有很强的吸湿性,直接制成颗粒在制备和贮存过程中都有一定的困难,需加入适量辅料以满足制备工艺和制剂要求,一般按浸膏:辅料=1:0.5~5 的比例加入辅料。
制颗粒方法有干法制粒和湿法制粒,本实验选择湿法制粒。
1.辅料的选择和用量
制颗粒常用辅料有:蔗糖、糊精、可溶性淀粉、微粉硅胶(二氧化硅)等多种,本试验选择无糖型辅料可溶性淀粉、微粉硅胶(二氧化硅),进行了制颗粒研究。
2.润湿剂的选择
不同浓度的乙醇为常用的润湿剂。本试验采用50~75%的乙醇为润湿剂,调整软材的干湿度。
3.制颗粒方法的考察
1)可溶性淀粉制颗粒研究
浸膏:可溶性淀粉=1:2,能制成颗粒。
2)微粉硅胶制颗粒研究
浸膏:微粉硅胶=1:0.2,能制成颗粒。
由于微粉硅胶有极强的稀释性,而且有助流作用,加入适量,余下辅料加可溶性淀粉调整制成总量,可制成无糖型颗粒。
3)微粉硅胶和可溶性淀粉制颗粒研究
取浸膏适量,加微粉硅胶和可溶性淀粉进行制颗粒研究,结果见表7:

根据上述结果,选择2号比例作为本制剂的辅料用量。
4. 制剂规格和用法用量的确定
原处方为:茯苓20g、白术20g、泽泻20g、车前子15g、荷叶10g、决明子10g、莱菔子10g、山楂10g、何首乌10g、火麻仁10g、郁李仁10g、陈皮10g,总生药量为155g,水煎服,每剂服2天,是临床患者二天(6次)的服用量,则每日需要服用生药量为155÷2=77.5g。
现制剂处方总生药量为1240g,制成1000g颗粒,如果按口服,一日服3次,每次服用量为20g,即1240÷1000×20×3=74.40g
此用量与临床处方量相近,由此推算出处方:

【制法】
以上十二味,分别加入10、8、8倍量水,煎煮三次,每次1小时,合并煎液,滤过,滤液减压浓缩至相对密度1.20~1.25(80℃),加微粉硅胶、可溶性淀粉适量,混匀,制成颗粒,干燥,制成1000g,即得。
【用法用量】:温开水冲服。一次2袋,一日3次。
【规格】:每袋装10g。
5.处方用量与药典用量比较
按上述处方、规格和用法用量与中国药典用量相比较,结果见表8:

6.湿颗粒干燥温度的考察
对湿颗粒干燥,选用了不同温度,结果见表9:

根据实验结果,选择湿颗粒干燥条件为:干燥温度80℃。
7.颗粒的流动性考察
取颗粒成品测定休止角,考察颗粒的流动性。
方法:将颗粒放在固定于培养平皿的中心点上面的漏斗中,颗粒从漏斗中流出,直至颗粒堆积至从平皿上缘溢出为止,测出圆锥陡堆的顶点到平皿上缘的高,测定培养平皿的半径,计算休止角,样品测定三次,取平均值,见表10:

以上结果表明:颗粒休止角为33.7度,表明颗粒流动性良好。
8.成品颗粒的吸湿性考察:
采用在25℃条件下测定颗粒在一定相对湿度时的吸湿情况,考察其吸湿性,试验方法如下:
①恒湿溶液配制:
分别取MgCl2、K2CO3、NaBr、NaCl、KCl、KNO3在25℃制成饱和水溶液,置密闭玻璃干燥器后,放入25℃恒温箱中平衡24小时后,即得相对湿度分别为33%、43%、58%、75%、82%、92%恒湿溶液。
②吸湿量的测定
分别取恒重的称量瓶,加入颗粒约2g,轻轻平铺,使药的厚度约3mm左右,盖好瓶盖,精密称定,打开瓶盖,分别置于上述盛有恒湿溶液的玻璃干燥器内,24小时后, 精密称定,求出增加的重量,计算平衡含水量%,结果见表11:以相对湿度为横坐标,以平衡含水量%为纵坐标作图,即得吸湿曲线。

由上表结果可知,当温度在25℃,相对湿度在43%以下时,颗粒吸湿率较低。所以在颗粒分装和贮存时,要控制温度、相对湿度。
(六)、包装材料的选择
由于成品颗粒中,浸膏所占的比例较大,有一定的吸湿性,故我们选定防水防气性能较好的复合膜为内包装材料,以保证制剂的稳定。
(七)、三批中试样品数据
三批中试试验,按处方量的10倍量投料,中试结果见表12。

由以上数据可见,各项数据接近,稳定性较好,可以作为大生产条件依据。

茯荷减肥颗粒与西药奥利司他治疗单纯性肥胖70例临床观察
1 临床资料和方法
1.1 一般资料
对象均为长春中医药大学附属医院门诊患者,采用随机抽样的方法,将140个病例分为治疗组70例,对照组70例。治疗组70例,其中女42例,男28例,年龄17~63(36.45±15.52)岁;病程最短7个月,最长38年,平均(15.23±3.18)年,轻度肥胖者18例,中度肥胖36例,重度肥胖者16例;对照组70例,女39例,男31例;年龄17~57(35.18±17.72)岁;病程最短8个月,最长42年,平均(17.11±3.12)年;轻度肥胖者28例,中度肥胖32例,重度肥胖10例。两组病例的性别、年龄、病程、病情分布情况均相似,具有可比性(P >0.05)
1.2 诊断标准
参照《实用临床肥胖病学》有关标准:①体重指数(bodymassindex,BMI)≥25。②合并有与肥胖有关的健康障碍10项中的1项以上(2型糖尿病、脂质代谢异常、高血压、高尿酸血症、冠心病、脑梗死、睡眠呼吸暂停综合征、脂肪肝、变形性关节炎、月经异常)。③除外继发性肥胖病。实测体重超过标准体重20%~30%为轻度肥胖,31%~50%为中度肥胖,超过标准体重50%以上为重度肥胖。不能按医嘱完成本疗程的患者,不纳入本临床观察。
1.3 治疗方法
(1)对照组:奥利司他(Orlistat)120mg,3次/d,进餐时服药。
(2)治疗组:茯荷减肥颗粒,温开水冲服,一次2袋,一日3次。
两组患者均要求低脂、低盐、低糖、高蛋白饮食,饥饿感明显时,可吃含膳食纤维丰富的食品如蔬菜、水、豆制品等。
2组病例均治疗15天后评定疗效。
1.4 疗效判断
疗效评定标准:显效:体重下降≥5kg,BMI下降≥4。有效:体重下降≥2Kg,BMI下降≥2。无效:体重下降<2kg,BMI下降<2。
1.5 统计学分析
所得数据以均数±标准差(X±S)表示。所有数据均用SPSS11.5统计软件处理,以计量资料的配对t检验进行统计分析。
2 结果
茯荷减肥颗粒治疗单纯性肥胖有较好的临床疗效,优于口服西药奥利司他。差异有显著性及非常显著性意义,见表1、表2。

两组显效率、总有效率相比均有非常显著性差异,P<0.01。

3 讨论
单纯性肥胖是肥胖症中较常见的类型,特指除外由遗传性疾病、代谢性疾病、外伤或其他疾病所引起的继发性、病理性肥胖,对多种慢性病发生、发展有着直接的影响。因此,寻求无副作用、简便、易行的减肥方法,已成为当今国内外医学界所面临的一个重大课题。有运动减肥、口服药物、针灸等多种方法,但是有副作用较大、难于坚持的缺点,而传统中药治疗肥胖症具有一定的优势。
在本临床观察中,茯荷减肥颗粒治疗单纯性肥胖的临床疗效优于奥利司他。奥利司他是目前大家比较公认的减肥药,但是其胃肠反应较重,难于坚持。因此,笔者认为,茯荷减肥颗粒治疗单纯性肥胖,疗效确切,见效快、费用低,而且操作性强,无副作用,较西药减肥更有优越性,易于在临床推广。

1.一种用于减肥的中药,其特征在于:制成有效成分的重量份组成为:
茯苓20~200、白术20~200、泽泻20~200、车前子15~150、荷叶10~100、决明子10~100、莱菔子10~100、山楂10~100、何首乌10~100、火麻仁10~100、郁李仁10~100、陈皮10~100。
2.根据权利要求1所述的用于减肥的中药,其特征在于:制成有效成分的重量份组成为:茯苓160、白术160、泽泻160、车前子120、荷叶80、决明子80、莱菔子80、山楂80、何首乌80、火麻仁80、郁李仁80、陈皮80。
