Abstract
本発明は、近視を予防、治療するための方法およびそれに使用する組成物を提供することを目的とする。本発明により、M2マクロファージ分極誘導物質、マストセルスタビライザー、ケミカルメディエーター遊離抑制薬などの脈絡膜常在性免疫細胞機能調整物質、または乳酸菌を投与することによって、近視を抑制、改善もしくは治療するための方法および組成物が提供される。脈絡膜におけるM2マクロファージへの分極を誘導すること、またはマストセルスタビライザー、ケミカルメディエーター遊離抑制薬、もしくは乳酸菌を投与することなどの脈絡膜常在性免疫細胞の機能を調整することは、近視予防、治療に効果的である。
Description
本発明は、近視を予防または治療するための方法および組成物に関し、より詳細には、近視の進行の抑制のために脈絡膜における免疫細胞に作用する組成物に関し、より詳細には、M2マクロファージへの分極を誘導すること、またはマストセルスタビライザー、ケミカルメディエーター遊離抑制薬、もしくは乳酸菌を投与することなどの脈絡膜常在性免疫細胞の機能を調整することによる方法および組成物に関する。
近視は、眼軸長の伸長のために網膜の前に焦点が結ばれる状態を指し、眼軸長が長いほど強い近視と言える。近視の有病率の増加につれ、近視の発症に関与する要因と近視進行抑制や予防法に対する関心が高まっている。近年の研究により、脈絡膜が眼の成長の調節と近視の発症に重要であることが示唆されており、脈絡膜の菲薄化は近視の構造的特徴であることが示されている。脈絡膜の厚さと軸方向の長さの間に負の相関があることから、脈絡膜の厚さの変化が眼軸長の伸長の予測バイオマーカーである可能性が示唆されている。しかし、脈絡膜が近視の発症や進行に関与する詳細なメカニズムは未だに不明である。
脈絡膜は細かい血管が豊富な網膜の外側を覆う組織であり、網膜の細胞へ酸素や栄養を供給する機能に加えて、眼球外側の強膜の組織リモデリングおよび眼の成長の調節に関与する成長因子を供給する機能を持つ。脈絡膜の厚さの減少、もしくは血流の減少が強膜の虚血と低酸素症に寄与し、眼軸長が伸びる強膜構造の変化へ影響していると考えられる。そこで、脈絡膜の厚さや血流を維持、増加することは近視の予防や治療における新しいターゲットとして注目されている。
脈絡膜の厚さを維持、増加させることにより近視進行を抑制するための方法が提案されており、例えば、クロセチン摂取(非特許文献1参照)、バイオレットライトの照射(非特許文献2)等が提案されている。
クロセチンは抗炎症性作用や免疫調整作用があることが知られており、T細胞のTh1/Th2及びTh17/Tregのバランスの調整やマクロファージなど炎症メディエーター(TNF-α, IL-6, IFN-γ等)を産生する細胞のNF-κB経路の抑制を介してその作用を発現することが知られている(非特許文献3~5)。
脈絡膜には13種類の細胞が存在し、そのうち4種類は免疫細胞である。組織常在性の免疫細胞は免疫応答以外に組織恒常性や構造の維持を担っており、例えばマクロファージが欠損したマウスや肥満細胞の脱顆粒を誘導したマウスでは脈絡膜が菲薄化することが報告された(非特許文献6~7)。

本開示は、近視を予防もしくは治療するための方法、およびそれに用いる組成物、特に脈絡膜常在性免疫細胞、特にマクロファージや肥満細胞を標的とした組成物を提供することを目的とする。
本発明者らは、脈絡膜におけるマクロファージのM2マクロファージへの分極が、脈絡膜の菲薄化を抑制し、近視の予防、治療に効果的であることを見出した。
また、肥満細胞(マストセル)は小血管周囲に多く分布し、血管からなる組織である脈絡膜においてもその存在が認められ、脈絡膜の形態維持への関与が示唆されているところ、本発明者らは、肥満細胞の脱顆粒を抑制する薬剤(マストセルスタビライザー)の点眼により近視進行が抑制されることを見出した。すなわち、本発明者らは、マイナスレンズ装用による近視モデルマウスに抗アレルギー薬の一種であるマストセルスタビライザー(クロモグリク酸ナトリウム、ペミロラストカリウム)を点眼投与することで近視進行を抑制することを見出した。なお、マイナスレンズ装用による近視誘導中のマウスに別の抗アレルギー薬である抗ヒスタミン薬(レボカバスチン)を点眼投与しても、近視進行の抑制は認められなかった。
さらに、Lactobacillus paracasei菌などのLactobacillus属などの乳酸菌はその投与によりM2マクロファージへの分極を誘導・活性化することが知られているところ、本発明者らは、Lactobacillus paracasei菌などのLactobacillus属などの乳酸菌の投与が近視進行を抑制することを見出した。これらのことに、先述の非特許文献6~7の知見を考え合わせれば、脈絡膜に常在する免疫細胞を標的とした介入により脈絡膜構造(厚み)を維持できれば近視進行に対して抑制的にはたらくと考えられ、これらの知見は、マクロファージや肥満細胞などの脈絡膜に存在する免疫細胞を標的とし、それぞれの性質を適切な状態へと誘導する組成物は、近視進行抑制作用を有すると考えられる。本発明は、このような知見に基づくものであり、以下の態様を包含する。

本開示によれば、脈絡膜常在性免疫細胞の機能を調整することによる、より具体的には、脈絡膜におけるM2マクロファージへの分極を誘導することによる、またはマストセルスタビライザー、ケミカルメディエーター遊離抑制薬、もしくは乳酸菌を投与することによる、近視の予防、治療の新たな方法を提供することができる。
本発明に係る詳細について説明する。本発明は、以下の実施形態及び実施例の内容に限定されず、本発明の要旨を包含する範囲で種々の変形例や応用例を含む。
[近視を抑制もしくは治療するための方法]
上記のとおり、近年、脈絡膜の菲薄化という変化が近視の発症に関与することが明らかになってきた。本発明者らは、この菲薄化を制御する因子について鋭意研究した結果、脈絡膜におけるマクロファージの状態の変化(分極)によって脈絡膜の厚みが変化し、近視進行を制御していることを見出した。
マクロファージの状態には、M1マクロファージとM2マクロファージがあり、M1マクロファージは主に炎症の惹起に関与するのに対して、M2マクロファージは炎症の終焉・抑制に関与するように拮抗的な働きをする。本発明者らは、脈絡膜においてM1マクロファージへの分極を誘導することにより、脈絡膜の菲薄化および近視の進行が認められること、一方、M2マクロファージへの分極を誘導することにより、脈絡膜の菲薄化および近視の進行が抑制されることを初めて見出した。また、肥満細胞(マストセル)は小血管周囲に多く分布し、血管からなる組織である脈絡膜においてもその存在が認められ、脈絡膜の形態維持への関与が示唆されているところ、本発明者らは、肥満細胞の脱顆粒を抑制する薬剤(マストセルスタビライザー)の点眼により近視進行が抑制されることを見出した。さらに、Lactobacillus paracasei菌などのLactobacillus属などの乳酸菌はその投与によりM2マクロファージへの分極を誘導・活性化することが知られているところ、本発明者らは、Lactobacillus paracasei菌などのLactobacillus属などの乳酸菌の投与が近視進行を抑制することを見出した。このように、本発明者らは、脈絡膜常在性免疫細胞の機能を調整することにより、近視の進行を抑制することができることを見出した。
したがって、本開示の一つの側面は、脈絡膜常在性免疫細胞の機能を調整することによる、より具体的には、脈絡膜においてマクロファージをM2マクロファージへ分極させることによる、またはマストセルスタビライザー、ケミカルメディエーター遊離抑制薬、もしくは乳酸菌を投与することによる、近視を抑制、改善もしくは治療するための方法に関する。
本開示の一つの側面は、脈絡膜常在性免疫細胞の機能を調整することによる、より具体的には、M2マクロファージ分極誘導物質を用いて、脈絡膜におけるM2マクロファージへの分極を誘導することによる、またはマストセルスタビライザー、ケミカルメディエーター遊離抑制薬、もしくは乳酸菌を投与することによる、近視を抑制、改善もしくは治療するための方法に関する。
本開示の一つの側面は、処置を必要とする対象において、脈絡膜常在性免疫細胞の機能を調整すること、より具体的には、対象の脈絡膜にM2マクロファージ分極誘導物質を送達すること、またはマストセルスタビライザー、ケミカルメディエーター遊離抑制薬、もしくは乳酸菌を投与することを含む、屈折の低下、眼軸の伸長、および/または脈絡膜の菲薄化を抑制するための方法に関する。
また、本開示の一つの側面は、近視発症のメカニズムである脈絡膜の菲薄化を制御するための、M2マクロファージへの分極を促す成分などの脈絡膜常在性免疫細胞の機能を調整する物質を探索するスクリーニング方法、および近視の予防および治療に有効な成分を探索するスクリーニング方法にも関する。

<脈絡膜常在性免疫細胞機能調整物質>
脈絡膜常在性免疫細胞機能調整物質は、脈絡膜常在性免疫細胞の機能を調整する物質である。脈絡膜常在性免疫細胞の機能を調整する物質としては、好ましくは、脈絡膜における炎症を抑制するための調整をする物質、脈絡膜を抗炎症的な環境とするよう調整する物質が挙げられる。脈絡膜常在性免疫細胞としては、例えば、マクロファージ、肥満細胞などが挙げられる。脈絡膜常在性免疫細胞機能調整物質としては、M2マクロファージ分極誘導物質、マストセルスタビライザー、ケミカルメディエーター遊離抑制薬、乳酸菌などが挙げられる。
脈絡膜常在性免疫細胞機能調整物質は、脈絡膜常在性免疫細胞の機能を調整する物質である。脈絡膜常在性免疫細胞の機能を調整する物質としては、好ましくは、脈絡膜における炎症を抑制するための調整をする物質、脈絡膜を抗炎症的な環境とするよう調整する物質が挙げられる。脈絡膜常在性免疫細胞としては、例えば、マクロファージ、肥満細胞などが挙げられる。脈絡膜常在性免疫細胞機能調整物質としては、M2マクロファージ分極誘導物質、マストセルスタビライザー、ケミカルメディエーター遊離抑制薬、乳酸菌などが挙げられる。
<M2マクロファージ分極誘導物質>
マクロファージは機能の異なる炎症型のM1マクロファージと抗炎症型のM2マクロファージに分類され、サイトカインや刺激性成分などによるシグナルに応答して分極化し特異的な機能発現を行うようになる。
したがって、本開示におけるM2マクロファージ分極誘導物質には、脈絡膜におけるM2マクロファージへの分極を誘導するシグナル伝達に関与する各種の物質が包含され、タンパク質、ペプチド、核酸医薬、低分子化合物、高分子化合物等を特に制限なく使用することができる。
その例としては、Th2細胞が産生するIL-4、IL-13などのサイトカインの他、ロイシンジッパー転写因子c-Maf、炭水化物結合レクチンgalectin-3等が挙げられる。また、M2マクロファージにはいくつかのポピュレーションがあることが知られており、M2aマクロファージを誘導する前出のIL-4、IL-13の他、M2cマクロファージの誘導に関与するIL-10、糖質コルチコイドホルモン(GC)、M2dマクロファージの誘導に関与するTLR2、TLR4、TLR7、TLR9及びそのリガンド(TLRに対するリガンド)等も本開示におけるM2マクロファージ分極誘導物質として挙げることができる。

M2マクロファージ分極誘導物質として、上記物質のアゴニストも使用することができる。なお、当業者は、例えば、RAW264細胞、J774細胞、U937細胞などの単球系細胞株、マウス腹腔内マクロファージ、骨髄由来マクロファージなどの細胞を評価対象の物質を含む培地で培養し、M2マクロファージマーカーの発現を評価するin vitroの評価系などを用いて、物質がM2マクロファージ分極誘導能を有するか否かを評価することで、本明細書に具体的に記載されているM2マクロファージ分極誘導物質以外にも、本開示の方法および組成物における使用に適したM2マクロファージ分極誘導物質を得ることができる。
M2マクロファージ分極誘導物質が、タンパク質やペプチドである場合は、それらをコードするDNA、RNAの形態で対象に投与されうる。M2マクロファージ分極誘導物質をコードする核酸はプラスミドあるいは発現ベクターを用いて対象に投与してもよい。発現ベクターは、例えば、ウイルスベクター、特にアデノウイルスベクターでありうるが、これに限定はされない。他の使用可能なウイルスベクターとしては、例えば、レトロウイルス、アデノ随伴ウイルス、ポックス、バキュロウイルス、ワクシニア、単純ヘルペス、エプスタインバール、ジェミニウイルス、およびカリモウイルスベクターなどが挙げられる。M2マクロファージ分極誘導物質をコードする核酸は、タンパク質を脈絡膜またはRPE特異的に発現させるための調節エレメントを含んでいてもよい。つまり、M2マクロファージ分極誘導物質をコードする核酸は、プロモーターやエンハンサーなどの調節エレメントと作動可能に連結されていてもよい。
<マストセルスタビライザー>
マストセルスタビライザー(mast cell stabilizer)は、マスト細胞安定化剤や、マスト細胞安定薬などとも言われる。マストセルスタビライザーは、例えば、マスト細胞からのアレルギー物質の放出を抑制する薬であり、例えば、マスト細胞の細胞膜を安定化させてマスト細胞からのアレルギー物質の放出を抑制する薬である。マストセルスタビライザーとしては、例えば、アシタザノラスト水和物液(ゼペリン)、アンレキサノクス(エリックス)、ペミロラストカリウム(アレギサール)、ペミロラストカリウム(ペミラストン)、クロモグリク酸ナトリウム(インタール)、トラニラスト(リザベン)、トラニラスト(トラメラス)、イブジラスト(ケタス)、β2-アドレナリンアゴニスト、クロモリンナトリウム、クロモグリク酸、ケトチフェン、メチルキサンチン、オマリズマブ、ペミロラスト、ケルセチンなどが挙げられ、好ましくは、クロモグリク酸もしくはペミロラストまたはその塩などが挙げられる。
マストセルスタビライザー(mast cell stabilizer)は、マスト細胞安定化剤や、マスト細胞安定薬などとも言われる。マストセルスタビライザーは、例えば、マスト細胞からのアレルギー物質の放出を抑制する薬であり、例えば、マスト細胞の細胞膜を安定化させてマスト細胞からのアレルギー物質の放出を抑制する薬である。マストセルスタビライザーとしては、例えば、アシタザノラスト水和物液(ゼペリン)、アンレキサノクス(エリックス)、ペミロラストカリウム(アレギサール)、ペミロラストカリウム(ペミラストン)、クロモグリク酸ナトリウム(インタール)、トラニラスト(リザベン)、トラニラスト(トラメラス)、イブジラスト(ケタス)、β2-アドレナリンアゴニスト、クロモリンナトリウム、クロモグリク酸、ケトチフェン、メチルキサンチン、オマリズマブ、ペミロラスト、ケルセチンなどが挙げられ、好ましくは、クロモグリク酸もしくはペミロラストまたはその塩などが挙げられる。
<ケミカルメディエーター遊離抑制薬>
ケミカルメディエーター遊離抑制薬は、例えば、肥満細胞などの免疫細胞からのケミカルメディエーターの放出を抑えることでアレルギー反応を抑える薬であり、例えば、肥満細胞(マスト細胞)の細胞膜を安定化させて、ヒスタミンなどが外に放出されないようにする薬であり、例えば、肥満細胞からヒスタミン、LTB4、LTC4、LTD4、PGD2、TXB2、PAFなどのアレルギー物質(アレルギー反応を引き起こす物質)が遊離するのを抑制する薬である。ケミカルメディエーター遊離抑制薬としては、例えば、アシタザノラスト水和物液(ゼペリン)、アンレキサノクス(エリックス)、ペミロラストカリウム(アレギサール)、ペミロラストカリウム(ペミラストン)、クロモグリク酸ナトリウム(インタール)、トラニラスト(リザベン)、トラニラスト(トラメラス)、イブジラスト(ケタス)、β2-アドレナリンアゴニスト、クロモリンナトリウム、クロモグリク酸、ケトチフェン、メチルキサンチン、オマリズマブ、ペミロラスト、ケルセチンなどが挙げられ、好ましくは、クロモグリク酸もしくはペミロラストまたはその塩などが挙げられる。
ケミカルメディエーター遊離抑制薬は、例えば、肥満細胞などの免疫細胞からのケミカルメディエーターの放出を抑えることでアレルギー反応を抑える薬であり、例えば、肥満細胞(マスト細胞)の細胞膜を安定化させて、ヒスタミンなどが外に放出されないようにする薬であり、例えば、肥満細胞からヒスタミン、LTB4、LTC4、LTD4、PGD2、TXB2、PAFなどのアレルギー物質(アレルギー反応を引き起こす物質)が遊離するのを抑制する薬である。ケミカルメディエーター遊離抑制薬としては、例えば、アシタザノラスト水和物液(ゼペリン)、アンレキサノクス(エリックス)、ペミロラストカリウム(アレギサール)、ペミロラストカリウム(ペミラストン)、クロモグリク酸ナトリウム(インタール)、トラニラスト(リザベン)、トラニラスト(トラメラス)、イブジラスト(ケタス)、β2-アドレナリンアゴニスト、クロモリンナトリウム、クロモグリク酸、ケトチフェン、メチルキサンチン、オマリズマブ、ペミロラスト、ケルセチンなどが挙げられ、好ましくは、クロモグリク酸もしくはペミロラストまたはその塩などが挙げられる。
<乳酸菌>
乳酸菌としては、例えば、ホモ乳酸菌、ヘテロ乳酸菌などが挙げられる。また、乳酸菌としては、例えば、球状の乳酸球菌、桿状の乳酸桿菌などが挙げられる。乳酸菌としては、例えば、グラム陽性であり、桿菌または球菌であり、芽胞がなく、運動性がなく、消費ブドウ糖に対して50%以上の乳酸を生成し、ナイアシン(B3)を必須要求する菌類が挙げられる。乳酸菌としては、例えば、腸管系乳酸菌、動物性乳酸菌、植物性乳酸菌、海洋乳酸菌などが挙げられる。乳酸菌としては、例えば、ラクトバシラス目の乳酸菌、放線菌門の乳酸菌などが挙げられる。ラクトバシラス目の乳酸菌としては、例えば、ラクトバシラス属(Lactobacillus)の乳酸菌、エンテロコッカス属(Enterococcus)の乳酸菌、ラクトコッカス属(Lactococcus)の乳酸菌、ペディオコッカス属(Pediococcus)の乳酸菌、ロイコノストック属(Leuconostoc)の乳酸菌、ストレプトコッカス属(レンサ球菌属)(Streptococcus)の乳酸菌などが挙げられる。放線菌門の乳酸菌としては、例えば、ビフィドバクテリウム属(Bifidobacterium)の乳酸菌などが挙げられる。Lactobacillus属の乳酸菌としては、例えば、Lactobacillus paracasei菌、Lactobacillus plantarum菌、Lactobacillus brevis菌、Lactobacillus delbrueckii菌、Lactobacillus acidophilus菌、Lactobacillus casei菌、Lactobacillus bulgaricus菌、Lactobacillus gasseri菌、Lactobacillus acidophilus菌、Lactobacillus fructivorans菌、Lactobacillus hilgardii菌、Lactobacillus rhamnosus菌、Lactobacillus plantarum菌、Lactobacillus casei Shirota菌などが挙げられる。Enterococcus属の乳酸菌としては、例えば、Enterococcus faecalis菌、Enterococcus faecium菌などが挙げられる。Lactococcus属の乳酸菌としては、例えば、Lactococcus lactis菌、Lactococcus cremoris菌などが挙げられる。Pediococcus属の乳酸菌としては、例えば、Pediococcus damnosus菌などが挙げられる。Leuconostoc属の乳酸菌としては、例えば、Leuconostoc mesenteroides菌などが挙げられる。Streptococcus属の乳酸菌としては、例えば、Streptococcus thermophiles菌、Streptococcus mutans菌などが挙げられる。Bifidobacterium属の乳酸菌としては、例えば、Bifidobacterium bifidum菌、Bifidobacterium adolescentis菌などが挙げられる。
Lactobacillus paracasei菌の投与により肝臓のKupffer cell(肝常在マクロファージ)をM2へシフトすることで非アルコール性脂肪肝炎の症状を改善したことが知られている(Sohn et al, Dig Dis Sci, 2015 Nov;60(11):3340-50)。Lactobacillus paracasei菌の投与はM2マクロファージを活性化することでブルーライトによる網膜変性を緩和することが知られている(Morita et al, Nutrients, 2018 Dec15;10(12):1991)。Lactobacillus plantarum菌の投与はM1マクロファージからM2マクロファージへの分極を促すことにより大腸炎を和らげることが知られている(Jang et al, Int Immunopharmacol, 2014 Jul;21(1):186-92)。Lactobacillus brevis菌の投与はM1マクロファージからM2マクロファージへの分極を促すことにより大腸炎を和らげることが知られている(Jang et al, J Appl Microbiol, 2013 Sep;115(3):888-96)。このように、Lactobacillus属などのLactobacillus目の乳酸菌はその投与によりM2マクロファージへの分極を誘導・活性化することが知られている。したがって、乳酸菌は、好ましくは、Lactobacillus属などのLactobacillus目の乳酸菌である。
乳酸菌としては、例えば、ホモ乳酸菌、ヘテロ乳酸菌などが挙げられる。また、乳酸菌としては、例えば、球状の乳酸球菌、桿状の乳酸桿菌などが挙げられる。乳酸菌としては、例えば、グラム陽性であり、桿菌または球菌であり、芽胞がなく、運動性がなく、消費ブドウ糖に対して50%以上の乳酸を生成し、ナイアシン(B3)を必須要求する菌類が挙げられる。乳酸菌としては、例えば、腸管系乳酸菌、動物性乳酸菌、植物性乳酸菌、海洋乳酸菌などが挙げられる。乳酸菌としては、例えば、ラクトバシラス目の乳酸菌、放線菌門の乳酸菌などが挙げられる。ラクトバシラス目の乳酸菌としては、例えば、ラクトバシラス属(Lactobacillus)の乳酸菌、エンテロコッカス属(Enterococcus)の乳酸菌、ラクトコッカス属(Lactococcus)の乳酸菌、ペディオコッカス属(Pediococcus)の乳酸菌、ロイコノストック属(Leuconostoc)の乳酸菌、ストレプトコッカス属(レンサ球菌属)(Streptococcus)の乳酸菌などが挙げられる。放線菌門の乳酸菌としては、例えば、ビフィドバクテリウム属(Bifidobacterium)の乳酸菌などが挙げられる。Lactobacillus属の乳酸菌としては、例えば、Lactobacillus paracasei菌、Lactobacillus plantarum菌、Lactobacillus brevis菌、Lactobacillus delbrueckii菌、Lactobacillus acidophilus菌、Lactobacillus casei菌、Lactobacillus bulgaricus菌、Lactobacillus gasseri菌、Lactobacillus acidophilus菌、Lactobacillus fructivorans菌、Lactobacillus hilgardii菌、Lactobacillus rhamnosus菌、Lactobacillus plantarum菌、Lactobacillus casei Shirota菌などが挙げられる。Enterococcus属の乳酸菌としては、例えば、Enterococcus faecalis菌、Enterococcus faecium菌などが挙げられる。Lactococcus属の乳酸菌としては、例えば、Lactococcus lactis菌、Lactococcus cremoris菌などが挙げられる。Pediococcus属の乳酸菌としては、例えば、Pediococcus damnosus菌などが挙げられる。Leuconostoc属の乳酸菌としては、例えば、Leuconostoc mesenteroides菌などが挙げられる。Streptococcus属の乳酸菌としては、例えば、Streptococcus thermophiles菌、Streptococcus mutans菌などが挙げられる。Bifidobacterium属の乳酸菌としては、例えば、Bifidobacterium bifidum菌、Bifidobacterium adolescentis菌などが挙げられる。
Lactobacillus paracasei菌の投与により肝臓のKupffer cell(肝常在マクロファージ)をM2へシフトすることで非アルコール性脂肪肝炎の症状を改善したことが知られている(Sohn et al, Dig Dis Sci, 2015 Nov;60(11):3340-50)。Lactobacillus paracasei菌の投与はM2マクロファージを活性化することでブルーライトによる網膜変性を緩和することが知られている(Morita et al, Nutrients, 2018 Dec15;10(12):1991)。Lactobacillus plantarum菌の投与はM1マクロファージからM2マクロファージへの分極を促すことにより大腸炎を和らげることが知られている(Jang et al, Int Immunopharmacol, 2014 Jul;21(1):186-92)。Lactobacillus brevis菌の投与はM1マクロファージからM2マクロファージへの分極を促すことにより大腸炎を和らげることが知られている(Jang et al, J Appl Microbiol, 2013 Sep;115(3):888-96)。このように、Lactobacillus属などのLactobacillus目の乳酸菌はその投与によりM2マクロファージへの分極を誘導・活性化することが知られている。したがって、乳酸菌は、好ましくは、Lactobacillus属などのLactobacillus目の乳酸菌である。
M2マクロファージ分極誘導物質、マストセルスタビライザー、ケミカルメディエーター遊離抑制薬などの脈絡膜常在性免疫細胞機能調整物質の対象の脈絡膜への送達にDDSを用いてもよい。

対象の脈絡膜へのM2マクロファージ分極誘導物質、マストセルスタビライザー、ケミカルメディエーター遊離抑制薬などの脈絡膜常在性免疫細胞機能調整物質の送達は、M2マクロファージ分極誘導物質、マストセルスタビライザー、ケミカルメディエーターなどの脈絡膜常在性免疫細胞機能調整物質を分泌する細胞を対象に移植することにより行ってもよい。M2マクロファージ分極誘導物質、マストセルスタビライザー、ケミカルメディエーターなどの脈絡膜常在性免疫細胞機能調整物質を分泌する細胞は、例えば、網膜色素上皮細胞(RPE)などでありうる。あるいは、遺伝子工学的にM2マクロファージ分極誘導物質、マストセルスタビライザー、ケミカルメディエーターなどの脈絡膜常在性免疫細胞機能調整物質を分泌するように改変された細胞であってもよい。
本開示の近視を抑制もしくは治療するための方法における対象は、ヒトを含む哺乳動物、イヌ、ネコ、ウシ、ウマを含む非ヒト哺乳動物でありうるが、好ましくはヒトである。
[近視を抑制もしくは治療するための組成物]
本開示の一つの側面は、M2マクロファージ分極誘導物質、マストセルスタビライザー、ケミカルメディエーター遊離抑制薬などの脈絡膜常在性免疫細胞機能調整物質、または乳酸菌を用いて近視を抑制、改善もしくは治療するための組成物に関する。より詳細には、本開示の態様の一つは、治療有効量のM2マクロファージ分極誘導物質、マストセルスタビライザー、ケミカルメディエーター遊離抑制薬などの脈絡膜常在性免疫細胞機能調整物質、または乳酸菌を含有する、近視を抑制もしくは治療するための組成物に関する。このような組成物は、対象の脈絡膜にM2マクロファージ分極誘導物質、マストセルスタビライザー、ケミカルメディエーター遊離抑制薬などの脈絡膜常在性免疫細胞機能調整物質、または乳酸菌を送達するために使用されうる。このような組成物は、医薬組成物、食品、またはサプリメントであり得る。

本開示に係る医薬組成物は、例えば、眼局所に投与される。本組成物の投与形態としては、例えば、点眼投与(眼軟膏の点入、洗眼も含むものとする)、結膜下投与、結膜嚢内投与、テノン嚢下投与などが挙げられる。
本組成物の剤形は、特に限定はされないが、例えば、点眼剤、眼軟膏、注射剤、貼付剤、ゲル、挿入剤などが挙げられ、例えば、点眼剤が好ましい。なお、これらは当該分野で汎用されている通常の技術を用いて調製することができる。
点眼剤は、塩化ナトリウム、塩化カリウム、濃グリセリンなどの等張化剤;リン酸ナトリウム、酢酸ナトリウム、イプシロン-アミノカプロン酸などの緩衝化剤;ポリオキシエチレンソルビタンモノオレート、ステアリン酸ポリオキシル40、ポリオキシエチレン硬化ヒマシ油などの界面活性剤;クエン酸ナトリウム、エデト酸ナトリウムなどの安定化剤;パラベンなどの防腐剤などから必要に応じて選択して用い、調製することができ、pHは眼科製剤に許容される範囲内にあればよいが、通常4~8の範囲内が好ましい。眼軟膏は、白色ワセリン、流動パラフィンなどの汎用される基剤を用い、調製することができる。
また、本開示に係る医薬組成物は、眼局所への投与に限られず、経腸投与(経口、経管、経注等)、非経口投与(経静脈、経動脈、経皮、筋肉注等)等の任意の投与経路で投与することもできる。これらの投与形態に用いる組成物の剤型は適宜選択できるが、例えば錠剤、顆粒剤、散剤、カプセル剤、咀嚼剤等の固形製剤や、液剤、シロップ剤、注射剤、点滴剤等の液体製剤等とすることができる。
本開示に係る医薬組成物には、本発明の効果を損なわない範囲で、その他の成分を適量配合することができる。その他の成分としては、任意の担体、緩衝剤、希釈剤、賦形剤、懸濁剤、潤滑剤、アジュバント、媒体、送達システム、乳化剤、錠剤分解物質、吸収剤、保存剤、界面活性剤、着色剤、香料、または甘味料等が挙げられる。これらの成分は、1種単独で又は2種以上を適宜組み合わせて配合することができる。

本開示に係る医薬組成物の投与量としては、特に制限はなく、投与形態や、投与対象の年齢、体重、所望の効果の程度等に応じて適宜選択することができる。M2マクロファージ分極誘導物質、マストセルスタビライザー、ケミカルメディエーター遊離抑制薬などの脈絡膜常在性免疫細胞機能調整物質、または乳酸菌の投与量は、例えば、1日あたり100~1000000nmol、好ましくは、150~100000nmolとし、投与頻度は、例えば、1月あたり1~100回とすることができる。
また、本開示に係る組成物は、食品またはサプリメントであり得る。食品またはサプリメントの形態としては、例えば、液体状、固形状、錠剤状、顆粒状、粉状、カプセル状、ペースト状、ゲル状など、例えば上記の固形または液体製剤等を任意に選択することができる。食品の具体例として、例えば、果汁飲料、野菜ジュース、清涼飲料、茶等の飲料類、スープ、プリン、ヨーグルト、ケーキプレミックス製品、菓子類、クッキー、キャンディー、グミ、ガム等の各種一般加工食品のほか、特別用途食品、特定保健用食品、栄養機能食品、機能性食品、栄養補助食品、健康補助食品、栄養強化食品、栄養調整食品等、例えばサプリメント、ドリンク剤等が挙げられる。

以下、実験例により本発明をさらに詳しく説明する。
<試験例1 LPS投与による脈絡膜菲薄化ならびに近視化>
M1マクロファージへの分極が脈絡膜の菲薄化ならびに近視化に関与するかどうかを確認するために、LPS投与後の脈絡膜における分極マーカー遺伝子の発現と脈絡膜厚、眼軸長および屈折値の測定を行なった。

2週間LPSを投与したマウスとPBSを投与した対照マウスにおいて脈絡膜厚、眼軸長および屈折値の測定を行い、その変化量を算出した(図1A、B)。その後眼球を摘出し、定量PCR法にて分極マーカー遺伝子の発現を測定した(図2)。
C57BL6Jマウス(各群n=4)を用いた。
LPS投与群には、LPS溶液を、10mg/kg BWの投与量で毎日腹腔内投与した。
PBS投与群には、LPS溶液に代えてPBSを投与した。

(眼軸長、脈絡膜の厚さ、屈折度の測定)
各群のマウスの眼軸長、脈絡膜の厚さ、屈折度を測定した。スペクトラルドメイン光コヒーレンストモグラフィー(Envisu R4310、Leica社製)を用いて眼軸長の測定と脈絡膜の厚さの測定を行った。マウス用赤外線フォトリフレクター(Infrared photorefractor for mice、Tubingen大学Schaeffel教授作製)を用いて屈折度の測定を行った。
各群のマウスの眼軸長、脈絡膜の厚さ、屈折度を測定した。スペクトラルドメイン光コヒーレンストモグラフィー(Envisu R4310、Leica社製)を用いて眼軸長の測定と脈絡膜の厚さの測定を行った。マウス用赤外線フォトリフレクター(Infrared photorefractor for mice、Tubingen大学Schaeffel教授作製)を用いて屈折度の測定を行った。
(電子顕微鏡による脈絡膜の観察)
各群のマウスの眼を採取し、PBS(Phosphate Buffered Saline)中の2.5%グルタルアルデヒドにより4℃で一晩固定し、0.1Mカコジル酸ナトリウム緩衝液で1時間すすいだ。 次に、0.1Mカコジル酸緩衝液中の1%OsO4で2時間固定した後、段階的エタノール溶液で脱水を行った。さらに、眼をプロピレンオキシドとエポン-アラルダイトの1:2混合物中に一晩浸透させ、100%樹脂に包埋した。ブロックを切断し、透過型電子顕微鏡(JEM1400 plus;JEOL)を用いて加速電圧100kVで観察した。

(遺伝子マーカーの発現評価)
上記のとおり2週間LPSを投与したマウスから脈絡膜、網膜、および強膜サンプルを採取し、定量PCR法にて、M1マクロファージマーカー遺伝子および酸化ストレス関連遺伝子の発現解析を行った。

(結果)
PBS投与を行なった対照群に対して、LPS投与群では1週間、2週間どちらの投与期間においても屈折の低下(図1B左)、眼軸の伸長(図1B中央)、脈絡膜の菲薄化(図1B右)が認められた。

また、遺伝子発現解析の結果、特に脈絡膜においてM1マーカー遺伝子、および酸化ストレス関連遺伝子の発現亢進が認められた(図2)。
これらの結果にあるように、脈絡膜におけるM1マクロファージの増加は近視化を誘導することが確認された。
<試験例2 IL-4、IL-13投与によるM2マクロファージへの分極ならびに近視抑制効果>
M1マクロファージは主に炎症の惹起に関与するのに対して、M2マクロファージは炎症の終焉・抑制に関与するように、拮抗的なはたらきを有する。M1マクロファージが近視誘導作用を有することからM2マクロファージは近視症・進行に対して抑制的に作用すると考えられる。
そこで、M2マクロファージへの分極に必要なサイトカインであるIL-4をマウスに投与し、脈絡膜においてM2への分極が生じるかをM2マクロファージマーカーであるCD163、CD206の発現量解析によって行なった(図3A、B)。
M1マクロファージは主に炎症の惹起に関与するのに対して、M2マクロファージは炎症の終焉・抑制に関与するように、拮抗的なはたらきを有する。M1マクロファージが近視誘導作用を有することからM2マクロファージは近視症・進行に対して抑制的に作用すると考えられる。
そこで、M2マクロファージへの分極に必要なサイトカインであるIL-4をマウスに投与し、脈絡膜においてM2への分極が生じるかをM2マクロファージマーカーであるCD163、CD206の発現量解析によって行なった(図3A、B)。

C57BL6Jマウス(各群n=4)に、IL-4溶液0.1μg/100μl(10μg/kg BW)を腹腔内投与し、0時間(投与前)および投与4、24、48時間後に脈絡膜、網膜、および肝臓サンプルを採取し、ウェスタンブロットおよびリアルタイムPCRにて、M2マクロファージマーカー遺伝子の発現解析を行った(図3)。また、0時間(投与前)および投与後4、24時間に脈絡膜および網膜サンプルを採取し、リアルタイムPCRにて、酸化ストレス関連遺伝子の発現解析を行った(図4)。
C57BL6Jマウス(各群n=4)に、IL-4溶液0.1μg/100μl(10μg/kg BW)を腹腔内投与し、0時間(投与前)および投与4、24、48時間後に脈絡膜、網膜、および肝臓サンプルを採取し、ウェスタンブロットおよびリアルタイムPCRにて、M2マクロファージマーカー遺伝子の発現解析を行った(図3)。また、0時間(投与前)および投与後4、24時間に脈絡膜および網膜サンプルを採取し、リアルタイムPCRにて、酸化ストレス関連遺伝子の発現解析を行った(図4)。
また近視誘導中のマウスへIL-4を投与し、近視抑制効果があるかどうかを評価した(図4)。
近視誘導中のC57BL6マウス(-30Dレンズ、n=4)に、IL-4溶液0.1μg/100μl(10μg/kg BW)を腹腔内に3週間投与した。対照群として、近視誘導中のC57BL6Jマウス(0Dレンズ、-30Dレンズ、各群n=4)を用いた。3週間後、試験例1と同様にして、屈折、眼軸の伸長、脈絡膜の菲薄化を測定した。
近視誘導中のC57BL6マウス(-30Dレンズ、n=4)に、IL-4溶液0.1μg/100μl(10μg/kg BW)を腹腔内に3週間投与した。対照群として、近視誘導中のC57BL6Jマウス(0Dレンズ、-30Dレンズ、各群n=4)を用いた。3週間後、試験例1と同様にして、屈折、眼軸の伸長、脈絡膜の菲薄化を測定した。
(結果)
その結果、IL-4の投与によって脈絡膜においてM2マーカーであるCD163とCD206の発現量の増加が認められ(図3B、C)、M2マクロファージへの分極が確認された。また、遺伝子発現解析により、M2マクロファージマーカー遺伝子の発現亢進、および酸化ストレス関連遺伝子の発現抑制が認められた(図5)。これらの結果から、IL-4の投与により、M2マクロファージへの分極が誘導され、酸化ストレス関連遺伝子の発現が抑制され、近視の進行が抑制されることが示唆された(図5)
その結果、IL-4の投与によって脈絡膜においてM2マーカーであるCD163とCD206の発現量の増加が認められ(図3B、C)、M2マクロファージへの分極が確認された。また、遺伝子発現解析により、M2マクロファージマーカー遺伝子の発現亢進、および酸化ストレス関連遺伝子の発現抑制が認められた(図5)。これらの結果から、IL-4の投与により、M2マクロファージへの分極が誘導され、酸化ストレス関連遺伝子の発現が抑制され、近視の進行が抑制されることが示唆された(図5)
また、近視誘導期間中にIL-4を投与することで屈折の低下の抑制(図5B左)、眼軸の伸長の抑制(図5B中央)、脈絡膜の菲薄化の抑制(図5B右)が確認された。
以上の結果から、IL-4投与による脈絡膜マクロファージのM2への分極は近視抑制効果を示すことが確認された。
<IL-13投与>
IL-4と同様にマクロファージをM2へ分極させるサイトカインとして知られるIL-13を近視誘導期間中に投与することで、IL-4の結果と同様に、マイナスレンズ装用による眼軸の伸長、屈折の近視化並びに脈絡膜の菲薄化が抑制されること、およびM2マクロファージへの分極が誘導されることがC57BL6Jマウスを用いて確認された(図6)。
なお、血流変化、マクロファージ数の計測は以下のとおり行った。
血流変化は波長掃引型光干渉断層計(OCT S-1, Canon)を用いて測定した。
マクロファージ数は脈絡膜の消化後、F4/80抗体並びにCD11b抗体にて染色し、フローサイトメーター(CytoFLEX S、ベックマンコールター)にて測定した。M2マクロファージ数の測定は、CD206抗体による染色過程を追加した他は、マクロファージ数の測定と同様にして測定した。
IL-4と同様にマクロファージをM2へ分極させるサイトカインとして知られるIL-13を近視誘導期間中に投与することで、IL-4の結果と同様に、マイナスレンズ装用による眼軸の伸長、屈折の近視化並びに脈絡膜の菲薄化が抑制されること、およびM2マクロファージへの分極が誘導されることがC57BL6Jマウスを用いて確認された(図6)。
なお、血流変化、マクロファージ数の計測は以下のとおり行った。

試験例1、2の結果にあるように、特に脈絡膜におけるマクロファージの状態を制御することで近視を誘導したり抑制することができることが確認された。近視進行抑制および治療という観点から見ると、特にM2マクロファージへの分極を促すことがその達成に有効であることを示している。
<試験例3 マストセルスタビライザー点眼による近視化の抑制>
マストセルスタビライザー点眼による近視進行の抑制効果を検証するために、マウスにマイナスレンズを装用することで近視を誘導するLens-induced myopiaモデルを用いて、その近視誘導期間中に1日1回クロモグリク酸溶液(4%溶液)またはぺミロラストカリウム(0.1%溶液)を点眼投与した。
マストセルスタビライザー点眼による近視進行の抑制効果を検証するために、マウスにマイナスレンズを装用することで近視を誘導するLens-induced myopiaモデルを用いて、その近視誘導期間中に1日1回クロモグリク酸溶液(4%溶液)またはぺミロラストカリウム(0.1%溶液)を点眼投与した。
上記の試験方法に従って、3週間の近視誘導およびクロモグリク酸溶液またはぺミロラストカリウム溶液の点眼投与を行ったマウスと対照マウスにおいて眼軸長、屈折値および脈絡膜厚の測定を行い、その変化量を算出した(図7A,B,C)。
その結果、PBS投与を行なった対照群のマイナスレンズ装用眼は対照眼と比較し、眼軸の伸長(図7A)、屈折の近視化(図7B)、および脈絡膜の菲薄化(図7C)が認められた。一方、クロモグリク酸溶液またはぺミロラストカリウム溶液の点眼投与の投与群ではPBS投与群で認められた上述の変化が認められなかった。
これらの結果にあるように、マストセルスタビライザー点眼投与は近視進行を抑制することが確認された。
<試験例4 マストセルスタビライザー点眼とヒスタミン受容体阻害薬点眼の近視抑制効果の比較>
抗アレルギー薬は、マストセルの脱顆粒を阻害するマストセルスタビライザーと、マストセルの脱顆粒により分泌されるヒスタミンのはたらきを阻害するヒスタミン受容体阻害薬の2種類に大別される。試験例3のマストセルスタビライザー点眼の近視抑制効果が抗アレルギー薬の作用であるのか、マストセルの脱顆粒を阻害したことによるものなのかを明らかにするために、LIMによる近視誘導を施したマウスにマストセルスタビライザーとしてぺミロラストカリウム(0.1%溶液)を、ヒスタミン受容体阻害薬としてレボカバスチン溶液(0.025%溶液)を点眼投与し、その近視抑制効果を比較した。結果を図8に示す。
抗アレルギー薬は、マストセルの脱顆粒を阻害するマストセルスタビライザーと、マストセルの脱顆粒により分泌されるヒスタミンのはたらきを阻害するヒスタミン受容体阻害薬の2種類に大別される。試験例3のマストセルスタビライザー点眼の近視抑制効果が抗アレルギー薬の作用であるのか、マストセルの脱顆粒を阻害したことによるものなのかを明らかにするために、LIMによる近視誘導を施したマウスにマストセルスタビライザーとしてぺミロラストカリウム(0.1%溶液)を、ヒスタミン受容体阻害薬としてレボカバスチン溶液(0.025%溶液)を点眼投与し、その近視抑制効果を比較した。結果を図8に示す。
図8に示すとおり、その結果、試験例3と同様に、ぺミロラストカリウム投与群では、対照群で認められる眼軸の伸長、屈折の近視化、および脈絡膜の菲薄化が認められなかった。一方、レボカバスチン投与群では、対照群と同様に、眼軸の伸長、屈折の近視化、および脈絡膜の菲薄化が認められた。
以上の結果から、抗アレルギー薬の中でも、マストセルスタビライザーによる近視抑制効果が確認された。
<試験例5 Lactobacillus paracasei菌の投与による近視進行の抑制>
M2マクロファージの誘導が近視進行を抑制できるため、乳酸菌としてヤクルト(株式会社ヤクルト本社)に含まれるLactobacillus paracasei菌を培養・増殖させ、近視モデルマウスに投与し、試験例3~試験例4と同様に、眼軸長、屈折値および脈絡膜厚の測定を行い、その変化量を算出した(図9)。その結果、乳酸菌の投与は近視進行を抑制することが確認された(図9)。
M2マクロファージの誘導が近視進行を抑制できるため、乳酸菌としてヤクルト(株式会社ヤクルト本社)に含まれるLactobacillus paracasei菌を培養・増殖させ、近視モデルマウスに投与し、試験例3~試験例4と同様に、眼軸長、屈折値および脈絡膜厚の測定を行い、その変化量を算出した(図9)。その結果、乳酸菌の投与は近視進行を抑制することが確認された(図9)。
Claims (14)
- 処置を必要とする対象に対して、治療有効量の脈絡膜常在性免疫細胞機能調整物質、または乳酸菌を投与することを含む、近視を抑制、改善もしくは治療するための方法。
- 脈絡膜常在性免疫細胞機能調整物質が、M2マクロファージ分極誘導物質、マストセルスタビライザー、またはケミカルメディエーター遊離抑制薬である、請求項1記載の方法。
- M2マクロファージ分極誘導物質が、タンパク質、ペプチド、核酸、低分子化合物、および高分子化合物からなる群より選択される、請求項2記載の方法。
- M2マクロファージ分極誘導物質が、IL-4、IL-10、IL-13、TLR2,4,7,9及びそのリガンド、ビサントレン二塩酸塩、トリプトライド(triptolide)、ロバスタチン(lovastatin)、QS11、レゴラフェニブ(regorafenib)、ソラフェニブ(sorafenib)、イキサゾミブ(ixazomib)、GW-843682X、KW 2449、アキシチニブ(axitinib)、JTE 013、プルモルファミン(purmorphamine)、アルシリアフラビンA(arcyriaflavin A)、ダサチニブ(dasatinib)、NVP-LDE225、1-ナフチルPP1(1-naphthyl PP1)、MGCD-265、ボスチニブ(bosutinib)からなる群より選択される、請求項2記載の方法。
- マストセルスタビライザー、またはケミカルメディエーター遊離抑制薬が、クロモグリク酸、もしくはぺミロラスト、またはその塩である、請求項2記載の方法。
- 乳酸菌が、Lactobacillus属である、請求項1記載の方法。
- 乳酸菌が、Lactobacillus paracasei菌である、請求項1記載の方法。
- 脈絡膜常在性免疫細胞機能調整物質、または乳酸菌が医薬組成物、サプリメントまたは食品中に含まれている、請求項1記載の方法。
- 屈折の低下、眼軸の伸長、および/または脈絡膜の菲薄化が抑制される、請求項1記載の方法。
- 治療有効量の脈絡膜常在性免疫細胞機能調整物質、または乳酸菌を含有する、近視を抑制もしくは治療するための組成物。
- 有効量の脈絡膜常在性免疫細胞機能調整物質、または乳酸菌を含有する、近視を抑制または改善するためのサプリメント。
- 有効量の脈絡膜常在性免疫細胞機能調整物質、または乳酸菌を含有する、近視を抑制または改善するための食品。
- 近視を抑制もしくは治療するための医薬の製造における、脈絡膜常在性免疫細胞機能調整物質、または乳酸菌の使用。
- 近視を抑制もしくは治療するための脈絡膜常在性免疫細胞機能調整物質、または乳酸菌の使用。
