本发明涉及一种用于修复皮肤溃疡的干细胞制剂,包括间充质干细胞和外源性细胞因子。通过采用间充质干细胞结合外源性细胞因子共同制成干细胞制剂,使得间充质干细胞能够定向向内皮细胞分化,并通过间充质干细胞的旁分泌及外源性细胞因子能够刺激内皮细胞的生长,增加创面中成纤维细胞、毛细血管的生成以及胶原合成和肉芽的生长,并能够增加皮肤角化细胞及内皮细胞的增殖,从而能够实现快速有效修复因溃疡损伤的皮肤的目的。

技术领域
本发明涉及细胞工程技术领域,特别涉及一种用于修复皮肤溃疡的干细胞制剂。
背景技术
慢性皮肤溃疡又称难治性皮肤溃疡,是一种常见难治性疾病,具有病因复杂、病程长和反复发作特点,可引起皮肤表皮和部分真皮甚至皮下脂肪缺损。致病因素包括糖尿病、外伤、压迫和感染等,其中糖尿病为主要原因。目前,我国糖尿病患者约9200万,15%~25%患者在其疾病过程中将发生足溃疡。
目前,尚无慢性皮肤溃疡治疗指南,临床上多采用清创术、负压疗法、抗感染药物及敷料处理,这些方法都达不到兼具促进创面愈合和保护创面的作用,治疗效果不尽人意。
发明内容
本发明所要解决的技术问题是,针对现有技术中治疗皮肤溃疡效果差等缺陷,提供一种能够快速有效治疗皮肤溃疡的用于修复皮肤溃疡的干细胞制剂。
本发明解决其技术问题所采用的技术方案是:提供一种用于修复皮肤溃疡的干细胞制剂,包括间充质干细胞和外源性细胞因子。
在本发明提供的用于修复皮肤溃疡的干细胞制剂中,所述干细胞制剂中,所述间充质干细胞的浓度为(2~8)×107个/ml。
在本发明提供的用于修复皮肤溃疡的干细胞制剂中,所述外源性细胞因子包括成纤维细胞生长因子、转化生长因子、表皮生长因子、纤维母细胞生长因子、血管内皮生长因子、血小板源生长因子、类胰岛素生长因子和P物质。
在本发明提供的用于修复皮肤溃疡的干细胞制剂中,所述外源性细胞因子来源于血小板血浆裂解液。
在本发明提供的用于修复皮肤溃疡的干细胞制剂中,所述血小板血浆裂解液的制备过程包括以下步骤:
无菌条件下,取骨髓,加入肝素钠抗凝,离心至骨髓分层,取下层细胞层,加入标准培养基进行原代培养;取原代培养后的细胞悬液离心后吸取上层血浆和下层红细胞进行第二次离心,再次取上层血浆和下层红细胞进行第三次离心,弃上清液后,混匀,置于-50~-80℃保存,24~36小时后取出,水浴解冻,反复冻存解冻至少三次,即可获得血小板血浆裂解液。
在本发明提供的用于修复皮肤溃疡的干细胞制剂中,所述标准培养基为含有体积分数为5~15%血清、浓度为2~15U/ml的强力霉素以及1~5U/ml肝素的DMEM培养基。
在本发明提供的用于修复皮肤溃疡的干细胞制剂中,所述干细胞制剂中,所述血小板血浆裂解液的体积分数为5~20%。
在本发明提供的用于修复皮肤溃疡的干细胞制剂中,所述间充质干细胞的获取过程包括以下步骤:
获取脐带,于无菌条件下去除动静脉及外膜,PBS清洗,剪成段,再次清洗去除血液,然后处理成肉糜状,水浴消化后,加入基础培养基反复吹打,过滤,收集滤液离心,弃上清,将沉淀的细胞接种于完全培养基中进行培养,次日首次换液,之后每2~3天换液一次;待细胞融合,不完全消化后,进行传代纯化,获得第三代间充质干细胞完全消化后,继续进行传代培养。
在本发明提供的用于修复皮肤溃疡的干细胞制剂中,所述干细胞制剂还包括20~80mg/ml的血清蛋白、柠檬酸钠、0.1~0.5mol/L的氯化钠和5~15μmol/L法舒地尔。
在本发明提供的用于修复皮肤溃疡的干细胞制剂中,还包括1~15U/mL的强力霉素。
实施本发明提供的用于修复皮肤溃疡的干细胞制剂,可以达到以下有益效果:通过采用间充质干细胞结合外源性细胞因子共同制成干细胞制剂,使得间充质干细胞能够定向向内皮细胞分化,并通过间充质干细胞的旁分泌及外源性细胞因子能够刺激内皮细胞的生长,增加创面中成纤维细胞、毛细血管的生成以及胶原合成和肉芽的生长,并能够增加皮肤角化细胞及内皮细胞的增殖,从而能够实现快速有效修复因溃疡损伤的皮肤的目的。
具体实施方式
为解决现有技术中治疗皮肤溃疡存在治疗效果不佳,治疗周期长等缺陷,本发明的创新点在于提供一种含有间充质干细胞和外源性细胞因子的干细胞制剂,不仅能够提高皮肤溃疡的治疗效果,而且能够缩短修复皮肤的时间;另外,因干细胞制剂的作用时间较长,从而能够实现彻底修复皮肤溃疡的目的。
具体地,本发明提供的用于修复皮肤溃疡的干细胞制剂,包括间充质干细胞和细胞因子。
其中,间充质干细胞可来源于骨髓、脐带、胎盘等,来源广泛、扩增潜能较强,横向分化能力较强,以及可逃避免疫识别和抑制免疫反应等,其他干细胞无法比拟的优势细胞活力较好、扩增能力稳定、无免疫原性、无致瘤性等优点,更加适合移植治疗。
而间充质干细胞能够向内皮细胞和成纤维细胞分化,成纤维细胞是皮肤重建的重要细胞,内皮细胞在皮肤组织重建中可形成血管并参与组织重建,参与创伤愈合的各个阶段;同时,间充质干细胞还能够分泌对创伤愈合起关键作用的多种细胞因子和生长因子,这些因子通过不同的基质在局部的炎症反应、上皮再生成、肉芽组织形成、新生血管形成、细胞外基质的分泌中起着重要作用,具有治疗皮肤溃疡的潜在价值。
间充质干细胞具有强大的分泌潜能,能够分泌大量的细胞因子,包括趋化因子、细胞生长与营养类因子、细胞因子受体、血管生成因子、促炎症反应因子以及抗炎症反应因子;这些高分泌的细胞因子间相互作用相互关联,形成复杂的关系网络,其中包括了促炎因子IL-6(白介素-6)、IL-8(白介素-8)、TNF-α(TumorNecrosisFactor,肿瘤坏死因子-α),抑炎因子IL-10(白介素-10)、TNF-β(肿瘤坏死因子-β)、IL-13(白介素-13)、基质金属蛋白酶MMP1及抑制剂可能调控皮肤局部血管再生,改善皮肤血供,促进皮肤再生,从而能够修复皮肤。除此之外,间充质干细胞移植后能够通过促进皮肤细胞的增殖、抑制细胞凋亡,从而使皮肤能够再生。
间充质干细胞的获取过程包括以下步骤:

其中,基础培养基为DMEM/F12培养基。
进一步地,本发明中的细胞因子为外源性细胞因子,其中包括成纤维细胞生长因子、转化生长因子、表皮生长因子、纤维母细胞生长因子、血管内皮生长因子、血小板源生长因子、类胰岛素生长因子、P物质等。
其中,转化生长因子可分为转化生长因子-α和转化生长因子-β,转化生长因子-α在结构上与表皮生长因子相似,通过与表皮生长因子受体结合发挥作用,但需要转化生长因子-β的协同作用才能诱导细胞不依赖性生长。转化生长因子-β主要分布于表皮基底层、巨噬细胞、成纤维细胞、汗腺和毛囊上皮细胞等,其中,转化生长因子-β2在毛囊细胞内表达明显,转化生长因子-β3则在角质细胞和血管内皮细胞胞浆中表达明显。转化生长因子-β可以促进毛囊等皮肤辅助结构的形成,而转化生长因子-β3对角质细胞和内皮细胞的增殖具有强烈的抑制作用。与转化生长因子-β2和转化生长因子-β3相比,转化生长因子-β1与伤口修复愈合的关系最为密切。伤口处于炎症反应期时,细胞内信使分子Smad3的高表达一方面可能参与单核巨噬细胞的趋化反应,另一方面也反映了机体存在着抑制单核巨噬细胞过渡激活的反馈机制,从而防止愈合组织过度激活的反馈机制,从而防止愈合组织过度炎症反应的发生;而伤后10-14天正是大量合成胞外基质,肉芽组织向瘢痕组织过渡的阶段。因此,此间Smad2,Smad3,Smad4的强表达应与胶原基质的沉积密切相关。转化生长因子-β因子是最强的致纤维化因子,激活细胞内信号转导分子Smad3是其促进胶原合成的必由路径。由于转化生长因子-β1能够刺激成纤维细胞增殖,促进细胞外基质的大量合成和在胞外无规律沉淀,加速创伤部位纤维化,有助于皮肤修复。
表皮生长因子通过增加胶原生成细胞的数量或直接刺激组织修复细胞内DNA,RNA、蛋白质以及细胞外大分子物质等来实现加速皮肤组织修复的目的。表皮生长因子受体则主要定位于表皮细胞和内皮细胞的质膜上和胞浆内。表皮生长因子与表皮生长因子受体结合后,酪氨酸自动磷酸化,激活了酪氨酸蛋白激酶,细胞内蛋白磷酸化,导致表皮生长因子-表皮生长因子受体复合物向细胞内传递信息,使细胞进行分化增殖。
纤维母细胞生长因子通过激活特异性的酪氨酸激酶受体,促进细胞的分裂和增殖,使细胞的增殖速度增加,细胞生长周期明显缩短,能够使皮肤溃疡伤口快速修复。而纤维母细胞生长因子-7不仅能使加快表皮增生和血管的重建,而且能够提高新生表皮的抗菌性,同时纤维母细胞生长因子-7还能促进血管内皮生长因子等营养因子的分泌,最终促进皮肤溃疡的愈合。
碱性成纤维细胞生长因子,能够改善皮肤血液循环,使基底层细胞加速增生,以补充角化层细胞的脱落,减少湿性皮肤反应的发生率,同时亦可减轻湿性脱皮的炎症反应,促进上皮修复,缩短愈合时间。
血管内皮生长因子除了在上皮细胞表达外,在血小板、巨噬细胞及新生毛细血管内皮细胞内呈强阳性表达。血管内皮生长因子通过与存在于内皮细胞表面的血管内皮生长因子特异性受体结合来发挥作用。血管内皮生长因子可以通过上调尿激酶型纤溶酶原激活物和组织型纤溶酶原激活物以及纤溶酶原激活物抑制剂的表达,以及诱导胶原酶和蛋白水解酶的表达,满足血管新生时基质降解的要求,从而诱导血管生成。
血小板源生长因子能促进成纤维细胞和血管内皮细胞的增殖,导致胶原聚集、大量的毛细血管生成和肉芽的产生,对炎性细胞具有强烈的趋化作用;同时,还能够诱导创面血管平滑肌细胞的迁移和内皮细胞增殖、迁移以及血管腔的形成。
类胰岛素生长因子与成纤维细胞增殖、活跃程度密切相关,它与Ⅰ型胶原的分泌和创面愈合有着固有的内在联系,成纤维细胞产生的Ⅰ、Ⅲ型胶原是主要的间质胶原,也是皮肤结缔组织中的主要胶原类型;它不仅能填充组织缺损,同时还为表皮细胞迁移提供支架,所以创面中成纤维细胞是否增生活跃直接影响着创面的愈合时间。除此之外,类胰岛素生长因子能够促进创面新生血管生成和肉芽组织生长及再上皮话,进而使肉芽组织生长活跃、创面收缩与再上皮化。
神经肽P物质能够促进修复细胞DNA合成以及创面肌成纤维细胞的增生,其在体外能够诱导内皮细胞的增殖和迁移,在体内能够促进新生血管的形成,通过促进创伤后的炎症反应、修复细胞增殖和组织重建,作用于伤口愈合的各个环节。
细胞因子间的相互作用:转化生长因子-β主要作用于成纤维细胞,表皮生长因子主要作用于上皮细胞,碱性成纤维细胞生长因子主要作用于内皮细胞和成纤维细胞。急性创伤会抑制碱性成纤维细胞生长因子和表皮生长因子基因表达,从而导致创面愈合迟缓甚至形成溃疡。而通过激活细胞内α-平滑肌肌动蛋白表达,最终能够引起皮肤组织中肌成纤维细胞数量增多和创面愈合后瘢痕形成。碱性成纤维细胞生长因子导致转化生长因子-β1表达增强,随后加快成纤维细胞转化成肌成纤维细胞,使创面收缩加快,愈合时间缩短。
神经肽P物质与其他细胞因子的相互作用:神经肽P物质能够促进转化生长因子-β诱导的白细胞介素26和前列腺素E2的表达,通过它们之间的相互作用,神经肽P物质能明显增强星形胶质细胞的免疫调节和神经营养功能。神经肽P物质和表皮生长因子可协同刺激成纤维细胞的增殖。
综上所述,上述细胞因子能够刺激内皮细胞生长,增加创面中成纤维细胞及趋化中性粒细胞和单核细胞的生长,促进毛细血管生成、胶原合成增加及肉芽生长,并能够增加皮肤角化细胞的迁移和增殖,促进内皮细胞增殖,诱导新生血管形成,增加微小血管的通透性,为细胞生长和新生毛细血管网的建立提供营养,因此,与间充质干细胞共同配合能够促进皮肤快速修复。
进一步优选地,细胞因子来源于血小板血浆,一方面由于血小板裂解液中含有大量丰富的细胞因子,如:血小板衍生生长因子AB、转化生长因子β、表皮细胞生长因子、类胰岛素生长因子等;另一方面,由于血小板血浆存在以下优点:1、自源性;2、富含多种高浓度的生长因子;3、可与凝血酶作用,形成胶冻状,即富血小板凝胶;4、收缩创面,促凝血;5、防止感染;6、对患者损伤小及无致畸、致肿瘤等作用。
在本发明中,血小板血浆裂解液的制备过程,包括以下步骤:
无菌条件下,取骨髓,加入肝素钠抗凝,离心至骨髓液分为三层,取下层细胞层,加入标准培养基进行原代培养;取原代培养后的细胞悬液离心后吸取上层血浆和下层红细胞进行第二次离心,再次取上层血浆和下层红细胞进行第三次离心,弃上清液后,混匀,置于-50~-80℃保存,24~36小时后取出,水浴解冻,反复冻存解冻至少三次,即可获得血小板血浆裂解液。
其中,标准培养基为:含有体积分数为5~15%血清、浓度为2~15U/ml强力霉素以及1~5U/ml肝素的DMEM培养基。
血清中含有大量的氨基酸、维生素、无机物、脂类物质和核酸衍生物等,能够为细胞生长提供基本的营养物质,激素、各种生长因子、结合蛋白以及促接触和伸展因子使细胞贴壁免受机械损伤等;强力霉素抗菌谱广,抗菌作用强,且作用维持时间长,可用于防止细胞生长过程中,细菌滋生;而肝素用以起到抗凝的作用,防止细胞堆积,影响其细胞活性及正常生长。
进一步优选地,在干细胞制剂中,血小板血浆裂解液的体积分数为5~20%。
进一步地,本发明提供的用于修复皮肤溃疡的干细胞制剂还包括血清蛋白、柠檬酸钠、氯化钠和法舒地尔。其中,血清蛋白是血液中脂肪酸的携带者,脂肪酸是建造脂质的成分,而脂质又构成了细胞周围和细胞内所有的生物膜,且含有丰富的间充质干细胞细胞生长所必需的营养成份,优选地,血清蛋白的浓度为20~80mg/ml。柠檬酸钠用于起到抗凝作用,在本发明中优先采用PH值为5.0~6.0的柠檬酸钠调节注射剂。氯化钠用于维持细胞外液的渗透压,参与体内酸碱平衡的调节,氯化钠的浓度为0.1~0.5mol/L。法舒地尔为RHO激酶抑制物,通过增加肌球蛋白轻链磷酸酶的活性扩张血管,降低内皮细胞的张力,改善表皮细胞的微循环,同时可拮抗炎性因子,保护皮肤细胞抗凋亡,促进皮肤再生;法舒地尔的浓度为5~15μmol/L。
进一步地,该肝细胞制剂中还包括强力霉素,用于抑制细菌滋生。优选地,在干细胞制剂中,强力霉素的浓度为1~15U/mL。
为了使本发明的目的、技术方案及优点更加清楚明白,以下结合具体实施例,对本发明进行进一步详细说明。应当理解,此处所描述的具体实施例仅用以解释本发明,并不用于限定本发明。
实施例1
本发明提供的用于修复皮肤溃疡的干细胞制剂,包括以下成分;且细胞制剂中,各成分浓度如下:
间充质干细胞5×107个/ml;
血小板血浆裂解液15%;
人血清蛋白40mg/ml;
柠檬酸钠调节注射剂PH=5.5;
氯化钠0.2mol/L;
法舒地尔10μmol/L;
强力霉素8U/mL。
其中,间充质干细胞的来源过程包括以下步骤:
S1a、获取脐带;
取足月剖宫产胎儿的脐带约4~6cm长,在无菌条件下抽经脐带血,PBS中于4℃保存,并于4小时内进行处理。
S2a、分离纯化
将约4~6cm长的脐带放入超净台中,去除动静脉及外膜,PBS(其中,PBS溶液中包括0.2g的KH2PO4,0.2g的KCl,8.0g的NaCl,1.15g的Na2HPO4,定容至1L,PH=7.4)进行多次清洗,然后剪成1cm小段,再用PBS多次漂洗以尽可能去除血液,最后剪成肉糜状后,移入50mL离心管,加入5mL、经37℃预热的、质量分数为0.1%的胶原酶Ⅱ(即,100ml的PBS缓冲液溶解0.1g胶原酶获得的混合液),放入37℃水浴摇床中振荡消化180min。加入30mL的DMEM/F12,反复吹打,细胞筛过滤,收集滤液。滤液用1000转/min离心10min,弃上清,将离心沉淀下来的细胞按1×106/mL的密度用含10%FBS(胎牛血清)的DMEM/F12接种于培养瓶中,移入培养箱中进行培养。次日首次换液,以后每3d换液1次。
S3a、传代培养
当细胞达融合后,用胰蛋白酶(胰蛋白酶溶液配制:PBS缓冲液200mL,胰蛋白酶0.5g,EDTA(乙二胺四乙酸)0.04g)不完全消化进行传代纯化,至获得第三代间充质干细胞。
血小板血浆裂解液的制备过程包括以下步骤:
S1b、无菌条件下用含100U/ml低分子量肝素钠的无菌针管从髂后上脊取骨髓30ml,生物安全柜中转入50ml的无菌离心管中,在200g/min的条件下离心6min,离心后可见骨髓分为3层,吸取上层及中间富含有核细胞层至另一离心管,1000g离心6min,弃上清液;下层加入DMEM培养液(含体积分数为10%血清+8U/ml强力霉素+3U/ml肝素),于37℃,体积分数为5%CO2饱和湿度培养箱中原代培养。
S2b、取原代培养后的细胞悬液于200g条件下离心5分钟,吸取上层血浆和下层2~3mm红细胞层进行第二次离心200g,离心5分钟,取上层血浆和下层1mm红细胞层进行第三次离心800g,离心10分钟,至细胞液分三层,弃上层大部分血清,将剩余血清、白膜样物质层和下层细胞混匀后置于-80℃冰箱内保存,第二天取出,为防止蛋白质沉淀,在37℃恒温水浴中解冻时间不能超过5分钟,反复冻存解冻至少3次以上便可得到血小板血浆裂解液。
实施例2
本实施例提供的用于修复皮肤溃疡的干细胞制剂,包括以下成分,且各成分在细胞制剂中的浓度如下:
间充质干细胞2×107个/ml;
血小板血浆裂解液5%;
人血清蛋白20mg/ml;
柠檬酸钠调节注射剂PH=5.0;
氯化钠0.1mol/L;
法舒地尔5μmol/L;
强力霉素1U/mL。
其中,间充质干细胞和血小板血浆裂解液的来源过程与实施例1基本相同。
实施例3
本实施例提供的用于修复皮肤溃疡的干细胞制剂,包括以下成分,且各成分在细胞制剂中的浓度如下:
间充质干细胞8×107个/ml;
血小板血浆裂解液20%;
人血清蛋白80mg/ml;
柠檬酸钠调节注射剂PH=6.0;
氯化钠0.5mol/L;
法舒地尔10μmol/L;
强力霉素15U/mL。
其中,间充质干细胞和血小板血浆裂解液的来源过程与实施例1基本相同。
为进一步验证本发明提供的用于修复皮肤溃疡的干细胞制剂的显著效果,通过以下实验及实验数据进行具体说明。
实验动物分组和模型构建
SD免疫缺陷鼠,雌雄各半,体重约200~250g,随机分为对照组(注射未加入其他的单纯干细胞制剂)和治疗组1-3(分别注射对应本发明中实施例1-3)。通过化学方法灼伤建立皮肤损伤模型,分别隔离相同环境下饲养。以上各组治疗开始后,分别于3d、7d、14d观察伤口愈合情况,计算伤口愈合指数(WCI):WCI(%)=(1-现创面面积/原创面面积)×100%,结果如下表1:

实验结果:由表1数据可知,治疗组1~3皮肤伤口愈合面积远远大于对照组,且治疗组1~3的皮肤伤口愈合速度较快;由此可知,通过本发明提供的干细胞制剂能够提高皮肤伤口愈合的效率,能够快速实现修复皮肤溃疡的目的。
综上所述,本发明提供的用于修复皮肤溃疡的干细胞制剂,通过采用间充质干细胞结合外源性细胞因子共同制成干细胞制剂,使得间充质干细胞能够定向向内皮细胞分化,而外源性细胞因子能够刺激内皮细胞的生长,增加创面中成纤维细胞、毛细血管的生成以及胶原合成和肉芽的生长,并能够增加皮肤角化细胞及内皮细胞的增殖,从而能够实现快速修复因溃疡损伤的皮肤的目的。

3.根据权利要求1所述的用于修复皮肤溃疡的干细胞制剂,其特征在于,所述外源性细胞因子包括成纤维细胞生长因子、转化生长因子、表皮生长因子、纤维母细胞生长因子、血管内皮生长因子、血小板源生长因子、类胰岛素生长因子和P物质。

无菌条件下,取骨髓,加入肝素钠抗凝,离心至骨髓分层,取下层细胞层,加入标准培养基进行原代培养;取原代培养后的细胞悬液离心后吸取上层血浆和下层红细胞进行第二次离心,再次取上层血浆和下层红细胞进行第三次离心,弃上清液后,混匀,置于-50~-80℃保存,24~36小时后取出,水浴解冻,反复冻存解冻至少三次,即可获得血小板血浆裂解液。
6.根据权利要求5所述的用于修复皮肤溃疡的干细胞制剂,其特征在于,所述标准培养基为含有体积分数为5~15%血清、浓度为2~15U/ml的强力霉素以及1~5U/ml肝素的DMEM培养基。
7.根据权利要求6所述的用于修复皮肤溃疡的干细胞制剂,其特征在于,所述干细胞制剂中,所述血小板血浆裂解液的体积分数为5~20%。
8.根据权利要求1所述的用于修复皮肤溃疡的干细胞制剂,其特征在于,所述间充质干细胞的获取过程包括以下步骤:
获取脐带,于无菌条件下去除动静脉及外膜,PBS清洗,剪成段,再次清洗去除血液,然后处理成肉糜状,水浴消化后,加入基础培养基反复吹打,过滤,收集滤液离心,弃上清,将沉淀的细胞接种于完全培养基中进行培养,次日首次换液,之后每2~3天换液一次;待细胞融合,不完全消化后,进行传代纯化,获得第三代间充质干细胞完全消化后,继续进行传代培养。
9.根据权利要求1所述的用于修复皮肤溃疡的干细胞制剂,其特征在于,所述干细胞制剂还包括20~80mg/ml的血清蛋白、柠檬酸钠、0.1~0.5mol/L的氯化钠和5~15μmol/L法舒地尔。
10.根据权利要求1所述的用于修复皮肤溃疡的干细胞制剂,其特征在于,还包括1~15U/mL的强力霉素。
