Abstract

技术领域

背景技术
在经历细菌感染的患者中观察到癌症消退的相关性并在至少早在1868年进行了报道。报道活的减毒的沙门氏菌属生物体至具有实体瘤的动物的全身施用产生肿瘤治疗。参见,如,美国专利第6,685,935号以及Pawelek等人(Lancet Oncol.4(9):548-56,2003)。另外,减毒的革兰氏阳性分支杆菌(BCG)的膀胱内(非全身)施用在美国批准用于治疗和预防膀胱的原位癌(CIS)。

最近,已经制备在外膜中缺乏LPS,包括脂质A或6-酰基脂多糖但维持活力的革兰氏阴性细菌的突变体。例如,美国专利公布2010/0272758报道了在3-脱氧-d-甘露-辛-2-酮糖酸(Kdo)合成中存在缺陷的大肠杆菌K-12菌株KPM22。KPM22具有主要由脂质IVA组成的外膜(OM)。通过存在促进由内膜至外膜的脂质IVA的传输的次位抑制物实现这些生物体的活力。报道该抑制物减轻内膜中脂质IVA积聚的毒性副作用并提供足量的LPS前体以支持OM生物发生。通过该菌株产生的LPS前体缺乏内毒素活性,如通过在LPS前体剂量为高达1μg/mL下不能诱导人单核细胞的TNFα分泌所确定。也参见,Mamat等人(Mol Microbiol.67(3):633-48,2008)。
与感染和败血性休克相关的剂量限制性副作用显著地限制活细菌至癌症患者的全身施用。该限制已经与野生型细菌相关(参见综述如,Wiemann和Starnes,Pharmac.Ther.64:529-564,1994),并且也已经与基因减毒的细菌相关,所述基因减毒的细菌在肿瘤组织中选择性增殖并表达修饰的脂质A(参见如,Toso等人,J.Clin.Oncol.20(1):142-152,2002)。这些限制已经导致使用加热杀死的细菌用于癌症治疗。参见如,Havas等人(Med.Oncol.&Tumour Pharmacother.10(4):145-158,1993),Ryoma等人(AnticancerRes.24:3295-3302,2004),Maletzki等人(Clin.Develop.Immunol.2012:1-16,2012),美国专利第8,034,359B2号(Gunn),欧洲专利第EP 1,765,391B1号(Gunn)以及综述Wiemann和Starnes(Pharmac.Ther.64:529-564,1994)。然而,非传染性、杀死的细菌仍诱导与LPS-来源的内毒素和其它细胞组分相关的显著剂量限制性毒性,其为热原的并且可产生败血性休克的症状。因此,需要用细菌治疗癌症的进一步改善。
发明内容
本文提供了用于治疗被诊断为患有癌症的哺乳动物(如,人)中癌症的组合物和方法,其通过向该哺乳动物施用一定数量的革兰氏阴性细菌进行,其中所述细菌(i)在哺乳动物中为非活性或大体上非活性,(ii)内毒素活性和/或致热原性显著降低,并且(iii)以足以抑制癌症生长或转移潜能的量施用。在一些实施方式中,在施用于哺乳动物前通过用以下处理使革兰氏阴性细菌变成非活性或大体上非活性:(i)辐射,(ii)化学杀菌剂,(iii)使内毒素失活的抗生素(如,多粘菌素B或多粘菌素E),或(iv)破坏KDO2-脂质IVA生物合成的抗生素。可选地或除任何一种或多种上述处理之外,革兰氏阴性细菌还包括破坏或部分破坏KDO2-脂质IVA的生物合成或防止KDO2-脂质IVA的O-酰化的遗传缺陷。破坏或部分破坏KDO2-脂质IVA的O-酰化的遗传缺陷包括,例如,在功能上破坏msbB和lpxM基因座的缺陷。
在本公开的一方面中,组合物包含内毒素活性和/或致热原性显著降低的大体上非活性革兰氏阴性细菌和药学上可接受的赋形剂。在一个实施方式中,通过用戊二醛处理使革兰氏阴性细菌非活性。在另一实施方式中,通过用多粘菌素B或多粘菌素E处理降低内毒素活性和/或致热原性。在另一实施方式中,通过用戊二醛处理降低内毒素活性和/或致热原性。
在另一方面中,提供了治疗被诊断为患有癌症的哺乳动物的方法,其包括施用一定数量的内毒素活性和/或致热原性显著降低的大体上非活性革兰氏阴性细菌,其中施用的量足以抑制癌症的生长或转移。
在另一方面中,本公开提供了用于治疗被诊断为患有癌症的哺乳动物(如,人)的癌症的方法,其通过向该哺乳动物施用一定数量的革兰氏阴性细菌进行,其中所述细菌为活的,可以为或可以不为减毒的且具有导致细菌外膜内的多糖显著丧失或全部丧失的遗传缺陷并且其中施用的量足以抑制癌症的生长或转移潜能。

在其它实施方式中,向哺乳动物进一步施用化疗剂,包括,例如,环磷酰胺。在其它实施方式中,向哺乳动物进一步施用抑制免疫功能的受体的拮抗剂或受体激动剂,包括,例如,抑制T细胞受体或T细胞受体配体(如,CTLA-4、PD-1、PD-L1和PD-L2)的功能。
在其它实施方式中,向哺乳动物进一步施用刺激免疫功能的受体的激动剂,包括,例如,刺激T细胞受体的激动剂。合适的受体靶标包括,例如,GITR、4-1BB、CD40和OX40。
在其它实施方式中,向哺乳动物进一步施用刺激免疫功能的细胞因子,包括,例如,干扰素α、干扰素β、干扰素γ、粒细胞-巨噬细胞集落刺激因子、白细胞介素-2和白细胞介素-12。
在一些实施方式中,革兰氏阴性细菌为沙门氏菌或埃希氏菌。
在另一实施方式中,本公开提供了通过用多粘菌素B和戊二醛处理细菌来杀伤并降低革兰氏阴性细菌的内毒素活性和/或致热原性的方法。在一个实施方式中,活力降至0%并且内毒素活性或致热原性降低约90%或96%。

具体实施方式
本文提供了包含非活性革兰氏阴性细菌生物体并且内毒素和/或热原活性显著降低的组合物以及治疗癌症的方法,所述方法包括向患有癌症的哺乳动物施用一定数量的内毒素或热原活性显著降低的非活性革兰氏阴性细菌生物体,其中施用的量足以抑制癌症的生长或转移。
负责细菌介导的抗肿瘤活性的可能机制包括活细菌生物体在肿瘤组织中的选择性增殖以及宿主免疫响应的刺激,特别是经由LPS(内毒素)-介导的来自宿主单核细胞的杀肿瘤细胞因子释放的诱导。然而,可以认为,活细菌的增殖以及LPS(内毒素)-介导的细胞因子的诱导(甚至用通过msbB突变而减毒的LPS)负责与用活细菌治疗哺乳动物相关的剂量限制性毒性。Toso等人(J.Clin.Oncol.20(1):142-152,2002)用活的msbB-减毒的沙门氏菌治疗癌症患者并且剂量限制性毒性包括菌血症和与细胞因子释放相关的副作用。细菌在肿瘤组织中的增殖低于小鼠中见到的人肿瘤异种移植模型并且对细胞因子-介导的毒性的敏感性高于小鼠中见到的人肿瘤异种移植模型。可以认为,活细菌的全身性增殖和/或由缺乏一个仲酰基链的LPS部分介导的细胞因子相关的毒性可以防止将安全且有效剂量的活的、减毒的革兰氏阴性细菌施用于除小鼠之外的一些哺乳动物(诸如人),已知所述小鼠相对而言耐细菌感染以及细胞因子诱导的相关败血症后果。
尽管不希望受理论束缚,但可以认为,可将具有大体上降低的内毒素活性和/或致热原性的杀死的或非活性革兰氏阴性生物体以与使用活的或活性生物体相比毒性较低且治疗癌症更有效的量施用于癌症患者,所述活的或活性生物体以从业者不能控制的或增殖不足以产生治疗效果或增殖太多的可变方式在每名患者的正常组织和肿瘤组织中增殖,从而产生不可接受的毒性。也可以认为,可将具有大体上降低的内毒素活性和/或致热原性的杀死的或非活性革兰氏阴性生物体以与使用表达野生型水平的内毒素活性和/或致热原性的杀死的细菌相比毒性较低且治疗癌症更有效的量施用于癌症患者。
也可以认为,在导致细菌外膜中的糖基化的脂质A和LPS的量显著降低的LPS的形成中具有遗传缺陷的活的革兰氏阴性生物体(无论施用活的或减毒的)或以杀死的生物体在癌症的治疗中可为有效的,从而防止在哺乳动物宿主中的进一步增殖。虽然此类生物体缺乏引起内毒素休克以及为宿主的免疫系统提供刺激的功能性LPS分子,但可以认为,存在将刺激宿主的先天性或组合的先天性和适应性免疫响应的革兰氏阴性细菌的其它特征以实现肿瘤细胞杀死或肿瘤生长抑制。
在一个实施方式中,在癌症治疗中使用的如本文公开的革兰氏阴性生物体不含有编码或表达非细菌蛋白质(如,肿瘤特异性抗原)的DNA。因此,革兰氏阴性生物体不为癌症疫苗,因为它们不直接诱导针对肿瘤抗原的特异性免疫响应。反之,这些生物体以佐剂或生物响应修饰剂(BRM)发挥作用,所述佐剂或生物响应修饰剂(BRM)通常可以刺激宿主先天性免疫响应以及可能间接地刺激适应性抗肿瘤免疫响应。在一些实施方式中,将革兰氏阴性生物体直接注射在肿瘤位点或肿瘤位点附近,或全身注射并在肿瘤中或肿瘤附近积聚。然后,针对生物体的增加的先天性免疫响应可以继发性变成针对肿瘤。另外,或可选地,针对生物体的免疫响应可以刺激或激活能够参与适应性抗肿瘤响应的预存肿瘤抗原-特异性免疫细胞。
在可选的实施方式中,革兰氏阴性生物体表达编码非细菌蛋白质,包括,例如,肿瘤特异性抗原或刺激免疫系统的蛋白质的表达的DNA。在这里再一次地,可以将生物体再次注入肿瘤位点或肿瘤位点附近,或全身注入以及诱导针对生物体、肿瘤特异性抗原或两者的先天性或适应性免疫响应。
如本文所用,术语肿瘤特异性抗原是指由肿瘤表达但来自源于肿瘤的生物体的任何正常细胞不表达的抗原。术语肿瘤相关的抗原是指由肿瘤表达但也可以通过来自源于肿瘤的生物体的正常细胞以限制方式表达的抗原。限制方式的表达可以反映在正常细胞中的表达水平比在肿瘤中的表达水平低,由限制类型的正常细胞表达或仅在胚胎发育期间由正常细胞表达(即,胚胎抗原)。如本文所用,抗原为可由免疫响应,抗体或由免疫细胞(如,T细胞)识别的任何分子。
如本文所用,术语“佐剂”和“生物响应修饰剂”是指增强对抗原、肿瘤或肿瘤相关的细胞的免疫响应的任何物质。因此,佐剂或生物响应修饰剂被用于刺激免疫系统以更剧烈地响应外源抗原或引起疾病的或疾病相关的细胞表达新的抗原,或结构上改变的或异常水平的现存抗原。然而,在一些实施方式中,设想表达,如,肿瘤特异性抗原或肿瘤相关的抗原或人免疫激活蛋白诸如细胞因子或趋化因子的重组形式的革兰氏阴性细菌用于公开的方法中。在可选的实施方式中,在施用前将纯化的免疫激活蛋白质诸如细胞因子或趋化因子与革兰氏阴性生物体混合,或将纯化的免疫激活蛋白质诸如细胞因子或趋化因子施用在革兰氏阴性生物体之前或之后。
如本文所用,术语哺乳动物包括任何哺乳动物,例如人、犬、猫、牛、绵羊等。优选的哺乳动物为人。
术语“革兰氏阴性细菌”是指未保留作为革兰氏染色已知的部分程序的最初碱性染料染色剂(如,结晶紫)的细菌。在示例性革兰氏染色中,首先通过加热将细胞固定在载玻片上并用碱性染料(如,结晶紫)染色,所述碱性染料被革兰氏阴性细菌和革兰氏阳性细菌两者吸收。然后,将载玻片用媒染剂(如,革兰氏碘液)处理,所述媒染剂结合于碱性染料(如,结晶紫)并将其截留在细胞中。然后,将细胞用丙酮或乙醇洗涤,然后用不同颜色的第二染料复染色(如,番红)。革兰氏阳性生物体保留最初的紫色染色,而革兰氏阴性生物体通过有机洗涤溶剂脱色,因此显示复染色。示例性革兰氏阴性细菌包括但不限于埃希氏菌属种、志贺氏属种、沙门氏菌属种、弯曲菌属种、奈瑟氏菌属种、嗜血杆菌属种、气单胞菌属种、弗朗西斯氏菌属种、耶尔森菌属某种、克雷伯菌属某种、博德特氏菌属某种、军团菌属种、棒状杆菌属种、柠檬酸杆菌属种、衣原体属种、布鲁氏菌属种、假单胞菌属种、螺杆菌属种和弧菌属种的细菌。
革兰氏阴性生物体内的为肠杆菌科,其为大的家族,包括连同许多无害的共生体一起的许多熟知的病原菌,诸如沙门氏菌属、埃希氏菌属、鼠疫耶尔森氏杆菌、克雷伯杆菌属(Klebsiella)和志贺氏菌属、变形杆菌属、肠杆菌属、沙雷氏菌属和柠檬酸杆菌属的细菌。肠杆菌科的成员已经被称为肠道菌,如动物肠道内居住的若干成员。
肠杆菌科为杆状的,长度通常为1μm至5μm。它们为兼性厌氧菌,其发酵糖类以产生乳酸和各种其它的终产物。大部分也将硝酸盐还原成亚硝酸盐并通常缺乏细胞色素C氧化酶。大部分具有许多针对运动性的鞭毛,但一些为不动的。肠杆菌科为不产生孢子的。
术语“载体”是指核酸分子,其能够转运以单个核酸连接的另一核酸分子。能够定向其可操作地连接至表达基因的载体在本文被称为“表达载体”。本文所用的术语“表达系统”是指使在表达载体中的序列能够转录成RNA,折叠成结构RNA或翻译成蛋白质的组分的组合。表达系统可以为体外表达系统,诸如可商购获得的或根据已知的方法容易制备,或可以为体内表达系统,诸如含有表达载体的真核或原核宿主细胞。通常,可用于重组DNA技术的表达载体可为“质粒”,其通常是指环状双链DNA,呈其载体形式的所述DNA不结合于细菌染色体。本领域熟知的其它表达载体也可用于表达系统(如,粘粒、噬菌粒和噬菌体载体)。
术语“核酸”是指多核苷酸或寡核苷酸,诸如脱氧核糖核酸(DNA),以及,在适当的情况下,核糖核酸(RNA)。也应理解术语包括作为由核苷酸类似物制备的RNA或DNA的等同物、类似物以及作为适用于正描述的单链(正义或反义)和双链多核苷酸的实施方式。

A.候选细菌生物体
可以通过本文方法使用的候选细菌生物体为革兰氏阴性的并来源于具有作为野生型生物体的内毒素活性的那些。示例性革兰氏阴性细菌包括但不限于埃希氏菌属种、志贺氏属种、沙门氏菌属种、弯曲菌属种、奈瑟氏菌属种、嗜血杆菌属种、气单胞菌属种、弗朗西斯氏菌属种、耶尔森菌属某种、克雷伯菌属某种、博德特氏菌属某种、军团菌属种、棒状杆菌属种、柠檬酸杆菌属种、衣原体属种、布鲁氏菌属种、假单胞菌属种、螺杆菌属种和弧菌属种。候选革兰氏阴性生物体也可以是落入肠杆菌科、假单胞菌科、奈瑟氏菌科、韦荣球菌科、拟杆菌科、弧菌科、巴斯德氏菌科和梭形菌科(Fusobacteriaceae)家族的那些。在一些实施方式中,候选生物体为沙门氏菌属或埃希氏菌属种的物种。
一种候选沙门氏菌属生物体,VNP20009,已经由Luo等人,Oncol Res.12(11-12):501-8,2001描述。VNP20009为删除msbB和purI基因座的鼠伤寒沙门菌的遗传修饰的菌株。将剂量范围为1x 104至3x 106cfu/小鼠的活的VNP20009静脉内施用至具有肿瘤的小鼠抑制皮下植入的B16F10鼠黑素瘤以及人肿瘤异种移植物Lox、DLD-1、A549、WiDr、HTB177和MDA-MB-231的生长。静脉内给予的VNP20009也抑制这些动物中肺转移瘤的生长。也参见美国专利第7,354,592号(Bermudes等人)。
另一候选沙门氏菌生物体为由Eisenstein等人Med.Oncol.12(2):103-8,1995描述的SL3235。SL3235为沙门氏菌属的减毒菌株,当施用活的时可治愈小鼠中浆细胞瘤肿瘤生长。
其它候选沙门氏菌包括由Hoffman等人,Amino Acids 37:509-521,2009报道的营养缺陷型突变体。鼠伤寒沙门菌A1-R突变体为leu-arg营养缺陷型并且具有高的抗肿瘤毒力。在体外,A1-R感染肿瘤细胞并引起细胞核破坏。A1-R施用治疗原位植入裸鼠的转移性人前列腺肿瘤和乳腺肿瘤。静脉内(i.v.)施用至具有原发性骨肉瘤和肺转移瘤的裸鼠的A1-R为有效的,特别是对抗转移瘤。当脾内施用至裸鼠时,也报道A1-R有效对抗胰腺癌肝脏转移瘤。也参见美国专利公布20090300779(Zhao等人)和Zhao等人(Proc.Natl.Acad.Sci.(USA)102(3):775-760,2005)。
适合于治疗实体瘤的多种革兰氏阴性生物体在美国专利第6,685,935号中报道(Pawelek等人)。这些生物体被称为超感染性的,因为它们优先在施用后的肿瘤内复制。包括沙门氏菌属种,如鼠伤寒沙门菌的超感染性的、肿瘤-特异性突变体。还描述了含有自杀基因诸如来自单纯性疱疹病毒的胸苷激酶、来自大肠杆菌的胞嘧啶脱氨酶或人微粒体p450氧化还原酶的沙门氏菌属种的超感染性的、肿瘤-特异性突变体。也参见Pawelek等人,(Lancet Oncol.4(9):548-56,2003)。
在一个实施方式中,选择大肠杆菌作为生物体。所设想的一个特定的菌株为大肠杆菌菌株2617-143-312,(Migula)Castellani和Chalmers(13070TM)。可以使用的另外的大肠杆菌菌株包括MG1655(47076)和KY8284(21272)。
在本文的方法中使用的革兰氏阴性生物体不需要为含有或表达对生物体的野生型形式而言为外源的DNA的重组生物体。然而,在一些实施方式中,可以修饰生物体以表达一些非天然分子。例如,美国专利第7,452,531号报道了用于将一种或多种初级效应分子递送至实体瘤位置的减毒的肿瘤-靶向的细菌载体的制备和使用。根据所述方法,通过对宿主具有降低的毒性的减毒的肿瘤-靶向的细菌可将效应分子局部递送至肿瘤,当全身施用于宿主时,所述效应分子可以为毒性的。具体地说,减毒的肿瘤-靶向的细菌可为兼性需氧菌或兼性厌氧菌,其可被修饰以编码一种或多种初级效应分子。初级效应分子包括TNF细胞因子家族、抗血管生成因子和细胞毒性多肽或肽的成员。
公开的初级效应分子可用于,例如,治疗实体瘤癌症,诸如癌、黑素瘤、淋巴瘤、肉瘤或来源于这些肿瘤的转移瘤。
B.降低细菌内毒素活性
可以使用各种方法以降低细菌生物体的内毒素活性和/或致热原性。如本文所用,术语“内毒素活性”是指可引起毒性,包括致热原性和败血性休克的革兰氏阴性细菌部分。已经发现归因于内毒素的毒性作用与存在于或来源于革兰氏阴性细菌的外膜的脂多糖分子的糖基化的脂质A部分相关。
术语“脂多糖”(LPS)是指由脂质和通过共价键连接的多糖(糖磷脂)组成的大分子。LPS包括三部分:1)O抗原;2)核心寡糖,以及3)脂质A。O-抗原为连接于核心寡糖的重复性聚糖聚合物,并包括LPS分子的最外层结构域。核心寡糖直接连接于脂质A并通常包含诸如庚糖和3-脱氧-D-甘露辛酮糖酸(也称为KDO,酮脱氧辛糖酸盐)的糖。脂质A为连接于多个脂肪酸的磷酸化葡糖胺二糖。脂肪酸将LPS锚定入细菌膜,剩余的LPS从细胞表面伸出。如果LPS突变或被去除,则可产生细胞死亡。
内毒素活性存在于LPS的脂质A结构域部分。当细菌细胞被免疫系统溶解时,含有脂质A的膜的片段被释放至循环,从而引起发烧(致热原性)、腹泻以及可能致命的休克(被称为内毒素休克或败血性休克)。LPS的毒性由通过B-细胞与哺乳动物免疫系统的巨噬细胞之间的相互作用,即导致可对宿主具有致命后果的促炎细胞因子,主要为肿瘤坏死因子(TNF)的分泌的过程的脂质A来表达。脂质A也体外激活人T-淋巴细胞(Th-1)以及体内激活鼠CD4+和CD8+T细胞,即,使宿主的免疫系统对LPS的可变大小的糖链引发特异性、记忆的IgG抗体响应的性质。在此基础上,最近已经将LPS识别为“体内”T细胞依赖性抗原。
内毒素活性可通过本领域熟知的方法测量,包括,例如,鲎变形细胞溶解物(Limulus Amebocyte Lysate)(LAL)测定,其利用来自鲎的血液,可检测非常低水平的LPS。内毒素活性的存在将导致由于经由酶的级联而扩增引起的鲎血液溶解物的凝固。LAL测定的凝胶凝血、浊度法(turbidometric)以及显色形式可商购获得的。参见,如,Lonza,Allendale,NJ和Clongen Labs,Germantown,MD。
基于酶联免疫吸附测定(ELISA)的内毒素活性测定也为已知的,诸如来自德国慕尼黑地区的Hyglos的该测定使用连接于固相的LPS特异性噬菌体蛋白质以捕获LPS,随后进行洗涤步骤,LPS的存在通过加入重组因子C确定,当由LPS激活时,所述重组因子C切割化合物,然后所述化合物发出荧光。在鲎变形细胞溶解物中存在的因子C通常以酶原存在,并且为在LAL测试中发生的凝固级联的引物。
内毒素活性也可通过评估来自体外的原始外周血单核细胞的TNF-α分泌的诱导或通过用疑似的内毒素源处理动物并约1至4小时后测量从动物中获得的血浆中的TNF-α水平来测量。原始哺乳动物外周血单核细胞可从诸如Lonza(Allendale,NJ,USA)的公司购买。细胞上清液或血浆中的TNF-α水平可用ELISA药剂盒,诸如从Thermo Scientific(Rockford,IL,USA)、Abcam(Cambridge,MA,USA)或eBioscience(San Diego,CA,USA)获得的那些确定。

与野生型生物体的内毒素活性和/或致热原性相比,革兰氏阴性生物体的内毒素活性和/或致热原性可大体上降低。内毒素活性的显著降低优选大于约70%、大于约75%、大于约80%、大于约85%、大于约90%、大于95%和大于约99%。
多种方法可获得用于降低革兰氏阴性生物体的内毒素活性。这些方法包括用结合于LPS或破坏其形成的药剂处理生物体或通过基因操作细菌生物体以修饰LPS或抑制LPS形成。
在一个实施方式中,内毒素活性或致热原性的降低通过用使内毒素失活的抗生素处理细菌生物体来实现。合适的此类抗生素为多粘菌素B或多粘菌素E。例如,Cooperstock等人,Infect Immun.1981Jul;33(1):315-8报道了多粘菌素B处理可降低革兰氏阴性细菌,包括百日咳杆菌(Bordetella pertussis)、大肠杆菌、流感嗜血杆菌和铜绿假单胞菌的疫苗中LPS的炎性反应性。确定用于处理的抗生素的量以及条件在本领域技术人员范围内。在一个实施方式中,可以以约3微克至5,000微克/1x107至5x1010个细菌/毫升的浓度使用多粘菌素(多粘菌素B或多粘菌素E)。在另一实施方式中,多粘菌素的浓度可以为约200微克至5,000微克/1x107至5x1010细菌/毫升。在一个实施方式中,将抗生素应用于细菌保持10分钟至4小时或约30分钟至约3小时。在一个实施方式中,在存在呈MgCl2形式的镁(Mg)的情况下并在存在MgCl2的情况下用多粘菌素处理以及在适合于维持细菌完整性的温度下使细菌生长。在一个实施方式中,生长培养基中MgCl2的浓度为约0.5mM至约5.0mM或约2mM,而处理培养基中MgCl2的浓度为约5.0mM至约30mM或约20mM。在一个实施方式中,处理培养基的温度为约2℃至约10℃,或约4℃。在3,000x g下离心10分钟后通过在明确界定的沉淀中回收的效率以及通过电子显微镜确定细菌完整性。在优选的实施方式中,处理和洗涤后的细菌回收率大于约80%并且通过电子显微镜细菌看起来完整的。
在另一实施方式中,通过用已知破坏KDO2-脂质IVA的生物合成的抗生素处理细菌生物体来实现内毒素活性的降低。例如,Goldman等人,J Bacteriol.170(5):2185-91,1988描述了抗菌剂,包括抗菌剂III,其特异性地抑制CTP:CMP-3-脱氧-D-甘露-辛酮糖酸胞苷酰转移酶活性并可用于阻断3-脱氧-D-甘露-辛酮糖酸(KDO)掺入革兰氏阴性生物体的LPS。随着LPS合成停止,细菌生长停止。将KDO加入至LPS前体物质脂质IVA为鼠伤寒沙门菌和大肠杆菌两者中脂质A-KDO形成的主要途径。在一个实施方式中,抗生素为抗菌剂III并用合适的量,诸如,例如5微克/毫升至500微克/毫升处理革兰氏阴性细菌保持合适的时间,例如2小时至8小时。
内毒素活性的降低可以通过将遗传缺陷引入生物体来实现。本文使用的关于基因或基因表达的术语“缺陷的”意指基因不同于正常(野生型)基因或与野生型基因的表达相比,基因的表达处于较低水平的表达。缺陷基因可以由该基因的突变或调控该基因的表达的突变(如,转录或转录后)引起。
在一个实施方式中,内毒素活性的降低可以通过引入破坏KDO2-脂质IVA的生物合成的遗传缺陷来实现。例如,Woodard等人,美国专利公布20100272758,报道了外膜中大体上缺乏LPS的活性非毒性革兰氏阴性细菌(如,大肠杆菌)。作者描述了如在3-脱氧-d-甘露-辛酮糖酸(Kdo)的合成中有缺陷的大肠杆菌K-12菌株KPM22。KPM22具有主要由脂质IVA,即缺乏糖基化的LPS前体组成的外膜(OM)。生物体的活力通过将来自内膜(IM)的脂质IVA转运至外膜的次位抑制物的存在来实现。该抑制物被报道减轻内膜中脂质IVA积聚的毒性副作用并提供足量的LPS前体以支持OM生物发生。也参见,Mamat等人,(Mol Microbiol.67(3):633-48,2008)。
在另一实施方式中,Bramhill等人,美国专利公布2011-0224097描述了包含外膜的活性革兰氏阴性细菌,所述外膜大体上缺乏配体,诸如充当TLR4/MD2的激动剂的脂质A或6-酰基脂多糖。根据Bramhill,细菌可以例如在转运蛋白中包含降低活性的阿拉伯糖-5-磷酸异构酶以及一种或多种抑制突变,从而增加转运蛋白转运脂质IVA的能力或细菌可以例如在膜蛋白YhjD中包含降低活性的阿拉伯糖-5-磷酸异构酶以及一种或多种抑制突变。可以大体上删除一种或多种基因(如,IpxL、IpxM、pagP、IpxP和/或eptA)和/或可以大体上失活一种或多种酶(如,LpxL、LpxM、PagP、LpxP和/或EptA)。
在另一实施方式中,内毒素活性的降低可以通过引入防止Kdo合成的遗传缺陷来实现。例如,Rick等人,(Proc Natl Acad Sci U S A.69(12):3756-60,1972)报道了在LPS的3-脱氧-D-甘露辛酮糖酸(酮脱氧辛酸)区域的合成中有缺陷且需要D-阿拉伯糖-5-磷酸用于生长的鼠伤寒沙门菌的营养缺陷型突变体。突变缺陷是由于改变的酮脱氧辛酸-8-磷酸合成酶(kdsA)对D-阿拉伯糖-5-磷酸的表观K(m)比亲本酶对D-阿拉伯糖-5-磷酸的表观K(m)高35倍。这导致突变菌株的生长以及完整LPS的合成菌依赖于外源的D-阿拉伯糖-5-磷酸。在另一实例中,Belunis等人(J.Biol.Chem.270(46):27646-27652,1995)破坏大肠杆菌中的Kdo转移酶(kdtA)基因,这防止了Kdo掺入脂质IVA。该突变为致死的,但可通过编码kdtA的温敏质粒的条件性存在拯救。在Kdo合成途径中条件突变体的形成允许细菌生长,随后转移至非允许条件,从而导致充分的生长或存活以产生内毒素活性显著降低的的非活性细菌。
除LPS-来源的内毒素之外,革兰氏阴性生物体的各种其它成分可诱导或有助于致热原性和败血性休克,包括外膜蛋白质、纤毛、性菌毛、脂肽和脂蛋白(由Jones,M.,Int.J.Pharm.Compd.,5(4):259-263,2001综述)。致热原性可通过本领域熟知的家兔法测量,包括静脉内施用假定的致热源后评价直肠温度。
已经发现用多粘菌素B和戊二醛的组合处理革兰氏阴性生物体产生的致热原性减少30倍,如在家兔中测量。在一个实施方式中,使用1,000微克/毫升(μg/mL)的多粘菌素B以及1%的戊二醛以产生减少30倍的致热原性,如在家兔中测量。通过多粘菌素B与LPS的反应以及戊二醛与LPS的反应性和/或其它细菌成分的组合降低致热原性。戊二醛在该环境中通过也杀死细菌起到双重作用。因此,在一个实施方式中提供了降低内毒素活性和致热原性以及通过用1,000μg/mL的多粘菌素B和1%的戊二醛的组合处理所述细菌而杀死革兰氏阴性细菌微生物的方法。在另一实施方式中,用剂量范围为约3μg/mL至约1,000μg/mL的多粘菌素B以及剂量范围为约0.1%至约1.0%的戊二醛的组合处理革兰氏阴性细菌。在进一步实施方式中,多粘菌素B的剂量范围为约100μg/mL至约1,000μg/mL,并且戊二醛的剂量范围为约0.5%至约1.0%。另外,可以,例如用剂量范围为约1,000μg/mL至约3,000μg/mL的多粘菌素B并且剂量范围为约0.5%至约1.0%的戊二醛处理革兰氏阴性细菌。在另一方面,可以,例如用剂量范围为约3,000μg/mL至约5,000μg/mL的多粘菌素B并且剂量范围为约0.5%至约2.0%的戊二醛处理革兰氏阴性细菌。在一个实施方式中,内毒素活性降低约70%、或约75%、或约80%、或约85%、或约90%、或约92%,并且致热原性降低约75%、或约80%、或约85%、或约90%、或约95%、或约97%。
C.使细菌变成非活性的
根据公开的方法用于施用的细菌在施用前变成非活性的或变成大体上非活性的或在施用后变成非活性的。“非活性”意指通过用外源药剂处理杀死生物体,和/或含有导致生物体不能在哺乳动物宿主中存活的突变。大体上非活性细菌为其活力降低至少80%、85%、90%、95%、99%或更多的菌株。在未被杀死或未被完全杀死的细菌的优选实施方式中,将进一步处理或修饰细菌使得它们不能在哺乳动物宿主内增殖。在大体上不产生LPS的一些实施方式中,设想施用非活性、减毒的或活性细菌。
使细菌变成非活性的优选方法为用结合于LPS的化合物处理,从而阻断其内毒素活性,或用干扰LPS生物合成的化合物处理。在两种情况下,LPS结合以及干扰LPS合成,由于细胞被膜的透化导致活力降低。另一方法为使在生长期间抑制LPS生物合成途径中的条件突变的细菌菌株生长,然后转移至激活突变并破坏LPS生物合成的非允许条件。在每种情况下,应用的程序为通过在每种环境中确定处理的最佳时间或化合物的剂量使细菌变成非活性的,使得活力已经大体上丧失同时保留显著的细菌细胞完整性。在非活力小于100%的情况下,可使用含有防止活细菌在哺乳动物宿主中进一步增殖的突变的细菌(如二氨基庚二酸营养缺陷型,如由Bukhari和Taylor,J.Bacteriol.105(3):844-854,1971以及Curtiss等人,Immunol.Invest.18(1-4):583-596,1989描述)。
如果期望使细菌变成非活性的替代性方法或另外的方法,用于杀死细菌的优选的方法为电离辐射(γ射线或电子束),但也可通过其它标准的灭菌方法诸如湿热灭菌或干热灭菌、消毒气体或蒸汽进行(参见,如Shintani等人,Biocontrol Science,16(3):85-94,2011)。可使用的最终灭菌的另外的非标准方法包括化学处理诸如化学杀菌剂,并由Rutala和Weber(Emerg.Infect.Dis.7(2):348-353,2001)以及Yaman(Curr.Opin.DrugDiscov.Develop.4(6):760-763,2001)概述。化学气体、蒸汽和液体杀菌剂的实例包括环氧乙烷气体(EOG)、二氧化氯、液态过氧化氢的蒸汽相(VHP)、甲醛、戊二醛(如,≥0.05%保持≥10分钟)、邻-苯二醛(OPA)(如≥0.1%保持≥5分钟)和苯酚。杀死细菌的方法可影响生物体的完整性。例如,与辐射的使用相反,加热可损害细菌完整性。提及本文所用的细菌生物体包括完全完整的生物体以及当杀死生物体时可出现的部分降解形式的生物体,但并不延伸至已与其它细胞组分,诸如细胞壁部分(制备)或细胞壁骨架(参见如,美国专利No.4,436,727)、细胞质部分等分离的生物体的亚细胞组分。
D.组合物
在一个实施方式中,提供了组合物,其包含内毒素和/或热原活性显著降低的非活性革兰氏阴性细菌生物体以及药学上可接受的赋形剂。在另一实施方式中,至少约80%的生物体为非活性的或至少约90%的生物体为非活性的,或约100%的生物体为非活性的。在一个实施方式中,生物体具有其降低约80%、或约85%、或约90%、或约95%或约100%的活力。
在一个实施方式中,内毒素和/或热原活性降低约70%、或约75%、或约80%、或约85%、或约90%、或约95%。组合物可以含有任何预期量的非活性或活力降低的生物体与内毒素或热原毒性的任何预期降低的组合。在另一实施方式中,组合物包含至少约100%的非活性生物体,其具有至少约95%的降低的内毒素活性和致热原性。
可以以多种方式配制本文所述的组合物用于本文所述的方法中。在一个实施方式中,组合物包含如通篇所述的生物体以及药学上可接受的载体。
“药学上可接受的载体”是指可以在组合物中使用的任何稀释剂、赋形剂或载体。药学上可接受的载体包括离子交换剂、氧化铝、硬脂酸铝、卵磷脂、血清蛋白(诸如人血清白蛋白)、缓冲物质(诸如磷酸盐)、甘氨酸、山梨酸、山梨酸钾、饱和的植物脂肪酸、水、盐或电解质的部分甘油酯的混合物诸如鱼精蛋白硫酸盐、磷酸氢二钠、磷酸氢钾、氯化钠、锌盐、硅胶、三硅酸镁、聚乙烯吡咯烷酮、基于纤维素的物质、聚乙二醇、羧甲基纤维素钠、聚丙烯酸酯、蜡、聚乙烯聚丙烯嵌段聚合物、聚乙二醇和羊毛脂。合适的药物载体在《雷氏药学大全》(Remington's Pharmaceutical Sciences)(本领域的标准参考教科书),Mack PublishingCompany中描述。选择它们的旨在施用形式,即,口服片剂、胶囊、酏剂、糖浆等,并与常规药物实践相一致。
药物组合物可以通过本领域熟知的方法制造,诸如发酵罐中微生物生长,随后通过离心浓缩并洗涤、过滤或透析、常规造粒、混合、溶解、包封、冻干或乳化过程及其它方法。可以以各种形式产生组合物,包括颗粒、沉淀或微粒、粉末,包括冷冻干燥、旋转干燥或喷雾干燥的粉末、无定形粉末、注射剂、乳液、酏剂、混悬剂或溶液。制剂可以任选地含有稳定剂、pH调节剂、表面活性剂、生物利用率调节剂以及这些的组合。
可以使用无菌液体,诸如油、水、乙醇及其组合以液体混悬剂或溶液制备药物组合物。可以添加药学上合适的表面活性剂、悬浮剂或乳化剂用于口服或胃肠外施用。混悬剂可以包括油,诸如花生油、芝麻油、棉籽油、玉米油和橄榄油。混悬剂制剂还可含有脂肪酸的酯诸如油酸乙酯、肉豆蔻酸异丙酯、脂肪酸甘油酯和乙酰化的脂肪酸甘油酯。混悬剂制剂可以包括醇,诸如乙醇、异丙基醇、十六烷醇、甘油和丙二醇。在混悬剂制剂中也可以使用醚(诸如聚(乙二醇)),石油烃(诸如矿物油和矿脂),和水。
将组合物配制用于至哺乳动物,优选为人的药物施用。本发明的此类药物组合物可以以多种方式施用,包括胃肠外。本文所用的术语“胃肠外”包括皮下、静脉内、肌肉内、关节内、滑膜内、胸骨内、鞘内、肝内、病灶内及颅内注射或输注技术。
组合物的无菌可注射形式可以为水性混悬剂或油性混悬剂。这些混悬剂可以根据本领域已知的技术使用合适的分散剂或湿润剂和助悬剂配制。无菌可注射制剂也可以为无毒、肠胃外可接受的稀释剂或溶剂中的无菌可注射的溶液或混悬剂,诸如1,3-丁二醇中的溶液。可以使用的可接受的媒介物和溶剂为水、林格氏溶液(Ringer's solution)和等渗氯化钠溶液。另外,通常将无菌、不挥发性油用作溶剂或助悬介质。为此目的,可以采用任何温和不挥发性油,包括合成的单甘油酯或二甘油酯。与天然药学上可接受的油诸如橄榄油或蓖麻油(尤其呈聚氧乙基化形式)一样,脂肪酸诸如油酸及其甘油酯衍生物可用于制备注射液。这些油溶液或混悬液还可以含有长链醇稀释剂或分散剂,诸如羧甲基纤维素或类似的分散剂,其通常用于配制药学上可接受的剂型(包括乳液和混悬液)。其它常用的表面活性剂诸如Tweens,Spans及其它常用于制造药学上可接受的固体、液体或其它剂型的乳化剂或生物利用率增强剂也可以用于配制的目的。化合物可以配制用于通过注射,诸如通过一次性注射或连续输注进行胃肠外施用。
E.治疗癌症的方法
适合通过本文方法治疗的癌症通常包括癌、白血病或淋巴瘤以及肉瘤。癌可以为肛门癌、胆道癌、膀胱癌、乳腺癌、结肠癌、直肠癌、肺癌、口咽癌、下咽癌、食道癌、胃癌、胰腺癌、肝癌、肾癌、胆囊癌和胆管癌、小肠癌、尿道癌、女性生殖道癌、男性生殖道癌、内分泌腺癌、甲状腺癌和皮肤癌。其它合适的癌症包括类癌瘤、胃肠道间质瘤、头颈肿瘤、未知的原发性肿瘤、血管瘤、黑素瘤、恶性间皮瘤、多发性骨髓瘤以及脑肿瘤、神经肿瘤、眼肿瘤和脑膜肿瘤。
在一些实施方式中,待治疗的癌症形式为实体瘤,诸如癌、肉瘤、黑素瘤和淋巴瘤。
如本文所述的癌症治疗通过施用足以抑制癌症生长或转移的一定数量的革兰氏阴性(视情况为活的或死的)生物体实现。如本文所用,短语“足够量”是指足以赋予其接受者有益作用的剂量(或系列剂量)。针对任何特定受试者的特异性治疗有效剂量水平将取决于多种因素,包括正治疗的癌症类型、癌症的严重度、特异性生物体或组合组合物的活性、施用途径、生物体或组合组合物的清除率、治疗的持续时间、与生物体组合使用的药物(如果有的话)、受试者的年龄、体重、性别、饮食和一般健康状况以及医学领域和科学熟知的类似的因素。在确定“治疗有效量”中考虑到的各种一般考虑因素对本领域技术人员为已知的并且描述于如,Gilman等人,eds.,Goodman And Gilman's:The Pharmacological Basesof Therapeutics,第8版,Pergamon Press,1990;以及Remington's PharmaceuticalSciences,第17版,Mack Publishing Co.,Easton,Pa.,1990。剂量水平通常落入约0.001之上至100mg/kg/天的范围内;化合物通常可适用的水平范围为约0.05之上至10mg/kg/天。施用的生物体的剂量水平通常落入约106至1012/m2的范围内。可胃肠外,诸如血管内、静脉内、动脉内、肌内、皮下、口服等施用组合物。可胃肠外,诸如血管内、静脉内、动脉内、肌内、皮下、腹膜内或膀胱内施用细菌生物体。
治疗有效剂量可通过领域熟知的方法估计。癌症动物模型诸如具有鼠肿瘤的免疫感受态小鼠或具有人肿瘤异种移植物的免疫缺陷小鼠(如裸鼠)为本领域熟知的并在本文供参考并入的许多参考文献中广泛描述。将此类信息与大鼠、犬和/或非人灵长类中的安全性研究组合使用以便确定人中的安全且可能有用的最初剂量。用于估计生物体的剂量的另外的信息可来自实际人癌的研究。例如,Toso等人(J Clin Oncol.20(1):142-52,2002)报道了I期临床试验,其中将活的VNP20009施用至患有转移性黑素瘤的患者。患者接受含有10(6)至10(9)cfu/m(2)VNP20009的30分钟静脉内推注输注。最大耐受剂量为3x 10(8)cfu/m(2)。剂量限制性毒性在接受1x 10(9)cfu/m(2)的患者中观察到,所述剂量限制性毒性包括血小板减少、贫血、持续性菌血症、高胆红素血症、腹泻、呕吐、恶心、升高的碱性磷酸酶和血磷酸盐过少。
可以将生物体以药学上可接受的制剂施用。术语“药学上可接受的”意指物质是非生物的或者不期望的,即,可以将所述物质与所选择的生物体或组合的化合物一起施用于个体而不会引起任何不期望的生物学效应或与任何其它施用的药剂以有害方式相互作用。这在上文进行了更充分地描述。
如本文所用,术语“治疗”受试者的病患或疾病旨在涵盖治愈以及缓解病患或疾病的至少一种症状。如果患者的癌症得以治愈,则癌症患者得到治疗,癌症进入缓解,存活率以统计上显著的方式延长,肿瘤进展的时间以统计上显著的方式增加,基于针对每种类型的淋巴细胞性或造血性恶性肿瘤建立的标准淋巴细胞性或造血性肿瘤负荷存在降低,或实体瘤负荷已经如通过响应实体瘤中的评估标准所定义的有所降低(RECIST 1.0或RECIST1.1,Therasse等人J Natl.Cancer Inst.92(3):205-216,2000以及Eisenhauer等人Eur.J.Cancer 45:228-247,2009)。如本文所用,“缓解”是指在先前患有癌症迹象的患者中不存在生长的癌细胞。因此,在缓解中的癌症患者或者其癌症得以治愈或癌症虽存在但不易于检测。因此,当肿瘤未能增大或转移时,癌症可以缓解。如本文所用的完全缓解为如通过诊断方法,诸如成像,诸如x-射线、MRI、CT和PET,或血液或骨髓活检所指示的不存在疾病。当癌症患者进入缓解时,这可能随后复发,在那癌症再现。
除非另外指示,否则术语“大体上”意指大于约80%、大于约90%、大于约95%以及大于约99%。
F.治疗癌症的组合
本文所述的癌症治疗的方法可以使用革兰氏阴性生物体连同受体或配体的一种或多种拮抗剂一起施用,所述拮抗剂负调节宿主免疫响应。拮抗剂可以涉及PD-1、PD-L1或CTLA-4并且通常例如以每1周至4周的剂量范围为约0.03毫克/千克至约30毫克/千克静脉内施用。
程序性细胞死亡蛋白质1(PD-1)为在人中由PDCD1基因编码的蛋白质。PD-1也已经被命名为CD279(分化279的集群)。PD-1为268个氨基酸的I型膜蛋白。PD-1为T细胞调节因子的扩展的CD28/CTLA-4家族的成员。参见,如,Ishida等人,EMBO J.11(11):3887–95,1992。蛋白质含有细胞外IgV结构域,随后跨膜区域和细胞内尾部。细胞内尾部在基于免疫受体酪氨酸的抑制基序和免疫受体酪氨酸基开关基序内含有两个磷酸化位点。这表明PD-1负调控TCR信号传导。PD-1在激活的T细胞、B细胞和巨噬细胞的表面表达。PD-1为免疫响应的广泛负调节因子。

CTLA-4(细胞毒性T-淋巴细胞抗原4),也称为CD152(分化152的集群),为下调免疫系统的蛋白质受体。CTLA-4在辅助、效应子和免疫调节T细胞的表面表达,这致使细胞免疫攻击抗原。通过刺激CD28受体打开T细胞或通过刺激CTLA-4受体关闭受体。CTLA-4(类似于T细胞共刺激蛋白质的CTLA-4)、CD28分别结合于抗原呈递细胞上的CD80和CD86(也被称为B7-1和B7-2)。通过T细胞受体和CD28的T细胞激活导致CTLA-4(B7分子的抑制受体)表达的增加。

本文所述的癌症治疗的方法也可以使用革兰氏阴性生物体连同正调节宿主免疫响应的受体或配体的一种或多种激动剂一起施用。激动剂针对4-1BB(CD137)、GITR、CD40或OX40(CD134)并且可例如以每1至4周的剂量范围为约0.03毫克/千克至约30毫克/千克静脉内施用。

本文所述的癌症治疗的方法也可以使用革兰氏阴性生物体连同一种或多种化疗剂一起施用。此类药剂可以包括环磷酰胺。预期当将环磷酰胺用于本文所述的方法时,可以将其在每天或每21天以5mg/m2至750mg/m2的剂量静脉内或口服施用。可选地,可以例如,以5mg至100mg每天口服的剂量在节拍式方案中施用环磷酰胺。参见,例如,Jia等人,Int.J.Cancer 121(3):666-674,2007。

本文所述的癌症治疗的方法也可以使用连同重组表达的或分离的和纯化的干扰素α一起施用的革兰氏阴性细菌。可以以每周1、3、5或7次的剂量范围为约3x 105至约3x108IU皮下、肌内或静脉内施用干扰素α。在另一实施方式中,革兰氏阴性细菌可以连同干扰素β一起施用。在某些实施方式中,将以每周一次或每隔一天的剂量范围为约0.01毫克至约5毫克皮下或静脉内施用干扰素β。也可以共施用干扰素γ。在一个实施方式中,可以以或一次或每天的剂量范围为约1x 105IU至约1x 109IU皮下或静脉内施用干扰素γ。
在另外的方法中,可以将白细胞介素(如,白细胞介素-2和白细胞介素-12)共施用。在一个实施方式中,可以以每周一次或高达一天三次的约1x 104至约1x 107IU的剂量与革兰氏阴性细菌组合静脉内施用白细胞介素。另外的方法包括每天或每月的通常剂量范围为约5微克至约5毫克皮下、皮内或静脉内例如与粒细胞-巨噬细胞集落刺激因子施用的革兰氏阴性细菌。在通篇记录的任何组合治疗中,预期可以在另外的癌症治疗之前或之后施用生物体。也可以将其同时施用。
以下实施例用于说明本公开。这些实施例决不旨在限制该公开的范围。
实施例
实施例1:
通过在37℃下于磷酸盐缓冲盐水(PBS)中用1-100μg/mL的多粘菌素B将每种细菌菌株的浓缩的晚对数细菌(109至1011/mL)温育2分钟至6小时的不同时间确定使脂多糖相关的内毒素活性失活并通过多粘菌素B杀死细菌细胞而不丧失细胞完整性的最优条件。通过在生长相容的琼脂平板上对对照和处理的细菌混悬剂进行系列稀释涂板,随后温育过夜以及菌落计数确定活力。通过肉眼(显微镜)检查以及600nm处吸光度的分析确定细胞完整性。通过鲎变形细胞溶解物(LAL)检验来确定内毒素活性。可溶或过量多粘菌素和细胞碎片(包括可溶内毒素)通过用0.9%NaCl(生理盐水)的离心介导的洗涤去除。
可选地,用于分离具有来自条件突变的缺陷LPS的完整的、非活性细菌的最优条件如多粘菌素处理来确定,除了细菌在非允许条件下于LB(Lysogeny肉汤)中生长并在不同时间去除,随后如多粘菌素处理所述分析和处理。
将多粘菌素-处理的细菌或盐水洗涤的晚对数期LPS突变体/缺陷细菌使用海藻糖作为冷冻保护剂进行冷冻干燥(参见,如,Leslie等人,App.Environment.Microbiol.61(10):3592-3597,1995;Gu等人,J.Biotech.88:95-105,2001和American Type CultureCollection Bacterial Culture Guide)。如果需要,则在足以将活力降至0%而不会丧失细菌完整性的剂量下通过用电离辐射处理进一步降低细菌活力。
在抗肿瘤研究中使用之前,将冷冻干燥细菌重新悬浮在无菌水中。将PBS洗涤的鼠肿瘤细胞(B16和B16F10黑素瘤,CT-26结直肠癌、Panc02胰腺癌或Lewis肺癌(105-107个细胞,取决于细胞系)在剃毛C57BL/6小鼠的背部皮下植入。将小鼠随机化并当可首次触诊肿瘤时,当肿瘤的平均体积达到75mm3时,或当肿瘤的平均体积达到300mm3时(如通过卡尺测量估计)开始治疗。在以每周一次至两次的单独剂量范围为103至1010/0.1-0.2mL注射体积下经由尾静脉或腹膜内(i.p.)注射重新悬浮的细菌。以每周一次至两次的在单独剂量为3-100微克下i.p.施用针对T细胞受体的抗体拮抗剂或激动剂。在每隔一天高达150mg/kg下保持5天(MTD给药)或在饮用水中25mg/kg/天(节拍式给药)i.p.施用环磷酰胺。对小鼠每周进行两次称重并记录临床观察。每周进行两次肿瘤测量(通过卡尺),并在如果/当肿瘤达到1,000mm3时,变成坏死的或如果观察到≥15%的重量减轻时将小鼠人道地处死。去除肿瘤并称重,并用处死的小鼠进行最小尸检。如果观察到长期肿瘤消退或治愈,则可以用肿瘤细胞植入来重新攻击小鼠。
实施例2:
在实施例2中,使用大肠杆菌菌株2617-143-312(Migula)Castellani和Chalmers(13070TM)。该无害的革兰氏阴性细菌需要外源的二氨基庚二酸(DAP)用于生长。由于哺乳动物不能制备DAP,所以该细菌菌株不为活性的并且不能引起哺乳动物感染。另外,在体外研究期间,DAP营养缺陷型可用于监测污染。在37℃下使细菌在含有2mM MgCl2、0.5%葡萄糖和1mM DAP的LB Miller肉汤中生长至晚对数期(基于O.D.600)同时在300rpm下恒定振荡。通过在2,000x g下离心保持15分钟并在4℃下于含有20mM MgCl2,0.5%葡萄糖和0.1mM DAP(PMB处理培养基)的LB Miller肉汤中重新悬浮将培养物洗涤三次。基于1O.D.600等于1.12x109个细菌/mL,在2x1010个细菌/mL下制备最终重新悬浮。将培养物的单独等分试样在4℃下于不含和含有不同浓度的多粘菌素B(PMB)(Calbiochem#5291)中温育保持1小时同时恒定搅拌。然后,通过在3,000x g离心保持10分钟并在2x109个细菌/mL下重新悬浮细菌用4℃新鲜PMB处理培养基将细菌洗涤三次。通过下述O.D.600监测细菌回收效率。在PMB处理和洗涤后的细菌回收率大于用高达300μg/mLPMB处理的所有样品的90%,并超过用1,000μg/mL PMB处理的80%。
在图1中,通过用鲎变形细胞溶解物(LAL)Endosafe Endochrome-K动力学测定试剂盒(Charles River Endosafe,Charleston,SC)分析未处理的和处理的细菌培养物的系列稀释液确定内毒素活性。未处理的培养物通常含有约50-100内毒素单位/1x106个细菌。在四个独立的实验中针对用1,000μg/mLPMB处理观察到类似的内毒素降低(平均=未处理的17%)。
在图2中,通过将每个样品系列稀释并在含有2mM MgCl2,0.5%葡萄糖,含有和不含1mM DAP的LB Miller琼脂平板(分别监测活力和污染)上涂板确定细菌活力。将板在37℃温育过夜,确定每个平板上的菌落数,然后通过将每个平板上的菌落数乘以稀释因子计算活力。每个混悬剂中细菌的总数目通过O.D.600乘以每1O.D.600的每11.12x109个细菌/mL的转换因子计算。将活力(活细菌%)计算为相对于细菌的总数目的活细菌/mL的百分比。用1,000μg/mLPMB处理将细菌活力降至0%。在随后的放大实验中,在四个独立的实验中,1,000μg PMB将活力降至平均11%。
实施例3:
如实施例2中所述进行实验,除了用含有20mM MgCl2的磷酸盐缓冲盐水(PBS;不含Mg和Ca)pH 7.5进行预处理洗涤、戊二醛(GA)处理和处理后洗涤。在GA处理后在所有测试浓度下的细菌回收率通常为80%至100%。图3证明用1%GA处理将内毒素活性降低96%。相对于未处理的培养基,用1%GA处理的2升放大实验产生的内毒素活性降低82%。
在大于0.05%的剂量下用GA处理始终产生100%的细菌杀死,如在图4中证明。
相对于未处理的培养物(表1),使用2升晚对数期培养物的1,000μg/mLPMB处理随后1%GA处理的组合产生的细菌的活力为0%并且内毒素活性降低92%(12倍)。
实施例4:
在实施例4中,使细菌生长并用如在实施例2和3的方案中所述的1,000μg/mLPMB,1%GA或两者处理。将样品用含有1%GA的PBS,pH 7.5(如果先前未暴露于GA)稀释并固定10分钟。将25微升含有细菌的小滴置于石蜡膜上,然后用100目聚醋酸甲基乙烯脂(formvar)+碳EM网格(EMS,Hatfield,PA)覆盖,将其用0.1%聚-L-赖氨酸预涂覆。使样品粘附保持10分钟,然后通过置于200微升水滴上将网格短暂地洗涤三次。通过在100微升含2%乙酸双氧铀的水的小滴上放置保持1分钟对网格负染色。将过量染色用3M滤纸吸走,随后空气干燥。使用配备有底部安装Eagle 4k(16兆像素)数码相机的FEI Tecnai Spirit G2BioTWIN透射电子显微镜可视化样品(放大倍数为1,200x和11,000x)。在图5B、图5C和图5D中的图像确认根据本方法进行的PMB和/或GA处理保留细菌完整性,这为期望的结果。多糖荚膜在未处理的细菌上为可见的(模糊面)(图5A),但在所有处理的细菌中似乎已经去除或压成一团(图5B、图5C和图5D)。
实施例5:
在实施例5中,使大肠杆菌生长并用1,000μg/mL PMB加1%GA处理,并如实施例2和3所述确定活力和内毒素水平。在最终洗涤后,将未处理的和PMB+GA-处理的细菌以1.1x1011个细菌/mL的浓度重新悬浮在50%PBS,pH 7.5,0.5mM MgCl2,12%海藻糖中,将其等分,快速冷冻并在-80℃储存。基本上如在美国药典,第151章中所述确定致热原性阈值。使用称重为至少2.0kg的成年雌性新西兰白色家兔。在热原测试之前用假测试使所有动物适应不多于7天。用一只家兔/剂量进行剂量范围-发现,随后用两只家兔/剂量确认这些结果。将细菌稀释成用于注射的无菌盐水。经由静脉内途径以10mL体积递送所有剂量。在测试药剂施用的三小时内的任何时间点处产生的温度增加0.5-1.0℃的测试药剂的最低浓度被认为是代表致热原性阈值。在基线处以及在测试药剂注射后1至3小时之间的30分钟间隔处记录直肠温度。用于储存未处理的和处理的大肠杆菌的盐水稀释的媒介物未显示出热原的。将3x104个未处理的细菌施用至两只家兔产生的温度增加0.8和1.0℃。3x105个PMB+GA-处理的细菌的施用未产生增加大于0.1℃的温度,但将9x105个PMB+GA-处理的细菌施用至两只家兔产生的温度增加0.7和1.0℃,从而证明存在30x-倍的致热原性阈值差异。可能的是PMB仅中和脂多糖-介导的热原活性。然而,GA可以中和脂多糖以及细菌的任何其它成分介导的致热原性。
表1证明未处理的细菌以及用1,000μg/mL PMB和1%GA两者处理的细菌的致热原性(发热反应)阈值,如通过标准的体内家兔测试测量。将结果与用体外LAL测定确定的内毒素水平进行比较,显示与未处理的细菌相比,虽然PMB+GA处理将内毒素水平降低12-倍,但由相同样品介导的致热原性降低30倍。

实施例6:
在实施例6中,使大肠杆菌生长并如在实施例2和实施例3的方案中所述用1,000μg/mL多粘菌素B加1%GA处理。将未处理的和处理的细菌的冷冻储液在37℃迅速解冻并稀释至少10-倍成用于注射的无菌盐水(i.v.剂量≤3x109个细菌)或在3,000x g下离心10分钟并重新悬浮在用于注射的无菌盐水中(i.v.剂量≥5x109)。经由尾静脉以100微升体积i.v.注射细菌或媒介物。
使用八周龄的C57BL/6或BALB/c雌性小鼠并在研究之前适应至少7天。每天进行一次或两次死亡率和临床观察。另外的观察在注射时以及注射后1-4小时进行。确认了媒介物缺乏毒性。笼侧观察包括但不限于以下:
皮肤变化、皮毛变化、眼睛变化、粘膜变化、步态变化、姿势变化以及处理出现的分泌物/排泄物或自发性活动诸如流泪、立毛、不同寻常的呼吸模式的其它迹象的响应的变化;出现癫痫发作;一般警觉性的变化;刻板行为诸如过度梳理和重复的转圈;不寻常行为(自残);肿块/隆起物(肿瘤,脓肿等)形成;紧张体征和/或呼吸症状的形成;刺激和发炎体征的注射部位的观察;进食和饮水的变化以及尿和排粪量的变化。
多-剂量研究的细菌施用以每周两次保持两周进行(4次处理)。毒性的评估包括监测动物体重。在多-剂量研究中使用的小鼠报道了1x109个PMB+GA-处理的细菌为具有肿瘤的。表2中报道的所有其它小鼠为不具有肿瘤的。

实施例7:
在实施例7中,将1,000μg/mL PMB+1%GA-处理的细菌(DB103)如在实施例2和实施例3的方案中所述进行制备。将八周龄的雌性C57BL/6J小鼠在注射部位剃毛并在右侧皮下注射2x105个B16F10鼠黑素瘤细胞(ATCC CRL-6475)。三天后经由尾静脉i.v.施用开始处理并每周持续两次保持总计5次处理。将含DB103的50%PBS,pH 7.5,0.5mM MgCl2,12%海藻糖(浓度为1.1x1011/mL)用无菌盐水稀释11-倍(1x109剂量)或220-倍(5x107剂量)并以100微升的最终体积注射。将储液媒介物稀释11-倍用于媒介物对照处理组。用卡尺每周两次测量肿瘤并使用式(长度x宽度2)/2确定肿瘤体积。未观察到化合物相关的死亡。所有动物患有肿瘤,除了用1x109个DB103处理的两只动物。观察到高达3%(低剂量组)和7%(高剂量组)的短暂的体重减轻,但在最后治疗后恢复(图6)。
实施例8:
对于实施例8,将大肠杆菌(未处理的和1%GA-处理的)如在实施例2和实施例3的方案所述制备。将实验如在实施例7的方案所述进行,除了当肿瘤仅可感知时,在第11天开始处理。对于大多数组而言,在24天后未记录组测量,因为由于肿瘤负荷,每个这些组中的动物子集不得不施行安乐死。在所有动物中形成肿瘤。在1x109个GA组中最大的重量减轻为11%。1x109个未处理的大肠杆菌的施用排除毒性(参见表2)。
实施例9:
在实施例9中,将1,000μg PMB+1%GA-处理的细菌(DB103)如实施例2和3的方案所述制备。如实施例7的方案所述进行实验,除了将1x105个鼠CT26结直肠癌细胞皮下注射入BALB/c小鼠右侧。经由尾静脉i.v.施用三天后开始DB103处理并每周持续两次保持总计6次处理。在第3天开始,以~20mg/kg/天(在水中为0.133mg/mL)经由饮用水持续施用环磷酰胺(LKT Laboratories,#C9606)。将含100μg抗-鼠CTLA-4抗体(BioXcell#BE0164)的200微升PBS在第3天、第6天和第9天i.p.施用。每天记录临床观察和死亡率。用卡尺每周两次测量肿瘤并用式(长度x宽度2)/2确定肿瘤体积。在媒介物组中在所有小鼠中形成肿瘤。在任何组中未观察到重量减轻以及化合物相关的死亡。媒介物、低剂量和高剂量DB103组的数据在图8A和图8B中相同。

Claims (24)
1.组合物用于制造用于治疗癌症的药物的用途,其中,所述组合物包含(a)完整的革兰氏阴性细菌细胞以及(b)药学上可接受的赋形剂,所述完整的革兰氏阴性细菌细胞已经用多粘菌素在2℃至10℃的温度进行处理并用戊二醛进行处理使得至少90%的所述细菌细胞为非活性的,并且与未处理的、野生型革兰氏阴性细胞相比,当通过鲎变形细胞溶解物(LAL)测定法测定时,所述细菌细胞的脂多糖(LPS)衍生的内毒素活性降低至少70%。
2.根据权利要求1所述的用途,其中至少95%的所述细菌细胞为非活性的。
