本发明的背景

MHC分子有两类:MHCI类分子可以在大多数含有细胞核的细胞中找到,它提呈内源性蛋白、DRiP和大肽水解切割产生的肽。MHCII类分子主要可以在专职性抗原提呈细胞(APC)中发现,它提呈被APC吞噬、随后处理的外源性蛋白的肽(CresswellP.,《年度免疫学评论》,1994年,12:259-93)。肽和MHCI类分子的复合物被携带有适当T细胞受体(TCR)的CD8+CTL识别,而肽和MHCII类分子的复合物被携带有适当TCR的CD4+辅助T细胞识别。因此,TCR、肽和MHC因此按照1:1:1的化学计量呈现,这一点已是共识。
引发细胞免疫反应的肽必须结合到MHC分子上。这个过程取决于MHC分子的等位基因和肽的氨基酸序列。MHCI类分子结合肽长度通常为8、9或10个氨基酸残基,在与MHC分子对应结合槽相互作用的序列中有保守残基(“锚定物”)。因此,每个MHC等位基因都有一个决定哪些肽可以特异性结合到结合槽的“结合基序”(RammenseeH.G.,BachmannJ.Stevanovic,S.《MHC配体和肽基序》,查普曼和霍尔公司,1988年)。为了引发免疫反应,肽不仅要结合到某些MHC分子上,还必须能够被带有特异性TCR的T细胞识别。有效免疫反应的另外一个前提条件是对抗原没有免疫耐受性。
来源于CTL所识别表位的肿瘤相关抗原(TAA)可以是来自所有类型蛋白质(如酶、受体、转录因子等)的分子,它们在各自的肿瘤细胞中是上调的。另外,抗原可能具有肿瘤特异性,即每种肿瘤细胞特有的抗原(如突变基因、或来自于选择性开放读码框(ORF)、或来自于蛋白质剪接的产物)(VigneronN,StroobantV,ChapiroJ,OomsA,DegiovanniG,MorelS,vanderBruggenP,BoonT,VandenEyndeBJ.《科学》,2004年4月23日,304(5670):587-90)。另一类重要抗原是组织特异性抗原,如“肿瘤睾丸”(CT)抗原,该类抗原在各种肿瘤以及睾丸的健康组织中表达。
因此,TAA是肿瘤疫苗研制的起点。TAA的识别和鉴定的方法基于比如使用从患者或健康受试者中分离的CTL或肿瘤和正常组织产生的差异性转录图谱或差异性肽表达图谱(LemmelC.,WeikS.,EberleU.,DengjelJ.,KrattT.,BeckerH.D.,RammenseeH.G.,StevanovicS.,《国家生物科技》,2004年4月,22(4):450-4;T.Weinschenk,C.Gouttefangeas,M.Schirle,F.Obermayr,S.Walter,O.Schoor,R.Kurek,W.Loeser,K.H.Bichler,D.Wernet,S.Stevanovic,H.G.Rammensee,《癌症研究》,2002年,62(20):5818-5827)。
但是,如果相应抗原是基于T细胞免疫疗法的有效靶点,那么对于在肿瘤组织或人肿瘤细胞系中过度表达或选择性表达的基因,其鉴定不能提供足够的信息。这是因为只有这些抗原表位的个别亚群a)被呈现并b)被携带有相应TCR的T细胞识别。此外,这一特定表位应不具有免疫耐受性,或其免疫耐受性可忽略不计。因此,应仅从显示与MHC分子相连并且是功能性T细胞靶点的过度或选择性表达蛋白中选择肽,这一点至关重要。功能性T细胞是带有特异性抗原的T细胞,该细胞在受刺激时能进行克隆式扩增,并能执行效应物的功能(“效应T细胞”)。
在抗肿瘤免疫中,T辅助细胞在协调CTL的效应功能方面起重要作用。引发TH1型T辅助细胞反应的T辅助细胞表位支持CD8+CTL的效应功能,这包括针对在细胞表面提呈肿瘤相关肽/MHC复合物的肿瘤细胞的细胞毒性功能。这样,肿瘤相关T辅助细胞表位可以单独或与其他肿瘤相关肽一起,作为刺激抗肿瘤免疫反应的疫苗组合物的活性药物成分。
因为CD8和CD4依赖这两种反应共同、协同产生抗肿瘤效果,所以CD8+CTL所识别的TAA(配体:MHCI类分子+肽表位)和CD4+T辅助细胞所识别的TAA(配体:MHCII类分子+肽表位)的识别和鉴定对有效肿瘤疫苗的研制和基于这些疫苗的有效治疗至关重要。
在欧洲,肾细胞癌(RCC)是男性第七大最常见的恶性肿瘤,每年新增29,600例(占全部癌症的3.5%)。在女性中,每年有16,700例(排名第十二位或占全部癌症的2.3%)。RCC在40岁之前很少见,超过这个年龄,男性患病率是女性的两倍。随着年龄增加,发病率迅速攀升,在40岁以下人群中,每10万人中每年患者数为2人,而在65–69岁年龄段的人群中,每10万人中每年患者数为38人。在75岁以上的人群中,每10万人中每年患者数上升到46人。
RCC患者中共有25–30%的患者在首发症状时出现明显的转移。约三分之一的RCC患者会随着时间推移而患转移性疾病。因此,在所有RCC患者中,有将近50–60%的患者最终会出现转移性疾病。在转移性疾病患者中,约75%的患者有肺部转移瘤,36%的患者有淋巴结和/或软组织受累,20%的患者有骨骼受累,18%的患者有肝脏受累。
与五年存活率分别为82%和100%的膀胱癌或前列腺癌相比较,RCC是泌尿生殖系肿瘤中最致命的癌症,其五年存活率为65%(美国1972-2001年数据)。在欧洲,RCC确诊(1990-1994)后五年(截至1999)平均存活率大约是58%,一些作者将RCC归为仅中度预后的癌症。总的来说,RCC对近80%的患者有致命性。该数据表明,患者对有效的早期临床随访和复发治疗有强烈的需求。
存活期在很大程度上取决于肿瘤是在哪一阶段诊断出来的:有远处转移病变的患者,其五年存活率仅为12%,但是患有局部恶性肿瘤的患者,其五年存活率为80%。
结肠直肠癌(CRC)是全球第三大常见癌症。其每年新增病例约为100万例,与大多数其他肿瘤不同,CRC的男女患者数量并无显著区别(比例为1.2:1)。在欧洲,CRC是第二大常见癌症,也是男性和女性第二大常见癌症相关死因,每年新增病例约38万例,疾病相关的死亡率约为20万例。2002年,男性和女性的原始发病率分别为每10万人中88.3例和84.0例,每10万人中原始死亡率分别为34.8例和35.2例。这些数据清楚地反映了CRC作为个人和社会负担一大来源的重要性。CRC是一种老年人癌症,因为男性和女性病症表现的平均年龄分别是69岁和75岁。除了饮食和生活方式因素(如肥胖、缺乏身体锻炼、吸烟、经常饮酒)外,其他危险因素包括:家族性CRC、遗传性CRC(家族性腺瘤性息肉病[FAP]、衰减型FAP[衰减型腺瘤性息肉病;AAPC]、遗传性非息肉病性结肠直肠癌[HNPCC]、错构瘤息肉综合症)和溃疡性结肠炎、克罗恩病等炎性肠疾病。
CRC主要表现为直肠、乙状结肠、横结肠/降结肠和升结肠/盲肠黏膜上的腺癌。早期结肠直肠癌可以通过一期手术治愈。但是,远处转移扩散到局部淋巴结和肝、肺以及其他器官(如CNS)。因为具有非特异性症状,CRC经常是在相对晚期才被诊断出来,约有25%的CRC患者在第一次就医时已患有转移性疾病(mCRC)。在新诊断出患有局部可切除CRC的患者中,另外有30%的患者随后会出现转移复发。
EP2105740描述了在患有或易患癌症的受试者中,包括载脂蛋白AI(APOA1)在内的一些蛋白质受c-myc过度表达的调控。因此,EP2105740描述了生物标记APOA1在癌症(特别是肺癌)诊断、预后和/或治疗监测中的使用,但未提供对治疗有效性的预测,更未提及免疫疗法。
WO2010/076322描述了一种用于预测癌症患者对化疗的反应和/或化疗对患者益处的方法,这包括(i)把肿瘤至少分成两类,(ii)确定肿瘤样品中表明每一单独类别肿瘤对化疗反应的至少一个标记基因的表达,(iii)根据上述基因表达,预测上述反应和/或益处;其中一个标记基因是CXCL13。WO2010/076322同样未提供对治疗有效性的预测,更未提及免疫疗法。
同样,WO2010/003773描述了对曾被诊断为淋巴结阳性并接受了细胞毒性化疗的癌症患者进行癌症疗效预测的方法;其中一个标记基因是CXCL13。WO2010/003773未提供对免疫治疗有效性的预测。
EP1777523A1涉及患者癌症治疗效果的预后,预后是基于表明上述患者对上述癌症有适应性免疫反应或该适应性免疫反应水平的一个或几个生物标记的量化。总的来说,EP1777523A1公布了大量标记。此外,EP1777523A1还涉及患者癌症治疗效果的预后(而非预测),这是基于表明上述患者在肿瘤部位对上述癌症有适应性免疫反应或该适应性免疫反应水平的一个或几个生物标记的检测和/或量化。
目前,尽管我们在RCC和CRC诊断和治疗方面取得了新进展,但是为进一步提高存活率和更好地调整患者的治疗,我们仍然需要用于提高诊断(特别是预测免疫疗法能否达到预期有益效果)的生物标记。此外,这些标记还应该能够对上述癌症治疗方法的效果进行预测。因此,本发明的目的是提供相应的生物标记以及诊断、预测和预后方法。
在本发明第一个方面,上述目的是通过提供预测癌症患者免疫治疗效果的方法来实现的,包括a)确定上述癌症患者样品中至少一个以下标记的水平:载脂蛋白A1(ApoA1)、CCL17/TARC、嗜酸性粒细胞(用绝对值或%表示)、单核细胞(用绝对值或%表示)、CD95/Fas、天冬氨酸转氨酶/血清谷草转氨酶(ASAT/SGOT)、癌抗原19-9(CA19-9)、乳酸脱氢酶(LDH)、苏氨酸、免疫球蛋白E(IgE)和基质金属蛋白酶3(MMP-3),其中与特定癌症患者人群的中值相比,标记水平升高(或增加)表明免疫疗法对上述患者产生了有益效果,或b)确定上述癌症患者样品中至少一个以下标记的水平:CXCL13/BCA-1、中性粒细胞(用%表示)、白细胞介素6(IL-6)和短链酰基肉毒碱,其中与特定癌症患者人群的中值(+/-10%)相比,标记水平降低(或减少)表明对上述患者产生了有益的免疫治疗效果。
本方案中的发明涉及单变量(也称为“单一”或“独立”)标记,用于预测效果,特别是文中所述癌症患者免疫治疗的有益效果,如较长的总存活期、免疫疗法诱导单个和/或多个T细胞反应产生、抑制肿瘤生长、肿瘤萎缩或较长的无进展存活期。
本发明中的术语“标记”、“生物标记”、“分析物”或“参数”可互换使用,都是指本发明方法中分析的标记。
本发明中所称“预测”或“预测标记”应以本发明标记为依据,本发明标记提供了有关癌症免疫治疗(如使用文中所述疫苗)有效性的充分信息。
经确定,CXCL13/BCA-1、载脂蛋白A1、中性粒细胞、嗜酸性粒细胞、单核细胞、CD95/Fas、天冬氨酸转氨酶/血清谷草转氨酶、CCL17/TARC、短链酰基肉毒碱和CA19-9可以预测总存活期,在某些情况下,还可确定T细胞反应(通过单变量分析)。
首选方法为本发明确定的方法,即将ApoA1和/或CCL17/TARC作为上述标记。高于各相应研究人群中值的CCL17/TARC水平以及高于相应检测供应商所示中值(如RulesBasedMedicine(RBM)为0.288mg/ml)的ApoA1水平均为阳性。

此外,首选标记组合选自CXCL13/BCA-1、中性粒细胞%、ApoA1、嗜酸性粒细胞%、嗜酸性粒细胞ABS(绝对值)、单核细胞%、FAS、TARC、LDH、Thr、IL-6、白蛋白、IgE、MMP-3、CA19-9、单核细胞ABS、ASAT、胆红素、酰基肉毒碱(scAC)和IL-33。这些标记组合均应作为特别首选。
ApoA1是高密度脂蛋白(HDL)的主要蛋白质成分,在临床分析中可以代替HDL进行测量。在正常情况下,HDL显示抗动脉粥样硬化、抗氧化剂、抗血栓形成和抗炎特性。但是,在慢性炎症/氧化情况下(如慢性感染、自体免疫疾病、代谢综合症和癌症),HDL将失去这些特性,集中表现为促炎特性。在促炎HDL(piHDL)中,ApoA1(负急性期蛋白)减少,而血清淀粉样蛋白A(SAA,正急性期蛋白)等其他蛋白质增加。因此,ApoA1水平低表明有慢性炎症或氧化情况,反之则可能促进已知有利于癌症发展的情况。据报道,多种癌症的ApoA1水平降低,这一现象也曾在IMA901-202(第II期)研究组的RCC患者中观察到。特别是,有研究表明相关蛋白ApoA2在转移性RCC中预后良好,小鼠模型显示ApoA1有抑制癌症进展的功能(Su等.,2010年;Vermaat等,2010年)。除癌症促进作用外,已知慢性急性期反应和氧化情况也不利于适应性免疫反应(Haeryfar和Berczi,2001年;Muller等,2008年;Vallejo等,2004年)。
CCL17/TARC是一种趋化因子,最初被归为趋化因子吸引TH2细胞,但是它也有其他细胞靶点,如TH1-和TH2-型效应/记忆T细胞、特别是那些存在于皮肤的细胞、CD8+T细胞亚群、TH17记忆T细胞、NK和NKT细胞、树突状细胞等。小鼠研究表明,肿瘤内的CCL17/TARC表达有利于免疫排斥。血清水平可能反映髓系树突状细胞、巨噬细胞和单核细胞的活性,这些细胞是该趋化因子的主要来源。树突状细胞在稳态下产生CCL17,这是该细胞在T细胞中引发非抗原依赖性反应这一独特功能的前提条件。在特应性皮炎、某些其他过敏或自体免疫等情况下,CCL17/TARC的病理性增加,表明TH2活性有所提高。与此相反,IMA901-202研究人群的CCL17/TARC水平在正常范围内,与TH1细胞因子如IL-12和IFN-γ相平衡。
在本发明中,ApoA1水平和CCL17/TARC水平高于晚期RCC患者中发现的中值水平,视为有利于治疗成功。与有两个因素低于截断值的患者比较,有至少一个因素高于指定截断值(统计学上约为75%)的患者在用癌症疫苗IMA901(德国图宾根immaticsBiotechnologies公司)接种后,预计会有改善的临床结果。有两个因素高于指定截断值(统计学上约为25%)的患者预计会从治疗中受益最多。ApoA1的正常低限(LLN)取决于用于分析的检测方法。使用Luminex基于微球的多重技术(RBM),LLN为0.288mg/ml。使用不同的方法时,必须通过利用供应商指定的资料,比较检测方法和已确定方法来弥补实验或测量统计学相关数量的健康供者样品来调整LLN。特定患者人群CCL17/TARC和/或ApoA1水平的中值取决于所选人群和用于定量分析的检测方法。
首选标记(特别是ApoA1和CCL17/TARC)测量方法可从ELISA、基于微球/芯片/板的多重免疫测定法、蛋白质组学等免疫法中选择,或从质谱法、生物学测定、电泳、免疫浊度测定法、免疫比浊法、酶法、比色法或荧光法(如可用光度测定法评估的荧光法),和基于荧光激活细胞分选(FACS)技术中选出。中性粒细胞、嗜酸性粒细胞和单核细胞可以用FACS或其他临床上已建立的血液化验法测定。ApoA1与HDL胆固醇高度相关,因此也可以用各种测定HDL胆固醇的方法来测定。
此外,B细胞吸引趋化因子1(CXCL13/BCA-1)已被确定为标记。CXCL13/BCA-1似乎是在淋巴器官中组织淋巴细胞组合的必要成分。在一些肿瘤实体中,该成分含量的增加与转移和不良预后有关,也可能与受损的抗肿瘤免疫反应有关。
以百分比和绝对值表示的嗜酸性粒细胞水平被确定为标记,该细胞作用的报告可能会相互矛盾,该细胞对癌症起着多个并相反的作用。在几个病例中,嗜酸性粒细胞聚集在肿瘤处,显然是阳性的。在黑色素瘤中,嗜酸性粒细胞聚集在GM-CSF注射部位。
以百分比和绝对值表示的单核细胞水平被确定为标记,该细胞聚集在炎症部位或GM-CSF给药部位。单核细胞可分化成巨噬细胞或树突状细胞,其本身可以作为抗原提呈细胞。单核细胞可以取代外移组织树突状细胞(如朗格汉斯细胞)。研究表明,高水平的单核细胞是癌症和RCCIL-2治疗的不利因素,这可能因为它产生活性氧簇(ROS)并抑制NK细胞和T细胞。
可溶性CD95/Fas是由选择性剪接产生的,它与膜结合CD95竞争结合CD95L/FasL。因此,它可能抑制T细胞凋亡(如在肿瘤部位,称为肿瘤反击)。它也可能抵消促进癌细胞迁移和侵袭的CD95信号传导的非凋亡功能。CD95还可能与嗜酸性粒细胞相关,抑制该细胞的凋亡。与此相反,CD95也可能抑制CD95L介导的T细胞对肿瘤杀伤,在一些实体中,它与高肿瘤负荷和不良预后有关。
中性粒细胞是已知的癌症不良预后标记。在几个癌症实体中,中性粒细胞和淋巴细胞的高比例表明不良预后。这里还给出了癌症免疫治疗的负面预测影响。中性粒细胞可能通过抑制T细胞和NK细胞来抵消免疫反应。
在肾小球滤过率(GFR)降低的情况下,短链酰基肉毒碱增加,表明对肾脏的损害增大,这可能导致进入肿瘤的血细胞减少(Wanner1988年)。同时,有报告称癌症患者的酸溶性(自由和短链)酰基肉毒碱低于对照组(Sachean1987年)。
CA19-9是唾液酸化路易斯A结构(位于MUC1等黏蛋白上的一种糖类抗原)上的表位。血清中的CA19-9可能与肿瘤的MUC1表达有关,从而与一种疫苗抗原相关。而血清肿瘤标记通常是不良预后标记。
天冬氨酸转氨酶/血清谷草转氨酶(ASAT/SGOT)是一种参与氨基酸代谢的酶。与LDH类似,血清中ASAT/SGOT的出现频率增加表明细胞快速更新,如同在肝脏和其他器官损伤情况下出现,也在或癌症出现。我们的结果证实,高ASAT/SGOT和高LDH是癌症的不良预后因素,但意外的是,结果也表明,高ASAT/SGOT和高LDH是预测免疫治疗效果的积极因素。其原因尚不清楚。据推测,肿瘤内的细胞死亡(特别是坏死形式的细胞死亡)可能有利于肿瘤部位的免疫反应。
另一方面,本发明中的方法还包括c)在上述癌症患者的上述样品中,从白蛋白和直接胆红素中确定至少一个标记,其中与特定癌症患者人群中值(+/-10%)相比,较高的水平表明对上述患者产生有益的免疫治疗效果,或d)确定上述癌症患者的上述样品中白细胞介素33(IL-33)标记的水平,其中与特定癌症患者人群中值(+/-10%)相比,较低的水平表明对上述患者产生有益的免疫治疗效果。
本发明这一方面使用指定的标记以进一步支持本发明第一方面所述的单变量标记。因此,使用标记的目的是为诊断创建一个“多变量”标记组或标记面板。
特别首选方法为本发明中的方法,其中上述多变量标记组或面板包含以下标记:CXCL13/BCA-1、ApoA1、中性粒细胞、嗜酸性粒细胞(用百分比和绝对值表示)、单核细胞(用百分比和绝对值表示)、CD95/FAS、天冬氨酸转氨酶/血清谷草转氨酶、CCL17/TARC、LDH、Thr、IL-6、短链酰基肉毒碱、白蛋白、胆红素、IgE、IL-33、MMP-3和CA19-9。
考虑到本发明的目的,标记参考值是每个标记的阈值,该阈值定义了患者是否从免疫治疗中受益。取决于标记是阴性或阳性标记,如果是阴性标记,患者的标记水平高于参考值表明免疫治疗是有益的;相反,如果是阳性标记,患者的标记水平低于参考值表明免疫治疗是有益的。在一个方案中,参考值是在特定癌症患者人群中观察到的中值浓度+/-10%。更应优选的是,取决于参考值是负值或正值,如果参考值为负值,则该值是特定癌症患者人群的上四分之一、上五分之一或上十分之一数值;如果如果参考值为正值,则该值是特定癌症患者人群的下四分之一、下五分之一或下十分之一数值。最应优选的是,认为患者从治疗中受益的阴性标记其参考值是70%、80%,90%(首选)。对于阳性标记,是从认为患者从治疗中受益的参考值开始,最好是30%、20%,10%(首选)。
本发明的另一重要方面涉及本发明的方法,其中上述免疫疗法包括使用抗癌疫苗及可选的佐剂诸如GM-CSF进行接种。
描述的免疫疗法和各种疫苗都处于先进水平;癌症患者免疫治疗目的是专门激活免疫系统的细胞(特别是抗肿瘤细胞)但不抑制健康组织的细胞毒性T细胞(CTL,也称为“杀伤细胞”、CD8+T细胞)。肿瘤细胞与健康细胞的不同之处在于肿瘤细胞表达肿瘤相关和肿瘤特异性蛋白。细胞表面的HLA分子把部分细胞内容物呈现在外部,以便CTL区分健康细胞和肿瘤细胞。这是通过把细胞内的所有蛋白质分解成短肽,然后短肽附着到HLA分子上并在细胞表面呈现来实现的。在人体肿瘤细胞而非健康细胞或极小数量的健康细胞上呈现的肽称为肿瘤相关肽(TUMAP)。产生肿瘤特异性T细胞所识别表位的抗原,可以是来自所有蛋白质类,如酶、受体、转录因子等的分子。
但是,激发一种CTL通常不足以去除所有的肿瘤细胞。肿瘤非常容易致突变,通过改变其蛋白质图谱来躲避CTL的识别,从而能够快速对CTL攻击做出反应。为了反击肿瘤的逃逸机制,接种时使用多种特异性肽。这样,几种CTL克隆,可以同时对肿瘤发起大范围地并行攻击。这可以减少肿瘤逃避免疫反应的机率。这种假设最近在治疗后期黑色素瘤患者的临床研究中得到了证实。仅有少数例外情况,至少有三种不同T细胞反应的患者出现客观临床反应或病情稳定及存活期增加,而T细胞反应少于三种的大多数患者被诊断患有进展性疾病(Banchereau等,2001年)。
本发明中方法的首选疗法/组合物是肽肿瘤疫苗。其他首选治疗包括基于DNA或RNA的疫苗,如Weide等所述的疫苗(WeideB,GarbeC,RammenseeHG,PascoloS,《免疫学快报》,2008年1月15日,115(1):33-42。电子版,2007年10月26日)、树突状细胞疫苗、使用原发肿瘤细胞或细胞系溶解产物的疫苗或包括全蛋白或热激蛋白在内的肿瘤细胞选择性成分。药剂可以每天通过肌肉注射、皮下注射、腹腔注射及静脉注射直接注入患者,即受影响的器官或全身,或在体外用于来自患者的细胞或人类细胞系,然后注入患者,或在体内用于选择来自患者的免疫细胞亚群,然后再注入患者。肽等疫苗抗原可能单独使用,或与免疫刺激佐剂(如下所示)、免疫激活细胞因子或合适的缓释系统(如脂质体)同时使用。肽也可能与合适的载体如钥孔虫戚血蓝蛋白(KLH)或甘露聚糖(见WO95/18145及Longenecker等(1993年))偶联。肽也可能被标记,可能是融合蛋白或杂交分子。在本发明中给出序列的肽预计能刺激CD4或CD8T细胞。但是,在CD4T辅助细胞的帮助下,CD8CTL刺激会更加有效。因此,对于刺激CD8CTL的MHCI类表位,杂交分子的融合配偶体或融合部分适当提供刺激CD4+T细胞的表位。CD4+刺激表位是本领域所熟知的,它包括那些在破伤风类毒素中识别的表位。在进一步优选方案中,肽是一种融合蛋白,含有HLA-DR抗原相关不变链(Ii)的N端氨基酸。在一个方案中,本发明中所称肽是一种截短的人体蛋白或蛋白质片段和其他多肽部分的融合蛋白,如果人体部分含有本发明的一个或多个氨基酸序列。
为了便于使用,疫苗也可能含有一种或多种佐剂。首选佐剂是咪喹莫特、瑞喹莫德、GM-CSF、环磷酰胺、舒尼替尼、贝伐单抗、干扰素α、CpG寡核苷酸及衍生物、多聚(I:C)及衍生物、RNA、西地那非和PLG或病毒颗粒的微粒制剂。如前所述,药剂用于非消化道给药,如皮下、皮内、肌肉注射或口服给药。为此,肽及可选的其他分子溶解或悬浮在药学上可接受的、最好是在水性载体中。此外,组合物可能含有辅料如缓冲液、粘合剂、爆破剂、稀释剂、香精、润滑剂等。肽也可能与免疫刺激物质(如细胞因子)一起使用。能够在此组合物中使用的辅料长名单可参考美国药学协会和医药出版社出版的《药用辅料手册》(作者为A.Kibbe)第3版。组合物可以用于癌症疾病诸如RCC和CRC的防止、预防和/或治疗。本发明中使用的有代表性的疫苗肽组合物分别指定为IMA901、IMA910、IMA941和IMA950,见表1A至表4D。

综上,根据本发明方法的另一优选方面,上述抗癌疫苗是从含有至少一种致免疫肽的抗癌疫苗中选出的,致免疫肽是从序列号为1至37的一组中选出的,例如包括序列号1-10、11-19及1、5、8和9,序列号20-29和30-37。
根据本发明方法的另一方面是,上述患者已接受过治疗或预先治疗。这种预先治疗可能包括如治愈性手术、放疗和/或化疗。首选预先治疗方法是使用从细胞因子和酪氨酸激酶抑制剂(TKI,如索拉菲尼、舒尼替尼和环磷酰胺)中选出的抗癌药进行治疗。在这一方面,上述癌症治疗可以从上述治疗方法中选择,首选是免疫疗法,最好包括抗癌疫苗的使用,可选择与GM-CSF一起使用。
适用的癌症包括对免疫疗法有反应的所有癌症,最好是选自RCC、CRC、胃癌(GC)、黑色素瘤、非小细胞肺癌(NSCLC)、胶质母细胞瘤或各种类型腺癌。
本发明中分析的样品可从全血、外周血或其中部分、血清、血沉棕黄层、肿瘤组织、淋巴液、尿液、骨髓、EDTA抗凝血浆、肝素化血浆、柠檬酸钠抗凝血浆、肝素化全血和冻存肝素化全血中选择。待分析样品的选择还将取决于待分析的标记,技术人员将能相应地选择合适的样品。
在首选方案中,本发明方法还可能包括患者癌症免疫治疗效果的预后,其中上述患者最好用环磷酰胺预先治疗过(如上所述)。本发明所称“预后”或“预后标记”应以文中所述标记为依据,这些标记提供了有关基于常规或常见癌症化疗方法(如使用紫杉醇、铂化合物以及在癌症化疗中使用其他常见药剂)效果的充分信息。预测效果可以从总存活期、对免疫治疗产生单一和/或多个T细胞反应、抑制肿瘤生长或无进展存活期中选择。因此,本发明中的标记可用于有预测和预后用途的“混合”方案。
本发明的另一方面是本发明方法还包括对上述患者上述癌症治疗效果的监测,包括至少重复一次文中公布的决定步骤a)和/或b),步骤c)和d)为可选步骤。在治疗过程中监测通常是定期进行的,如每周一次、一周两次甚至一月一次。
本发明中的方法为首选方法,其中上述决定步骤包括以下至少一种方法:免疫测定法、基于微球的免疫测定法、多重免疫测定法、ELISA、基于微阵列测定法、表观遗传学分析法、表达分析法、FACS分析法、质谱法、临床血液学方法及其他常规临床检验方法(如电泳、免疫浊度测定法、免疫比浊法、酶法、比色法或荧光法)。以上所有方法均为该领域专业人士熟知的方法,并在文献中有所描述。

试剂盒还可能包括一种或多种(iii)缓冲液、(iv)稀释剂、(v)过滤器、(vi)注射针或(v)注射器。容器最好是瓶子、小瓶、注射器或试管,可以为多用途容器。容器可由各种各样的材料如玻璃或塑料制成。试剂盒和/或容器最好有容器或关于容器的说明书,指明重组和/或使用的方法。例如,标签可能说明冻干制剂要与适用于上述方法(如ELISA)的某些抗体浓缩液进行重组。
此外,本发明另一优选方面涉及癌症患者治疗的改进方法,其中包括a)执行本发明的方法,同上;b)根据步骤a)获得的结果,给上述癌症患者进行适当的抗癌免疫治疗;和c)(可选项)重复步骤a)和b)。

示例
1.引言
IMA901是一种肽疫苗,能够诱导特异性T细胞产生针对RCC中发现的肽-MHC复合物的反应。IMA901-202(第II期)研究利用单变量和多变量分析,确定可以单独或联合用作转移性RCC患者IMA901治疗免疫反应诱导和总存活期延长成功预测标记的参数。
为达到此目的,利用IMA901-202(第II期)研究人群的预先治疗样品对约450个参数(患者或肿瘤相关参数以及血清或尿液中测量的分析物和细胞参数)进行分析。对于多变量分析,选出一组几个参数,为限制以后试验的测量成本,需要制备外周血单核细胞(PBMC)的这些参数不包括在内。对参数与总存活期和T细胞反应的相关性进行了分析。
因为环磷酰胺预先治疗后的IMA901治疗对总存活期有更大影响,并且只在+CY组免疫反应的产生与总存活期提高有关,所以+CY组被视为研究的治疗组,与作为对照的–CY组进行比较。因此,选择显示与环磷酰胺预先治疗有相互作用或预测+CY组总存活期好于–CY组的参数。从而可以避免选择纯预后标记,按照定义这些标记可能显示与两组的总存活期都有关。
2.研究患者与年龄相仿的健康供者的比较
为了对预先治疗水平和健康供者进行比较,仅选择70岁以下的意向性治疗(ITT)患者来匹配年纪最大的健康对照供者。患者(N=52)和健康供者(N=22)所产生的分组在年龄、性别和巨细胞病毒(CMV)血清阳性方面是平衡的。
3.材料与方法
样品采集
所有样品是在任何治疗研究干预前,或接种前三天及紧接+CY组环磷酰胺注射前,或紧接接种前采集。血清样品是利用血清/凝胶真空采血管(贝克顿迪肯森公司,5ml)采集,倒置孵育最少30分钟。采血管在至少1200xg离心15分钟,血清(约2ml)被转移到NUNC低温管中(3.6ml)。低温管立即在≤-20℃下保存,直至测量。用于血液学参数分析的EDTA抗凝血被收集在3ml的EDTA真空采血管中,该管不进行离心,在室温下保存直至分析。尿液在接种前不久采集。对于试纸检验,只使用新鲜尿液。用于凝血分析的柠檬酸钠抗凝血浆在接种前约两周采集。使用柠檬酸钠真空采血管采集样品,并在室温下至少1200xg离心30分钟。上清血浆被转移到微管中。
测量
生物标记测量方法包括ELISA、多重免疫测定法、表观遗传学分析法、质谱法、FACS分析法、血液学常规临床方法、临床化学和尿液分析法以及其他方法。最终选定的参数是用RBM(ApoA1、CD95/FAS、IL-6、IgE、MMP-3、CA19-9)和密理博(CXCL13/BCA-1、CCL17/TARC、IL-33)提供的多重测定方法以及Biocrates(苏氨酸、短链酰基肉毒碱)提供的质谱法进行测量的。其他的参数由中心实验室提供,并用血液学(中性粒细胞、嗜酸性粒细胞、单核细胞)和临床化学(天冬氨酸转氨酶/血清谷草转氨酶、LDH、白蛋白、胆红素)常规方法进行测量。
T细胞反应评估
除了总存活期外,必须检测单一或多个T细胞反应的产生与生物标记的相关性,使用酶联免疫斑点法和肽-MHC复合物四聚体染色法,在接种后前后数次测量特异性T细胞反应。
单变量统计分析:
利用Welcht检验评估参数和T细胞反应的相关性。预测所有患者和/或+CY组T细胞反应、p<0.05的参数被认为具有统计学意义。
利用Cox比例风险模型评估参数与CY预先治疗的相互作用。在不是分布两端的任何截断值(即选择或取消选择>~5%的患者)处,显示相互作用p值<0.05的候选参数被认为是需要关注的。
此外,由于环磷酰胺预先治疗和不利生物标记组的总存活期存在负相关,排除了具有显著相互作用的参数,附加标准与阳性生物标记组的总存活期呈显著相关性(p<0.05)。与+CY组而非–CY组的总存活期有显著相关性(p<0.05)被视为参数预测而非预后质量的额外提示。
参数选择的其他标准是基于其生物学及其与环磷酰胺预先治疗相互作用和与患者如TKI预先治疗和细胞因子预先治疗患者亚组+CY组而非–CY组总存活期相关性模式的持久性。
计算参数之间的相关性。与剩余组中其他参数高度相关(校正的p值<0.005)的参数被排除。
在利用这些步骤确定单个参数后,检验这些参数组合是否以及哪些组合在预测上述患者亚组总存活期、T细胞反应和稳定性方面具有最佳效果。
多变量统计分析:
根据Cox比例风险模型确定多变量标记,其特性和延伸特性描述如下。
所有参数均为广义线性模型的协变量,所以都应考虑在内,同时,为避免用小单位表示参数的不良影响,将参数线性转换为零平均值和单位方差(“标准化”参数)。为限制该组的测量成本,只包括不需要进行PBMC分离的参数。因为相应的最大偏似然问题尚未确定,即参数数量超出了患者数量,所以用高斯先验模型来扩大模型,这限制了优化生物标记中每个参数的相对影响。已知相应的最大后验(MAP)优化也能提高最大似然法结果(“规则化”)的预测性能。模型用拉普拉斯先验模型进一步扩大,这导致优化只使用一小组考虑过的参数。通过这种方式,包含在生物标记中的参数数量限制在20个。
此外,期望多变量生物标记对-CY组没有预测性,或至少对那些患者的预测性比对+CY组的差。为达到这一点,对-CY组的重复预测模型进行了优化,从参数空间向外延伸产生的方向,直到基本上只存在非预测方向。然后,在+CY组参数的剩余子空间确定生物标记自身。为了评估生物标记的预测准确性,整个优化过程按留一法方式应用。所有患者的预测都是以这种方式计算、收集,然后与已知存活时间进行比较。
结果:单变量分析
参数选择
在检验的约450个参数中,59个显著预测所有患者和/或+CY组患者的T细胞反应的发生。118个参数显示在其优化截断值处与环磷酰胺预先治疗有显著相互作用。利用上述T细胞反应预测,与环磷酰胺预先治疗相互作用,与+CY组总存活期的相关性比–CY组好,与其他参数没有冗余,有合理生物原理的标准,四个参数(ApoA1、CXCL13/BCA-1、单核细胞和CCL17/TARC)可以被确定为优选的单变量生物标记候选者。
截断值定义
就上述确定的四个参数中的每一个而言,定义了离散截断值,把患者分成两组(生物标记阳性或阴性),它们显示有显著不同的IMA901治疗效果。需要选择截断值,因为参数尽管与总存活期和T细胞反应有连续而非离散相关性,但是应该用它们做出肯定/否定的治疗决定。出于这一原因,适当的截断值应a)包括足够的患者,b)是客观的。
对于ApoA1,超过50%的IMA901-202患者显示病理性水平降低,截断值被选为是在检测供应商(RBM,0.288mg/ml)指定的健康供者中确定的LLN,它确定ApoA1水平在正常范围内的患者为生物标记阳性组。该截断值接近IMA901-202研究患者的中值(0.264mg/ml)。因此,该截断值是最客观的,与ApoA1的生物学原理有关。或者,ApoA1也使用中值。其他参数大部分在IMA901-202患者的正常范围内。对于这些参数,研究人群的中值被选为截断值,因为这也是客观的并包括足够多(50%)的患者。
使用单个单变量生物标记预测总存活期和T细胞反应
Cox比例风险分析表明,由单变量分析选择的所有四个参数都能显著预测所有患者和环磷酰胺预先治疗患者亚组的总存活期,但是不能预测没有接受环磷酰胺预先治疗亚组的总存活期(表2,图1)。在生物标记阳性组中,根据相应生物标记截断值的定义,环磷酰胺预先治疗显示对总存活期有显著正向影响(p<0.05)(CCL17/TARC,单核细胞)或对总存活期有正向影响的趋势(ApoA1,CXCL13/BCA-1)。相互作用分析表明,环磷酰胺预先治疗效果在根据单核细胞水平划分为阳性或阴性的患者中显著不同(p<0.05)。CCL17/TARC有与环磷酰胺预先治疗相互作用的趋势。ApoA1和CXCL13/BCA-1在选定的截断值处无显著性差异。
特别是,在+CY组中,若患者根据其CCL17/TARC水平分组,可以达到最佳危害比(HR)(表2,HR=0.18,p=0.003)。反之亦然,CCL17/TARC阳性患者从环磷酰胺预先治疗中获得最大好处(表2,HR=0.22,p=0.017)。
在考虑到75%的CCL17/TARC阳性环磷酰胺预先治疗患者在研究结束时仍然活着的这一事实,CCL17/TARC也是特例(图1D)。
观察到的总存活期预测模式对TKI或细胞因子预先治疗的患者亚群仍然是稳定的(数据未显示)。
关于T细胞反应预测,CCL17/TARC和ApoA1表现最好,都能显著预测所有患者及+CY和–CY亚组的反应者,并能预测所有患者的多肽反应者。与之相比,CXCL13/BCA-1和单核细胞是较弱的反应预测指标(表3)。
5.结果:多变量分析
参数选择
计算总患者人群的20参数生物标记组,该标记组预测在用IMA901治疗时+CY组患者的总存活期比–CY组的好。包含在该组的参数见表4。它们包括细胞因子、趋化因子及血清中可测的其他蛋白、标准血液学分析法可测的细胞参数和质谱法可测的代谢组学参数。除所有患者的优化(训练版)外,生物标记组用留一法交叉验证来计算(检验版)。后者分析更相关,因为它允许在随访研究中结果有鲁棒性。这些实验中显示的数据代表试验结果。

表5显示了包含在20参数多变量生物标记中的参数,以及表明它们在该组中相对重要性的权重和排名。
每位患者的生物标记值是含有的20个参数测量数值的线性组合。该等式的常数因子是按生物标记分布中值接近零的方式选择的。这种方式计算的生物标记值在IMA901-202患者人群中接近正态分布,它们在+CY组和–CY组患者间均匀分布。
使用多变量参数组预测总存活期和T细胞反应
为了验证生物标记的预测能力,通过根据患者的生物标记值把他们分为两组来进行存活分析。整个患者人群的生物标记密度分布中值被选为截断值,包括数值在该截断值以上的生物标记阳性组的50%的患者。在+CY组患者中,生物标记阳性患者极显著地受益于接种,而生物标记在–CY组中显示没有效果。反之亦然,在生物标记阳性组中,环磷酰胺预先治疗导致明显更高的总存活期,而在生物标记阴性组中效果不显著或甚至(可能)相反(表4,图5)。相应地,多变量生物标记和环磷酰胺预先治疗之间有极显著相互作用。
尽管生物标记未经优化预测免疫反应,但发明者对生物标记值分布是否也与存在或不存在T细胞反应有关进行了检验。实际上,在反应者和多肽反应者组(特别是+CY组患者)中,存在生物标记值升高的趋势(图6)。
使用参数组合预测总存活期和T细胞反应
单个参数可以一种方式组合,就是根据组合中含有的所有参数值给患者分配一个生物标记得分。对于双参数组合,每个参数根据其截断值可以是阳性或阴性,对于组合标记,患者可以有两个(两个参数阳性)、一个(一个参数阳性)或零个(没有参数阳性)生物标记得分。
出于一些原因,几个参数组合来计算生物标记得分可能是有利的。首先,它导致更大的鲁棒性,因为a)尽管参数与结果有连续相关性,但固定截断值问题在某种程度上仍要克服。b)测量离群值的影响被降低。c)大量参数能够深入了解可能与抗癌免疫有关的更多生物过程。其次,评分模型能让患者选择有更多灵活性:然而,以中值作为客观截断值的单个参数可能必定只选择50%的患者进行治疗,双参数得分(单个参数的截断值在分布中值)把患者分成三组。若只排除没有阳性标记(即生物标记得分为0)的患者,约75%的患者可以包括在治疗组中。但是,如果该组没有显著性差异,可以排除得分为0和1的患者,在治疗组中留下显示生物标记得分为2的约25%的患者。
出于这些原因,对上面确定的单个参数组合是否在预测总存活期和T细胞反应方面有更好表现进行了检验。而且,还对上述患者亚群效果的稳定性进行了评估。在这些方面,几个标记组合胜过单个参数的用途,其中最好的是ApoA1和CCL17/TARC组合(图3,图4)。

Claims (17)
1.至少一个标记-特异性抗体在制备用于预测癌症患者中有益的免疫治疗效果的试剂盒中的用途,其中所述标记选自:载脂蛋白A1(ApoA1),
-其中测定所述标记在获得自所述癌症患者的样品中的水平,所述水平与该标记在特定癌症患者人群的中值相比增加或升高表明对上述癌症患者产生有益的免疫治疗效果,
-其中所述癌症患者正经历选自肾细胞癌(RCC)、结肠直肠癌(CRC)、胃癌(GC)、黑色素瘤、非小细胞肺癌(NSCLC)、胶质母细胞瘤、和任何类型的腺癌;
-其中所述样品选自全血、全血的部分、或肝素化全血,以及它们的冻存样品;
-其中所述免疫治疗包括用抗癌疫苗接种,其中所述接种是至少一种疫苗,所述至少一种疫苗包含选自SEQIDNO:1-37的肽的至少一种致免疫肽;以及
-其中所述有益的免疫治疗效果选自更长的总存活期、对免疫治疗产生单一和/或多个T细胞反应、抑制肿瘤生长、肿瘤萎缩、和更长的无进展存活期。
2.根据权利要求1的用途,其中所述测定还包括测定所述癌症患者样品中选自以下的至少一种标记的水平:B细胞吸引趋化因子(CXCL13/BCA-1)、中性粒细胞、白细胞介素6(IL-6)和短链酰基肉毒碱,其中所述标记的水平与该标记在特定癌症患者人群的中值相比减少或降低表明对上述患者产生有益的免疫治疗效果。
3.根据权利要求1或2的用途,其中使用所述至少一种疫苗与GM-CSF进行所述接种。
4.根据权利要求1的用途,其中所述测定还包括测定所述患者的所述样品中选自白蛋白和直接胆红素的至少一种标记的水平,其中所述标记的水平与该标记在特定癌症患者人群的中值相比增加或升高表明对上述患者产生有益的免疫治疗效果,
和,任选地,测定所述患者的所述样品中包括白细胞介素33(IL-33)的标记的水平,其中所述标记的水平与该标记在特定癌症患者人群的中值相比减少或降低表明对上述患者产生有益的免疫治疗效果。
5.根据权利要求2或4的用途,其中所述测定包括选自以下的至少一种方法:免疫法、比色法、荧光法、质谱法、血液学已有方法、和临床化学常规方法。
6.根据权利要求1的用途,其中上述患者接受手术、放疗和/或化疗疗法的治疗,或曾接受手术、放疗和/或化疗疗法的预先治疗。
7.根据权利要求6的用途,其中所述疗法是上述患者用选自细胞因子和酪氨酸激酶抑制剂(TKI)的抗癌药进行治疗或预先治疗。
8.根据权利要求7的用途,其中所述酪氨酸激酶抑制剂(TKI)选自索拉菲尼、舒尼替尼和环磷酰胺。
9.根据权利要求1的用途,还包括对上述患者上述免疫治疗效果的预后。
10.根据权利要求1的用途,还包括对上述患者上述癌症治疗效果的监测,包括重复所述测定步骤至少一次。
11.根据权利要求1的用途,其中所述至少一种致免疫肽选自SEQIDNO:1-10。
