Abstract
본 발명은 HVEM (Herpesvirus entry mediator) 또는 이의 리간드(ligand)의 저해제, 및 HSP (Heat shock protein) 저해제를 포함하는, 암 치료 또는 예방용 조성물에 관한 것이다. HVEM 또는 이의 리간드의 저해제, 및 HSP 저해제의 병용은 이를 단독으로 사용하는 것에 비하여 항암 활성에 대해 시너지 효과를 나타내며, 투여 72시간 내의 빠른 시간 안에 효과를 나타낸다. 따라서, 상기 HVEM 또는 이의 리간드의 저해제, 및 HSP 저해제의 병용은 암의 예방 또는 치료에 유용하게 활용될 수 있다.
Description
본 발명은 HVEM (Herpesvirus entry mediator) 또는 이의 리간드(ligand)의 저해제, 및 HSP (Heat shock protein) 저해제를 포함하는, 암 치료 또는 예방용 조성물에 관한 것이다.
지난 수년간 암에 대한 집중적인 연구가 이루어졌음에도 불구하고 여전히 암은 전세계적으로 주요한 사망 원인이다. 다수의 암 치료법이 개발되었으나 모든 암종에 대해 그리고 모든 환자에 대해 효과적이지 않다. 암을 치료하기 위해 현재 대부분 사용되고 있는 방법은 상대적으로 비선택적이다. 수술에 의해 질병을 갖는 조직을 제거하거나, 방사선 치료에 의해 고형 종양의 크기를 감소시키거나, 화학치료에 의해 암세포를 빠르게 사멸시킨다. 특히, 화학치료는 약물에 대한 내성을 발달시킬 수 있으며, 일부 경우에는 투여가능한 용량을 제한하여 결국 잠재적으로 유효한 약물의 사용을 배제시킬 정도로 심각한 부작용을 야기시킨다. 따라서, 타겟 특이적이고 보다 효과적인 암 치료 방법의 개발이 시급하다.
이에, 암의 예방 또는 치료에 항암제를 병용하는 방법 등이 개발되고 있다. 예로서, 항-CD20 항체인 리툭시맙(rituximab)과 BCL2 단백질의 저해제인 베네토클락스(venetoclax)를 이의 항암 효과를 분석한 연구 결과가 발표되었다(John F Seymour et al., N Engl J Med. 2018 Mar 22, 378(12):1107-1120.). 전술한 연구 결과에서는 '베네토클락스-리툭시맙' 병용 요법은 독성을 나타내지 않으며, 나아가 베네토클락스 단일 요법보다 항암 효과가 더욱 우수하여 2년 무진행 생존률(2-year rate of progression-free survival)이 81.5%에 달하는 효과를 나타낼 수 있음을 보고하였다. 다만, 암세포의 DNA와 결합하여 암세포의 사멸을 야기하는 벤다무스틴(bendamustine)을 리툭시맙과 병용하는 경우, 2년 무진행 생존률은 27.8% 정도에 그쳤음을 언급하였다.
이는, 항암제를 병용하더라도 항상 시너지 효과를 나타내지는 않는다는 점을 시사하고 있는 것이므로, 당업계에서는 항암제의 병용 시 암의 치료에 시너지 효과를 낼 수 있는 최적 조합을 발견하고 제시하는 필요성이 절실하게 요구되고 있다.
본 발명자들은 병용 시 항암 활성에 시너지 효과를 나타낼 수 있는 항암제의 조합을 규명하기 위한 연구를 진행하였다. 그 결과, HVEM 또는 이의 리간드의 저해제, 및 HSP 저해제를 병용하는 경우, 이들을 단독으로 사용하는 것에 비하여 항암 활성이 현저히 증가하는 시너지 효과가 나타나며, 특히 72시간 내 매우 빠른 속도로 종양의 성장을 억제할 수 있음을 실험적으로 입증함에 따라 본 발명을 완성하기에 이르렀다.
본 발명의 하나의 양태는 (a) HVEM (Herpesvirus entry mediator) 또는 이의 리간드(ligand)의 저해제, 및 (b) HSP (Heat shock protein) 저해제를 포함하는, 암 예방 또는 치료용 약학 조성물을 제공한다.

이하, 실시예를 통하여 본 발명을 더욱 상세히 설명하고자 한다. 이들 실시예는 본 발명을 보다 구체적으로 설명하기 위한 것으로 본 발명의 범위가 이들 실시예에 의해 제한되지 않는다.
실시예 1. HVEM 저해제 및 HSP 저해제 병용의 in vitro 항암 효과
암의 예방 또는 치료 약물로서, HVEM 저해제 및 HSP 저해제의 병용이 시너지 효과를 나타내는지 확인하고자 하였다.
구체적으로, HVEM 저해제 단독, HSP 저해제 단독, 또는 HVEM 저해제 및 HSP 저해제의 병용을 암 오가노이드에 처리하였고, 암 오가노이드의 생존 정도를 확인함으로써 저해제 병용의 항암 효능을 평가하였다.
한편, 오가노이드는 조직 또는 기관에서 유래한 세포를 3차원 배양하여 제조한 장기 유사체이다. 오가노이드는 장기배양, 동결보존이 가능하고 조작과 관찰이 용이하다는 장점을 가진다. 동시에 불멸화가 필요 없어 그에 따른 세포의 본래 특성이 유지됨은 물론 생체 내에서만 볼 수 있었던 세포 계층적, 조직학적 구조를 재현함으로써 세포보다 한 단계 높은 차원의 생리현상을 연구할 수 있는 실험모델이다. 이러한 특성으로 인해 오가노이드는 세포의 내재적 특성이 변화된 불멸화된 세포주, 또는 인체와 구조가 다른 동물 모델에 비해 높은 정확도로 약물 평가를 할 수 있고, 특히 환자 유래 조직을 이용하므로, 사람을 대상으로 한 임상에 앞서 약물의 안전성은 물론 효능까지 미리 확인할 수 있는 장점이 있다.
HVEM(Herpesvirus entry mediator) 저해제로는 항-HVEM 항체(CW10, Santacruze사 #sc-21718; R&D사 #MAB356-SP)를 10 nM로 사용하였고, HVEM의 리간드 저해제로는 항-BTLA 항체(Myciosource사 #MBS633646; Adipogen사 #AG-20B-0049)를 10 nM로 사용하였고, HSP(Heat shock protein) 저해제로서 루미네스핍(Luminespib, AUY922, Selleck사 #s1069; HSP90 타겟), 가네테스핍(Ganetespib, STA-9090, Selleck사 #s1159; HSP90 타겟), 타네스피마이신(Tanespimycin, 17-AAG, Selleck사 #s1141; HSP90 타겟), BIIB021(Selleck사 #s1175; HSP90 타겟), 오날레스핍(Onalespib, AT13387, Selleck사 #s1163; HSP90 타겟), HSP990(HSP990, Selleck사 #s7097; HSP90 타겟), KNK437(Selleck사 #s7750; HSP40 타겟) 또는 VER-155008(Sellcek사 #s7751; HSP70 타겟)을 사용하였다.
이후, 암 오가노이드의 생존 정도는 아래의 방법으로 분석한 암 오가노이드 성장률 및 암 오가노이드 사멸률을 통해 확인하였다.
암 오가노이드 사멸률 (%) = [1 - (저해제 처리군의 암 오가노이드 성장률*)/(저해제 미처리군의 암 오가노이드 성장률**)] × 100
*저해제 처리군의 암 오가노이드 성장률 (%) = (저해제 처리 24, 48 또는 72시간 후의 암 오가노이드 면적)/(0시간의 암 오가노이드 면적) × 100
**저해제 미처리군의 암 오가노이드 성장률 (%) = (저해제 미처리 24, 48 또는 72시간 후의 암 오가노이드 면적)/(0시간의 암 오가노이드 면적) × 100
전술한 바와 같이 분석한 HVEM 저해제 및 HSP 저해제 병용의 암 치료에 대한 시너지 효과는 아래 실시예 1.1 내지 1.5에 기재하였다.
실시예 1.1. HVEM 저해제의 농도 의존적 효과 확인
도 1에 나타낸 바와 같이, 대장암 오가노이드에 HVEM 저해제를 농도별로 처리하는 경우, 처리 시간이 증가함에 따라 대장암 오가노이드의 성장이 저해됨을 확인하였으며(도 1의 A), 암 오가노이드 사멸율 또한 증가함을 확인하였다(도 1의 B). 또한, HVEM 저해제를 1 내지 5 nM의 저농도로 처리하는 것보다 10 nM 이상의 고농도로 처리하는 경우, 암 오가노이드 사멸율이 2.8배 이상 증가하는 것을 확인하였다. 특히, 동일한 농도를 처리하는 경우에는, 대조군 항체인 IgG1 단독에 비하여 HVEM 저해제는 효능이 약 10배 정도 증가하는 것을 확인하였다.
실시예 1.2. HSP 저해제의 농도 의존적 효과 확인
도 2에 나타낸 바와 같이, 대장암 오가노이드에 HSP90 저해제를 농도별로 처리하는 경우, 처리 시간이 증가함에 따라 대장암 오가노이드의 성장이 저해됨을 확인하였으며(도 2의 A), 암 오가노이드 사멸율 또한 증가함을 확인하였다(도 2의 B). 또한, HSP90 저해제를 1 내지 5 nM의 저농도로 처리하는 것보다 100 nM 이상의 고농도로 처리하는 경우, 암 오가노이드 사멸율이 3.6배 증가하는 것을 확인하였다.
실시예 1.3. HVEM 저해제 및 HSP90 저해제 병용의 시너지 효과
도 3의 A에 나타낸 바와 같이, 대장암 오가노이드에서 HVEM 저해제를 단독으로 처리하는 경우에 비하여, HVEM 저해제 및 HSP90 저해제를 병용으로 처리하는 경우, 암 오가노이드 사멸율이 증가함을 확인하였다. 구체적으로, Santa Cruze사의 HVEM 저해제 또는 R&D사의 HVEM 저해제 등 제조사에 상관없이 우수한 시너지 효과를 나타내었으며, HVEM 저해제 단독을 처리하는 경우보다 HVEM 저해제 및 HSP90 저해제를 처리하는 경우 암 오가노이드 사멸율이 약 1.5배 증가하는 것을 확인하였다.
또한, 도 3의 B에 나타낸 바와 같이, 폐암 오가노이드에서도 HVEM 저해제를 단독으로 처리하는 경우에 비하여, HVEM 저해제 및 HSP90 저해제를 병용으로 처리하는 경우, 암 오가노이드 사멸율 또한 증가함을 확인하였다. 구체적으로, Santa Cruze사의 HVEM 저해제 또는 R&D사의 HVEM 저해제 등 제조사에 상관없이 우수한 시너지 효과를 나타내었으며, HVEM 저해제 단독을 처리하는 경우보다 HVEM 저해제 및 HSP90 저해제를 처리하는 경우 암 오가노이드 사멸율이 약 2.1배 내지 3.7배 이상 증가하는 것을 확인하였다.
상기 결과를 통해, HVEM 저해제와 HSP90 저해제의 병용 사용은 암 오가노이드 사멸에 대하여 우수한 시너지 효과를 나타내는데, 특히 종양 미세환경의 암을 타겟하여 사멸시키는 효과가 매우 우수하므로, 대장암, 폐암 등 다양한 암의 예방 또는 치료에 유용하게 활용될 수 있음을 알 수 있었다.
실시예 1.4. BTLA 저해제 및 HSP 저해제 병용의 시너지 효과
HVEM 저해제와 HSP90 저해제 병용의 시너지 효과를 더욱 구체적으로 확인하기 위하여, HVEM의 리간드 저해제인 BTLA 저해제와의 시너지 효과를 확인하였다.
도 4의 A에 나타낸 바와 같이, 대장암 오가노이드에서 BTLA 저해제를 단독으로 처리하는 경우에 비하여, BTLA 저해제 및 HSP90 저해제를 병용으로 처리하는 경우, 암 오가노이드 사멸율 또한 증가함을 확인하였다. 구체적으로, Mybiosource사의 BTLA 저해제 또는 Adipogen사의 BTLA 저해제 등 제조사에 상관없이 우수한 시너지 효과를 나타내었으며, BTLA 저해제 단독을 처리하는 경우보다 BTLA 저해제 및 HSP90 저해제를 처리하는 경우 암 오가노이드 사멸율이 약 1.6배 이상 증가하는 것을 확인하였다.
또한, 도 4의 B에 나타낸 바와 같이, 폐암 오가노이드에서 BTLA 저해제을 단독으로 처리하는 경우에 비하여, BTLA 저해제 및 HSP90 저해제를 병용으로 처리하는 경우, 암 오가노이드 사멸율 또한 증가함을 확인하였다. 구체적으로, Mybiosource사의 BTLA 저해제 또는 Adipogen사의 BTLA 저해제 등 제조사에 상관없이 우수한 시너지 효과를 나타내었으며, BTLA 저해제 단독을 처리하는 경우보다 BTLA 저해제 및 HSP90 저해제를 처리하는 경우 암 오가노이드 사멸율이 약 1.7배 내지 2.0배 증가하는 것을 확인하였다.
상기 결과를 통해, BTLA 저해제와 HSP90 저해제의 병용 사용은 암 오가노이드 사멸에 대하여 높은 효과를 나타내는데, 특히 종양 미세환경의 암을 타겟하여 사멸시키는 효과가 매우 우수하므로, 대장암, 폐암 등 다양한 암의 예방 또는 치료에 유용하게 활용될 수 있음을 알 수 있었다.
실시예 1.5. HVEM 저해제 및 HSP 패밀리 저해제 병용의 시너지 효과
상기 실시예 1.3. 및 1.4.를 통해, HVEM 저해제 단독을 사용하는 경우보다 HVEM 저해제 및 HSP90 저해제 병용을 사용하는 경우, 암 오가노이드 사멸에 대하여 시너지 효과가 나타남을 확인하였다. 이에, 본 실시예에서는 HSP90 이외에 다른 HSP 패밀리 단백질에 대한 저해제도 HVEM 저해제와 시너지 효과를 나타내는지 확인하였다.
도 5의 A에 나타낸 바와 같이, 췌장암 오가노이드에서, HVEM 저해제(항-HVEM 항체), BTLA 저해제(항-BTLA 항체), HSP70 저해제(VER-155008) 또는 HSP90 저해제(AUY922) 단독에 비하여, HVEM 저해제 및 HSP70 저해제 병용은 효능이 약 2배 증가함을 확인하였고, HVEM 저해제 또는 HSP90 저해제 단독에 비하여, HVEM 저해제 및 HSP90 저해제 병용은 효능이 약 5배 증가함을 확인하였다. 또한, BTLA 저해제 또는 HSP70 저해제 단독에 비하여, BTLA 저해제 및 HSP70 저해제 병용은 효능이 약 2배 증가함을 확인하였고, BTLA 저해제 또는 HSP90 저해제 단독에 비하여, BTLA 저해제 및 HSP90 저해제 병용은 효능이 약 3배 증가함을 확인하였다. 이러한 시너지 효과는 도 5의 B에 나타낸 바와 같이 담도암 오가노이드에서도 공통적으로 나타남을 확인하였다.
도 6에 나타낸 바와 같이, HVEM 저해제와의 시너지 효과는 HSP 패밀리 단백질에 속하는 서로 다른 종류의 HSP90, HSP40 및 HSP70에 대한 저해제에서 모두 동일하게 나타남을 확인하였다. 구체적으로, HVEM 저해제 단독에 비하여, HVEM 저해제 및 HSP90 저해제 병용은 효능이 약 2.9배 (STA-9090 또는 17-AAG) 내지 3.4배 (AUY922) 이상 증가함을 확인하였고, 이러한 효과는 화학구조가 서로 다르지만 HSP90을 타겟하는 저해제 모두에서 공통적으로 나타남을 확인하였다. 또한, HVEM 저해제 단독에 비하여, HVEM 저해제 및 HSP40 저해제 병용은 암 오가노이드 사멸 효능이 약 2.9배 이상 증가함을 확인하였다. 또한, HVEM 저해제 단독에 비하여, HVEM 저해제 및 HSP70 저해제 병용은 암 오가노이드 사멸 효능이 약 3.1배 이상 증가함을 확인하였다.
상기 결과를 통해, BTLA 저해제와 HSP90 저해제의 병용 사용은 암 오가노이드 사멸에 대하여 높은 효과를 나타내며, HSP90, HSP70, HSP40 등 서로 다른 HSP 패밀리 단백질에 대한 저해제와도 우수한 시너지 효과를 나타내므로, 암의 예방 또는 치료에 유용하게 활용될 수 있음을 알 수 있었다.
실시예 2. HVEM 저해제 및 HSP 저해제 병용의 in vivo 항암 효과
암의 예방 또는 치료 약물로서, HVEM 저해제 및 HSP 저해제의 병용이 in vivo에서도 시너지 효과를 나타내는지 확인하고자 하였다.
구체적으로, HVEM 저해제 단독, BTLA 저해제 단독, HSP 저해제 단독, HVEM 저해제 및 HSP 저해제의 병용, BTLA 저해제 및 HSP 저해제의 병용, 또는 HVEM 저해제, BTAL 저해제 및 HSP 저해제의 병용을 대장암 오가노이드를 이용하여 마우스 종양 xenograft 모델에 투여하였다. 또한 HVEM 및 이의 리간드의 저해제의 경우 면역세포가 존재해야 약물의 효과를 나타낼 수 있기 때문에 xenograft 모델을 구축한 마우스에 PBMC를 넣어 인간화 마우스를 제작하였다. 상기 저해제는 마우스에 종양 이식 후 15일, 18일, 21일 및 24일째에 총 4차례에 걸쳐 투여하였다.
HVEM(Herpesvirus entry mediator) 저해제로는 항-HVEM 항체(CW10, Santacruze사 #sc-21718)를 1 mg/kg로 사용하였고, HVEM의 리간드 저해제로는 항-BTLA 항체(Myciosource사 #MBS633646)를 1 mg/kg로 사용하였고, HSP(Heat shock protein) 저해제로서 루미네스핍(Luminespib, AUY922, Selleck사 #s1069; HSP90 타겟)을 0.5 mg/kg로 사용하였다.
이후, 종양 이식 7일 후부터 매주 3회씩 종양의 크기를 측정하였고, 실험 종료 후 채취한 종양의 형태를 분석하고 종양의 무게를 측정하였으며, 상기 채취한 종양 조직에 대해 IHC (hCD8) 염색을 수행함으로써 조직 내로 침투한 hCD8+ T세포가 존재하는지 확인하였다.
그 결과, 도 7 및 8에서 볼 수 있듯이, HVEM 저해제, BTLA 저해제 또는 HSP90 저해제를 단독으로 처리하는 경우에 비하여, HVEM 저해제 및 HSP90 저해제를 병용으로 처리하거나, BTLA 저해제 및 HSP90 저해제를 병용으로 처리하거나, HVEM 저해제, BTLA 저해제 및 HSP90 저해제를 병용으로 처리하는 경우, 종양의 크기가 현저히 감소하는 것을 확인하였다.
또한, 도 9에서 볼 수 있듯이, HVEM 저해제, BTLA 저해제 또는 HSP90 저해제를 단독으로 처리하는 경우에 비하여, HVEM 저해제 및 HSP90 저해제를 병용으로 처리하거나, HVEM 저해제, BTLA 저해제 및 HSP90 저해제를 병용으로 처리하는 경우, 종양의 무게가 약 2배 내지 3배 이상 현저히 감소하는 것을 확인하였다.
아울러, 도 10에서 볼 수 있듯이, HVEM 저해제, BTLA 저해제 또는 HSP90 저해제를 단독으로 처리하는 경우에는 종양 조직으로 침투된(infiltration) hCD8+ T 세포가 거의 존재하지 않았으나, HVEM 저해제 및 HSP90 저해제를 병용으로 처리하거나, HVEM 저해제, BTLA 저해제 및 HSP90 저해제를 병용으로 처리하는 경우에는 종양 조직으로 침투된 hCD8+ T 세포가 발견되었다.
상기 결과를 통해, HVEM 저해제 또는 BTLA 저해제와 HSP 저해제의 병용 투여는 생체 내 암 세포에 대하여 높은 사멸 효과를 나타내는데, 특히 종양 미세환경의 암을 타겟하여 사멸시키는 효과가 매우 우수하므로, 대장암, 폐암 등 다양한 암의 예방 또는 치료에 유용하게 활용될 수 있음을 알 수 있었다.
Claims (10)
- (a) HVEM (Herpesvirus entry mediator) 또는 이의 리간드(ligand)의 저해제, 및 (b) HSP (Heat shock protein) 저해제를 포함하는, 암 예방 또는 치료용 약학 조성물.
- 제1항에 있어서,상기 HSP는 HSP40, HSP60, HSP70 및 HSP90으로 이루어진 그룹에서 선택되는 하나 이상인, 약학 조성물.
- 제1항에 있어서,상기 저해제는 HVEM 또는 HSP 단백질에 특이적으로 결합하는 화합물, 펩티드, 펩티드 미메틱스, 융합 단백질, 항체, 앱타머 및 항체-약물 복합체(ADC; Antibody Drug Conjugate)로 이루어진 그룹에서 선택되는 하나 이상인, 약학 조성물.
- 제1항에 있어서,상기 저해제는 HVEM 또는 HSP의 DNA 또는 mRNA에 상보적으로 결합하는 안티센스 핵산, siRNA, shRNA, miRNA 및 리보자임으로 이루어진 그룹에서 선택되는 하나 이상인, 약학 조성물.
- 제1항에 있어서,상기 암은 담도암, 위암, 폐암, 간암, 대장암, 결장암, 소장암, 췌장암, 뇌암, 뼈암, 흑색종, 유방암, 경화성선증, 자궁암, 자궁경부암, 두경부암, 식도암, 갑상선암, 부갑상선암, 신장암, 육종, 전립선암, 요도암, 방광암, 혈액암, 림프종 및 섬유선종으로 이루어진 그룹에서 선택되는 하나 이상인, 약학 조성물.
- (a) HVEM 저해제 후보물질 또는 HVEM의 리간드 저해제 후보물질; 및 HSP 저해제 후보물질을 암을 앓는 대상체로부터 분리한 생물학적 시료 또는 암 질환 동물 모델에 처리하는 단계;(b) 상기 (a) 단계에서 저해제 후보물질이 처리된 군에서 항암 활성을 분석하는 단계; 및(c) 상기 (b) 단계에서 분석된 항암 활성이 대조군에 비하여 증가한 경우, 상기 HVEM 저해제 후보물질 또는 HVEM의 리간드 저해제 후보물질; 및 HSP 저해제 후보물질을 항암제로 판정하는 단계를 포함하는, 항암제 스크리닝 방법.
- 제6항에 있어서,상기 (a) 단계에서, HVEM 저해제 후보물질 또는 HVEM의 리간드 저해제 후보물질 및 HSP 저해제 후보물질은 동시에, 연속적으로, 또는 순차적으로 처리하는 것인, 방법.
- 제6항에 있어서,상기 생물학적 시료는 세포, 조직, 혈액 또는 이로부터 제조한 오가노이드인 것인, 방법.
- 제6항에 있어서,상기 (b) 단계에서 항암 활성은 암세포의 사멸 증가, 종양의 크기 감소 또는 종양의 중량 감소인, 방법.
