Description
본 발명은 화학식 1로 표시되는 화합물 또는 이의 약학적으로 허용 가능한 염을 포함하는 말초 T 세포 림프종의 예방 또는 치료용 조성물에 관한 것이다. 구체적으로, 본 발명은 조절 T 세포를 감소시키고 CD8+ T 세포를 증가시킴으로써, 말초 T 세포 림프종을 효과적으로 개선할 수 있는 약학적 조성물에 관한 것이다.
포스파티딜이노시톨 3-키나아제 (PI3K; PI3 kinase)는 단백질 대신 지질 분자를 인산화하는 지질 키나아제 (lipid kinase)이고, 세포생존 (cell survival), 신호전달 (signal transduction), 세포막 투과조절 (control of membrane trafficking)등에서 중요한 역할을 한다. 이들 조절에 문제가 생기면, 암, 염증성 질환, 자가면역 질환 등이 발생한다.
최근 PI3 키나아제에 대하여 선택적으로 억제하는 효과를 나타낼 수 있는 신규한 구조의 화합물을 개발하는 연구결과가 보고되고 있고, 구체적으로, 국제공개특허 WO 2016/204429호는 PI3K 저해 활성을 보유하고, 암, 자가면역 질환 및 호흡기 질환 등의 치료에 유용한 화합물을 개시하고 있으며, 본 명세서에서 참고문헌으로서 포함된다. 그러나 PI3K-δ 단독 저해제 요법은 악성 림프종인 비호치킨성 림프종에서 예후가 좋지 않은 것으로 알려져 있다,
한편, PI3K-δ 단독 저해제 이외 다수의 PI3K 아형 및/또는 DNA-PK 활성화를 억제하는 저해제는 세포 주기 정지 및 세포사멸을 효율적으로 유도하고 발암성 물질의 안정성 감소를 자극하여, 지연성 비호치킨성 림프종에서도 효과적으로 항암 활성을 나타낼 것으로 예상된다.
현재 비호지킨성 림프종은 매년 국내에서 약 4300여명, 전세계적으로는 약 51만 여명의 환자가 발생한다고 알려져 있으나, 다수의 PI3K 아형 및/또는 DNA-PK 활성화를 억제하는 다중 저해제와 관련된 비호지킨성 림프종 치료제는 알려져 있지 않으며, 이의 개발이 필요한 실정이다.

본 발명에 있어서, 상기 약학적 조성물은 말초 T 세포 림프종 (PTCL, peripheral T-cell lymphoma) 이외에 재발성/불응성 B세포 림프종 (relapsed/refractory B cell lymphoma), 만성 림프구성 백혈병 (CLL, chronic lymphocytic leukemia)/소 림프구성 백혈병 (SLL, small lymphocytic leukemia), 말초 T 세포 림프종 (PTCL)의 예방 또는 치료를 위해 사용될 수 있고, 구체적으로, MYC 변성 미만성 거대 B 세포 림프종 및 변형된 미만성 거대 B 세포 림프종을 포함하는, 미만성 거대 B 세포 림프종 (DLBCL, Diffuse large B-cell lymphoma); 소포성 림프종 (follicular lymphoma); 만성 림프구성 백혈병 (CLL)/소 림프구성 백혈병 (SLL); 이에 제한되지 않으나, 외투세포 림프종 (MCL, mantle cell lymphoma), 변연부 림프종 (MZL; marginal zone lymphoma, 발덴스트롬 마크로글로불린혈증 (Waldenstrom macroglobulinemia)과 같은 다른 B 세포 림프종; 및 피부 T세포 림프종/균상식 육종 (composite CTCL-MF, Cutaneous T cell lymphoma-Mycosis fungoide)의 예방 또는 치료를 위해 사용될 수 있다.
상기 화학식 1로 표시되는 화합물은 PI3K 및 DNA-PK 이중 저해 활성을 갖는 것으로 알려진 헤테로아릴 유도체 중 하나이고, "4-[[(1S) -1-(4,8-디클로로-1-옥소-2-페닐-3-이소퀴놀릴)에틸]아미노]-8H-피리도[2,3-d]피리딘-5-온 {4-[[(1S) -1-(4,8-Dichloro-1-oxo-2-phenyl-3-isoquinolyl)ethyl]amino]-8H-pyrido[2,3-d]pyrimidin-5-one}"이라 명명된다.
본 발명에 있어서, 약학적으로 허용 가능한 염은 의약업계에서 통상적으로 사용되는 염을 의미하며, 예를 들어 칼슘, 포타슘, 소듐 또는 마그네슘 등으로 제조된 무기이온염, 염산, 질산, 인산, 브롬산, 요오드산, 과염소산 또는 황산 등으로 제조된 무기산염; 아세트산, 트라이플루오로아세트산, 시트르산, 말레산, 숙신산, 옥살산, 벤조산, 타르타르산, 푸마르산, 만델산, 프로피온산, 젖산, 글리콜산, 글루콘산, 갈락투론산, 글루탐산, 글루타르산, 글루쿠론산, 아스파르트산, 아스코르브산, 카본산 또는, 바닐릭산 등으로 제조된 유기산염; 메탄설폰산, 에탄설폰산, 벤젠설폰산, p-톨루엔설폰산 또는 나프탈렌설폰산 등으로 제조된 설폰산염; 글리신, 아르기닌, 라이신 등으로 제조된 아미노산염; 또는 트라이메틸아민, 트라이에틸아민, 암모니아, 피리딘, 피콜린 등으로 제조된 아민염 등이 있으나, 열거된 이들 염에 의해 본 발명에서 의미하는 염의 종류가 한정되는 것은 아니다.
본 발명에 있어서, 상기 조성물은 상기 화학식 1로 표시되는 화합물 또는 이의 약학적으로 허용 가능한 염을, 상기 화합물로서 25mg 이상 325mg 미만, 50mg 이상 325mg 미만, 100mg 이상 325mg 미만 또는 200mg 이상 325mg 미만으로 포함할 수 있으나, 이에 제한되는 것은 아니다.
본 발명의 일 구현예에서 상기 조성물은 상기 화학식 1로 표시되는 화합물 또는 이의 약학적으로 허용 가능한 염을, 상기 화합물로서 50mg, 100mg 또는 200mg으로 포함할 수 있고, 바람직하게는 200mg으로 포함할 수 있다.
본 발명에 따라 말초 T 세포 림프종 (PTCL) 대상체에 상기 조성물을 투여함에 있어서, 상기 화학식 1로 표시되는 화합물의 최대내약용량 (MTD, maximum tolerated dose)은 200mg QD일 수 있고, 이 용량범위에서 우수한 내약성 및 안전성을 보일 수 있다, 또한, 상기 내약성 및 안전성에 기초하여, 상기 화학식 1로 표시되는 화합물 200mg QD는 임상 Ib/II에 대한 시작용량(RP2D) 으로 권고될 수 있다.
본 발명에 따른 조성물에서 상기 용량범위는 말초 T 세포 림프종 (PTCL) 대상체에서 안전성 및 내약성이 있어 말초 T 세포 림프종 (PTCL)을 진단받은 대상체의 치료에 사용될 수 있다.
본 발명에 따른 조성물에서 상기 화학식 1로 표시되는 화합물의 약학적으로 허용 가능한 염은, 상기 화학식 1로 표시되는 화합물로서 25mg 이상 325mg 미만, 50mg 이상 325mg 미만, 100mg 이상 325mg 미만, 200mg 이상 325mg 미만인 것을 의미할 수 있으나 이에 제한되는 것은 아니고, 바람직하게는 25mg, 50mg, 100mg 또는 200mg인 것을 의미할 수 있으며, 더욱 바람직하게는 200mg인 것을 의미할 수 있다.
본 발명은 상기 용량 범위에서 말초 T 세포 림프종 (PTCL) 대상체에 대해 우수한 안전성 및 내약성을 나타내어 말초 T 세포 림프종 (PTCL)을 효과적으로 예방 또는 치료할 수 있다.
본 발명에 있어서, 상기 약학적 조성물은 이를 필요로 하는 대상체에게 25mg 이상 325mg 미만, 50mg 이상 325mg 미만, 100mg 이상 325mg 미만, 200mg 이상 325mg 미만, 바람직하게는 25mg, 50mg, 100mg 또는 200mg으로 1일 1회 (QD) 연속 28일 (제1주기, 4주) 이상 투여되는 것일 수 있다.
예를 들면, 상기 제1주기를 완료한 대상체들은, 종양의 진행, 내약 한도를 초과한 독성 또는 동의 철회가 발생할 때까지, 시험약물을 제1주기에서와 동일한 용량으로 계속 투여받을 수 있고, 4주 이상, 8주 이상, 12주 이상, 16주 이상, 20주 이상, 24주 이상, 28주 이상, 32주 이상 또는 36주 이상일 수 있으며, 이에 제한되는 것은 아니다.
본 발명에서 있어서, 상기 1일 1회 (QD)와 같은 투여용법은 새로운 안전성, 내약성 및 약동학 데이터를 고려하여 더 빈번한 투여가 적절하다고 판단되는 경우 1일 2회 (BID)로 변경될 수 있으나, 이에 제한되는 것은 아니다. 또한, 본 발명에 따른 상기 화학식 1로 표시되는 화합물의 용량은 QD 투여 후 안전하고 허용되는 것으로 확립된 AUC를 초과하지 않는 한, 새로운 용법의 일일 총 노출 (농도-시간 곡선하면적, AUC)을 고려하여 변경될 수 있다.
본 발명에 있어서, 상기 약학적 조성물은 말초 T 세포 림프종 (PTCL)을 진단받은 대상체, 구체적으로 말초 T 세포 림프종 (PTCL)을 진단받은 후 적어도 1회 이상의 항암화학요법을 받은 대상체에 투여되는 것일 수 있다. 상기 항암화학요법은 약물 (항암제)을 이용하여 암세포가 자라는 것을 막거나 죽이는 전신적인 치료 방법을 의미하고, 이는 수술, 방사선요법과 함께 암의 표준치료 방법이며, 안트라사이클린 기반의 화학요법 (anthracycline-based regimens), 리툭시맙 및/또는 니볼루맙 기반의 면역요법을 포함하나, 이에 제한되는 것은 아니다.
본 발명에서 "대상체"는 인간을 포함한 포유류를 의미할 수 있고, 구체적으로는 인간을 의미할 수 있다.
본 발명에 있어서, 상기 약학적 조성물은 경구 투여되거나 비경구 투여 (예를 들면, 정맥 내, 피하 내, 복강 내 또는 국소에 적용)될 수 있으며, 투여 경로에 따라 용량을 적절히 변경할 수 있다. 본 발명의 일 구현예에서는 상기 조성물은 경구 투여될 수 있다.
본 발명에서 용어 "경구 투여"는, 활성물질이 소화되도록 제조된 물질, 즉 흡수를 위한 위장기관으로 투여되는 것을 의미한다. 상기 경구 투여용 제제의 비제한적인 예로는, 정제, 트로키제 (troches), 로젠지 (lozenge), 수용성 현탁액, 유성 현탁액, 조제 분말, 과립, 에멀젼, 하드 캡슐, 소프트 캡슐, 시럽 또는 엘릭시르제 등을 들 수 있다. 본 발명의 약학적 조성물을 경구 투여용으로 제제화하기 위하여, 락토오스, 사카로오스, 솔비톨, 만니톨, 전분, 아밀로펙틴, 셀룰로오스 또는 젤라틴 등과 같은 결합제; 디칼슘 포스페이트 등과 같은 부형제; 옥수수 전분 또는 고구마 전분 등과 같은 붕해제; 스테아르산 마그네슘, 스테아르산 칼슘, 스테아릴 푸마르산 나트륨 또는 폴리에틸렌 글리콜 왁스 등과 같은 윤활유 등을 사용할 수 있으며, 감미제, 방향제, 시럽제 등도 사용할 수 있다. 나아가 캡슐제의 경우에는 상기 언급한 물질 외에도 지방유와 같은 액체 담체 등을 추가로 사용할 수 있다.
상기 약학적 조성물은 당업자에게 자명하게 경구용 또는 비경구용 제제 형태로 제조될 수 있다. 본 발명의 일 구현예에서 약학적 조성물은 경구 투여용 제제일 수 있다. 본 발명에 있어서, 상기 경구 투여를 위한 제제는 정제, 환제, 산제, 과립제, 캡슐제 등의 고형 제제이거나 또는 현탁제, 내용액제, 유제, 시럽제 등의 액상 제제일 수 있으며, 바람직하게는 고형 제제일 수 있으며, 보다 바람직하게는 캡슐제일 수 있다.
본 발명의 약학적 조성물이 경구 고형 제제로 제제화되는 경우, 사용되는 첨가제의 예로는 셀룰로오스, 규산칼슘, 옥수수전분, 락토오스, 수크로스, 덱스트로스, 칼슘 포스페이트, 스테아르산, 스테아르산 마그네슘, 스테아르산 칼슘, 젤라틴, 탈크 등을 들 수 있다.

본 발명의 약학적 조성물이 주사제의 형태로 제조되는 경우 상기 첨가제로는 물, 식염수, 포도당 수용액, 유사 당수용액, 알콜, 글리콜, 에테르(예: 폴리에틸렌글리콜 400), 오일, 지방산, 지방산에스테르, 글리세라이드 등을 들 수 있다.
본 발명에 있어서, 상기 약학적 조성물은 조절 T 세포 (regulatory T cell, Treg)를 감소시키고 CD8+ T 세포를 증가시킬 수 있다. 상기 약학적 조성물은 면역 반응을 억제하는 조절 T 세포를 감소시키고, 항암 효과를 나타내는 CD8+ T 세포를 증가시킴으로써, 진행성 혈액암에 대한 우수한 항암활성을 나타낼 수 있다. 본 발명의 일 구현예에서 상기 진행성 혈액암은 말초 T 세포 림프종 (PTCL)일 수 있다.
본 발명에 있어서, 상기 약학적 조성물은 상기 화학식 1로 표시되는 화합물 또는 이의 약학적으로 허용가능한 염 외에 다른 약리활성을 가지는 유효성분을 더 포함할 수 있다.
본 발명에 있어서, 상기 약학적 조성물은 약학적으로 허용 가능한 첨가제를 더 포함할 수 있다. 상기 "약학적으로 허용 가능한"이란 생리학적으로 허용되고 인간에게 투여될 때, 통상적으로 위장 장애, 현기증과 같은 알레르기 반응 또는 이와 유사한 반응을 일으키지 않고 이 분야의 통상의 지식을 가진 자가 의약 조성물 제조 시 통상적으로 사용하는 것으로, Remington's Pharmaceutical Science(최신판), Mack Publishing Company, Easton PA의 문헌을 참조할 수 있다.
상기 첨가제는 담체, 부형제, 증량제, 항산화제, 완충액, 충진제, 항응집제, 윤활제, 습윤제, 향료, 유화제, 현탁제, 계면활성제 및 방부제 등일 수 있다. 예를 들면, 상기 첨가제는 락토즈, 덱스트로즈, 규산칼슘, 옥수수전분, 수크로즈, 솔비톨, 만니톨, 자일리톨, 에리스리톨, 말티톨, 전분, 아카시아고무, 알지네이트, 젤라틴, 칼슘 포스페이트, 칼슘 실리케이트, 셀룰로오스, 메틸 셀룰로오스, 폴리비닐피롤리돈, 물, 메틸하이드록시벤조에이트, 프로필하이드록시벤조에이트, 탈크, 스테아르산, 스테아르산 마그네슘, 스테아르산 칼슘, 광물유, 식염수, 포도당 수용액, 유사 당수용액, 알콜, 글리콜, 에테르(예: 폴리에틸렌글리콜 400), 오일, 지방산, 지방산에스테르, 글리세라이드 또는 이들의 혼합물일 수 있다. 그러나 본 발명에 따른 조성물에 포함될 수 있는 첨가제는 상기 열거된 물질들로 한정되는 것은 아니며, 이들은 단지 예시에 불과하다. 본 발명에 있어서, 상기 약학적 조성물에 포함되는 첨가제의 함량은 특별히 한정되는 것은 아니며 통상의 제제화에 사용되는 함량 범위 내에서 적절하게 조절될 수 있다.
본 발명에 있어서, 용어 "예방"이란, 질병, 장애 또는 질환의 발병을 억제하거나 지연을 의미한다. 질병, 장애 또는 질환의 발병이 예정된 기간 동안 억제되거나 지연된 경우 예방은 완전한 것으로 간주될 수 있다.
본 발명에서 용어 "치료"란, 특정 질병, 장애 및/또는 질환 또는 질환에 따른 증상을을 부분적으로 또는 완전히 경감, 개선, 완화, 저해 또는 지연시키며, 중증도를 감소시키거나, 하나 이상의 증상 또는 특징의 발생을 감소시키는 것을 의미한다.
본 발명은 하기 화학식 1로 표시되는 화합물 또는 이의 약학적으로 허용 가능한 염을, 화학식 1로 표시되는 화합물로서 25mg 이상 325mg 미만으로 포함하는 약학적 조성물의 진행성 혈액암의 예방 또는 치료 용도를 제공한다. 본 발명의 일 구현예에서 상기 진행성 혈액암은 말초 T 세포 림프종 (PTCL)일 수 있다.
본 발명은 하기 화학식 1로 표시되는 화합물 또는 이의 약학적으로 허용 가능한 염을, 화학식 1로 표시되는 화합물로서 25mg 이상 325mg 미만으로 포함하는 약학적 조성물의 진행성 혈액암의 예방 또는 치료용 약제 제조를 위한 용도를 제공한다. 본 발명의 일 구현예에서 상기 진행성 혈액암은 말초 T 세포 림프종 (PTCL)일 수 있다.
본 발명은 하기 화학식 1로 표시되는 화합물 또는 이의 약학적으로 허용 가능한 염을, 화학식 1로 표시되는 화합물로서 25mg 이상 325mg 미만으로 포함하는 약학적 조성물을 대상체에 투여하여 진행성 혈액암을 예방 또는 치료하는 방법을 제공한다. 본 발명의 일 구현예에서 상기 진행성 혈액암은 말초 T 세포 림프종 (PTCL)일 수 있다.
본 발명의 약학적 조성물, 치료 방법, 용도에서 언급된 사항은 서로 모순되지 않는 한 동일하게 적용된다.
본 발명의 약학적 조성물은 말초 T 세포 림프종 치료에 있어서, 안전성 및 내약성이 우수하다.
또한, 본 발명의 약학적 조성물은 면역 반응을 억제하는 조절 T 세포를 감소시키고, 항암 효과를 나타내는 CD8+ T 세포를 증가시킴으로써, 말초 T 세포 림프종을 효과적으로 치료할 수 있다.

이하, 본 발명의 이해를 돕기 위하여 실시예를 들어 상세하게 설명하기로 한다. 다만 하기의 실시예는 본 발명의 내용을 예시하는 것일 뿐 본 발명의 범위가 하기 실시예에 한정되는 것은 아니다. 본 발명의 실시예는 당업계에서 평균적인 지식을 가진 자에게 본 발명을 보다 완전하게 설명하기 위해 제공되는 것이다.
실시예
1. 시험대상자 선정
본 임상시험 Ia 부분의 진행을 위해, 총 12명의 진행성 혈액암 (advanced hematologic malignancies)이 있는 시험대상자를 선정하였다.
위 시험대상자에는 말초 T세포 림프종 (PTCL) (8명), 미만성 거대 B 세포 림프종 (DLBCL) (2명), 변연부 림프종 (MZL) (1명) 및 피부 T세포 림프종/균상식 육종 (Composite CTCL-MF) (1명)의 병리학적 아형이 포함되었다.
위 시험대상자의 인구학적 기초자료 및 기저 특성은 다음과 같다:
- 남성 (9명) 및 여성 (3명)
- 중간 연령 (63세, 30세 - 76세 범위)
- ECOG PS (0-1)
- 모든 시험대상자는 이전에 1회 이상 화학요법을 경험
가. 선정기준 (Inclusion Criteria)
시험대상자는 다음 기준들을 모두 충족할 경우에만 시험 참여에 적합한 것으로 간주한다.
1) 서명된 사전 동의서를 제공할 수 있어야 한다. 여기에는 시험대상자 동의서 (ICF) 및 임상시험계획서에 제시된 요건 및 제한사항을 준수하는 것도 포함된다.
2) 만 18세 이상의 여성 또는 남성 시험대상자 (임상시험 참여에 대한 법적 동의 연령이 > 18세인 경우, 현지 규제 요건을 따른다.)
3) ECOG 전신수행상태 ≤ 2
4) 3개월을 초과하는 기대 여명
5) 제 Ia상 (용량증량 부분에 한정):
표준요법을 받았거나 표준요법에 대한 내약성이 없는 자 및/또는 표준요법이 존재하지 않는 시험대상자로서 세계보건기구 (WHO) 분류에 따른 재발성 및/또는 불응성 B세포 림프종, 만성 림프구성 백혈병 (CLL)/소 림프구성 백혈병 (SLL) 및 말초 T 세포 림프종 (PTCL)으로 진단받은 시험대상자
6) Ib상 (용량확장 부분에 한정):
표준요법을 받았거나 표준요법에 대한 내약성이 없는 자 또는 표준요법이 존재하지 않으며 다음의 군들 중 하나에 적합한 자로서 (세계보건기구 분류에 따른) 재발성 및/또는 불응성 림프종 진단을 받은 시험대상자:
- 그룹 A: MYC-변성 미만성 거대 B 세포 림프종 (MYC-altered DLBCL) 및 변형된 미만성 거대 B 세포 림프종 (transformed DLBCL)을 포함하는, 미만성 거대 B 세포 림프종 (DCBCL) 진단을 받은 시험대상자
- 그룹 B: 등급 1 내지 3a의 소포성 림프종 (follicular lymphoma)진단을 받은 시험대상자
- 그룹 C: CLL/SLL 및 그 외 B세포 림프종 (예를 들면, 외투세포림프종 (MCL), 변연부 림프종 (MZL), 발덴스트롬 마크로글로불린혈증 (Waldenstrom macroglobulinemia)또는 말초 T 세포 림프종 (PTCL)을 포함하여 이에 국한되지 않음) 진단을 받은 시험대상자
7) 제Ia상 및 제Ib상 (용량 확장)에 등록된 모든 시험대상자들은 해당하는 종양 유형 기준에 근거하여 평가 가능한 질병이 있어야 한다.
8) 시험자 평가에 따라, 현재 전신요법 필요한 시험대상자
9) 림프종 시험대상자의 경우 보관/신선 종양조직 (즉, 약 15개의 슬라이드 시리즈로 구성된 조직 블록 또는 슬라이드)이 필요하고, 상기 조직들은 스크리닝 방문 중에 제공하도록 한다. CLL 및 발덴스트롬 마크로글로불린혈증의 경우, 이용 가능한 종양조직이 없다면, 종양조직은 필요로 하지 않는다.
10) 제Ia상 및 제Ib상의 각 군에 속한 첫 7명의 시험대상자들은 임상시험 기간 동안 신선 종양생검을 받을 준비를 해야 한다. CLL 및 발덴스트롬 마크로글로불린혈증의 경우, 이는 의무가 아니다. 생검에 부적합한 질병 부위가 있는 시험대상자들에 대해서는 의뢰자와 논의 후 예외 사항들을 고려할 수 있다.
11) 여성 시험대상자들은 반드시 외과적으로 불임 상태이거나, 또는 외과적으로 불임인 단혼 파트너가 있거나, 또는 폐경 후 상태 (최소 1년 무월경 또는 혈청 난포자극호르몬 > 40 mIU/mL 및 에스트라디올 (estradiol) < 20 pg/mL 또는 관련 실험에 대한 "폐경 후 (postmenopausal)" 범위의 정의에 따른 상태)인 자, 또는 임상시험 기간 동안 및 마지막 시험약물 투여 후 3개월 동안 효과적인 피임법을 사용하기로 동의해야한다.
12) 성생활을 하는 남성은 시험약물을 복용기간 및 시험약물 투여를 중단한 후 3개월 동안 효과적인 형태의 피임법을 사용하기로 반드시 동의해야 한다. 정관 절제술을 받은 남성은 정액을 통한 약물 전달을 방지하기 위해 콘돔을 사용해야한다.
13) 다음과 같이 정의된 실험실 검사 수치를 나타내는 자:
- 크레아티닌 청소율 (콕크로프트-골트 공식을 사용하여 계산되거나 측정됨) > 60 mL/분
- 총 빌리루빈 < 1.5 X ULN (upper limit of normal, 정상치상한)(단, 총 빌리루빈 > 3.0 X ULN 또는 직접 빌리루빈 < 1.5 X ULN인 경우에 제외되는 길버트 증후군(Gilbert's syndrome) 시험대상자는 제외)
- ALT < 2.5 X ULN (단, ALT < 5 X ULN인 경우에 시험대상자로 선정되는 자로서 간에 침범한 종양이 있는 시험대상자들은 제외)
- 절대 호중구 수 (ANC) > 1.0 X 109/L
- 혈소판 수 > 75 X 109/L
- 헤모글로빈 > 8 g/dL
나. 제외기준 (Exclusion Criteria)
다음 기준 중 하나라도 해당하는 시험 대상자는 본 임상시험에서 제외된다.
1) 명백한 연수막 전이 또는 활성 중추신경계 (CNS) 전이의 존재, 또는 국부적인 CNS 표적 요법 (예: 방사선 요법 또는 수술) 또는 이전 2주 이내에 코르티코스테로이드의 용량 증가를 요하는 CNS 전이. 치료를 받은 뇌 전이 시험대상자들은 신경학적으로 안정된 상태여야 하고 (투여 후 4주 동안 및 임상시험 등록 전), 시험약물을 투여하기 전 적어도 2주 이상 스테로이드 투여를 중단해야 한다.
2) 다음 중 해당하는 사항을 포함하는, 심기능 장애 또는 임상적으로 유의한 심질환:
- 요법이 필요한 울혈성 심부전 (뉴욕심장학회분류 ≥ 2등급); 다중 게이트 획득 (MUGA) 스캔 또는 심초음파 (ECHO) 검사를 통해 판정된 좌심실 박출률 < 50 %; 비조절성 고혈압; 또는 임상적으로 유의한 부정맥
- 프리데리시아공식 (Fridericia's formula)을 사용한 심박수 보정 QT 간격 (QTcF) > 450ms ECG 또는 스크리닝 방문시 선천성의 긴 QT 증후군 (congenital long QT syndrome). 만약 좌각차단 (LBBB, Left Bundle Branch Block)과 같은 기저 질환으로 인해 QTcF를 적절하게 계산할 수 없는 경우, 시험대상자의 적합성은 의학 모니터 요원 및 의뢰자와 논의하고, 적합성 결정은 적절히 문서화한다.
- QT/QTc 간격의 현저한 베이스라인 연장 (예: QTc 간격 > 450ms가 반복적으로 시연됨)
- QT/QTc 간격을 연장하는 병용 약물의 의무적인 사용
- 급성 심근경색 (Acute myocardial infarction). 또는 임상시험 등록 전 3개월 미만의 불안정 협심증 (unstable angina pectoris)
3) 간질성 폐렴 (interstitial pneumonia) 또는 약물 유도 간질 폐렴 (drug-induced interstitial pneumonia)/간질성 폐렴 (pneumonitis)의 병력이 있는 시험대상자
4) 인체면역결핍바이러스(HIV, Human immunodeficiency virus) 감염
5) B형 간염 표면 항원 (HbsAg) 또는 C형 간염 바이러스 항체에 양성인 시험대상자. HbsAg가 음성이며 다만 항 B형 간염 핵심항체가 양성인 경우, 시험 참여에 적합하려면 B형 간염 바이러스-데옥시리보핵산 (DNA) 바이러스량이 검출되어서는 안된다.
6) 만성 간 질환 또는 만성 간염 (Child-Pugh B급 또는 C급의 간 장애) 시험대상자
7) 시험약물의 흡수를 방해하거나 정제 또는 캡슐을 삼킬 수 없는 모든 위장관 질환
8) 본 임상시험에서 투여 대상의 질환에 속하지 않는 그 밖의 악성질환. 이러한 제외조건에 대한 예외 사항에는 다음이 포함된다:
- 시험약물 투여 전 2년 이내에 치료된 후 재발하지 않은 악성종양
- 수술을 통해 완전 절제된 기저 세포 및 편평세포 피부암
- 지연형 질환으로 간주되고, 요법을 필요로 하지 않는 악성종양
- 모든 유형의 완전 절제된 상피내암종 (carcinoma in situ)
9) 이전 PI3K 저해제는 본 시험의 용량 증량 부분 (Ia)에서만 허용된다.
10) 림프종 시험대상자의 경우:
- 임상시험용 의약품의 첫번째 용량을 투여하기 전 3주 또는 반감기의 5배에 해당하는 기간(둘 중 비교적 짧은 기간을 기준) 내에 실시한 전신 항종양 요법 (세포독성 화학요법, α-인터페론 (INF) 및 독소 면역접합체 포함) 또는 모든 실험 요법
- 시험약물의 첫번째 용량을 투여하기 전, 반감기의 5배에 해당하는 기간 이내에 티로신 키나아제 저해제를 사용한 요법
- 시험약물의 첫번째 용량을 투여하기 < 6주 전에 비접합 단일클론항체 요법 실시
11) 전신적인 만성 스테로이드 요법 또는 면역억제요법 (> 10 mg/일 용량의 프레드니손 또는 해당량의 요법)을 받는 시험대상자, 국소, 흡입, 비강 및 안과용 스테로이드는 허용된다.
12) 시험약물 투여 개시 후 4주 이내에 감염성 질환에 대한 생백신의 사용.
13) 시험약물 투여 시작일을 기준으로 2주 이내에 조혈 집락 자극 성장 인자 (예: 과립구 집락 자극인자 (GCSF), 과립구-대식세포 집락 자극인자 (MCSF), 대식세포 집락 자극인자 (MCSF)), 트롬보포이에틴 유사약 (thrombopoietin mimetics) 또는 적혈구 자극제 (erythroid-stimulating agents)의 사용. 만약 시험약물의 첫번째 용량 투여일을 기준으로 2주 이상전부터 적혈구 자극제 투여를 시작했으며, 해당 시험대상자가 안정 용량으로 투여 중에 있다면 동 자극제를 계속 사용할 수 있다.
14) 뇌졸중의 병력이 있거나 활성 신경학적 증상이 있는 시험대상자 (단, 신경과 전문의, 연구자, 의뢰자의 소견에 따라, 시험약물 투여를 받는 동안 진행 중인 신경학적 평가에 영향을 미치지 않을 것으로 판단되는 만성질환은 제외).
15) 전신 요법 또는 항바이러스 항생제 요법을 요하는 활성 감염
16) 활성 거대세포바이러스 (CMV; Cytomegalovirus) 감염 시험대상자
17) 중등도 (moderate) 또는 고강도의 CYP3A4 저해제 또는 유도제를 복용한 시험대상자로서, 시험약물의 첫번째 용량 투여일을 기준으로 2주 전까지 또는 반감기의 5배 이상에 해당하는 기간 (둘 중 비교적 짧은 기간을 기준)이 도래할 때까지 시험 참여를 철회할 수 없는 시험대상자
18) 시험약물의 첫번째 용량을 투여일을 기준으로 2주 이내에 실시한 대수술 (종격내시경술(mediastinoscopy), 중심 정맥 접근 장치 (central venous access device)의 삽입, 및 영양관 삽입은 대수술로 간주되지 않음)
19) 시험약물의 첫번째 용량을 투여한 날로부터 2주 이내에 실시한 방사선 요법 (뼈 통증 또는 국소 통증성 종괴에 대한 요법과 같이 제한된 범위의 고식적 방사선 요법은 제외).
20) 이전 항암요법으로 인해 2등급 이상의 독성에 해당하는 CTCAE의 존재 (3등급 이상의 CTCAE가 존재할 경우에 제외되는 탈모증, 말초 신경병증 및 내이신경독성 (耳毒性, ototoxicity)은 예외로 함)
21) 시험약물의 첫번째 용량을 투여한 날로부터 2주 또는 반감기의 5배에 해당하는 기간(둘 중 비교적 짧은 기간을 기준) 이내에 실시되는 중재적, 탐색적 연구에 참여
22) 시험자의 소견으로 볼 때 안전 문제, 임상시험 절차의 준수 또는 시험결과의 해석으로 인해 시험대상자의 임상시험 참여를 방해하는 것으로 판단되는 모든 의학적 상태
23) 수태 후 임신 기간이 끝날 때까지 여성의 상태로 정의되며 인간 융모막 성선 자극 호르몬 실험실 검사에서 양성으로 확인된 임신을 경험한 임부 또는 수유부
2. 임상시험 설계
가. 시험약물
시험약물은 4-[[(1S) -1-(4,8-디클로로-1-옥소-2-페닐-3-이소퀴놀릴)에틸]아미노]-8H-피리도[2,3-d]피리딘-5-온 {4-[[(1S) -1-(4,8-Dichloro-1-oxo-2-phenyl-3-isoquinolyl)ethyl]amino]-8H-pyrido[2,3-d]pyrimidin-5-one}이다 (표 1).
나. 투여용량 및 투여용법
등록된 임상시험 대상자 총 12명은 시험약물을 28일 주기 동안 1일 1회 경구 투여하였다 (공개, 다기관) (표 2).
본 임상시험에서 시험약물의 초기 용량 (50 mg QD)은 동물을 대상으로 한 전임상시험결과 (안전성, 내약성 및 약동학 데이터)와 임상에서 PI3K-δ 저해제와 유사한 기전을 가지는 약물로부터 얻어진 독성 및 효능에 기반하여 결정되었다.
본 임상시험에서 사용된 투여용법은 새로운 안전성, 내약성 및 약동학 데이터를 고려하여 BID와 같이 더 빈번한 투여가 적절하다고 판단되는 경우 투여용법을 변경할 수 있다. 또한, 본 임상시험의 투여용량은 QD 용량 투여 후 안전하고 허용되는 것으로 확립된 AUC를 초과하지 않는 한, 새로운 용법의 일일 총 노출 (농도-시간 곡선하면적, AUC)을 고려하여 변경될 수 있다 (표 3).
시험약물 투여 시 주의사항은 다음과 같다:
1) 시험대상자들은 시험자의 지시에 따라 식사 시점으로부터 적어도 2시간 전에 또는 식후 2시간 경과 시 시험약물을 복용해야한다.
2) 투여 중에 구토할 경우, 다음 예정된 용량의 시험약을 복용하기 전에 재투약을 해서는 안된다.
3) QD 요법에서 시험대상자가 해당 용량의 복용을 잊어버리고 통상적인 투여 시점으로부터 12시간 이내에 누락된 용량을 복용할 수 있다면, 그러한 용량분을 복용하는 것이 허용된다. 그러한 재시도가 여의치 않을 경우, 당일에 누락한 해당 용량분의 투여를 보류하고 다음 예정일에 투약을 재개하도록 한다. BID 요법의 경우, 해당 용량은 적용 가능하다면 통상적인 투여 시점으로부터 6시간 이내에만 복용할 수 있다.
4) PK 프로파일 분석을 위한 연속 채혈을 실시하는 날 (제1주기의 1일차 및 15일차)에 시험대상자들은 시험자의 지시에 따라 시험약물을 공복 상태에서 복용 (자정 이후에는 음식이나 물을 섭취하지 말 것)해야 하고, 시험약물을 투여한 후 2시간 동안 식사를 금해야 한다. 이 시간이 끝나면 음식물을 섭취해도 된다.
5) PET 스캔을 실시하는 날에 시험대상자는 PET 스캔 시점을 기준으로 6시간 전부터 금식해야 하며 다만 필요에 따라 물을 얼마든지 음용할 수는 있다. 스캔 검사 당일에 시험약물 용량을 계획할 경우, 다음 요건들이 적용된다.
- 스캔 검사 시간이 오전에 예정된 경우, 시험약물은 물과 함께 복용할 수 있으며 다만 시험대상자는 아침 식사를 해서는 안 된다 (대체로 시험자의 지시에 따름).
- PET 스캔이 오후에 예정된 경우, 시험대상자는 PET 검사 예약 시점으로부터 4시간 (또는 그 이상) 전에 간단한 아침 식사를 할 수 있으며 아침 식사 후 4시간 이내에 시험약물을 복용해도 된다. 복용 후 PET 스캔이 완료될 때까지 시험대상자는 계속 금식해야 한다.
3. 전체 설계
본 제I상 임상시험은 2개의 시험 부분 (Ia상 및 Ib상)으로 구성된 다기관, 공개, 최초의 인체 대상 (FIH) 시험이다.
이 시험의 Ia상 부분 (용량 증량)은 재발성/불응성 B세포 림프종, 만성 림프구성 백혈병(CLL)/소림프구백혈병(SLL) 및 말초 T세포 림프종(PTCL)이 있는 시험대상자에서 시험약물의 안전성, 내약성, 최대내약용량 (MTD)/임상 Ib/II에 대한 시작용량(RP2D)을 결정할 수 있도록 설계되었다.
이 시험의 1b상 부분 (용량 확장)은 Ia상에서 확인된 용량으로 시험약물을 투여 시 진행성이 있는 특정 재발성/불응성 B세포 림프종, 만성 림프구성 백혈병(CLL)/소림프구백혈병(SLL) 및 말초 T세포 림프종 (PTCL)에서 종양 반응 및 안전성을 평가하도록 설계되었다. Ia상 (용량 증량)에서 확장용 권장 용량 (RDE)이 결정되면 1b상 부분 (용량 확장)이 시작된다.
전체 임상시험에서 등록된 각 시험대상자에 대해 예정된 총 시험 기간은 약 10개월이고, 질병 진행이 없는 시험대상자들의 경우 시험 기간을 연장할 수 있으며, 전체 임상시험은 스크리닝 기간, 시험약물 투여기간 및 추적기간으로 구성된다. 각 기간에 관한 구체 내용은 다음과 같다.
가. 스크리닝 기간
임상시험과 관련된 절차 또는 평가에 앞서 시험대상자 동의서를 획득하였다. 모든 잠재적인 시험대상자들은 시험대상자 동의서에 서명한 후 스크리닝을 받았다 (-21일 ~ -1일).
나. 시험약물 투여 기간
1) 투여 기간
시험약물 투여 기간은 제1주기의 제1일에 시험약물의 첫 번째 용량을 투여하는 것으로 시작되었고, 캡슐 형태로 경구 투여되는 시험약물 (QD 또는 안전성 모니터링 위원회가 정한 그 밖의 요법)이 연속 28일 동안 투여되었다. 여기서, 1주기는 상기 28일로 구성된다. 임상시험에 참여하는 대상자들은 질병이 진행될 때까지 시험약물의 투여를 계속 받을 수 있고, 상기 시험약물의 투여 중단은 임상시험지침 및 시험자의 판단에 따른다.
2) 시험약물의 용량 증량 방법
치료 기간 중 (Ia상 부분) 용량 증량 방법은 다음과 같다. 코호트 1 및 2에는 최소 1명 이상, 최대 6명의 시험대상자가 등록될 수 있다. 후속 코호트에는 3명 내지 6명의 시험대상자가 등록될 수 있다. 각 코호트의 첫 번째 임상시험 대상자는 해당 코호트에 속한 다음 2명의 임상시험 대상자에게 임상시험 약물이 투여되기 전에 7일 동안 치료되고 관찰된다.
(1) 코호트 1에 처음 등록된 임상시험 대상자가 시험약물과 관련된 등급 1을 초과 (등급 2 이상)의 이상반응을 경험하는 경우, 다른 임상시험 대상자가 해당 코호트에 추가로 등록되고 모든 후속 코호트에서는 최소 3명의 임상시험 대상자들이 등록된다. 코호트 1의 시험절차가 단 1명의 시험대상자로 완료될 경우 용량 증량이 진행되며, 코호트 2는 1명의 시험대상자로 시작할 수 있다. 그 다음 코호트 2에 처음 등록된 시험대상자가 시험약물과 관련된 이상반응을 경험하는 경우, 해당 코호트에 시험대상자가 추가로 등록되고 모든 후속 코호트에는 최소 3명의 임상시험 대상자들이 포함된다 (도 2).
(2) 코호트 3 및 모든 후속 코호트들은 표준 3 + 3 디자인 (standard 3 + 3 design)을 따른다. 3명의 시험대상자를 등록하는 코호트의 경우, 첫 3명의 시험대상자들에게서 용량제한독성 (DLT, dose limiting toxicity)이 없다면 안전성 모니터링 위원회의 권고에 따라 그 다음으로 높은 용량 수준으로 용량 증량을 진행할 수 있다. 시험대상자 3명 중 1명이 용량제한독성 (DLT)을 경험할 경우, 해당 용량 수준을 6명의 시험대상자로 확장한다 (시험대상자 3명 추가 등록할 것). 주어진 용량 수준에서 시험대상자 2명이 용량제한독성 (DLT)을 경험할 경우, 최대내약용량 (MTD, maximum tolerated dose)을 초과한 것으로 간주하고, 해당 코호트에 대한 투여 확대는 종료된다. 다음으로 낮은 용량 수준을 6명의 시험대상자로 확장하거나, 이미 6명의 시험대상자가 등록되었고 6명 중 1명 이하가 용량제한독성 (DLT)을 경험한 경우 상기 낮은 용량 수준을 최대내약용량 (MTD)으로 간주한다 (도 3).
(3) 용량제한독성 (DLT)을 경험하는 임상시험 대상자는 용량제한독성 (DLT)이 소실되거나, 감소된 용량을 시작하기 전 용량제한독성 (DLT) 등급 또는 등급 1 (두가지 중 더 큰 등급 기준)로 감소한 경우 용량제한독성 (DLT)을 경험 중인 시험대상자들은 감소한 용량을 계속 투여할 수 있다. 감소된 용량을 투여한 임상시험 대상자가 다른 용량제한독성 (DLT)을 경험하는 경우, 상기 시험 대상자는 더 이상의 용량을 투여받지 않아야 한다.
(4) 제1주기에서 시험 참여를 중단한 자로서, 다음과 같이 총 28일의 투약 예정 일수 중 실제 투약 일수가 20일 미만인 시험대상자들은 조기 탈락자로 간주한다.
① 명목상 이상반응과 관련이 없거나 독성과 무관한 원인으로 인해 시험 참여를 중단한 시험 대상자;
② 이상반응 또는 독성 이외의 원인으로 인해 시험 참여를 중단된 시험 대상자
(5) 조기 탈락자가 발생하면 모든 코호트에서 새로운 시험 대상자를 추가로 등록하게 되고, 이러한 추가 등록은 시험팀, 의뢰자 및 시험자와의 논의를 거쳐 결정하도록 하며, 추가 등록 절차는 아래와 같다.
① 조기 탈락자가 단일 시험대상자에 대해 계획된 코호트 (코호트 1 또는 2)에서 발생하면, 단일 시험대상자 코호트를 표준 3 + 3 시험설계로 진행한다. 따라서 해당 코호트는 시험설계에 따라 최소 4명의 시험 대상자로 확장된다. 후속 코호트에서는 표준 3 + 3 시험설계에 따라 최소 3명의 시험 대상자를 등록한다. 이러한 군에서 1건의 용량제한독성 (DLT)이 발생할 경우, 상기의 표준 3 + 3 시험설계에 관한 설명과 같이, 3명의 시험대상자를 추가로 등록하여 용량 확장을 실시한다.
② 조기 탈락자가 시험 대상자 3명에 대해 계획된 어떤 코호트의 일부에 속하는 경우, 이 코호트는 시험 대상자 4명으로 확장된다. 이러한 군에서 1건의 용량 제한 독성이 발생할 경우, 표준 3 + 3 시험설계의 경우와 같이, 3명의 시험 대상자를 추가로 등록하여 용량 확장을 실시한다.
③ 조기 탈락자가 발생하고, 안전성 모니터링 위원회가 해당 코호트에 시험 대상자를 추가할 것을 권고하지 않을 경우, 조기 탈락자는 이러한 용량 증량 결정 절차에서 용량제한독성 (DLT)을 경험한 것으로 간주된다.
(6) 위 용량제한독성 (DLT)은 미국 국립암연구소 (National Cancer Institute)의 이상반응에 대한 공통 용어 기준 (CTCAE, Common Terminology Criteria for Adverse Events) 3등급 이상에 해당하며 질병, 질병진행, 병발 질환 (intercurrent illness) 또는 병용약물과 관련 없는 것으로 시험자가 평가한 반응 또는 이상으로서, 본 시험의 Ia상 부분 (용량 증량)을 진행하는 동안 시험약물의 첫 투여 주기 내에서 발생하는 이상반응 또는 실험실 검사 수치 이상으로 정의된다. 본 임상시험에서 용량제한독성 (DLT)은 다음을 포함할 수 있다.
① 모든 5등급 독성
② 혈액학적 용량제한독성
(i) 5일 이하로 지속되는 4등급의 호중구감소증 (neutropenia), 림프구감소증 (lymphopenia), 또는 모든 등급의 백혈구감소증을 제외한 4등급 이상의 모든 혈액학적 독성
(ii) 3등급 이상의 발열성 호중구감소증 (febrile neutropenia)(ANC <1000/mm3 및 단일 체온 > 38.3°C (101°F) 또는 1시간 넘게 체온 ≥ 38°C (100.4°F)로 지속).
(iii) 감염을 동반한 3등급 호중구감소증
(iv) 출혈을 동반한 3등급 혈소판감소증
③ 비혈액학적 용량제한독성
(i) 용량제한독성 기간 중 다음을 제외한 3등급 이상의 모든 독성
- 최적요법 이후 3일 미만의 기간동안 3등급의 설사
- 최적요법을 진행 중인 3일 미만의 기간 동안 3등급의 메스꺼움 및 구토
- 충분한 지지요법을 시작한 후 7일 미만의 기간동안 3등급의 피로
- 의학적 관리 후 7일 이내에 1등급 이하로 소실되는 무증상의 3등급 간기능 검사 수치 증가 (알라닌 아미노전이효소 [ALT], 아스파라테이트 아미노전이효소 [AST] 또는 감마 글루타밀전이효소 [GGT])
- 시험요법과 관련성이 없고, 임상적 상관관계가 없으며, 충분한 의학적 관리를 통해 7일 이내에 1등급 이하로 소실되는 정상 외 범위의 기타 모든 실험실 검사 수치
(ii) 박리성 피부염 (dermatitis exfoliative), 홍반성 발진 (rash erythematous), 전신 발진 (rash generalized), 반점성 발진 (rash macular), 구진성 발진 (rash papular), 박탈성 발진 (exfoliative rash) 및 피부 질환을 포함하는 3등급 이상의 피부 반응
(iii) 의학적 관리 후 7일 이내에 소실되지 않는 2등급의 간질성 폐렴
④ 안전성 모니터링 위원회의 검토 후 용량제한독성으로 간주될 수도 있는 임상적으로 유의하거나 지속적인 모든 독성
다. 추적관찰기간
추적관찰기간은 시험약물의 마지막 용량을 투여한 후 30±2일차에 진행되는 안전성 추적관찰 방문, 후속 추적관찰 방문 (필요시) 및 시험 종료 (EOS) 방문으로 구성된다. 투여를 중단한 시험대상자들은 독성이 소실되거나 질병 진행이 확인될 때까지 추적관찰을 받아야 한다.
4. 평가 방법
가. 시험 평가 및 절차
시험 평가, 절차 및 시기는 표 4에 요약된 바와 같고, 여러 평가 일정이 동시에 예정된 경우, 평가는 심전도 (ECG) 및 활력 징후 평가 후, 채혈 검사 (PK 검체 채취 후 PD 검체 채취 및 안전성 실험 검체 채취) 순으로 진행된다.
나. 1차 평가 변수 ... 안전성 및 내약성의 평가
1) 이상반응, 활력 징후, 심전도 (ECG; electrocardiogram), 임상 실험실 검사, 신체검사, 마지막 안전성 추적관찰 방문까지의 ECOG PS 및 베이스라인의 의학적 상태에 따라 평가
2) 내약성 평가에는 투여중단, 용량감소 및 마지막 안전성 추적관찰 방문까지 투여 용량에 대한 검토가 포함된다.
3) 치료기간 제1주기 동안 용량제한독성 (DLT)을 기반으로 최대내약용량 (MTD), 임상 Ib/II에 대한 시작용량 (RP2D)이 결정된다.
다. 2차 평가 변수 ... 혈장 및 소변 PK 특성 규명 및 예비 항종양 활성 평가
1) 시험약물의 혈장 내 농도-시간 프로파일 (제1일 및 제15일), 소변 농도 데이터 (제15일) 및 단회투여 시 도출된 PK 매개변수 (혈장) 및 반복투여 시 도출된 PK 매개변수 (혈장 및 소변) (해당되는 경우)
- 시험약물의 혈장 내 농도를 측정하기 위해, 미리 정해진 일정에 따라 혈액 샘플을 수집한다 (표 5). Ia상 임상시험에서, 용량 투여 후 시작하여 미리 정해진 일정에 따라 각 투여 간격에 걸쳐 전체 소변을 수집해야 한다 (표 5).
- 혈장 및 소변에서 시험약물을 측정하기 위한 샘플은 적절하게 검증된 생체시료분석법을 사용하여 분석한다.
- 약동학 매개변수는 Phoenix® WinNonlin® Version 8.0 또는 이후 버전 (Certara, L.P. Princeton, New Jersey, USA) 및/또는 SAS® Version 9.2 또는 이후 버전 (SAS Institute, Inc., Cary, North Carolina, USA)을 통해 비구획적 방법을 사용하여 도출한다. 투여 후 실제 경과 시간은 최종 혈장 PK 매개변수 계산에 사용된다. 모든 매개변수 계산에는 최소 3개 이상의 투여 후 정량 농도 측정값이 필요하다.
- 치료 제1주기 제1일차 및 제1주기 제15일차에 혈장 내 시험약물 농도에 대하여 계산할 약동학 매개변수는 계산 가능한 경우 표 6에 제시된 매개변수를 포함하나, 이에 국한되는 것은 아니며, 적절하다고 판단되는 경우 추가적인 약동학 매개변수를 계산할 수 있다.
- 제1주기 제15일차의 소변 내 시험약물 농도에 대하여 계산할 PK 매개변수는 계산이 가능한 경우 다음 제시된 매개변수를 포함하되 이에 국한되지는 않는다 (표 7).
라. 탐색적 평가 변수 ... 약력학 (PD) 특성 규명 및 생체표지자 조사
1) 전신 사이토카인 및 케모카인의 절대값 및 베이스라인으로부터의 변화; 활성화된 면역세포의 표현형 특성화; 종양 생검 평가의 베이스라인으로부터의 변화; 및 시험약물에 노출 후의 기타 약력학 (PD) 생체표지자
- 탐색적 약력학 (PD) 및 생체표지자 (면역원성) 데이터에는 다음에 대한 분석이 포함될 수 있다.
(i) 전신 사이토카인 및 케모카인 (혈청): CCL3, CCL4, INF-γ, 인터루킨(IL)-6, IL-10, 종양괴사인자(TNF)-α
(ii) 전혈 약력학(PD) 생체표지자:
조절성 T 세포 - 프로그램화 세포 사멸 단백질 1(PD 1), 세포독성 T 세포 림프구 관련 단백질(CTLA)-4, 림프구 활성화 유전자(LAG)-3
① 분화 집단(CD)8 - PD 1, CTLA-4, LAG-3, T 세포 면역글로불린(Ig) 및 뮤신 영역 포함-3, Ig 포함 T 세포 면역수용체 및 면역수용체 티로신 기반 억제 모티프 영역, Ki67.
② 골수유도 억제세포
(iii) 종양 생검: C-MYC(IHC, FISH), rH2AX, pAKT (T308, S473)
- 짝을 이루는 종양 생검 (paired tumor biopsy)은 약력학 (PD) 생체표지자를 탐색하기 위해 계획된 일정에 따라, Ia상 (용량 증량)에서 코호트별 비액형 종양 시험대상자 1 내지 2명 및 Ia (용량 확장)에서 획득한다. 유사한 정보는 액형 종양에 대한 표준 종양 평가에서 얻을 수 있다. 림프종 시험대상자들의 경우, 포르말린 고정, 파라핀 포매 (FFPE) 조직 블록 또는 최소 15개의 염색되지 않은 슬라이드를 제출한다. 연구 선택은 충분하고 적합한 생물 검체 표본의 가용 여부에 따라 달라지고, 실험실 설명서에서 정의된다.
- 탐색적 생체표지자 분석은 잔여 종양 및/또는 혈액 검체 (PK 검체 포함)에서 진행할 수도 있다. 이들 시험에서는 시험약물의 효과를 조사하고 생체표지자의 변화가 시험약 노출 및 임상 결과와 어떤 관계를 나타낼 수 있는지를 판단하기 위해 잠재적 관련성이 있는 그 밖의 생체표지자들에 대한 검색을 확장한다. 이러한 추가 조사는 임상 결과, 시약 및 검체 가용 여부에 따라 달라진다
2) 시험약물 대사 산물의 프로파일링
마. 안전성 평가
1) 안전성 평가항목
모든 안전성 평가는 예정된 시점에 표 4에 따라 진행되고, 방법은 다음과 같다.
(1) 인구통계 정보 및 병력
인구통계 정보 및 관련 의학적/수술 병력은 전자 증례기록서 (eCRF, electronic case report form)에 기록된다.
(2) 신체검사
스크리닝 및 제1주기 1일차에 시험약물을 투여하기에 앞서 종합 신체검사가 진행되고, 이 검사에는 전체적 외양, 피부, 목 (갑상선 포함), 눈, 귀, 코, 인후, 폐, 심장, 복부, 등, 림프절, 사지 및 신경계에 대한 검사가 포함된다. 의학적 병력 및/또는 증상에 따른 적응증이 나타날 경우, 직장, 외부 생식기, 유방 및 골반 부위에 대한 검사를 진행한다. 이후의 평가에서는 전체적 외양 및 신체 부위에 대한 증상별 검사로 구성된 약식 신체검사를 실시하게 된다.
(3) 활력징후
활력 징후 (체온, 맥박, 호흡수 및 혈압)의 측정은 반듯이 누운 자세 또는 앉은 자세에서 적어도 5분 이상 휴식을 취한 후에 시험 진행 중 반듯이 누운 자세 또는 앉은 자세로 진행한다. 해당 자세는 eCRF에 기록한다. 활력 징후 측정 데이터는 동시에 예정된 채혈 (예: PK 또는 임상 실험실 검체 채취)에 앞서 수집된다. 활력 징후 측정은 의학적으로 필요하다면 시험자의 재량에 따라 시험 진행 중 언제든지 실시할 수 있다.
(4) 키 및 체중
키 (스크리닝 시에만 해당) 및 체중은 시험기관의 표준 절차에 따라 측정한다. 스크리닝 측정을 위해 체질량 지수(BMI: 체중[kg]/키2[mXm])를 계산한다.
(5) 전신수행상태
미국 동부종양학협력그룹 전신수행상태 (ECOG PS, Eastern Cooperative Oncology Group performance status)는 다음에 제시하였다 (표 8).
(6) 심전도
적어도 5분 이상 휴식을 취한 후에 심박수를 자동으로 계산하고 PR, QRS, QT 및 QTc 간격을 측정하는 ECG 기계를 사용하여 활동 일정 (표 4)에 및 표 9에 각각 제시된 측정 시기에 12 유도 심전도 검사를 중복하여 실시하고, 다음과 같은 매개변수들을 기록한다: 심박수, PR 간격, QRS 지속시간, QT 및 QTcF
각 시점에서 2회의 개별 ECG 추적 결과를 2분을 넘지 않게 최대한 근접한 시간 간격 내에서 연속으로 확보하도록 한다. 중복 측정은 5분 이내에 완료하도록 한다. 1차 임상시험 결과보고서 (CSR, Clinical study report) 마감일 (컷오프)에 도달한 후에는 EOT 평가로 심전도 (ECG)를 측정할 필요가 없다. 시험자의 재량에 따라 안전상의 이유로 심전도 검사를 추가로 진행할 수 있다.
(7) 심장 영상
좌심실 심장 기능은 스크리닝/베이스라인에서 ECHO 또는 MUGA 스캔을 통해 평가한다. 임상적 징후 또는 증상에 따라 필요하다면 심장 영상검사를 추가로 실시할 수 있다. 추가 영상검사도 동일한 방식으로 진행해야 한다.
(8) 임상적 안전성 실험실 평가
수행해야 할 임상 실험실 검사의 목록은 다음에 제시하였다 (표 10). 검사 시기 및 빈도는 표 4를 참조한다.
시험 참여 동안 및 시험 요법의 마지막 투여 후 30일 이내에 임상적으로 유의한 비정상으로 간주하는 수치를 보이는 모든 실험실 검사는 이러한 수치가 정상 수치 또는 베이스라인 수치로 돌아갈 때까지 또는 시험자나 의학 모니터 요원이 더이상 임상적으로 유의한 것으로 간주하지 않을 때까지 반복 검사해야 한다. 시험약물 투여를 실시하는 날에는 이들 매개변수에 대한 검체를 용량 투여에 앞서 채취한다. 의학적으로 필요하다면 시험자의 재량에 따라 추가적인 평가를 실시할 수 있다. 또한 시험대상자가 생체표지자 분석을 위해 생검용 짝을 이루는 종양 (paired tumor) 검체를 제공했다면 실험실 평가를 진행할 수 있다.
스크리닝 시 혈청 임신검사는 첫 번째 용량 투여일에 앞서 3일 이내에 실시해야 한다. 혈청 임신검사는 시험 기간 동안 (제2주기부터 시작되는 각 주기의 제1일) 그리고 EOT, 추적관찰 방문 및 EOS 시점에 반드시 실시해야 한다. 시험대상자 (여성)가 임신할 경우, 시험약 투여를 반드시 즉시 중단해야 한다. 임신검사 결과는 양성이지만 해당 시험대상자 (여성)가 임신하지 않은 것으로 판단될 경우, 해당 검사 결과가 양성으로 잘못 판정되었으며 임신이 배제된 것으로 확인될 때까지 시험약 투여를 보류해야 한다.
2) 이상반응
(1) 이상반응 및 중대한 이상반응의 정의
본 임상시험에서 정의된 이상반응 (AE, Adverse event)이란, 시험 요법과 관련된 것으로 간주하는지 여부와 관계없이 시간적으로 시험 요법의 사용과 관련이 있는 사건으로서 시험대상자에서 발생한 바람직하지 않은 의학적 사건을 말한다 (표 11).
본 임상시험에서 어떤 사건이 위에서 정의한 이상반응에 해당하지 않는 경우, 중대한 상태에 부합하더라도 중대한 이상반응 (SAE, Serious adverse event)으로 간주할 수 없다 (예: 시험 대상인 질병의 징후/증상으로 인한 입원 또는 질병 진행으로 인한 사망). 중대한 이상반응의 정의는 다음에 요약하였다 (표 12).
또한, 본 임상시험의 중증도 평가 기준은 다음에서 요약되었다 (표 13).
- 모든 이상반응 및 중대한 이상반응은 시험대상자 동의서 (ICF)에 서명한 시점부터 계획된 일정에 명시된 각 시점에서 30일차 안전성 추적관찰을 실시할 때까지 수집된다.
- 이상반응 및 중대한 이상반응의 확인 시 편향이 발생하지 않도록 주의해야하고, 시험대상자에게 유도 질문이 아닌 개방형 질문을 하여 이상반응에 관해 질문하는 것이 선호된다.
- 여성 시험대상자 및 남성 시험대상자의 여성 파트너에게서 발생한 모든 임신에 대한 세부 정보는 시험약 투여를 시작할 때부터 마지막 투여 후 3개월까지 수집한다. 임신 결과 이상 (예: 자연 유산, 태아 사망, 사산, 선천성 기형 및 자궁 외 임신)은 중대한 이상반응으로 간주한다.
바. 유효성 평가
1) 종양 반응은 Ia상 및 Ib상에서 모두 평가된다. 시험약물의 항암활성도는 임상시험의 2주기 마다 및 요법종료 (EOT) 시 (선택사항)에서 영상 검사를 통해 제반 기준을 적용하여 평가하거나 (하기 질병 평가 기준에 관한 표 14 참조), 액형 종양의 경우 골수 천자/생검 및 혈액 평가를 실시한다. 평가되는 종양 반응 매개변수는 다음을 포함한다:
(1) 최적 전체 반응 (best overall response): 완전반응 (CR, complete response), 부분반응 (PR, partial response), 안정병변 (SD, Stable Disease, 12주 지속), 또는 질병진행 (DP, disease progression)
(2) 객관적 반응률 (ORR, objective response rate): 완전반응 (CR) + 부분반응 (PR)
(3) 임상 유익률 (CBR, Clinical Benefit Rate): 완전반응 (CR) 또는 부분반응 (PR) 또는 최소 12주간 지속되는 안정병변 (SD)
(4) 반응지속기간 (Duration of response) 또는 안정병변 지속기간 (Duration of SD)
(5) 종양 진행까지의 시간 (TTP, time to tumor progression), 무진행생존 (PFS, Progression-free survival), 및 시험 관찰 기간 중 전체 생존 (OS, overall survival)도 평가된다.
2) 질병 평가 기준
질병평가기준 및 종양반응 매개변수를 결정하는데 사용하는 도구에 관한 세부 사항은 다음과 같다 (표 14).
3) 베이스라인 종양평가
스크리닝에서 림프종을 가진 잠재적인 시험대상자들은 PET-CT 스캔 검사와 함께 2 데옥시-2-[플루오르-18]플루오로-D-글루코스를 이용한 양전자방출 단층촬영 (PET) 검사를 받게 된다. 스캔 검사는 혈액 검체의 방사능 오염을 막기 위해 채혈일로부터 최소 3일 전 내지는 최대 10일 전에 실시하도록 한다.
스크리닝에서는 모든 시험대상자의 흉부/복부/골반/알려진 추가 병변 부위를 대상으로 IV 조영제 주사를 병행한 진단영상화질의 CT 스캔 검사를 실시한다 (PET-CT 스캔을 이용한 영상 검사를 받은 시험대상자들의 경우, 영상 시험을 함께 진행할 수 있음). 스크리닝 시 어떤 시험대상자가 CT IV 조영제에 대해 의학적 금기가 있거나 시험 중 금기가 발생할 경우, 다음과 같은 방사선 평가를 실시한다. 비조영 흉부 CT 및 조영 복부/골반 자기공명영상(MRI) 검사. 흉부 X선 검사 및 초음파 검사는 종양 병변을 측정하는 데 사용해서는 안 된다.
시험약물 투여를 시작하기 전 21일 이내에 완료된 PET/CT 스캔은 사용 가능하며 품질이 수용가능하다면 (예: PET/CT 스캔의 CT 부분이 조영제 투여와 함께 실시된 경우) 베이스라인 방사선 평가에 사용해도 된다.
이후의 모든 후속 질병 평가는 IV 조영제 투여를 병행한 CT 스캔으로 진행한다. CT 스캔으로 병기가 판정된 시험대상자들의 경우, 최대 크기의 표적 림프절, 림프절 종괴 또는 그 외 2가지 직경 (최장 직경[LDi] 및 최단 직경)으로 측정 가능한 림프성 병변 중 최대 6개의 항목을 시험대상자의 전체 질병 부담을 나타내는 다양한 신체 부위에서 식별해야 하며, 이환 시 종격 및 후복막 부위의 질환도 그러한 항목에 포함되어야 한다. 측정 가능한 림프절의 LDi는 1.5cm 이상이어야 한다. 림프절외 부위에서 측정 가능한 질환은 6개의 대표적인 표적 병변에 포함될 수 있다. 측정 가능한 림프절외 부위의 병변은 LDi가 1.0cm 이상이어야 한다. 그 외 모든 병변 (림프절 및 림프절외 부위의 질환과 평가 가능한 질환을 포함)은 비표적 질환으로서 추적해야 한다.
뇌 MRI 또는 CT는 임상적으로 필요한 경우에만 스크리닝 시 완료하도록 한다. 조영 증강 뇌 MRI를 실시할 것을 권장한다. 다만 MRI 조영제에 대한 금기증이 나타날 경우, 조영제 제외 MRI 검사 또는 조영제 포함/제외 CT 검사를 허용할 수 있다.
4) 베이스라인 종양평가
베이스라인 시점에서 측정된 각 병변은 임상시험 기간 내내 동일한 방법 (방사선학적 또는 핵의학적 방법)으로 재측정해야 하며, 그래야만 일관된 비교 결과를 얻을 수 있다. CT 스캔의 경우, 예정된 평가를 위해 ±7일의 기간이 할애된다. PET 스캔이 필요한 질환이 있는 시험대상자들은 완전반응 (CR)을 확인하기 위해 반복적인 PET-CT 스캔 검사를 받게 된다. PET-CT 스캔 검사는 혈액 검체의 방사능 오염을 막기 위해 채혈일로부터 최소 3일 전 내지는 최대 10일 전에 실시하도록 한다. 필요하다면 시험대상자에 대한 유효성 평가를 지원하기 위해 시험자의 재량에 따라 시험 중 언제든지 영상 평가를 추가로 실시해도 된다. 어느 때든지 질병 진행에 대한 임상적 의심이 있을 경우, 다음 예정된 영상 평가가 진행될 때까지 기다리기보다는 신체검사 및 영상 평가를 즉시 실시해야 한다. 질병 진행이 의심되는 이유로 일정 외 영상 평가를 진행하는 경우, 이후의 영상 평가는 애초 예정된 영상검사 일정에 따라 진행하도록 한다.
완전반응 (CR)의 경우, 의학 모니터 요원과 상의한 후 완전반응 (CR) 확인용 PET 스캔 또는 PET-CT 스캔을 실시할 수 있다. PET-CT 스캔은 채혈일을 기준으로 최소 3일 전 내지 최대 10일 전에 실시하도록 한다. 시험을 시작하기 전에 골수 침범 림프종 진단을 받은 시험대상자들의 경우, 완전반응 (CR)을 확인하기 위해 골수 생검을 실시하도록 한다.
사. 분석 모집단
분석을 위한 분석군 정의는 다음과 같다 (표 15).
5. 통계적 방법
가. 유효성 분석
제 Ia상 및 제 Ib상에서 시험약물 투여에 따른종양 반응 (림프종(B세포 및 T세포), CLL/SLL, 발덴스트롬 마크로글로불린혈증)은 표 14의 질병평가기준에 따라 평가된다. 종양 매개 변수는 위 적용된 기준에 따라 달라지고 다음을 포함할 수 있다.
1) 최적 전체 반응: 완전반응 (CR), 부분반응 (PR), 안정병변 (SD) 또는 질병 진행 (DP)
최적 전체 반응은 투여 후 모든 시점 (CR, PR, SD 또는 DP 순으로)에 걸쳐 가장 좋은 반응으로 정의된다. 최적 전체 반응 (CR, PR, SD 또는 DP)은 시험대상자 수 및 백분율에 따라 요약된다. 최적 반응이 안정병변 (SD)와 일치하는 경우, 안정병변 (SD)의 최소 지속 시간이 충족되어야 한다. 안정병변 (SD)의 비율을 계산하려는 경우, 그 비율은 최소 12주 이상의 기간 동안 안정병변 (SD)를 지속하는 시험대상자의 백분율 (%)을 나타낸다.
2) 객관적 반응률 (ORR): CR + PR
객관적 반응률 (ORR)은 완전반응 (CR) 또는 부분반응 (PR)의 최적 전체 반응이 나타나는 시험대상자들의 비율을 가리킨다. 상기 ORR 및 이의 95 % CI를 제시한다. 95% (정확) CI는 클로퍼-피어슨 방법에 따라 계산된다.
3) 임상적 유익률 (CBR): CR 또는 PR 또는 최소 12주간 이상 지속하는 SD
임상적 유익률(CBR)은 반응 평가를 위한 루가노 기준 개정안 에 따라 완전반응 (CR), 부분반응 (PR), 또는 지속적 안정병변 (SD) (SD ≥12 weeks) 기준을 충족하는 시험대상자들의 비율로 정의된다. CBR과 95% (정확) CI는 클로퍼-피어슨방법으로 추정된다.
4) 반응지속기간 (Duration of response)
반응자의 반응지속기간 (CR 또는 PR)은 적절한 반응이 처음 관찰된 날짜와 질병 진행 또는 사망(원인과 관계없이) 중 하나가 먼저 발생한 날짜 사이의 시간 간격으로 정의된다. 적절한 반응을 보인 후 질병 진행없이 생존해 있는 시험대상자의 경우, 반응지속기간은 마지막 종양 평가일 또는 질병 진행에 대한 마지막 추적관찰 방문일에 중도 절단된다. 완전반응 (CR) 또는 부분반응 (PR)이 있는 시험대상자들의 반응지속기간은 Kaplan-Meier 방법을 사용하여 요약된다. 중앙 값에 대한 중앙 사건 시간, 사분위수 Q1, Q3 및 양측 95% CI는 Brookmeyer-Crowley 방법을 사용하여 제공된다.
5) 안정병변지속기간 (Duration of SD)
안정병변지속기간은 시험약물 투여 후 처음으로 안정병변 (SD)에 도달한 날부터 시작해 어떤 원인이든 간에 질병 진행 또는 사망이 발생한 날 중 더 빠른 날까지 완전반응 (CR) 또는 부분반응 (PR)이 없을 때 시간 간격으로 정의된다. 질병의 진행 없이 생존 중인 시험대상자의 경우, 안정병변 (SD)지속기간은 평가 가능한 마지막 종양 평가일에 중도 절단된다. 안정병변 (SD)이 있는 시험대상자들의 반응 지속 기간은 Kaplan-Meier 방법을 사용하여 요약된다. 중앙 값에 대한 중앙 사건 시간, Q1, Q3 및 양측 95% CI는 Brookmeyer-Crowley 방법을 사용하여 제공된다.
6) 종양 진행까지의 시간 (TTP)
종양 진행까지의 시간 (TTP)은 첫 시험 용량의 투여일로부터 질병 진행까지 걸린 시간으로 정의된다. 질병 진행이 없는 시험대상자들은 '평가 가능한 마지막 종양 평가일' 또는 첫 용량의 투여일에 중도 절단된다. 베이스라인 이후 평가 데이터가 없다면, TTP는 첫 용량의 투여일에 중도 절단된다. 종양 진행까지의 시간은 Kaplan-Meier 곡선 그래프로 표시된다. 중앙 값에 대한 중앙 사건 시간, Q1, Q3 및 양측 95% CI는 Brookmeyer-Crowley 방법을 사용하여 제공된다.
7) 무진행생존 (Progression-free survival)
무진행생존 (PFS)은 시험대상자의 첫 시험용량 투여일로부터 질병 진행 또는 사망 중 먼저 도래하는 날까지 경과한 시간으로 정의된다. 사례가 발생하지 않은 (질병 진행 또는 사망이 발생하지 않은) 시험대상자들은 '마지막 종양 평가' 날짜에 중도 절단된다. 베이스라인 후 종양평가를 받지않은 시험대상자들은 첫 투여 시 절단된다. PFS는 종양 진행까지의 시간 (TTP)과 같은 방식으로 요약된다.
8) 전체 생존 (Overall survival during study observation period)
전체생존 (OS, Overall survival)은 첫번째 용량을 투여한 날로부터 어떤 원인으로든 사망한 날짜까지 경과한 시간으로 정의된다. 연구종료시점(EOS) 추적관찰 시점까지 사망한 것으로 알려지지 않은 시험대상자의 경우, OS는 마지막 연락일에 중도 절단된다. 마지막 연락일은 다음 날짜들 중 가장 최근의 날짜로 정의된다:
투여 중단일, 마지막 용량투여일, 마지막 질병 평가일 또는 시험대상자가 생존한 것으로 알려진 마지막 추적관찰일 OS는 Kaplan-Meier 곡선 그래프로 표시된다. OS는 무진행생존 (PFS)과 동일한 방식으로 요약된다.
나. 안전성 분석
1) Ia상 (용량 증량)
용량제한독성 (DLT)을 경험하는 시험대상자 수 및 비율은 용량제한독성 평가 가능군을 기준으로 하여 요약된다. 클로퍼-피어슨 방법에 따라 상응하는 95% 정확 CI가 제공된다. 최대내약용량 (MTD)/ 임상 Ib/II상을 위한 권장 용량 (RP2D) 의 추정은 본 명세서에 제공된 방법에 따른다 (3. 전체 설계, 2) 시험약물의 용량 증량 방법 참조).
2) Ia상 (용량증량) 및 Ib상 (용량확장)
안전성 분석군에 대해 다음 분석이 수행된다.
(1) 요법 후 발생한 이상반응 (TEAE, treatment emergent adverse event):
요법 후 발생한 이상반응 (TEAE)은 시험약물의 첫 투여를 시작한 날부터 시험약물의 마지막 투여 후 30 일이 경과할 때까지 발생하는 모든 이상반응으로 정의되고 또는 베이스라인 시점에 존재하지만 중증도 면에서 악화되는 모든 사례 (반응)로 정의된다. TEAE 는 규제 활동을 위한 의학용어사전 (MedDRA) 버전 21.1 또는 후속 버전을 사용하여 코드화 및 그룹화하도록 한다. 모든 이상반응 및 독성의 등급은 CTCAE (버전 5.0)에 따라 평가된다. 분석은 다음을 포함하되 이에 국한되지 않는다:
TEAE, 요법 관련 TEAE, 3 등급 이상의 TEAE, 3 등급 이상의 요법 관련 TEAE, 시험약물 투여 중단으로 인해 발생한 TEAE, 중대한 TEAE, 중대한 요법 관련 TEAE 및 사망을 초래한 TEAE. 시험대상자의 수와 비율은 상기의 항목들에 대해 요약하며, MedDRA 기관계 분류(SOC) 및 대표 용어(PT)에 따라서도 요약한다. 각 TEAE 범주에 속한 대상자들의 비율에 대해 양측 95% CI가 제공된다. TEAE 의 중증도 및 인과관계를 요약한다. 시험에서 발생하는 모든 AE 는 자료 목록에 표시되며 TEAE 는 별도로 구분된다.
또한, 특별 관심대상 이상반응을 이용한 추가 요약이 제공될 수도 있다. 특별 관심대상 이상반응은 다음을 포함하지만 이에 국한되지 않는다:
간독성, 피부 발진, 설사/대장염, 간질성 폐렴, 백혈구 감소, 빈혈, PJP 및 CMV 감염과 같은 기회감염을 포함한 감염률 증가
(2) 임상 실험실 매개변수 (혈액학, 임상 화학검사, 응고, 갑상선, 당화 혈색소[HbA1c] 및 소변 검사)들은 각기 예정된 시간에 요약한다. 시간 경과에 따른 변화는 기술 통계를 사용하여 설명하고, 정상 범위를 벗어난 값을 나타내는 시험대상자들의 비율을 표시한다. 고등급 및 저등급 실험실 평가 배정은 물론, 임상적으로 중요한 실험실 평가도 확인한다. 임상적으로 중요한 실험실 평가 결과에 따른 비정상적 수치의 발생을 제시한다. 임상 실험실 데이터의 등급은 CTCAE 에 따라 평가한다. CTCAE 에 따라 등급이 정의되지 않은 실험실 검사의 경우, 검사 결과의 등급은 실험실 정상 범위에 따라 낮음/정상/높음으로 구분하여 평가한다. 빈도 표는 3등급 또는 4등급 요법에서 새로 발생하는 AE에 대해 사용한다. 선택된 실험실 매개변수에 대한 각 검체 채취 시점에서 베이스라인 수치와 베이스라인 이후 수치를 서로 비교하기 위해 변화표 (shift table)를 작성한다. 기술 통계는 활력 징후 결과 (맥박수, 호흡수, 체온, 수축기 혈압 및 이완기 혈압)와 베이스라인 이후의 변화를 각각 요약하는 데 사용된다. 중앙 ECG 검토자의 데이터는 요약 발표 자료에 사용된다. 중앙 검토자의 데이터를 출처로 하여 어느 한 시점에서 각 ECG 매개변수의 값 2 개는 대상자의 시간 특이 매개변수를 결정하기 위해 평균된 후 요약에 사용된다. 심전도 변수들은 베이스라인 이후의 변화를 포함해 예정된 각 시점에 기술 통계를 사용하여 요약한다. 전반적인 해석을 위해 중앙 검토자가 평가한 바에 따라 ECG 결과가 정상 및 비정상으로 판정된 대상자들의 수와 비율은 시점마다 표로 작성한다. 베이스라인 시점부터 베이스라인 이후 최악의 결과가 측정된 시점까지 수치 변화를 표로 작성한다.
(3) ECOG 전신수행상태는 방문별로 요약한다.
(4) 신체검사는 목록으로만 표시된다. 임상적으로 유의한 모든 이상 소견들은 의학적 병력 또는 이상반응으로 기록된다.
(5) 여성에 대한 임신검사 결과들을 열거한다.
(6) 투여 주기 횟수, 투여된 용량, 누적 투여량, 용량 강도, 상대적 용량 강도 및 전체 노출 기간은 안전성 분석군(SAS)에 대해 요약되며 표준 요약 통계를 사용하여 요약된다. 각 시험대상자에 대한 주기별 총 용량은 기술 통계를 사용하여 요약한다. 시험약물의 내약성은 투여 지연, 중단 및 용량 감소의 횟수를 요약하여 평가한다. 투여 지연, 중단 및 용량 감소의 제반 사유는 시험대상자별로 열거하고 요약한다. 투여 중단 사유는 시험약물의 첫 투여일 및 마지막 투여일, 시험약물 노출 기간 및 투여 중단일과 함께 요약 및 열거한다.
(7) 모든 사전 약물 및 병용 약물 데이터가 열거된다. 사전 약물이란 시험약물의 첫 용량을 투여하기 전에 중단된 약물을 가리킨다. 병용 약물이란 시험약물의 첫 용량을 투여 시 또는 그 이후에 적어도 1 회 이상 복용한 약물을 가리킨다. 시험약물의 첫 투여일 전에 투여하기 시작해 동 시험약물의 첫 투여일에 투여를 중단한 약물은 사전 약물로만 간주한다. 모든 약물은 세계보건기구 약물사전/약물참고목록(WHO Drug Dictionary/WHO Drug Reference List)의 최신 버전을 사용하여 코드화한다. 시험대상자의 수와 백분율은 사전 약물 및 병용 약물에 대한 PT 및 국제약물분류체계(ATC) 1 단계 및 2 단계에 따라 제시된다.
8) 비약물 요법 정보를 열거한다. 비약물 요법의 명칭은 MedDRA 를 사용하여 코드화된다. 시험대상자의 수와 백분율은 PT 를 통해 제시한다.
9) 전체 분석군 (FAS)에 대한 인구통계, 베이스라인 시점의 건강 상태, 의학적 병력 및 그 외 베이스라인 특성들을 요약 및 열거한다.
다. 약동학 (PK) 분석
시험약물의 혈장 내 농도 및 PK 매개변수는 기술 통계를 사용하여 용량, 주기 및 시험 단계에 따라 열거 및 요약한다.
시험약물의 소변 내 농도와 매개변수는 기술 통계를 사용하여 용량별로 열거 및 요약한다. 시험약물의 혈장 내 농도-시간 프로파일은 그래프로 표시한다.
투여 간격[AUC(0-tau)], 0 시간부터 무한대까지 외삽한 혈장 내 약물 농도-시간 곡선하 면적[AUC(0-inf)] 또는 0 시간부터 마지막으로 측정 가능한 농도 시점까지 혈장 내 약물 농도-시간 곡선하 면적[AUC(0-last)](대부분의 시험대상자들에게서 AUC(0-inf)를 계산할 수 없는 경우)을 포함해 적용 가능한 PK 노출 매개변수의 개별 값 및 기하평균을 나타낸 산점도 그리고 Cmax 대용량의 비교 결과를 시험일에 따라 제시한다. 투여 용량 범위에서 Cmax, AUC(0-tau) 및 AUC(0-inf)[또는 AUC(0-last): 대부분의 시험대상자들에게서 AUC(0-inf)를 계산할 수 없는 경우]에 대한 PK 노출 매개변수의 용량 비례는 검정력 모델을 사용해 조사할 수 있다. 시간 선형성과 시험약물 용량은 분산 분석 모델을 사용하여 평가할 수 있다. 성별, 연령 및/또는 체중에 따른 영향은 그래프 또는 추론 분석 활용을 통해 탐색할 수 있다. 계획된 통계 분석에 관한 자세한 내용은 통계분석계획(SAP)에 제시한다. 정상 상태 조건의 달성은 투여 전 시험약물 농도의 그래프 표시를 통해 육안으로 평가할 수 있다.
라. 약력학 (PD) 분석
탐색적 PD 및 예측적 생체표지자 데이터가 열거되고, 적절하다면 관측된 값 및 베이스라인 수치 대비 변화에 대한 기술적 요약을 통해 해당 데이터를 제시한다.
마. 약동학 (PK)-약력학(PD) 분석
시험약물 노출과 종양 크기 감소의 상관관계는 산점도 (scatter plot)를 사용해 그래프로 탐색할 수 있다.
바. 중간 분석
실시하는 중간 분석에 대한 세부사항은 통계분석계획 (SAP)에 기술한다.
1) 제 Ia 상 부분 (용량 증량): 제 Ia 상 시험 중 다음과 같이 2 건의 중간 분석을 실시한다.
- 코호트 3 마지막 시험대상자의 1 주기 완료 후
- 제 1a 상 마지막 코호트의 1 주기 완료 후
2) 제 1b 상 부분 (용량 확장): 제 1b 상의 모든 시험대상자가 최소 6 주기 이상 요법을 받았거나 완료하기에 충분한 시간 동안 추적관찰을 받은 시점 또는 6 주기 미만일 경우 요법 중단까지 추적관찰을 받은 시점에 1 건의 중간 분석을 실시한다.
6. 평가 결과
가. 안전성 평가 결과
11명의 임상시험 대상자 중 2명이 3등급의 피부 반응 (skin reaction)을 보였고, 3명은 간독성 (hepatotoxicites)을 보였음을 확인하였다. 위 모든 이상반응은 관리 가능하였고, 시험약물 중단 시 0등급 또는 1등급으로 회복되었다.
코호트 1 내지 3에 포함된 5명의 임상시험 대상자 (DLBCL (1명) 및 PTCL (4명))에 대한 안전성 평가 결과는 다음과 같다:
요법 후 발생 이상반응 (TEAE)은 5명 중 4명 (80.0 %)의 시험대상자에서 발생되었다. 이 중 3명의 시험대상자에서 발생한 TEAE는 시험약물과 관련성이 있는 것으로 평가되었고 (60.0 %, 3건), 투여와 관련된 사망은 발생하지 않았다. 2명의 시험대상자가 2등급(moderate)의 TEAE를 보였고, 1등급(mild) 및 4등급(life-threatening)의 TEAE를 각각 1명씩 보였다. 상기 4등급의 TEAE는 혈소판감소증이고 시험약물과 관련성이 없었다.
위 결과에 따르면, 코호트 1 내지 3에서 3등급(severe) 내지 5등급(death)으로 평가된 중대한 이상반응은 발생하지 않았고, 이에 따라 코호트 1 내지 3의 용량범위에서는 용량제한독성 (DLT)이 없음을 확인하였다.
코호트 4에 대한 안전성 평가 결과는 다음과 같다:
코호트 4 (325mg QD)에서는 2건의 용량제한독성 (DLT)이 관찰되었다. 이에 따라, 최대내약용량 (MTD)을 초과한 것으로 간주하여 해당 코호트에 대한 투여 확대는 종료하였다. 투여 용량은 이전의 낮은 용량 수준으로 감소되었고 (200mg QD), 감소된 용량에 대해 3명의 추가 시험대상자 등록으로 확장하였다. 해당 확장 코호트 (200mg QD) 6명의 시험대상자에서 용량제한독성이 없음을 확인하였다.
위 코호트 1 내지 4에 관한 안전성 평가 결과로부터 최대내약용량 (MTD)이 200mg QD임을 확인하였고, 이 용량범위에서 내약성이 우수함을 확인하였다. 또한, 안전성 모니터링 위원회를 통해 임상 Ib/II에 대한 시작용량 (RP2D)으로 200mg QD가 권고되었다.
나. 유효성 평가 결과 ... 종양 반응 평가
임상 Ia상 (용량 증량 부분)에서 등록된 12명 중 11명의 시험대상자에서 종양 반응 평가가 확인되었다. 구체적으로, 1명의 시험대상자에서 완전반응 (CR), 2명의 시험대상자에서 부분반응 (PR)이 확인되었으며, 또한 6명의 시험대상자에서 안정병변 (SD)이 관찰되었다. 또한, 2명의 시험대상자에서 질병 진행 (PD)이 확인되었다.
12명의 등록 시험대상자 중 특히, 말초 T세포 림프종(PTCL) 진단을 받은 8명의 시험대상자에 대한 종양 반응 평가에서, 1명의 완전반응 (CR), 2명 부분반응 (PR), 4명의 안정병변 (SD) 및 1명의 질병 진행(PD)이 관찰되었고, 질병 조절율 (Disease Control Rate, DCR)은 87.5%였다.
위 결과로부터 시험약물이 말초 T 세포 림프종 (PTCL) 시험대상자에서 예비적인 활성을 보임을 확인하였다.
다. 약동학 및 약력학 평가 결과
1) 약동학
제1주기 제1일차 (C1D1), 임상시험 대상자에서 약동학 분석을 위한 혈장 샘플을 시험약물 투여 전과 투여 후 (0.5, 1, 1.5, 2, 3, 4, 6, 9, 12 및 24시간)에 수집하여 약동학 분석 결과, 시험약물의 50mg QD, 100mg QD 및 200mg QD 단회투여 후 최대 혈장 농도는 2443 ng/ml 범위이고 또한 전신 노출 (AUC0-24 기준)은 mL당 2760~21850 ng·h 범위임을 확인하였으며, 50mg 내지 200mg의 용량범위에서 시험약물의 노출 농도가 용량 의존적으로 증가함을 확인하였다 (도 4).
또한, 50mg QD, 100mg QD, 200mg QD 코호트의 반복투여 범위에서 평균 혈장 농도 값은 단회투여의 Cmax 및 AUCtau를 기준으로 각각 약 2.92배 및 4.97배의 노출 증가를 보였다. 또한, 50mg 내지 200mg 용량범위에서 시험약물의 노출 농도가 용량 의존적으로 증가함을 확인하였고, 약물축적은 관찰되지 않음을 확인하였다 (도 5).
2) 약력학
시험약물의 약력학 분석을 위한 혈액 샘플은 Ia상 (용량 증량 부분)에서 수집되었다. 시험약물이 조절 T 세포 (Treg)와 CD8+ T 세포에 미치는 효과를 관찰하기 위해, 전혈 샘플에 대해 CD3+, CD4+, CD25+, CD127low 및 CD8+에 대한 공동면역염색을 수행한 후 유세포 분석 (flow-cytometry)을 하였다.
그 결과, 시험약물의 혈장 농도가 증가할수록 베이스라인 값 대비 조절 T 세포 (Treg)가 감소하고, CD8+ T 세포가 증가함을 확인하였다 (도 6a 및 6b). 위 결과는 약동학에 따른 시험약물의 노출에 의존하여 조절 T 세포 (Treg)의 감소 및 CD8+ T 세포의 증가가 발생함을 확인한 것이고, 이로써 시험약물의 약동학과 약력학이 상관관계가 있음을 확인한 것이다.
Claims (7)
- 제1항에 있어서, 상기 조성물은 상기 화학식 1로 표시되는 화합물 또는 이의 약학적으로 허용 가능한 염을, 화학식 1로 표시되는 화합물로서 200 mg 이상 325mg 미만으로 포함하는 것인, 약학적 조성물.
- 제1항에 있어서, 상기 조성물은 상기 화학식 1로 표시되는 화합물 또는 이의 약학적으로 허용 가능한 염을, 화학식 1로 표시되는 화합물로서 200mg으로 포함하는 것인, 약학적 조성물.
- 제1항에 있어서, 상기 조성물은 말초 T 세포 림프종을 진단받은 후 적어도 1회 이상의 항암화학요법을 받은 대상체에 투여되는 것인, 약학적 조성물.
- 제1항 내지 제4항 중 어느 한 항에 있어서, 상기 조성물은 1일 1회로 4주 이상 투여되는 것인, 약학적 조성물.
- 제1항 내지 제4항 중 어느 한 항에 있어서, 상기 조성물은 경구 투여되는 것인, 약학적 조성물.
- 제1항 내지 제4항 중 어느 한 항에 있어서, 상기 조성물은 조절 T 세포 (regulatory T cell, Treg)를 감소시키고 CD8+ T 세포를 증가시키는 것인, 약학적 조성물.
