Abstract

技术领域
本发明涉及一种评估肝癌预后的试剂盒,还涉及肝癌预后标记物(markerforprognosisoflivercancer);涉及用于评估肝癌预后的组合物,该组合物包含能够检测所述肝癌预后标记物的表达水平的变化的物质;涉及用于评估肝癌预后的试剂盒,该试剂盒包含所述用于评估肝癌预后的组合物;涉及使用所述肝癌预后标记物进行肝癌预后评估的方法;以及涉及使用所述肝癌预后标记物来筛选肝癌的治疗剂的方法。
背景技术
癌症与恶性肿瘤有相同的意义。它是指这样一种疾病,由于各种原因,调节细胞增殖的功能受到损害,因此异常的细胞不受控制并过度增殖从而穿透至周围的组织和器官,形成块状物并破坏正常的组织。由于癌症的快速生长、渗透性的(穿透性的或扩散的)生长、扩散(从其发生区域移动很远)等原因,源于人体内各种组织的癌症会给人的生命带来危险。
在癌症当中,已知肝癌是世界上最致命的癌症之一。尤其是,据报导称,在亚洲和次撒哈拉的非洲区,每年至少约有50万人死于肝癌。肝癌主要分为起源于肝细胞自身的肝细胞癌和癌症由其他组织扩散至肝脏的转移性肝癌。大约至少90%的肝癌为肝细胞癌,通常理解术语肝癌是指肝细胞癌。
据报导大多数肝癌的起因是受到乙型肝炎病毒或丙型肝炎病毒的急性或慢性感染。然而,有关肝癌发生和发展的细胞内的分子机制还没有被阐明。根据传统研究,据报导称在诸如各种生长因子基因等原癌基因由于各种原因突变成癌基因而过度表达或过度激活的情况下,或者在诸如Rb蛋白或p53蛋白之类的肿瘤抑制基因由于各种原因突变而低表达或丧失功能的情况下,可以引起包括肝癌在内的多种癌症的发生和发展。尤其是,关于肝癌,已经证实例如经修饰的p53、β-连环蛋白(beta-catenin)、AXINI、p21(WAF1/CIP1)和p27Kip等基因为与其相关。但是,近来认识到,包括肝癌在内的大多数癌症的发生和发展并不由于单独的特定基因,而是由于与细胞周期、信号传递等有关的多种基因的复杂相互作用。因而,有必要从只集中在个别的基因或蛋白的表达或功能中脱离出来,而进行对各种基因或蛋白的整体研究。
肝癌的预后是指预见患有肝癌的患者的各种情况,例如从肝癌中全面康复的可能性,经治疗后复发的可能性,肝癌诊断后患者生存的可能性等等。这可以根据疾病的严重性、诊断时间、治疗进展等各种情况而变化。只有根据其预后适当地应用各种治疗方法时,肝癌才能够得到有效的治疗。例如,对于被评估为有良好的预后的患者,有必要避免可能对患者引起严重副作用的危险的治疗方法,例如激进的化疗或手术、放疗等,而选择那些相对温和、保守和安全的治疗方法。另一方面,对于被评估为有较差的预后的患者,应当积极地进行化疗或手术,例如放疗等治疗方法以力图增加生存期或生存率。
根据迄今为止进行的研究,已经发展的肝癌的预后非常糟糕,显示了自诊断起6个月内死亡的高致死率,剩余仅四个月的平均寿命长度。但是,大小小于3厘米的肝癌有良好的预后,且已知无需任何特别治疗,一年的生存率为90%,且手术后5年的生存率为约40~50%。然而,根据现有技术很难精确地评估肝癌患者的预后。为了精确评估预后,需要一种把患者划分为各风险组群的分析方法。然而,迄今为止,仅仅依靠在诊断和初次手术治疗时的临床病理肝癌阶段来确定预后,而没有一种用于精确评估肝癌的方法。但是,不幸的是,仅仅通过肝癌阶段无法精确确定每一位肝癌患者的预后。
CBS蛋白(胱硫醚β-合酶)是一种将同型半胱氨酸转变成胱硫醚的酶,已知其在转硫(transsulfuration)通路中起作用,并在体内蛋氨酸代谢中发挥重要作用[JBiolChem.1994May20;269(20):14835-40]。
NNMT蛋白(烟酰胺N甲基转移酶)是一种进行烟酰胺和吡啶的N-甲基化的酶,已知其与各种药物和异生物的代谢有关[Genomics.1998Sep15;52(3):312-24]。
TKT蛋白(转酮醇酶)是一种在非氧化的磷酸戊糖途径中发挥核心作用的酶,作为该族的成员,已知有TKTL1(转酮醇样酶1)、TKTL2(转酮醇样酶2)[JBiolChem.1993Jan15;268(2):1397-404]。
AIFM1蛋白(线粒体相关的凋亡诱导因子1)是也称为AIF的黄素蛋白。已知若发生细胞凋亡,则该蛋白移动至细胞核,引起染色体的聚集和断裂,并诱导来自线粒体的细胞色素c和半胱天冬酶9的分泌[Nature.1999Feb4;397(6718):441-6]。
AKT蛋白是与AKT2、AKT3有关的蛋白,并且已知其在通过磷脂酰肌醇3激酶生长因子信号传递(即,血小板源生长因子(PDGF)、表皮生长因子(EGF)、胰岛素和胰岛素样生长因子I(IGF-I)等的信号传递)中起作用,当其被激活时,已知能够磷酸化凋亡相关蛋白并使它们失活[ProcNatlAcadSciUSA.1987Jul;84(14):5034-7]。
ATG3蛋白(自噬相关3同源物)参与了诸如GATE-16、GABARAP、MAP-LC3等人ATG8同源物与脂类之间的结合。有报道称缺乏ATG3的小鼠不形成自噬小体,并在出生后一天内死亡[JBiolChem.2002Apr19;277(16):13739-44.Epub2002Feb1]。
ATG5蛋白(自噬相关5同源物)与自噬作用有关。ATG5已知能够通过ATG7和ATG10的作用形成ATG12-ATG5缀合物,并且该ATG12-ATG5缀合物移动到自噬小体膜处以在ATG8和磷脂酰乙醇胺的结合中起作用。有报道称缺乏ATG5的小鼠不形成自噬小体,并在出生后立即死亡,且截断的ATG5能够诱导细胞色素c的分泌和激活线粒体内的半胱天冬酶9[Nature432:1032-1036(2004)]。
ATG7蛋白(自噬相关7同源物)与自噬作用有关。有报导称ATG7与ATG10一起形成ATG12-ATG5缀合物,而ATG7与ATG3一起参与ATG8和磷脂酰乙醇胺的结合。缺乏ATG7的小鼠不形成自噬小体,并在出生后立即死亡[JBiolChem.2001Jan19;276(3):1701-6.Epub2000Nov28]。
ATG12蛋白(自噬相关12同源物)与自噬作用有关。已知ATG12通过ATG7和ATG10的作用形成ATG12-ATG5缀合物,并且该ATG12-ATG5缀合物移动到自噬小体膜处以在ATG8和磷脂酰乙醇胺的结合中起作用[JBiolChem.1998Dec18;273(51):33889-92]。
BAX蛋白(BCL2相关X蛋白)BCL2蛋白族的一员,已知其通过与BCL2的结合促进细胞凋亡,并且与线粒体膜电位的损失和细胞色素c的分泌有关[Cell.1993Aug27;74(4):609-19]。
BCL2蛋白(B细胞淋巴瘤蛋白2)是一种线粒体的膜蛋白,能够抑制细胞凋亡。已知其抑制细胞色素c从线粒体的分泌,激活半胱天冬酶并通过与凋亡激活因子的结合抑制细胞凋亡[ProcNatlAcadSciUSA.1986Jul;83(14):5214-8]。
BCL2L1蛋白(Bcl-2样1蛋白)是BCL2蛋白组的一员,已知其位于线粒体的外膜并与线粒体膜通道的分配有关。它以两种亚型存在。较长形式的BCL-XL已知能够抑制细胞凋亡,而较短形式的BCL-XS能够促进细胞凋亡[Cell.1993Aug27;74(4):597-608]。
BNIP3蛋白(BCL2/腺病毒E1B19kD-相互作用蛋白3)能够与E1B19kDa蛋白和BCL2结合,且已知可以诱导细胞凋亡和自噬作用[JExpMed.1997Dec15;186(12):1975-83]。
CASP8蛋白(半胱天冬酶8,凋亡相关的半胱天冬酶)是属于半胱天冬酶家族的酶,它是通过FAS在细胞凋亡机制初始阶段起作用的酶。如果其通过与FADD的结合和蛋白水解性切割而激活,它激活各种其他的半胱天冬酶[ProcNatlAcadSciUSA.1996Dec10;93(25):14486-91;BiochemJ.1997Aug15;326(Pt1):1-16]。
CSE1L蛋白(CSE1染色体分离1样)已知在将importin-α再次从细胞核发送至细胞质中起作用,并与细胞凋亡或细胞增殖有关[ProcNatlAcadSciUSA.1995Oct24;92(22):10427-31]。
DIABLO蛋白(具有低等电点的直接的IAP结合蛋白)是一种位于线粒体内的蛋白,且如果发生细胞凋亡,则它移动至细胞质与IAP(凋亡抑制物蛋白)结合以帮助半胱天冬酶的激活[Cell.2000Jul7;102(1):43-53]。
DRAM蛋白(损伤调节自噬调控蛋白)是一种溶酶体的膜蛋白,已知其与p53肿瘤抑制子控制的自噬作用有关并且在p53控制的细胞凋亡中是必须的[Cell.2006Jul14;126(1):121-34]。
E2F1蛋白(E2F转录因子1)是E2F家族转录因子的一员,已知其与细胞周期和肿瘤抑制子的控制密切相关,并且通过与Rb蛋白的结合促进细胞生长或诱导与p53相关的细胞凋亡[Gene.1996Sep16;173(2):163-9]。
FAS蛋白(APO-1,CD95,TNFRSF6)是TNF受体家族的一员。已知其接受FAS配体的信号并与FADD(Fas相关死亡结构域蛋白)、半胱天冬酶8以及半胱天冬酶10一起组成DISC(死亡诱导信号复合物)来诱导细胞凋亡[JBiolChem.1992May25;267(15):10709-15]。
FRAP1蛋白(哺乳动物雷帕霉素靶蛋白)也称为mTOR。已知其调节诸如细胞内DNA损伤、营养耗尽等与压力有关的反应,且包含FRAP1的TORC2复合物已知是细胞周期阻滞和FKBP12-雷帕霉素复合物的抑制免疫作用的靶点[Nature.1994Jun30;369(6483):756-8]。
LAMP1蛋白(溶酶体相关膜蛋白1)也称为CD107a抗原。它是一种膜糖蛋白,且似乎与癌细胞的扩散有关[JBiolChem.1990May5;265(13):7548-51]。
LC3蛋白(MAP1轻链3样蛋白1)是微管结合蛋白,它与微管与细胞骨架间的相互作用有关。LC3蛋白是酵母ATG8的同源物,已知其能够通过多种自噬蛋白的作用与磷脂酰乙醇胺结合并组成自噬小体[JBiolChem.1994Apr15;269(15):11492-7]。
PRKAA1蛋白(AMP激活的蛋白激酶,催化性,α-1)是AMPK的催化亚基。该AMPK是起到检测细胞内能量水平的作用的蛋白。已知它在当AMP/ATP比例增高时激活以限制各种生物合成反应[FEBSLett.1994Dec12;356(1):117-21]。
PTEN蛋白(磷酸酶和张力蛋白同源物)已知是一种肿瘤抑制子,在多种类型的癌症中失活。它既是一种蛋白磷酸酶同时也是一种磷脂酰肌醇-3,4,5-三磷酸3磷酸酶,已知其能够破坏PI3K-AKT/PKB信号传递[NatGenet.1997Apr;15(4):356-62]。
ULK1蛋白(Unc-51样激酶1)是一种与轴突生长有关的丝氨酸/苏氨酸激酶,也称为ATG1同源物。对于自噬作用而言,已知其能够被mTOR磷酸化[Genomics.1998Jul1;51(1):76-85]。
XIAP蛋白(X连锁凋亡蛋白抑制子)是IAP家族的一员,且在IAP中,它有强的细胞凋亡抑制效果。已知其通过与肿瘤坏死因子受体关联因子TRAF1、TRAF2的结合抑制细胞凋亡并抑制半胱天冬酶的活性[Nature.1996Jan25;379(6563):349-53]。
但是,还不清楚这些蛋白表达水平和表达模式在肝脏组织中如何具体变化,以及如何使用这些蛋白评估肝癌预后。而且,迄今为止还没有使用蛋白或编码蛋白的基因作为用于肝癌预后标记物的实例。
发明内容
技术课题
本发明的一个目的是提供一种有关肝癌预后的生物标记物,以容易且精确地评估肝癌患者的预后,和还提供一个开发用于肝癌的治疗剂的重要的起点。因而,本发明提供了一种肝癌预后标记物;一种用于评估肝癌预后的组合物,该组合物包含能够检测所述肝癌预后标记物的表达水平的变化的物质;一种用于评估肝癌预后的试剂盒,所述试剂盒包含所述用于评估肝癌预后的组合物;一种使用所述肝癌预后标记物进行肝癌预后评估的方法;以及一种使用所述肝癌预后标记物来筛选肝癌的治疗剂的方法。
技术方案
本发明比较取自多位诊断为患有肝癌的患者的肝癌组织中的基因表达程度,并根据每位患者的进展检测特异性大量或小量表达的基因,从而发现能够用于评估肝癌预后的肝癌预后标记物。

肝癌的预后可以从多个方面来评估。然而,典型地从复发的可能性、生存的可能性以及无病生存的可能性等方面来确定。本说明书中所描述的研究通过下述方式进行:从多位肝癌患者的肝癌组织中获取基因表达谱,总体考虑复发的可能性、生存的可能性以及无病生存的可能性等方面以通过进行统计数据处理过程来检测在表达水平显著差异的基因,并发现能够评估肝癌预后的标记物。
在本说明书中,“肝癌预后标记物”是指一种基因标记物,它是用于评估肝癌发生后患者具有好的或差的预后的标准。本发明中,在有着良好的预后的患者的肝癌组织内该标记物的表达水平与通过实验预先确定的标准水平相比显著的高或低。尤其是,在本发明中,该标记物具有显著低的p值,该值是基于有着良好的预后和较差的预后的患者的肝癌组织中的表达差异计算的。优选,p值小于0.05。
对于多位已知患有肝癌的患者的肝癌组织,本发明通过将有着良好的或较差的治疗进展的患者的肝癌组织的基因表达谱与标准水平比较而进行分析。因此,如此确定的标记物能够特异性区别有着良好的肝癌预后或较差的肝癌预后的患者,因而其能够有效地用于肝癌预后的评估。而且,考虑到如此确定的标记物在有着特定预后的患者中的肝癌组织中特异性大量表达或小量表达,该标记物的生理学功能有可能直接涉及有关肝癌的发生和进展的生理学功能,尤其是肝癌的预后。因此,该标记物能够有效地用做肝癌的发生和进展的研究中的靶标或用于肝癌的治疗剂的开发。
特别地,在本发明的第一个方面中的肝癌预后标记物是基于空前的多位患者的肝癌组织的统计分析而发现的。因而,与基于仅仅一些患者的肝癌组织的基因表达谱而发现的关于肝癌的常规标记物相比,所述标记物有着较大的显著性,具有较高的临床利用价值,且具有用于评估肝癌预后的更精确的评估力。
本发明的第二个方面涉及用于评估肝癌预后的组合物,所述组合物包含有能够特异性检测第一方面中所述的肝癌预后标记物的表达水平和/或表达模式的物质。
这里,可以通过确定由相应基因转录产生的mRNA的水平或模式的一般生化分析方法来检测每种肝癌预后标记物的表达水平或表达模式。像这样的用于确定mRNA的水平或模式的分析方法有RT-PCR、竞争性RT-PCR、实时RT-PCR、RNA酶保护实验、Northern印记、DNA微阵列等等。此外,可以使用在相关领域中通常使用的方法。
通过上述的分析方法,能够将肝癌患者的生物学样本中的mRNA的水平或模式与标准水平进行比较,并从中检测本发明的肝癌预后标记物的表达水平和/或表达模式的差异。这使得能够评估肝癌患者的预后。在本说明书中,“生物学样本”是指能够检测到所述肝癌预后标记物的表达水平或表达模式,或者由所述肝癌预后标记物所编码的蛋白的存在水平或存在模式的分离自人体内细胞、组织等等。其可以例如是尿、血液、血浆、血清等,但不具体仅限于这些。
用于通过RT-PCR测定mRNA的水平或模式的试剂盒包含有本发明的所述肝癌预后标记物的mRNA的特异引物。在本说明书中,“引物”是指能够互补地与模板结合并使逆转录酶或DNA聚合酶能够启动模板复制的具有自由3’羟基的核酸序列。引物是具有与特定基因核酸序列互补的序列的核苷酸,可以使用长度约7bp~50bp、优选约10bp~30bp的引物。其他的RT-PCR试剂盒根据具体的实施方式可以包括试管或其他适合的容器、反应缓冲液、脱氧核苷酸(dNTP)、酶如Taq聚合酶和逆转录酶、DNA酶、RNA酶抑制剂、DEPC-水、无菌水等。在合适的缓冲溶液中和温度下,引物能够在聚合试剂(即DNA聚合酶或逆转录酶)和四种不同的三磷酸核苷以及温度存在的情况下启动DNA的合成。所述引物可以包含不改变在DNA合成起始点起作用的引物的基本性质的其他碱基序列。可以使用公知的方法化学合成引物,且可以使用相关领域中公知的多种方法来将核酸序列转化。
本发明的第二个方面中的“能够特异性检测肝癌预后标记物的表达水平和/或表达模式的物质”可以是在相应的肝癌预后标记物的表达水平或表达模式的分析方法中特异性用于相应标记物的任何物质,或者是所述物质中至少两种的组合,并不限于用于RT-PCR的引物。本发明的第二个方面的技术特性在于将被试肝癌患者的肝癌组织中的所述肝癌预后标记物的表达水平或表达模式与标准水平比较,并检测差异。因而,任何能够检测这样差异的物质可以被用作“能够特异性检测肝癌预后标记物的表达水平和/或表达模式的物质”,并实现本发明的目标技术效果。而且,本领域的普通技术人员根据具体的实施方式参照相关领域的公知常识能够选择并使用合适的物质。
本发明的第三个方面涉及到用于评估肝癌预后的组合物,所述组合物包含有能够特异性检测第一方面中的所述肝癌预后标记物所编码的蛋白的存在水平和/或存在模式的物质。
关于本发明的所述肝癌预后标记物的表达水平或表达模式,除了本发明的第二个方面中的检测根据每一种标记物的mRNA的水平或模式的方法之外,还可以使用用于检测样本中每一种标记物所编码的蛋白的存在水平或存在模式以估计每一种标记物的表达水平或表达模式的方法。
本发明中所述肝癌预后标记物所编码的蛋白的存在水平或存在模式的特异性检测是指确定生物学样本中存在多少本发明中所述肝癌预后标记物所编码的蛋白和其以何种模式存在的方法。例如,与所述肝癌预后标记物编码的蛋白特异结合的抗体能够用于确定存在水平或存在模式的过程。本说明书中,“抗体”是指能够与抗原的表位特异结合的蛋白,它是一个包含有多克隆抗体、单克隆抗体以及重组抗体的概念。
对于使用抗体测定蛋白的存在水平或存在模式的方法,有蛋白质印迹、ELISA(酶联免疫吸附测定)、放射免疫测定、放射免疫扩散、Ouchterlony免疫扩散、火箭免疫电泳(rocketimmunoelectrophoresis)、组织免疫染色、免疫沉淀测定、补体结合试验、FACS(荧光活化细胞分类)、蛋白芯片等。但是,除上述以外,可以使用在相关领域中普遍采用的任何方法。
通过上述的分析方法,可以将被试肝癌患者的生物学样本中的抗原抗体复合物的形成水平或形成模式与标准水平比较,且能够从中确定本发明的所述肝癌预后标记物所编码的蛋白的存在水平或存在模式,并最终能够评估肝癌患者的预后。
这里,“抗原抗体复合物”是指所述肝癌预后标记物所编码的蛋白和它的特异抗体的复合物。一般通过检测与二抗偶联的检测标记信号的大小和模式来测定抗原抗体复合物的形成水平或形成模式。这样的检测标记例如可以是酶、荧光物质、配体、发光物质、纳米粒子、氧化还原分子、放射性同位素等等,但并不限于这些。当使用酶作为检测标记时,作为能够使用的酶有β-葡糖醛酸糖苷酶(β-gluculonidase)、β-D-葡萄糖苷酶、β-D-半乳糖苷酶、尿素酶、过氧化物酶或碱性磷酸酶、乙酰胆碱酯酶、葡萄糖氧化酶、己糖激酶和GDP酶、核糖核酸酶、葡萄糖氧化酶、荧光素酶、磷酸果糖激酶、磷酸烯醇式丙酮酸羧化酶、天冬氨酸转氨酶、磷酸烯醇式丙酮酸脱羧酶、β-内酰胺酶等等,但所述酶并不限于这些。当使用荧光物质作为检测标记时,作为能够使用的荧光物质有荧光素(fluorescein)、异硫氰酸盐、罗丹明、藻红素、藻青素、别藻蓝素、邻苯二甲醛、荧光胺等,但所述荧光物质并不限于这些。当使用配体作为检测标记时,作为能够使用的配体有生物素衍生物等等,但所述配体并不限于这些。当使用发光物质作为检测标记时,作为能够使用的发光物质有吖啶酯、发光素(luciferin)、发光素酶(luciferase)等,但所述发光物质并不限于这些。当使用纳米粒子作为检测标记时,作为能够使用的纳米粒子有胶体金、着色胶乳(tintedlatex)等等,但所述纳米粒子并不限于这些。当使用氧化还原分子作为检测标记时,作为能够使用的氧化还原分子有二茂铁、钌络合物、紫罗碱(viologen)、醌、Ti离子、Cs离子、二酰亚胺、1,4-苯醌、氢醌、K4W(CN)8、[Os(bpy)3]2+、[RU(bpy)3]2+、[MO(CN)8]4-等,但所述氧化还原分子并不限于这些。当使用放射性同位素作为检测标记时,作为能够使用的放射性同位素有3H、14C、32P、35S、36Cl、51Cr、57Co、58Co、59Fe、90Y、125I、131Ior186Re等等,但所述放射性同位素并不限于这些。
在本发明的第三个方面中,“能够特异性检测所述肝癌预后标记物所编码的蛋白的存在水平和/或存在模式的物质”可以是在检测所述肝癌预后标记物所编码的蛋白的存在水平和/或存在模式的物质的分析方法中特异性用于所述标记物所编码蛋白的任何物质,并不一定限于抗体。本发明的第三个方面的技术特征在于将取自肝癌患者的生物样品中的所述肝癌预后标记物所编码的蛋白的存在水平或存在模式与基准比较,并检测差异。因而,任何的物质均可以被用作“能够特异性检测肝癌预后标记物所编码的蛋白的存在水平和/或存在模式的物质”并能实现本发明旨在达到的技术效果,只要该物质能够检测到这样差异即可。本领域的普通技术人员基于本领域的一般常识和已知技术,根据具体实施方式能够容易选择/拣选合适的物质。
本发明的第四个方面涉及预测肝癌预后的试剂盒,所述试剂盒包含有本发明第二或第三方面所述的用于预测肝癌预后的组合物。
除了包含在用于评估肝癌预后的组合物中的能够特异性检测肝癌预后标记物的表达水平和/或表达模式的物质,或能够特异性检测肝癌预后标记物所编码的蛋白的存在水平和/或存在模式的物质,本发明中用于评估肝癌预后的试剂盒还可以包括适用于分析基因的表达水平或表达方式的方法或分析蛋白的存在水平或存在模式的方法的一种或多种其他成份、溶液或装置。例如,对于检测基因的表达水平或表达模式的诊断试剂盒,该诊断试剂盒可以包括进行RT-PCR所需的基本成份,除了标记物基因的mRNA各自特异性的引物外,该RT-PCR试剂盒根据具体的实施方式可以包括例如试管或其他适合的容器、反应缓冲液、脱氧核苷酸(dNTPs)、酶例如Taq聚合酶和逆转录酶、DNA酶、RNA酶抑制剂、DEPC-水(DEPC水)、无菌水、用作定量的对照组的基因特异性引物对等。同时,在诊断试剂盒用于检测蛋白的存在水平或存在模式的情况下,该诊断试剂盒可以包括例如进行ELISA所需的基本成份。该ELISA试剂盒可以包括能够检测结合抗体的组分,例如,标记的二抗、生色团、酶(例如与抗体连接的酶)及其底物,和对定量对照组蛋白特异的抗体。进一步地,根据具体的实施方式,该诊断试剂盒可以包括DNA微阵列或蛋白微阵列。

例如,对在肝癌预后良好的患者的肝癌组织中表达更多的肝癌预后标记物进行表达水平差异检测时,如果来自被试肝癌患者的生物样本中该标记物的表达水平高于基准,可以得知肝癌的预后相对较好。同时,例如对在肝癌预后差的患者的肝癌组织中表达更多的肝癌预后标记物进行表达水平差异检测时,如果来自被试肝癌患者的生物样本中该标记物的表达水平高于基准,可以得知肝癌的预后相对较差。
同时,例如,对在肝癌预后良好的患者的肝癌组织中表达更多的肝癌预后标记物所编码的蛋白进行存在水平差异检测时,如果来自被试肝癌患者的生物样本中该标记物所编码的蛋白的存在水平高于基准,可以得知肝癌的预后相对较好。同时,例如在对肝癌预后较差的患者的肝癌组织中表达更多的肝癌预后标记物所编码的蛋白进行存在水平差异检测时,如果来自被试肝癌患者的生物样本中该标记物所编码的蛋白的存在水平高于基准,可以得知肝癌的预后相对较差。
本发明的第六个方面涉及使用本发明的第一个方面的肝癌预后标记物来筛选肝癌治疗剂的方法。由于本发明的第一个方面的肝癌预后标记物根据肝癌患者的预后而显示出表达模式的显著差异,它可以是直接参与和肝癌的发生或发展特别是预后有关的生理学功能的基因。因此,该标记物所编码的蛋白能够有效地用做研究肝癌的发生或发展机制或发明肝癌治疗剂的目标蛋白。即,本发明第一个方面所述肝癌预后标记物符合开发肝癌的治疗剂的重要前提,并因此该使用该标记物来筛选肝癌治疗剂的方法也涵盖在本发明的保护范围内。
作为本发明的第六个方面的一个实例,提供了筛选肝癌的治疗剂的方法,所述方法包括检测测试化合物是否能促进或抑制本发明的第一个方面的肝癌预后标记物的表达的步骤。作为筛选治疗剂的方法,可以使用例如RT-PCR、竞争性RT-PCR、实时RT-PCR、RNA酶保护实验、northern印记、DNA微阵列、SAGE[基因表达系列分析;Velculescu等,Science270:484-7]、MPSS[大规模平行标记测序;Brenner等,PNAS.USA.97,1665-1670]等。除了以上方法外,如有必要可以使用本领域的各种已知方法。
同时,作为本发明的第六个方面的另一个实例,提供了筛选肝癌的治疗剂的方法,所述方法包括:步骤1,将测试化合物与本发明的第一个方面的肝癌预后标记物所编码的蛋白结合;和步骤2,检测该测试化合物能否促进或抑制所述蛋白的生理学活性。作为筛选治疗剂的方法,例如,可以使用将本发明的第一个方面的用于肝癌早期诊断的蛋白标记物固定到亲和柱上并将其与样本接触来纯化样品的方法[Pandya等,VirusRes87:135-143,2002],使用酵母双杂交系统、蛋白质印迹的方法,高通量筛选的方法[Aviezer等,JBiomolScreen6:171-7,2001]等。除了以上方法外,如有必要可以使用本领域的各种已知方法。
在本发明的第六个方面中,作为用于筛选的测试化合物,可以使用例如组织提取物、基因文库的表达产物、合成化合物、合成多肽、天然化合物等,但是用于筛选的测试化合物并不限于这些。
本发明第七个方面涉及特异性识别由本发明第一个方面的肝癌预后标记物编码的蛋白的抗体。
特异性识别本发明的第一个方面的肝癌预后标记物的抗体是特异性检测肝癌预后标记物所编码的蛋白的存在水平或存在模式的代表性物质,因而能够有效地用于评估肝癌预后。此外,根据情况,所述抗体可以特异性促进或抑制在肝癌的发生或发展中起重要作用的蛋白的活性,因而能够用作用于肝癌的治疗剂。
由于已经找到本发明的第一个方面的肝癌预后标记物,通过使用本领域广泛知晓的技术能够容易地进行所述肝癌预后标记物所编码的蛋白的多克隆抗体、单克隆抗体以及重组抗体的制备。
通过本领域广泛知晓的将本发明的第一个方面的肝癌预后标记物所编码的蛋白注射进动物体内,并从动物身上采血以获得包含抗体的血清的方法,能够制备多克隆抗体。这些多克隆抗体能够从各种动物宿主中制备,例如山羊、兔、绵羊、猴、马、猪、牛、狗等,且制备方法是本领域公知的。
单克隆抗体可以通过使用杂交瘤方法[Kohler和Milstein(1976)EuropeanJounralofImmunology6:511-519]或噬菌体抗体文库技术[Clackson等,Nature,352:624-628,1991;Marks等,J.Mol.Biol.,222:58,1-597,1991]等本领域广泛知晓的方法来制备。通常,杂交瘤方法使用从免疫学上适合的宿主动物例如小鼠获得的细胞和作为另一群体的癌症或骨髓瘤细胞系,所述动物已注入本发明的第一个方面的肝癌预后标记物所编码的蛋白抗原。从两个群体中所获得的组织通过如聚乙二醇等本领域广泛知晓的方法来融合,然后以标准组织培养方法来使抗体生成细胞增殖。根据有限稀释技术通过亚克隆获得同质细胞群后,按照已知技术在体内或体外大量地培养能够产生所需抗体的杂交瘤。噬菌体抗体文库方法是这样一种制备单克隆抗体的方法:获得所需抗体的基因,在噬菌体表面上以融合蛋白的形式表达该基因,从而在体外产生抗体文库,并从该文库中分离单克隆抗体。通过以上方法制备的单克隆抗体可以通过使用例如凝胶电泳、透析、盐沉淀、离子交换层析、亲和层析等已知方法来分离。
本发明第七个方面的抗体除了包含两条全长轻链和两条全长重链的完美形状之外,还包含抗体分子的功能片段。所述抗体分子的功能片段是指具有抗原结合功能的片段,包括Fab、F(ab')、F(ab')2、Fv等等。
发明效果
根据本发明,提供了一种肝癌预后标记物;一种用于评估肝癌预后的组合物,该组合物包含能够检测到所述肝癌预后标记物的表达水平的变化的物质;一种用于评估肝癌预后的试剂盒,所述试剂盒包含所述用于评估肝癌预后的组合物;一种使用所述肝癌预后标记物进行肝癌预后评估的方法;以及一种使用所述肝癌预后标记物筛选肝癌治疗剂的方法。
所述肝癌预后标记物能够有效地用于肝癌患者中的简单且正确的预后评估。此外,所述标记物的生理学功能可直接与肝癌的发生或发展有关。因而,该标记物能够有效用于肝癌的发生或发展机制的研究,或用作开发肝癌治疗剂的靶标。
上述肝癌预后标记物是那些更显著的、临床高度有用的、且能够更准确地预测肝癌预后评估的标记物,这是因为它们是通过对取自空前的多位患者的肝癌组织进行统计学分析而发现的。

具体实施方式
下面会通过实施方式详细阐述本发明;然而,描述所述实施方式仅仅是为了帮助理解本发明,而不是以任何方式限制本发明的范围。
实施例
实施例1:RNA的提取和cDNA的合成
获得取自120位已诊断为肝癌已经发生并且肝癌的发展已经得到确认的肝癌患者的肝癌组织和相邻的正常组织。根据下面的方法提取各份组织的RNA并进行cDNA的合成。
根据产品说明书,使用RNeasyMinikit(Qiagen,德国)从肝癌组织和相邻的正常组织中提取总RNA。使用Bioanalyzer2100(AgilentTechnologies,美国)对获得的RNA提取物中的总RNA称量。在提取的步骤中进行DNaseI的处理以从RNA提取物中除去污染的基因组DNA。含有4μg总RNA的样品与2μl的1μM的d(T)18引物(Genotech,Korea)在70℃温育7分钟并在冰上冷却5分钟。单独制备酶混合物[通过添加2μl0.1M的DTT(Duchefa,Netherlands)、2μl的10×逆转录缓冲液、5μl2mM的dNTP、1μl200U/μl的MMLV逆转录酶和1μl的40U/μlRNA酶抑制剂(Enzynomics,Korea)至总体积为11μl]。将该酶混合物添加至含有RNA的样品中后,将其在42℃温育90分钟,然后在80℃温育10分钟来使逆转录酶失活。通过添加焦碳酸二乙酯(DEPC)处理的水使上述样品的终体积为400μl。
实施例2:定量实时RCR
根据产品手册,使用PRISM7900HT(AppliedBiosystems,美国)对实施例1中获得的每一个cDNA样品的下面两个基因标记物实施定量实时RCR扩增:
CBS(胱硫醚β-合酶;NCBIGI:209862802;SEQIDNO:79);和
NNMT(烟酰胺N-甲基转移酶;NCBIGI:62953139;SEQIDNO:80)。
所述定量实时RCR分析在10μl的总体积中进行,所述总体积包括5μl的2×Taqman基因表达主引物(mastermix)(AppliedBiosystems,USA)、5μM正向和反向引物各1μl、1μl的1μM探针(Genotech,Korea)和2μl的cDNA(在对照组中为等量的水)。扩增以下述循环进行:95℃解离10分钟的步骤,然后是95℃解离15秒的步骤,和60℃合成1分钟的步骤。引物和探针序列使用PrimerExpress3.0(AppliedBiosystems,美国)设计,且所有探针序列在5'端标有FAM,3'端标有TAMRA。下面表1中的下列引物和探针是用于每种标记物的。

每种标记物基因表达的测定都重复三次,然后通过与5种参照基因(B2M、GAPDH、HMBS、HPRT1和SDHA)的平均表达相减来标准化。测定每种标记物的CT(达到阈值所需的循环数),并计算ΔCT值(每种标记物的CT减去所述参照基因的平均CT)。按照2-ΔCT来计算mRNA的拷贝数。根据一系列稀释后的cDNA样品的同时扩增结果来构建标准曲线。
实施例3:数据分析
考虑到实施例2中获得的每种标记物的标准化的表达和提供肝癌组织的患者的进展,完成了Kaplan-Meier曲线,随后进行显著性分析。
根据120位提供肝癌组织的患者的进展,对于复发、总生存以及无病生存的各自的病例,所述患者以时期递升的次序列出。通过暴露于风险中的患者数量除生存者(或复发的患者)的数量计算出中间生存率(intervalsurvivalrate)(或中间复发率(intervalrecurrencerate))。累积生存率(或累积复发率)是条件概率,其通过先前的累积生存率(或累积复发率)乘以现在的中间复发率(中间复发率)来计算。以生存时间(或观察期)为横轴并且以累积生存率(或累积复发率)为纵轴,通过阶梯函数来构建Kaplan-Meier曲线。
对于每种标记物,完成了关于复发、总生存和无病生存的Kaplan-Meier曲线。根据实验确定的统计学显著基准将实施例2中测定的各标记物的表达划分为高表达和低表达,并且对于各标记物,将高表达情况和低表达情况相互分开,从而完成Kaplan-Meier曲线。图1示出了完成了的Kaplan-Meier曲线。
由图1能够确定,在对于复发、总生存和无病生存完成的Kaplan-Meier曲线中,每种标记物形成了高表达情况和低表达情况彼此明显区分的曲线。这意味着在各标记物表现为高表达和低表达的情况之间,中间复发率或中间生存率和以此为基础的累积复发率或累积生存率存在明显的差异,并意味着,因此每种标记物的表达模式可以是显示患者复发可能性或生存可能性的指数。
通过计算每一个复发点或死亡点的观察值和预期值获得卡方检验统计信息,对各标记物或其组合通过log秩检验(log-ranktest)进行显著性检验。从而计算出p值,下面的表2显示了计算出的p值。

从上述表2可以确定,各标记物或其组合显示了足够低的p值,以至于能够认为对于复发、总生存和无病生存全部具有显著性。特别是,在两种标记物组合的情况下,复发、总生存和无病生存的所有p值均小于0.05,这是所希望的。当p值变得越低,统计学显著性就变得越高。因此,低p值意味着通过各标记物或其组合对肝癌预后的评估是高度精确的。
实施例4:另外的标记物的发现
按照与实施例1至实施例3相同的方式进行了实验,不同之处在于以取自185位肝癌患者的癌症组织和相邻的正常组织进行实验,从而通过使用下列基因作为标记物获得了Kaplan-Meier曲线和p值:
TKT(转酮醇酶;NCBIGI:205277461;SEQIDNO:81)。
下面的表3显示了所使用的引物和探针,图2显示了Kaplan-Meier曲线;且下面的表4显示了计算出的p值。

由图2可以看出,在对复发、总生存和无病生存而完成的Kaplan-Meier曲线中,上述标记物形成了高表达情况和低表达情况相互显著区分的曲线。这意味着在该标记物高表达和低表达的情况之间,中间复发率或中间生存率,和以此为基础的累积复发率或累积生存率存在显著的差异,并且意味着因此,该标记物的表达模式可以是显示患者复发可能性或生存可能性的指数。
从上述表4可以确定,在复发、总生存和无病生存的所有方面,上述标记物显示出小于0.05的p值,这是所希望的低。当p值变得更低时,统计学显著性变得更高。因此,低p值意味着通过该标记物对肝癌预后的评估是高度精确的。
实施例5:另外的标记物的发现
按照与实施例1至实施例3相同的方式进行了实验,不同之处在于以取自369位肝癌患者的癌症组织和相邻的正常组织进行实验,从而利用下述23个基因作为标记物获得了Kaplan-Meier曲线和p值:

由图3至图10能够确定,在对于复发、总生存和无病生存完成的Kaplan-Meier曲线中,各标记物形成了高表达情况和低表达情况相互显著区分的曲线。这意味着在各标记物表现为高表达和低表达的情况之间,中间复发率或中间生存率,和以此为基础的累积复发率或累积生存率存在明显的差异,并且因此,各标记物的表达模式可以是显示患者复发可能性或生存可能性的指数。
从上述表6至表9可以确定,每种标记物或所有的两种标记物的组合显示了足够低的p值,以至于能够认为其在复发、总生存和无病生存方面具有显著性。当p值变得更低时,统计学显著性变得更高。因此,低p值意味着通过每种标记物或它们的组合进行的肝癌预后的评估是准确的。特别是,绝大多数各标记物或所有的两种标记物组合均显示出小于0.05的p值,这是所希望的低p值。
所有的三种以上标记物的组合的p值均小于0.05。下面的表10给出了显示出最低p值的单一标记物或两种以上的标记物的组合以及相应的p值。

通过以上表10可以看出,可以发现随着组合的本发明的标记物越多,p值就越低。这意味着组合越多的本发明标记物,久显示出越低的p值(p值越低意味着显著性越高),从而表明基于标记物组合的预后评估中实现了进一步改善的准确性。
实施例6:交叉验证
对在实施例5中具有统计学显著性的标记物的组合进行了交叉验证。
将369位肝癌患者随机分为两组(阳性组:185位患者;测试组:184位患者)。对于阳性组,根据与实施例3中相同的方法通过实验建立了被认为统计学上显著的基准,并进行了高表达和低表达的分类。通过将这样建立的基准固定,对于测试组,计算出在p<0.05或p<0.001时对复发、总生存和无病生存的评估的准确性水平。
下面是对复发、总生存和无病生存的预后显示优异精确性的代表性实例:
复发:AIFM1_AKT1_LC3(77.3%,p<0.05的水平)
总生存:ATG5_DRAM_FAS_XIAP(87.3%,p<0.001的水平)
无病生存:AIFM1_AKT1_LC3(71.3%,p<0.05的水平)。
Claims (2)
1.一种评估肝癌预后的试剂盒,所述试剂盒用于:
步骤1,用用于评估肝癌预后的组合物处理取自被试肝癌患者的肝癌组织样本,所述组合物包含用于特异性检测LAMP1(溶酶体相关膜糖蛋白1;NCBIGI:112380627;SEQIDNO:99)的表达水平的物质,所述LAMP1为肝癌预后标记物;和
步骤2,检测如此处理的肝癌组织样本中LAMP1(溶酶体相关膜糖蛋白1;NCBIGI:112380627;SEQIDNO:99)的表达水平;
步骤3,通过将所述被试肝癌患者划分为下述组群而评估肝癌的预后:肝癌复发的高风险组群、肝癌的总生存和/或肝癌的无病生存和肝癌复发的低风险组群、肝癌的总生存和/或肝癌的无病生存,其中将在如此处理的肝癌组织样本中检测的LAMP1(溶酶体相关膜糖蛋白1;NCBIGI:112380627;SEQIDNO:99)的表达水平低于基准的患者划分为所述高风险组群,将在如此处理的肝癌组织样本中检测的LAMP1(溶酶体相关膜糖蛋白1;NCBIGI:112380627;SEQIDNO:99)的表达水平高于基准的患者划分为所述低风险组群。
2.一种评估肝癌预后的试剂盒,所述试剂盒用于:
步骤1,用用于评估肝癌预后的组合物处理取自肝癌患者的肝癌组织样本,所述组合物包含用于特异性检测由LAMP1(溶酶体相关膜糖蛋白1;NCBIGI:112380627;SEQIDNO:99)所编码的蛋白的表达水平的物质,所述LAMP1为肝癌预后标记物;和
步骤2,检测如此处理的肝癌组织样本中由LAMP1(溶酶体相关膜糖蛋白1;NCBIGI:112380627;SEQIDNO:99)所编码的蛋白的表达水平;
步骤3,通过将所述被试肝癌患者划分为下述组群而评估肝癌的预后:肝癌复发的高风险组群、肝癌的总生存和/或肝癌的无病生存和肝癌复发的低风险组群、肝癌的总生存和/或肝癌的无病生存,其中将在如此处理的肝癌组织样本中检测的由LAMP1(溶酶体相关膜糖蛋白1;NCBIGI:112380627;SEQIDNO:99)所编码的蛋白的表达水平低于基准的患者划分为所述高风险组群,将在如此处理的肝癌组织样本中检测的由LAMP1(溶酶体相关膜糖蛋白1;NCBIGI:112380627;SEQIDNO:99)所编码的蛋白的表达水平高于基准的患者划分为所述低风险组群。
