Abstract

磁共振成像(MRI)和计算机断层扫描(CT)是组织成像的所选的方法。磁共振成像基于高强度磁场,产生一个净磁矢量暂时改变高度水和组织中的质子的取向。磁共振成像主要适用于韧带、肌腱及脊髓的损伤和脑部肿瘤的成像。然而,此项技术应用于骨组织成像时,并不能获得如同计算机断层扫描(CT)成像一般的细节图像。
计算机断层扫描(CT)是基于X射线衰减原理,利用检测器检测X射线衰减,并计算像素的值,然后将其转化为图像。定量计算机断层扫描(QCT)能够提供骨密度测量,相对于二维的骨密度面积能够检测三个维度的真实体积(毫克/立方厘米)。这使得操作员能够区分皮质和松质骨,并且能够准确地确定骨随时间推移的变化。因此,定量CT主要用来进行椎体松质骨密度检查,定量CT可以选择性地评估具有代谢活性和结构上十分重要的骨小梁,并且能够区分椎骨破裂,测量骨质损失。然而,定量CT非常昂贵,而且相对于双能X线吸收测定法,定量CT使得患者暴露于高剂量的电离辐射。
对于磁共振成像和计算断层扫描而言,造影剂是改善成像技术中不可缺少的,因为它们提高了图像的清晰度。目前在磁共振成像中使用的造影剂对组织没有特异性,这些造影剂是基于铁氧化物纳米颗粒或者钆。然而,虽然现在使用的造影剂能够提供良好的成像效果,对患者是安全的,但是这些造影剂是无法识别特定的组织和细胞类型。此外,目前CT所使用的造影剂,如基于碘的化合物有几个局限性,包括由于快速肾清除引起的成像时间较短,同时导致肾毒性,血管渗透。
尽管骨质疏松症的发病机制目前还不清楚,数据表明,它是由破骨细胞的骨吸收活动和成骨细胞的骨形成活动之间的不平衡所造成的。这种不平衡的细胞活动导致骨组织的渐进性弱化,最终导致形成微骨折,微骨折是在临床显著的骨折的起始点。流行病学的研究表明,这些微骨折往往在特定的解剖部位发生,包括椎体,股骨转子下(sub-throcantericfemouralbone)和手腕。尽管不平衡细胞活动在大多数骨质疏松症病例中起关键作用,其中骨质流失(即骨质疏松)的相对贡献可能有所不同,这取决于许多不同的因素,包括年龄,性别,遗传易感性骨质疏松,缺乏锻炼,药物,健康和营养。一般情况下,骨质疏松症被归类为原发性(即老年期)或继发性(即非年龄相关)。
原发性骨质疏松症可以进一步分为I型(即绝经后)或II型(即年龄相关型)。Ⅰ型骨质疏松症主要发生在50岁至70岁人群,主要是由于绝经后的雌激素损失及其对骨小梁的影响。Ⅱ型与衰老过程密切相关,通常发生在70岁及以上人群,衰老影响骨小梁和皮质骨。然而,众所周知,骨密度的变化在每个人身上都发生,与年龄无关。在正常的成长期,骨形成比骨质流失更快,直到30-35岁时达到骨质峰值(PBM)。一旦达到骨质峰值,男性和女性都开始发生骨质流失,其流失率范围在0.5-2%每年,取决于个体差异。在女性中,更年期进一步加速骨质流失率,时间段约为10年。
继发性骨质疏松症是由一种隐形的医疗疾病或生活方式(如饮酒、药物滥用或不良饮食)引起的并发症,并可能影响到所有年龄段的人。事实上,已经有报道表明年龄仅仅在几周龄的婴儿,其骨密度低于预期值,会发展为骨质疏松症。可以引起继发性骨质疏松症的医疗疾病包括激素失衡,类风湿性关节炎,肝衰竭,肾衰竭疾病,胃肠功能受损,多发性硬化症,坏血病,神经性厌食症和运动员三联症。在某些情况下,医疗疾病并不导致骨质疏松症的条件,但治疗所使用的药物能够导致骨质疏松症。一些用来进行治疗医疗疾病的药物,如皮质类固醇,一些激素和锂,与继发性骨质疏松症的发展有关。一般而言,骨质疏松症在女性比男性多发具有更多的临床显著性。
骨肉瘤是最常见的原发性肉瘤(其发生率为:0.2–3/每100000人/年),是成骨细胞分化导致类骨质组织不良或骨生长不良,从而影响骨完整性。尽管骨肉瘤也能够发展为恶性骨肉瘤,其在恶性肿瘤中的绝对发病率低。在严格的组织学定义中,骨肉瘤病变在组织学特征上是多样化的,其预后不仅取决于这些参数,还依赖于骨肉瘤的解剖学部位。骨肉瘤的另一个特点是有产生不同数量软骨基质和纤维组织的趋势,在某些情况下,主导了实质的骨质形成。因此,形成三个亚型的分类,即成骨细胞型骨肉瘤,软骨母细胞型骨肉瘤和成纤维细胞型骨肉瘤。在骨骼系统内部,骨肉瘤通常发生在四肢骨的干骨后端,在成人中观察到,骨肉瘤最通常的是在膝关节周围,以及在轴向骨架或颅面骨周围。就骨而言,骨肉瘤可能发生在骨内(髓内或皮层内室),骨的表面,以及骨外位点。
骨转移的特点是成骨性表型,溶骨性表型,或者同时具备成骨性和溶骨性表型。不同的恶性肿瘤表现出促骨效应和骨高亲和力,癌症最常见的转移沉积在骨中。常见的恶性肿瘤,包括乳腺癌,前列腺癌和肺癌最终转移为骨癌。因此,骨癌被广泛认为是肿瘤领域的一个重大挑战。一旦转移至骨,转移细胞增加成骨细胞的增殖和活性,包括通过释放可溶性介质或通过细胞与细胞的接触来增加核因子-κB受体活化因子配基(RANKL)的表达和其释放。这将通过RANKL-核因子-κB受体活化因子配基(RANK)之间的相互作用激活破骨细胞前体的分化和成熟破骨细胞的活性。破骨细胞的骨吸收所释放的细胞因子和其他生长因子如TGF-β和胰岛素样生长因子(IGF)对肿瘤细胞是必需的,从而提高肿瘤的生长和延续的过程。骨吸收增加所留下的溶骨性病变可以通过X射线、密度扫描技术和磁共振成像进行检测。成骨细胞异常转移所增加的骨形成,在骨中表现为高密度区,可以通过X-射线和磁共振成像(MRI)观测。
健康和病变的骨之间的骨吸收活化的频率的差异大于骨吸收和形成阶段之间的差异。骨激活频率是由骨表面的所谓的多发性骨细胞单元(BMU)的量所表征。多发性骨细胞单元在骨质疏松的骨表面的含量大于正常骨表面的含量,而且其含量与骨中破骨细胞含量及骨吸收陷窝的增加相关。在系统性疾病如骨质疏松症中的骨穿刺组织学染色,可能被用来作为诊断方法,而且染色会使人们有可能观察到用于诊断和治疗的微观定位的固体颗粒材料(比如普鲁士蓝染色铁离子)。
根据上述观点,认为提供新的手段来诊断和/或治疗骨相关疾病的需求将越来越多。
因此,根据本发明的第一个方面,是提供了一种骨生物特异性试剂,其包括造影剂核心,此造影剂核心是可以被磁共振成像(MRI)或计算机断层扫描(CT)观测到;造影剂核心被一个聚合物壳所包围,这个聚合物壳起到官能化骨靶向肽的作用,其中所使用到的肽是具有骨生物学特异性的。
本发明中的骨生物特异性试剂的发明是基于设计,开发和完善一系列不同的纳米粒子,亚微米粒子和原子或分子的元素,以及它们在磁共振成像(MRI)和计算机断层扫描(CT)成像技术中的应用,这将在下面详细描述。骨生物特异性试剂包括核心,此核心包括已经被能够特异性地识别骨细胞(如成骨细胞或破骨细胞)或矿化骨细胞外基质(如羟基磷灰石)的肽官能化的常规造影剂。因此,所述肽使所述试剂对骨病理学具有特异性生物识别。如实施例1-3中所描述和图5E所示,发明人已经准备了一系列不同的骨特异性试剂,其中一定范围的不同骨靶向肽将被用于官能化聚合物壳。仔细选择用于造影剂核心的材料,形成聚合物壳的聚合物,和骨特异性的官能化肽,使得骨生物特异性试剂能够用于诊断和/或治疗各种骨相关疾病。因此,生物特异性试剂可用于骨重塑活动的成像、检测病理情况(如骨吸收或骨肿瘤)和或/骨折或手术干预后的组织修复过程。更为有利的是,发明的生物特异性试剂已经被设计为骨重建过程中必不可少的与疾病骨特异性相互作用的必要元素,同时可以精心定制用于更少入侵性早期诊断和/或治疗骨疾病的可注射材料。
优选地,造影剂形成或组成为本发明生物特异性试剂的核心,其由外层的聚合物壳包围。造影剂核心的平均直径可为5nm和30nm之间,或在10nm和20nm之间。
在使用磁共振成像或计算机断层扫描中可见的造影剂核心可以包含金属或非金属材料。造影剂核心可以包括磁性或非磁性材料。在造影剂是磁性的实施方式中,它可以包括磁共振成像造影剂。造影剂可以包括顺磁性或超顺磁性材料。例如,造影剂核心可能包括铁、镍、钴或镝,或化合物,如氧化物或合金,其中包含一种或多种的这些元素。优选地,所述造影剂包括磁铁矿(Fe3O4)。

所述聚合物壳通过物理吸附或共价键连接到造影剂核心,连接方式取决于聚合物的化学性质和造影剂核心的表面的化学性质。聚合物壳可以进一步设计衍生物以达到有效地骨靶向肽的官能化是符合需要的。例如,聚合物壳可以通过聚合物与琥珀酸酐反应进衍生。这样可以提供聚合物和骨靶向肽之间的空间,以减少空间位阻。它也可以提高所使用的聚合物(如壳聚糖)的溶解性和生理pH。琥珀酸酐又名二氢-2,5-呋喃二酮,分子式C4H4O3。可以由琥珀酸酐反应得到聚合物的衍生物的方法是本领域技术人员所知晓的,例如壳聚糖,这些将在实施例1中描述。

例如,骨靶向肽可以将生物特异性试剂导向至骨中存在的细胞,例如成骨细胞,骨细胞,破骨细胞,骨细胞祖细胞,破骨细胞祖细胞或骨衬里细胞。优选地,骨靶向肽将生物特异性试剂导向成骨细胞和破骨细胞。具有能够模仿GAP-连接通讯(例如连接蛋白43,Cx43)的肽也可以作为骨靶向肽,GAP-连接通讯特异性于帧间和帧内的成骨细胞和破骨细胞的细胞通讯。此外,骨靶向肽可以将生物特异性试剂导至骨的骨矿物相,即羟基磷灰石。因而,包括羟基磷灰石靶向肽的生物特异性试剂可以作为一个有价值的工具,用来监测骨组织创伤或疾病进展(如骨质疏松症)过程中的形成骨组织的矿化作用。
众所周知,许多人骨疾病与骨重建循环失调有关,如佩吉特氏病,和癌症骨转移,预示了护骨素(osteoprogeterin)(OPG)和RANKL之间的不平衡。发明者由此相信RANK-RANKL-OPG途径及其相关途径可能被用于开发骨代谢疾病的治疗和诊断的具有成本效益的、生物特异性材料。
因此,骨靶向肽可以与细胞间隙连接通讯(GJIC)和RANK-RANKL-OPG三联途径相关。事实上,本发明中的生物特异性试剂对于RANK-RANKL具有特异性的事实,意味着它会在样本中识别和定位靶细胞。在骨肉瘤和骨转移等疾病中,肿瘤可以通过X射线和MRI来定位,而生物特异性试剂可以提高信号,提高分辨率,从而可以发现许多可能会被错过的更小的病灶。
骨靶向肽可以包含氨基酸序列,其通过结合RANK来模拟OPG从而减少或者防止RANKL诱导的破骨细胞分化及其活性。另外,骨靶向肽同样可以模仿类例如连接蛋白43的蛋白的功能,参与抑制破骨细胞-破骨细胞或者破骨细胞-成骨细胞间的相互作用。骨靶向肽也可用于识别成骨细胞的迁移,或者骨的骨质相(即羟基磷灰石)。因此骨靶向肽可以包含如下的氨基酸序列:
(a)SRPTEKTIFII(SEQIDNo.1)。这种肽来源于连接蛋白43模拟肽(Cx43)Gap27,可以被用来阻断破骨细胞-破骨细胞和/或破骨细胞-成骨细胞间的相互作用。在此处引用到时,这个肽被指定为Gap27p。它是一个已知的序列,并且能够抑制破骨细胞-成骨细胞接触所依赖的细胞(sell)间通信。

(h)VTKHLNQISQSY(SEQIDNo.8)。这个肽对于羟基磷灰石同样具有高亲和力。
本发明人已经发现,在一些实施方式中,优选使用连接肽或者间隔肽将骨靶向肽连接到聚合物壳上,优选地这些肽都是它们的衍生物(如与琥珀酸的衍生物)。这样的连接肽在水溶液中具有提高的溶解度,从而有利于肽和试剂的嫁接,进而提高生物配体递呈至细胞。
在一种实施方式中,可使用的合适的连接肽包括氨基酸序列K-KK)。这种肽被指定SEQIDNo.9或者在引用到时被称为G1PL。这个肽是一个极性分子,因而能够提高溶解度和可接近性。
在另一个实施方式中,连接肽可以包含氨基酸序列K(KK)-(KKKK)。这种肽被指定SEQIDNo.10或者在引用到时被称为G2PL。
在另一实施方式中,连接肽可以包含氨基酸序列K(KK)-(KKKK)-(KKKKKKKK)。这种肽被指定SEQIDNo.11或者在引用到时被称为G3PL。
因此,骨靶向肽可以包含任何SEQIDNo.9-11,或者是该片段的功能变体。例如,任何指定的SEQIDNo.1-8的肽可以结合任何SEQIDNo.9-11的肽。因此,在另一个实施方式中,骨靶向肽可以包括以下氨基酸序列,或功能性片段或它们的变体:

DOTA分子是相当大的,具有较强的酸性和反应活性,所以该分子需要加入一个空间或者连接肽以避免损害骨靶向肽的效价。
正如实施例子中所描述的,骨靶向肽可以通过已知方法进行合成,比如,通过固相肽合成(SPPS),使用传统的9-芴甲氧羰基(Fmoc)来实行保护/去保护策略。
骨靶向肽可以进行环化。环化形式的骨靶向肽在化学性质上更加稳定。另有一些报道认为环化骨靶向肽能够提高活性(见ShinJetal2008),比如将二甲亚砜(DMSO)进行氧化形成半胱氨酸-半胱氨酸二硫键。
骨靶向肽可以通过共价键连接至骨生物特异性试剂的聚合物壳。优选地,聚合物壳都含有壳聚糖,这是一类衍生物,比如说使用琥珀酸酐产生的衍生物。在一种实施方式中,肽可以通过使用碳化二亚胺共价连接至聚合物壳,以产生发明的纳米颗粒。
生物特异性试剂可以包括生物活性化合物,其可以通过骨靶向肽被输送到骨。例如,生物活性化合物可以选自一组分子,其中包含:染料,电化学介质,蛋白质,肽,化学化合物(如药物),遗传物质(如寡核苷酸,DNA,RNA),小分子,抗体,酶,和其他生物活性分子。生物活性化合物可以被连接至生物特异性试剂,例如通过交联、吸附、离子相互作用或直接共价连接到聚合物覆层达到包埋的效果。
在一种实施方式中,骨生物特异性试剂可以包含纳米颗粒。在另一种实施方式中,骨生物特异性试剂可以包括亚微米粒子。纳米粒子可以基本上是球形的形状。
生物特异性试剂的平均颗粒直径可以为亚微米级别,即小于1000纳米。生物特异性试剂的平均颗粒直径可以为100~450纳米。
生物特异性试剂可以通过首先采用离子交联步骤,接着以预定的浓度溶解聚合物和交联剂,然后内部造影剂核心可以加到混合物中。该聚合物也可以溶解在含有交联剂的溶液中(例如,在连续搅拌条件下逐步滴入)。混合物可以发生反应(例如,至少30分钟)。然后将混合物离心,所得到的颗粒(即生物特异性试剂)通过合适的溶剂(如乙醇或水)收集。收集到的颗粒可以通过冷冻干燥进行储存和/或描述特征。可以加入连接肽或者间隔肽,比如在存在琥珀酸酐的条件下通过开环反应进行连接。最后,可以连接骨靶向肽,比如通过碳化二亚胺化学法。
如实施例4中所描述的,发明者已经证明本发明的生物特异性试剂能够有效地应用于磁共振成像(MRI)或计算机断层扫描(CT)成像技术,依赖于所使用的造影剂材料。
因此,根据第二方面,提供了根据第一方面的骨生物特异性试剂用于诊断。
可以理解的是,骨生物特异性试剂将在一系列不同生物成像应用中被用作生物传感器。比如生物特异性试剂作为生物标记优选用于磁共振成像(MRI)或计算机断层扫描(CT)成像技术。

发明者已经证明,骨生物特异性试剂可以用于骨重塑活动的成像、骨病理情况检测(例如,骨吸收、骨肿瘤、骨质疏松等),和/或骨折后的骨组织修复过程。此外,除了各种成像技术可以利用具有强大的骨靶向特性的骨特异性试剂,实施例5也解释了本发明的生物特异性试剂是如何有效地抑制破骨细胞及破骨细胞活性,从而可以防止骨吸收。发明者因此认为生物特异性试剂可以应用于治疗骨疾病的各种治疗应用。

在优选的实施方式中,本发明的生物特异性试剂以及根据本发明的组合物,可以通过注射进入血液或者直接导入需要治疗的位点,也就是骨给药给被试者。注射可以是静脉注射(大丸药或注射)或皮下注射(大丸药或注射),或皮内(大丸药或注射)或骨内注射。
应当理解的是,本发明的生物特异性试剂所需要的量是由其生物活性和生物可利用度来决定,这反过来又依赖于给药方式,生物特异性试剂的理化性质,以及生物特异性试剂是作为单药治疗或联合治疗。给药的频率也会受到生物特异性试剂在治疗被试者体内的半衰期所影响。最佳给药剂量可以由本领域的技术人员确定,而且随着特定生物特异性试剂的使用、药物组合物的强度、给药方式以及疾病诊断和治疗的推进而发生改变。其他的影响因素取决于治疗的特殊被试者差异会导致需要调整剂量,被试者因素包括受试者年龄、体重、性别、饮食和给药时间。
一般来说,每天0.01微克~0.5克/公斤体重剂量的生物特异性试剂可用于治疗、改善或预防骨疾病。
本发明的试剂可以在疾病发作期之前、之中或之后进行给予。每日剂量可以单次给药(例如,每日注射一次)。或者,生物特异性试剂可以在一天内需要给药两次或更多次。作为一个例子,试剂可以每日给药两次(或更多次,取决于治疗的骨疾病的严重程度),每日剂量25毫克和7000毫克(即假设被试者体重为70公斤)之间。病人在早晨醒来可以接受第一次给药剂量,然后在晚上接受第二次给药剂量(如果是每日两个剂量治疗机制),或之后以3或四小时的间隔给药。另外,缓释装置可用于向患者提供最佳剂量,而不需要进行重复剂量的给药。
已知的程序,如那些制药行业所通常采用的(例如在体内实验,临床试验等),同样可以被用于形成特定的配方,其中包括生物特异试剂和精确的治疗机制(如每日试剂的剂量和给药频率)。
因此,在本发明的第九方面,提供了一种包括第一方面的骨生物特异性试剂以及药学上可接受的载体的药物组合物。
第十方面,本发明还提供了制备根据第九方面的组合物的制备方法,该方法包括将治疗有效剂量的第一方面的骨生物特异性试剂和药学上可接受的载体接触。
“被试者”可以是脊椎动物,哺乳动物或家畜。因此,根据本发明的组合物和药物可用于治疗任何哺乳动物,例如牲畜(如马),宠物,或可用于其他兽医应用。然而,最优选地,被试者是人。
当给药给被试者时,生物特异性试剂的“有效治疗剂量”可以是任何剂量,是治疗骨疾病,或产生预期的效果所需药物或药剂的量。例如,所使用的试剂的治疗有效剂量可以从约0.01毫克到约800毫克。
此处的“药学上可接受的载体”是任何已知的化合物或已知的化合物组合,本领域技术人员已知这些化合物在制备药物组合物时是有用的。
在一种实施方式中,药学上可接受的载体可以是固体,药物组合物可以是粉末剂或片剂的形式。药学上可接受的固体载体可以包括一个或多个物质,该物质同时也可以作为调味剂,润滑剂,助溶剂,助悬剂,染料,填料,助流剂,压缩助剂(compressionaid),惰性粘合剂,甜味剂,防腐剂,染料,涂料,或片剂崩解剂。载体也可能是封装材料。在粉末剂中,该载体是精细分割的固体,其与本发明的精细分割的活性剂存在混合物中。在片剂中,活性剂(如生物特异性试剂)可与适当比例的具有必要的压缩性能的载体混合,并压缩成一定形状和需要的尺寸。粉末剂和片剂优选包含高达99%胶囊或小室(cell)。合适的固体载体包括,例如磷酸钙,硬脂酸镁,滑石,糖,乳糖,糊精,淀粉,明胶,纤维素,聚乙烯吡咯烷酮,低熔点蜡和离子交换树脂。在另一实施方式中,该药物载体可以是凝胶,该组合物可以采用乳膏或类似的形式。
然而,该药物载体也可能是液体,药物组合物是溶液的形式。液体载体是用于制备溶液,悬浮液,乳液,糖浆,酏剂和加压的组合物。本发明的试剂可以溶解或悬浮在药学上可接受的液体载体,如水,有机溶剂,水/有机溶剂混合物或者药学上可接受的油或脂。液体载体可以包含其他合适的药物添加剂,如溶解剂(solubiliser),乳化剂,缓冲剂,防腐剂,甜味剂,调味剂,悬浮剂,增稠剂,着色剂(colour),粘度调节剂,稳定剂或渗透调节剂。口服和肠胃外给药的液体载体的合适例子包括水(部分含有上述添加剂,例如纤维素衍生物,优选羧甲基纤维素钠溶液),乙醇(包括一元醇和多元醇,如乙二醇)和它们的衍生物,和油(如分馏椰子油和花生油)。
胃肠外给药,载体也可以是油酯,如油酸乙酯和肉豆蔻酸异丙酯。无菌液体载体可以用于无菌液体形式的肠胃外给药组合物中。
无菌溶液或悬浮液的液体药物组合物可以用于,例如,肌肉内注射,鞘内注射,硬膜外注射,腹腔内注射,静脉注射,特别是皮下注射。试剂可制备成无菌固体组合物,该无菌固体组合物在给药时可以溶解或者悬浮于无菌水、盐水或其他适当的无菌注射媒介。
本发明的生物特异性试剂和药物组合物可以以无菌溶液或悬浮液的形式口服给药,所述无菌溶液或悬浮液含有其他溶质或悬浮剂(例如,足够生理盐水或葡萄糖使得溶液等渗),胆汁盐,阿拉伯胶,明胶,山梨醇单油酸酯,聚山梨酯80(山梨醇油酸酯和其酸酐共聚环氧乙烷)以及其他类似物。本发明的生物特异性试剂,也可通过液体或者固体的组合物形式进行口服。适用于口服的组合物包括固体剂型,如丸剂,胶囊剂,颗粒剂,片剂,粉末剂,液体剂的形式,例如溶液,糖浆,酏剂,和悬浮液。用于肠胃外给药的形式包括无菌溶液、乳液和悬浮液。

实施例
发明者有兴趣提供改进的仪器和方法,用于诊断(例如,通过磁共振成像或计算机断层扫描成像)或治疗骨相关疾病。因此,他们已经设计和开发了新型骨特异性试剂2(如纳米粒子,亚微米颗粒,和原子或分子元素),如图5E所示,包括造影剂核心4(例如,离子氧化物或金金属核心),造影剂核心4被聚合物6(例如,壳聚糖)包被,聚合物6本身由一个或者多个肽8或其他仅仅存在于骨的肽衍生或者官能化,肽8可以识别骨细胞(如破骨细胞,成骨细胞),其他仅仅存在于骨的肽为例如羟基磷灰石特异性识别肽。根据精心选择的核心4的材料,形成外壳6的聚合物涂层,和连接至聚合物外壳6的骨特异性的官能化肽8,纳米粒子2等,即可用于诊断或治疗。例如,颗粒2可用于骨重建活动的成像,检测病理条件和/或组织修复过程。下面的例子说明了他们的研究结果。
例1–连接琥珀酸酐至壳聚糖
发明者已经表明,壳聚糖(CS)是一种可用于造影剂或者其他药物活性成分的涂层,能够与琥珀酸酐连接。壳聚糖琥珀酸酯结合物在本领域是已知可以作为具有生物相容性和可生物降解性的药物输送剂,该药物输送剂可用于片剂。
1.材料和方法
(1)壳聚糖(CS)衍生化
CS是通过琥珀酸酐(Suc-Chi)使用已知的开环反应衍生(Yanetal.,2006,YakugakuZasshi,126,789-793)。1%(W/V)CS溶液(溶于1%v/v醋酸)通过0.8微米的微孔膜(Millipore)过滤并然后用甲醇稀释(1:4)。琥珀酸酐(≥99%GC,SigmaAldrich)以4%(w/v)浓度溶解在5毫升丙酮中,琥珀酸酐在磁力搅拌下逐滴加入,室温搅拌下过夜。从过量的溶液中除去形成的凝胶,用甲醇双倍稀释,并用超纯水透析3天。每天更换两次超纯水,得到的沉淀物经离心收集和冻干。
(2)Suc-Chi亚微米珠的生产(即纳米颗粒)
Suc-Chi珠使用已经建立的离子凝胶法生产(Agnihotri,etal.,2004)。简要地说,三聚磷酸钠(TPP)溶液(1毫克/毫升)在磁力搅拌下逐滴加入1毫克/毫升Suc-CS溶液(如上所述),在达到体积比为1:5时,在室温下反应45分钟。为生产磁共振成像(MRI)或CT成像的生物特异性造影剂(即本发明的纳米粒子2),在加入到Suc-CS溶液之前,铁氧化物核心4颗粒(Fe3O4,平均直径10纳米)或金核心4颗粒(平均直径小于20纳米)通过超声波被分散在TPP溶液中。所加入的核心颗粒4的重量为所溶解的聚合物6的一半。核心颗粒4然后通过乙醇离心洗涤(消毒),然后用超纯水离心洗涤以及用无菌PBS重构。
(3)肽合成
表1中所列的肽8及其相应的氨基酸序列被合成,然后用于官能化核心颗粒4。

肽2通过固相肽合成(SPPS)方法进行合成,在以TentaGelSNH2树脂(0.1毫摩尔)和二甲基甲酰胺(DMF)为反应溶剂的条件下,使用传统的9-芴甲氧羰基(Fmoc)来实行保护/去保护策略。酸性(acidliable)Fmoc-Rink-酰胺连接剂(连接剂)首先加入到树脂中为了后续肽8的断裂。然后加入氨基酸序列C-末端的第一个氨基酸,依次通过耦合/脱保护随后的氨基酸步骤,按照肽序列合成表1所示的整个氨基酸序列。耦合反应(30分钟,2次)通过使用HBTU(O-苯并三氮唑-N,N,N’,N’-四甲基-脲-六氟-磷酸酯)来进行氨基基团活化,和N,N-二异丙基乙胺(DIPEA)作为叔碱。在二甲基甲酰胺中的20%哌啶(v/v)断裂(2分钟,3次)Fmoc保护基团使得保护的氨基基团得以暴露。在所有制备中,树脂,连接剂和氨基酸分别以1:4:4的摩尔比加入。HBTU和DIPEA(二异丙基乙胺)分别以氨基酸的1倍和2倍摩尔浓度加入。每个耦合和/或脱保护步骤之后进行清洗步骤(用DMF3次)。
护骨素(Osteoprogeterin)(OPG)模拟肽,op3-1op3-4,如表1所列,是通过二甲基亚砜(DMSO)氧化形成半胱氨酸-半胱氨酸二硫键来进行环化,如文献中描述(Góngora-Benítez,etal.,2011)。OP3是OPG蛋白的一个片段。成骨细胞表面上的RANKL(有时释放为可溶性形式)与破骨细胞上的RANK相互作用,从而启动级联反应导致破骨细胞分化和活性增加。OPG(由成骨细胞释放)是RANKL的诱饵,OPG结合RANK能够抑制RANKL-RANK相互作用,从而停止级联反应。因此,OPG类似物将作为骨相关的靶细胞的受体的配体。合成完成后,需要环化的肽在氮气环境中从树脂上断裂3小时。断裂后,通过冷乙醚收集肽,离心分离并通过氮气流进行干燥。肽溶解在60毫升的氧化缓冲液(100mM磷酸二氢钠和2mM盐酸胍,5%二甲亚砜,pH7.0),振摇12小时。然后将溶液用1MHCO2H(250微升)进行酸化,并通过LC-MS进行纯化。合并纯化组分并冻干。环化程度(二硫键的形成)通过传统的Ellman’s试剂定量自由巯基来确定。肽最终通过高效液相色谱法(HPLC)和质谱法(MS)进行鉴定。
(4)肽连接的生物特异性造影剂的生产(即纳米颗粒)
肽2通过碳二亚胺化学方法以共价连接到核心粒子4以形成本发明的纳米颗粒2。非衍生的颗粒被分散在2毫升2-(N-吗啉)乙磺酸(MES)缓冲液(0.1MMES,0.3MNaCl,pH6.5)中以获得1毫克/毫升珠浓度。然后通过添加1-乙基-3-(3-二甲基氨基丙基)碳二亚胺(EDC,4mM)和N-羟基琥珀酰亚胺(NHS,10mM)来活化核心粒子4的羧基基团。活化反应在室温下进行30分钟。加入β-巯基乙醇(2.8微升)使得过量的EDC失活,通过脱盐膜清洗核心粒子4。从表1中所选择的肽(例如肽OP3-4,序列YCEIEFCYLIR)以1毫克/毫升浓度溶解于MES缓冲液,随后以体积比1:1加入到核心粒子4的溶液中。在室温下磁力搅拌反应3小时进行结合反应。结合反应通过加入终浓度为5-10mM的羟胺来结束。
参照图5E,这是本发明的纳米颗粒2的一个实施方式的示意图。图显示了纳米颗粒2具有内核心4(例如,离子氧化物或金),其被聚合物外壳6(如壳聚糖)包覆。外壳6以可以识别骨细胞的肽8或者其他的只存在于骨的肽(例如羟基磷灰石)的覆层所官能化。所得到的纳米颗粒2通过超滤离心柱纯化(截留分子量(MWCO)100000),冻干后保存在-20℃。图1显示了CS衍生为Suc-Chi及其后续被OP3-4肽所官能化的示意图。
2.结果
CS和Suc-Chi的1H核磁共振谱图如图2所示。简要地说,谱峰信号标识如下:2.50-2.70ppm(H-Ac)是乙酰葡萄糖胺单体的乙酰基蛋白;2.50-2.75ppm(H2D)是葡萄糖胺单体的质子2;3.95-4.65ppm(H2-2)是葡萄糖胺和乙酰葡萄糖胺单体的质子2至6;4.65-4.90ppm(HOD)对应溶剂(HOD);5.10-5.30ppm(H1-A)对应乙酰葡萄糖胺单体的质子1;5.30-5.65ppm(H1-D)对应葡萄糖胺单体质子1。相比于CS的核磁共振图谱,Suc-Chi的谱图证实琥珀酸酐成功连接到CS。这在5.00-5.70ppm范围内看到对H1(D),H1-A和H2D解析不足或者信号丢失(见图2B,前两个箭头)和3.6-3.9ppm范围的H2D解析不足或者信号丢失。在Suc-Chi图谱中可见2.34–2.57ppm范围的新信号,该信号在CS图谱中缺失,该信号对应于琥珀酰基团的两个甲烷氢基团(–COCH2CH2COOH),这与之前他人的报道相一致(Liang,etal.,2012,Xiangyang,etal.,2007)。
通过滴定法测定的取代度为25.5%,而通过1H核磁共振图谱测定的取代度为30.6%。尽管精确度稍低,电位滴定法可以用来测定CS和Suc-Chi中游离氨基的摩尔量。如图3所示,计算得出CS衍生化的程度(DD)为79.92%(±5.85),Suc-Chi为54.4%(±3.7)。对于两种聚合物,重复5次滴定。这些数据表明通过与琥珀酸酐反应,CS衍生化后-NH2基团数量减少。存在的非衍生化的-NH2基团对于确保纳米颗粒2生产过程中通过TPP的CS交联很重要。
如图4所示,红外光谱结果表明壳聚糖成功衍生为Suc-Chi及其后续OP3-4成功交联。光谱波段1(3500~3200cm-1)包括伯胺和仲胺的N-H伸缩和O-H伸缩。OP3-4光谱波段1的主要贡献来自参与酰胺键的胺。波段2(1640-1580cm-1)可能对应于伯胺的N-H变形,存在四种;对于OP3-4、Suc-Chi和Suc-Chi-OP3-4,存在酰胺的N-H变形振动以及仲酰胺的羰基伸缩。波段3,(1722cm-1),这是目前在Suc-Chi和未解析的Suc-Chi-OP3-4所存在的,可能是由于在前面所述的(波段3)酰胺键连接之外的,琥珀酰基通过酯键连接至多糖所引起的羰基的伸缩振动。Suc-Chi-OP3-4的酪氨酸和苯丙氨酸的芳香结构可以由波段4(3000cm-1)证实,这源于不饱和类的C-H伸缩。Suc-Chi-OP3-1波谱的波段5(500–540cm-1)的一个大峰,在OP3-4样品中被削弱,在Suc-Chi和SC中消失,可以归结于二硫键的存在。从1H核磁共振谱图和滴定结果,得出的结论是,琥珀酰基团成功连接至CS的胺基基团。红外光谱的结果则进一步表明了肽OP3-4成功连接至Suc-Chi。
动态光散射分析(DLS)和扫描电子显微镜(SEM)表明,纳米颗粒的平均流体力学直径(Z平均)为3664.0nm(多分散性指数,PDI为0.4),在结合Fe3O4核心粒子4(10nm)后增加为408.3nm(PDI为0.5),即可生成MRI可检测的纳米颗粒2,如图5A-D所示。壳聚糖分子量的变化,有无过滤步骤的浓度变化,允许纳米颗粒2的大小和分散性指数(PDI)在可控范围内调整,见图6A-D。
例2---OP3与DOTA连接
发明者然后设置实验以确定蛋白护骨素3(OP3)是否能够结合钆特酸葡甲胺(DOTA);OP3特异性表达于骨细胞,如破骨细胞和成骨细胞;DOTA是一种螯合剂,可用作包被各种造影剂,包括钆。钆特酸葡甲胺是一种钆特酸,具有已知大环结构的基于钆的造影剂,其中包含有机酸DOTA作为螯合剂。
1.材料和方法
一种新型的DOTA-OP3-4连接蛋白采用Fmoc化学法通过固相肽合成方法进行合成,如在OP3-1和OP3-4的合成中所描述。然而在这种情况下,核心氨基酸赖氨酸首先被加入,接着将DOTA连接到由Mtt所保护的NH2基团。随后连接两个甘氨酸以形成间隔,随后进行OP3-4肽的组装。在DOTA分子和OP3-4序列之间引入赖氨酸-甘氨酸-甘氨酸间隔,被认为在肽合成和其他任何可能影响肽潜在效率的过程中,对于避免潜在空间位阻十分重要。DOTA-OP3-4的结构如图7所示。
相同的固相肽合成方法被用来合成新的衍生化的生物特异性肽,该肽含有直链端(见图8)或支链端(见图10),可以有利于稳定肽与造影剂的结合。
2.结果
图9和图11显示了肽OP3-4的直链(图9)和支链(图11)的典型的质谱谱图。这些证明了成功合成这些肽,对于稳定结合和官能化核心粒子4十分必要。合成的肽经纯化处理后纯度达95%以上。
例3-生成生物特异性的造影剂-被连接到OP3的DOTA包被的钆
发明者接下来设置实验以确定基于钆(Gd)的造影剂是否能够通过将骨-特异性蛋白护骨素3(OP3)与DOTA包被的钆(Gd)相连接形成本发明的纳米粒子2。期望这样生成的纳米粒子2可以被用于磁共振成像和/或计算及断层扫描成像技术。
1.材料和方法
新型肽被用于制造可用于MRI和CT的生物特异性造影剂(即官能化的纳米粒子2)(见表1)。核心粒子4螯合钆的反应通过在反缓冲液中将DOTA-OP3-4和氯化钆共保温15小时进行。DOTA的部分可以作为多齿配体被金属阳离子所包围,在这种情况下络合Gd3+,以形成MRI可见的肽。复合物中DOTA配体和金属离子的配位取决于配体的构象与金属阳离子的几何倾向。对自身而言,DOTA作为八齿配体,通过四个氨基和四个羧基基团结合金属。在这项研究中,DOTA分子作为七齿隔配体发挥作用,其中一个羧基基团与肽形成共价键。然而,连接DOTA和肽的氨基酸的羧基基团提供了第八个配体并恢复了八齿配体状态,从而形成了高度稳定的配合物(Viola-Villegas,etal.,2009)。
所得到的纳米粒子2通过直接连接肽8和磁性核心粒子4(即MRI造影剂)或者金核心粒子4(即CT造影剂)而得到,其中肽8具有直链或者支链根,磁性粒子4(如氧化铁)包被有薄层聚合物6或者陶瓷。在这种情况下,聚合物6和陶瓷的聚合物羧基或羟基基团的表面官能化通过氨基酸被活化和衍生化,所述氨基酸是从表1中选择的用于共价键枝接的具有选择生物特异性的肽。
2.结果

重要的是,在纳米粒子-OP3-4和纳米粒子-DOTA-Gd-OP3-4中,计算得出肽8的结合数量为4.2毫摩尔每克纳米粒子2。单个氨基酸按照反映OP3-4序列中的数量的摩尔比例进行检测。
例4,测试纳米粒子2在磁共振成像中的用途
发明者然后测试所述生物特异性肽官能化的纳米粒子2在T1和T2模式下的阳性MRI信号。通过衍生化的肽8包被钆核心4并且枝接于之前用生物活性肽官能化后的纳米粒子获得生物特异性纳米粒子2。
1.材料和方法
位于PBS缓冲液中的肽(DOTA-OP3-4和DOTA-Gd-OP3-4)溶液通过肽(DOTA-OP3-4和DOTA-Gd-OP3-4)首先溶解于最少量二甲基亚砜(DMSO)中,然后用PBS(1%DMSO)稀释至20微克/毫升的肽储液,并用0.1M盐酸调整pH至7.2制备。各种纳米粒子2(即,单独的核心粒子4,纳米粒子-OP3-4配合物,纳米粒子-DOTA-Gd-OP3-4配合物)悬浮于同样的PBS缓冲溶液。制备DOTA-Gd3+储液(20微克/毫升)和Fe3O4纳米粒子储液(10纳米,20微克/毫升),并分别作为基于钆和基于Fe3O4的造影剂的阳性对照。
首先比较了纳米粒子-OP3-4配合物和纳米粒子-DOTA-Gd-OP3-4配合物。为此,将Whatman滤纸(圆形,直径15毫米,目录号:1441150,美国)浸泡在储液中。
在纳米粒子2和肽8的浓度影响研究中,通过对储液的一系列双稀释获得不同浓度的肽8和纳米粒子2。DOTA-Gd-OP3-4的浓度为(10,5,2.5,1.25,0.625,0.313,0.078,0.039,0.020,0.010,和0.005微克/毫升),单独的核心粒子4的浓度为(20,10,5,2.5,1.25,0.625,0.313,0.078,0.039,0.020,和0.010微克/毫升)。分析物以每孔500微升的量加入24孔培养板。所有MRI成像使用英国布莱顿和萨塞克斯医学院临床影像科学中心的西门子AVANTO1.5T磁共振扫描仪完成。
2.结果
图13显示了阴性对照用磷酸盐缓冲液(PBS)浸润的滤膜的典型的MRI扫描图谱,阴性对照含有DOTA-OP3-4肽8,但不含有造影剂核心4,以及钆螯合的DOTA-OP3-4纳米粒子2。扫描清楚地显示,虽然阴性对照仅仅显示出噪音信号,肽8螯合钆核心4给出了一个明显的阳性信号。通过对用钆螯合DOTA-OP3-4和肽官能化磁性纳米粒子2浸润的滤膜的比较分析显示,在选定检测模式下,两种造影剂获得了预期典型的明场和暗场图像(图14)。
例5-体外测试OP3和OP4配合的纳米粒子的抑制作用
发明者接着确定纳米粒子2是否能够体外抑制破骨细胞生成和破骨细胞活性,纳米粒子2包含了DOTA包被的钆核心4配合至护骨素3或4(OP3或者OP4)肽8。
1.材料和方法
根据常规方法基于用RANK和M-CSF加标细胞方法或者成骨细胞单核细胞共培养体系中加标M-CSF方法,从健康人供体的外周血中新鲜分离单核细胞中获取破骨细胞。肽以及肽连接的纳米粒子2(即纳米粒子-OP3-4,磁性纳米粒子-OP3-4,纳米粒子-OP3-DOTA和纳米粒子-OP3-4-Gd-DOTA)按照肽浓度行动于50μM加入到细胞中,肽浓度通过氨基酸分析测定。阴性对照不添加任何测试材料,而阳性对照中加入50(纳克/毫升)的rhOPG。无论在单核细胞分化为破骨细胞之前或者之后,都可以进行加标流程。
对破骨细胞生成和破骨细胞活性的抑制作用是通过使用光学显微镜对TRAP阳性多核细胞(MNC)的计数和使用落射荧光显微镜对递呈F-肌动蛋白环的多核细胞的计数来进行定量的。通过扫描电镜观察骨切片上形成的骨吸收陷窝的数目分析可以定性评价破骨细胞活性。每3天更换一次新鲜培养基,培养基中添加各种生长因子和试验材料。
采用三种不同的方法确定破骨细胞生成和破骨细胞活性的抑制作用。这些方法是:(1)对TRAP阳性多核细胞进行计数;(2)通过Hoechst33258和罗丹明-鬼笔环肽双染色对具有肌动蛋白环(MNC-AR+)的多核细胞计数;(3)通过骨切片上培养细胞后形成的骨吸收陷窝评估来确定骨吸收程度。当细胞数量太多而无法通过显微镜计数,使用图像编辑软件(ImageJv1.44p)来识别和计数细胞。该软件允许根据破骨细胞和非破骨细胞的颜色和形状来清点细胞,破骨细胞生成的程度则用每个视野范围内的TRAP阳性细胞的百分比来表示(Labno)。
2.结果
被OP3-4肽官能化及其螯合钆衍生物的纳米粒子2对破骨细胞生成的影响研究结果如图15所示。在所有制剂中,相对于对照,修饰的肽8游离和结合纳米粒子2以及钆降低破骨细胞形成水平的显著,但是具有不同程度的疗效(图15)。
此外,与非官能化的纳米粒子相比,肽官能化的磁性纳米粒子2能够显著降低破骨细胞形成(见图16)。细胞培养中添加纳米粒子-OP3-4,纳米粒子-DOTA-OP3-4和纳米粒子-DOTA-Gd-OP3-4,TRAP阳性多核细胞数目并没有显著差异,表明了对于肽的不同修饰以及其枝接于纳米粒子2并不改变肽抑制破骨细胞生成的能力。当使用其本身时,Gd3+无论在体内还是体外都是有毒的。然而,大环类螯合剂如DOTA紧密捕集Gd3+,提高离子溶解度从而避免细胞毒性。事实上,具有许多自由羧基基团并提高了溶解度的Suc-Chi会进一步含有Gd3+和提高DOTA-Gd-OP3-4的生物相容性。
对已经分化的破骨细胞的活性的明显抑制作用如图17所示。
最后,通过扫描电镜观察已经分化的破骨细胞活性的抑制作用,发现对照细胞中存在的骨吸收陷窝消失,如图18所示。
总结
广泛提倡局部注射药物对于骨病理学(如骨质疏松症)进行诊断和治疗。发明者目前已经开发出了新型造影剂用于磁共振成像和计算机断层扫描成像,该造影剂能够识别骨细胞、成骨细胞和破骨细胞,以及矿物化的骨外基质。这些生物识别性能是通过合成新型的衍生肽获得,这些肽对于不同骨细胞和骨的矿物质相具有特异性。对于肽的衍生化的设计是为了在不影响其成像特性的前提下,有利于肽稳定结合纳米粒子造影剂或者离子组合物的造影剂。特定类型的造影剂以磁化聚合珠的形式,主要是壳聚糖纳米珠,通过包被磁性核心或者枝接钆修饰肽或者在交联介质中分散离子的方法来生成。识别骨的细胞和结构组份的这种能力一般耦合控制细胞行为的能力。能够在单核细胞分化为破骨细胞的过程中识别单核细胞,同时能够识别并抑制分化的破骨细胞的活性的生物特异性造影剂,可以与能够促进成骨细胞迁移的试剂同时获取。
总之:-
1.具有成骨细胞-和破骨细胞-特异性肽以及羟基磷灰石-特异性肽的表面官能化的亚微米颗粒,如Fe3O4纳米粒子(即MRI造影剂)和金纳米粒子(即CT造影剂)是优选的。
2.包埋在聚合物珠中的钆(即MRI造影剂)和碘或硼-硫酸(即CT造影剂),聚合物珠被成骨细胞-和破骨细胞-特异性肽以及羟基磷灰石-特异性肽所官能化。
3.具有成骨细胞-和破骨细胞-特异性肽以及羟基磷灰石-特异性肽的钆(即MRI造影剂)和碘(即CT造影剂)复合物。
4.通过包被合成的或者天然的聚合物形成的纳米粒子,其中聚合物的形态和大小是聚合物的调节的物理化学性质确定,而且聚合物的生物识别性能和生物活性经由能够识别组织细胞的特异性肽衍生化得到。
5.通过交联(主要是离子交联)合成的和天然的聚合物(如壳聚糖)形成的纳米粒子,其中聚合物已经提前被组织特异性肽所官能化,调节交联和生物官能化使得纳米粒子稳定性和递呈生物特异性/生物活性分子的能力最优化。这些纳米粒子包括在其配方中分散的磁共振成像(MRI)和计算机断层扫描(CT)的造影剂。
在所有情况下,这些生物特异性试剂耦合造影剂的特性,具有结合的,固有的生物识别特性,能够诱导组织成像和再生生物活性。
体外成骨细胞和破骨细胞的单层培养和共培养的研究表明了骨-特异性肽对于细胞的功能。给定的成骨细胞-特异性肽(如FHRRIKA)刺激细胞过程的功能,以及OPG模拟肽抑制破骨细胞生成的功能,同样能够被当作治疗骨缺陷症的诊疗试剂(即结合治疗和诊断)。

6.根据上述任何一项权利要求所述的生物特异性试剂,其中,所述造影剂包含磁铁矿(Fe3O4)。
7.根据权利要求1-3的任何一项所述的生物特异性试剂,其中,所述造影剂核心包括非磁性材料,是MRI和CT的造影剂。

12.根据上述任何一项权利要求所述的生物特异性试剂,其中,所述聚合物壳包含壳聚糖。
13.根据上述任何一项权利要求所述的生物特异性试剂,其中,所述聚合物壳由琥珀酸酐衍生化。
14.根据上述任何一项权利要求所述的生物特异性试剂,其中,所述聚合物壳被一种、两种或者两种以上的骨靶向肽所官能化,其中所述骨-靶向肽将所述生物特异性试剂导向至骨。
15.根据上述任何一项权利要求所述的生物特异性试剂,其中,所述骨-靶向肽将所述生物特异性试剂导向至只在骨中存在的细胞,如成骨细胞,骨细胞,破骨细胞,骨细胞祖细胞,破骨细胞祖细胞或骨衬里细胞。
16.根据上述任何一项权利要求所述的生物特异性试剂,其中,所述骨-靶向肽将所述生物特异性试剂导向至成骨细胞和破骨细胞。
17.根据上述任何一项权利要求所述的生物特异性试剂,其中,所述骨-靶向肽将所述生物特异性试剂导向至骨的矿物质相,例如羟基磷灰石。
18.根据上述任何一项权利要求所述的生物特异性试剂,其中,所述骨-靶向肽含有氨基酸序列,该氨基酸序列通过结合RANK模拟OPG的作用,从而减少和防止RANKL所诱导的破骨细胞分化及活性。
