Abstract
本发明涉及医药技术领域。特别涉及一种蛤及抗痨丸组方及制备工艺:将组方十八味药材中的蛤蚧、太子参、白及、郁金、鸡内金、茯苓、僵蚕、浙贝母、地骨皮粉碎成细粉,过100目筛,混合均匀,备用;其余九味药材生地黄、枸杞子、桔梗、百部、炙黄芪、瓜萎、石斛、陈皮、乌梅加水提取2次,第一次加8倍量水浸泡2小时,煎煮2小时,第二次加6倍量水煎煮2小时,滤过,合并滤液,滤液浓缩至相对密度为1.25~1.30(60℃测)的浸膏,加入上述药粉,充分搅拌均匀,制丸,50℃干燥,打光,制成1000g,包装,即得。本发明在于提高了药物吸收速度和质量稳定性,便于患者服用。

技术领域
本发明涉及医药技术领域,具体涉及一种蛤及抗痨丸的制备工艺及处方
背景技术
肺结核、肺气肿、哮喘、支气管扩张等疾病均为目前临床上的常见病,多发病,随着环境污染、空气质量的下降、人口老龄化的发生,得到全社会越来越大的重视。
中医认为此类病属于肺痨、肺胀、肺痈、哮证、喘证等病的范畴,病位均在肺,主要由于各种外界因素引起的久咳、久喘、久哮,使肺之体用俱损,气阴双亏,而出现面色苍白,形体消瘦,咳嗽气喘,语声低微,口干咽燥,咳痰带血,舌红少苔,脉细数等临床症状,具有病程较长,缠绵难愈的特点。中医辨证属气阴两虚,以肺肾两虚,气阴双亏,脾虚痰阻,肝火犯肺为主要病机,故对其应以补气养阴、补肺益肾、健脾化痰、清肝敛肺、止血化瘀之法治疗。
我们在中医药整体观念理论的指导下,辨证论治,采用价格低廉、无毒副作用的中药制剂进行治疗,疗效确切。蛤及抗痨丸根据我们在长期临床的实践经验,并在《医疗机构制剂注册管理办法》的指导下,参照《药品注册管理办法》,严格按照医疗机构制剂研究的技术要求等文件精神,根据中医药理论,参照现代药理研究成果,结合长期临床应用的实践,形成中药复方医疗机构制剂,为临床治疗结核病(耐药性结核病)提供一种新的治疗药物选择,以取得良好的社会效益及显著的经济效益。
发明内容
本发明的目的在于提高了药物吸收速度和质量稳定性,便于患者服用。
本发明为解所述问题的技术解决方案是:一种蛤及抗痨丸的制备工艺及处方
其特征为,1、方解:肺结核、肺气肿等疾病均为目前临床上的常见病,多发病,随着环境污染、空气质量的下降、人口老龄化的发生,得到全社会越来越大的重视。中医认为此类病属于肺痨、肺胀、肺痈等病的范畴,病位均在肺,主要由于各种外界因素引起的久咳、久喘、久哮,使肺之体用俱损,气阴双亏,而出现面色苍白,形体消瘦,咳嗽气喘,语声低微,口干咽燥,咳痰带血,舌红少苔,脉细数等临床症状,具有病程较长,缠绵难愈的特点。中医辨证属气阴两虚,以肺肾两虚,气阴双亏,脾虚痰阻,肝火犯肺为主要病机,故对其应以补气养阴、补肺益肾、健脾化痰、清肝敛肺、止血化瘀之法治疗。蛤及抗痨丸正是根据这一中医药理论,在我们长期临床医疗经验的基础上,参照现代药理研究成果,研制而成的中药复方制剂。
蛤及抗痨丸系蛤蚧、太子参、白及、生地黄、枸杞子、郁金、桔梗、百部、鸡内金、炙黄芪、茯苓、瓜蒌、僵蚕、石斛、浙贝母、陈皮、乌梅、地骨皮等中药组成的中药复方制剂,功能补气养阴、补肺益肾、健脾化痰、清肝敛肺、止血化瘀。肺结核、肺气肿、哮喘、支气管扩张中医辨证属气阴双虚症者,证见面色苍白,形体消瘦,咳嗽气喘,语声低微,口干咽燥,咳痰带血,舌红少苔,脉细数等症。
方中蛤蚧味咸,性平,归肺、肾经,功能补肺益肾,纳气定喘,助阳益精,《本草经疏》说其“主久肺劳咳嗽、淋沥者,皆肺肾为病,劳极则肺肾虚而生热,故外邪易侵,内证兼发也”;枸杞子味甘,性平,归肝、肾经,滋补肝肾,益精明目。《纲目》谓其能“滋肾,润肺,明目”,以上两位药补肺益肾,纳气定喘,针对本病肺肾两虚主要病机而设,为君药。
方中太子参味甘、微苦,性平,归脾、胃经,功能益气健脾,生津润肺,《本草再新》谓其“治气虚肺燥,补脾土,消水肿,化痰止渴”;生地黄味甘、苦,性寒,归心、肝、肾经,功能清热凉血,养阴,生津;石斛味甘,性微寒,归胃、肺、肾经,功能生津益胃,清热养阴,《本草再新》说其能“理胃气,清胃火,除心中烦渴,疗肾经虚热,安神定惊,解盗汗,能散暑”,以上三味药,太子参补气,生地黄、石斛滋阴,针对本病气阴两亏病机而设,助君药补肺益肾,为臣药。
方中浙贝母味苦,性寒,归肺、心经,功能清热化痰止咳,解毒散结消痈;瓜蒌味甘、微苦,性寒,归肺、胃、大肠经,功能清热涤痰,宽胸散结,润燥滑肠;茯苓味甘、淡,性平,归心、肺、脾、肾经,功能利水渗湿,健脾,宁心,《日华子本草》谓其能“补五劳七伤,安胎,暖腰膝,开心益智,止健忘”;百部味甘、苦,性微温,归肺经,功能润肺下气止咳,杀虫,《滇南本草》谓其能“润肺,治肺热咳嗽:消痰定喘,止虚痨咳嗽,杀虫”;桔梗味苦、辛,性平,归肺经,功能宣肺,利咽,祛痰,排脓;陈皮味苦、辛,性温,归肺、脾经,功能理气健脾,燥湿化痰,以上六味药,健脾化痰、止咳平喘,针对本病脾虚痰阻病机而设,为佐药。
方中僵蚕味咸、辛,性平,归肝、肺、胃经,功能祛风定惊,化痰散结;郁金味辛、苦,性寒,归肝、心、肺经,功能活血止痛,行气解郁,清心凉血,利胆退黄;乌梅味酸、涩,性平,归肝、脾、肺、大肠经,功能敛肺,涩肠,生津,安蛔;白及味苦、甘、涩,性微寒,归肺、肝、胃经,功能收敛止血,消肿生肌;地骨皮味甘,性寒,归肺,肝,肾经,功能凉血除蒸,清肺降火,以上五味药,清肝敛肺,止血化瘀,针对本病肝火犯肺,灼伤血络病机而设,为使药。
综观全方,谨守病机,配伍精当,药专力宏。全方在注重补气养阴的同时兼顾清肝敛肺止血,同时化痰健胃,顾护脾肾,辨证论治,整体调理,共奏补肺益肾,健脾化痰,止咳平喘,清肝敛肺,止血化瘀之功,是治疗肺结核、肺气肿等疾病的良方。
2、制备::A.粉碎操作规程:十八味药材中,将蛤蚧、太子参、白及、郁金、鸡内金、茯苓、僵蚕、浙贝母、地骨皮按处方量取饮片干燥、破碎初混、粉碎终混成细粉,过100目筛,混合均匀,备用。B.水提取浓缩操作规程:其余九味药材按处方量取饮片,加水提取2次,第一次加8倍量水浸泡2小时,煎煮2小时,第二次加6倍量水煎煮2小时,滤过,合并滤液,滤液浓缩至相对密度为1.25~1.30(60℃测)的浸膏,备用。C.制软材:取药粉、药膏备用,每锅配料药粉、药膏的比例为100:(29-37),称量、复核后按比例加入槽式混合机中,执行CH-200/2型槽式混合机操作规程进行混合,搅拌25分钟,合好的药坨,放置于洁净的容器内;再将药坨加到炼药机内,执行GHL-30C型炼药机操作规程,交制丸工序。d.制丸:选择出条板孔径6.0mm、切刀口距6.3mm,调节出条和切刀速度,调节润滑剂(95%乙醇)的流速,每隔30分钟测定一次湿丸重,调整出条速度,控制10粒丸重在1.55-1.65g之内。制好的湿基丸置糖衣锅内旋转30min,至丸面光滑。e.干燥:按照复合圆振动干燥机操作规程开启干燥机,待蒸汽压力达到(0.2~0.3)Mpa,药温(47.5±2.5)℃时,振动频率28.5Hz,开始上湿丸。将吸料管放入盛装湿丸的桶内,开启上料器将丸粒吸入干燥机内,边上边出至完。放出的丸粒盛装于洁净的不锈钢桶中。再将吸料管放入桶内,开启上料器将收得的丸粒吸入干燥机内,待全部丸粒上完后,开始干燥计时。干燥时间:50min。收毛干丸。f.选丸:将药丸置溜丸机上分离,收集合格药丸于洁净容器中,g.打光:将筛选合格的药丸置于糖衣机中,启动糖衣机,连续加入适量纯化水,旋转10分钟,观察色泽,再旋转10分钟,开启热风(移动热风头均匀加热),再转5min,停止转动,每隔5min点动一次,吹热风共20min,再吹凉风(不加热)至干燥,交中间站请验、待检。得干燥丸总收率不低于86.8%~98.5%%,物料平衡不低于87.8%~99.0%%。包装。

五、剂型选择
本处方原为一临床验方,该处方用于临床多年,疗效确切。浓缩丸是目前丸剂中较好的一种剂型,其特点是药物全部或部分经过提取浓缩,体积缩小,易于服用和吸收,发挥药效好;同时利于保存,不易霉变。因此,拟制成浓缩丸剂。
六、工艺研究
(一)药材的来源及鉴定依据
处方中十八味药材,均收载于中国药典2010年版一部,见表1。

(二)工艺路线框架的设计
根据处方中药材所含主要有效成分的理化性质和药理作用,处方中生地黄、枸杞子、桔梗、百部、炙黄芪、瓜蒌、石斛、陈皮、乌梅等九味药材其有效成分均可溶于水,故用水煎煮的方法对其进行提取。处方中蛤蚧、太子参、白及、郁金、鸡内金、茯苓、僵蚕、浙贝母、地骨皮等九味药材易于粉碎,为保留其全部活性成分,故采用粉碎成细粉直接入药。同时也可起到辅形剂作用,降低成本。
(三)工艺考察
1.处方中蛤蚧、太子参、白及、郁金、鸡内金、茯苓、僵蚕、浙贝母、地骨皮等九味药材的出粉率测定
按照处方比例称取蛤蚧9g、太子参10g、白及20g、郁金10g、鸡内金20g、茯苓12g、僵蚕9g、浙贝母15g、地骨皮12g,共117g,共取3份,粉碎,过100目筛,称定过100目筛后的药粉,按下式计算出粉率,结果见表2。

由结果可知,药材混合粉碎出粉率为96.9%。
2.处方中生地黄、枸杞子、桔梗、百部、炙黄芪、瓜蒌、石斛、陈皮、乌梅等九味药材的吸水率测定
按处方比例称取生地黄12g、枸杞子12g、桔梗10g、百部12g、炙黄芪30g、瓜蒌10g、石斛15g、陈皮10g、乌梅20g,共131g,共取3份,分别加入1050ml水,约2小时后,处方中质地比较坚硬的药材均浸泡透心,过滤,沥干水分,称定浸泡后药材的重量,按下式计算吸水率,结果见表2。

由结果可知,药材浸泡2小时即可透心,吸水率201.7%,因此,在提取时,第一次应多加2倍量水,浸泡2小时。
3.水煎提取工艺参数的确定
为了确定最佳的提取参数,拟采用正交试验对提取工艺中所涉及的条件进行优选。通常情况下,提取次数、煎煮时间、加水量对提取结果影响较大,因此,以提取次数、煎煮时间、加水量作为影响因素,进行正交试验。
水煎煮提取,对于提取次数,一般最多提取三次,因此,提取次数的水平数我们选择1、2、3次;对于煎煮时间,由于处方中大多数均为质地疏松的药材,且药材在提取前已浸泡至透心,因此我们选择最长提取时间为2.5小时,提取时间我们选取1.5小时、2.0小时、2.5小时;对于加水量,加水量过少提取不完全,过多则影响效率,因此加水量我们选取5倍、6倍、7倍。

(1)评价指标确定:药材主要有效成分为苷类和生物碱类,其都可以溶于乙醇,因此我们以醇浸出物作为提取工艺研究的一个重要指标,权重系数定为0.7;而出膏量的多少可以间接衡量未知有效成份的多少,权重系数定为0.3。所以,综合考虑,选择醇浸出物与出膏率的综合评分作为此次试验的评价指标。
(2)试验方法:按处方比例称取生地黄12g、枸杞子12g、桔梗10g、百部12g、炙黄芪30g、瓜蒌10g、石斛15g、陈皮10g、乌梅20g,共131g,共取9份,按正交表中各试验号对应的条件安排试验,收集提取液,浓缩,定容至500ml,备用。
(3)出膏率测定方法:精密量取各试验项下浓缩液20ml,分别置恒重好的干燥蒸发皿中水浴蒸干,置60℃烘箱中烘干,取出,迅速置干燥器中放冷,精密称定重量,计算出膏率。
(4)醇浸出物测定:分别取出膏率项下各干浸膏粉2~4g,精密称定,置100~250mL的锥形瓶中,精密加95%乙醇50ml,密塞,精密称定重量,静置1小时后,连接回流冷凝管,加热至沸腾,并保持微沸1小时。放冷后,取下锥形瓶,密塞,再称定重量,用95%乙醇补足减失的重量,摇匀,用干燥滤器过滤,精密量取滤液25ml,置已干燥至恒重的蒸发皿中,在水浴上蒸干后,于105℃干燥3小时,迅速置干燥器中冷却30分钟,精密称定重量。计算干浸膏粉中醇溶性浸出物的含量。
(5)试验的设计及测定结果见表5。

(6)方差分析
将上表的正交试验结果进行方差分析,结果见表6。

注:F0.05(2,2)=19.0,F0.01(2,2)=99.0。
方差分析结果表明,提取次数对醇浸出物和出膏率的影响最为显著;其次是提取时间,最后是加水量。正交试验结果表明,最佳提取工艺为A2B2C2。即:加6倍量水煎煮提取两次,每次2小时。考虑到药材的吸水率为201.7%,因此,第一次煎煮加8倍量水,第二次煎煮加6倍量水。
(7)验证试验:按处方量称取生地黄90g、枸杞子90g、桔梗75g、百部90g、炙黄芪225g、瓜蒌75g、石斛112.5g、陈皮75g、乌梅150g,共982.5g,共取3份,按照所选提取条件试验,分别浓缩定容至5000ml,量取100ml药液测定出膏率;再按照正交试验醇浸出物测定方法测定醇浸出物含量。醇浸出物测定结果以及出膏率测定结果见表7。

三次验证试验表明,检测指标接近或者高于正交表中最优值,说明优选的提取条件基本可行。
4.浓缩工艺参数的确定
由于真空浓缩具有浓缩时间短,对药液中有效成分破坏小等优点,故选择真空浓缩。取验证试验项下的滤液3000ml,均分为3份,分别于70℃、75℃、80℃真空(0.08Mpa)浓缩,观察药液浓缩过程中的状态,结果见表8。

以上结果显示,浓缩温度70~80℃,真空度在0.08MPa时对药物状态无影响,因此,浓缩温度暂定为70~80℃,真空度在0.08MPa。
七、制剂成型工艺研究
(一)干燥温度的确定
由于处方中郁金含有挥发性成分,根据《中国药典》2010年版一部附录I A项下的规定,应在60℃以下进行干燥,所以我们选择制剂在50℃进行干燥。
(二)规格的确定
该制剂处方组成为蛤蚧67.5g、太子参75g、白及150g、生地黄90g、枸杞子90g、郁金75g、桔梗75g、百部90g、鸡内金150g、炙黄芪225g、茯苓90g、瓜蒌75g、僵蚕67.5g、石斛112.5g、浙贝母112.5g、陈皮75g、乌梅150g、地骨皮90g,共1860g,制成1000g浓缩水丸。因此,暂定该制剂规格为每10丸重1.3g。
八、三批中试研究
根据工艺研究结果,进行三批中试生产,批号分别为20130201、20130202、20130203,试验数据及中间参数见下表。

九、小结
本处方为十八味中药材制成的浓缩水丸。根据药材所含成分性质,采取相应的工艺制法,在工艺中对各工艺参数进行了考察,并确定了制剂成型的干燥温度,按此条件进行了中试试验,结果三批样品均符合有关规定,说明本工艺较稳定,具大生产可行性。
