Abstract

Description
发明背景
本公开涉及肿瘤学领域,更具体来说涉及癌症检测剂的放射性标记。
前哨淋巴结活检作为黑素瘤和乳癌诊断的常规程序正快速得到接受(Vera,D.R.等,(2001)J.Nucl.Med.42,951-959)。该技术还未标准化;它典型包括使用99mTc-胶体和蓝色染料。在胶体成像剂和本发明中使用的放射性同位素99m锝,具有几种理想的性质:它容易获得、相对低价、成像质量出色并具有6小时的短半衰期。该放射性示踪剂用于在手术前确定前哨淋巴结的位置,然后在手术进行中用于找准前哨淋巴结的解剖位置。通过淋巴管和淋巴结快速清除的蓝色染料,被用于目测证实所选的放射活性淋巴结是前哨淋巴结。因为这种活检程序随着各个从业人员而变,难以用一致的技术规定来训练从业人员,因此这些活检产生了大范围的假阴性报告率(即0至12%,参见Vera,D.R.,同上)。

因此,开发被设计用于满足最优前哨淋巴结检测目标(即快速的注射位点清除和低的远距离淋巴结积累)的核成像诊断试剂盒,在乳癌和黑素瘤治疗中是尚未满足的医疗需要。
发明简述
本公开提供了含有与双功能螯合剂例如DTPA偶联的葡聚糖的组合物,其易于用作包含单个冷冻干燥管和液体稀释剂管、在放射性标记后具有高放射化学纯度的“即用型”试剂盒。本公开还提供了长期储存稳定性以及足够的重构稳定性,以便于其药物或临床应用,易于作为诊断成像药剂操作和给药。
在添加99mTc-高锝酸钠后,本公开显示出双功能配体DTPA的高放射化学纯度(即>90%的99mTc-DTPA-葡聚糖纯度),所述DTPA通过与其5个羧基臂之一的酰胺键偶联到葡聚糖分子上的多个氨基封端的束带上。尽管游离的DTPA无疑使其所有5个去质子化羧基与重金属离子例如111铟结合,但作为潜在的八齿配体(还含有三个氮原子,参见H.R.Maecke等,(1989)J.Nucl.Med.30,1235-1239),七齿DTPA以降低的热力学稳定性结合,这使它对结合99Tc离子的竞争更敏感,可能引起放射化学纯度降低。
99mTc-DTPA-葡聚糖的高放射化学纯度,通过将pH降低到约2至4之间、筛选非竞争性组分并鉴定理想的转移螯合剂甘氨酸(其也用作pH缓冲剂),以及利用下述事实,得以实现:(1)用于99mTc的竞争性配体的分布由结合速率常数决定,并且(2)99mTc从其DTPA-葡聚糖复合物上的解离速率常数非常低并且依赖于pH。因此,通过在高酸性条件下暂时与甘氨酸结合,在通过其稀释剂将该“即用型”试剂盒的pH移向略酸性条件后,甘氨酸将放射性同位素转移到与其结合亲和性更强并持留锝-99m(由于其慢的解离速率常数)的DTPA-葡聚糖,增加了对DTPA-葡聚糖进行放射性标记的高效率。
本公开还提供了磷酸盐缓冲盐水稀释剂,其通过将pH从在注射时引起疼痛的强酸性条件(即约3至4之间的pH)移向更容易忍受的适度酸性条件(即pH>~5),能够使患者舒适(M.Stranz和E.S.Kastango(2002)Int.J.Pharm.Compound.6(3),216-220)。
本公开还提供了还原剂例如L-抗坏血酸,其使含有过量亚锡离子或锡离子的放射性标记的DTPA-葡聚糖制剂进一步稳定化,防止形成Sn-胶体或其他放射化学杂质例如Sn4+。本公开还通过在制剂中包含L-抗坏血酸防止了药物物质及其组成部分的氧化性降解和防止了放射性标记的药物产物的辐射自分解。
此外,本公开还为DTPA-葡聚糖在无定形二糖冷冻干燥饼中提供了稳定和美观的环境,允许用99mTc-高锝酸钠快速重构并添加缓冲盐水稀释剂以产生易于使用的、透明的、无颗粒的液体。本公开还通过用药品级氮气回填冷冻干燥管提供了含有惰性气体的顶部空间,进一步稳定亚锡离子,为还原99mTc-高锝酸钠(或99mTcO4 -)提供超过本发明的储存寿命的超额能力。
因此,本发明是用于产生高放射化学纯度99mTc(III)(以及可能的99mTc(IV))DTPA-葡聚糖复合物的改进的方法,所述方法使用单一的冷冻干燥管,将其用pH缓冲的稀释剂进一步重构以改变最终溶液pH,产生了稳定至少6小时并有助于患者舒适的溶液(Russell,CD.(1980)J.Nucl.Med.21,354-360;Russell,CD.和Speiser,A.G.(1982)Int.J.Appl.Radiat.Isot.33,903-906)。用于DTPA-葡聚糖的冷冻干燥冷冻试剂盒的制剂是“即用型”试剂盒,在氮气环境下使还原固体白色冷冻干燥饼中的99mTc-高锝酸钠所需的氯化亚锡稳定,其具有长期储存稳定性。该试剂盒通过在高酸性条件下用Sn2+还原99mTc-高锝酸盐,产生了高放射化学纯度,同时在用磷酸盐缓冲盐水溶液稀释以将重构溶液的pH移向中性后,使99m锝-DTPA-葡聚糖复合物维持在高于90%的放射化学得率。

详细描述
开发用于前哨淋巴结诊断的商业化“即用型”试剂盒的关键是具有最佳前哨淋巴结检测所需性质的成像剂的合理设计。这些性质是小的分子直径和高的受体亲和性,产生具有快的注射位点清除速率和低的远端淋巴结积累的放射药剂(Vera,D.R.,同上)。在本发明中,所使用的药物利用葡聚糖平台来递送放射性标记物。葡聚糖骨架是药品级、平均分子量约为9,500的聚合物,其非常亲水、缺少电荷并且是柔性的。所有这些物理性质减少了跨膜壁迁移,其促进快速注射位点清除。葡聚糖聚合物偶联到与DTPA基团结合的胺封端的束带上,为分子提供了高受体亲和性以络合99m锝。99mTc-DTPA-甘露糖基-葡聚糖的高信号强度,由于较高的信号背景比,能够更好地检测前哨淋巴结。

为了将这种重要的核成像剂商业化,已经开发了Lymphoseek配体药物产品的组合物(制剂)以及制造这种组合物的方法,这是本公开的主题内容。99m锝-标记的核成像“即用型”试剂盒的开发,是高放射化学效率与形成非特异性99mTc-标记的材料(即99mTc-胶体或99mTc-标记的制剂赋形剂)之间的精巧平衡。此外,需要防止还原的99m锝重新氧化成99mTcO4 -。因此,开发了冷冻干燥制剂,以使亚锡离子在惰性氮气环境下稳定。

这些专利描述了本技术领域已知的转移螯合剂的范围,例如柠檬酸盐、酒石酸盐、磷酸盐、膦酸盐、萄庚糖酸盐以及甚至抗坏血酸。但是,这些转移螯合剂主要用于弱酸性至中性pH制剂中,并能够干扰活性成分以高效进行放射性标记。使用抗坏血酸作为转移螯合剂的最适pH为pH 4.5至6.2(Liang等(1987)Nucl.Med.Biol.14,555-562)。这是由于其羧基的pKa为pH 4.10(CRC:《化学和物理学手册》第75版(Handbook of Chemistry and Physics,75th Edition)David R.Lide,Ph.D.(CRC Press,London))。甘氨酸羧基的pKa为2.34。它的羧基在高酸性条件下仍具有功能(例如在pH 2下部分去质子化,在pH 4下完全去质子化)。在本公开的优选实施方案中,抗坏血酸完全质子化。因此,组合物降低了抗坏血酸的潜在干扰,利用了这种抗氧化剂的有益性质,同时将甘氨酸用作最适转移螯合剂。
当共价连接的DTPA与还原的99m锝结合时,在酸性条件下可能的主要氧化态是99mTc(III),这将产生带有零净电荷的稳定复合物(Russell,CD.(1980)J.Nucl.Med.21,354-360)。液体组合物在pH 2下成功放射性标记DTPA-葡聚糖,在添加稀释剂后移向较高pH。但是,在低于或等于2.7的pH下,DTPA基团完全质子化,可能导致冷冻干燥的制剂在pH 2下完全瓦解(Hnatowich,D.J.等,(1995)J.Nucl.Med.36,2306-2314)。因此,优选实施方案是使组合物在pH从约3至约4的范围内能够获得高的放射化学效能,同时在将重构的“即用型”试剂盒用磷酸盐缓冲盐水稀释剂稀释后使pH移到高于约pH 5,其能够被患者良好耐受。在这种应用中,所有成分都该是USP级的(美国药典(UnitedStates Pharmacopeia))。此外,“q.s.”具有其标准的制药意义,即“足量”。
具体实施方式
实施例1
99m
Tc-标记的DTPA-甘露糖基-葡聚糖和DTPA的洗脱图
图1显示了使用孔径排阻色谱(SEC),通过放射活性(NaI,设置在1000cps/伏特)检测器测量的重构的99m锝标记的Lymphoseek配体药物产品(99mTc-DTPA-甘露糖基-葡聚糖)批号NMK001的典型洗脱图。该SEC放射化学纯度方法的条件如下:使用TSKgel柱,TosohBioscience,G3000PWXL(7.8×30cm,6μm,柱温为25±5℃),使用50mM pH 7.2的磷酸盐缓冲液和300mM氯化钠的等度流动相。将冷冻干燥管用0.8cc 10毫居里的99mTc-高锝酸盐重构,混合,并允许在环境室温下放射性标记至少10分钟,然后在0.2cc磷酸盐缓冲盐水中将样品进行部分中和。注入15μL冷冻的药物样品并以0.6mL/分钟的速率运行,运行时间为40分钟;99mTc-DTPA-甘露糖基-葡聚糖(99mTc-DMD)峰的保留时间是约12至12.5分钟,在9至15分钟之间伸展有99mTc标记的赋形剂的拖尾的肩,放射活性峰在约15至15.5分钟处洗脱。
洗脱图与使用相同柱子和流动相、利用折光率检测器(因为没有UV/VIS吸收值)的效价方法所获得的非常类似。99mTc-DTPA-甘露糖基-葡聚糖的宽洗脱峰是葡聚糖聚合物不均质性的结果,这被甘露糖基和DTPA基团与葡聚糖上氨基封端的束带的偶联的不均质性进一步加剧(Vera,D.R.等,(2001)J.Nucl.Med.42,951-959)。DTPA-甘露糖基-葡聚糖制剂的目的是在大批液体药物制剂中获得高于95%的放射化学纯度,并在重构的冷冻干燥药物产物中获得超过90%的放射化学纯度。
图2显示了使用如上所述的SEC放射化学纯度方法,通过放射活性(NaI)检测器测量的使用冷冻干燥的Lymphoseek配体药物产品安慰剂(即4.5mM L-甘氨酸,pH 3,2.5mM L(+)-抗坏血酸钠,2%(w/v)α,α-海藻糖和75μg/mL氯化亚锡二水合物),用10毫居里99mTc-高锝酸盐放射性标记的99m锝标记的DTPA标准品的典型洗脱图。99mTc-DTPA峰的保留时间是约15分钟,在14到16分钟之间洗脱,其是几乎所有99mTc-标记的低分子量赋形剂的近似保留时间(数据未显示)。
实施例2
初始试验性制剂:调查干扰性赋形剂
在图3中,最顶部的堆积放射化学洗脱图显示了用10毫居里99mTc-高锝酸盐重构并通过SEC放射化学纯度方法运行的初始冷冻干燥制剂试验品(5μM(0.1mg/mL)DTPA-甘露糖基-葡聚糖,20mM柠檬酸钠,pH 5.6,5.7mM L-半胱氨酸钠,2%(w/v)D-甘露糖醇和75μg/mL氯化亚锡二水合物)。(就在SEC放射化学纯度方法的开发之前制备的初始冷冻干燥药物产品制剂试验品)。该洗脱图清楚地显示,99mTc-DMD峰具有低于约25%的放射化学纯度。
使用了下面的筛选方法(按照添加的次序)来确定试验性制剂中的潜在干扰性赋形剂:(1)对于药物安慰剂制剂来说,向1.5mL带盖塑料试管加入50μL脱气的盐水;对于药物制剂来说,加入50μL在脱气盐水中的1.2mg/mL DTPA-甘露糖基-葡聚糖至终浓度为0.3mg/mLDMD;(2)为了测试不同赋形剂,加入50μL四倍浓缩的脱气的溶液;(3)为了还原99mTc-高锝酸盐,加入50μL在0.01N脱气盐酸中的300μg/mL氯化亚锡二水合物;并紧接在加入SnCL2后,(4)为了用还原的99m锝对制剂进行放射性标记,加入50μL 50毫居里的99mTc-高锝酸盐,至终浓度为12.5mCi的99mTc-高锝酸盐。(注:溶液通过鼓入氮气至少一小时进行脱气。)。然后混合并在环境温度下放置至少10分钟,然后转移到带盖的HPLC自动进样器管中以进行SEC放射化学纯度测定。
图3显示,初始试验性制剂的三种赋形剂显示出显著的99mTc-标记的峰。从最顶部的堆积放射化学洗脱图开始以向下的方式继续,第二张洗脱图显示出在RT~14.5min处的显著的99mTc-柠檬酸的峰,其可以解释在最顶部图中RT~14.5min处的显著量的99mTc-标记干扰。柠檬酸盐在pH5到6的范围内是已知的DTPA的转移螯合剂(Hnatowich,D.J.,第8章,第175页,《使用放射性标记抗体的癌症成像》(Cancer Imagingwith Radiolabeled Antibodies)(Goldenberg,D.M.主编,1990:KluwerAcademic Publishers,Boston/Dordrecht/London)),但是它似乎太强而不能用于当前制剂中。在第三张放射化学洗脱图中,似乎D-甘露糖醇竞争99mTc,在RT~16min处洗脱。这种意料之外的干扰可能是由于该天然产物中存在的杂质。在第四和第五张放射化学洗脱图中,使用了两种不同的L-半胱氨酸浓度:L-半胱氨酸终浓度分别为0.25和1.0mg/mL。尽管第四张洗脱图显示出一些干扰结合,但在1.0mg/mL L-半胱氨酸下的第五张洗脱图清楚地显示出半胱氨酸结合99mTc并干扰柠檬酸盐的转移螯合,在21至23分钟范围内的保留时间处洗脱。第六张放射化学洗脱图涉及向柠檬酸盐制剂添加1mg/mL L(+)-抗坏血酸钠二水合物;抗坏血酸似乎不干扰99mTc-柠檬酸盐。
图4显示了初始药物产品制剂与液体药物制剂试验品的比较。最顶端的堆积放射化学洗脱图是初始药物产品制剂,第二张图是在盐水中的柠檬酸钠中添加SnCL2以还原12.5mCi 99mTc-高锝酸盐的洗脱图。在第三和第四张放射化学洗脱图中,将DTPA-甘露糖基-葡聚糖药物进行部分放射性标记,在约14.5分钟处有显著的99mTc-柠檬酸洗脱。因此,使用柠檬酸钠不是适合的pH缓冲剂/转移螯合剂选择。
实施例3
筛选pH缓冲剂、转移螯合剂和增量赋形剂以增强DTPA-甘露糖基
糖-葡聚糖的放射性标记
图5是通过SEC放射化学纯度方法测量的含有磷酸钠pH缓冲剂以及转移螯合剂、还原剂和增量剂的不同组合的液体DTPA-甘露糖基-葡聚糖药物安慰剂制剂试验品的堆积SEC放射化学洗脱图。最顶端堆积放射化学洗脱图显示出使用pH 4的20mM磷酸钠缓冲液和1.5mg/mL抗坏血酸钠时小的99mTc-标记的干扰峰。第二到第六张洗脱图显示了20mMpH 4的磷酸钠、75μg/mL SnCL2·2H2O和12.5mCi 99mTc-高锝酸盐,并且含有下列相应的潜在赋形剂:1mg/mL柠檬酸钠;1%PEG 8000;1mg/mL柠檬酸钠和1.5mg/mL抗坏血酸钠;1.5mg/mL抗坏血酸钠和1%PEG 8000;以及1.5mg/mL抗坏血酸钠、1mg/mL柠檬酸钠和1%PEG8000。它们都显示出显著的99mTc-标记的干扰峰,对于99mTc-PEG 8000来说洗脱早在RT~14分钟处,对于99mTc-柠檬酸来说洗脱在~15分钟的更低分子量保留时间处。
在图6中,可以看到含有磷酸钠pH缓冲剂的相应液体DTPA-甘露糖基-葡聚糖药物安慰剂制剂试验品的堆积SEC放射化学洗脱图。对于图6中含有0.3mg/mL或15μM DMD的DTPA-甘露糖基-葡聚糖制剂来说,堆积放射化学洗脱图显示出药物的不太显著的放射性标记。从顶上起第三张图显示了99mTc-DMD的背景水平,表明磷酸盐和PEG 8000不能用作满意的转移螯合剂。在其五分之一的pH缓冲强度下,柠檬酸在药物的放射性标记中不太有效,并且仍干扰这些制剂。此外,PEG 8000明显通过其羟基干扰药物产率,并且不适合作为增量剂。因为对于冷冻干燥来说磷酸钠不是理想的pH缓冲剂,因此筛选了可选的公认安全(GRAS)的pH缓冲剂。
在图7A和7B中,散布有含20mM pH 4的乙酸钠缓冲液的液体DTPA-甘露糖基-葡聚糖药物和安慰剂制剂试验品的堆积SEC放射化学洗脱图。在图7A中,最顶端的放射化学洗脱图是含有1.5mg/mL潜在的转移螯合剂酒石酸钠的药物制剂,显示药物的增强的放射性标记并伴有明显的干扰峰99mTc-酒石酸(对于相应的安慰剂制剂参见第四张洗脱图)。图7A中的第二和第三张洗脱图显示在存在抗坏血酸钠和PEG 8000的情况下几乎没有差别。在图7B中,放射化学洗脱图证实了抗坏血酸钠和PEG 8000与酒石酸盐组合的药物制剂,在对药物进行放射性标记中具有较低效率。最后,含有乙酸钠和抗坏血酸钠的DTPA标准品在pH4下具有小的边缘拖尾的肩。因此,选择酒石酸钠作为乙酸盐pH缓冲剂中的潜在转移螯合剂是不令人满意的。
在图8中,还散布有含有20mM pH 4和6的乙酸钠缓冲液的液体DTPA-甘露糖基-葡聚糖药物和安慰剂制剂试验品的堆积SEC放射化学洗脱图。最顶端和第二张放射化学洗脱图显示在pH 6下,1.5mg/mL抗坏血酸钠的存在增加了药物的放射化学纯度,但是第三和第四张图表明抗坏血酸盐可能造成了分别在RT~13.5分钟和~15分钟处的明显和较小的99mTc-抗坏血酸盐干扰峰。第五张洗脱图证实了放射化学纯度是pH依赖性的,在抗坏血酸钠存在下主要在pH 4下对药物进行放射性标记。第五张图可能包含一些与99mTc-DMD峰共洗脱的干扰性材料,正如在药物峰的拖尾边的轻微肩型突出部分中以及RT~16分钟处的小的99mTc-标记的峰中所观察到的。
在图9A和9B中,使用带有伯胺的还原糖和两性离子氨基酸即氨基葡萄糖钠和甘氨酸进行筛选研究,所述研究是关于根据经验进行的猜测,即这些赋形剂将具有一定的与99m锝的短暂相互作用,因为这种放射性同位素与胺和酰胺的氮、羧酸的氧以及硫醇盐和硫醚的硫、优选为硫醇盐的硫形成稳定复合物(Giblin,M.F.等,(1998)PNAS USA95,12814-12818)。图9A显示了液体DTPA-甘露糖基-葡聚糖药物安慰剂制剂试验品,其含有20mM pH 4的乙酸钠缓冲液以及1.5mg/mL氨基葡萄糖钠或甘氨酸,并且含有或不含抗坏血酸钠(1.5mg/mL)。这些赋形剂显示出背景水平的放射活性,在RT~15分钟处具有小的99mTc-标记的峰,例外的是仅含有氨基葡萄糖钠的第三张洗脱图,其具有高于背景的放射活性。在图9B中,含有20mM pH 4的乙酸钠缓冲液以及1.5mg/mL氨基葡萄糖钠或甘氨酸的液体药物制剂试验品的堆积放射化学洗脱图几乎一致,证实了作为转移螯合剂有效地放射性标记99mTc-DMD峰(在RT 12.4min处)的能力。因为氨基葡萄糖钠不是GRAS赋形剂,它在随后的制剂中没有继续使用。甘氨酸被鉴定为潜在的、非干扰性转移螯合剂。
因为甘氨酸和抗坏血酸钠显示出与药物增加的放射化学纯度相容,因此调查了这些赋形剂的范围。在两种终浓度下对甘氨酸和抗坏血酸钠进行了评估:对于Gly1和Gly2来说,终浓度分别为0.5和2.0mg/mL;对于AA1和AA2来说,终浓度分别为1.5和0.38mg/mL。对于甘氨酸#1和2药物制剂(即15μM DTPA-甘露糖基-葡聚糖,20mM乙酸钠,pH 4,75μg/mL SnCL2·2H2O和12.5mCi 99mTc-高锝酸盐)来说,当通过SEC放射化学纯度方法测量时,对于Gly1和Gly2进行的两个放射性标记研究的平均值分别为90.7%和88.7%99mTc-DMD纯度。在Gly1存在下,对AA1和AA2药物制剂进行的两个放射性标记研究的平均值分别为80.3和90.3%99mTc-DMD纯度(参见图10A和图10B)。
在含有甘氨酸作为转移螯合剂和抗坏血酸钠作为抗氧化剂/还原剂的20mM pH 4至5的乙酸钠制剂中,对适合用于冷冻干燥的增量剂进行筛选。确定了聚合赋形剂例如PEG 2000和聚乙烯吡咯烷酮干扰放射性标记DTPA-甘露糖基-葡聚糖的效率(数据未显示)。最后,鉴定到α,α-海藻糖(2%w/v)作为用于液体药物制剂的潜在的非干扰性增量剂。在图11中,具有20mM pH范围为5至4的乙酸钠缓冲液以及甘氨酸、抗坏血酸钠的液体药物制剂试验品的堆积SEC放射化学洗脱图,显示出2%α,α-海藻糖对99mTc-DMD的放射化学纯度具有如果有的话也是很少的干扰。此外,在乙酸钠制剂(15μM DTPA-露糖基-葡聚糖,20mM乙酸钠,1mg/mL甘氨酸,1mg/mL抗坏血酸钠,2%(w/v)α,α-海藻糖,38.5mM氯化钠,75μg/mL SnCL2·2H2O和12.5mCi 99mTc-高锝酸盐)中存在明显的pH敏感性,对于pH 5.0、4.5和4.0来说分别提供了86.8%、88.4%和93.8%99mTc-DMD纯度。因此,完成了用于鉴定可潜在应用于冷冻干燥药物产品试剂盒制剂中的适合赋形剂的筛选过程。下一步是优化制剂,证实冷冻干燥试剂盒格式的可行性并开发重构程序。
实施例4
用于增强冷冻干燥的DTPA-甘露糖基-葡聚糖药物产品的放射性
标记的制剂
需要调查在降低的pH下提供99mTc-DMD的增加的放射化学纯度的乙酸盐缓冲制剂的表观pH敏感性。初始pH研究使用pH 2和3的10mM磷酸钠和pH 4的20mM乙酸钠。图12显示了添加有12.5mCi 99mTc-高锝酸盐的DMD药物制剂(含有25μM DTPA-甘露糖基-葡聚糖(0.5mg/mL)、pH缓冲剂、0.5mg/mL甘氨酸、0.5mg/mL抗坏血酸钠、2%(w/v)α,α-海藻糖、38.5mm氯化钠和75μg/mL SnCL2·2H2O)的堆积SEC放射化学洗脱图。最顶端的洗脱图显示了pH 4的乙酸盐制剂,其通过SEC放射化学纯度方法测量具有96.9%的99mTc-DMD纯度。该制剂满足我们用于液体药物制剂的目标(即高于95%99mTc-DMD纯度)。不幸的是,pH3和2的10mM磷酸钠制剂在RT~14.0分钟处具有显著的99mTc-标记的干扰峰(参见图12)。因此,确定了甘氨酸/盐酸应该用作适合的酸性pH缓冲剂以及潜在的非干扰性转移螯合剂。图13显示了DMD药物制剂在pH 3、2和4下的堆积SEC放射化学洗脱图,所述DMD药物制剂含有下列赋形剂:25μM DTPA-甘露糖基-葡聚糖(0.5mg/mL)、0.5mg/mL甘氨酸、0.5mg/mL抗坏血酸钠、2%(w/v)α,α-海藻糖和75μg/mL SnCL2·2H2O(在pH 4下还含有10mM乙酸钠)。幸运的是使用了甘氨酸盐酸作为酸性pH缓冲剂和转移螯合剂,因为pH 3和pH 2的药物制剂分别显示出97.6%和97.1%的99mTc-DMD纯度,并满足药物制剂的所需目标(参见图13)。在图13中,pH 4的乙酸盐制剂不能满足药物制剂的目标(93.6%对>95%的99mTc-DMD纯度),但这可能是由于制剂制备中不同天的变差性、溶液的脱气不完全、氯化亚锡二水合物的混合不充分等。除了乙酸盐缓冲剂之外,在pH 4制剂中可以利用甘氨酸盐酸缓冲液。
将1.05mL该pH研究的除菌过滤的等份试样装入3mL I类玻璃管中。将塞子置于这些管的颈部,并将管置于VirTis冷冻干燥机架子上进行冷冻干燥。在冷冻干燥循环完成后,用氮气回填管并用塞子塞住。随后,将塞住的管子用铝密封物束缚。在目测检查时,对于pH4的乙酸盐和pH3的甘氨酸药物产品制剂管来说,冷冻干燥饼保持其无定形结构,并似乎已干燥至低残留水分含量。相反,pH 2的甘氨酸药物产品制剂管完全塌陷(即不具有结构)。本发明的优选实施方案是pH 3的药物产品制剂(即12.5至25μM DTPA-甘露糖基-葡聚糖(0.25至0.5mg/mL),0.5mg/mL甘氨酸,pH 3,0.5mg/mL抗坏血酸钠,2%(w/v)α,α-海藻糖和75μg/mL SnCL2·2H2O)。
实施例5
开发重构程序,包括使用磷酸盐缓冲盐水稀释剂来改进对冷冻干
燥DTPA-甘露糖基-葡聚糖药物产品进行放射性标记的易用性
由于Lymphoseek配体药物产品制剂的最终pH约为pH 3,低于肠胃外药物的推荐pH(Stranz,M.和Kastango,E.S.(2002)Int.J.Pharm.Compound.6(3),216-220),因此决定使用在用99mTc-高锝酸盐重构后将pH中和到较少疼痛和无害pH(例如pH 5至9之间)的稀释剂。使用0.9%的氯化钠或等渗盐水将99mTc-高锝酸钠从钼-99发生器洗脱下来。将Lymphoseek配体药物产品在用99mTc-高锝酸盐重构后配制成满足输液护理学会(Infusion Nursing Society)推荐的低于500mOsm/L的量。鉴定了适合用于人类肠胃外给药的稀释剂,即来自Greer Laboratories的注射用缓冲盐水。该稀释剂的配方是:0.107%七水磷酸钠,0.036%磷酸钾(理想情况下为USP-NF,美国药典-国家配方手册(United StatesPharmacopeia-National Formulary)),0.5%氯化钠和0.4%苯酚。推荐将冷冻干燥的Lymphoseek配体药物产品管用0.7cc 10至50mCi的99mTc-高锝酸钠在环境室温下重构至少10分钟,间歇混合,然后用0.3cc注射用缓冲盐水稀释。Lymphoseek配体药物产品具有至少12小时的重构稳定性,但是推荐将重构的药物产品在6小时内给药(数据未显示)。因此,中和后的99mTc-标记的Lymphoseek配体药物产品将在真皮内注射后被患者良好耐受。

Claims (18)
1.将二亚乙基三胺五乙酸(DTPA)-葡聚糖用锝-99m进行放射性标记的组合物,其包含:
(a)浓度高达0.50mg/管的DTPA-葡聚糖;
(b)浓度高达2%(w/v)的选自非还原二糖的糖;
(c)浓度为约0.5mg/管的非巯基抗氧化剂;
(d)亚锡盐,其中亚锡盐的二水合物形式的浓度高达75微克/管;
(e)浓度范围高达约0.5mg/管的选自一组pH缓冲剂的pH缓冲剂;以及
(f)注射用水(WFI),其中水用惰性气体脱气和除气。

14.权利要求10的方法,其中亚锡盐是氯化亚锡二水合物。
15.权利要求10的方法,其中DTPA-葡聚糖含有多个以约2∶1至12∶1的摩尔比范围与葡聚糖偶联的DTPA基团。
16.权利要求10的方法,其中DTPA-葡聚糖所含葡聚糖的平均分子量范围为约5,000至20,000道尔顿。
17.权利要求10的方法,其中DTPA-甘露糖基-葡聚糖所含的偶联的甘露糖基与DTPA-葡聚糖的摩尔比范围为约2∶1至12∶1。
18.使用99mTc-高锝酸钠溶液和稀释剂对DTPA-葡聚糖冷冻试剂盒进行放射性标记以用作诊断放射药物和用于调整最终溶液pH以使患者舒适的方法,所述方法包含下列步骤:
(a)加入水性高锝酸钠(Tc99m)组合物,其包含:
(i)体积为0.7mL的超过约100居里的高锝酸钠/mmol DTPA-葡聚糖;
(ii)在1.0mL终体积中的0.2mL剂量中,允许Tc99m剂量范围从约0.3至5.0微居里Tc99m;
(b)加入体积为0.3ml的水性缓冲盐水稀释剂组合物,其包含:
(i)0.5%(w/v)氯化钠,USP;
(ii)0.107%七水磷酸钠;
(iii)0.036%磷酸钾;
(iv)0.4%苯酚;
(v)加入注射用水(wfi)补足到终体积;以及
(vi)根据需要加入浓氢氧化钠或盐酸,将pH调整到约pH7.0。
