介入治疗是是利用现代高科技手段进行的一种微创性治疗--就是在医学影像设备的引导下,将特制的导管,导丝等精密器械,引入人体,对体内病态进行诊断和局部治疗。介入治疗应用数字技术,扩大了医生的视野,借助导管,导丝延长了医生的双手,它的切口(穿刺点),仅有米粒大小,不用切开人体组织,就可治疗许多过去无法治疗,必须手术治疗或内科治疗疗效欠佳的疾病,如肿瘤,血管瘤,各种出血等。介入治疗具有不开刀,创伤小,恢复快,效果好的特点。是未来医学的发展趋势。
但是介入诊疗技术多采用的是穿刺、植入、插管等侵入性操作方法,属发生医院感染的高危因素之一,在日常医院感染监测中发现介入术后发热的病例较多,据有关研究发现,高龄、恶性肿瘤、植入、抗菌药物应用种数、住院时间长是介入手术后发热的主要危险因素。患者体内某些恶性肿瘤细胞产生内源性致热原,或患者长期消耗体力、接受其他治疗引起白细胞降低、机体抵抗力明显减弱,极易引起感染,这些都是引起患者发热的原因。
目前,对于介入术后发热的患者,一般会采用静脉补液,或者服用退烧药,甚至采用激素输液退烧,但是据临床患者反映,停药后会继续发烧,并且长期使用抗生素感染病毒会产生耐药性,注射激素使患者出现恶心、头晕等不良反正,西药的治疗周期长,副作用大,长期服用西药还会对肝肾功能造成不可恢复的损伤。患者经受着精神上和肉体上的双重痛苦,同时给亲人和家庭带来沉重的精神负担和经济负担。
发明内容
本发明的目的在于提供一种治疗介入术后发热的中药制剂,该中药制剂对于介入术后发热具有疗效好,治愈率高,无毒副作用,服用方便,价格低廉,治愈时间短的特点。减轻了患者的痛苦,为患者及患者的家人减轻了心里负担以及经济压力。
发明人在多年的临床医疗实践中,观察到介入术后发热的患者在治疗中的痛苦,根据该病症的发展规律,以及中药的配伍原则,并通过大量的临床验证,反复探索总结出一种治疗介入术后发热的中药制剂的新配方,能够在较短的时间内使患者恢复正常体温。本发明具有疗效显著,见效快,治愈率高,无毒副作用,服用方便,价格低廉的特点。
本发明中药物的作用:
其中:
生地黄:为玄参科植物地黄Rehmannia glutinosa Libosch. 的新鲜或干燥块根。性味:甘、寒。归心、肝、肾经。功效:清热凉血,养阴生津。可用于津伤口渴,内热消渴。本品甘寒,能于清热之中养阴生津,性寒而入血分,不仅能清营血分之热而凉血,而且味甘性寒能生津,有滋阴清热的作用,常与沙参同用。
郁金:为姜科植物温郁金Curcuma wenyujin Y. H. Chen et C. Ling、姜黄Curcuma longa L.、广西莪术Curcuma kwangsiensis S. G. Lee et C. F. Liang或蓬莪术Curcuma phaeocaulis Val.的干燥块根。性味:辛、苦,寒。归肝、心、肺经。功效:行气化瘀,清心解郁,利胆退黄,凉血破瘀。主治:胸腹胁肋诸痛,失心癫狂,热病神昏,吐血,衄血,尿血,血淋,妇女倒经,黄疸。郁金辛散苦泄,芳香宣达,性寒清热,常与柴胡、莪术配伍。
银柴胡:为石竹科繁缕属植物银柴胡Stellaria dichotoma L. var.lanceolata Bge. 的干燥根。性味:甘、微寒。归肝、胃经。功效:清虚热,除疳热。用于阴虚发热,骨蒸劳热,小儿疳热。该品甘寒益阴,清热凉血,退热而不苦泄,理阴而不升腾,为退虚热除骨蒸之常用药,常与青蒿、地骨皮、鳖甲等药同用。银柴胡不仅有清热的作用,还兼有凉血的作用。
地骨皮:为茄科植物枸杞Lycium chinense Mill. 或宁夏枸杞Lycium barbarum L.的干燥根皮。性味:甘,寒。归肺、肝、肾经。功用主治:凉血除蒸,清肺降火。治虚劳潮热盗汗,肺热咳喘,吐血,衄血,血淋,消渴,高血压,痈肿,恶疮,地骨皮和银柴胡相须为用清热凉血功效增强。地骨皮的乙醇提取物,水提取物及乙醚残渣水提物IG或IV对热原发热家免有显着解热作用。地骨皮的乙醇部分水提取物相当生药0.75-7.5g/kg时也有强的解热作用。地骨皮煎剂按2g/kg,IG或IV对大肠杆菌细胞壁提得的糖酯类所致发热家免具较强的退热作用。
北沙参:为伞形科植物珊瑚菜Glehnia littoralis Fr. Schmidt ex Miq. 的干燥根。性味:甘、微苦、微寒。归肺、肾经。功效:养阴清肺、益胃生津。用于肺热燥咳、劳嗽痰血、热病津伤口渴。研究显示:北沙参不仅有解热镇痛的作用,还可以增强人体免疫力。
生黄芪:为豆科植物蒙古黄芪 Astragalus membranaceus (Fisch.) Bge. var. mongholicus (Bge.)Hsiao 或膜荚黄芪 Astragalus membranaceus (Fisch.) Bge.的干燥根。性味:甘,微温。归肺、脾、肝、肾经。功效:补气升阳、固表敛汗、利水消肿、托疮排脓、通调水道。本品在体外对志贺氏痢疾杆菌、炭疽杆菌、甲型溶血性链球菌、乙型溶血性链球菌、白喉杆菌、假白喉杆菌、肺炎双球菌、金黄色葡萄球菌、柠檬色葡萄球菌、枯草杆菌等均有抑制作用。
丹参:为双子叶植物唇形科Labiatae鼠尾草属植物丹参Salvia /miltiorrhizaBge的干燥根及根茎。性味:苦,微寒。归经:入心、肝经。功效:活血调经,祛瘀止痛,凉血消痈,清心除烦,养血安神。用于胸肋胁痛,风湿痹痛,症瘕结块。丹参能活血祛瘀,对消除肝脏纤维结缔组织的增生,也可能有一定的作用。
茯苓:为多孔菌科真菌茯苓 Poria cocos (Schw.) Wolf 的干燥菌核。性味:甘、淡、平。归心、肺、脾、肾经。功效:利水渗湿,健脾宁心。本品甘补淡渗,既补又利,作用和缓,性平而无寒热之偏,而且益心脾而安心神,多用于心脾两虚。研究发现茯苓还具有一定的镇静作用。
香附:为莎草科植物莎草Cyperus rotundus L. 的干燥根茎。性味:辛、微苦、微甘,平。归肝、脾、三焦经。功效:行气解郁,调经止痛,安胎,理气舒郁,调和肝胃。本品辛散苦泄,为疏肝理气之要药,香附还可以调和诸药,为使药。
川芎:为伞形科植物川芎Ligusticum chuanxiong Hort.的干燥根茎。性味:辛,温。归肝、胆、心经。功效:活血行气,祛风止痛,本品辛散温通,能上行巅顶,下达血海,外彻皮毛,旁通四肢,既能活血又能行气,昔人谓川芎为血中之气药,殆言其寓辛散、解郁、通达、止痛等功能。川芎有明显的镇静作用。抗菌作用等体外试验:川芎对大肠、痢疾(宋内氏)、变形、绿脓、伤寒、副伤寒杆菌及霍乱弧菌等有抑制作用。
苍术:为菊科植物茅苍术Atractylodes lancea (Thunb.) DC.或北苍术Atractylodes chinensis (DC.) Koidz.的干燥根茎。
性味:辛、苦,温;归脾、胃、肝经;功效:燥湿健脾、祛风散寒、明目;本品苦辛性温,芳烈燥散,可升可降,走而不守,能是湿邪除而恢复脾的运化功能。苍术和香附配伍理气舒郁功效增加。
延胡索:为罂粟科植物延胡索Corydalis yanhusuo W. T. Wang 的干燥块茎。性味:辛、苦、温。归肝、脾经。功效:活血,利气,止痛。用于胸胁、脘腹疼痛,经闭痛经,产后瘀阻,跌扑肿痛。本品辛温通散,能行血中气滞,气中血滞,活血行气止痛。《本草求真》中记载:延胡索,不论是血是气,积而不散者,服此力能通达,以其性温,则于气血能行能畅,味辛则于气血能润能散,所以理一身上下诸痛,往往独行功多。
蒺藜:为蒺藜科一年生或多年生草本植物蒺藜Tribulus ter—restris L. 的成熟果实。性味:辛、苦,微温;有小毒。归肝经。功效:平肝解郁,活血祛风,明目,止痒。用于头痛眩晕,胸胁胀痛,乳闭乳痈,目赤翳障,风疹瘙痒。《本草正》中记载:"蒺藜,凉血养血,亦善补阴。用补宜炒熟去刺,用凉宜连刺生捣。去风解毒,白者良"。
三棱:为黑三棱科植物三棱 Spaganium stoloniferum Buch.-Ham. 的干燥块茎。性味辛、涩,凉。入肝、脾经。功效:破血行气,消积止痛。三棱配香附、莪术则消磨积块。
莪术:为单子叶植物姜科 Zingiberaceae 蓬莪术Curcuma phaeocaulis Val. ,或温郁金(又称温莪术)C. wenyujin Y. H. Chen et C. Ling ,广西莪术(又称桂莪术)C.kwangsiensis S. lee et C. F. Liang 的干燥根茎。性味辛、苦,温。归肝、脾经。功效:破血祛瘀,行气止痛。常配三棱。抗菌作用:莪术挥发油试管内能抑制金黄色葡萄球菌。β-溶血性链球菌、大肠埃希菌、伤寒杆菌、霍乱弧菌等的生长。莪术还具有直接杀瘤细胞的作用。
白薇:为萝藦科植物白薇Cynanchum atratum Bge。或蔓生白薇Cynanchum versicolor Bge。的干燥根及根茎。性味:苦、咸、寒。归胃、肝、肾经。功效:清热凉血,利尿通淋,解毒疗疮。用于温邪伤营发热,阴虚发热,骨蒸劳热,产后血虚发热,热淋,血淋,痈疽肿毒。本品苦寒、善入血分,有清热凉血,益阴除热之功,既能清实热,又能退虚热,用于治疗温热病热入营血,高热烦渴,神昏舌绛,常与生地黄、青蒿、地骨皮同用。
青蒿:为菊科植物黄花蒿Artemisia annua L.的干燥地上部分。性味:苦、辛,寒。归肝、胆经。功效:清热解暑,除蒸,截疟。用于暑邪发热,阴虚发热,夜热早凉,骨蒸劳热,疟疾寒热,湿热黄疸。青蒿水浸剂(1:3)在试管内对某些皮肤真菌有些抑制作用。其乙醇提取物在试管内对钩端螺旋体的抗菌浓度为7.8毫克/毫升,效力与连翘、黄柏、蚤休相似。《本草新编》中记载:"青蒿,专解骨蒸劳热,尤能泄暑热之火,泄火热而不耗气血,用之以佐气血之药,大建奇功,可君可臣,而又可佐可使,无不宜也。但必须多用,因其体既轻,而性兼补阴,少用转不得力。又青蒿之退阴火,退骨中之火也,然不独退骨中之火,即肌肤之火,未尝不共泻之也,故阴虚而又感邪者,最宜用耳。又青蒿最宜沙参、地骨皮共享,则泻阴火更捷,青蒿能引骨中之火,行于肌表,而沙参、地骨皮只能凉骨中之火,而不能外泄也"。
鳖甲:为鳖科动物鳖 Trionyx sinensis Wiegmann的背甲。性味:咸,微寒。归肝、肾经。功效:滋阴潜阳,软坚散结,退热除蒸。用于阴虚发热,劳热骨蒸,虚风内动,经闭,癓瘕,久疟疟母。银柴胡配伍鳖甲,银柴胡甘寒,清热凉血、退虚热、除骨蒸;鳖甲咸寒,滋阴潜阳、退热除蒸.二者合用,共奏养阴清热除蒸之功效,用于治疗阴虚发热、骨蒸、小儿疳积;或热病后期、余热未尽者。
神曲:为辣蓼、青蒿、杏仁等药加入面粉或麸皮混和后,经发酵而成的曲剂。性味:甘、辛、温。归脾、胃经。功效:健脾和胃,消食调中。
炒麦芽:为禾本科植物大麦Hordeum vulgare L. 的成熟果实经发芽干燥而得。性味:甘,平。归脾、胃经。功效:行气消食,健脾开胃,退乳消胀。
炒山楂:为蔷薇科植物山里红Crataegus pinnatifida Bge.var.major N.E.Br.或山楂Crataegus pinnatifida Bge.的干燥成熟果实。性味:酸、甘、微温。归脾、胃、肝经。功效:消食健胃,行气散瘀。
陈皮:为芸香科植物橘 Citrus reticulata Blanco 及其栽培变种的干燥成熟果皮。性味温,辛、苦。入脾、胃、肺经。功效:理气健脾,调中,燥湿,化痰。本品温能行气,辛能发散,苦而泄水。陈皮与枳实同用,味辛行气,芳香入脾,故能行气止痛,健脾和中,陈皮与半夏同用既能燥湿化痰,又能温化寒痰。
本发明的方解是:方中生地黄重用生地黄清热凉血、养阴生津,兼有益气活血之功,为君药之首,郁金行气化瘀,清心解郁,银柴胡配伍地骨皮除虚热,降肺火,北沙参养阴清肺,生黄芪补气升阳,配伍北沙参滋补身体,扶正祛邪,以上六味共为君药。丹参、川芎活血调经助君药益气活血,茯苓、香附、苍术、延胡索燥湿健脾、行气解郁助君药祛风散寒、滋阴养血,以上六味共为臣药。蒺藜平肝解郁,三棱、莪术辅助香附消积止痛,白薇、青蒿、鳖甲辅佐银柴胡、生地黄、地骨皮益气活血、滋阴清热,神曲、炒麦芽、炒山楂、陈皮同用辅佐君药健脾理气,以上几种共为佐药,香附兼有调和诸药的功效,为使药。方中诸药配伍、君臣协力、佐使共辅,使之具有益气活血,滋阴清热,健脾理气的功效,能够有效治疗介入术后发热。
本发明药物原料用量是经发明人进行大量摸索总结得出的,各原料用量为在下述重量份范围都具有较好的疗效:生地黄15-25份、郁金7-17份、银柴胡10-20份、地骨皮15-25份、北沙参10-20份、生黄芪19-29份、丹参19-29份、茯苓10-20份、香附10-20份、川芎7-17份、苍术10-20份、延胡索13-23份、蒺藜10-20份、三棱7-17份、莪术7-17份、白薇10-20份、青蒿15-25份、鳖甲13-23份、神曲7-17份、炒麦芽7-17份、炒山楂7-17份、陈皮10-20份。
其中优选为:生地黄20份、郁金12份、银柴胡15份、地骨皮20份、北沙参15份、生黄芪24份、丹参24份、茯苓15份、香附15份、川芎12份、苍术15份、延胡索18份、蒺藜15份、三棱12份、莪术12份、白薇15份、青蒿20份、鳖甲18份、神曲12份、炒麦芽12份、炒山楂12份、陈皮15份。
本发明药物可以采用中药制剂的常规制备工艺,可制成任何一种药剂学上所说的剂型,如丸剂、散剂、片剂、胶囊剂、颗粒剂、糖浆剂、口服液等。例如可以将这些原料药研成粉末状,粒度为80-120目,将所得细粉投入混合药物搅拌机充分混匀,制成散剂温水冲服。可以用水煎,经过滤得滤液,加入防腐剂制成口服液;必要时也可以直接服用汤剂。
将上述各中药原料制成本发明中药丸剂优选的生产方法是:
步骤一:按本发明重量配比,称取生地黄、郁金、银柴胡、地骨皮、北沙参、生黄芪、丹参、川芎放入容器中,用水蒸气蒸馏法提取挥发油,挥发油和蒸馏后的水溶液备用,滤过,药渣备用;
步骤二:称取香附、苍术、延胡索、蒺藜、三棱、莪术、白薇、青蒿、陈皮加水煎煮两次,第一次2小时,第二次加入步骤一中的药渣再煎煮0.5小时,合并两次煎液,滤过;将所得滤液浓缩至75-85℃时,加入75%乙醇,放置沉淀,过滤,弃沉淀,滤液回收乙醇,在70℃环境下相对密度1.10~1.15的清膏;
步骤三:称取茯苓、鳖甲、神曲、炒麦芽、炒山楂,将其研磨粉碎至200目筛;
步骤四:将步骤一所得挥发油和步骤三所得细粉加入步骤二所得清高中,搅拌混着均匀,加入蜂蜜,既得治疗介入术后发热的丸剂。
本发明药物具有益气活血,滋阴清热,健脾理气的功效,且疗效显著,见效快,治愈率高,无毒副作用,服用方便,价格低廉,该中药制剂采用的方式治疗可以减少西药对肝肾功能的损害,退烧后不易反复。据对临床240例患者观察统计,使用本发明药物一个疗程总有效率90%,两个疗程总有效率95%。

实施例二、生地黄20份、郁金12份、银柴胡15份、地骨皮20份、北沙参15份、生黄芪24份、丹参24份、茯苓15份、香附15份、川芎12份、苍术15份、延胡索18份、蒺藜15份、三棱12份、莪术12份、白薇15份、青蒿20份、鳖甲18份、神曲12份、炒麦芽12份、炒山楂12份、陈皮15份。
实施例三、生地黄25份、郁金17份、银柴胡20份、地骨皮25份、北沙参20份、生黄芪29份、丹参29份、茯苓20份、香附20份、川芎17份、苍术20份、延胡索23份、蒺藜20份、三棱17份、莪术17份、白薇20份、青蒿25份、鳖甲23份、神曲17份、炒麦芽17份、炒山楂17份、陈皮20份。
下面是患者使用本发明药物的临床统计资料:
一、一般资料及治疗方法
2011年11月至2013年10月治疗介入术后发热患者240例。按照本发明药物实施例二,以及优选的制备方法制备的蜜丸,早晚各服一次,每次2粒,温服。3日为一个疗程。
二、疗效评定标准:
1、治愈:患者体温恢复正常,其他临床症状消失,停药后未见复发;
2、好转:患者体温得到控制,其他临床症状好转;
3、未愈:患者一直高烧不退,临床症状无任何改善。
三、治疗结果统计
由上述可以看出,本发明中药在治疗介入术后发热具有良好的临床效果,能有效提高治疗的治愈率,值得在临床应用上推广。
典型病例及疗效:
病例一、黄某,男,35岁,突然腹部疼痛难忍,后确诊为腹主动脉夹层瘤三型,一周后进行了介入手术,术后一周发烧不断,有时高烧有时低烧,所做检查都为正常,医院采用静脉滴注,未见效果,后采用本发明实施例二中所制蜜丸,服用3天后退烧,继续服用2天,停药后连续观察2个星期,未见复发。
病例二、刘某,女,52岁,在医院行冠脉造影及支架介入治疗。术后常规应用头孢类抗生素静脉滴注,第2天患者发热,最高 39℃,使用消炎痛栓肛塞,温度降到 37. 5℃,下午体温再度反弹,达到 39. 4℃。持续5天左右,患者一切检查均正常,后采用本发明实施例二中所制蜜丸,服用1个疗程后体温恢复正常,全身乏力、厌食症状消失,继续服用该药物1个疗程,患者临床症状完全消失各项体征恢复正常,停药后连续观察2个星期,未见复发。
Claims (4)
1.一种治疗介入术后发热的中药制剂,其特征在于,该中药制剂是由以下重量份的药物制成:生地黄15-25份、郁金7-17份、银柴胡10-20份、地骨皮15-25份、北沙参10-20份、生黄芪19-29份、丹参19-29份、茯苓10-20份、香附10-20份、川芎7-17份、苍术10-20份、延胡索13-23份、蒺藜10-20份、三棱7-17份、莪术7-17份、白薇10-20份、青蒿15-25份、鳖甲13-23份、神曲7-17份、炒麦芽7-17份、炒山楂7-17份、陈皮10-20份。
2.根据权利要求1所述的治疗介入术后发热的中药制剂,其特征在于,由以下重量份的药物制成:生地黄20份、郁金12份、银柴胡15份、地骨皮20份、北沙参15份、生黄芪24份、丹参24份、茯苓15份、香附15份、川芎12份、苍术15份、延胡索18份、蒺藜15份、三棱12份、莪术12份、白薇15份、青蒿20份、鳖甲18份、神曲12份、炒麦芽12份、炒山楂12份、陈皮15份。
3.根据权利要求1或2所述的治疗介入术后发热的中药制剂,其特征在于:所述中药制剂为丸剂。
4.一种治疗介入术后发热的中药制剂的制备方法,其特征在于其制备方法如下:步骤一:按本发明重量配比,称取生地黄、郁金、银柴胡、地骨皮、北沙参、生黄芪、丹参、川芎放入容器中,用水蒸气蒸馏法提取挥发油,挥发油和蒸馏后的水溶液备用,滤过,药渣备用;步骤二:称取香附、苍术、延胡索、蒺藜、三棱、莪术、白薇、青蒿、陈皮加水煎煮两次,第一次2小时,第二次加入步骤一中的药渣再煎煮0.5小时,合并两次煎液,滤过;将所得滤液浓缩至75-85℃时,加入75%乙醇,放置沉淀,过滤,弃沉淀,滤液回收乙醇,在70℃环境下相对密度1.10~1.15的清膏;步骤三:称取茯苓、鳖甲、神曲、炒麦芽、炒山楂,将其研磨粉碎至200目筛;步骤四:将步骤一所得挥发油和步骤三所得细粉加入步骤二所得清高中,搅拌混着均匀,加入蜂蜜,既得治疗介入术后发热的丸剂。
