Abstract

本发明公开了一种治疗妇科慢性盆腔炎微型凝胶灌肠剂,该灌肠剂是以当归,赤芍,鸡血藤,元胡,红藤,败酱草,薏米仁,小茴香原料药,经过水提,澄清、浓缩,得流浸膏;再将辅料与流浸膏混合,搅拌至辅料完全溶胀;然后用三乙醇胺调pH=6,即本得发明的中药微型凝胶灌肠剂。本发明制备的微型凝胶灌肠剂,不会破坏有效成分的药效,成药中含有效生药量高,使用中有效成分可以直接通过直肠静脉网吸收进入盆腔静脉网,有效避免了口服剂型的肝脏“首过效应”,生物利用度显著提高,主要应用于用于治疗慢性附件炎、盆腔炎、阴道炎、膀胱炎、慢性阑尾炎、尿路感染等疾病,相比丸、片、胶囊、颗粒、滴丸等口服剂型,能显著提高治疗效果和缩短疗程。

Description

一种治疗慢性盆腔炎的中药微型灌肠剂
技术领域
本发明涉及一种治疗妇科慢性盆腔炎微型凝胶灌肠剂及其制备方法,属于中药加工技术领域。
背景技术
慢性盆腔炎是妇科临床常见病、多发病,病情顽固,易复发。主要表现为下腹痛、腰骶酸痛、白带多、痛经、月经不调等症状。中医学认为,慢性盆腔炎病位在盆腔,其病因多由湿热蕴结,气滞血瘀造成,迁延难愈。治疗以活血化瘀、清热解毒是治疗本病的首要法则。清热可解毒和利湿消炎,有利于祛邪解毒;化瘀可散瘀,通过降低炎性组织毛细血管的通透性,有利于炎症的吸收。目前用于治疗慢性盆腔炎的药物制剂主要有妇炎康丸、妇炎康胶囊、妇炎康颗粒、妇科千金片等,其剂型均通过口服给药,由于首过效应,生物利用度较低,起效慢,治疗效果差,疗程长。
微型灌肠剂是一种直肠给药的新剂型,用量通常在5ml以下。一般制成溶液或使用凝胶辅料制成凝胶状制剂。药物以分子或微小粒子状态分散,没有栓剂的熔融、释放于体液的过程,有利于药物的吸收。申请人经过多年的临床实验,获得了治疗慢性盆腔炎病的药物组方 (由当归、赤芍、大血藤、鸡血藤等中药组成),并通过中药材直接煎煮得到的“复方当归妇炎灌肠剂”进行直肠给药。临床资料显示,采用灌肠剂后,避免了长期、反复应用抗生素可能产生耐药性,使耐药菌株增加等不良反应,同时药物通过直肠黏膜的吸收,直达病所,疗效明显优于使用抗生素治疗。具体见《中医临床研究》2015 年第 7 卷 第 6 期第67-69页“复方当归妇炎灌肠剂治疗盆腔炎性疾病 100 例临床观察”。然而,该灌肠剂虽然疗效明确,但是该灌肠剂是中药材直接煎煮所得,用药量大,使用不便。
发明内容
本发明的目的是针对现有技术治疗慢性盆腔炎口服制剂存在的问题,提供一种治疗慢性盆腔炎的中药微型凝胶灌肠剂。
一、中药微型凝胶灌肠剂的制备
本发明治疗慢性盆腔炎的中药微型凝胶灌肠剂,是由以下原料配方和工艺制备而成:
原料配方:以重量份计
当归100份,赤芍30份,鸡血藤100份,元胡50份,大血藤(红藤)100份,败酱草100份,薏米仁100份,小茴香30份。
制备工艺:包括以下步骤:
(1)将当归,赤芍,鸡血藤,元胡,大血藤(红藤),败酱草粉碎成大米粒大小;薏米仁,小茴香打裂;混合两组药材,加水浸泡1~1.5h后煎煮三次,第一次加入药物重量10倍量的水,煎煮1 h,第二次加入药物重量8倍量的水,煎煮1 h,第三次加入药物重量5倍量的水,煎煮0.5 h;合并数次水煎液,滤过,得药液,于3~6℃(置冰箱中)放置12 ~ 24 h;
(2)将上述所得药液高速离心(离心速度为7000~9000r/min),取上清液,再将上清液与生药(是指原料中药材)按1:1的质量比浓缩,得流浸膏;
(3)将辅料与流浸膏按25~27mL/100mL流浸膏混合,搅拌至辅料完全溶胀;再用三乙醇胺调pH=6,即本得发明的中药微型凝胶灌肠剂。
其中辅料由以下方法制得:将1g防腐剂尼泊尔金乙酯溶入100mL甘油中,混匀后加入7g 卡波姆-940,充分研磨,既得。
二、中药微型凝胶灌肠剂的药效学实验
1、对小鼠耳廓二甲苯炎症作用
将60只SPF级昆明种雌性小鼠按随机数字表法分为6个组:空白组,妇炎康组,布洛芬组,本发明大剂量组,本发明中剂量组,本发明小剂量组各组ig给药,1次/d,连续7d。高、中、低剂量除相应的药物外,还进行灌胃生理盐水 1mL /100g。妇炎康组还进行直肠给予生理盐水 0.2mL /100g;;空白组和模型组给予直肠和灌胃生理盐水双重处理,每日 1 次,连续给药 21d。末次给药1h后,右耳涂以二甲苯0.05 mL/只,左耳作对照,不做处理。30min后处死动物,沿耳廓基线剪下双耳,用直径9mm打孔器将双耳同部位等面积切下,称切下耳片质量,用下面公式计算肿胀度和抑制率,数据用 ±s表示,组间比较用t 检验分析,结果见表1。

抑制率 =(空白肿胀度组-药物组肿胀度 )/空白组肿胀度× 100%

表1结果表明,空白组小鼠在给予二甲苯致炎后,右耳明显肿胀。与空白对照组比较,布洛芬组、妇炎康片组、本发明灌肠剂高剂量组、本发明灌肠剂中剂量组、本发明灌肠剂低剂量组,均能显著减轻二甲苯致炎小鼠的耳肿胀度(P<0.05或P<0.01),提示各实验组均有较好的抗炎作用。与妇炎康片组比较,本发明灌肠剂高、中剂量组作用强度均有显著增强。
2、对小鼠热板法镇痛实验
将60只(18~22g)SPF级昆明种雌性小鼠置于热板上(55℃ ± 0.5℃),记录小鼠自放入热板至小鼠添后足所需要的时间,为该小鼠的痛阈值,筛选痛阈值为5 ~ 30 s的小鼠。按照痛阈值均等的原则分为6个组:空白组,妇炎康组,布洛芬组,本发明大剂量组,本发明中剂量组,本发明小剂量组各组ig给药,1次/d,连续7d。高、中、低剂量除相应的药物外,还进行灌胃生理盐水 1mL/100g。妇炎康组还进行直肠给予生理盐水 0.2mL/100g;;空白组和模型组给予直肠和灌胃生理盐水双重处理,每日 1 次,连续7d。于末次给药后 30 min、1 h、2h、3h、4 h分别测量小鼠的痛阈值,若小鼠在60s内仍无痛觉反应,取出,按60s计,比较各组间差异,结果见表2。

表2结果表明,与空白对照组比较,布洛芬组、妇炎康片组、本发明灌肠剂大剂量组、本发明灌肠剂中剂量组、本发明灌肠剂低小剂量组,均能显著抑制热板所致小鼠的疼痛(P值均<0.01),并且本发明灌肠剂高、中、低组呈良好的量效关系,提示各实验组均有较好的镇痛作用。与妇炎康片组比较,本发明灌肠剂高、中剂量组作用强度均有显著增强(P值均<0.01),提示本发明灌肠剂镇痛作用强于妇炎康片。
3、对冰醋酸致小鼠扭体反应实验
将60只(18~22g ) SPF级昆明种雌性小鼠,按随机数字表法分为6个组:空白组,妇炎康组,布洛芬组,本发明大剂量组,本发明中剂量组,本发明小剂量组各组ig给药,1次/d,连续7d。高、中、低剂量除相应的药物外,还进行灌胃生理盐水 1mL / 100g。妇炎康组还进行直肠给予生理盐水 0.2 mL / 100g ;空白组和模型组给予直肠和灌胃生理盐水双重处理,每日1 次,给药 7d。实验前动物禁食12 h,末次给药1 h后,各组小鼠腹腔注射0.6%的冰醋酸0.1mL+·10g-1,诱导小鼠扭体反应。记录注射致痛剂后,首次出现扭体反应的时间和15min内各鼠扭体时间及出现扭体反应的次数,比较各组间差异,观察药物镇痛作用。结果见表3。

表3结果表明,与空白组比较,布洛芬组、妇炎康片组、本发明灌肠剂大剂量组、本发明灌肠剂中剂量组、本发明灌肠剂低小剂量组,均能显著显著延长冰醋酸致小鼠内脏疼痛的反应潜伏时间和减少15min内扭体次数,并且本发明灌肠剂高、中、低组呈良好的量效关系,提示各实验组均有较好的镇痛作用。其大、中剂量镇痛作用强于妇炎康片。
综上所述,本发明制备的微型凝胶灌肠剂,不会破坏有效成分的药效,成药中含有效生药量高,使用中有效成分可以直接通过直肠静脉网吸收进入盆腔静脉网,有效避免了口服剂型的肝脏“首过效应”,生物利用度显著提高,主要应用于用于治疗慢性附件炎、盆腔炎、阴道炎、膀胱炎、慢性阑尾炎、尿路感染等疾病,相比丸、片、胶囊、颗粒、滴丸等口服剂型,能显著提高治疗效果和缩短疗程。另外,本发明通过剂型改造,使载药量加大,用量减少,便于贮藏,使用携带方便;并且做了药效学和质量标准研究,使质量稳定,用量易控,疗效可靠。
具体实施方式
下面通过具体实施例对本发明微型凝胶灌肠剂的制备作进一步说明。
药物组方:当归10g,赤芍3g,元胡5g,大血藤(红藤)10g,鸡血藤10g,薏米仁10g,败酱草10g,小茴香3g。
制备工艺:
(1)将当归,赤芍,鸡血藤,元胡,大血藤(红藤),败酱草粉碎成大米粒大小;薏米仁,小茴香打裂;混合两组药材后,加10倍量水浸泡1h,煎煮三次:第一次加入10倍量的水,煎煮1h,第二次加入8倍量的水,煎煮1 h,第三次加入5倍量的水,煎煮0.5 h;合并数次水煎液,滤过,得药液,置冰箱中(3 ~6℃)放置12 ~ 24 h;
(2)将药液用高速离心机离心(转速:8000r/min,离心时间25 min),取上清液;再将上清液与生药量按1:1体积质量比浓缩,得流浸膏;
(3)辅料的制备:将1g防腐剂尼泊尔金乙酯溶入100mL甘油中混匀;取甘油21mL,加1.4g卡波姆-940,充分研磨,既得辅料;
(4)微型凝胶灌肠剂的制备:取流浸膏100mL,加入4g辅料,搅拌至其中的卡波姆-940完全溶胀后,用三乙醇胺调至pH=6,即本得发明的中药微型凝胶灌肠剂。
上述制备的凝胶灌肠剂每100mL含有相当于61g原药材。

Claims (6)

1.一种治疗慢性盆腔炎的中药微型凝胶灌肠剂,是由以下原料配方和工艺制备而成:
原料配方:以重量份计
当归100份,赤芍30份,鸡血藤100份,元胡50份,大血藤(红藤)100份,败酱草100份,薏米仁100份,小茴香30份;
制备工艺:包括以下步骤:
(1)将当归,赤芍,鸡血藤,元胡,大血藤(红藤),败酱草粉碎成大米粒大小;薏米仁,小茴香打裂;混合两组药材,加水浸泡1~1.5h后煎煮三次,合并数次水煎液,滤过,得药液,于3~6℃放置12 ~ 24 h;
(2)将上述所得药液高速离心,取上清液,浓缩,得流浸膏;
(3)将辅料与流浸膏混合,搅拌至辅料完全溶胀;再用三乙醇胺调pH=6,即本得发明的中药微型凝胶灌肠剂。

5.如权利要求1所述治疗慢性盆腔炎的中药微型凝胶灌肠剂,其特征在于:制备工艺步骤(3)的辅料由以下方法制得:将1g防腐剂尼泊尔金乙酯溶入100mL甘油中,混匀,再加入7g卡波姆-940,充分研磨,既得。
6.如权利要求1所述治疗慢性盆腔炎的中药微型凝胶灌肠剂,其特征在于:制备工艺步骤(3)中,辅料与流浸膏按25~27mL/100mL流浸膏混合。
