Description

用于肝总管的导管
本申请要求于2013年12月17日在韩国知识产权局提交的第10-2013-0157385号韩国专利申请的优先权和权益,该韩国专利申请的全部内容通过引用被包含于此。
技术领域
本发明涉及一种能够利用超声内镜(EUS)将支架植入到胆管等中的用于肝总管的导管。
背景技术
通常,胆道提供胆汁从肝脏排出到肠所经的通道,并控制胆汁的流动。胆汁从肝脏的左肝管和右肝管运动到胆囊。在胆囊中集中的胆汁通过胆管被排出,由此通过胆总管最终流入到肠中。
当因胆管等的病变而需要进行支架植入时,通过以下步骤进行支架植入:使用附有针的穿孔器对胃和十二指肠进行穿孔;将导丝插入到胆管中;然后通过导丝将支架植入机构植入到胆管中。
典型的支架植入在多次扩张胃和十二指肠的穿孔部方面有困难,因此增加了手术时间并增加了植入机构的植入失败率,使得支架植入难以进行。

本发明的示例性实施例提供一种用于肝总管的导管,包括:内引导管;穿孔器,与所述内引导管的前端相结合;推压构件,与内引导管相结合,同时与穿孔器保持预定间隔;支架,设置在所述穿孔器和所述推压构件之间,并且设置在所述内引导管的外周表面上;以及外引导管,设置在所述内引导管的外周表面上,其中,所述穿孔器具有圆锥体形状或多边形锥体形状,并且形成为使得从穿孔器的中心线到穿孔器的外侧的距离沿着长度方向朝向穿孔器的前端减小。
在所述穿孔器中,在长度方向上沿相对于中心线的垂直方向截取的横切面可以以圆形、三角形、五边形、六边形和八边形中的任意一种形状形成。
在所述穿孔器中,与外引导管的前端接触的部分可以相对于穿孔器的插入到外引导管中的部分设置有突出部。
所述穿孔器可以包括:第一倾斜部,该第一倾斜部的尺寸沿着长度方向从外引导管的前端朝向穿孔器的前端减小;以及第二倾斜部,该第二倾斜部的尺寸从第一倾斜部朝向穿孔器的前端减小,并且第一倾斜部相对于中心线的倾斜角可以形成为小于第二倾斜部相对于中心线的倾斜角。
所述穿孔器中的外周表面可以设置有棱角部,并且棱角部可以设置有沿着径向方向突出的切割部。
在所述切割部中,当沿长度方向从沿着基于中心线的垂直方向切割的切割表面观察时,由切割部形成的角度的范围可以为30°到45°。
如上所述,根据本发明的示例性实施例,在通过对胃和十二指肠进行穿孔而在胆管、胆囊等中进行支架植入的时候,通过允许穿孔器对胃和十二指肠进行穿孔然后在胆管、胆囊等中直接地进行支架植入,可缩短支架的手术时间并简便地进行支架手术。

如图1至图3所示,根据本发明的示例性实施例的用于肝总管的导管包括内引导管1、穿孔器3、推压构件5(在图3中示出)、支架7(在图3中示出)以及外引导管9。
内引导管1具有细长管的形状,并且其一端可以与穿孔器3结合且其另一端可以与手柄1a结合。手柄1a可以被手术者的手握住,并且因此沿着内引导管1的长度方向运动从而能够使内引导管1运动。
穿孔器3是可以对胃壁和十二指肠壁进行穿孔的部件,并且穿孔器3具有朝向其前端渐细的形状。穿孔器3可以具有圆锥体形状或多边形锥体形状。
穿孔器3优选地形成为使得从中心线O到外侧的距离(由图3中的T1和T2表示)沿着长度方向(基于内引导管或穿孔器的长度方向)O朝向前端减小(参见图3)。
即,在穿孔器3中,在长度方向上沿着相对于中心线O的垂直方向截取的横切面形成为圆形、三角形、五边形、六边形和八边形中的任意一种形状。
当穿孔器3以三角形的锥体形状、五边形的锥体形状、六边形的锥体形状或八边形的锥体形状形成时,倾斜的棱角部3b可以用于在对胃壁和十二指肠壁进行穿孔的时候扩张胃壁和十二指肠壁。
此外,在穿孔器3中,与外引导管9的前端接触的部分设置有相对于穿孔器的插入到外引导管中的部分的突出部3a。
此外,根据本发明的示例性实施例的另一个示例,穿孔器3可以具有占从突出部3a(或外引导管的前端)朝向穿孔器3的前端的总长度的大约2/3的第一倾斜部11以及从第一倾斜部11到穿孔器3的前端的第二倾斜部13(参见图3)。
第一倾斜部11相对于中心线O的倾斜角b优选地形成为小于第二倾斜部13相对于中心线O的倾斜角c(参见图3)。
同时,推压构件5在与穿孔器3保持预定间隔的同时与内引导管1相结合。推压构件5可以用于根据内引导管1的运动而将支架7推动到外引导管9的外部。
此外,支架7设置在穿孔器3和推压构件5之间。支架7设置在内引导管1的外周表面上,并且因此被推压构件5推动以被排出到外引导管9的外部。支架7被植入到有病变的胆管和胆囊中,并且可以用于对胆管进行穿孔。作为被植入在有病变的胆管和胆囊中的支架7,可以使用已知的支架,并且因此将省略对支架的详细描述。
外引导管9可以由细长的柔性管形成,并且设置在内引导管1的外周表面上。外引导管9的前端可以粘附到如上所述的穿孔器3的突出部3a(参见图1)。
以上所描述的内引导管1、推压构件5以及支架7优选地设置在外引导管9的内部。
现在将详细地描述如上所述地构造的本发明的示例性实施例的操作。
在手术者利用超声内镜进行手术的状态下,使用超声内镜通过工作通道将根据本发明的示例性实施例的用于肝总管的导管插入到胃和十二指肠中。此外,手术者推动外引导管9,以对胆管和胆囊附近的胃壁和十二指肠壁进行穿孔。在这种情况下,穿孔器3具有尖锐的顶端部分,具有穿孔器3的尺寸从穿孔器3的前端朝向其后端增大的结构,并具有当穿孔器3形成为多棱锥时,棱角倾斜的形状。因此,手术者可以将穿孔器3推动到胃壁和十二指肠壁以能够容易地对胃壁和十二指肠壁进行穿孔。另外,手术者可以通过一次操作对胃壁和十二指肠壁进行穿孔,并且将胃壁和十二指肠壁穿孔为足够的尺寸以使外引导管9从其间通过。
然后,手术者移动穿孔器3直到支架植入其中的胆管和胆囊。另外,手术者将穿孔器3置于有病变的胆管部分和胆囊部分处,并且沿着内引导管1的长度方向移动手柄1a。
然后,在内引导管1运动的同时推压构件5运动,并且推压构件5推动支架7。因此,支架7离开外引导管9,并被植入到有病变的胆管中。
在手术者在病变处进行支架7的植入之后,拉动手柄1a以使内引导管1容纳在外引导管9中。然后,使穿孔器3的突出部3a与外引导管9的前端紧密地接触。然后,手术者拉动外引导管9,以从人体内腔移除用于肝总管的导管。
根据本发明的示例性实施例的用于肝总管的导管可以通过一次操作对胃壁和十二指肠壁进行穿孔,然后将支架7植入在胆管和胆囊中,因此可方便地被使用并显著地缩短手术时间。
图5是用于描述本发明的示例性实施例的另一个示例的对应于图4的示意图,并且示出了穿孔器3的横截面。
在本发明的示例性实施例的另一个示例中,与上述描述中的组件相同的组件由上述描述代替,并且将仅描述与上述描述中的组件不同的组件。
安装在具有多边形锥体形状的穿孔器3的外周侧的棱角部3b沿着棱角部3b的长度方向设置有切割部3c,切割部3c沿着穿孔器3的径向方向突出。在切割部3c中,当沿长度方向从沿着基于中心线O的垂直方向切割的切割表面观看时,由切割部3c形成的角度a的范围优选地为30°到45°。
当穿孔器3对胃壁和十二指肠壁进行穿孔时,切割部3c可以作为刀刃以容易地对胃壁和十二指肠壁进行穿孔。切割部3c可以更容易地对胃壁和十二指肠壁进行穿孔。

Claims (5)

1.一种用于肝总管的导管,包括:
内引导管;
外引导管,设置在所述内引导管的外周表面上,
穿孔器,与所述内引导管的前端相结合并且与所述外引导管的前端接触;
推压构件,设置在所述外引导管的内部,与内引导管相结合,同时与穿孔器保持预定间隔;
支架,设置在所述外引导管的内部且设置在所述穿孔器和所述推压构件之间,并且设置在所述内引导管的外周表面上,能够被所述推压构件推动以被排出到所述外引导管的外部;
其中,所述穿孔器具有圆锥体形状或多边形锥体形状,并且形成为使得从穿孔器的中心线到穿孔器的外侧的距离沿着长度方向朝向穿孔器的前端减小,
其中,所述穿孔器的外周表面设置有棱角部,并且该棱角部设置有沿着径向方向突出的切割部。
2.根据权利要求1所述的用于肝总管的导管,其中,在所述穿孔器中,沿相对于中心线垂直的方向截取的横切面以圆形、三角形、五边形、六边形和八边形中的任意一种形状形成。
3.根据权利要求1所述的用于肝总管的导管,其中,在所述穿孔器中,与外引导管的前端接触的部分相对于穿孔器的插入到外引导管中的部分形成突出部。
4.根据权利要求1所述的用于肝总管的导管,其中,所述穿孔器包括:
