发明内容
已显示AON在体外和体内在多种疾病状态包括例如杜氏肌营养不良(Duchennemuscular dystrophy,DMD)中成功地调节基因表达。具体地,经设计以靶向并移除框内外显子23的PMO已在DMD的mdx小鼠模型中成功恢复抗肌萎缩蛋白(dystrophin)功能。
然而,AON还显现在骨骼肌和心肌细胞中不良吸收的概貌,这妨碍其影响mRNA转录和翻译的能力。(WO 2009/147368)具体地,PMO,2-O-甲基寡核苷酸和肽核酸(PNA)不会显著地累积在骨骼肌中且其在心肌中的吸收是可忽略的。
在1980年代晚期发现了细胞穿透肽(CPP)。[Frankel和Pabo Cell 1988;Green和Loewenstein Cell 1988]这些化合物帮助转运不同部分穿过细胞膜,以其自身穿过细胞膜本来有困难。在发现CPP约20年后,使用mdx小鼠模型的数个研究显示,称为“K”(RXRRXRRXRRXRXB,SEQ ID NO.:43)和“B”(RXRRBRRXRRBRXB,SEQ ID NO.:44)的CCP连接于(tethered to)经设计以跳过编码抗肌萎缩蛋白的mRNA的外显子23(PMO23)的单一PMO极大地增加骨骼肌和心肌中的外显子跳过。[Jearawiriyapaisarn等Mol Therapy 2008]这些研究的结果证明了单一PMO-CPP缀合物在骨胳肌中呈现>85%外显子23的跳过,而裸PMO化合物则造成<15%外显子跳过。相比裸PMO(其在心肌中并未造成外显子23跳过),在心肌中还观察到适度的效果(~60%)[如前文所述]。
WO 2009/144481涉及包括细胞递送肽与生物活性化合物如AON(包括例如PMO)的复合的构建体。WO 2004/097017(US 2004/0265879和US2009/0082547)涉及增强分子递送的方法,其包括公开了生物作用剂(如PMO)和肽转运子的缀合物。WO 2009/147368公开了新的CPP,其可与例如PNA和PMO缀合。US 2010/0130591公开了能结合人抗肌萎缩蛋白基因中所选靶向位置的PMO,其可与CPP缀合。
虽有以单一PMO-CPP缀合物观察到的增强的效力,该缀合物相比裸PMO具备增加的毒性[引用]。例如,对于使用CPP-PMO23B或CPP-PMO23K的mdx小鼠外显子23跳过研究,最大耐受剂量显示为30mg/kg。在60mg/kg的量时,小鼠体重减少而150mg/kg的剂量则为致死的。[Amantana等Bioconjugate Chem 2007]相反地,裸PMO23可以高至250mg/kg的量施用而无任何明显的毒性作用[如前文所述]。WO 2009/005793公开了具有下列四个X(6-氨基己酸)残基的CPP,包括CPP“B”呈现比之前鉴定的CPP更低的毒性。然而,如上所示,这些与单一PMO偶联的CPP相比裸PMO仍显示不可接受的毒性[如前面的引用文及Moulton和MoultonBiochemica et Biophysica Acta 2010]。
因此,本公开的主要目标是克服裸PMO的不良吸收及与单一PMO-CPP构建体有关的毒性增加的问题。通过在D-或L-对映异构位置添加谷氨酸导入CPP上的多个缀合位点以降低靠近CPP末端处的多个PMO的立体位阻。使用在P1具有氨基酸苯丙氨酸-赖氨酸的肽酶裂解位点,将-P1位置导入CPP和PMO之间。肽酶裂解位点的裂解将PMO从CPP释放。例如,CPP可经由马来酰亚胺酯化的接头以可在半胱氨酸位置导入的酶裂解位点苯丙氨酸-赖氨酸或缬氨酸-瓜氨酸与PMO附接。CPP还可经由酰胺键附接。
使用肽内化进入细胞可产生细胞内蛋白酶,包括溶酶体酶,其裂解该裂解位点而且游离的PMO会具有来自肽结合的缀合物的较低的立体位阻。该较低的立体局限可增加外显子跳过效力而具有对细胞核和细胞质较小的毒性。

实施方案的说明/发明详述
如上文所述,本发明的一个目标是改善单一PMO-CPP缀合物的安全概貌,而不损害缀合物进入目标细胞的摄取。一般优选的是在有效剂量和无不良作用水平(NOAEL)之间具有至少10-倍安全边际。在上述mdx/外显子23实验中,发现有效剂量为30mg/kg而NOAEL也为30mg/kg。结果,仅有极少或无安全边际。在本公开的至少一个实施方案中,多个PMO-CPP缀合物具有比上述mdx/外显子23实验更好的安全边际,如为后者的2-倍,如为5-倍,如为6-倍,如为10-倍。10-倍安全边际指该有效剂量比NOAEL低10倍(为NOAEL的1/10)。因此,当在适当的模型中测试时,根据本发明的多个PMO-CPP缀合物可具有为对应的单一PMO-CPP缀合物至少2-倍(好)的安全边际。本发明包括与单一CPP附接的多个AON,包含PMO AON。

令人惊奇地发现,根据本公开的具有可裂解接头位点的多个PMO-CPP缀合物在一些情况下呈现甚至比无可裂解接头位点的多个PMO-CPP缀合物更好的效力。该结果为预料不到的,因为已显示,如果肽部分降解太快,则单一PMO-CPP缀合物被吸收入溶酶体中并保持陷入。换言之,如果单一PMO-CPP缀合物的CPP部分快速降解,则PMO变得陷入溶酶体中且无法到达其细胞靶标,因而减少效力。(Youngblood等,2007)作为此现象的结果,已预期在缀合物中加入可裂解接头位点会促进CPP的降解,导致效力减少。然而,在如下面具体实施例中描述的根据本发明的具有可裂解接头位点的一些多个PMO-CPP缀合物中情况并非如此。因此,在本公开的至少一个实施方案中,多个PMO-CPP缀合物具有至少一个可裂解接头位点。在本公开的至少另一个实施方案中,多个PMO-CPP缀合物具有至少一个组织蛋白酶可裂解接头位点。

当用作为药物时,本文公开的多个CPP-PMO缀合物反义寡核苷酸可与药物可接受的赋形剂或载剂配制以配制成药物组合物。
当用作为药物时,多个CPP-PMO缀合物反义寡核苷酸可以药物组合物的形式施用。这些化合物可通过多种路径施用,包括口服、直肠、经皮、皮下、静脉内、肌肉内和鼻内。这些化合物作为可注射和口服组合物二者都是有效的。这样的组合物以药物领域已知的方式制备并包含至少一种活性化合物。

在配制物的制备中,必需的是在与其他的活性成份组合之前磨制活性化合物以提供适当的粒度。如果活性化合物基本上为不溶性的,其一般磨制成低于200目(mesh)的粒度。如果活性化合物为基本上水溶性的,一般通过磨制来调整粒度以提供配制物中基本上均匀的分布,例如约40目。

多个CPP-PMO缀合物反义寡核苷酸在广泛的剂量范围有效,且一般以治疗有效量施用。然而,应该理解的是,实际上施用的多个CPP-PMO缀合物反义寡核苷酸的量会由医师依照相关的情况(包括待治疗的病况、选择的施用路径、实际施用的化合物、个体患者的年龄、重量和响应、患者症状的严重程度等)来确定。
对于制备固体组合物如片剂,将主要活性成份/多个CPP-PMO缀合物反义寡核苷酸与药物赋形剂或载剂混合以形成含有本发明化合物的均质混合物的固体预配制组合物。当称这些预配制组合物为均质时,是指该活性成份均匀地分散遍及组合物,使得组合物可容易地细分成同等有效的单位剂型,例如片剂、丸剂和胶囊。

本领域的普通技术人员会能够设计可实现本公开的目标(如增强的细胞穿透)的其他CPP序列。结果,目前公开的多个PMO-CPP和AON-CPP缀合物并不限于本文公开的CPP组分。
如本文使用的,NOAEL指未观察到不利影响的剂量水平。换言之,NOAEL为最大安全剂量。
在一些实施方案中,接头为含有裂解基序的序列。在一些实施方案中,裂解基序可通过任何水解酶裂解。在一些实施方案中,裂解基序可通过肽酶或蛋白酶,例如组织蛋白酶或胰蛋白酶裂解。如本文使用的,组织蛋白酶可裂解接头与组织蛋白酶裂解位点同义。根据本公开的可裂解接头能被胞内酶裂解,即化学降解。在一些实施方案中,接头可经设计以包括由特定丝氨酸蛋白酶、苏氨酸蛋白酶、半胱氨酸蛋白酶、天冬氨酸蛋白酶、谷氨酸蛋白酶或金属蛋白酶或多于一个肽酶的组识别的裂解基序。在一些实施方案中,接头可包括两个或多个重叠或非重叠的裂解基序。在一些实施方案中,接头可不含有裂解基序。在一些实施例中,接头可经设计使得少于100%、少于75%、少于50%或少于10%的接头被裂解。在一些实施方案中,接头可经设计使得多于90%或99%的接头被裂解。

实施例
肽合成
使用Ranin Symphony自动肽合成仪,使用标准Fmoc化学在预装载的CLEAR(交联的聚氧乙烯醚丙烯酸酯树脂(Ethoxylate Acrylate Resin))(Peptides International,Louisville,KY)上合成肽。将氨基酸(EMD Biosciences,San Diego,CA或Anaspec,SanJose,CA)以叔丁氧基羰基(BOC)、叔丁基(tBu)2,2,4,6,7-五甲基二氢-苯并呋喃-5-磺酰基(Pbf)或三苯甲基(Trt)基团正交保护。使用摩尔比为5/5/10/1的氨基酸/HCTU/N-甲基吗啉/树脂实施偶联。使用DMF中的20%哌啶、2%1,8-二氮杂双环[5.4.0]-十一-7-烯(DBU)在各循环过程中从胺端移除Fmoc。使用DMF中摩尔比为30/8/1的乙酸酐/NMM/树脂在树脂上进行N-端乙酰化。使用2.5%水/2.5%TIS/5%苯甲醚/90%TFA体积比的15ml/0.1mM树脂的混合物将从树脂去保护/裂解进行3小时。上清在二乙醚中(-80℃)析出并在3000rpm沉淀10分钟。将乙醚倾倒出并再次清洗沉淀。将粗肽冻干并使用半制备式逆相HPLC(XBridge C18,10x250mm,5μm粒度)纯化。于5ml/分钟以99%缓冲液-A(水,0.1%TFA)和1%缓冲液-B(乙腈,0.1%TFA)的装载梯度(梯度斜率,0.3%B/分钟)实施纯化。以逆相分析型HPLC和MALDI-TOF(Waters Synapt)评估肽纯度>95%。将级分汇集、冻干并再溶解于适当的缀合缓冲液中。(所有的溶剂为HPLC等级,购自EMD Biosciences,San Diego,CA或Sigma Aldrich,St.Louis,MO)。
肽(CPP)-PMO缀合
PMO以5’-端伯氨排序并如上述合成肽。与PMO相比,在单一PMO-CPP缀合物中使用2-倍摩尔过量的肽,在多个PMO-CPP缀合物中,使用下列方程计算PMO摩尔数:(1.2-倍)x(缀合物数目)x(肽摩尔数)。将PMO溶于DMSO(5mM)并放置一旁。将肽溶于DMF(50mM)并与摩尔当量的基于铵的偶联剂2-(6-氯-1-H-苯并三唑-1-基)-1,1,3,3-四甲基六氟磷酸铵混合。将2摩尔当量的4-甲基吗啉(NMM)加入肽混合物中并立刻添加至PMO溶液。反应于37℃进行1.5小时并使用4体积当量的水停止。将混合物添加至CM琼脂糖(sepharose)(Sigma Aldrich,St.Louis,MO)柱并以10个柱体积洗涤以移除未缀合的PMO和反应物。以1M胍-HCL、1M NaCl从柱洗脱肽CPP-PMO缀合物并针对100mM NH4HCO3以3个缓冲液交换透析以移除盐和未缀合的肽。将透析的材料冻干并以分析型HPLC分析。

在给药结束后,在麻醉下收获组织并于液氮中冷冻。处理冷冻的心脏和四头肌以分离RNA。使用nanodrop ND1000分光光度计定量样本。将样本进一步稀释成15ng/μl的浓度。将样品使用具有下列序列的引物扩增:CAGAATTCTGCCAATTGCTGAG(SEQ ID NO.:83)及TTCTTCAGCTTGTGTCATCC(SEQ ID NO.:84)。以55℃/30分钟;94℃/2分钟;94℃/15秒x 30次;55℃/30秒;68℃/1.5分钟的运行序列进行PCR。

如上所示,通过将多于一个PMO附接于单一的CPP-K而非CPP-B而改进了效力。通过IV注射递送的单一剂量CPP-B(其与2个PMO外显子23部分缀合)(编号1239)效力远不及与单一PMO外显子23缀合的CPP-B(编号1204)。令人惊奇的是,我们发现以完全相同的化学方式与2个PMO缀合的单一剂量CPP-K(编号1119)效力为与单一PMO外显子23缀合的单一剂量CPP-K(编号1118)的大约2倍。有趣的是,作为单体构建体,CPP-PMO23B比CPP-PMO23K更有效。在其中3个PMO与CPP连接的多个CPP-PMO缀合物研究中,3个PMO23部分与单一肽B缀合(编号1216)与二-PMO B相比活性加倍(尽管三-PMO23-B就其外显子跳过能力而言与二-PMO23-K类似)。这些结果表明哪些多个CPP-PMO缀合物会具有预期的性质无法事先预知,且因此本领域的普通技术人员必须使用本公开所述的方法实施初步研究以确定制备的每种缀合物的效力。

其中所述至少两个AON化合物具有与靶标前-mRNA或mRNA的至少75%的碱基互补的核苷酸碱基序列;
其中所述至少两个AON化合物具有0至3个重复的亚基,其中B为G;
其中所述至少两个AON化合物具有少于60%的所述亚基,其中B为C或G;
其中所述至少两个AON化合物具有非自我互补的序列;且
其中所述至少两个AON化合物与聚阳离子肽缀合。
