Abstract
本发明涉及一种治疗原发性高血压的中药制剂及其制备方法。该中药制剂的主要原料有泽泻、川芎、黄芪、白术、草决明、菊花、山楂、桑寄生、钩藤、地龙、夏枯草、罗布麻、五味子、薄荷、滑石、甘草等中药材,将这些中药材用本发明研制的方法制成临床需要的成品制剂,对原发性高血压不但具有标本兼治、不易复发、没有药物依赖、无任何毒副作用等治疗技术优势,还具有制备工艺简便、制剂药效稳定、普遍适应性好、易于推广应用等推广优势。
Description
技术领域
本发明涉及一种治疗原发性高血压的中药制剂及其制备方法,属于中药领域。
背景技术
西医认为,基于目前的医学发展水平和检查手段,能够发现导致血压升高的确切病因,称之为继发性高血压;反之,不能发现导致血压升高的确切病因,则称为原发性高血压。高血压人群中多数为原发性高血压,但明确诊断原发性高血压,需首先除外继发性高血压。目前认为,继发性高血压占高血压人群的5%~10%,但随着医学发展水平和检查手段的不断进展,继发性高血压的比例将不断增加,原发性高血压的比例会不断下降。
目前,西医治疗原发性高血压的手段主要是使用药物降血压,只能缓解病症,不能根除,且容易造成药物依赖,使患者服用药物的剂量越来越大,效果却越来越差。
中医认为,通过辩证分析,高血压分为以下6型:
(1)、肝阳上亢型:表现以血压升高兼见眩晕,伴头目胀痛、面红耳赤、烦躁易怒、舌红苔黄、脉弦数为辨证要点。治疗:宜用平肝潜阳、滋养肝肾之法。
(2)、肝肾阴虚型:以血压升高兼见眩晕,伴头痛耳鸣、腰膝酸软、舌红少苔、脉细数为辨证要点。治疗:宜用滋补肝肾、养阴填精法。
(3)、阴阳两虚型:以血压升高兼见头晕目眩、心悸失眠、腰腿酸软、畏寒肢冷、小便清长、舌淡、脉沉细为辨证要点。治疗:宜用滋阴助阳法。
(4)、痰浊中阻型:此证多见于肥胖型高血压病患者。以血压升高兼见头晕头胀、沉重如裹、胸闷多痰、肢体沉重麻木、苔腻、脉滑为辨证要点。治疗:宜用化痰祛湿、健脾和胃法。
(5)、淤血阻滞型:以血压升高兼见头晕头痛如刺、痛有定处、胸闷心悸、舌质紫暗、脉细涩为辨证要点。治疗:宜活血化淤、理气止痛。
(6)、冲任失调型:本证多见妇女更年期前后,血压不稳定,多随情绪变化而波动。以血压升高兼见头晕头痛、心烦易怒、两胁胀痛、舌质红、脉弦细为辨证要点。治疗:宜用滋补肝肾、调和冲任法。
本发明正是基于以上对高血压的辨证认知,结合自身的行医经验和临床的反复摸索验证,兼顾各种分型,重新配伍药材,形成本发明的技术方案。
发明内容
本发明要解决的技术问题是,开发一种治疗原发性高血压的中药制剂及其制备方法,要求该制剂对原发性高血压有较好的治疗效果,并且易于推广应用。
发明人解决上述技术问题的方案为,依据中医对原发性高血压的辨证认知,结合自身多年的行医经验和现代临床用药特点,重新配伍药材,发明一个新的中药组方,该组方不但对原发性高血压有十分显著的治疗效果,而且可以利用为其研发的专用制备工艺制成成品制剂,并能保证疗效和药效的稳定性。该中药组方以原料的重量份表示为:
泽泻16-32份、川芎11-25份、黄芪11-29份、白术15-29份、草决明10-24份、菊花9-23份、山楂10-26份、桑寄生8-23份、钩藤18-34份、地龙8-14份、夏枯草8-18份、罗布麻5-11份、五味子8-20份、薄荷5-10份、滑石7-13份、甘草8-14份。
实践中优选出来的4个疗效显著的配比为(以重量份计):
1、泽泻24份、川芎18份、黄芪20份、白术22份、草决明17份、菊花16份、山楂18份、桑寄生15份、钩藤26份、地龙11份、夏枯草13份、罗布麻8份、五味子14份、薄荷8份、滑石10份、甘草11份。
2、泽泻26份、川芎19份、黄芪23份、白术21份、草决明19份、菊花17份、山楂17份、桑寄生14份、钩藤24份、地龙10份、夏枯草14份、罗布麻7份、五味子15份、薄荷8份、滑石11份、甘草12份。
3、泽泻25份、川芎17份、黄芪21份、白术23份、草决明16份、菊花18份、山楂16份、桑寄生17份、钩藤28份、地龙12份、夏枯草14份、罗布麻9份、五味子16份、薄荷7份、滑石11份、甘草13份。
4、泽泻26份、川芎20份、黄芪22份、白术20份、草决明19份、菊花19份、山楂17份、桑寄生18份、钩藤29份、地龙13份、夏枯草15份、罗布麻9份、五味子13份、薄荷9份、滑石9份、甘草12份。
为上述中药组方做成成品制剂而开发的制备工艺为以下步骤:
(1)、按本发明中药组方比例称取各味中药材;
(2)、将川芎洗净,微火烘至半干,切片,放入锅内,加入0.3倍重量黄酒拌匀吸干,文火炒至干燥,取出,晾干研磨,过120目筛,得细粉a,备用;
(3)、取泽泻、菊花、草决明、山楂、桑寄生、钩藤、地龙、夏枯草、罗布麻、五味子、薄荷、滑石放入容器中,第一次加7-11倍重量的纯化水,浸泡8-16个小时,回流煎煮3-5小时,过滤;第二次加5-8倍重量的纯化水,回流煎煮1-3小时,过滤;第三次加3-6倍重量的纯化水,回流煎煮0.5-2小时,过滤;合并三次滤液,减压浓缩成浸膏,继续减压干燥成干浸膏,粉碎研磨,过120目筛,得细粉b,备用;
(4)、取黄芪、白术、甘草置于锅内,用文火加热,麸炒至焦黄色,粉碎研磨,过120目筛,得细粉c,备用;
(5)、将细粉a、b、c混合均匀,利用现代通用的中药制剂技术制成临床所需要的剂型,如片剂、丸剂、颗粒剂、散剂或胶囊剂等。
本发明中药制剂所用原材料的来源、性味归经及功效:
泽泻:本品为泽泻科植物泽泻的干燥块茎。味甘,性寒;归肾、膀胱经;利小便,清湿热。
川芎:本品为伞形科植物川芎的干燥根茎。味辛,性温;归肝、胆、心包经;活血行气,祛风止痛。
黄芪:本品为豆科植物蒙古黄芪或膜荚黄芪的干燥根。味甘,性温;归肺、脾经;补气固表,利尿托毒,排脓,敛疮生肌。
白术:本品为菊科植物白术的干燥根茎。味苦、甘,性温;归脾、胃经;健脾益气,燥湿利水,止汗,安胎。
草决明:本品为豆科植物决明或小决明的干燥成熟种子。味甘、苦、咸,性微寒;归肝、大肠经;清热明目,润肠通便。
菊花:本品为菊科植物菊的干燥头状花序。味甘、苦,性微寒;归肺、肝经;散风清热,平肝明目。
山楂:本品为蔷薇科植物山里红或山楂的干燥成熟果实。味酸、甘、性微温;归脾、胃、肝经;消食健胃,行气散瘀。
桑寄生:本品为桑寄生科植物桑寄生的干燥带叶茎枝。味苦、甘,性平;归肝、肾经;补肝肾,强筋骨,祛风湿,安胎元。
钩藤:本品为茜草科植物钩藤、大叶钩藤、毛钩藤、华钩藤或无柄果钩藤的干燥带钩茎枝。味甘,性凉;归肝、心包经;清热平肝,息风定惊。
地龙:本品为巨蚓科动物参环毛蚓、通俗环毛蚓、威廉环毛蚓或栉盲环毛蚓的干燥体。味咸,性寒;归肝、脾、膀胱经;清热定惊,通络,平喘,利尿。
夏枯草:本品为唇形科植物夏枯草的干燥果穗。味辛、苦,性寒;归肝、胆经;清火,明目,散结,消肿。
罗布麻:为夹竹桃科植物罗布麻的全草。味甘苦,性凉;清火,降压,强心,利尿。
五味子:本品为木兰科植物五味子或华中五味子的干燥成熟果实。味酸、甘,性温;归肺,心、肾经;收敛固涩,益气生津,补肾宁心。
薄荷:本品为唇形科薄荷属植物薄荷的干燥地上部分。味辛,性凉;归肺、肝经;宣散风热,清头目,透疹。
滑石:本品多为块状集合体,呈不规则的块状。味甘、淡,性寒;归膀胱、肺、胃经;利尿通淋,清热解暑,祛湿敛疮。
甘草:本品为豆科植物甘草、胀果甘草或光果甘草的干燥根。味甘,性平;归心、肺、脾、胃经;补脾益气,清热解毒,祛痰止咳,缓急止痛,调和诸药。
本发明中药制剂的中药组方分析:
该组方是以泽泻、白术、黄芪、钩藤为君药,清热燥湿、利尿托毒、平肝息风;以川芎、草决明、桑寄生、罗布麻为臣药,活血行气、清热润肠、补肾强骨、降压强心;以菊花、山楂、夏枯草、五味子为佐药,散风清热、行气散瘀、清火消肿、补肾宁心;以地龙、薄荷、滑石为使药,通络利尿、散热透疹、通淋祛湿;以甘草调和诸药性,诸药协同配合、相互促进,可以达到清热燥湿、平肝息风、滋肝补肾、利尿托毒、祛痰化瘀等功效,对原发性高血压具有十分显著的治疗效果。
与现有技术相比,本发明中药制剂具有标本兼治、不易复发、没有药物依赖、无任何毒副作用等治疗技术优势,此外,还具有制备工艺简便、制剂药效稳定、普遍适应性好、易于推广应用等推广优势。

实施例一
(1)、按本发明中药组方比例称取泽泻2400g、川芎1800g、黄芪2000g、白术2200g、草决明1700g、菊花1600g、山楂1800g、桑寄生1500g、钩藤2600g、地龙1100g、夏枯草1300g、罗布麻800g、五味子1400g、薄荷800g、滑石1000g、甘草1100g;
(2)、将川芎洗净,微火烘至半干,切片,放入锅内,加入0.3倍重量黄酒拌匀吸干,文火炒至干燥,取出,晾干研磨,过120目筛,得细粉a,备用;

(2)、将川芎洗净,微火烘至半干,切片,放入锅内,加入0.3倍重量黄酒拌匀吸干,文火炒至干燥,取出,晾干研磨,过120目筛,得细粉a,备用;

(2)、将川芎洗净,微火烘至半干,切片,放入锅内,加入0.3倍重量黄酒拌匀吸干,文火炒至干燥,取出,晾干研磨,过120目筛,得细粉a,备用;

实施例八
动物毒性实验
(1)试验准备
试验药品:本发明实施例一至七所制得中药丸剂,将药丸加纯化水调制成稀糊状,备用。
试验所用动物:普通级NIH小鼠,体重23g±4g,雌雄各半,雌性小鼠均无孕;SD大鼠,初始体重220±11g,雌雄各半,雌性大鼠均无孕。
(2)急性毒性试验
应用小鼠进行急性毒性实验表明:小鼠灌胃本发明的中药稀糊,在589.5g生药/kg剂量下,给药后小鼠出现轻微活动减少,1小时左右恢复正常,给药后连续观察7天,无一小鼠死亡,且全身状况、饮食、摄水、小便和体重增长均正常。实验结果表明:小鼠灌胃本发明的中药丸剂的最大给药量为589.5g生药/kg/d(LD50>589.5g生药/kg)。本发明的中药制剂每日临床用药总量为0.16g生药/kg/d;按体重计,小鼠灌胃本发明中药制剂的耐受量为临床病人的3684.4倍。提示该药急性毒性极低,临床用药安全。
(3)、长期毒性试验
选用SD大鼠,给予三种剂量的本发明中药口服液:28.8g生药/kg、9.6g生药/kg、3.2g生药/kg(单位体重给药量按临床用药量的20倍计算,临床用药量依据《中华人民共和国药典》2010 年版一部)。每天灌胃一次,连续90天,末次给药后24小时各组活杀1/2动物(雌雄各半),其余1/2动物继续观察2周后活杀。试验期间观察动物的外观、一般行为、摄食量、体重变化;给药后90天和停药2周进行血液学(RBC、HB、网织红细胞、PLT、CT、WBC及分类)和血液生化(AST、ALT、ALP、Glu、BUN、Crea、TP、T.BIL、ALB、CHOL)、尿液生化、脏器系数、病理组织学等指标检查。
试验结果表明:本发明的中药制剂在高、中、低剂量组动物一般状态良好,外观体征、行为活动、进食量和体重增长均无异常变化;三个剂量组及对照组血液学检查、血液生化学、尿液生化检查均在正常范围,组间无显著差异;各组主要脏器组织病理学检查未见明显异常。上述指标停药2周后也未见改变。本试验用药剂量分别为临床用药剂量的180、60、20倍,根据试验结果:本发明中药制剂在高、中、低三个剂量(28.8g生药/kg、9.6g生药/kg、3.2g生药/kg)连续90天给药对大鼠无明显影响,无明确的毒性靶器官和敏感指标,恢复期观察也未见延迟性毒性反应,提示本发明的中药制剂临床应用的剂量安全性很高。
实施例九
本发明中药制剂的临床试验
1、受试者资料
所有受试者均由某市人民医院心脏内科提供,该科室从2011年5月至2013年12月,共收治了469名原发性高血压患者,从这些患者中筛选出280名愿意参与临床试验的患者,其中,男140名,年龄35-67岁,女140名,年龄38-69岁。
2、受试者分组
将受试者按年龄、性别、病程、病情严重程度等因素平均分成5个组,每组56人,男女各半,5个组分别编号为:对照组、试验1、试验2、试验3、试验4。
3、试验药品
对照药:缬沙坦分散片,哈尔滨三联药业股份有限公司,国药准字H20061058。
试验药:本发明实施例1-4所生产的丸剂(按4个最优配比生产,规格:折合生药含量约为0.48g生药/g丸剂)。
4、试验方案
对照组口服缬沙坦分散片,每次80mg,每日1次。试验1-4组分别口服实施例1-4所生产的丸剂,每次10g(折合生药量为4.8g/次),每日2次。4周为一个疗程,连续治疗3个疗程。记录用药后患者体征变化、不良反应等情况,并对治愈者在停药六个月后进行跟踪回访,记录复发病例。
5、试验结果评判
治愈:血压水平恢复到正常范围以内,并经医生确诊已经痊愈。
显效:血压水平有所降低,但仍比正常血压值高,需要继续用药。
无效:血压水平没有变化,甚至升高。
复发:治愈者在停药6个月内重新复发高血压。
6、试验结果统计分析
(1)、患者表征对比:试验组患者在服用药物后,不但血压水平下降,而且明显感觉肢体轻健,心烦失眠好转,胸闷心悸多痰症状减轻,头痛耳鸣消失,中气充盈等;对照药除血压水平降低外,此类的感觉不明显。对照组有7人出现了不同程度的头痛、上呼吸道感染、腹泻、水肿、失眠、无力、背痛、腹痛、恶心等不良反应症状;试验组4个组均未出现不良反应。
(2)、试验数据统计分析表:

7、结论
从上述试验结果统计可以看出:
(1)、表征对比:试验组充分展现了中药对疾病标本兼治、调治结合的优势,对患者身体的调理优势显著,对照组只是治标;试验组没有不良反应,对照组有7例不良反应,不良反应率高达12.5%。因此,试验组相对于对照组有显著的治疗优势。
(2)、试验数据对比:试验组平均治愈率为55.4%,是对照组的1.7倍;试验组的平均总有效率为97.3%,比对照组高11.3个百分点;试验组的平均复发率为6.4%,对照组的复发率是试验组的9.5倍。因此,试验组相对于对照组有十分显著的治疗优势。
综上,与现有技术相比,本发明中药制剂在治疗原发性高血压方面有显著的技术优势,取得了创造性的、难以预料的技术效果。
Claims (7)
1.一种治疗原发性高血压的中药制剂,其特征在于 ,所述中药制剂是由以下重量份的原料制成的:
泽泻16-32份、川芎11-25份、黄芪11-29份、白术15-29份、草决明10-24份、菊花9-23份、山楂10-26份、桑寄生8-23份、钩藤18-34份、地龙8-14份、夏枯草8-18份、罗布麻5-11份、五味子8-20份、薄荷5-10份、滑石7-13份、甘草8-14份。
2.如权利要求1所述的一种治疗原发性高血压的中药制剂,其特征在于 ,所述中药制剂还可以是由以下重量份的原料制成的:
泽泻24份、川芎18份、黄芪20份、白术22份、草决明17份、菊花16份、山楂18份、桑寄生15份、钩藤26份、地龙11份、夏枯草13份、罗布麻8份、五味子14份、薄荷8份、滑石10份、甘草11份。
3.如权利要求1所述的一种治疗原发性高血压的中药制剂,其特征在于 ,所述中药制剂还可以是由以下重量份的原料制成的:
泽泻26份、川芎19份、黄芪23份、白术21份、草决明19份、菊花17份、山楂17份、桑寄生14份、钩藤24份、地龙10份、夏枯草14份、罗布麻7份、五味子15份、薄荷8份、滑石11份、甘草12份。

泽泻25份、川芎17份、黄芪21份、白术23份、草决明16份、菊花18份、山楂16份、桑寄生17份、钩藤28份、地龙12份、夏枯草14份、罗布麻9份、五味子16份、薄荷7份、滑石11份、甘草13份。
5.如权利要求1所述的一种治疗原发性高血压的中药制剂,其特征在于 ,所述中药制剂还可以是由以下重量份的原料制成的:
泽泻26份、川芎20份、黄芪22份、白术20份、草决明19份、菊花19份、山楂17份、桑寄生18份、钩藤29份、地龙13份、夏枯草15份、罗布麻9份、五味子13份、薄荷9份、滑石9份、甘草12份。
6.如权利要求1-5所述的一种治疗原发性高血压的中药制剂,其特征在于 ,所述中药制剂的制备工艺为以下步骤:
步骤1、按本发明中药组方比例称取各味中药材;
步骤2、将川芎洗净,微火烘至半干,切片,放入锅内,加入0.3倍重量黄酒拌匀吸干,文火炒至干燥,取出,晾干研磨,过120目筛,得细粉a,备用;
步骤3、取泽泻、菊花、草决明、山楂、桑寄生、钩藤、地龙、夏枯草、罗布麻、五味子、薄荷、滑石放入容器中,第一次加7-11倍重量的纯化水,浸泡8-16个小时,回流煎煮3-5小时,过滤;第二次加5-8倍重量的纯化水,回流煎煮1-3小时,过滤;第三次加3-6倍重量的纯化水,回流煎煮0.5-2小时,过滤;合并三次滤液,减压浓缩成浸膏,继续减压干燥成干浸膏,粉碎研磨,过120目筛,得细粉b,备用;
步骤4、取黄芪、白术、甘草置于锅内,用文火加热,麸炒至焦黄色,粉碎研磨,过120目筛,得细粉c,备用;
步骤5、将细粉a、b、c混合均匀,利用现代通用的中药制剂技术制成临床所需要的剂型。
7.如权利要求1-5所述的一种治疗原发性高血压的中药制剂,其特征在于 ,所述中药制剂可以制成片剂、丸剂、颗粒剂、散剂或胶囊剂。
