技术领域
本发明属于中医药技术领域,具体而言,涉及一种治疗反流性食管炎的药物组合物及其用途。
背景技术
反流性食管炎(reflux esophagitis)是由食管廓清能力下降、食管下括约肌功能异常、胃酸以及胆汁等多因素引发的一种食管黏膜炎症、糜烂、溃疡或纤维化等病变的疾病。内镜下可见食管远端黏膜破损。胃酸对食管黏膜的直接或/和间接损害是导致反流性食管炎的病理基础。反流性食管炎的组织学表现主要为食管上皮基底层细胞明显增生,且上皮细胞的增生程度随反流的时间延长而加重。上皮细胞的增生过程中,染色体突变积累,容易导致组织细胞的异常增殖。因此,抑制食管上皮细胞的病理增生对预防和治疗该病有重要意义。
发明内容
本发明的目的在于提供一种治疗反流性食管炎的药物组合物,该药可通过降低患者食管下段黏膜的酸度,抑制胃酸反流等作用来改善反流性食管炎的病理状况,进而促进反流性食管炎的恢复。
本发明的目的是这样实现的:
一种治疗反流性食管炎的药物组合物,由中药材原料和可药用辅料制备而成,按重量份计,所述的中药材原料包括:威灵仙20-40份,麦冬12-20份,白茅根21-30份,瓜蒌12-20份,大青叶9-15份,吴茱萸3-6份,棉花壳9-15份,代赭石9-15份,当归3-6份,紫苏叶5-9份,白及6-15份,炙甘草9-15份。
优选地,如上所述治疗反流性食管炎的药物组合物,其中按重量份计,所述的中药材原料由威灵仙26-30份,麦冬15-17份,白茅根24-26份,瓜蒌15-17份,大青叶11-13份,吴茱萸4-5份,棉花壳11-13份,代赭石11-13份,当归4-5份,紫苏叶6-8份,白及8-12份和炙甘草11-13份组成。
在本发明的一个最优选的实施例中,如上所述治疗反流性食管炎的药物组合物,其中按重量份计,所述的中药材原料由威灵仙28份,麦冬16份,白茅根25份,瓜蒌16份,大青叶12份,吴茱萸5份,棉花壳12份,代赭石12份,当归5份,紫苏叶7份,白及10份和炙甘草12份组成。
本发明的药物组合物是将上述中药材原料制备成中药提取物,然后将该提取物干粉与任何一种或一种以上药剂学上可接受的辅料如淀粉、糊精、乳糖、微晶纤维素、羟丙甲基纤维素、羧甲基淀粉钠、硬脂酸镁、微粉硅胶等混合制成的各种剂型,例如,可制成口服液、片剂、缓释片、滴丸、颗粒剂、胶囊剂。优选剂型为颗粒剂。
本发明涉及的中药制剂的制备方法包括如下步骤:称取清洗干净且干燥的威灵仙20-40份,麦冬12-20份,白茅根21-30份,瓜蒌12-20份,大青叶9-15份,吴茱萸3-6份,棉花壳9-15份,代赭石9-15份,当归3-6份,紫苏叶5-9份,白及6-15份,炙甘草9-15份,合并,加水煮沸,然后文火煎煮两次,合并煎液,减压浓缩至70℃时相对密度为1.12-1.20,加乙醇使含醇量的体积百分比达72-80%,搅拌,静置,过滤,滤液减压浓缩至70℃时相对密度为1.10-1.20并回收乙醇得浓缩液。将浓缩液加水及辅料配制成口服液,或者将该浓缩液真空干燥(60℃以下)成干膏,粉碎,加药用辅料,制成颗粒剂、胶囊剂、片剂。
需要说明的是,本发明的药物组合物在制备过程中所采用的中药材原料具有如下来源:威灵仙选用为毛茛科植物威灵仙Clematis chinensis Osbeck的干燥根及根茎。麦冬选用百合科植物麦冬(沿阶草)Ophiopogon japonicus(Thunb.)Ker-Gawl.的干燥块根。白茅根选用禾本科植物白茅Imperata cylindrica Beauv.var.major(Nees)C.E.Hubb.的干燥根茎。瓜蒌选用葫芦科植物瓜蒌的干燥果实。大青叶选用十字花科植物菘蓝Isatisindigotica Fort.的干燥叶。吴茱萸选用芸香科植物吴茱萸Evodia rutaecarpa(Juss.)Benth.的干燥近成熟果实。棉花壳选用锦葵科植物草棉Gossypium herbaceum L.等的外果皮。代赭石选用氧化物类矿物赤铁矿Haematite的矿石。紫苏叶选用唇形科植物紫苏Perilla frutescens(L.)Britt的干燥叶。白及选用兰科植物白及Bletilla striata(Thunb.)Reichb.f.的干燥块茎。
本发明人在试验研究中发现,模型对照组大鼠食管和胃pH值较假手术组明显降低,中药组食管和胃pH值较模型组明显升高,这表明本发明的药物组合物对胃酸及酸反流有抑制作用,可能是通过抑制胃酸分泌,促进食管酸清除机制实现的。另外,模型对照组食管组织上皮细胞层厚度、乳头延伸高度、基底层细胞厚度较假手术组均明显升高,而中药组食管组织上皮细胞层厚度、乳头延伸高度、基底层细胞厚度较模型对照组均明显下降,这表明本发明的药物组合物对大鼠食管组织增生有抑制作用。综上所述,与现有技术相比,本发明的药物组合物通过降低患者食管下段黏膜的酸度,抑制胃酸反流等作用来改善反流性食管炎的病理状况,进而促进反流性食管炎的恢复。
具体实施方式
以下是本发明的具体实施例和临床应用例,对本发明的技术方案和技术效果做进一步作描述,但是本发明的保护范围并不限于这些实施例。
实施例1中药颗粒剂的制备
称取威灵仙280g,麦冬160g,白茅根250g,瓜蒌160g,大青叶120g,吴茱萸50g,棉花壳120g,代赭石120g,当归50g,紫苏叶70g,白及100g,炙甘草120g,合并,粉碎,加水煮沸,然后文火煎煮两次,第一次加水为药材总重量的10倍量,煎煮3h,第二次加水为药材总重量的8倍量,煎煮2h,合并煎液,0.08MPa、70℃下减压浓缩至70℃时相对密度为1.15左右,加95%(v/v)乙醇使含醇量达75%(v/v),搅拌,静置过夜,过滤,滤液在0.08MPa、65℃条件下减压浓缩成至65℃时相对密度为1.15左右的浸膏,浸膏置不锈钢盘中,真空干燥(60℃以下)成干膏,粉碎,过100筛,加入1倍量的微晶纤维素、2倍量的蔗糖粉和1倍量的糊精,混匀,干法制粒,包装,每包10g。
实施例2中药口服液的制备
称取威灵仙260g,麦冬120g,白茅根300g,瓜蒌160g,大青叶100g,吴茱萸50g,棉花壳150g,代赭石100g,当归30g,紫苏叶50g,白及150g,炙甘草100g,合并,粉碎,加水煮沸,然后文火煎煮两次,第一次加水为药材总重量的10倍量,煎煮2h,第二次加水为药材总重量的8倍量,煎煮2h,合并煎液,0.08MPa、70℃下减压浓缩至70℃时相对密度为1.15左右,加95%(v/v)乙醇使含醇量达80%(v/v),搅拌,静置过夜,过滤,滤液在0.08MPa、65℃条件下减压浓缩成至65℃时相对密度为1.15左右,加水及蔗糖配制成口服液。
实施例3中药口服液的制备
称取威灵仙200g,麦冬200g,白茅根260g,瓜蒌200g,大青叶150g,吴茱萸50g,棉花壳150g,代赭石90g,当归60g,紫苏叶80g,白及100g,炙甘草150g,合并,粉碎,加水煮沸,然后文火煎煮两次,第一次加水为药材总重量的10倍量,煎煮3h,第二次加水为药材总重量的8倍量,煎煮2h,合并煎液,0.08MPa、70℃下减压浓缩至70℃时相对密度为1.18左右,加95%(v/v)乙醇使含醇量达78%(v/v),搅拌,静置过夜,过滤,滤液在0.08MPa、65℃条件下减压浓缩成至65℃时相对密度为1.15左右,加水及蔗糖配制成口服液。
实施例4中药对反流性食管炎动物模型的治疗试验
中药样品的制备:称取威灵仙280g,麦冬160g,白茅根250g,瓜蒌160g,大青叶120g,吴茱萸50g,棉花壳120g,代赭石120g,当归50g,紫苏叶70g,白及100g,炙甘草120g,合并,粉碎,加水煮沸,然后文火煎煮两次,第一次加水为药材总重量的10倍量,煎煮3h,第二次加水为药材总重量的8倍量,煎煮2h,合并煎液,0.08MPa、70℃下减压浓缩至70℃时相对密度为1.15左右,加95%(v/v)乙醇使含醇量达75%(v/v),搅拌,静置过夜,过滤,滤液在0.08MPa、65℃条件下减压浓缩成至65℃时相对密度为1.15左右的浸膏,浸膏置不锈钢盘中,真空干燥(60℃以下)成干膏,粉碎,过100筛。
动物清洁级健康SD大鼠50只,雌雄各半,体质量(220±20)g。试验开始后,按如下方法建立反流性食管炎模型(造模前24h禁食,不禁水):应用1%戊巴比妥钠麻醉(30mg/kg腹腔注射)后,大鼠固定于手术台,取上腹正中切口进腹。行半幽门结扎联合贲门肌切开术:半幽门缝扎时充分暴露幽门,用细针及细线缝扎一半幽门,并避开血管,并于食管-胃交界处纵行切开贲门肌约1cm,分离至黏膜层完全暴露于视野中,以加强胃反流,后关腹。假手术组行假手术,即进腹后不进行任何手术即关腹。造模过程中因部分大鼠不能耐受实验而死亡,造模结束后,运用随机数字表将建模成功的大鼠随机分组为:模型对照组、中药高、低剂量组,每组10只。造模1周后开始灌胃给药,中药高、低剂量组分别给予上述制备的中药干膏粉0.92、0.46g/kg/d,假手术组和模型对照组给予同体积的生理盐水灌胃,1次/d,共给药4周。
各组实验大鼠分别在给药结束时脱臼处死,取食管下段及胃,采用pH值测定仪分别测定食管下段和胃的pH值。各组大鼠的食管下段及胃pH值的试验统计结果见表1。通过表1的试验结果可以看出,模型组食管下段和胃的pH值较假手术组明显下降(P<0.05);中药高、低剂量组食管下段及胃pH值均较模型组明显升高(P<0.05)。

另外,各组大鼠处死后观察食管组织病理学改变,并参照1999-08全国反流性食管病(炎)研讨会制定的“反流性食管病(炎)诊断及治疗方案(试行)”标准评分。通过观察各组的食管组织发现,假手术组、中药高组大鼠食管黏膜基本正常;模型对照组大鼠食管黏膜可见明显鳞状上皮增生,黏膜固有层乳头延伸,炎细胞浸润,以中性粒细胞、淋巴细胞为主。各组大鼠的食管下段组织鳞状上皮厚度、乳头延伸高度及基底细胞层厚度的病理学指标统计结果见表2。通过表2的试验结果可以看出,模型对照组食管组织鳞状上皮厚度、乳头延伸高度及基底细胞层厚度较假手术组明显升高(P<0.05);中药各组上述各指标均较模型对照组明显降低(P<0.05)。

Claims (1)

1.一种中药组合物在制备治疗反流性食管炎的药物中的用途,其特征在于,该药物由中药材原料和可药用辅料制备而成,按重量份计,所述的中药材原料由威灵仙28份,麦冬16份,白茅根25份,瓜蒌16份,大青叶12份,吴茱萸5份,棉花壳12份,代赭石12份,当归5份,紫苏叶7份,白及10份和炙甘草12份组成;
该药物的制备方法包括如下步骤:称取清洗干净且干燥的威灵仙,麦冬,白茅根,瓜蒌,大青叶,吴茱萸,棉花壳,代赭石,当归,紫苏叶,白及,炙甘草,合并,加水煮沸,然后文火煎煮两次,合并煎液,减压浓缩至70℃时相对密度为1.12-1.20,加乙醇使含醇量的体积百分比达72-80%,搅拌,静置,过滤,滤液减压浓缩至70℃时相对密度为1.10-1.20并回收乙醇得浓缩液,将浓缩液加水及辅料配制成口服液,或者将该浓缩液60℃以下真空干燥成干膏,粉碎,加药用辅料,制成颗粒剂、胶囊剂或片剂。
