背景技术
甲状腺良性肿块就是良性甲状腺肿瘤，包括甲状腺腺瘤和囊肿等，多见于青年妇女。本病的发生与缺碘、内分泌失调和颈部放射线治疗有一定关系。 现代西医学多采用外科手术治疗，但术后较易复发。良性甲状腺肿瘤归属中医学"瘿病"范畴，与"瘿瘤""肉瘿"相似。关于瘿的记载首见于《山海经》。隋代《诸病源候论》将瘿分为"血瘿、息肉瘿、气瘿"三种，其中息肉瘿与本病颇为相似，并提出"息肉瘿可割之"的治疗方法。《外台秘要》对本病分类更详，共有36种治瘿方，其中多数为含碘药物。
本发明提供一种纯中药组方，治疗不适合采用手术治疗的甲状腺良性肿块患者，效果好，见效快，费用低，毒副作用低。

本发明采用的技术方案如下：
一种治疗甲状腺良性肿块的中药组合物，其特征在于，由以下重量配比的中药原料药制成 ：柴胡18-22g，当归28-32g、赤芍28-32g、郁金28-32g、莪术28-32g、生牡蛎48-52g、昆布48-52g、海藻48-52g、夏枯草48-52g、白芥子48-52g、陈皮23-26g、青皮23-26g、大贝母23-26g；桔梗8-10g、黄药子8-10g。
所述的治疗甲状腺良性肿块的中药组合物，其特征在于，由以下重量配比的中药原料药制成：柴胡20g，当归30g、赤芍30g、郁金30g、莪术30g、生牡蛎50g、昆布50g、海藻50g、夏枯草50g、白芥子50g、陈皮25g、青皮25g、大贝母25g、桔梗9g、黄药子9g。
所述的治疗甲状腺良性肿块的中药组合物的制备方法，其特征在于，包括以下步骤：
（1）按配方比例称取各组分原料；
（2）用黄酒将海藻浸泡30-40分钟，海藻和黄酒的重量比为1:4-5，将海藻和黄酒一起研磨成浆，然后加入混合浆料总重量1%的蜂蜜，充分搅拌均匀，得混合浆料；
（3）将生牡蛎焙干，喷香醋，然后进行超微粉碎，得到生牡蛎超细粉，备用；
（4）将配方中的其余原料药干燥混合粉碎，然后过60-80目筛，向粗粉中加入粗粉质量1-2%的低聚果糖、1-2%的面粉、1-2%的绿豆粉、150-180%的纯净水，配制培养基，并于121 ℃灭菌16 min，冷却后，按1-1.5％的接种量接种嗜酸乳杆菌、按3-4%接种量接种酵母，在温度为30-34 ℃的条件下，发酵20-24h，，然后降温至25-26℃的条件下，再发酵10-12h，发酵结束后，在50-55℃条件下干燥，粉碎，得到发酵混合粉；
（5）向步骤（4）得到发酵混合粉中加8-10倍量的乙醇回流提取2-3次，每次3-4h，合并滤液，得到发酵提取液，减压回收乙醇至无醇味，得发酵提取稠膏；
（6）乙醇回流提取后的药渣加8-10倍量的水煎煮2-3次，每次1-1.5h ,合并水煎液，浓缩至50-60℃相对密度为1.1-1.2的浸膏；
（7）将步骤（2）的混合浆料、步骤（5）发酵提取稠膏、步骤（6）水提浸膏及步骤（3）的获得的超细粉，混合均匀，得到药物混合粉；该药物混合粉单独或与药物可接受的载体混合，依照制剂学常规技术制成各种常规剂型。

治疗甲状腺良性肿块疗效好，见效快。 共治疗50例，其中治愈42例，占84%；有效8例，占16%。总有效率为100%。
具体实施方式
一种治疗甲状腺良性肿块的中药组合物，由下列原料药制成，各组分的重量（g）为：
柴胡20g，当归30g、赤芍30g、郁金30g、莪术30g、生牡蛎50g、昆布50g、海藻50g、夏枯草50g、白芥子50g、陈皮25g、青皮25g、大贝母25g、桔梗9g、黄药子9g。
中药组合物制成的药丸的制备方法， 包括以下步骤：
（1）按配方比例称取各组分原料；
（2）用黄酒将海藻浸泡30-40分钟，海藻和黄酒的重量比为1:4-5，将海藻和黄酒一起研磨成浆，然后加入混合浆料总重量1%的蜂蜜，充分搅拌均匀，得混合浆料；
（3）将生牡蛎焙干，喷香醋，然后进行超微粉碎，得到生牡蛎超细粉，备用；
（4）将配方中的其余原料药干燥混合粉碎，然后过60-80目筛，向粗粉中加入粗粉质量1-2%的低聚果糖、1-2%的面粉、1-2%的绿豆粉、150-180%的纯净水，配制培养基，并于121 ℃灭菌16 min，冷却后，按1-1.5％的接种量接种嗜酸乳杆菌、按3-4%接种量接种酵母，在温度为30-34 ℃的条件下，发酵20-24h，，然后降温至25-26℃的条件下，再发酵10-12h，发酵结束后，在50-55℃条件下干燥，粉碎，得到发酵混合粉；
（5）向步骤（4）得到发酵混合粉中加8-10倍量的乙醇回流提取2-3次，每次3-4h，合并滤液，得到发酵提取液，减压回收乙醇至无醇味，得发酵提取稠膏；
（6）乙醇回流提取后的药渣加8-10倍量的水煎煮2-3次，每次1-1.5h ,合
并水煎液，浓缩至50-60℃相对密度为1.1-1.2的浸膏；
（7）将步骤（2）的混合浆料、步骤（5）发酵提取稠膏、步骤（6）水提浸膏及步骤（3）的获得的超细粉，混合均匀，得到药物混合粉，加入适量蜂蜜，制成药丸。
用法用量：一日2次， 每次5-8g ，早晚各1次，药丸放口中含化，一般15天为一疗程。
典型病例
1、李某，女，50岁。1999年1月26日来诊，颈部不适3月，近日发现颈部有一包块。刻诊：甲状腺下有一3.4cm×3.6cm肿块，质中，触之疼痛，伴胸闷、心悸。T3、T4及TSH检查正常。同位素扫描示：热结节。彩超检查提示：甲状腺肿块存在。后经上海某医院做细胞学检查，排除甲状腺癌。经用本发明中药1疗程，肿块缩小为2.0cm×2.1cm，触之不疼痛，颈部不适感消失，胸闷、心悸亦好转；效不更方，继用药3个疗程，肿块消失。随访5年，未见复发。
2、瞿某，女，45岁。1999年6月2日就诊，颈部不适3月余，近日发现甲状腺下有一3.1cm×3.2cm肿块，触之微痛。相关检查T3、T4及TSH属正常。同位素扫描示：温结节。彩超、CT均提示：甲状腺肿块存在。病理学检查，排除甲状腺恶性肿瘤。用本发明中药1疗程，症状好转，肿块缩小至2.3cm×2.5cm。嘱其再服1 疗程，肿块消至1.5cm×1.7cm，该患者共用药4个疗程，肿块完全消失，随访5年未复发。
3、郑某，男，47岁。2001年5月6日初诊，近期发现甲状腺下长有1.9cm×2.1cm肿块，平时无不适，亦无异常发现。为慎重起见，立即给做相关检查，均提示甲状腺肿块存在。细胞学检查，排除甲状腺癌。经用本发明中药，治疗3个疗程，肿块完全消失，随访至今未复发。
4、龚某，女，21岁。2001年10月3日来诊，甲状腺下有一3.0cm×3.2cm肿块。多家医院检查，确诊为甲状腺良性肿块，建议手术治疗。患者惧怕手术和留下疤痕，要求保守治疗。经用本发明中药2疗程后，肿块缩小为1.6cm×1.7cm。先后用了4个疗程，肿块消失殆尽。随访5年未复发。
5、耿某，男，57岁。2010年7月6日初诊，最近发现甲状腺下长有1.7cm×1.9cm肿块，质中，触之痛。相关检查均支持属甲状腺良性肿块。病理学检查，排除甲状腺恶性肿瘤。遂用本发明中药3个疗程，肿块消失，随访5年未复发。

Claims (4)

1.一种治疗甲状腺良性肿块的中药组合物，其特征在于，由以下重量配比的中药原料药制成 ：柴胡18-22g，当归28-32g、赤芍28-32g、郁金28-32g、莪术28-32g、生牡蛎48-52g、昆布48-52g、海藻48-52g、夏枯草48-52g、白芥子48-52g、陈皮23-26g、青皮23-26g、大贝母23-26g；桔梗8-10g、黄药子8-10g。
2.根据权利要求1所述的治疗甲状腺良性肿块的中药组合物，其特征在于，由以下重量配比的中药原料药制成：柴胡20g，当归30g、赤芍30g、郁金30g、莪术30g、生牡蛎50g、昆布50g、海藻50g、夏枯草50g、白芥子50g、陈皮25g、青皮25g、大贝母25g、桔梗9g、黄药子9g。
3.如权利要求1所述的治疗甲状腺良性肿块的中药组合物的制备方法，其特征在于，包括以下步骤：
（1）按配方比例称取各组分原料；
（2）用黄酒将海藻浸泡30-40分钟，海藻和黄酒的重量比为1:4-5，将海藻和黄酒一起研磨成浆，然后加入混合浆料总重量1%的蜂蜜，充分搅拌均匀，得混合浆料；
(3) 将生牡蛎焙干，喷香醋，然后进行超微粉碎，得到生牡蛎超细粉，备用；
（4）将配方中的其余原料药干燥混合粉碎，然后过60-80目筛，向粗粉中加入粗粉质量1-2%的低聚果糖、1-2%的面粉、1-2%的绿豆粉、150-180%的纯净水，配制培养基，并于121 ℃灭菌16 min，冷却后，按1-1.5％的接种量接种嗜酸乳杆菌、按3-4%接种量接种酵母，在温度为30-34 ℃的条件下，发酵20-24h，然后降温至25-26℃的条件下，再发酵10-12h，发酵结束后，在50-55℃条件下干燥，粉碎，得到发酵混合粉；
（5）向步骤（4）得到发酵混合粉中加8-10倍量的乙醇回流提取2-3次，每次3-4h，合并滤液，得到发酵提取液，减压回收乙醇至无醇味，得发酵提取稠膏；
（6）乙醇回流提取后的药渣加8-10倍量的水煎煮2-3次，每次1-1.5h ,合并水煎液，浓缩至50-60℃相对密度为1.1-1.2的浸膏；
（7）将步骤（2）的混合浆料、步骤（5）发酵提取稠膏、步骤（6）水提浸膏及步骤（3）的获得的超细粉，混合均匀，得到药物混合粉；该药物混合粉单独或与药物可接受的载体混合，依照制剂学常规技术制成各种常规剂型。
4.根据权利要求1所述的治疗甲状腺良性肿块的中药组合物，其特征在于：
所述的组合物制成加入适量蜂蜜制成药丸，每颗药丸重5-8g。
