Abstract
本发明公开了预防并治疗慢性腹泻的药物胶囊,由主药组分和胶囊辅料制成,其特征在于:主药组分由白术、苍术、干姜、茯苓、炙甘草、防风、陈皮、白芍、肉蔻、吴萸、补骨脂、白芷、生苡仁、苏梗、藿香、芡实米、炒山药、五味子的提取物组成。本发明药物胶囊能显著降低慢性腹泻的发生及进展,具有改善肠道内环境,促进肠粘膜修复再生,抗御有害微生物的侵袭及肠道对水份的吸收和改善消化系统的功能,健脾益肾,扶正固本,标本兼治的作用。

技术领域
本发明属于药物制剂领域,特别涉及预防并治疗慢性腹泻的药物胶囊。
背景技术
慢性腹泻为常见病、多发病,中医称之为泄泻,病程长而缠绵难愈,以大便次数增多,大便稀溏或如水样,或有粘液,肠鸣腹痛,但无里急后重为特征。有的病程多年,反复发作,久治不愈之腹泻为顽固性腹泻。慢性腹泻是一种常见病症,包括慢性结肠炎、慢性痢疾、胃肠功能紊乱、肠易激综合症及饮食生冷、荤腥油腻、感受风寒、过敏、精神紧张等引起肠胃疾病。常见症状有腹泻、大便急迫、腹泻稀便及衡水样便,或夹粘胨,日行数次或十数次,病程达3个月以上。
虽然也有一些治疗慢性腹泻之中成药及西药,但由于效果不明显,或只有近期效果而无远期及根本疗效,长期服用增加肝肾负担,有毒副作用,不宜长期服用。
中医认为,“泄泻之本,无不由于脾胃。”病多因感受外邪,如湿热、暑湿、寒湿之邪;情志所伤,忧思郁怒导致肝失疏泄,横逆犯脾而成泄泻;饮食不节,过食肥甘厚味,或进食不洁腐败之物。 《内经》中关于本证有“濡泄”、“洞泄”、“飧泄”、“注泄”、“溏泄”、“鹜泄”等记载,并对其病因病机等有较全面论述。
《医宗必读》为本病治疗设有九法,全面系统地论述了泄泻的治法,是泄泻治疗学上的里程碑。辨证病因:
(1)感受外邪:外感寒湿暑热之邪均可引起泄泻,其中以湿邪最为多见。有“无湿不成泻”之说。
(2)饮食所伤:饮食不节或过食肥甘,致使宿食内停,滞碍肠胃,影响脾胃之运化;或多食生冷,误食不洁之物,则损伤脾胃,致使水谷精微不能输布,水湿内停,而发生泄泻。
(3)情志失调:素体脾胃虚弱,复因情志影响,忧思恼怒则伤脾,致使脾胃气机失调;恼怒伤肝,肝气郁结,横逆犯脾,脾伤则运化失常而成泄泻。
(4)脾胃虚弱:脾阳不振则运化功能减弱,不能腐熟水谷运化精微,以至水谷停滞,并入大肠而泄泻。
(5)肾阳虚衰:泄泻日久不愈,损伤肾阳,肾阳受损又可影响脾阳之不足,致成脾肾阳虚,则泄泻缠绵不止。
治泻九法:淡渗、升提、清凉、疏利、甘缓、酸收、燥脾、温肾、固涩。
本发明基于以上辨证认知,兼顾各种类型,搭配组方,经过临床验证,对各种类型的腹痛、腹泻均有显著的治疗效果,对于慢性腹痛、腹泻效果尤为显著。
发明内容
本发明的目的在于提供一种慢性腹泻的药物胶囊。该药物胶囊能显著降低慢性腹泻的发生及进展,具有改善肠道内环境,促进肠粘膜修复再生,抗御有害微生物的侵袭及肠道对水份的吸收和改善消化系统的功能,健脾益肾,扶正固本,标本兼治的作用。
本发明采用的技术方案如下。
预防并治疗慢性腹泻的药物胶囊,由主药组分和胶囊辅料制成,其特征在于:主药组分由白术、苍术、干姜、茯苓、炙甘草、防风、陈皮、白芍、肉蔻、吴萸、补骨脂、白芷、生苡仁、苏梗、藿香、芡实米、炒山药、五味子的提取物组成;
以每一千粒胶囊计,该药物胶囊各主药组分的用量分别为:白术60-80g、苍术60-80g、干姜60-80g、茯苓60-90g、炙甘草30-40g、防风60-80g、陈皮60-70g、白芍50-70g、肉蔻50-70g、吴萸30-40g、补骨脂60-75g、白芷50-70g、生苡仁180-240g、苏梗50-80g、藿香50-80g、芡实米60-100g、炒山药60-80g、五味子60-80g。
主药组分是采用以下方法制备:
步骤1:将白术、苍术、生苡仁、炒山药、芡实米研成细粉,过筛,备用;
步骤2:取干姜、茯苓、炙甘草、防风、陈皮、白芍、肉蔻、吴萸、补骨脂、白芷、苏梗、藿香、五味子,加4-6倍量水浸泡1-1.5小时,第一煎大火烧沸后改小火煎1-1.5小时,滗出药汁备用;第二煎加1.5-2.5倍量水煎煮0.5-1小时,滗出药液与第一煎汁合并、过滤制得滤液;
步骤3:将步骤1制得的粉末与步骤2制得的滤液混合,熬成糊状物;
步骤4:将步骤3制得的糊状物低温烘干,粉碎,制得粉末。
胶囊辅料包括粘合剂、填充剂、崩解剂、吸湿剂、润滑剂中的一种或数种; 所述填充剂为红薯淀粉。
主药组分制备完成后加入胶囊辅料混合搅拌,干燥,装入胶囊,即制得药物胶囊。
进一步,粘合剂为95%乙醇、聚乙烯吡咯烷酮、聚乙烯吡咯烷酮/乙酸乙烯酯共聚物中的任意一种。
进一步,填充剂为蔗糖、磷酸二氢钙、淀粉、海藻多糖、壳寡糖中的一种或几种。
进一步,崩解剂为羧甲基淀粉钠、低取代羟丙基纤维素、交联聚乙烯吡咯烷酮中的任意一种。
进一步,吸湿剂采用微粉硅胶。
进一步,所述润滑剂为滑石粉、硬脂酸富马酸钠、硬脂酸锌、硬脂酸镁、二氧化硅、聚乙二醇6000中的一种或几种。
进一步,所述低温烘干的温度控制在50-55℃。
本发明所述原料药的来源如下。
苍术:为菊科植物茅苍术的干燥根茎。春、秋二季采挖,除去泥沙,晒干,摘去须根。用于湿阻中焦,脘腹胀满,泄泻,水肿,脚气痿蹙,风湿痹痛,风寒感冒,夜盲,眼目昏涩。
白术:别名桴蓟,于术,冬白术,淅术,杨桴,吴术,片术、苍术等,属于菊科、苍术属多年生草本植物。喜凉爽气候,以根茎入药,具有多项药用功能。主要分布于四川、云南、贵州等山区湿地。白术具有健脾益气,燥湿利水,止汗,安胎的功效,用于脾虚食少,腹胀泄泻,痰饮眩悸,水肿,自汗,胎动不安。《医学启源》记载:“除湿益燥,和中益气,温中,去脾胃中湿,除胃热,强脾胃,进饮食,安胎。”
干姜:为姜科植物姜的干燥根茎。冬季采挖,除去须根和泥沙,晒干或低温干燥。趁鲜切片晒干或低温干燥者称为“干姜片”。
茯苓:茯苓,中药名。为多孔菌科真菌茯苓的干燥菌核。多于7~9月采挖,挖出后除去泥沙,堆置“发汗”后,摊开晾至表面干燥,再“发汗”,反复数次至现皱纹、内部水分大部散失后,阴干,称为“茯苓个”;或将鲜茯苓按不同部位切制,阴干,分别称为“茯苓块”和“茯苓片”。
炙甘草:系豆科植物,炙甘草是用蜜烘制的甘草,俗称炮制,就是将蜂蜜置锅中炼成中蜜,改用文火加生甘草片拌炒均匀,3~5 min出锅,置烤房或烘箱60-C烘至不粘手时取出,放凉即可。炙甘草为类圆形或椭圆形切片,表面红棕色或灰棕色,微有光泽,切面黄色至深黄色,形成层环明显,射线放射状。炙甘草汤具有益气滋阴,通阳复脉治疗功效。
防风:别名铜芸、回云、回草、百枝、百种。是一种药草的名字,多年生草本植物,其喜凉爽气候,耐寒,耐干旱,主产于河北、黑龙江、四川、内蒙古等地。防风的根可生用。味辛、甘,性微温。有祛风解表,胜湿止痛,止痉的功效。
陈皮:为芸香科植物橘及其栽培变种的干燥成熟果皮。药材分为“陈皮”和“广陈皮”。采摘成熟果实,剥取果皮,晒干或低温干燥。
白芍:也称白花芍药,是芍药科芍药属植物。在中国已有悠久的栽培历史,驰名中外,其称白花芍药。其根可入药。多年生草本或亚灌木,生于山坡、山谷的灌木丛或草丛中。我国大部分地区均有栽植。平肝止痛,养血调经,敛阴止汗。用于头痛眩晕,胁痛,腹痛,四肢挛痛,血虚萎黄,月经不调,自汗,盗汗。
肉蔻:常绿乔木。叶互生,革质,长椭圆形,先端锐尖,全缘,叶面暗绿色。总状花序,腋生,花单性,异株,花冠黄白色。果实梨形,淡黄色或橙黄色,成熟时纵裂成两瓣,露出绯红色的假种皮,称为“豆蔻瓣”(Mace),内含种子1枚,称“肉豆蔻”。种皮红褐色,木质坚硬。肉豆蔻所含挥发油,少量能促进胃液的分泌及胃 肠蠕动,而有开胃和促进食欲,消胀止痛的功效;但大量服用则有抑制作用,且有较显著的麻醉作用;挥发油中的萜类成分对细菌和霉菌均有抑制作用。肉豆蔻醚对正常人有致幻、抗炎作用;肉豆蔻及肉豆蔻醚能增强色胺的作用,体内外试验均对单胺氧化酶有中度的抑制作用。肉豆蔻对MCA和DMBA诱发的小鼠子宫癌及皮肤乳头状瘤有抑制作用。
吴萸:常绿灌木或小乔木,高3-10m。树皮青灰褐色,幼枝紫褐色,有细小圆形的皮孔;幼技、叶轴及花轴均被锈色绒毛。奇数羽状复叶对生,连叶柄长20-40cm;叶柄长4-8cm,小叶柄长2-5mm;小叶5-9,椭圆形至卵形,长5.5-15cm,宽3-7cm,先端骤狭成短尖,基部楔形至广楔形或圆形,全缘或有不明显的钝锯齿,侧脉不明显,两面均被淡黄褐色长柔毛,脉上尤多,有明显的油点,厚纸质或纸质。雌雄异株,聚伞圆锥花序,顶生;花轴粗壮,密被黄褐色长柔毛,花轴基部有小叶片状的狭小对生苞片2枚;萼片5,广卵形,长约1-2mm,被短柔毛;花瓣5,白色,长圆形,长4-6mm;雄花具5雄蕊,插生在极小的花盘上,花药基着,椭圆形,花丝粗短,被毛,退化子房先端4-5裂;雌花的花瓣较雄花瓣大,退化雄蕊鳞片状,子房上位,长圆形,心皮5,花后增宽成扁圆形,有粗大的腺点,花柱粗短,柱头先端4-5钱裂。果实扁球形,成熟时裂开成5个果瓣,呈蓇葖果状,紫红色,表面有粗大油腺点,每分果有种子1个,黑色,有光泽。花期6-8月,果期9-10月。《本草经集注》:蓼实为之使。恶丹参、消石、白垩。畏紫石英。配黄连:清肝和胃;配党参:温中补气;配五味子:温敛固涩;配当归:温经治血,调经止痛;配木瓜:和胃化湿,温中止痛。
补骨脂:又名:破故纸,婆固脂,胡韭子。属蔷薇目,豆科一年生直立草本,高60-150cm。叶为单叶,长7-8mm;叶柄长2-4.5cm,叶宽卵形,花序腋生,有花10-30朵,花冠黄色或蓝色,花瓣明显具瓣柄,旗瓣倒卵形,荚果卵形,花、果期7-10月。
白芷:多年生高大草本,高1-2.5米,根圆柱形,茎基部径2-5厘米,基生叶一回羽状分裂,复伞形花序顶生或侧生,果实长圆形至卵圆形。
以根入药,有祛病除湿、排脓生肌、活血止痛等功能。主治风寒感冒、头痛、鼻炎、牙痛。赤白带下、痛疖肿毒等症,亦可作香料。北方的一些省区有栽培,多自产自销,少数调省外。一般生于林下、林缘、溪旁、灌丛和山谷草地。白芷煎剂对大肠杆菌、痢疾杆菌、变形杆菌、伤寒杆菌、副伤寒杆菌、绿脓杆菌、霍乱弧菌、人型结核杆菌等均有抑制作用。本品所含的氧前胡素体外试验对11种菌株有抗菌作用;欧前胡素亦有抗菌作用花椒毒素;对人型结核杆菌H37RV的米IC为100米cg/米l。水浸剂对奥杜盎氏小芽胞癣菌等致病真菌也有一定抑制作用。上行头目,下抵肠胃,中达肢体,遍通肌肤以至毛窍,而利泄邪气。如头风头痛,目眩目昏;如四肢麻痛,脚弱痿痹;如疮溃糜烂,排脓长肉;如两目作障,痛痒赤涩;如女人血闭,阴肿漏带;如小儿痘疮,行浆作痒,白芷皆能治之。第性味辛散,如头痛、麻痹、眼目、漏带、痈疡诸症,不因于风湿寒邪,而因于阴虚气弱及阴虚火炽者,俱禁用之。
生苡仁:含有丰富的水溶性纤维,可以由吸附胆盐(负责消化脂肪),使肠道对脂肪的吸收率变差,进而降低血脂肪、降血糖。可以促进体内血液和水分的新陈代谢,有利尿、消水肿等作用,并可帮助排便,所以可以帮助减轻体重。苡仁内含的薏苡脂有阻止癌细胞生长及伤害作用。
苏梗:是唇形科植物紫苏的干燥茎。含挥发油,其中主要为紫苏醛、左旋柠檬稀及少量a-蒎烯等。苏叶煎剂有缓和的解热作用;有促进消化液分泌,增进胃肠蠕动的作用;能减少支气管分泌,缓解支气管痉挛。本品水煎剂对大肠杆菌、痢疾杆菌、葡萄球菌均有抑制作用。
藿香:藿香有杀菌功能,口含一叶可除口臭,预防传染病,并能用作防腐剂。
芡实米:别名鸡头米、鸡头莲、刺莲等。是我国较常用中药材。应用历史悠久,在国内外久负盛名,本品为睡莲科植物芡的干燥成熟种仁。主要分布于黑龙江、吉林、辽宁、河北、河南、山东、江苏等地。芡实可以加强小肠吸收功能,提高尿木糖排泄率,增加血清胡萝卜素浓度;实验证明,血清胡萝卜素水平的提高,可使肺癌、胃癌的发病机率下降,大大减少癌症发生的机会。
炒山药:有健脾、除湿、补气、益肺、固肾、益精的功效,山药含有可溶性纤维,能推迟胃内食物的排空,控制饭后血糖升高。还能助消化、降血糖。用于糖尿病脾虚泄泻,小便频数。
五味子:为木兰科植物五味子S或华中五味子的干燥成熟果实。前者习称“北五味子”,后者习称“南五味子”。秋季果实成熟时采摘,晒干或蒸后晒干,除去果梗及杂质。唐等《新修本草》载“五味皮肉甘酸,核中辛苦,都有咸味”,故有五味子之名。五味子分为南、北二种。古医书称它荎蕏、玄及、会及,最早列于神农本草经上品中药,能滋补强壮之力,药用价值极高,有强身健体之效,与琼珍灵芝合用治疗失眠。五味子醇提取物能降低由四氯化碳、硫代乙醇胺等引起的实验动物谷丙转氨酶升高.γ-五味子素(五味子乙素)具抗肝损伤作用。五味子素有广泛的中枢抑制作用,并且有安定作用的特点。五味子醇提取物5-10mg/kg灌胃可减少小鼠自发活动,协同戊巴比妥钠对小鼠的睡眠作用,对抗苯丙胺引起的小鼠兴奋,对抗咖啡因、烟碱引起的小鼠惊厥。五味子素(五味子醇甲)10-100mg/kg腹腔注射,随剂量的增加而减少小鼠自发活动,10mg/kg腹腔注射对抗咖啡因、苯丙胺引起的小鼠兴奋;60-120mg/kg腹腔注射明显延长小鼠巴比妥钠及戊巴比妥钠引起的小鼠睡眠时间;对抗电休克、戊四唑等引起的惊厥。五味子素有广泛的中枢抑制作用,并且有安定作用的特点。五味子有强心作用,其水浸液及稀醇浸液,可加强心肌收缩力,增加血管张力。煎剂离体实验对大鼠心肌细胞膜ATP酶活性有抑制作用;并对麻醉犬有降压作用。能增强机体对非特异性刺激的防御能力。
与现有技术相比,本发明的有益效果如下。
1、本发明的中药采用提取物,服用剂量小与现有技术相比,在患者长期服药过程中,不良反应少,同时,剂型为胶囊剂,中药成分提取效果好,明显提高了患者服药的耐受性。
2、本药品针对顽固性腹泻(又称为慢性腹泻)的脾肾阳虚、肝气乘脾,寒湿内阻、下注大肠的病机而设。肝肾阳虚,寒从中生,脾运失键则用苍术、白术、干姜、补骨脂、肉豆蔻、吴茱萸,温阳散寒,补火暖脾;湿阻气机,中焦不运,脘腹胀满,则用苏梗、藿香、白芷、生苡仁、茯苓、苍术、白术、陈皮,燥湿健脾,理气除满;脾胃气虚,肝木横侮,肠鸣腹痛,则用白芍、吴茱萸、防风、炙甘草,疏肝理气,缓急止痛;五味子、芡实米、炒山药,则补脾涩肠。炙甘草和诸药性,使君臣佐使诸药配伍,互为辅助,各味药协同起效,强化脾肾肝功能,平衡体内气血阴阳,保障气体正常升降疏通运行,使正气存内,邪有去路,起到健脾和胃,止泻固肠的作用,而且简便易行,费用低,无毒副作用、不良反应小。
3、本发明为中药制剂,不但抗湿性强,而且颗粒流动性好,易于填充,装量差异准确,崩解快,溶出度好,生物利用度高。与各类治疗慢性腹泻的药物相比,其特征是见效快、无毒副作用,且能调节人体神经系统功能,调整机体失调,增加机体免疫力,对阳虚体质及气血两虚患者均有显著疗效,副作用小。
4.本药剂从慢性腹泻的病机出发,调节肝脾肾三脏的虚实寒热错杂,综合痛泻药方、肾着汤、四神丸多方化裁加减,中药制剂具有见效快、标本兼治、兼顾调理、不易复发的效果,尤其对于慢性腹痛、腹泻、消化不良的治疗,有很显著的调理治疗作用,治愈后不复发,患者的消化功能变强,可解决患者病程长且缠绵难愈,大便次数多且大便稀溏或如水样的问题。

试验例。本发明中药对慢性腹泻的防治效果,临床选择172例作为观察对象,并随机分为治疗组136例。治疗组腹痛46例,发烧21例,呕吐14例,稀水样便49例,粘液便56例,均是采用其它中西药无明显疗效者。服用本发明所述的治疗腹泻的胶囊,每日3次,每次6粒,30天为1疗程,一般1-3个疗程,必要时加服1-2个疗程。
服用方法:对观察对象口服实施例的口服液,每日三次,一次一剂 200ml,3天为一疗程,观察期6天,共两个疗程。
根据《中医病证诊断疗效标准》疗效评定标准如下:
1、治愈:大便正常,腹痛肠鸣和发热等其他症状消失,临床检验正常。
2、好转:大便次数明显减少,腹痛肠鸣和发热等其他症状改善。
3、未愈:症状未见改善。
治疗效果:治愈的有 95例,好转的有36例,未愈的5例,总有效率96.3%,并且在观察期间未发现有明显的毒副作用。
以上所述仅为本发明的优选实施例而已,并不用于限制本发明,对于本领域的技术人员来说,本发明可以有各种更改和变化。凡在本发明的精神和原则之内,所作的任何修改、等同替换、改进等,均应包含在本发明的保护范围之内。
Claims (7)
1.预防并治疗慢性腹泻的药物胶囊,由主药组分和胶囊辅料制成,其特征在于:主药组分由白术、苍术、干姜、茯苓、炙甘草、防风、陈皮、白芍、肉蔻、吴萸、补骨脂、白芷、生苡仁、苏梗、藿香、芡实米、炒山药、五味子的提取物组成;
以每一千粒胶囊计,该药物胶囊各主药组分的用量分别为:白术60-80g、苍术60-80g、干姜60-80g、茯苓60-90g、炙甘草30-40g、防风60-80g、陈皮60-70g、白芍50-70g、肉蔻50-70g、吴萸30-40g、补骨脂60-75g、白芷50-70g、生苡仁180-240g、苏梗50-80g、藿香50-80g、芡实米60-100g、炒山药60-80g、五味子60-80g;
主药组分是采用以下方法制备:
步骤1:将白术、苍术、生苡仁、炒山药、芡实米研成细粉,过筛,备用;
步骤2:取干姜、茯苓、炙甘草、防风、陈皮、白芍、肉蔻、吴萸、补骨脂、白芷、苏梗、藿香、五味子,加4-6倍量水浸泡1-1.5小时,第一煎大火烧沸后改小火煎1-1.5小时,滗出药汁备用;第二煎加1.5-2.5倍量水煎煮0.5-1小时,滗出药液与第一煎汁合并、过滤制得滤液;
步骤3:将步骤1制得的粉末与步骤2制得的滤液混合,熬成糊状物;
步骤4:将步骤3制得的糊状物低温烘干,粉碎,制得粉末;
胶囊辅料包括粘合剂、填充剂、崩解剂、吸湿剂、润滑剂中的一种或数种; 所述填充剂为红薯淀粉;
主药组分制备完成后加入胶囊辅料混合搅拌,干燥,装入胶囊,即制得药物胶囊。
2.如权利要求1所述的预防并治疗慢性腹泻的药物胶囊,其特征在于:粘合剂为95%乙醇、聚乙烯吡咯烷酮、聚乙烯吡咯烷酮/乙酸乙烯酯共聚物中的任意一种。
3.如权利要求1所述的预防并治疗慢性腹泻的药物胶囊,其特征在于:填充剂为蔗糖、磷酸二氢钙、淀粉、海藻多糖、壳寡糖中的一种或几种。
4.如权利要求1所述的预防并治疗慢性腹泻的药物胶囊,其特征在于:崩解剂为羧甲基淀粉钠、低取代羟丙基纤维素、交联聚乙烯吡咯烷酮中的任意一种。
5.如权利要求1所述的预防并治疗慢性腹泻的药物胶囊,其特征在于:吸湿剂采用微粉硅胶。
6.如权利要求1所述的预防并治疗慢性腹泻的药物胶囊,其特征在于:所述润滑剂为滑石粉、硬脂酸富马酸钠、硬脂酸锌、硬脂酸镁、二氧化硅、聚乙二醇6000中的一种或几种。
7.如权利要求1所述的预防并治疗慢性腹泻的药物胶囊,其特征在于:所述低温烘干的温度控制在50-55℃。
