技术领域
本公开涉及手术吻合设备。更具体地,本公开涉及用于动力柔性环形吻合器的插入器械、接合器组件和保护器组件。
背景技术
环形吻合器被用以执行端端吻合术。在典型的手术操作中,环形吻合器的砧座被输送到手术部位,同时支撑在细长轴上的环形吻合器的吻合钉钉仓被插入穿过例如腹壁中的切口。或者,环形吻合器可以包括柔性轴,所述柔性轴允许通过自然身体孔口(例如,嘴或肛门)将吻合钉钉仓引入到手术部位。环形吻合器可以是手动的或动力的,并且可以被修改用于与机器人手术系统一起使用。
为了更好地便利环形吻合器的柔性轴的引入和定位,将会有益的是具有如下的环形吻合器:该环形吻合器包括用于在手术操作期间将环形吻合器的砧座组件连接到环形吻合器的细长主体的有效机构。同样会有益的是具有如下的引入器(introducer):该引入器例如在将环形吻合器引入到患者中的期间使对患者和/或对环形吻合器的损害最小化。进一步会有益的是具有如下的环形吻合器:该环形吻合增进手术操作的可视化。同样会有益的是具有如下的环形吻合器:该环形吻合器带有能够容纳引导线缆和/或允许将生理盐水喷射到吻合术部位以例如测试密封并使操纵简易的细长主体。
发明内容
提供了用于将手柄组件连接于装载单元的接合器组件。所述接合器组件包括:壳体;细长主体,其从所述壳体延伸;以及套管针组件,其被支撑在所述细长主体内并且包括套管针构件。所述套管针构件从所述细长主体延伸,其中,所述套管针构件带有磁力。
在实施例中,所述接合器组件包括砧座组件。所述砧座组件可包括中心杆,所述中心杆带有磁力从而以所述中心杆被磁性地吸引到所述套管针构件的这种方式来与所述套管针构件互补。所述砧座组件可包括可移除顶端。所述可移除顶端可以带有磁力从而以所述可移除顶端被磁性地吸引到所述套管针构件的这种方式来与所述套管针构件互补。所述套管针组件可包括电磁体,所述电磁体围绕所述套管针构件被接收以用于选择性地使所述套管针构件带有磁力。所述接合器组件可进一步包括光源。

继续参考图1,根据本公开的实施例的接合器组件总体上被示出为接合器组件100。接合器组件100将仅描述到完全公开本公开的方案所必需的程度。对于示例性接合器组件的详细说明,请参考2015年10月9日提交的、序列号为62/239,301的共有的美国临时专利申请和2015年11月5日提交的、序列号为62/251,300的共有的美国临时专利申请,每个申请的内容通过引用整体上并入本文。
接合器组件100可释放地连接到动力手柄组件10。接合器组件100包括可操作地连接到动力手柄组件10的壳体102和从壳体102延伸的细长主体104。装载单元110可以与接合器组件100形成一体,或者可以可释放地联接到接合器组件100以允许接合器组件100的再使用。细长主体104是柔性的,以便于将装载单元110插入体内。
套管针构件106从细长主体104的远侧端延伸,用于可释放地接合砧座组件,即砧座组件120(图2)。如图所示,套管针构件106的近侧部106a包括第一磁极,即南“S”,并且套管针构件106的远侧部106b包括第二磁极,即北“N”。正如通过下面的描述将变得显而易见的,可以交换套管针构件106的近侧部106a和远侧部106b的极性。

简而言之,砧座组件120包括中心杆组件122和固定到中心杆组件122的砧座头组件124。砧座组件120的头组件124可以刚性地固定到中心杆组件122。或者,头组件124可以枢转地固定到中心杆组件122,以便于将砧座组件120插入穿过患者的管腔。砧座组件120的中心杆组件122包括中心杆126。在一个实施例中,中心杆126带有磁力。如图2中所示,中心杆126包括具有第一磁极(即,南“S”)的近侧部126a,以及具有第二磁极(即,北“N”)的远侧部126b。
如上所述,接合器组件100(图1)的套管针构件106(图1)以与砧座组件120的中心杆126类似的方式带有磁力。以这种方式,砧座组件120的中心杆126的近侧部126a被吸引到套管针构件106的远侧部106b。
在手术吻合操作期间,砧座组件120可以以穿过口部的方式或以任何其它方式被引入手术部位。在将待吻合的组织的第一部分(未示出)固定到砧座组件120之后,并且在将待吻合的组织的第二部分(未示出)围绕接合器组件100的装载单元110固定之后,使接合器组件100朝向砧座组件120移动。砧座组件120的中心杆126的近侧部126a与套管针构件106的远侧部106b之间的磁吸引力有助于使砧座组件120的中心杆126与接合器组件100的套管针构件106对准。在砧座组件120连接到接合器组件100期间当砧座组件120对于临床医生而言不可见时,该特征是特别有利的。

现在参照图5和图6,用于便于带磁力的砧座组件(即,砧座组件120)在患者体内的定位的器械总体上被示出为插入器械300。插入器械300包括手柄组件302和从手柄组件302延伸的细长主体304。插入器械300还包括电磁组件340和灯组件350。
电磁组件340类似于接合器组件200的电磁组件240,并且包括缠绕铁磁材料(即,杆构件348)的线圈342、连接到线圈342的电源344、以及用于激活电磁组件340的激活开关346。
灯组件350包括至少一个光源和控制开关,所述至少一个光源例如是安装在细长主体304的远侧端上的环形阵列的发光二极管352,所述控制开关用于激活光源352并用于控制光源352的强度。光源352可以由电磁组件340的电源344供电。或者,光源352可以由布置在手柄组件302内的独立的电源来供电。
在砧座组件120的定位期间,插入器械300的电磁组件340的激活产生磁场,所述磁场吸引砧座组件120的带磁力的中心杆126(图2)以便于砧座组件120在体腔内并穿过组织“T”定位。当砧座组件120穿过组织“T”定位时,灯组件350的光源352的激活便于观察砧座组件120。可以设想,光源352可以被放置在组织“T”后面以照亮组织“T”。
现在参照图7,总体上被示出为保护组件400的组件,其用于保护接合器组件100的细长主体104和固定到细长主体104的装载单元110并且用于在将接合器组件100引入到患者体内期间使对患者的组织的损伤最小化。保护组件400包括套筒构件402和可操作地连接到套筒构件402的注入口404。注入口404可以包括路厄连接器(luer connector)或其他合适的连接。保护组件400进一步包括注射器406或其它注入流体的源,即空气罐、风箱泵(bellow pump),其被构造为用于与注入口404可操作地连接。
另外参照图8,保护组件400的套筒构件402包括细长柔性主体410,细长柔性主体410构造成围绕接合器组件100(图7)的细长主体104(图7)和固定到接合器组件的细长主体102的装载单元110(图7)被接收。套筒构件402包括开口近侧端402a和开口远侧端402b,并且限定了与注入口404流体连通的围绕开口远侧端402b延伸的可膨胀环形腔或环形圈403。如下面将进一步详细描述的,可膨胀环形圈403被构造成邻近细长主体104的远侧端定位,用于在接合器组件100引入到患者内期间保护接合器组件100免于损坏,并且用于保护患者的组织。可膨胀环形圈403通过沿套筒构件402的细长柔性主体410的长度延伸的一个或多个充入通道405连接到注入口404。
套筒构件402的开口近侧端402a包括手柄构件或拉回手柄408,用于便于围绕接合器组件100的细长主体104接收套筒构件402,并且用于便于从细长主体104周围移除套筒构件402。套筒构件402可以包括沿着细长柔性主体410的全部或一部分长度延伸的齿孔(perforation)或撕裂线412。或者,套筒构件402可以由板材形成,其具有用于维持管结构的钩环式紧固件(例如,)或拉链锁紧连接件。
在手术操作期间,并且在将接合器组件100的细长主体104引入患者内之前,保护组件400的套筒构件402围绕细长主体104被接收。保护组件400的套筒构件402可以在套筒构件402处于膨胀构形(图9A)时,或在套筒构件402处于瘪缩构形(图9B)时围绕细长主体102被接收。套筒构件402围绕细长主体402收回,直到可膨胀环形腔403邻近细长主体104的远侧端布置。如果套筒构件402的可膨胀环形腔403尚未膨胀,则注射器406(图7)可用以使可膨胀环形腔403膨胀。
在将接合器组件100的细长主体104引入患者体内期间,保护组件400的套筒构件402保护细长主体104的远侧端免受损坏,同时还保护接合器组件100所引入穿过的组织。一旦接合器组件100已经被定位在患者体内,则可以使保护组件400的套筒构件402的可膨胀环形腔403瘪缩,以便于将套筒构件402从接合器组件100的细长主体104移除。如上所指出的,套筒构件402可以包括齿孔或撕裂线412,以便于将套筒构件402从细长主体104周围移除。一旦将保护组件400的套筒构件402从接合器组件100的细长主体104周围移除,则接合器组件100可以以传统的方式来使用。
现在参照图10至图12,用于保护接合器组件100的细长主体104和固定到细长主体104的装载单元110的组件的另一实施例总体上被示出为保护组件500,且保护组件500用于在将接合器组件100引入患者体内期间使对患者组织的损伤最小化。保护组件500包括套筒构件502和固定到套筒构件502的远侧端的帽构件504。套筒构件502构造成围绕接合器组件100的细长主体104被接收。帽构件504被构造成围绕固定至接合器组件100的细长主体104的远侧端的装载单元110被可释放地接收。
保护组件500的套筒构件502基本上类似于上述保护组件400的套筒构件402,并且包括细长柔性主体506。如图所示,帽构件504包括大致锥形的形状,并且可以是透明或半透明的。帽构件504由柔性塑料或其它合适的材料形成,并被分成多个叶片件或部分504a。例如,并且如图所示,帽构件504包括两个叶片件(图11A)、三个叶片件(图11B)、四个叶片件(图11C)或五个叶片件(图11D)。叶片件504a可以由脆性连接件504b(例如弱化的塑料桥形件)连接,脆性连接件504b构造成在套筒构件502相对于接合器组件100的细长主体104收回期间破裂。叶片件504a的数量越大,则当保护组件500从接合器组件100的细长主体104周围移除时,帽构件504将在径向上占据越少的空间。
套筒构件502的细长柔性主体506可由比帽构件504更柔性的材料构成。可以通过增加叶片件504a的壁厚度或通过使用较硬的材料构造帽构件504来使帽构件504更具刚性。
如图12中所示,保护组件500从接合器组件100的细长主体104周围的移除需要如由箭头“A”所标示的将手柄构件508相对于细长主体104向近侧和径向向外拉动,以引起套筒构件502沿着撕裂线512撕裂并且引起帽构件504的叶片件504a的分离。随着套筒构件502沿着撕裂线512分离并且套筒构件502相对于细长主体104向近侧收回,帽构件504的叶片件504a分离以允许装载单元110穿过其中。一旦从接合器组件100的细长主体104周围完全移除,则临床医生可以处理保护组件500,并且接合器组件100可以以传统的方式来使用。
转向图13至图15,用于保护接合器组件100的细长主体104和附接的装载单元110的组件的另一个实施例总体上被示出为保护组件600。保护组件600包括套筒构件602和固定到套筒构件602的远侧端的帽构件604。保护组件600进一步包括支撑在帽构件604内的相机组件606。相机组件606可以无线地或通过沿着套筒构件602的长度延伸的导线“W”(图15)与监控单元(未示出)连接。
帽构件604包括多个叶片件604a。相机组件606被支撑在叶片件604a中的一个的远侧端上。相机组件606可以是可移除的以允许再使用。当接合器组件100的细长主体104被引入患者体内时,相机组件606允许观察。相机组件606可以包括电荷耦合器件(CCD)相机或其他合适的相机。
现在参照图16和图17,用于保护接合器组件100的细长主体104和附接到细长主体104的装载单元110的组件被总体上示出为保护组件700。保护组件700基本上类似于上述的保护组件400、500、600。保护组件700包括:套筒构件702、与套筒构件702的远侧端一体地形成或固定至该远侧端的帽构件704、以及支撑在套筒构件702上并穿过套筒构件702延伸的转向组件706。
转向组件706包括转向环708以及从转向环708延伸并穿过套筒构件702延伸的多根转向线缆710。转向线缆710各自穿过套筒构件702的管腔705被接收。如图所示,保护组件700包括四(4)根转向线缆710。然而,可以设想,保护组件700可以具有多于或少于四(4)根转向线缆710。
在手术操作期间,在保护组件700围绕接合器组件100的细长主体104被接收之后,转向环708可以用以在患者内引导接合器组件100的细长主体104。更具体地,保护组件700的转向环708如由图16中的箭头“B”所标示地围绕接合器组件100的纵向轴线旋转,引起保护组件700的套筒构件702的相应移动,其移动接合器组件100的细长主体104。当接合器组件100被引入到患者体内时,转向环708相对于接合器组件100的旋转便于接合器组件100继续移动到期望的位置。
现在参照图18,用于保护固定到接合器组件100的细长主体104的远侧端的装载单元110的组件总体上示出为保护组件800。保护组件800包括可膨胀构件802和注入口804,所述可膨胀构件802构造成用于可操作地接合装载单元110,所述注入口804用于实现可膨胀构件802的注入。注入口804可以包括路厄连接器或其他合适的连接装置,并且通过充入管804a与可膨胀构件802流体连通。保护组件800可以进一步包括注射器、罐、风箱或用于使可膨胀构件802膨胀的其它合适的装置。
保护组件的可膨胀构件802具有第一大致球形部分802a和从第一大致球形部分802a延伸的第二大致球形部分802b。当可膨胀构件802处于膨胀状态时,第一大致球形部分802a在尺寸或直径上大于第二大致球形部分802b。如图18所示,当可膨胀构件802处于膨胀状态时,第二大致球形部分802b被牢固地接收在装载单元110的圆筒形腔111内,该装载单元110固定到接合器组件100的细长主体104的远侧端。当可膨胀构件802的第二大致球形部分802b被接收在装载单元110的圆筒形腔111内时,第一大致球形部分802a覆盖装载单元110的远侧端。以这种方式,随着接合器组件100和附接的保护组件800被引入患者体内,可膨胀构件802防止装载单元110的远侧端与组织接触。
可以设想,可膨胀构件802的第一大致球形部分802a可以被构造成与装载单元110的轮廓相匹配。以这种方式,可膨胀构件802与装载单元110更紧密地对准。
现在参照图19至图21,根据本公开的装载单元的实施例总体上被示出为装载单元910。装载单元910被构造为用于可操作地连接到接合器组件100(图1)的细长主体104(图1)的远侧端。简而言之,装载单元910包括外壳912和安装在外壳912的远侧端上的钉仓组件914。外壳912限定了延伸穿过其中的管腔915,用于提供通过钉仓组件914的路径。管腔915可以用以例如提供灌注的流体“I”(图21)到正被吻合的组织“T”。此外,管腔915可以用于在吻合操作期间接收例如导丝、观测设备“S”(图20)或其它合适的器械。连接器套筒916可以接收在管腔915的近侧端上,以便于将流体和/或器械接收到管腔中并穿过管腔。
可以想到,本公开的实施例可以被修改以用于各种机电手术器械和/或电手术器械。进一步可以想到,这些器械可以例如被构造为能够被可拆除地联接并且能够由机器人手术系统控制。
现在参照图22,示例性机器人手术系统总体上被示出为机器人手术系统1,并且可以包括:多个手术机器人手臂,例如手术机器人手臂2、3,每个手术机器人手臂具有器械驱动单元(例如,器械驱动单元11、12)和可移除地附接到该器械驱动单元的末端执行器(例如,手术吻合器21、22);控制装置,例如控制装置5;以及与控制装置5耦合的操作控制台(例如,操作控制台4)。如图所示,机器人手术系统1构造成用于对躺在手术台“ST”上待借助手术吻合器21、22以微创方式进行治疗的患者“P”使用。

Claims (20)
1.一种用于将手柄组件连接于装载单元的接合器组件,所述接合器组件包括:
壳体;
细长主体,其从所述壳体延伸;以及
套管针组件,其被支撑在所述细长主体内并且包括套管针构件,其中所述套管针构件从所述细长主体延伸,其中所述套管针构件带有磁力。
2.根据权利要求1所述的接合器组件,进一步包括砧座组件,其中所述砧座组件包括中心杆,所述中心杆带有磁力从而以所述中心杆被磁性地吸引到所述套管针构件的这种方式来与所述套管针构件互补。
3.根据权利要求1所述的接合器组件,进一步包括砧座组件,其中所述砧座组件包括可移除顶端,所述可移除顶端带有磁力从而以所述可移除顶端被磁性地吸引到所述套管针构件的这种方式来与所述套管针构件互补。
4.根据权利要求1所述的接合器组件,其中,所述套管针组件包括电磁体,所述电磁体围绕所述套管针构件被接收以用于选择性地使所述套管针构件带有磁力。
5.根据权利要求1所述的接合器组件,进一步包括光源。
6.一种插入器械,包括:
手柄组件;
细长主体,其从所述手柄组件延伸;
电磁体,其包括环绕铁磁体构件被接收的线圈,所述电磁体被布置在所述细长主体的远侧端上;以及
光源,其布置在所述细长主体的远侧端上。
7.根据权利要求6所述的插入器械,进一步包括用于选择性地激活所述电磁体的第一开关组件。
8.根据权利要求7所述的插入器械,进一步包括用于选择性地激活所述光源的第二开关组件。
9.一种用于在将手术吻合器引入患者内期间保护所述手术吻合器的功能端的组件,所述组件包括:
套筒,其具有近侧端和远侧端并且能够围绕细长主体被接收,所述套筒限定位于所述远侧端上的可膨胀腔,所述可膨胀腔构造成邻近所述细长主体的远侧端布置;以及
注入口,其与所述可膨胀腔流体连通以用于使所述可膨胀腔选择性地膨胀。
10.根据权利要求9所述的组件,其中,所述套筒进一步限定沿着所述套筒的长度延伸的弱化部。
11.根据权利要求9所述的组件,其中,所述套筒包括位于所述近侧端上的手柄部。
12.根据权利要求9所述的组件,进一步包括注入流体源。
13.根据权利要求12所述的组件,其中,所述注入流体源是注射器。
14.一种用于保护手术吻合器的功能端的组件,所述组件包括:
套筒,其具有近侧端和远侧端;以及
帽构件,其固定到所述套筒的所述远侧端,其中所述帽构件包括通过脆性连接件彼此连接的多个叶片件。
15.根据权利要求14所述的组件,其中,所述脆性连接件是弱化的桥形件。
16.根据权利要求14所述的组件,其中,所述套筒包括位于所述近侧端上的手柄部。
17.根据权利要求14所述的组件,其中,所述套筒进一步限定沿着所述套筒的长度延伸的弱化部。
18.根据权利要求14所述的组件,进一步包括布置在所述帽构件的远侧端上的相机。
19.根据权利要求14所述的组件,进一步包括转向组件,所述转向组件具有转向环和固定至所述转向环的多根转向线缆。
20.根据权利要求19所述的组件,其中,所述多根转向线缆沿着所述套筒的长度延伸。
