Abstract

本发明涉及一种中药制剂,具体涉及一种用于口疮的中药制剂,本发明还涉及该中药制剂的制备方法。
背景技术
口疮又称为口腔溃疡,是发生在口腔黏膜上的表浅性溃疡,大小可从米粒至黄豆大小、成圆形或卵形,溃疡表面为凹、周围充血。溃疡具有周期性、复发性及自限性等特点,好发于唇、颊、舌缘等。口腔溃疡为非特异性炎症。初期出现严重的血管及炎性反应,形成溃疡后,其表面有纤维素性假膜覆盖,下方有少量坏死组织。固有层有大量炎症细胞浸润,胶原纤维可水肿、玻璃样变或断裂消失。口疮同时伴有很多并发症,可并发口臭、牙龈红肿、慢性咽炎、便秘、头痛、头晕、恶心、乏力、烦躁、发热、淋巴结肿大等全身症状。
口疮相当于中医学中的“口疳”、“口疡”的范畴,最早记载于《黄帝内经》,云:“岁金不及,炎火乃行……民病口疮。”将口疮的病因归于为“火”。后世医家从不同角度对本病的病因病机进行了阐述和发挥,明确指出本病与心脾热盛有关。中医认为本病还与精神紧张、内分泌失调、营养缺乏、感染、遗传、免疫功能异常、消化系统疾病等因素有关。本病的发生,外因以热毒为主;内因多为情志内伤,饮食不节,房室劳倦所致。溃疡多数是散在性分布在容易活动的黏膜区域,常常疼痛难忍令患者寝食难安。
目前西医对该病的病因及致病机制仍不明确,并且每人发病过程、发病程度、病损表现多有不同,个体差异大,医药治疗主要以局部治疗为主,基本都是对症治疗,而且这些治疗方法主要以减轻疼痛为目的,效果不是很理想,且治疗过程长,容易复发,给患者带来极大的痛苦。
发明内容
本发明的目的在于提供一种用于口疮的中药制剂,该中药制剂对于口疮具有疗效好,时间短,不易复发,价格低廉的特点。使患者早日摆脱病痛,恢复正常生活。
发明人在多年的临床医疗实践中,根据该病症的发展规律,以及中药的配伍原则,并通过大量的临床验证,反复探索总结出一种用于口疮的中药制剂的新配方。本发明具有疗效好,时间短,不易复发,价格低廉的特点。
本发明中药物的作用:

黄柏:为芸香科植物黄皮树Phellodendron chinense Schneid.或黄檗Phellodendron amurense Rupr.的干燥树皮。性味:苦、寒。归肾、膀胱、大肠经。功效:清热燥湿,泻火解毒,除骨蒸清虚热。用于湿热泻痢,黄疸,带下,热淋,脚气,痿辟,骨蒸劳热,盗汗,遗精,疮疡肿毒,湿疹瘙痒。本品既能清热燥湿,又能泻火解毒,内服、外用均有效,并且黄柏具有广谱抗菌的作用。
知母:为百合科植物知母Anemarrhena asphodeloides Bunge.的干燥根茎。知母性味苦寒而不燥,上能清肺,中能凉胃,下能泻肾火。因此既能用于热病烦渴,也可清阴虚火旺,常与黄柏同用。
黄芩:为唇形科植物黄芩 Scutellaria baicalensis Georgi 的干燥根。性味:苦,寒。归肺、胆、脾、胃、大肠、小肠经。功效:清热燥湿,泻火解毒,止血,安胎,降血压。黄芩的临床应用抗菌比较好,而且不产生抗药性。本品有较强的泻火解毒作用,可用于热毒疮肿,常与金银花、连翘等同用。
生石膏:为硫酸盐类矿物硬石膏族石膏,主含含水硫酸钙(CaSO4·2H2O)。性味:甘、辛,大寒。归肺、胃经。功效:清热泻火,除烦止渴。用于外感热病,高热烦渴,肺热喘咳,胃火亢盛,头痛,牙痛。石膏药性大寒,善清气分实热,故适用于肺胃实热的症候,常与知母相须为用,以增强清里热的作用。
栀子:为茜草科植物栀子Gardenia jasminoides Ellis的干燥成熟果实。性味:苦,寒。归心、肺、三焦经。功效:泻火除烦,清热利尿,凉血解毒。用于热病心烦,黄疸尿赤,血淋涩痛,血热吐衄,目赤肿痛,火毒疮疡;外治扭挫伤痛。本品轻清上行,能泻肺火,去肌表热,在外感热病、表里有热之际,能起双解的作用;本品苦寒泄降,又能泄三焦火,凉血清心热,可用于热病心烦;血热妄行及热淋尿血等症。故栀子既能清气分热,又能清血分热。
黄连:为毛茛科植物黄连Coptis chinensis Franch.、三角叶黄连Coptis deltoidea C. Y. Cheng et Hsiao或云连Coptis teeta Wall.的干燥根茎。性味:苦、寒。归心、脾、胃、肝、胆、大肠经。功能主治:清热燥湿,泻火解毒。用于湿热痞满,呕吐吞酸,泻痢,黄疸,高热神昏,心火亢盛,心烦不寐,血热吐衄,目赤,牙痛,消渴,痈肿疔疮;外治湿疹,湿疮,耳道流脓。本品清热燥湿,泻火解毒,常配黄芩、栀子等治疗口舌生疮、痈肿疔毒等。
牡丹皮:为毛茛科植物牡丹 Paeonia suffruticosa Andr. 的干燥根皮。性味苦,凉。归心、肝、肾、肺经。功效:清热凉血,活血散瘀。本品通过清热凉血、活血散瘀以消痈,对于疮痈肿毒、肠痈等症,本品也是常用的药物。疗疮痈可配合清热解毒药如连翘、地丁草之类清热降火。
木通:为木通科植物白木通或三叶木通、木通的木质茎。性味:苦、寒。归心、脾、肾、小肠、膀胱经。功效:清热利尿;活血通脉。主小便赤赤;淋浊;水肿;胸中烦热;喉喉疼痛;口舌生疮;风湿痹痛;乳汁不通;经闭;痛经。本品能上清心火,下利小便,使湿热之邪下行从小便排出。
生地黄:为玄参科植物地黄Rehmannia glutinosa Libosch. 的新鲜或干燥块根。性味:甘、寒。归心、肝、肾经。功效:清热凉血,养阴生津。可用于津伤口渴,内热消渴。
淡竹叶:为禾本科植物淡竹叶Lophatherum gracile Brongn. 的干燥茎叶。性味:甘、淡,寒。归心、胃、小肠经。功效:清热除烦,利尿。用于热病烦渴,小便赤涩淋痛,口舌生疮。本品上能清心火而解热,下能降小肠火而利尿,能使火热之邪下行,从小便而清,因此可用于心火上炎之口舌生疮,以及心热下移小肠所致的小便短赤涩痛,常配木通、生地等。
大黄:为蓼科植物掌叶大黄Rheum palmatum L。、唐古特大黄Rheum tanguticum Maxim ex Balf。或药用大黄Rheum officinale Baill。的干燥根及根茎。性味:苦,寒。归脾、胃、大肠、肝、心包经。功效:泻热通肠,凉血解毒,逐瘀通经。用于实热便秘,血热吐衄,目赤,咽肿,肠痈腹痛,痈肿疔疮,瘀血经闭,跌打损伤。本品苦降,借其通便作用,能引火、引血下行,使热毒、火毒下泄,从大便排出,因此可用于治疗火热上炎所致的咽喉肿痛、牙龈肿痛以及目赤口疮等。
肉桂:为樟科植物肉桂Cinnamomum cassia Presl的树皮。性味大热,辛、甘。归肾、脾、心、肝经。功效:补火助阳,引火归源,散寒止痛,活血通经,通血脉。肉桂长于温里止痛,入下焦而补肾阳,归命火,本品甘热,入脾肾经,善于补阳气而温暖脾肾,散寒止痛,中医讲脾开窍于口,其华在唇,脾健则口疮自消。
吴茱萸:为芸香科植物吴茱萸Evodia rutaecarpa(Juss.)Benth.、石虎Evodia rutaecarpa(Juss.)Benth. var. officinalis (Dode)Huang 或疏毛吴茱萸Evodia rutaecarpa (Juss.)Benth. var. bodinieri (Dode)Huang的干燥近成熟果实。性味:辛、苦,热;有小毒。归肝、脾、胃、肾经。功效:散寒止痛,降逆止呕,助阳止泻。用于厥阴头痛,寒疝腹痛,寒湿脚气,经行腹痛,脘腹胀痛,呕吐吞酸,五更泄泻,外治口疮;高血压。本品温能补脾益肾,助阳止泻,为治脾肾阳虚之常用药。
天花粉:为葫芦科植物栝楼Trichosanthes kirilowii Maxim.或双边栝楼Trichosan-thes rosthornii Herms 的干燥根。性味:甘、微苦,微寒。归肺、胃经。功效:清热生津,消肿排脓。用于热病烦渴,肺热燥咳,内热消渴,疮疡肿毒。本品对疮疡未溃者有消肿作用,已溃脓出不畅者有排脓作用,但均以热毒炽盛者为宜,常与连翘、蒲公英、浙贝母等药同用。
麦冬:为百合科植物麦冬(沿阶草)Ophiopogon japonicus (Thunb.)Ker-Gawl.的干燥块根。性味:甘、微苦、微寒。归心、肺、胃经。功效:益胃生津、润肺养阴、清心除烦。
郁金:为姜科植物温郁金Curcuma wenyujin Y. H. Chen et C. Ling、姜黄Curcuma longa L.、广西莪术Curcuma kwangsiensis S. G. Lee et C. F. Liang或蓬莪术Curcuma phaeocaulis Val.的干燥块根。性味:辛、苦,寒。归肝、心、肺经。功效:行气化瘀,清心解郁,利胆退黄,凉血破瘀。本品辛散苦泄,性寒清热,能清心解郁开窍。郁金辛开苦降,芳香宣达,性寒又能清热。既入气分以行气解郁,又入血分以凉血祛瘀,为血中之气药。
佩兰:为菊科植物佩兰Eupatorium fortunei Turcz.的干燥地上部分。性味:辛,平。归脾、胃、肺经。功效:芳香化湿,醒脾开胃,发表解暑。用于湿浊中阻,脘痞呕恶,口中甜腻,口臭,多涎,暑湿表症,头胀胸闷。
白芨:为兰科多年生草本植物白芨Bletilla striata(Thunb.) Reichb. f.的干燥块茎。性味:辛,苦、甘、涩,微寒。归肺、肝、胃经。功效:收敛止血,消肿生肌。用于咳血吐血,外伤出血,疮疡肿毒,皮肤皲裂;肺结核咳血,溃疡病出血。《本草求真》中记载:白及,方书既载功能入肺止血,又载能治跌扑折骨,汤火灼伤,恶疮痈肿,败疽死肌,得非似收不收,似涩不涩,似止不止乎?不知书言功能止血者,是因性涩之谓也;书言能治痈疽损伤者,是因味辛能散之谓也。此药涩中有散,补中有破,故书又载去腐、逐瘀、生新。
西瓜霜:为西瓜皮和皮硝混合制成的白色结晶。性味:咸、寒。归脾、肺经。功效:清热、消肿。适用于咽喉肿痛,目赤肿痛及口疮等症。
通草:为五加科植物通脱木Tetrapanax papyriferus (Hook.)K. Koch 的干燥茎髓。性味:甘、淡、微寒。归肺、胃经。功效:清势利水;通乳。主淋症涩痛;小便不利;水肿;黄疸;湿温病;小便短赤;产后乳少;经闭;带下。
泽泻:为泽泻科植物泽泻Alisma orientalis (Sam.)Juzep.的干燥块茎。性味:甘、寒。归肾、膀胱经。功效:利小便,清湿热。用于水湿内停之水肿、小便不利。本品入肾、膀胱经,性寒善清下焦湿热。
蒲公英:蒲公英又称尿床草,为菊科植物蒲公英Toraxacum mongolicum Hand. Mazz.、碱地蒲公英Toraxacum borealisinense Kitam. 或同属数种植物的干燥全草。性味:甘、微苦、寒。归肝、胃经。功效:清热解毒,消肿散结。蒲公英具有消炎、抗菌、抗肿瘤、清热解毒的功效,现代临床上应用范围广,作用好。用于疔疮肿毒,乳痈,瘰疠,目赤,咽痛,肺痈,肠痈,湿热黄疸,热淋涩痛。对热毒所致的乳痈肿痛、疔疮有良好的效果,可单独煎汁内服,或外敷局部;也可配合其它清热解毒药同用,如银花、连翘、地丁草、野菊花、赤芍等。
连翘:为木犀科植物连翘Forsythia suspensa (Thunb.)Vahl的干燥果实。性味:苦,微寒。归肺、心、小肠经。功效:清热解毒,消肿散结。用于痈疽,瘰疠,乳痈,丹毒,风热感冒,温病初起,温热入营,高热烦渴,神昏发斑,热淋尿闭。连翘性凉味苦,轻清上浮,可治上焦诸热,尤能解毒消痈而散结,故为疮家的要药。
金莲花:为毛茛科金莲花植物金莲花Trollius chinensis Bunge和短瓣金莲花T. ledebouri Reichb.干燥花朵。性味:味苦;性微寒。归肺;胃经。功效:清热解毒;消肿;明目。主主治感冒发热;咽喉肿痛;口疮;牙龈肿痛;牙龈出血;目赤肿痛;疔疮肿毒。
生甘草:为豆科植物甘草 Glycyrrhiza uralensis Fisch.,胀果甘草 Glycyrrhiza Inflata Bat.或光果甘草Glycyrrhiza Glabra L.的干燥根及根茎。性味甘,平。归心、肺、脾、胃经。功效:补脾和胃,益气复脉。
本发明的方解是:方中黄柏、黄芩、黄连相须为用,清热燥湿,泻火解毒黄芩善泻肺火,黄连善泻心胃之火,黄柏善泻肾火,退虚热,三者合用泻火解毒作用增强,知母上能清肺,中能凉胃,下能泻肾火,生石膏配知母增强清里热的作用,栀子既能清气分热,又能清血分热,以上六味善清体内湿热,无论上、中、下焦,以及血分热或气分热均能去除,从根本上清除引起口疮的因素,牡丹皮、木通、生地黄助君药清热凉血、活血通脉,淡竹叶清热除烦,大黄泻热通肠,常用语缓解火热上炎,肉桂、吴茱萸补阳气而温暖脾肾,脾开窍于口,其华在唇,脾健则口疮自消。以上七味共为臣药,天花粉、麦冬清热生津,消肿排脓,郁金、佩兰芳香化湿,清心解郁,白芨、西瓜霜收敛止血,消肿生肌,通草、泽泻清湿热,蒲公英、连翘、金莲花清热解毒、抗菌,以上几味不仅加强了君、臣药清利湿热、生肌敛疮的作用,还具有广谱抗菌作用,减少口腔中的有害菌群,共为佐药。生甘草引药归经,调和诸药为使药。此方诸药合用,君臣协力,佐使共辅,具有清利湿热,生肌敛疮的功效,能够有效治疗口疮。
本发明药物原料用量是经发明人进行大量摸索总结得出的,各原料用量为在下述重量份范围都具有较好的疗效:黄柏7-17份、知母7-17份、黄芩10-20份、生石膏15-25份、栀子7-17份、黄连4-14份、牡丹皮10-20份、木通4-14份、生地黄13-23份、淡竹叶7-17份、大黄10-20份、肉桂4-14份、吴茱萸3-9份、天花粉13-23份、麦冬7-17份、郁金10-20份、佩兰7-17份、白芨7-17份、西瓜霜3-9份、通草4-14份、泽泻7-17份、蒲公英10-20份、连翘9-19份、金莲花4-14份、生甘草4-14份。
其中优选为:黄柏12份、知母12份、黄芩15份、生石膏20份、栀子12份、黄连9份、牡丹皮15份、木通9份、生地黄18份、淡竹叶12份、大黄15份、肉桂9份、吴茱萸6份、天花粉18份、麦冬12份、郁金15份、佩兰12份、白芨12份、西瓜霜6份、通草9份、泽泻12份、蒲公英15份、连翘14份、金莲花9份、生甘草9份。
一种用于口疮的中药制剂的制备方法如下:
步骤一:按本发明重量配比,称取知母、黄芩、黄连、牡丹皮、生地黄、大黄、肉桂、天花粉、麦冬、郁金、包好的生石膏、西瓜霜放入砂锅中,加8倍量(重量)的水,先用武火将水烧开,然后转为文火,煎煮0.5小时,加入黄柏、肉桂,并将栀子、吴茱萸、连翘用纱布包好放于锅中,继续煎煮1小时,最后加入木通、淡竹叶、佩兰、白芨、通草、泽泻、蒲公英、金莲花、生甘草,继续煎煮0.5小时,过滤,得滤液,备用;
步骤二:将步骤一所得滤液冷却至室温,加入适量防腐剂,静置2小时左右,取上清液灌入瓶内,既得本成品。
本发明药物具有清利湿热,生肌敛疮的功效,且疗效好,时间短,不易复发,价格低廉,该中药制剂采用的方式治疗可以有效治疗口疮,减轻患者的病痛,据对临床150例患者观察统计,使用本发明药物一个疗程总有效率90%,两个疗程总有效率98%。

下面是患者使用本发明药物的临床统计资料:
一、一般资料
2011年1月至2013年1月治疗口疮患者150例。其中男性79例,女性71例,男女比例1.1:1,最大年龄70岁,最小年龄12岁,病程最长者两年多,最短者4、5天,患者年龄、性别、病程等各项基本资料基本相同,差异无统计学意义(P>0.05)。
二、治疗方法:
按照本发明药物实施例二,以及所选的制备方法制备的含漱液,一天三次,每次约20ml,含漱约3分钟,5日为一个疗程。
三、疗效评定标准:
1、治愈:临床症状和体征消失,1年内未见复发;
2、好转:口疮疼痛好转,溃疡面慢慢愈合,复发次数减少;
3、无效: 治疗前后口疮数目、形态及疼痛感,等症状均无明显改善。
四、治疗结果统计
根据统计结果显示,本发明中药在治疗口疮具有良好的临床效果,能有效提高治疗的治愈率,值得在临床应用上推广。
典型病例及疗效:
病例一、患者赵某,男,54岁,患者口腔溃烂,疼痛,并伴有口舌干燥,反复发作,曾服用维生素B2和维生素C等两周后不见好转,后来我处就诊,采用本发明实施例二中所制制剂,以及所选的制备方法,每日含漱3次,患者使用一个疗程后症状减轻,溃烂程度明显好转,疼痛症状缓解,后又进行了一段时间治疗后症状消除,口疮及舌面完全正常,至今未见复发,痊愈。
病例二、患者张某,女,22岁,患者3年期间口腔溃疡经常反复发作,尤其是遇到考试等紧张的时候,舌尖、舌边有多处溃疡面。舌质红、苔薄黄、脉虚大。溃疡加重时,进食时口舌灼痛难忍,严重影响到患者生活,后来我处治疗,采用本发明实施例二中所制制剂,以及所选的制备方法,每日含漱3次,患者使用一个疗程后症状减轻,溃烂程度明显好转,疼痛症状减轻,继续坚持服用两个疗程后,口疮愈合,舌面光滑红润,情绪紧张时也未见反复,痊愈。
病例三、患者周某,女,41岁,患者口腔黏膜多处口疮,舌尖、舌边缘等溃烂处严重,中心呈白色脓苔, 周围粘膜红肿,后来我处治疗,采用本发明实施例二中所制制剂,以及所选的制备方法,每日含漱3次,患者使用两个疗程后疼痛消失,疮面愈合,至今未见复发,痊愈。

1.一种用于口疮的中药制剂,其特征在于,该中药制剂是由以下重量份的药物制成:黄柏7-17份、知母7-17份、黄芩10-20份、生石膏15-25份、栀子7-17份、黄连4-14份、牡丹皮10-20份、木通4-14份、生地黄13-23份、淡竹叶7-17份、大黄10-20份、肉桂4-14份、吴茱萸3-9份、天花粉13-23份、麦冬7-17份、郁金10-20份、佩兰7-17份、白芨7-17份、西瓜霜3-9份、通草4-14份、泽泻7-17份、蒲公英10-20份、连翘9-19份、金莲花4-14份、生甘草4-14份。
2.根据权利要求1所述的用于口疮的中药制剂,其特征在于,由以下重量份的药物制成:黄柏12份、知母12份、黄芩15份、生石膏20份、栀子12份、黄连9份、牡丹皮15份、木通9份、生地黄18份、淡竹叶12份、大黄15份、肉桂9份、吴茱萸6份、天花粉18份、麦冬12份、郁金15份、佩兰12份、白芨12份、西瓜霜6份、通草9份、泽泻12份、蒲公英15份、连翘14份、金莲花9份、生甘草9份。
3.根据权利要求1或2所述的用于口疮的中药制剂,其特征在于:所述中药制剂为外用含漱液。
4.一种用于口疮的中药制剂的制备方法,其特征在于,制备方法如下:步骤一:按本发明重量配比,称取知母、黄芩、黄连、牡丹皮、生地黄、大黄、肉桂、天花粉、麦冬、郁金、包好的生石膏、西瓜霜放入砂锅中,加8倍量(重量)的水,先用武火将水烧开,然后转为文火,煎煮0.5小时,加入黄柏、肉桂,并将栀子、吴茱萸、连翘用纱布包好放于锅中,继续煎煮1小时,最后加入木通、淡竹叶、佩兰、白芨、通草、泽泻、蒲公英、金莲花、生甘草,继续煎煮0.5小时,过滤,得滤液,备用;步骤二:将步骤一所得滤液冷却至室温,加入适量防腐剂,静置2小时左右,取上清液灌入瓶内,既得本成品。
