도네페질 또는 이의 약학적으로 허용가능한 염, 파모산, 및 생체적합성 고분자를 포함하는 서방성 미립구, 이러한 서방성 미립구를 포함하는 알츠하이머병 또는 인지기능 장애 예방, 개선 또는 치료용 약학적 조성물 및 상기 서방성 미립구의 제조방법이 제공된다.
Description
본 발명은 도네페질 및 파모산을 포함하는 장기지속형 미립구 주사제 및 그 제조방법에 관한 것이다.
아세틸콜린은 콜린성 뉴런에서 콜린과 아세틸-CoA로부터 합성되는 신경전달물질이다. 이러한 아세틸콜린의 농도가 저하되면 인지 및 지각능력에 손실이 올 수 있으며, 이는 알츠하이머와 밀접한 관련이 있다.
이러한 아세틸콜린의 농도 저하현상을 억제하기 위한 치매치료제로서 도네페질(Donepezil)이 널리 사용된다. 도네페질은 콜린에스터라아제 억제제로서 알츠하이머병으로 인한 치매환자에게서 나타나는 아세틸콜린 저하현상을 억제하여 아세틸콜린의 농도를 정상범위로 올려주는 역할을 한다.
현재 도네페질은 대부분 정제 형태로 1일 1회 경구 투여되고 있다. 하지만 한 달 이상 꾸준히 복용하여야 임상적인 약효를 나타낼 수 있으며, 알츠하이머병이 주로 노년층에서 걸리는 질병이라는 점에서 경구제제로의 복용은 한계가 있다. 따라서 항치매약물의 서방성 제제의 요구는 지속적으로 증가하고 있다.
종래 서방성 제제로 개발하기 위해 생체적합성 고분자를 이용한 미립구 제제는 활발한 연구관심분야와 임상응용분야로 발전하였다. 그런데, 이와 같이 장기간 지속적인 투여를 위해 미립구 제제를 제조하기 위해서는 미립구 내에 포함되어야 하는 약물이 투여 기간 및 투여량을 고려시 상당히 높은 양으로 포함시켜야 한다. 예를 들어 도네페질의 경구투여제제인 아리셉트®정의 경우 일일 투여용량이 각각 5mg, 10mg, 23mg이다. 이를 1개월 지속형 서방형 치료제로 적용할 경우, 1회 약물 투여량이 140mg 내지 280mg 혹은 그 이상이 필요할 수도 있다. 한편 서방형 미립구 주사제로 개발할 경우 약물과 고분자를 포함하는 미립구의 인체 투여량은 1 g 이상일 경우 1회에 투여하기 어렵다. 따라서 미립구는 1 g 이하 또는 더 적은 양으로 투약할 시에 투여가 용이하다. 투여되는 미립구 양을 줄이기 위해서는 미립구 내에 약물의 함량을 높이는 것이 바람직하다. 이와 같이 미립구 중에 약물을 높은 함량으로 포함시키는 경우, 분해속도가 충분히 긴 생체적합성 고분자를 적용시킨다 하여도, 약물의 고함량으로 인해 방출기간이 충분히 길어지지 않아 원하는 투여 기간을 맞출 수 없거나, 혹은 초기에 지나치게 빠른 약물방출이 일어나는 ‘initial burst’문제점을 일으킬 수 있고, 이 경우 급격한 약물의 혈중 농도 상승으로 인한 심각한 부작용이 발생할 수도 있다. 한편, 이러한 문제점을 해소하기 위해서 미립구 중에 포함되는 약물의 함량을 낮추는 경우에는 치료를 위한 약물의 함량이 충분치 않기 때문에 상대적으로 1회 투여되는 미립구의 양이 과도하게 증가되게 되어 환자의 복약순응도(compliance) 및 불편함을 초래할 수 있다는 문제점이 있다.
따라서 고함량의 도네페질을 봉입한 장기간동안 안정한 약물 방출의 특성을 갖는 도네페질 서방성 미립구 주사제의 개발이 요구되고 있다.
본 발명은 상기와 같은 종래의 문제점을 해결하기 위해 고안된 것으로, 미립구 내에 고함량의 도네페질을 봉입함에도 불구하고 장기간동안 안정한 약물 방출의 특성을 제공하는 도네페질 서방성 미립구 제제와 이를 제조하는 방법을 제공하는 것을 목적으로 한다.
본 발명의 일 측면으로, 유효성분으로서 도네페질 또는 이의 약학적으로 허용가능한 염; 파모산 및 생체적합성 고분자를 이용하여 제조되는 서방성 미립구로서,

상기 미립구는 도네페질로서 7일간 누적 방출량이 30% 미만인 특성을 갖는 것인 서방성 미립구에 관한 것이다.
또 하나의 양태로서, 본 발명은,
유효성분으로서 도네페질 또는 이의 약학적으로 허용가능한 염; 파모산 및 생체적합성 고분자을 포함하는 서방성 미립구로서,
상기 도네페질:파모산의 몰비가, 도네페질 자유염기 형태 기준으로 1 : 0.05 초과 1 : 0.25 이하일 수 있다.
상기 미립구는 도네페질로서 7일간 누적 방출량이 30% 미만인 특성을 갖는 것인 서방성 미립구에 관한 것이다.
종래 서방성 제제로 개발하기 위해 생체적합성 고분자를 이용한 미립구 제제는 활발한 연구관심분야와 임상응용분야로 발전하였다. 그런데, 이와 같이 장기간 지속적인 투여를 위해 미립구 제제를 제조하기 위해서는 미립구 내에 포함되어야 하는 약물이 투여 기간 및 투여량을 고려시 상당히 높은 양으로 포함되어야 한다. 이와 같이 미립구 중에 약물을 높은 함량으로 포함시키는 경우, 분해속도가 충분히 긴 생체적합성 고분자를 적용한다 하여도, 약물의 고함량으로 인해 방출기간이 충분히 길어지지 않아 원하는 투여 기간을 맞출 수 없거나, 혹은 방출초기에 지나치게 빠른 약물방출이 일어나는 ‘initial burst’문제점이 일어날 수 있고, 이 경우 급격한 약물의 혈중 농도 상승으로 인한 다양한 부작용이 발생할 수 있으며, 특히 치료 인덱스(therapeutic index)가 낮은 약물의 경우 심각한 문제가 발생할 수도 있다. 한편, 이러한 문제점을 해소하기 위해서 미립구 중에 포함되는 약물의 함량을 낮추는 경우에는 치료를 위한 약물의 함량이 충분치 않기 때문에 상대적으로 1회 투여되는 미립구의 양이 과도하게 증가되게 되어 환자의 복약순응도(compliance) 및 불편함을 초래할 수 있다는 문제점이 있다.
이에 본 발명자들은 미립구를 제조시 약물과 함께 파모산을 추가하는 경우 다량의 약물을 포함한다 하더라도 초기의 급격한 방출 및 약물이 충분한 기간동안 방출되지 않는 문제를 해소할 수 있다는 점을 확인하였다. 그런데, 이와 같이 파모산을 미립구 제조를 위해 사용시, 예기치 않게 도네페질과 함께 파모산을 상기 함량비로 사용하는 경우 다량의 약물을 포함한다 하더라도 초기의 급격한 방출 및 약물이 충분한 기간동안 방출되지 않는 문제를 해소할 수 있다는 점을 확인하였다.
구체적으로, 본 발명은 유효성분으로서 도네페질:파모산의 몰비가, 도네페질 자유염기 형태 기준으로(즉, 도네페질 자유염기:파모산의 몰비) 1 : 0.05 초과 1 : 0.25 이하, 바람직하게는 1 : 0.05 초과 1 : 0.2 이하, 1 : 0.05 초과 1 : 0.19 이하, 1 : 0.07 내지 1: 0.22, 1 : 0.08 내지 1 : 0.20, 1:0.09 내지 1 : 0.19, 1 : 0.1 내지 1 : 0.25, 1 : 0.125 내지 1 : 0.25 일 수 있다.
또 다른 하나의 양태로서, 본 발명에 따른 서방성 미립구 중의 파모산은 전체 미립구 중량 대비 2.5 내지 20중량%, 2.5 내지 17중량%, 2.5 내지 15중량% 또는 2.5 내지 13중량%으로 포함될 수 있으나 이에 제한되는 것은 아니다.
상기 생체적합성 고분자는 전체 미립구 중량 대비 20 내지 50중량% 또는 24 내지 50중량% 포함될 수 있으나, 이에 제한되는 것은 아니다.
상기 생체적합성 고분자는 폴리(락타이드-코-글리콜라이드), 폴리(락타이드-코-글리콜라이드)글루코스, 폴리락타이드, 폴리글리콜라이드, 폴리카프로락톤, 및 이들의 혼합물; 및 폴리글리콜라이드, 폴리락타이드 및 폴리글리콜라이드와 폴리락타이드 공중합체로 이루어지는 군으로부터 선택되는 적어도 하나일 수는 있으나 이에 제한되는 것은 아니다.
상기 생체적합성 고분자로는 바람직하게는 폴리락타이드가 사용될 수 있다. 또한 두 가지 이상의 고분자를 블렌딩하여 사용할 수 있다. 일 예로 폴리락타이드와 폴리(락타이드-코-글리콜라이드)를 블렌딩하여 제조될 수 있다. 이 경우 전체 고분자 중량 대비 폴리락타이드가 50중량% 이상 포함되는 것이 바람직하다.
바람직한 일 양태에서, 폴리글리콜라이드와 폴리락타이드 공중합체의 경우에는 상기 공중합체 내의 락타이드 대 글리콜라이드의 몰 비가 40:60 내지 90:10, 45:55 내지 85:15 또는 50:50 내지 75:25, 예를 들어 45:55, 50:50, 75:25, 또는 85:15일 수 있다.
이러한 생체적합성 고분자는 4,000 내지 240,000의 중량평균분자량을 갖는 것일 수 있다. 예를 들어, 상기 생체적합성 고분자의 중량평균분자량은 4,000 내지 100,000의 중량평균분자량, 7,000 내지 50,000의 중량평균분자량, 5,000 내지 20,000의 중량평균분자량, 10,000 내지 18,000의 중량평균분자량, 18,000 내지 28,000의 중량평균분자량 등 상기 범위 내 하위 수치 범위들을 모두 포함한다.
상기 생체적합성 고분자가 2종 이상으로 포함되는 경우, 상기 예시된 고분자들의 종류가 서로 상이한 고분자들의 조합 또는 블렌드일 수도 있으나, 동일한 종류의 고분자들이 서로 다른 고유점도 및/또는 단량체의 비율을 가지는 고분자들의 조합(예를 들어 서로 다른 고유점도를 갖는 폴리(락타이드-코-글리콜라이드) 둘 이상의 조합 또는 블렌드), 또는 말단기가 서로 다른(예를 들어 말단기가 에스터이거나 말단기가 산인 동일 종류의 고분자일 수도 있다. 본 발명에서 사용될 수 있는, 시판 중인 생체적합성 고분자의 예로는, 에보닉 롬 게엠베하(Evonik Rohm GmbH)사의 Resomer 계열인 RG 502H, RG 503H, RG 504H, RG 502, RG 503, RG 504, RG 653H, RG 752H, RG 753H, RG 752S, RG 755S, RG 756S, RG 858S, R 202H, R 203H, R 205H, R 202S, R 203S, R 205S, 코비온사의 PDL 02, PDL 04, PDL 05, PDL 06, PDLG 7502A, PDLG 7502, PDLG 7507, PDLG 5002A, PDLG 5002, PDLG 5004A, PDLG 5004, PDLG 5010, PL 10, PL 18, PL 24, PL 32, PL 38, PDL 20, PDL 45, PC 02, PC 04, PC 12, PC 17, PC 24를 단독으로 또는 조합 또는 블렌드한 것 등을 들 수 있으나 이에 제한되는 것은 아니다.
상기 생체적합성 고분자로는 바람직하게는 폴리락타이드가 사용될 수 있다. 바람직하게, 상기 폴리락타이드의 고유점도는 0.25dL/g 내지 2.0dL/g, 0.25dL/g 내지 1.7dL/g, 또는 0.25dL/g 내지 1.5dL/g일 수 있다. 폴리락타이드의 고유 점도가 0.25dL/g 미만으로 점도가 낮은 경우, 미립구 내에 유효성분이 잘 봉입되지 않는 문제점이 있고, 점도가 1.5dL/g를 초과하는 경우 미립구의 제조 수율이 낮아지게되어 경제성이 떨어진다는 문제점이 있다.
본 발명에서 사용한 폴리락타이드의 고유점도는 우벨로데(Ubbelohde) 점도계를 이용하여 25℃에서 클로로포름 중에서 0.1%(w/v) 농도로 측정된 것을 말한다.
또한 두 가지 이상의 고분자를 블렌딩하여 사용할 수 있다. 일 예로 폴리락타이드와 폴리(락타이드-코-글리콜라이드)를 블렌딩하여 제조될 수 있다. 이 경우 폴리락타이드가 50% 이상 포함되는 것이 바람직하다.
상기 서방성 미립구는 추가로 도네페질 파모산염을 포함하는 것일 수 있다.
일 실시 양태로, 본 발명에 따른 서방성 미립구 중에는 도네페질과 함께, 도네페질의 파모산염 형태 및/또는 파모산이 존재할 수 있다.
일 실시 양태로, 상기 도네페질 파모산염은 비결정형일 수 있다.
일 실시 양태로, 상기 도네페질은 도네페질 자유 염기일 수 있다.
일 실시 양태로, 상기 자유염기 형태를 기준으로 도네페질의 함량이 미립구 전체 중량 대비 75 중량% 이하, 40 내지 75 중량%, 40 내지 70 중량%, 40 내지 65 중량, 45 내지 75 중량%, 45 내지 70 중량%, 45 내지 65 중량%, 45 내지 60 중량%, 50 내지 75 중량%, 50 내지 70 중량%, 50 내지 65 중량%, 또는 50 내지 60 중량%일 수 있으나 이에 제한되는 것은 아니다.
상기 범위를 충족하는 경우 약물의 방출이 급격하거나 지나치게 지연되지 않는 적정한 방출률을 방출 기간 동안 유지할 수 있다는 장점을 갖는다.
본 발명의 미립구는 균일한 입자분포도를 갖는 것이 바람직하다. 균일한 입자분포도를 갖는 미립구는 불균일한 미립구에 비해 주사 시 편차가 작고 보다 정확한 양으로 투여가 가능하다.
본 발명의 미립구의 스팬값(Span value)이 1.2 이하인 것이 바람직하다.
본 발명에서 사용하는 용어 “스팬값(Span value)”이라 함은 미립구의 입자크기의 균일성을 나타내는 지표로서, 스팬값은 하기 수학식 1로 구한 값을 의미한다.

여기에서 Dv0.1은 미립구의 입도분포곡선에서 부피%의 10%에 해당하는 입도, Dv0.5는 미립구의 입도분포곡선에서 부피%의 50%에 해당하는 입도, Dv0.9는 미립구의 입도분포곡선에서 부피%의 90%에 해당하는 입도를 의미한다.
구체적인 일 실시 양태에서, 본 발명에 따른 미립구의 평균입경(Dv50)은 10um 이상, 바람직하게는 15um 이상, 20 um 이상, 30 um 이상, 150um이하, 120um이하, 100um 이하, 90 um 이하, 80 um 이하, 70 um 이하, 10 내지 150um, 10 내지 120um, 10 내지 100um, 10 내지 90um, 15 내지 90um, 15 내지 80um, 20 내지 90um, 20 내지 80um, 30 내지 80um, 또는 30 내지 70um일 수 있다.
본 발명에 따르면, 미립구 중의 상기 약물은 1일 초기 방출률이 10% 미만일 수 있다. 바람직하게, 상기 초기 방출률은 9% 미만, 8% 미만, 또는 7% 미만일 수 있으나 이에 제한되는 것은 아니다.
본 발명에 따르면, 미립구의 체내(in vivo) 1일 방출시의 AUC가 전체 AUC의 4% 미만일 수 있다. 바람직하게, 상기 1일 방출시의 AUC는 4% 미만 또는 3% 미만, 0.01% 이상, 0.3% 이상, 또는 0.5% 이상일 수 있으나 이에 제한되는 것은 아니다.
구체적인 일 실시 양태에서, 본 발명에 따른 서방성 미립구 중의 도네페질 또는 이의 약학적으로 허용가능한 염이 1개월 내지 6개월 동안 방출되는 것일 수 있다.
구체적인 일 실시 양태에서, 본 발명에 따른 서방성 미립구는 주사용인 것일 수 있다.
또 다른 양태로서, 본 발명은 상기 서방성 미립구를 포함하는 알츠하이머병 또는 인지기능 장애의 예방, 개선 또는 치료용 약학적 조성물에 관한 것이다. 보다 구체적으로, 상기 약학적 조성물은 약학적으로 허용가능한 담체를 더 포함할 수 있다. 바람직하게, 상기 약학적 조성물은 주사제제일 수 있다.
이하, 본 발명의 서방형 미립구의 제조 방법을 구체적으로 설명한다.
본 발명에 따른 서방형 미립구는 예를 들어, "용매 추출 및 증발법"을 사용하여 제조될 수 있으나, 제조방법은 이에 국한되지 않는다.
본 발명에 따른 서방형 미립구 제조방법의 구체적인 일례로, 다음의 단계를 포함하는 서방성 미립구의 제조방법이 제공된다:
(a) 유효성분으로서 도네페질 또는 이의 약학적으로 허용가능한 염; 파모산 및 생체적합성 고분자를 분산상용 용매에 용해시켜 분산상을 제조하는 단계;
(b) 상기 단계 (a)에서 제조된 분산상을, 연속상으로서 계면활성제를 함유한 수용액에 첨가하여 에멀젼을 제조하는 단계;
(c) 상기 단계 (b)에서 제조된 에멀젼의 분산상 중의 분산상용 용매를 연속상으로 추출하고, 상기 추출된 용매를 증발시켜 미립구를 형성시켜 미립구를 포함하는 현탁액을 제조하는 단계; 및
(d) 상기 단계 (c)의 현탁액으로부터 미립구를 회수하여 미립구를 제조하는 단계를 포함하고,
상기 유효성분:파모산의 몰비가 도네페질 자유염기 형태 기준으로 1 : 0.05 초과 1 : 0.25이하인 것을 특징으로 한다.
상기 제조방법에서, 달리 언급이 없다면 도네페질 또는 이의 약학적으로 허용가능한 염, 파모산 및 생체적합성 고분자 등에 관한 사항은 상기 미립구에서 기재된 사항을 그대로 적용할 수 있다.
구체적인 일 실시 양태에서, 상기 유효성분의 함량이 미립구 전체 중량 대비 75 중량% 이하인 것일 수 있다.
구체적인 일 실시 양태에서, 상기 생체적합성 고분자의 함량은 미립구 전체 중량 대비 20 내지 50 중량% 일 수 있다.
구체적인 일 실시 양태에서, 상기 파모산의 함량은 미립구 전체 중량 대비 2.5 내지 20 중량%일 수 있다.
또한 구체적인 일 실시 양태에서, 단계 (a)에서 상기 유효성분은 자유염기 형태이고, 파모산은 분말 또는 용액의 형태로 포함될 수 있다.
구체적인 일 실시 양태로서, 단계 (a)에서 분산상용 용매에 용해된 유효성분의 용액에, 파모산을 첨가하여 수행될 수 있다.
상기 분산상용 용매는 물과 혼화되지 않는 성질의 용매를 전체 혼합 용매 대비 최소한 50%(w/w) 이상 사용하는 것이 바람직하나, 이에 제한되는 것은 아니다.
이러한 분산상용 용매에 제한은 없으나, 디클로로메탄, 에틸아세테이트, 에틸포메이트, 메틸아세테이트, 메틸포메이트, 부틸아세테이트, n-프로필아세테이트, 이소프로필아세테이트, n-프로필포메이트, 글리코퓨롤, 메틸이소프로필케톤, 디메틸카보네이트 또는 이들의 혼합용매를 포함하는 것일 수 있다.
또한, 상기 분산상용 용매에, 클로로포름, 아세톤, 아세토니트릴, 디메틸설폭사이드, 디메틸포름아마이드, 메틸에틸케톤, 아세트산, 메틸알콜, 에틸알콜, 프로필알콜, 벤질알콜 및 이들의 혼합용매로 이루어지는 군으로부터 선택되는 1종 이상을 추가로 포함할 수 있다.
상기 단계 (b)에서 분산상과 계면활성제를 함유한 연속상을 균질하게 혼합하는 방법은 특별히 제한되지 않으나, 고속 교반기, 인라인 믹서기, 멤브레인 에멀젼법, 마이크로플루이딕스 에멀젼법 등을 이용하여 수행할 수 있다. 고속 교반기, 인라인 믹서기를 이용하여 에멀젼을 형성하는 경우, 균일한 에멀젼을 얻기가 어렵기 때문에 후술하는 단계 (c)와 단계 (d)사이에서 추가적으로 체과 공정 등을 수행하는 것이 바람직하다. 멤브레인 에멀젼법과 마이크로플루이딕스 에멀젼법을 이용할 경우 균일한 크기의 에멀젼을 얻을 수가 있어 후술하는 단계 (c)와 단계 (d)사이에서 추가적으로 체과 공정 등이 필요하지 않으므로 보다 바람직하다.
상기 단계 (b)에서 사용되는 계면활성제의 종류는 특별히 제한되지 않고, 분산상이 연속상 내에서 안정한 액적의 분산상을 형성할 수 있도록 도와줄 수 있는 것이라면 어느 것이라도 사용할 수 있다. 상기 계면활성제는 바람직하게는, 메틸셀룰로오스, 폴리비닐피롤리돈, 카르복시메틸셀룰로오스, 레시틴, 젤라틴, 폴리비닐알코올, 폴리옥시에틸렌 소르비탄 지방산 에스테르 및 폴리옥시에틸렌 피마자유 유도체 및 이들의 혼합물로 이루어진 군으로부터 선택될 수 있고, 가장 바람직하게는 폴리비닐알코올을 사용할 수 있다.
상기 단계 (b)에서, 계면활성제를 함유한 연속상 중의 계면활성제의 함량은 계면활성제를 포함한 연속상의 전체 부피를 기준으로, 0.01 w/v% 내지 20 w/v%, 바람직하게는 0.1 w/v% 내지 5 w/v%일 수 있다. 계면활성제의 함량이 0.01 w/v% 미만일 경우에는, 연속상 내에 액적 형태의 분산상 또는 에멀젼이 형성되지 않을 수 있고, 계면활성제의 함량이 20 w/v%를 초과할 경우에는, 과량의 계면활성제로 인해 연속상 내에 미립자가 형성된 후, 계면활성제를 제거하는데 어려움이 있을 수 있다.
상기 단계 (b)에서 사용되는 연속상은 물을 사용할 수 있으며, 에멀젼 상태의 분산상으로부터 유기용매의 추출 속도를 조절하기 위하여 메틸알콜, 에틸알콜, 프로필알콜, 에틸포메이트, 메틸아세테이트, 메틸포메이트, 부틸아세테이트, n-프로필아세테이트, 이소프로필아세테이트, n-프로필포메이트, 글리코퓨롤, 메틸이소프로필케톤, 디메틸카보네이트, 및 에틸아세테이트로 이루어진 군으로부터 선택되는 1종 이상이 일부 함유된 물을 사용할 수 있다.
상기 연속상은 분산상 용매 대비 5,000(v/w)% 내지 100,000(v/v)% 사용될 수 있으나, 이에 제한되는 것은 아니다.
상기 단계 (c)에서, 액적 형태의 분산상 및 계면활성제를 함유한 연속상을 포함하는 에멀젼을 유기 용매의 비등점 미만의 온도에서 일정 시간, 예를 들면, 2 시간 내지 48 시간 동안 유지 또는 교반하면, 액적형태의 분산상으로부터 연속상으로 유기 용매가 추출될 수 있다. 연속상으로 추출된 유기 용매의 일부는 표면으로부터 증발될 수 있다. 액적 형태의 분산상으로부터 유기 용매가 추출되면서, 상기 액적 형태의 분산상은 고형화되어 미립구를 형성할 수 있다.
상기 단계 (c)에서 유기 용매를 추가적으로 효율적으로 제거하기 위해서 연속상의 온도를 일정 시간 동안 열을 가할 수 있다.
상기 단계 (d)에서, 본 발명에 따른 미립구를 회수하는 방법은 여러 가지 공지 기술을 사용하여 수행될 수 있으며, 예를 들어 여과 또는 원심분리 등의 방법을 이용할 수 있다.
상기 단계 (c) 및 단계 (d) 사이에, 여과 및 세척을 통해 잔류하는 계면활성제를 제거하고, 다시 여과시켜 미립구를 회수할 수 있다.
잔존하는 계면활성제를 제거하기 위한 세척 단계는 통상적으로 물을 이용하여 수행할 수 있으며, 상기 세척 단계는 수회에 걸쳐 반복할 수 있다.
또한, 전술한 바와 같이 상기 단계 (b)에서 고속교반기, 인라인 믹서기를 이용하여 에멀젼을 형성한 경우, 상기 단계 (c) 및 단계 (d) 사이에, 체과 공정을 추가적으로 사용함으로 균일한 미립구를 얻을 수 있다. 공지 기술을 사용하여 체과 공정을 수행할 수 있으며 크기가 서로 다른 체막을 이용하여 작은 입자와 큰 입자의 미립구를 걸러내서 균일한 크기의 미립구를 얻을 수 있다.
본 발명의 제조 방법은, 상기 단계 (d) 이후 또는 상기 여과 및 세척 단계 이후, 수득된 미립구를 통상의 건조 방법을 이용하여 건조시켜 최종적으로 건조된 미립구를 얻을 수 있다.
이하, 본 발명의 이해를 돕기 위하여 바람직한 실시예를 제시하나, 하기 실시예는 본 발명을 예시하는 것일 뿐 본 발명의 범주 및 기술사상 범위 내에서 다양한 변경 및 수정이 가능함은 당업자에게 있어서 명백한 것이며, 이러한 변형 및 수정이 첨부된 특허청구범위에 속하는 것도 당연한 것이다.
[실시예]
이하, 실시예를 통하여 본 발명을 더욱 상세히 설명하기로 한다. 이들 실시예는 단지 본 발명을 예시하기 위한 것이므로, 본 발명의 범위가 이들 실시예에 의해 제한되는 것으로 해석되지는 않는다.
비교예 1. 파모산이 첨가되지 않은 도네페질 미립구 제조
분산상은 생체 적합성 고분자인 Purasorb PDL02A (제조사: Corbion, 네덜란드) 3.0g 및 도네페질 베이스 (제조사 : Neuland Laboratories, 인도) 2.0g을 디클로로메탄(제조사: J.T Baker, 미국) 9.0g과 혼합하여 육안 상 투명해질 때 까지 충분히 용해시킨 후 사용하였다. 연속상은 0.1% 폴리비닐알콜(점도: 4.8~5.8 mPa·s) 수용액을 사용하였으며, 연속상 3,000mL를 직경 40 μm의 다공성 멤브레인을 장착한 유화장치에 연결하는 동시에 준비된 분산상을 주입하여 미립구를 제조하였으며, 미립구 현탁액은 조제용기에 담아 200 rpm 속도로 교반하였다.
멤브레인 유화장치 및 조제용기 온도는 25℃를 유지하였으며, 분산상 주입이 끝나면 조제용기 내 현탁액을 200 rpm 속도로 교반하면서 온도 25℃에서 60분간 유지하였다. 이후 온도를 45℃로 3시간 유지하면서 유기용매를 제거하였다. 유기용매 제거가 끝나면 미립구 현탁액 온도를 25℃로 낮추었다. 미립구 현탁액을 초순수로 수차례 반복 세척하여 잔여 폴리비닐알콜을 제거하고 미립구를 동결건조 하였다.
비교예 2-6. 파모산의 비율이 다른 도네페질 미립구 제조
분산상은 생체 적합성 고분자인 및 도네페질 베이스 (제조사 : Neuland Laboratories, 인도) 를 디클로로메탄(제조사: J.T Baker, 미국) 과 혼합하여 육안 상 투명해질 때 까지 충분히 용해시킨 후 사용하였다. 연속상은 0.1% 폴리비닐알콜(점도: 4.8~5.8 mPa·s) 수용액을 사용하였으며, 연속상을 직경 40 μm의 다공성 멤브레인을 장착한 유화장치에 연결하는 동시에 준비된 분산상을 주입하여 미립구를 제조하였으며, 미립구 현탁액은 조제용기에 담아 200 rpm 속도로 교반하였다.
멤브레인 유화장치 및 조제용기 온도는 25℃를 유지하였으며, 분산상 주입이 끝나면 조제용기 내 현탁액을 200 rpm 속도로 교반하면서 온도 25℃에서 60분간 유지하였다. 이후 온도를 45℃로 3시간 유지하면서 유기용매를 제거하였다. 유기용매 제거가 끝나면 미립구 현탁액 온도를 25℃로 낮추었다. 미립구 현탁액을 초순수로 수차례 반복 세척하여 잔여 폴리비닐알콜을 제거하고 미립구를 동결건조 하였다.
본 비교예에서 사용된 고분자 종류 및 사용량, 약물의 사용량, 파모산의 사용량, 사용되는 용매, 분산상 용매의 사용량 및 연속산의 사용량은 다음 표 1에 나타낸 바와 같다. 하기 표 1에서 약물:파모산의 몰비는 약물 자유염기 형태를 기준으로 나타낸 것이다.
실시예 1-10. 파모산이 첨가된 도네페질 미립구 제조
분산상은 생체 적합성 고분자 및 도네페질 베이스 (제조사 : Neuland Laboratories, 인도), 파모산을 디클로로메탄(제조사: J.T Baker, 미국)과 혼합하여 육안상 투명해질 때 까지 충분히 용해시킨 후 사용하였다. 연속상은 0.1% 폴리비닐알콜 수용액(점도: 4.8~5.8 mPa·s)을 사용하였으며, 연속상을 직경 40 μm의 다공성 멤브레인을 장착한 유화장치에 연결하는 동시에 준비된 분산상을 주입하여 미립구를 제조하였으며, 미립구 현탁액은 조제용기에 담아 200 rpm 속도로 교반하였다.
멤브레인 유화장치 및 조제용기 온도는 25℃를 유지하였으며, 분산상 주입이 끝나면 조제용기 내 현탁액을 200 rpm 속도로 교반하면서 온도 25℃에서 60분간 유지하였다. 이후 온도를 45℃로 3시간 유지하면서 유기용매를 제거하였다. 유기용매 제거가 끝나면 미립구 현탁액 온도를 25℃로 낮추었다. 미립구 현탁액을 초순수로 수차례 반복 세척하여 잔여 폴리비닐알콜을 제거하고 미립구를 동결건조 하였다.
본 실시예에서 사용된 고분자 종류 및 사용량, 약물의 사용량, 파모산의 사용량, 사용되는 용매, 분산상 용매의 사용량 및 연속산의 사용량은 다음 표 1에 나타낸 바와 같다. 하기 표 1에서 약물:파모산의 몰비는 약물 자유염기 형태를 기준으로 나타낸 것이다.

실험예 1. 서방성 미립구의 약물 함량 측정
상기 실시예에서 제조된 미립구의 약물 함량을 측정하기 위하여, 미립구 10 mg을 디메틸 설폭사이드로 완전히 용해시킨 후, 이동상으로 희석하였다. 희석된 용액 20 uL를 HPLC에 주입하여 검출파장 271 nm에서 측정하였다. 본 측정에서 활용한 컬럼은 Inertsil ODS-3, 5 um, 4.6x150 mm이고 이동상은 인산완충액 (pH 5.0)과 아세토니트릴을 60:40(v/v) 비율로 혼합하여 사용하였다. 측정된 봉입량을 표 2에 나타내었다.
실험예 2. 서방성 미립구의 입도분석
미립구의 평균 입도, 분포 및 균일성을 정량적으로 측정하기 위하여 레이저 회절법을 이용하여 시험을 실시하였다. 실시예에서 제조된 미립구를 계면활성제를 함유한 초순수와 혼합하여 20초간 볼텍스 믹서로 혼합한 후 초음파발생기에 넣고 분산시켰다. 이용액을 입도분석장치(Paar, PSA 900L, 오스트리아)에 넣고 입도를 측정하였다.
상기 비교예에서 제조된 도네페질 함유 서방성 미립구의 입도 측정 결과를 하기 표 2에 나타내었다.

실험예 3: X-선 회절분석기를 통한 도페네질, 파모산, 및 이들의 혼합물의 결정 분석
도네페질 및 파모산의 물질 자체의 결정형태 및 미립구 내에 포함된 도네페질 및 파모산 혼합물의 결정형태가 동일한지 여부를 확인하고자 상기 실시예에서 제조된 미립구 중의 도네페질 및 파모산 혼합물의 결정형태 및 도네페질 및 파모산 물질 자체의 결정형태를 X-선 회절분석기를 사용하여 XRD 패턴을 확인하였다. 그 결과를 표 3 및 도 2에 각각 나타내었다.
표 3은 도네페질 및 파모산 물질 자체의 X-선 회절분석결과로 나타난 피크의 2-Theta(±0.2) 수치를 나타낸 것이다.

표 3 및 도 2에서 확인할 수 있는 바와 같이, 도네페질, 파모산, 및 도네페질 및 파모산의 혼합물은 표 3에서 나타난 바와 같이 XRD에서 특정 2-Theta 피크를 나타낸다. 그러나 도 2에서 확인할 수 있는 바와 같이 이러한 피크는 도네페질 및 파모산이 포함된 미립구에서는 나타내지 않음을 확인할 수 있었다.
이러한 결과에 따라, 본 발명의 미립구중의 도네페질과 파모산은 비결정형으로 존재하는 것을 알 수 있다.
실험예 4: 도네페질과 파모산의 함량비에 따른 미립구의 시험관 내(in vitro) 약물 초기방출 측정
상기 실시예에서 제조된 미립구에 사용된 도네페질 또는 이의 약학적으로 허용가능한 염과 파모산의 함량비에 따른 미립구의 약물 방출을 확인하기 위하여 다음의 실험을 수행하였다.
미립구 10 mg을 HDPE 광구병에 넣고 방출시험액(인산염 및 아지드화 나트륨이 포함된 pH 7.4 수용액) 50 mL을 채운 후 37°C 배양기에 보관한다. 검액으로 이 액 1 mL을 취하여 확보하고, 새로운 방출시험액 1 mL을 광구병에 추가한다.
정해진 시간에 검체를 채취하고 실험예 1과 동일한 분석조건으로 HPLC를 이용하여 함유량 및 방출률을 분석하였다. 그 결과를 표 4에 나타내었다.

상기 표 4에서 확인할 수 있는 바와 같이, 본 발명에 따른 미립구의 경우 사용된 도네페질과 파모산의 몰비가 1 : 0.05 초과 1 : 0.25 이하인 미립구는, 미립구의 투여시 약물의 과도한 초기 방출 문제없이 장기간에 걸쳐 약물이 서서히 방출되어 약물 방출면에서 바람직한 것을 확인할 수 있었다. 또한, 본 발명에 따른 미립구의 경우 사용된 도네페질과 파모산의 몰비가 1 : 0.05 초과 1 : 0.25 이하인 미립구는 파모산의 비율이 높아질수록 약물의 누적방출량이 줄어들기 때문에, 미립구 내 약물 함량을 더 높이더라도 약물의 과도한 초기 방출을 방지할 수 있다.
또한 본 발명의 미립구 중에서 도네페질의 봉입률이 89% 이상이고, 사용된 도네페질과 파모산의 몰비가 1:0.05 이하인 실시예 2, 4 및 5의 미립구의 경우 7일차 누적방출량이 50% 이상으로 과도하게 방출됨을 확인할 수 있었다.
또한, 본 발명의 미립구에서 사용된 도네페질의 함량이 80중량%인 경우 방출 1일차부터 80% 이상의 높은 방출률을 나타내어 급격한 초기방출을 나타냄을 알 수 있었다.
실험예 5: 동물을 이용한 본 발명에 따른 미립구의 단회 피하투여 약물동태 확인
상기 실시예에서 제조된 미립구에 사용된 도네페질 또는 이의 약학적으로 허용가능한 염과 파모산의 함량비에 따른 미립구의 약물 방출을 확인하기 위하여 동물에 대해 도네페질 단회 투여를 한 후 약물동태를 확인하고자 혈중 도네페질 농도를 측정하였다.
도네페질 투여용량은 26.04mg/head가 되도록 미립구를 계측하고 0.3mL 현탁액에 분산시킨 후 SD 랫드에 근육주사(intramuscular injection) 또는 피하주사(subcutaneous injection)하였다. 미리 계획된 시간마다 0.25~0.5 mL 혈액을 채취하고 HPLC를 이용하여 혈중 내 도네페질 농도를 측정하였다. 1일차의 AUC(%)를 표 5에 나타내었다.

Claims (21)
- 유효성분으로서 도네페질 또는 이의 약학적으로 허용가능한 염; 파모산 및 생체적합성 고분자를 이용하여 제조되는 서방성 미립구로서,상기 유효성분:파모산의 몰비가, 도네페질 자유염기 형태 기준으로 1 : 0.05 초과 1 : 0.25 이하이며,상기 미립구는 도네페질로서 7일간 누적 방출량이 30% 미만인 특성을 갖는 것인 서방성 미립구.
- 유효성분으로서 도네페질 또는 이의 약학적으로 허용가능한 염; 파모산 및 생체적합성 고분자를 포함하는 서방성 미립구로서,상기 유효성분:파모산의 몰비가, 도네페질 자유염기 형태 기준으로 1 : 0.05 초과 1 : 0.25 이하이며,상기 미립구는 도네페질로서 7일간 누적 방출량이 30% 미만인 특성을 갖는 것인 서방성 미립구.
- 제1항 또는 제2항에 있어서, 상기 도네페질 또는 이의 약학적으로 허용가능한 염 및 상기 파모산염은 비결정형인, 서방성 미립구.
- 제1항 또는 제2항에 있어서, 상기 자유염기 형태를 기준으로 도네페질의 함량이 미립구 전체 중량 대비 75 중량% 미만인 것인, 서방성 미립구.
- 제1항 또는 제2항에 있어서, 상기 도네페질은 도네페질 자유 염기인 것인, 서방성 미립구.
- 제1항 또는 제2항에 있어서, 상기 유효성분의 1일 방출량이 10% 미만인 것인, 서방성 미립구.
- 제1항 또는 제2항에 있어서, 미립구의 체내(in vivo) 1일 방출시의 AUC가 4% 미만인 것인, 서방성 미립구.
- 제1항 또는 제2항에 있어서, 상기 서방성 미립구 중의 약물이 1개월 내지 6개월 동안 방출되는 것인, 서방성 미립구.
- 제1항 또는 제2항에 있어서, 평균입경(Dv50)이 10um 이상인 것인, 서방성 미립구.
- 제1항 또는 제2항에 있어서, 상기 생체적합성 고분자는 폴리(락타이드-코-글리콜라이드), 폴리(락타이드-코-글리콜라이드)글루코스, 폴리락타이드, 폴리글리콜라이드, 폴리카프로락톤, 및 이들의 혼합물; 및 폴리글리콜라이드, 폴리락타이드 및 폴리글리콜라이드와 폴리락타이드 공중합체로 이루어지는 군으로부터 선택되는 적어도 하나인, 서방성 미립구.
- 제1항 또는 제2항에 있어서, 주사제제용인 것인 서방성 미립구.
- 제1항 또는 제2항에 따른 서방성 미립구를 포함하는 알츠하이머병 또는 인지기능 장애의 예방, 개선 또는 치료용 약학적 조성물.
- 제12항에 있어서, 약학적으로 허용가능한 담체를 더 포함하는 것인, 약학적 조성물.
- 제12항에 있어서, 주사제제인 것인, 약학적 조성물.
- (a) 유효성분으로서 도네페질 또는 이의 약학적으로 허용가능한 염; 파모산 및 생체적합성 고분자를 분산상용 용매에 용해시켜 분산상을 제조하는 단계;(b) 상기 단계 (a)에서 제조된 분산상을, 연속상으로서 계면활성제를 함유한 수용액에 첨가하여 에멀젼을 제조하는 단계;(c) 상기 단계 (b)에서 제조된 에멀젼의 분산상 중의 분산상용 용매를 연속상으로 추출하고, 상기 추출된 용매를 증발시켜 미립구를 형성시켜 미립구를 포함하는 현탁액을 제조하는 단계; 및(d) 상기 단계 (c)의 현탁액으로부터 미립구를 회수하여 미립구를 제조하는 단계를 포함하고,상기 유효성분:파모산의 몰비가 유효성분의 자유염기 형태 기준으로 1 : 0.05 초과 1 : 0.25 이하인 것을 특징으로 하는, 제1항 또는 제2항에 따른 서방성 미립구의 제조방법:
- 제15항에 있어서, 상기 유효성분의 함량이 미립구 전체 중량 대비 40 내지 70 중량%인 것인, 제조방법.
- 제15항에 있어서, 단계 (a)에서 상기 도네페질은 자유염기 형태인 것인, 제조방법.
- 제15항에 있어서, 단계 (a)에서 분산상용 용매에 용해된 약물의 용액에, 파모산을 첨가하여 수행되는 것인 제조방법.
- 제15항에 있어서, 상기 분산상용 용매는 디클로로메탄, 에틸아세테이트, 에틸포메이트, 메틸아세테이트, 메틸포메이트, 부틸아세테이트, n-프로필아세테이트, 이소프로필아세테이트, n-프로필포메이트, 글리코퓨롤, 메틸이소프로필케톤, 디메틸카보네이트, 또는 이들의 혼합용매를 포함하는 것인, 제조방법.
- 제15항에 있어서, 상기 분산상용 용매는 클로로포름, 아세톤, 아세토니트릴, 디메틸설폭사이드, 디메틸포름아마이드, 메틸에틸케톤, 아세트산, 메틸알콜, 에틸알콜, 프로필알콜, 벤질알콜 및 이들의 혼합용매로 이루어지는 군으로부터 선택되는 1종 이상을 추가로 포함하는 것인, 제조방법.
- 제15항에 있어서, 상기 생체적합성 고분자는 폴리(락타이드-코-글리콜라이드), 폴리(락타이드-코-글리콜라이드)글루코스, 폴리락타이드, 폴리글리콜라이드, 폴리카프로락톤, 및 이들의 혼합물; 및 폴리글리콜라이드, 폴리락타이드 및 폴리글리콜라이드와 폴리락타이드 공중합체로 이루어지는 군으로부터 선택되는 적어도 하나인, 제조방법.
