本发明涉及用于关节软骨重建的可再吸收装置。
举例而言,本发明适用于由创伤或关节(如膝、肩或还有其他关节)退行性疾病造成损伤的软骨的重建。
根据已知信息,在导致软骨组织病变的关节创伤或退行性关节病理的情况下,为了复位并允许关节简单运动,有必要进行外科手术。详细地说,在外科手术过程中,通过执行关节切开术或关节镜检查,将软骨碎片及受损软骨从关节区域移除。
在该清洁及平整手术之后,有必要从患者抽取血液并将所述血液置于培养物中,以从中获取富含蛋白质的血浆。
随后,将基质植入在受影响关节的骨上,并且以所述培养物对其冲灌。
为了完成重建,必须从手术时间开始随时间对经过这样手术的患者进行自体血液或额外培养物的另外浸渍。
该技术有一些显著的缺点。事实上,首先,软骨细胞培养是一种非常昂贵的方法,并且需要长时间才能完成。此外,该技术迫使患者进行多阶段的治疗。实际上,将培养的细胞进行移植发生于外科手术之后,并且在组织再生阶段中可还需要进一步的支持治疗。

装置1包含织物片(piece of fabric)2。将在下文中更加清楚的是,旨在将该片2固定于患者关节的骨“O”上。
特别地,该织物由PGA(聚乙交酯或聚乙醇酸)制成,优选为均聚物。还更特别地,所述织物用由PGA纤维衍生的超轻单丝或线制成。
PGA是高度生物相容且可再吸收的聚合物。详细来说,PGA的再吸收时间大约为一个月。
片2的织物可如下获得:以多种方式织造PGA线,产生编针织物(knitted fabric)、织造织物(woven fabric)或非织造织物(non-woven fabric)。
优选地,所述织物是编针织物,还更优选经编针织物(warp knitted fabric)。
在这种情况下,所述织物具有能够呈现具有足够小网眼的网状结构的较粗糙表面。
详细来说,其纬纱为使得其间隙空间小于200μm,优选为约160μm,相当于孔的平均面积等于约0.02mm2。这确保由片2覆盖的组织得到良好保护。
此外,所述织物优选地为有织纹的,从而赋予其甚至更大的表面粗糙度以及更大的刚性和不渗透性。
此外,所述织物优选地以密度为50至200旦尼尔的线获得。
仅举例来说,所述织物的厚度基本上为0.3mm至0.6mm,更优选地为0.4mm至0.53mm,并且还更优选基本上为0.45mm。
根据一些优选的实施方案,所述片2为正方形。优选地,所述片2的边基本为10cm。
在使用中,使用者可根据适合于待治疗区域的形状与尺寸切割所述片的一部分。
优选地,所述片2被胶原浸渍。还更优选地,所述片2被纯化的猪胶原浸渍。
所述装置1还包含对于将片2固定于骨所必需的限制部件(constraining means)3。
优选地,所述限制部件3包含纤维蛋白胶,其以获得固定的方式粘附于所述片2与骨“O”之间的特定点。
此外,所述限制部件3还包含螺钉(screw)和/或锚定件(anchor)和/或卡钉(staple)和/或针钉(pin)。在图3中,展示了两个卡钉4及一个针钉5。
所述螺钉和/或锚定件和/或卡钉4和/或针钉5由可再吸收的材料制成。优选地,所述螺钉和/或锚定件和/或卡钉4和/或针钉5由PGA/PLA制成。
根据用于实施本发明的装置1的方法,首先将所述片2浸入所述患者自身的血液中。
对于此阶段所必须的血液可通过注射器在手术过程中抽取。
在所述手术区域出血的情况下,源自该出血的血液可被有利地利用。
在进行装置1的植入之前,将具有软骨损伤的关节区域清洁。换言之,将软骨碎片及其受损的但仍固着于所述骨的条块移除。
仍在植入之前,优选地,可在骨“O”上的旨在由所述片2覆盖的区域中制造一个或更多个孔。这样的孔“F”的最大深度为3cm。
所述孔“F”的深度是这样的,其使得细胞及蛋白质从所述骨“O”自身中流向所述片2,从而促进新软骨的重建。
在此完成后,将所述片固定在骨“O”上将被重构的区域。
出于这样的目的,将所述纤维蛋白胶排布于所述骨“O”的有限数目的点上,并且之后使用片2.
随后优选地,应用其他限定部件3,例如上文提到的螺钉和/或锚定件如/或卡钉如/或针钉。
这些其他限制部件3的应用取决于所述片2的延展以及所述骨“O”将被覆盖的区域的位置及形式。

事实上,所描述的用于软骨重建的可再吸收装置的应用使得避免了培养,所述培养在相当程度上是极端漫长、昂贵并且复杂的手术。

所述软骨的重建事实上通过由所述片浸渍的血液及直接来自于穿孔的骨的蛋白质所确保。

Claims (9)

1.用于软骨重建的可再吸收装置,其包含:
-由PGA制成的织物片(2),其可在关节处固定于骨(O),所述片(2)优选地用胶原浸渍。
2.根据权利要求1的装置,其特征在于,所述片(2)的织物为经编针型的。
3.根据权利要求1或2的装置,其特征在于,所述片(2)的织物为有织纹的。
4.根据前述权利要求中任一项的装置,其特征在于,所述片(2)的织物的厚度为0.1mm至2mm,优选为0.3mm至0.6mm,更优选为0.4mm至0.53mm,还更优选为0.45mm。
5.根据前述权利要求中任一项的装置,其特征在于,所述片(2)的织物以密度为50至200旦尼尔的线获得。
6.根据前述权利要求中任一项的装置,其特征在于,其包含用于将所述片(2)固定于所述骨的限制部件(3)。
7.根据权利要求6的装置,其特征在于,所述限制部件(3)包含纤维蛋白胶。
